Abstract
This work reports on the spatio-temporal characterization of the multiple current pulse regime of diffuse barrier discharges driven by sine-wave feeding voltage at a frequency of 2 kHz in helium with small nitrogen admixtures. The discharge gap of 3 mm is bounded by glass plates on both plane electrodes. Priority is given to the lateral discharge inhomogeneities, underlying volume- and surface-memory effects, and the breakdown mechanism. Therefore, relevant processes in the discharge volume and on the dielectric surfaces were investigated by ICCD camera imaging and optical emission spectroscopy in combination with electrical measurements and surface charge diagnostics using the electro-optic Pockels effect of a bismuth silicon oxide crystal. The number of current pulses per half-cycle of the sine-wave voltage rises with increasing nitrogen admixture to helium due to the predominant role of the Penning ionization. Here, the transition from the first glow-like breakdown to the last Townsend-like breakdown is favored by residual species from the former breakdowns which enhance the secondary electron emission during the pre-phase of the later breakdowns. Moreover, the surface charge measurements reveal that the consecutive breakdowns occur alternately at central and peripheral regions on the electrode surface. These spatial inhomogeneities are conserved by the surface charge memory effect as pointed out by the recalculated spatio-temporal development of the gap voltage.
Background: Although 20-30% of all strokes occur in the posterior circulation, few studies have explored the characteristics of patients with strokes in the posterior compared to the anterior circulation so far. Especially data on young patients is missing. Methods: In this secondary analysis of data of the prospective multi-centre European sifap1 study that investigated stroke and transient ischemic attack (TIA) patients aged 18-55 years, we compared vascular risk factors, stroke aetiology, presence of white matter hyperintensities (WMH) and cerebral microbleeds (CMB) between patients with ischaemic posterior circulation stroke (PCS) and those having suffered from anterior circulation stroke (ACS) based on cerebral MRI. Results: We diagnosed PCS in 612 patients (29.1%, 407 men, 205 women) and ACS in 1,489 patients (70.9%). Their age (median 46 vs. 47 years, p = 0.205) and stroke severity (modified Rankin Scale: both 2, p = 0.375, Barthel Index 90 vs. 85, p = 0.412) were similar. PCS was found to be more frequent among the male gender (66.5 vs. 60.1% with ACS, p = 0.003). Vertebral artery (VA) dissection was more often the cause of PCS (16.8%) than was carotid artery dissection of ACS (7.9%, p < 0.001). Likewise, small vessel disease (Trial of Org 10172 in Acute Stroke Treatment [TOAST] = 3, PCS: 14.7%, ACS: 11.8%) and stroke of other determined aetiology (TOAST = 4, PCS: 24.5%, ACS: 16.0%) were more frequent in those with PCS. Furthermore, patent foramen ovale (PFO; PCS: 31.1%, ACS: 25.4%, p = 0.029) was more often detected in patients with PCS. In contrast, large-artery atherosclerosis (TOAST = 1, PCS: 15.4%, ACS: 22.2%) and cardio-embolic stroke (TOAST = 2, PCS: 15.6%, ACS: 18.0%) were less frequent in those with PCS (p < 0.001) as were preceding cerebrovascular events (10.1 vs. 14.1%, p = 0.014), TIA (4.8 vs. 7.7%, p = 0.016) and smoking (53.2 vs. 61.0%, p = 0.001). The presence, extent, and location of WMH and CMB did not differ between the 2 groups. Conclusions: Our data suggested a different pattern of aetiology and risk factors in young patients with PCS compared to those with ACS. These findings especially call for a higher awareness of VA dissection and potentially for more weight of a PFO as a risk factor in young patients with PCS. Enzalutamid ist seit 2012 ein zugelassenes Medikament für die Hormontherapie nach Docetaxel-Behandlung bei kastrationsresistenten Prostatakarzinom (CRPC). Als bekannter Wirkmechanismus gilt der Antagonismus am Androgenrezeptor (AR) und die Verhinderung der Translokation in den Nukleus, sowie die Inhibition der Transkription am AR.
