Für frühgeborene Kinder ist Muttermilch die empfohlene enterale Ernährung mit vielen bereits gut untersuchten positiven Effekten. Jedoch ist der Gehalt an Makronährstoffen zu niedrig, um die Bedürfnisse von Frühgeborenen zu decken. Zusätzlich zeigt native Muttermilch eine hohe inter- und intraindividuelle Variabilität der Makronährstoffe. Die Effizienz der allgemein verbreiteten Standardfortifizierung mit kommerziell erhältlichen Fortifiern ist durch diese Variabilität beeinträchtigt und kann so in einem Großteil der Muttermilchproben zu einer inadäquaten Nährstoffzufuhr führen, die zu niedrig, jedoch auch zu hoch ausfallen kann. Unzureichende Ernährung kann einen negativen Einfluss auf das Wachstum und die Körperzusammensetzung sowie Folgeerkrankungen im Erwachsenenalter haben.
Diese Studie zeigt anhand eines Datensatzes mit realen Messwerten zur Zusammensetzung der Muttermilch in über 3300 Proben, dass die Messung der Makronährstoffe in nativer Muttermilch und die anschließende, darauf basierende Targetfortifizierung mit der individuellen Ergänzung der fehlenden Makronährstoffen zu adäquaten Nährstoffeinträgen führt. Mit diesem Ansatz kann auf die Variabilität der nativen Muttermilch reagiert und die Variation der Makronährstoffeinfuhr signifikant verringert werden. In dieser Arbeit wurden drei verschiedene optimierte Standardfortifier entwickelt. Deren Zusammensetzung erhöht den Makronährstoffeintrag bereits unter Standardfortifizierung, aber noch mehr bei ein oder zwei Muttermilchanalysen pro Woche.
Weiterhin wurde in dieser Arbeit der Einfluss der durch die verschiedenen Fortifizierungsstrategien erzeugten Nährstoffzufuhren auf die Wachstumsraten berechnet, die für Standardfortifizierung ungenügende und für Targetfortifizierung hauptsächlich zufriedenstelle Ergebnisse zeigt.
Verschiedene Kombinationen von Pooling mit bis zu drei Proben oder von Pooling einer größeren Anzahl wie in einer Milchbank zu Verfügung stehenden Proben verringerten nicht die Variabilität der Nährstoffzufuhr. Ebenso zeigte die Gefriertrocknung keine zufriedenstellenden Ergebnisse für alle Makronährstoffkonzentrationen.
Die Ergebnisse dieser Arbeit verdeutlichen, dass standardfortifizierte Muttermilch trotz verschiedener Poolingstrategien keine adäquate Makronährstoffzufuhr für frühgeborene Kinder sicherstellt. Für den klinischen Alltag wird die Bedeutung der Targetfortifizierung deutlich, die mit regelmäßiger Muttermilchanalyse und unter Anwendung optimierter Standardfortifier zufriedenstellend gelingen kann und so zu physiologischen Wachstumsraten führen kann.
Immunotherapies against high-risk neuroblastoma (NB), using the anti-GD2 antibody (Ab) dinutuximab beta (DB), significantly improved patient survival. Ab-dependent cellular cytotoxicity (ADCC) is one of the main mechanisms of action and it is primarily mediated by NK cells. To further improve antitumor efficacy, we investigated here a combinatorial immunotherapy with DB and the double immune checkpoint blockade of T-cell immunoreceptor with immunoglobulin and ITIM domain (TIGIT) and programmed cell death ligand-1 (PD-L1). The effects of ADCC, mediated by DB against NB cells on NK-cell activity, and the expression of TIGIT and CD226 and their ligands CD112 and CD155, as well as of PD-1 and PD-L1 on NB and effector cells, were investigated using flow cytometry. ADCC was assessed with a calcein-AM-based cytotoxicity assay. The efficacy of a combinatorial immunotherapy with DB, given as a long-term treatment, and the double immune checkpoint blockade of TIGIT and PD-L1 was shown using a resistant murine model of NB, followed by an analysis of the tumor tissue. We detected both TIGIT ligands, CD112 and CD155, on all NB cell lines analyzed. Although ADCC by DB resulted in a strong activation of NK cells leading to an effective tumor cell lysis, a remarkable induction of PD-L1 expression on NB cells, and of TIGIT and PD-1 on effector cells, especially on NK cells, was observed. Additional anti-TIGIT or anti-PD-L1 treatments effectively inhibited tumor growth and improved survival of the mice treated with DB. The superior antitumor effects were observed in the “DB + double immune checkpoint blockade” group, showing an almost complete eradication of the tumors and the highest OS, even under resistant conditions. An analysis of tumor tissue revealed both TIGIT and TIGIT ligand expression on myeloid-derived suppressor cells (MDSCs), suggesting additional mechanisms of protumoral effects in NB. Our data show that the targeting of TIGIT and PD-L1 significantly improves the antitumor efficacy of anti-GD2 immunotherapy, with DB presenting a new effective combinatorial treatment strategy against high-risk tumors.
Treatment of high-risk neuroblastoma (NB) patients with the anti-GD2 antibody (Ab) dinutuximab beta (DB) improves survival by 15%. Ab-dependent cellular cytotoxicity (ADCC) is the major mechanism of action and is primarily mediated by NK cells. Since IL-2 co-treatment did not show a therapeutic benefit but strongly induced Treg, we investigated here a DB-based immunotherapy combined with the immunocytokine FAP-IL-2v, which comprises a fibroblast activation protein α (FAP)-specific Ab linked to a mutated IL-2 variant (IL-2v) with abolished binding to the high-affinity IL-2 receptor, thus stimulating NK cells without induction of Treg. Effects of FAP-IL-2v on NK cells, Treg and ADCC mediated by DB, as well as FAP expression in NB, were investigated by flow cytometry, calcein-AM-based cytotoxicity assay and RT-PCR analysis. Moreover, the impact of soluble factors released from tumor cells on FAP expression by primary fibroblasts was assessed. Finally, a combined immunotherapy with DB and FAP-IL-2v was evaluated using a resistant syngeneic murine NB model. Incubation of leukocytes with FAP-IL-2v enhanced DB-specific ADCC without induction of Treg. FAP expression on NB cells and myeloid-derived suppressor cells (MDCS) in tumor tissue was identified. A tumor-cell-dependent enhancement in FAP expression by primary fibroblasts was demonstrated. Combination with DB and FAP-IL-2v resulted in reduced tumor growth and improved survival. Analysis of tumor tissue revealed increased NK and cytotoxic T cell numbers and reduced Treg compared to controls. Our data show that FAP-IL-2v is a potent immunocytokine that augments the efficacy of DB against NB, providing a promising alternative to IL-2.
Background
Short-term infusions of dinutuximab beta plus isotretinoin and cytokines administered in previous immunotherapy studies in neuroblastoma were associated with severe pain. Here, long-term, continuous infusion of single-agent dinutuximab beta was evaluated in patients with relapsed/refractory neuroblastoma.
Methods
In this open-label, single-arm, Phase 2 study, patients with either refractory or relapsed high-risk neuroblastoma received dinutuximab beta by continuous infusion over 10 days of each cycle, for up to five cycles. The primary endpoint was objective response rate 24 weeks after the end of cycle 5. Secondary endpoints included adverse events, intravenous morphine use, best response, duration of response, and three-year progression-free and overall survival.
Results
Of the 40 patients included, 38 had evaluable response. Objective response rate was 26% and best response rate 37%. Median duration of response was 238 days (IQR 108–290). Three-year progression-free and overall survival rates were 31% (95% CI 17–47) and 66% (95% CI 47–79), respectively. Prophylactic intravenous morphine use and duration of use decreased with increasing cycles. The most common grade 3 treatment-related adverse events were pain, diarrhea, and hypokalemia.
Conclusion
Long-term continuous infusion of single-agent dinutuximab beta is tolerable and associated with clinically meaningful responses in patients with relapsed/refractory high-risk neuroblastoma.

Objective
Maternal pre-pregnancy underweight, overweight and obesity might increase the risk for worse short- and long-term outcome in the offspring. There is a need for further study into the relationship between maternal pre-pregnancy body mass index (BMI) and the combined outcome of physical development, state of health and social behavior in children. Question: Is maternal pre-pregnancy BMI associated with the child outcome in terms of physical development, state of health and social behavior (school and leisure time behavior) at the age of 9 to 15 years?
Methods
In the population-based birth cohort study Survey of Neonates in Pomerania (SNIP) children at the age 9–15 years and their families were re-examined by questionnaire-based follow-up. 5725 mother–child pairs were invited to SNiP-follow-up. This analysis is based on the recall fraction of 24.1% (n = 1379). Based on the maternal pre-pregnancy BMI (ppBMI), 4 groups were formed: underweight (ppBMI < 19 kg/m2, n = 117), normal weight (ppBMI 19–24.99 kg/m2, n = 913, reference), overweight (ppBMI 25–30 kg). /m2, n = 237) and obesity (ppBMI > 30 kg/m2, n = 109).
Results
In the multiple regression model, the BMI-z-score for children of mothers in the underweight group was −0.50 lower, and 0.50/1.07 higher in the overweight/obese group (p < 0.001) compared to reference at median age of 12 years. No differences were found in children of underweight mothers with regard to social behavior (interaction with friends and family), school and sports performance (coded from “very good” to “poor”), other leisure activities (watching television, using mobile phones, gaming), and health (occurrence of illnesses) compared to children of normal weight mothers. In contrast, maternal pre-pregnancy overweight and obesity were associated with lower school and sports performance, and higher screen time (smart phone, gaming, television) compared to children of normal weight mothers.
Conclusion
Maternal pre-pregnancy overweight and obesity but not underweight was negatively associated with school performance and leisure time behavior in the offspring at 9–15 years of age.
Predicting complications in pediatric Crohn's disease patients followed in CEDATA-GPGE registry

Background
Complications of Crohn's disease (CD) often impair patients' quality of life. It is necessary to predict and prevent these complications (surgery, stricturing [B2]/penetrating [B3] disease behavior, perianal disease, growth retardation and hospitalization). Our study investigated previously suggested and additional predictors by analyzing data of the CEDATA-GPGE registry.
Methods
Pediatric patients (< 18 years) diagnosed with CD with follow up data in the registry were included in the study. Potential risk factors for the selected complications were evaluated by performing Kaplan-Meier survival curves and cox regression models.
Results
For the complication surgery, the potential risk factors older age, B3 disease, severe perianal disease and initial therapy with corticosteroids at the time of diagnosis were identified. Older age, initial therapy with corticosteroids, low weight-for-age, anemia and emesis predict B2 disease. Low weight-for-age and severe perianal disease were risk factors for B3 disease. Low weight-for-age, growth retardation, older age, nutritional therapy, and extraintestinal manifestations (EIM) of the skin were identified as risk factors for growth retardation during the disease course. High disease activity and treatment with biologicals were predictors for hospitalization. As risk factors for perianal disease, the factors male sex, corticosteroids, B3 disease, a positive family history and EIM of liver and skin were identified.
Conclusion
We confirmed previously suggested predictors of CD course and identified new ones in one of the largest registries of pediatric CD patients. This may help to better stratify patients’ according to their individual risk profile and choose appropriate treatment strategies.