In dieser Arbeit wurde der Wirkmechanismus von Enzalutamid auf zellulärer und molekularer Ebene präzisiert. In der Wachstumskinetik zeigt sich eine Wachstumshemmung sowohl der AR-positiven LNCaP Zellen, als auch der AR- negativen PC-3 Zellen. Zur Untersuchung des antiproliferativen Effektes von Enzalutamid betrachtet diese Arbeit AR-assoziierte Wachstumsfaktoren, den AR selbst und Östrogenrezeptoren. Als Einflussgrößen gelten die AR-assoziierten Proteine, wie Hitzeschockprotein (HSP) 27, HSP70, HSP90α/β und die Co-Chaperone HSP40 und HSP 70/90 organizing Protein (HOP). Hierbei zeigte vor allem HSP27 aber auch HSP90β eine deutliche Suppression der Protein-Expression in beiden Zelllinien. Die Untersuchung der Proteinexpression der Co-Chaperone, HSP40 in beiden Zelllinien und HOP in LNCaP Zellen, ergab ebenfalls eine deutliche Supression. Diese Proteine, insbesondere HSP27, haben aufgrund ihrer zellulären Funktion Einfluss auf die Proliferation der PC-Zellen, somit kann ein supprimierender Effekt unter Enzalutamid die Erklärung für das eingeschränkte Wachstum der LNCaP Zellen aber auch der hormonresistenten PC-3 Zellen sein. In LNCaP Zellen konnte der supprimierende Effekt von Enzalutamid auf die AR-Expression bestätigt werden. Als weiteren möglichen Proliferationsfaktor wurde in dieser Arbeit der Östrogenrezeptor (ER) mit seinen Isoformen ERα, ERβ1 und ERβ2 untersucht. Es zeigte sich dabei eine deutliche Suppression des ERβ1, welcher in einem möglichen Zusammenhang mit dem AR stehen kann. Die Suppression der ERβ1-Expression zeigte sich aber auch deutlich in AR-negativen PC-3 Zellen, was auch als antiproliferativer Effekt gewertet werden kann. Die Untersuchung der weiteren ER-Isoformen unter Einwirkung von Enzalutamid zeigten keine Effekte.
Das Resultat dieser Arbeit ist, dass Enzalutamid nicht nur ein Hormonrezeptorantagonist ist, sondern auch auf AR-unabhängige Signalkaskaden Einfluss hat. Enzalutamid zeigt im Zellmodell einen deutlich antiproliferativen Effekt, was mit seiner effektiven Wirkung bei CRPC korreliert und auch an den in dieser Arbeit betrachteten Faktoren liegen kann.
Depression und Adipositas sind weit verbreitet und eng miteinander verbunden. Brain-
derived neurotrophic factor und Vitamin D stehen in Diskussion an neuronaler Plastizität beteiligt zu sein, insbesondere bei Depression. Es wurde berichtet, dass niedrige BDNF und Vitamin D Spiegel auch mit Adipositas assoziiert. Auch bei Adipositas scheint Neuroplastizität eine Rolle zu spielen. Serum Neuronen-spezifische Enolase, wird als Marker für neuronale Schädigung diskutiert. Über das Zusammenspiel von BDNF und Vitamin D ist bisher wenig bekannt. In der vorliegenden Dissertation wurden die vermuteten Zusammenhänge und Interaktionen von BDNF und Vitamin D mit Depression und Adipositas in einer großen, bevölkerungsbasierten Kohorte untersucht. Dabei wird auch auf den Zusammenhang zwischen sNSE und Adipositas eingegangen. Die Daten kamen aus der epidemiologischen Study of Health in Pomerania (SHIP)-Trend. Die logistischen Regressionsmodelle erbrachten eine negative Assoziation von Vitamin D mit Depression und Adipositas. Zwischen BDNF und Depression oder Adipositas zeigte sich in der logistischen Regression kein signifikanter Zusammenhang. Allerdings erbrachten multivariable lineare Regressionsmodelle einen U-förmigen Zusammenhang zwischen BDNF und WHR. Die Interaktion von BDNF und Vitamin D in Bezug auf Depression zeigte keine statistische Signifikanz (p-Wert=0,065). sNSE Spiegel und BMI waren nicht linear assoziiert, es zeigte sich ein parabelförmiger Zusammenhang mit fallenden sNSE Werten ab einem BMI von >25 kg/m2. Zusammenfassend lässt sich sagen, dass Vitamin D negativ mit Depression und Adipositas assoziiert war. BDNF zeigte einen Zusammenhang mit Adipositas, aber nicht mit Depression. In Übereinstimmung mit anderen Studien scheint es im Frühstadium von
Adipositas zu akuten neuronalen Schäden zu kommen, wobei die Auswirkungen auf das Gehirn mit steigendem Körperfett zunehmen. Um die Auswirkungen von Depression und Adipositas auf neuronale Schädigung und Differenzierung besser beurteilen zu können und BDNF, Vitamin D und sNSE als Biomarker für diese Prozesse einsetzen zu können, sind weitere Studien nötig. Im klinischen Alltag könnten sich so neue, wirksame Diagnostik- und Therapiestrategien ableiten lassen.