Objectives: Several clinical disease activity indices (DAIs) have been developed to noninvasively assess mucosal healing in pediatric Crohn’s disease (CD). However, their clinical application can be complex. Therefore, we present a new way to identify the most informative biomarkers for mucosal inflammation from current markers in use and, based on this, how to obtain an easy-to-use DAI for clinical practice. A further aim of our proof-of-concept study is to demonstrate how the performance of such a new DAI can be compared to that of existing DAIs.
Methods: The data of two independent study cohorts, with 167 visits from 109 children and adolescents with CD, were evaluated retrospectively. A variable selection based on a Bayesian ordinal regression model was applied to select clinical or standard laboratory parameters as predictors, using an endoscopic outcome. The predictive performance of the resulting model was compared to that of existing pediatric DAIs.
Results: With our proof-of-concept dataset, the resulting model included C-reactive protein (CRP) and fecal calprotectin (FC) as predictors. In general, our model performed better than the existing DAIs. To show how our Bayesian approach can be applied in practice, we developed a web application for predicting disease activity for a new CD patient or visit.
Conclusions: Our work serves as a proof-of-concept, showing that the statistical methods used here can identify biomarkers relevant for the prediction of a clinical outcome. In our case, a small number of biomarkers is sufficient, which, together with the web interface, facilitates the clinical application. However, the retrospective nature of our study, the rather small amount of data, and the lack of an external validation cohort do not allow us to consider our results as the establishment of a novel DAI for pediatric CD. This needs to be done with the help of a prospective study with more data and an external validation cohort in the future.
This article reports on the development, implementation and management of a German–Polish telemedicine network in the field of pediatric oncology and hematology in the Euroregion Pomerania. The achievements and challenges of joint medical case reviews involving patients and their care givers, as well as cross-border education activities for physicians, students and nursing staff, are presented. In addition to a progress report, the results of an evaluation of the participants and teachers, likewise the measurement of knowledge growth, are given.
Background. The German maternity guidelines require regular medical checkup (MC) during pregnancy as a measure of prevention. Socioeconomic factors such as education, profession, income and origin, but also age and parity may influence the preventive and health behavior of pregnant women. The aim was to investigate the influence of these factors on the participation rate in MC of pregnant women. Method. The current analysis is based on the prospective population-based birth cohort study Survey of Neonates in Pomerania, which was conducted in Western Pomerania, Germany. The data of 4092 pregnant women from 2004 to 2008 were analyzed regarding the antenatal care and health behavior. Up to 12 MC were regularly offered; participation in 10 MC is defined as standard screening according to maternity guidelines. Results. Women participated in the first preventive MC on average in the 10th (±3.8 SD) week of pregnancy. 1343 (34.2%) women participated in standard screening and 2039 (51.9%) took a screening above standard. 547 (13.92%) women participated in less than the 10 standard MCs. In addition, about one-third of the pregnancies investigated in this study were unplanned. Bivariate analyses showed an association between better antenatal care behavior and higher maternal age, stabile partnerships and mother born in Germany, p < 0.05. On the contrary antenatal care below standard were more often found by women with unplanned pregnancies, less educational women and women with lower equivalent income, p < 0.001. Health behaviors also influenced antenatal care. Whereas the risk of antenatal care below standard increased by smoking during pregnancy (RRR 1.64; 95% CI 1.25, 2.14) and alcohol consumption (RRR 1.31; 95% CI 1.01, 1.69), supplementation intake was associated with decreased risk (iodine—RRR 0.66; 95% CI 0.53, 0.81; folic acid—RRR 0.56; 95% CI 0.44, 0.72). The health behavior of pregnant women also differs according to their social status. Higher maternal income was negatively correlated with smoking during pregnancy (OR 0.2; 95% CI 0.15, 0.24), but positively associated with alcohol consumption during pregnancy (OR 1.3; 95% CI 1.15, 1.48) and lower pre-pregnancy BMI (Coef. = 0.083, p < 0.001). Lower maternal education was positively correlated with smoking during pregnancy (OR 59.0; 95% CI 28.68, 121.23). Conclusions. Prenatal care according to maternity guidelines is well established with a high participation rate in MC during pregnancy of more than 85%. However, targeted preventive measures may address younger age, socioeconomic status and health-damaging behaviors (smoking, drinking) of the pregnant women because these factors were associated with antenatal care below standard.
Inflammasome activation and formation of ASC specks in patients with juvenile idiopathic arthritis

Objective
The formation of large intracellular protein aggregates of the inflammasome adaptor ASC is a hallmark of inflammasome activation and characteristic of autoinflammation. Inflammasome activated cells release the highly proinflammatory cytokine IL-1β in addition to ASC specks into the extracellular space. Autoinflammatory activity has been demonstrated in systemic JIA, however minimal data exist on the role of inflammasomes in other JIA subtypes. We therefore investigated, if pyroptotic cells are present in the circulation of oligo- and poly-articular JIA.
Methods
Peripheral blood of JIA patients (n = 46) was investigated for ASC speck formation, a key step in inflammasome activation, by flow cytometry and immunofluorescence. Free ASC and proinflammatory cytokine levels were determined by ELISA and multiplex assay.
Results
Oligo-articular JIA patients showed a significantly increased proportion of ASC speck+ monocytes compared to poly-articular JIA patients. In serum free ASC alone is not sufficient to assess inflammasome activity and does not correlate with ASC speck+ monocytes. Compared to control several cytokines were significantly elevated in samples of JIA patients. JIA serum containing antinuclear antibodies, incubated with ASC specks boosts a secondary inflammation by IL-1β production in macrophages.
Conclusion
For the first time, we detect ex vivo inflammasome activation by ASC speck formation in oligo- and poly-articular JIA patients. Most notably, inflammasome activation was significantly higher in oligo- compared to poly-articular JIA patients. This data suggests that inflammasome derived autoinflammation may have a greater influence in the previously thought autoimmune oligo-articular JIA patients.
Background
Invasive mold infections are a well-known and life-threatening condition after allogeneic hematopoietic stem cell transplantation (HSCT). While Aspergillus species are recognized as predominant pathogens, Fusarium species should also be considered due to their broad environmental distribution and the expected poor outcome of invasive fusariosis. Particularly, splenic rupture as a complication of disseminated disease has not been reported yet.
Case presentation
Two weeks after allogeneic HSCT for severe aplastic anemia, a 16-year-old boy presented with painful, erythematous skin nodules affecting the entire integument. As disseminated mycosis was considered, treatment with liposomal amphotericin B and voriconazole (VCZ) was initiated. Invasive fusariosis was diagnosed after histological and previously unpublished polymerase chain reaction-based examination of skin biopsies. Microbiological tests revealed Fusarium solani species. Despite stable neutrophil engraftment and uninterrupted treatment with VCZ, he developed mold disease-associated splenic rupture with hypovolemic shock and fungal endocarditis. The latter induced a cardiac thrombus and subsequent embolic cerebral infarctions with unilateral hemiparesis. Following cardiac surgery, the patient did not regain consciousness because of diffuse cerebral ischemia, and he died on day +92 after HSCT.
Conclusion
Invasive fusariosis in immunocompromised patients is a life-threatening condition. Despite antimycotic treatment adapted to antifungal susceptibility testing, the patient reported here developed uncommon manifestations such as splenic rupture and fungal endocarditis.
Pulmonary manifestation (PM) of inflammatory bowel disease (IBD) in children is a rare condition. The exact pathogenesis is still unclear, but several explanatory concepts were postulated and several case reports in children were published. We performed a systematic Medline search between April 1976 and April 2022. Different pathophysiological concepts were identified, including the shared embryological origin, “miss-homing” of intestinal based neutrophils and T lymphocytes, inflammatory triggering via certain molecules (tripeptide proline-glycine-proline, interleukin 25), genetic factors and alterations in the microbiome. Most pediatric IBD patients with PM are asymptomatic, but can show alterations in pulmonary function tests and breathing tests. In children, the pulmonary parenchyma is more affected than the airways, leading histologically mainly to organizing pneumonia. Medication-associated lung injury has to be considered in pulmonary symptomatic pediatric IBD patients treated with certain agents (i.e., mesalamine, sulfasalazine or infliximab). Furthermore, the risk of pulmonary embolism is generally increased in pediatric IBD patients. The initial treatment of PM is based on corticosteroids, either inhaled for the larger airways or systemic for smaller airways and parenchymal disease. In summary, this review article summarizes the current knowledge about PM in pediatric IBD patients, focusing on pathophysiological and clinical aspects.
Cerebral oxygenation disturbances contribute to the pathogenesis of brain lesions in preterm infants with white matter damage. These children are at risk of developing long-term neurodevelopmental disabilities. Preterm birth is associated with sudden hormonal changes along with an untimely increase in oxygen tissue tension. There is a persistent high postnatal production of fetal zone steroids (FZS), which serve in the fetoplacental unit as precursors for placental estrogen synthesis during pregnancy. The role of FZS in events associated with oxygenation differences and their impact on the developing white matter is not well understood. Therefore, we investigated the effect of hyperoxia (80% O2) and subsequent administration of FZS on the protein composition and migration capabilities of immature oligodendrocytes using the OLN93 (rat-derived OPC) cell line as an experimental model. We tested the effect of the FZS, dehydroepiandrosterone (DHEA), 16α-OH-DHEA, and adiol (5-androstene-3β, 17β-diol). After 24-hour exposure to hyperoxia, we monitored the changes in the proteome profile following treatment and observed significant alterations in pathways regulating cytoskeletal remodelling, cell migration, and cell survival. Additionally, hyperoxia leads to impaired migration of the OLN93 cells in culture. Administration of the FZS showed positive effects on the migration process under normoxic conditions in general. However, under hyperoxic conditions, the trend was less prominent. The observed effects could be related to changes in levels of cofilin/LIMK pathway-associated proteins. Adiol had a negative effect when administered together with estradiol, and the proteomic data reveal the activation of ephrin receptor signalling that might be responsible for the attenuation of migration. The results suggest that FZS can differentially regulate pathways involved in the migration of OLN93 cells. A deeper insight into the precise role of endogenous FZS would be an essential prerequisite for developing new treatment strategies including supplementation of estradiol and other steroids in preterm infants.
Das Neuroblastom (NB) stellt als Tumor des sympathischen Nervenstranges den häufigsten extrakraniellen soliden Tumor des Kindesalters dar. Ein Bedarf innovativer Therapien, welche eine effektive Anti-Tumorantwort induzieren, ergibt sich aus dessen heterogener Charakteristik, höchst aggressivem Wachstum und trotz Therapien hohen Rezidivraten der high-risk-Patient*innen. In den letzten Jahren konnten durch zunehmende Erkenntnisse im Bereich der Tumor- immunologie verschiedene immuntherapeutische Verfahren etabliert werden. So stellt die Anti-GD2-Therapie (ch14.18/CHO) einen zentralen klinischen Pfeiler der multimodalen Therapie des high-risk-NB dar. Hierbei werden Anti-GD2-AK verab- reicht, die das NB-spezifische Antigen GD2 erkennen, was zur gezielten Aktivierung des körpereigenen Immunsystems hauptsächlich durch NK-Zellen mit anschlie- ßender Tumorzelllyse führt. Herausforderungen ergeben sich aus der Überwindung der immuninhibitorischen Tumorumgebung sowie der Reduzierung natürlicher Regulationsmechanismen.