Osteoporose und Atherosklerose wurden lange als unabhängige Erkrankungen mit
gemeinsamen Risikofaktoren betrachtet, jedoch wurde in den letzten Jahren auch ein
unabhängiger Zusammenhang zwischen beiden Erkrankungen postuliert. Bisherige Studien
zur Assoziation zwischen Osteoporose und Atherosklerose sind oft auf postmenopausale
Frauen beschrankt und es gibt es nur wenige Studien, die die Knochengesundheit anhand von Osteosonographiemesswerten beurteilen. Ziel der vorliegenden Arbeit war zu untersuchen, ob es eine unabhängige Assoziation zwischen den Steosonographiemesswerten an der Ferse und der IMT der A. carotis, dem Vorhandensein von Plaques der A. carotis und dem ABI gibt. Grundlage für die Analyse bilden die Daten von SHIP-2 und SHIP-Trend. Multivariate logistische Regressionsmodelle zeigten einen von klassischen kardiovaskularen Risikofaktoren (Hypertonie, Dyslipidamie, Diabetes mellitus) unabhängigen Zusammenhang zwischen den Osteosonographiemesswerten und dem Vorkommen von Plaques der A. carotis bei Männern. Bei Frauen konnte jedoch keine entsprechende Assoziation detektiert werden. Darüber hinaus konnten keine Assoziationen zwischen den Steosonographiemesswerten und der IMT der CCA oder dem ABI in vollständig adjustierten Modellen detektiert werden. IMT und Plaques sind Marker für Atherosklerose, die sich sowohl in ihrer Pathogenese, im Risikofaktorprofil sowie in ihrer Vorhersagefähigkeit fur kardiovaskulare Ereignisse unterscheiden. Plaques stellen dabei den härteren Endpunkt der Atherosklerose dar, was erklären konnte, warum unabhängige Assoziationen zwischen den Osteosonographiemesswerten und Plaques beobachtet wurden aber nicht zur IMT.
Geschlechtsspezifische Unterschiede im Knochenstoffwechsel und der kardiovaskularen
Gesundheit sind denkbare Ursachen für die ausschließlich bei den Männern beobachteten
Assoziationen. Der pathophysiologische Zusammenhang zwischen Osteoporose und
Atherosklerose ist Gegenstand aktueller Forschung. Diskutiert werden u.a. Einflüsse von
Vitamin D, RANK/RANK-L und OPG, MGP, FGF-23 und Klotho, Fetuin A und Cathepsin K.
Während sich die vorliegende Studie durch die große Anzahl an Probanden beider
Geschlechter, einem breiten Altersbereich und einer umfassenden Berücksichtigung von
Confoundern auszeichnet, lasst das querschnittliche Studiendesign keine Ruckschlusse auf
Kausalität der beobachteten Assoziation zu. Weitere Limitationen stellen u.a. die Einnahme
von kardioprotektiver Medikation, die eingeschränkte Vergleichbarkeit mit früheren Studien
aufgrund unterschiedlicher Messmethoden und die Beschränkung der Analysen zum ABI auf
eine Subgruppe dar. Die beobachtete signifikante Assoziation zwischen sinkendem
QUS-basierten SI und dem Vorkommen von Plaques bei Männern weist darauf hin, dass
Männer, die klinische Zeichen einer Osteoporose aufweisen, ein erhöhtes Risiko für
atherosklerotische Erkrankungen, unabhängig von kardiovaskularen Risikofaktoren, wie Alter, BMI, Rauchen, Bewegungsmangel, Diabetes mellitus, Hypertonie und Dyslipidamie haben, sodass diese von einer frühzeitigen Evaluation des atherosklerotischen Risikos profitieren konnten. Insgesamt ist der Zusammenhang zwischen Osteoporose und Atherosklerose komplex und in nachfolgenden Studien wäre es von großem Interesse speziell das männliche Geschlecht in den Fokus zu rucken. Darüber hinaus ist die Betrachtung einzelner Signalstoffe von großem Interesse, um die gegenseitige Beeinflussung beider Erkrankungen tiefgreifender zu verstehen.