Vorherige Arbeiten der Arbeitsgruppe haben eine Anti-GD2-Immuntherapie abhängige Induktion des Immuncheckpoints PD-1/PD-L1 sowie der regulatorischen myeloiden CD11b+-Zellen (MRC) gezeigt. Für die vorliegende Arbeit war nun die Rolle der CD11b+ MRC von besonderem Interesse. Hierfür wurden CD11b+ Zellen in einem syngenen NB-Mausmodell mittels Anti-CD11b-AK blockiert oder durch 5-FU selektiv depletiert, was zu einem deutlich reduzierten Tumorwachstum und verbessertem Überleben führte. Zusätzlich wurde mittels RT-PCR die relative Gen- expression MRC-modulierender Gene (Arg1, CCL2, GM-CSF, IDO, IFNγ, IL-1β, IL-4, IL-6, IL-6R, IL-8, IL-10, M-CSF, M-CSFR, iNOS, TGF-β1 und VEGF-A) in der Tumormikroumgebung sowie der Milz evaluiert. Hier wurde eine Reduktion der modulierenden Gene nicht nur in der Tumormikroumgebung, sondern sogar peri- pher durch die Depletion von MRC nachgewiesen. Somit konnte eine negative Rolle der MRC beim NB bestätigt werden.
Diese Ergebnisse stellten eine Grundlage für die kombinierte Immuntherapie aus Anti-GD2-AK und MRC-Depletion dar. Eine Steigerung der antitumoralen Wirksam- keit im Vergleich zu den entsprechenden Monotherapien wurde gezeigt. Dabei wurden eine verbesserte Überlebenswahrscheinlichkeit und ein reduziertes Tumor- wachstum festgestellt. Die vorliegenden Ergebnisse liefern daher die Grundlage für einen neuartigen kombinierten Therapieansatz, um die immer noch schlechten Überlebenschancen der high-risk-Patient*innen zu verbessern.
Frauenmilch ist die beste Nahrung in den ersten sechs Lebensmonaten. Bioaktive Peptide in der Muttermilch scheinen eine große Rolle bezüglich deren präventiver Wirkung zu spielen. In den letzten Jahren hat sich die Methode der Massenspektrometrie zur Erforschung des Peptidoms rasant entwickelt. Aber es fehlt noch an Grundlagenforschung und einheitlichen Protokollen zur präanalytischen Verarbeitung der Muttermilchproben. Studien sind daher nur bedingt vergleichbar. Nicht nur für die Wissenschaft, sondern auch im klinischen Kontext ist es für den Einsatz von Spenderfrauenmilch erforderlich, den Einfluss verschiedener Lagerungsbedingungen auf das Muttermilchpeptidom zu kennen.
Im Rahmen der vorliegenden Studie wurden anhand der Muttermilchproben von vier Spenderinnen vier verschiedene Lagerungsbedingungen mit anschließender Lagerung bei –80 °C, die im klinischen Alltag (Lagerung bei –20 °C für 120 h), beim Transport von Muttermilchproben (Lagerung bei 4 °C für 6 h) oder in der häuslichen Umgebung der Frauen (Lagerung bei Raumtemperatur für 24 h bzw. 4 h) vorkommen, untersucht. Die Pro-ben wurden mit direkt bei –80 °C gelagerten Proben verglichen.
Die Ergebnisse zeigen eine sinkende Anzahl an identifizierbaren Proteinen mit steigender Temperatur. Vor allem nach der Lagerung bei Raumtemperatur über 24 h nahm die Signalintensität vieler Peptide entweder ab oder die Peptide verschwanden komplett. Eine Erklärung könnte sein, dass die in der Muttermilch enthaltenen Proteasen bei Raumtemperatur weiterhin aktiv sind und zur proteolytischen Spaltung der Proteine führen.
Aufgrund der vorliegenden Ergebnisse ist zu empfehlen, Frauenmilch für Peptidomstudien direkt bei mindestens –20 °C zu lagern und im Anschluss bei –80 °C einzufrieren. Im klinischen Umgang mit Muttermilch müssen zusätzlich die negativen Einflüsse der Lagerung bei niedrigen Temperaturen berücksichtigt werden. Von einer bis zu 24-stündigen Lagerung bei Raumtemperatur, ist dringend abzuraten.
Research has already shown that the maternal anthropometry affects birth weight. The impact on birth weight, body length and head circumference percentiles was not yet investigated. The aim of our observational studies was to develop individualized percentile charts (birth weight, body length and head circumference) for neonates based on maternal weight and height. To achieve this, we analyzed data of 2.2 million newborns stratified by maternal height and weight from the German Perinatal Survey.
The percentiles based on 18 groups stratified by maternal anthropometry for both sexes showed significant differences between identical original percentiles. The differences were up to almost 800 g between identical percentiles for petite and grande mothers. Birth length differed by several centimeters for the same percentiles between groups of short and tall stature mothers, whereas birth head circumference differed up to 1.2 cm.
Our analysis showed that maternal anthropometry has a significant effect on the classification of newborns as LGA (large for gestational age), AGA (appropriate for gestational age) and SGA (small for gestational age). Individualized charts show higher specificity than percentile charts that do not include those data and provide more individual prediction of perinatal risks.
Ziel. Das Ziel dieser Arbeit bestand darin, mit Hilfe des Survey of Neonates in Pomerania
(SNiP) einen umfassenden Überblick über die Schwangerenvorsorge in Mecklenburg-
Vorpommern zu geben.