Background: The association of polyomaviruses BK and JC with other opportunistic infections and graft-versus-host disease (GvHD) in allogeneic stem cell transplantation is controversially discussed. Methods: We conducted a retrospective study of 64 adult patients who received their first allogeneic stem cell transplantation between March 2010 and December 2014; the follow-up time was 2 years. Results: Acute leukemia was the most frequent underlying disease (45.3%), and conditioning included myeloablative (67.2%) and nonmyeloablative protocols (32.8%). All patients received 10 mg of alemtuzumab on day -2 (20 mg in case of mismatch) as GvHD prophylaxis. Twenty-seven patients (41.5%) developed cytomegalovirus (CMV) reactivation. BKPyV-associated hemorrhagic cystitis was diagnosed in 10 patients (15.6%). Other opportunistic infections caused by viruses or protozoa occurred rarely (<10%). There was no association of BKPyV or JCPyV with CMV reactivation, Epstein-Barr virus reactivation, human herpes virus 6, or parvovirus B19 infection requiring treatment. There was a significant correlation of BKPyV-associated hemorrhagic cystitis with toxoplasmosis (p = 0.013). Additionally, there was a significant link of simultaneous BKPyV and JCPyV viruria with toxoplasmosis (p = 0.047). BKPyV and JCPyV were not associated with GvHD, relapse, or death. Conclusion: We found no association of BKPyV or JCPyV with viral infections or GvHD. Only the correlation of both polyomaviruses with toxoplasmosis was significant. This is a novel and interesting finding.
Background
There is a lack of data concerning socioeconomic outcome and quality of life (QoL) in patients after status epilepticus (SE) in Germany.
Patients and methods
Adult patients treated between 2011 and 2015 due to SE at the university hospitals in Frankfurt, Greifswald, and Marburg were asked to fill out a questionnaire regarding long-term outcome of at least 3 months after discharge. The SE cohort consisted of 25.9% patients with an acute symptomatic, 42% with a remote symptomatic and previous epilepsy, 22.2% with a new-onset remote symptomatic, and 9.9% with other or unknown etiology. A matched case–control analysis was applied for comparison with patients with drug refractory epilepsy and seizure remission, both not previously affected by SE.
Results
A total of 81 patients (mean age: 58.7 ± 18.0 years; 58% female) participated. A non-refractory course was present in 59.3%, while 27.2% had a refractory SE (RSE) and 13.6% had a superrefractory SE (SRSE). Before admission, a favorable modified Rankin Scale (mRS) of 0–3 was found in 82.7% (67/81), deteriorating to 38.3% (31/81) (p = 0.003) at discharge. The majority returned home [51.9% (42/81)], 32.1% entered a rehabilitation facility, while 12.3% were transferred to a nursing home and 3.7% to another hospital. The overall mRS at follow-up did not change; 61.8% (45/74) reached an mRS of 0–3. In RSE and SRSE, the proportion with a favorable mRS increased from 45.5% at discharge to 70% at follow-up, while QoL was comparable to a non-refractory SE course. Matched epilepsy controls in seizure remission were treated with a lower mean number of anticonvulsants (1.3 ± 0.7) compared to controls with drug refractory epilepsy (1.9 ± 0.8; p < 0.001) or SE (1.9 ± 1.1; p < 0.001). A major depression was found in 32.8% of patients with SE and in 36.8% of drug refractory epilepsy, but only in 20.3% of patients in seizure remission. QoL was reduced in all categories (QOLIE-31) in SE patients in comparison with patients in seizure remission, but was comparable to patients with drug refractory epilepsy.
Discussion
Patients after SE show substantial impairments in their QoL and daily life activities. However, in the long term, patients with RSE and SRSE had a relatively favorable outcome comparable to that of patients with a non-refractory SE course. This underlines the need for efficient therapeutic options in SE.
Mittels der vorliegenden Dissertation sollten, anhand eines größeren Patientinnenkollektivs, uterine Leiomyosarkome (LMS) umfassend charakterisiert werden um als Vergleichsmaterial zu regelhaften Leiomyomen (LM) für die Errechnung eines präoperativen LMS-Risiko-Scores zu dienen.
Zu diesem Zweck wurden insgesamt 240 LMS-Beratungsfälle des Deutschen klinischen Kompetenzzentrums für genitale Sarkome und Mischtumoren der Universitätsmedizin Greifswald (DKSM) aus dem Zeitraum August 2009 – September 2017 retro- und prospektiv erfasst und analysiert. Von diesen wurden 154 Fälle einer vorangegangenen Promotion (126) des DKSM entnommen. Letztere wurden mit erweiterten bzw. neu hinzugekommenen Kriterien aktualisiert. Um eine vergleichende Auswertung mit LM zu ermöglichen, war es erforderlich, die relevanten Kriterien in ein Schema zu integrieren, das simultan bei den LM-Analysen der anderen Arbeitsgruppen des DKSM zur Anwendung kam.
Das mediane Alter von 52 Jahren und die höchste Inzidenz zwischen 50 – 59 Jahren decken sich mit Angaben aus der Literatur. Der Anteil postmenopausaler Frauen lag mit 60,8 % über den ermittelten Werten anderer Autoren.
Die anamnestischen Angaben, wie eine Tamoxifenexposition, eine vorausgegangene medikamentöse Therapie mit Ulipristalacetat (UPA) und vorausgegangene Operationen wegen Varianten des LM und eines STUMP (glattmuskulärer Tumor mit unsicherem malignem Potential) spiegeln die Literaturangaben nur bedingt wider. Ein Therapieversagen von UPA bei symptomatischen LM scheint ein Hinweis auf ein uterines Sarkom zu sein. Allerdings wurden in dieser Untersuchung insgesamt nur 8 entsprechende Fälle gefunden. Eine vorausgegangene Operation wegen Varianten des LM bzw. eines STUMP fand sich nur in 2,1 % der Fälle.
Betreffs der Blutungsstörungen wurden bei 44 % der prämenopausalen Frauen intermenstruelle Zusatzblutungen und bei 45,2 % Blutungen in der Postmenopause beschrieben. Zudem wurden tumorbezogene Symptome ohne jegliche Blutungsstörungen, wie uterine oder Unterbauchschmerzen, ein vermehrter Harndrang sowie ein Druckgefühl oder eine Zunahme des Abdominalumfanges in 46,3 % der Fälle beobachtet.
In mehr als der Hälfte der Fälle (53,4 %) lag ein schnelles Tumorwachstum vor. Der Wert lag in der Postmenopause sogar bei 55,5 %. In 45,5 % wurde das LMS als weich bzw. auffällig weich beschrieben.
In 99,6 % aller Fälle wurde eine diagnostische Sonografie durchgeführt. Dabei zeigte sich in 82 % ein als auffällig bezeichneter Befund.
Der Durchmesser der beobachteten LMS betrug median 9 cm. Am häufigsten wiesen die LMS eine Größe zwischen 6 – 10 cm auf. Der Anteil der solitären LMS (keine zusätzlichen LM) lag bei 51,7 %. Der mittlere Durchmesser der solitären LMS betrug 10,3 cm.
Bei 73 der Fälle (30 %) wurde eine diagnostische Abrasio oder eine direkte Biopsie durchgeführt. Unter diesen wurde nur in 38,4 % primär die richtige Diagnose gestellt. Die Literaturangaben zur Stadienverteilung der LMS decken sich nicht mit eigenen Ergebnissen, nach denen sich 79,6 % der Fälle im Stadium I und nur 0,4 % im Stadium IV befanden. Ebenfalls widersprechen die eigenen Daten den Angaben bezüglich einer Lymphknotenbeteiligung. In insgesamt 80,4 % (n = 189 Fälle) wurde keine Lymphonodektomie (LNE) durchgeführt. Insgesamt 10,9 % der LMS mit einer LNE (n = 46 Fälle) wiesen positive Lymphknoten auf.
Die Literaturangaben zur Netz- und Zervixbeteiligung konnten bestätigt werden. Von 235 operierten Frauen fand nur in 21 Fällen (13,2 %) eine Netzresektion statt. In 4 dieser Fälle war das Netz vom Tumor befallen. In 15,1 % der Fälle der vorliegenden Arbeit wurde ein Zervixbefall im Sinne einer Infiltration durch einen korporalen Tumor bzw. einer primären Lokalisation in der Zervix gefunden.
In insgesamt 67,7 % aller Fälle wurde das LMS primär nicht erkannt und konsekutiv unter der falschen Indikation operiert. In 66,0 % bildete ein LM die Operationsindikation bei der Enddiagnose LMS. Dementsprechend wurden nur 35,7 % der LMS mittels totaler Hysterektomie ohne Uterus- und/oder Tumorverletzung operiert.