Methode. Analysiert wurden die Daten von 4092 Schwangeren aus den Jahren 2004 bis 2008
des populationsbasierten Survey of Neonates in Pomerania.
Ergebnisse. Die Teilnahmequote an serologischen und sonographischen Screenings lag
zwischen 91 % bis 99 %. Die erste Vorsorgeuntersuchung erfolgte durchschnittlich in der
10. Schwangerschaftswoche (SD = 3.8 Schwangerschaftswochen). 3382 (86.1 %) Frauen
nahmen an mindestens zehn der nach den Mutterschaftsrichtlinien vorgesehenen
Vorsorgeuntersuchungen teil (Standardvorsorge). Bei 547 (13.9 %) Schwangeren erfolgte eine
Vorsorge unter Standard. Signifikante Prädiktoren für eine Unterversorgung waren eine
niedrige mütterliche Schulbildung (relatives Risikoratio [RRR] = 2.5, 95 % CI 1.6 – 3.8), das
mütterliche Alter und die Parität (χ2(6) = 24.923, p < .001). Die Gesundheitsprävention,
gemessen an Supplementeinnahme (Jod: χ2(3) = 125.934, p < .001; Folsäure: χ2(3) = 222.798,
p < .001) und Wahrnehmung schwangerschaftsbegleitender Angebote
(Geburtsvorbereitungskurse, Vorstellung in der Geburtsklinik, χ2(3) = 385.279, p < .001) hing
ebenfalls von der mütterlichen Bildung ab. Negative Einflüsse auf die Schwangerschaft standen
im Zusammenhang zum Einkommen. Das Risiko zu rauchen (OR 0.19, 95 % CI 0.15 – 0.24)
sank ebenso wie der BMI (Coef. = -.83, p < .000) mit der Höhe des Einkommens. Der
Alkoholkonsum dagegen stieg bei höherem Einkommen (OR 1.30, 95 % CI 1.15 – 1.48) an.
Schlussfolgerungen. Die Schwangerschaftsvorsorge ist in Mecklenburg-Vorpommern gut
etabliert. Dennoch sollten vermehrt präventive Ansätze gerade für jüngere Schwangere und für
diejenigen mit geringerer Schulbildung und niedrigerem Einkommen in den Vordergrund
gerückt werden, um auch hier optimale Bedingungen in der Schwangerschaft, bei der Geburt
und für das künftige Leben des Kindes zu schaffen.
Zur Evaluation der Umsetzung von Impfungen werden auf Bevölkerungsebene Durch-immunisierungsraten bestimmt. Die Einhaltung der Impfterminvorgaben der STIKO- bzw. SIKO-Empfehlungen wurden in Deutschland bisher nicht im Detail untersucht. Ziel dieser Arbeit war es, das Impfverhalten von niedergelassenen Kinder- und Jugend-mediziner*innen sowie Allgemeinmediziner*innen und Praktischen Ärzt*innen mit der Frage zu untersuchen, ob bzw. wie die termingerechte Umsetzung der STIKO- bzw. SIKO-Empfehlungen (Stand: 2016/2017) am Beispiel der Grundimmunisierung von Kindern gegen Pneumokokken und Masern gelingt. Dazu wurden im Rahmen einer retrospektiven Querschnittserhebung deutschlandweit 4.000 niedergelassene Kinder- und Jugendmediziner*innen sowie 4.000 Allgemeinmediziner*innen und Praktische Ärzt*innen randomisiert ausgewählt und mittels eines Umfragebogens zu ihrem Impfverhalten befragt. Als Einflussfaktoren wurden die Fachrichtung, das Alter, das Geschlecht und der Praxissitz (nach Bundesland) untersucht. Das Impfverhalten wurde nach einem Score beurteilt. Ein multivariables lineares Regressionsmodell (Gesamtscorewert als abhängige Variable) wurde erstellt (p = 0,05). Gründe für das Einhalten bzw. Nichteinhalten der STIKO-/SIKO-Empfehlungen wurden im Detail erfragt.
Die Antwortrate entsprach 38% bei den Kinder- und Jugendmediziner*innen (N=1510) und 12% bei den Allgemeinmediziner*innen und Praktischen Ärzt*innen (N=488) (Gesamtresponse-Proportion 25%).
Hinsichtlich der Grundimmunisierung von Kindern gegen Pneumokokken gemäß STIKO bzw. SIKO-Empfehlungen gaben 97% der Kinder- und Jugendmediziner*innen aber nur 41% der Allgemeinmediziner*innen und Praktischen Ärzt*innen an, alle Kinder zu impfen. Mit zunehmendem Alter der Ärzt*innen zeigt sich ein abnehmender Trend, alle Kin-der zu impfen. Bezüglich der Einhaltung der empfohlenen Impfschemata ergab sich, dass sich 86% aller Studienteilnehmenden an das für reifgeborene Kinder empfohlene 2+1-Schema und 77% an das für frühgeborene Kinder empfohlene 3+1-Schema hielten. In der Untersuchung der Einflussfaktoren zeigte sich, dass sich Kinder- und Jugendmediziner*innen mit über 90%, im Vgl. zu den Allgemeinmediziner*innen und Praktischen Ärzt*innen (≤40%), deutlich häufiger an die empfohlenen Impfschemata hielten. In der detaillierten Analyse der Einhaltung der einzelnen Impftermine ergab sich, dass es den teilnehmenden Ärzt*innen insbesondere für die 3. Impfdosis und bei Frühgeborenen zusätzlich für die 4. Impfdosis schlechter gelang, diese wie empfohlen zu verabreichen. Anhand des entwickelten Scores konnte gezeigt werden, dass es insgesamt nur weniger als 40% der Studienteilnehmenden gelang, früh- bzw. reifgeborenen Kindern alle Impfungen zur vollständigen Grundimmunisierung „meistens“ zeitgerecht zu verabreichen. In den Regressionsanalysen zum Einfluss der Fachrichtung, des Alters, Geschlechtes und Praxissitzes auf das Impfverhalten von Früh- und Reifgeborenen ergaben sich vergleichbar große Unterschiede in der termingerechten Umsetzung der STIKO-/SIKO-Empfehlungen. Für die Fachrichtung zeigte sich ein signifikanter und klinisch relevanter Einfluss dahingehend, dass es Kinder- und Jugendmediziner*innen gelang, deutlich mehr Impftermine zeitgerecht umzusetzen. Ein Trend zu abnehmendem Gelingen der Einhaltung der Impftermine mit zunehmendem Alter bestand in beiden Arztgruppen. Für das Geschlecht und den Praxissitz ergab sich kein klinisch relevanter Einfluss.
In der Auswertung der Daten für die Einhaltung der Impftermine zur Grundimmunisierung von Kindern gegen Masern gemäß STIKO-Empfehlungen gaben 92% aller Studienteilnehmenden an, alle Kinder gegen Masern Grund zu immunisieren. Dies betraf sowohl den überwiegenden Anteil der Kinder- und Jugendmediziner*innen (99%) als auch der Allgemeinmediziner*innen und Praktischen Ärzt*innen (70%). Mit zunehmendem Alter ergab sich ein abnehmender Trend zu impfen. Für die Einhaltung der Impftermine zeigte sich, dass es den Studienteilnehmenden insbesondere für die zweite Impfdosis schlechter gelang, diese wie empfohlen termingerecht zu verabreichen. Anhand des von uns entwickelten Scores konnte gezeigt werden, dass nur 54% der Studienteilnehmenden alle Impfterminvorgaben zur vollständigen Grundimmunisierung „meistens“ zeitgerecht einhalten. In der zum Score gehörigen Regressionsanalyse konnte ein signifikanter und klinisch relevanter Einfluss der Fachrichtung gezeigt werden. Kinder- und Jugendmediziner*innen gelang es demnach deutlich häufiger, die Impftermine zeitgerecht umzusetzen. Ein Trend zu abnehmendem Gelingen der Einhaltung der Impftermine mit zunehmendem Alter bestand in beiden Arztgruppen. Für das Geschlecht und den Praxissitz ergab sich kein klinisch relevanter Einfluss.
Als gewichtigste Gründe für die Nicht-Einhaltung der Pneumokokken- wie auch der Ma-sern-Impfempfehlung wurde von den Studienteilnehmenden, unabhängig von Fachrichtung, Alter oder Geschlecht, allen voran der Wunsch der Eltern angegeben, ferner das Vorliegen von Kontraindikation sowie gute Erfahrungen mit späteren Impfzeitpunkten bzgl. Verträglichkeit und Nebenwirkungen.
Schlussfolgernd und im Vergleich mit anderen Erhebungen spiegeln die von uns, von den teilnehmenden Ärzt*innen erhobenen Daten eine geringere Compliance zur Umsetzung der STIKO-/SIKO-Empfehlungen bei der Grundimmunisierung von Kindern gegen Pneumokokken und Masern – bezogen auf die Impftermine - wider. Als ursächlich anzu-nehmen ist, dass unser Fokus auf der Einhaltung der einzelnen Impftermine lag. Unsere Ergebnisse führen dadurch vermutlich zu einer Unterschätzung der anzunehmenden Durchimmunisierungsraten. Dagegen verleiten die vom RKI erhobenen Impfquoten, welche primär auf der zahlenmäßigen Vollständigkeit der Impfdosen basieren, eher zu einer Überschätzung der STIKO- /SIKO-konformen Umsetzung der Grundimmunisierung von Kindern gegen Pneumokokken und Masern. Die gedankliche Verknüpfung unserer Daten mit den vom RKI erhobenen Impfquoten aber bietet einen realistischeren Blick auf die Einhaltung der STIKO- bzw. SIKO empfohlenen Impfpläne bzw. -konzepte. Abschließend ergeben sich aus den in dieser Studie gewonnenen Erkenntnissen wichtige Interventionsansätze. Dazu zählen eine gezieltere Schulung von Allgemeinmediziner*innen und Praktischen Ärzt*innen sowie von älteren Ärzt*innen, eine Verbesserung der Aufklärung zögernder oder ablehnender Eltern und die konsequente Überprüfung der Kontraindikationen, welche häufig zur Nicht-Einhaltung von Impfterminen herangezogen werden sowie eine qualitativ hochwertige und transparente Information darüber.
Zusammenfassend wurde festgestellt, dass eine Epilepsie im Kindes- und Jugendalter eine insgesamt gute Prognose hat. Von den untersuchten 666 Patienten wurden 65,3 % der Patienten während des Studienzeitraumes anfallsfrei, 29,1 % zeigten eine klinische Besserung.
Den größten prognostischen Vorhersagewert hatte dabei das EEG. Wurde ein normales EEG erreicht, dann wurden auch 96,0 % klinisch anfallsfrei oder verbesserten sich. Ebenfalls betrachtet wurden die initialen EEGs. Es stellte sich heraus, dass Patienten ohne Allgemeinveränderungen im initialen EEG einen besseren Therapieerfolg aufwiesen (85,6 % anfallsfrei oder verbessert).
An zweiter Stelle steht der mentale Status. Kinder mit einer normalen Intelligenz zeigten ein besseres Outcome als Kinder mit einer Lern- oder geistigen Behinderung. Eine IQ-Wert-Veränderung nach Medikamenteneinnahme konnte nicht nachgewiesen werden.
Ebenfalls einen signifikanten Einfluss hatte die Anzahl der Versuche. Die besten Ergebnisse wurden nach dem ersten Versuch erzielt. Mit steigender Versuchszahl verringerte sich der prozentuale Erfolg, aber auch mit mehr als 4 Versuchen konnten noch Verbesserungen oder auch Anfallsfreiheit erzielt werden.
Es zeigte sich, dass auch die Ätiologie einen gewissen Voraussagewert hat. Patienten mit einer idiopathischen Epilepsie hatten eine bessere Prognose als Kinder mit symptomati-schen oder kryptogenen Anfällen. Dabei spielte auch die Art der Epilepsie eine große Rolle, denn darauf bezieht sich die medikamentöse Behandlung. Die besten Ergebnisse zeigten sich bei den benignen Partialepilepsien (90,1% anfallsfrei oder verbessert), den Absencen (65,2 % anfallsfrei oder verbessert) und den primären Grand mal Anfällen (71,4 % anfallsfrei oder verbessert). Schlechter zu behandeln waren die fokalen und die sekundären Grand Mal Anfälle und die größten Schwierigkeiten fand man bei den syndromalen Epilepsien wie BNS-Anfällen und das Lennox-Gastaut Syndrom.
Die am häufigsten verwendeten Medikamente waren Valproat, Lamotrigin, Sultiam, Topi-ramat und Levetiracetam. Eine besonders gute Wirksamkeit zeigte bei Absencen Ethosu-ximid (74,1 % anfallsfrei oder verbessert), bei den benignen Partialepilepsien Sultiam (94,9 % anfallsfrei oder verbessert), bei fokalen Anfällen Oxcarbazepin (71,5 % anfallsfrei oder verbessert) und bei Grand mal Epilepsien Valproat (76,5 % anfallsfrei oder verbessert). Valproat besitzt ein breites Wirkspektrum und führte bei fast allen Anfallsarten zur klinischen Verbesserung oder Anfallsfreiheit. Allerdings zeigte es auch die meisten Nebenwirkungen (31,6 % aller Nebenwirkungen) zu denen Gewichtszunahme, negative Verhaltensänderungen und Sedierung zählten. Insgesamt sind traten in 21,7 % aller medikamentösen Therapieversuchen Nebenwirkungen auf.
Auch die Anzahl der Medikamente spielt eine Rolle bezüglich Prognose und Erfolg. Die besten Ergebnisse zeigten sich unter einer Monotherapie (98,6 % der Patienten mit einer Monotherapie wurden anfallsfrei oder verbesserten sich).
Die neuen Antiepileptika (69,7 % anfallsfrei oder verbessert) haben einen ähnlichen Be-handlungserfolg wie die alten Antiepileptika (71,6 % anfallsfrei oder verbessert). Bei einigen Anfallsarten sind die neuen Medikamente eine Verbesserung 68, 69, 70, 71 und zeigen vor allem einen Vorteil im Nebenwirkungsprofil. Allerdings bestehen noch Lücken in der Zulassung und sie haben höhere Therapiekosten. Vigabatrin wird vor allem bei BNS Anfällen und cortikalen Dysplasien angewendet. Lamotrigin sticht durch das breite Anwendungsspektrum mit wenig Nebenwirkungen hervor. Levetiracetam zeigt im EEG positive Resultate und auch klinisch vor allem bei Grand Mal Anfällen. Es hat keine Interaktionen und im Gegensatz zu Valproat kaum Teratogenität. Oxcarbazepin ist durch weniger Nebenwirkungen bei fokalen Anfällen eine gute Alternative. Aber trotz der höheren Nebenwirkungen sind die alten Antiepileptika unverzichtbar. Sultiam ist weiterhin das Medikament der ersten Wahl bei benignen Partialepilepsien mit einem sehr guten Erfolg.
Es wurde festgestellt, dass in der Kinder- und Jugendklinik der Universitätsmedizin Greifswald eine weitgehend gute Einstellung der Epilepsie erfolgen konnte. Ein Teil der Patienten konnte aber trotz mehrerer Versuche und Medikamente bis zum Ende des Studienzeitpunktes nicht zufriedenstellend behandelt werden. Dies und auch die bestehenden Nebenwirkungen zeigen, dass eine stetige Weiterentwicklung der antikonvulsiven Therapie wichtig ist und angestrebt werden sollte.
Rhinoviruses (RV) account for a significant number of asthma exacerbations, and RV species C may be associated with a severe course in vulnerable patient groups. Despite important evidence on the role of RV reported by clinicians and life scientists, there are still unanswered questions regarding their influence on asthma exacerbation in young patients. Thus, we measured the RVspecies-specific IgG titers in our German pediatric exacerbation cohort using a microarray-based technology. For this approach, human sera of patients with exacerbated asthma and wheeze, as well as healthy control subjects (n = 136) were included, and correlation analyses were performed. Concordantly with previously published results, we observed significantly higher cumulative levels of RV species A-specific IgG (p = 0.011) and RV-C-specific IgG (p = 0.051) in exacerbated asthma group compared to age-matched controls. Moreover, atopic wheezers had increased RV-specific IgG levels for species A (p = 0.0011) and species C (p = 0.0009) compared to non-atopic wheezers. Hypothesizing that bacterial infection positively correlates with immune memory against RV, we included nasopharyngeal swab results in our analyses and detected limited correlations. Interestingly, the eosinophil blood titer positively correlated with RV-specific IgG levels. With these observations, we add important observations to the existing data regarding exacerbation in pediatric and adolescent medicine. We propose that scientists and clinicians should pay more attention to the relevance of RV species in susceptible pediatric patients.