In 71,3 % der Fälle lagen Daten zur Mitosenzahl je 10 HPF vor. Der errechnete Mittelwert aller LMS betrug 20,7 M/10 HPF und speziell die der spindelzelligen LMS 21,7 M/10 HPF. Diese Zahlen decken sich mit den Angaben aus der Literatur. Das gilt gleichermaßen für die Expression des Ki67-Antigens (im Mittel 37,2 %). Die 5-Jahres- Überlebenswahrscheinlichkeit (Nomogramm Overall Survival Probability nach Zivanovic) lag bei 144 berechenbaren Fällen median bei 48 %. In den vorliegenden Fällen konnten in 8,3 % primäre Fernmetastasen nachgewiesen werden. Diese fanden sich mit 65,1 bzw. 25,3 % am häufigsten in der Lunge und/oder der Leber. Eine Ausbreitung in die Knochen sowie in die Knochen plus simultan in jegliche anderweitigen Strukturen lag nur in jeweils 6,0 % vor.
Die gewonnenen Ergebnisse wurden in der Zentrale des DKSM zur Berechnung des LMS-Risiko-Scores mittels Vergleich mit 670 prospektiv analysierten LMS aus 2 weiteren Promotionen verwendet. Der Score ist zwischenzeitlich publiziert worden.
Background: We assessed the effect of the uniform donor questionnaire (UDQ) on deferral rates in first-time and repeat donors. We focused on the introduced question about unprotected sexual contact with a new partner. Another goal was a stratified comparison of the deferral rates of the donor questionnaire (DQ) and UDQ. Methods: Data on donors and deferrals using the DQ and UDQ were collected at four blood establishments. The comparison included a 2-year period by questionnaire version. For the comparison of the questionnaires, an adjusted multinomial logistic regression was performed. Results: The analysis included 260,848 donations. First-time (FTD) and repeat donations (RD) showed higher deferral rates with the UDQ (FTD +5.4%, RD +1.4%). Deferral due to a new partner was 3.0% in first-time and 0.4% in repeat donors. The majority of these occurred in the youngest age groups. The most frequent deferral criterion was ‘disease' (5.1%). Conclusion: The regression revealed stronger predictors for deferral than the questionnaire version. Especially younger age carried a higher and independent risk for deferral. The additional deferrals of mainly young first-time donors due to a new sexual partner may identify those donors with potential heterosexual risk behavior who would otherwise not be identified.
The dissertation describes an Indigenous dream framework that underscores the significance of dreams as a mirror of trauma and a way that leads back to Indigenous knowledges. Significant differences of Western and Indigenous epistemology are exemplified by juxtaposing Western and Indigenous dream discourses. The selected prose fiction allows for a dream categorization that emphasizes the significance and meaning of dreams as a metaphorical as well as narrative device.
Nightmares/Anxiety dreams are often the result of the devastating effects of colonization and especially Residential School. Nightmares in the texts are often exact replicas of the abuse suffered in the boarding schools. They are discussed in the context of Robert Arthur Alexie’s novel Porcupines and China Dolls (2001/2009) represents dreams and traumatic nightmares and deals with the fictional Blue People First Nation. The community’s collective intergenerational trauma of Residential School experience keeps them stuck in dysfunctional dynamics dominated by suicides, sexual and physical abuse, drug and sex addictions.
Telling dreams, categorized as “instructing dreams,” and “announcing dreams,” teach the dreamer what will happen in the future. They are discussed in the context of Richard Van Camp’s short stories “On the Wings of this Prayer” and “The Fleshing” (Godless but Loyal to Heaven, 2012), which represent the category of the ecological nightmare as well as of telling dreams. Ecological nightmares display environmentally destructive effects of capitalist globalization that have come to “infect” the world. The Windigo figure in the stories serves as a manifestation of resource and in particular petro-capitalism and Western society’s constant need to subjugate nature. Ecological dreams hence call for ecological vigilance and establish Indigenous knowledges as a source of resurgence and restoration.
Existential dreams function as decolonizing tools that facilitate liberation. The thesis provides a literary analysis of Richard Wagamese’ novel Ragged Company (2008) and Cherie Dimaline’s short story “room 414” (Red Rooms, 2007) where homelessness is postulated as the manifestation of individual and tribal/communal disjointedness and isolation. Through existential dreaming, the urban lives of most of the characters dwelling in the shadows and margins of society are existentially transformed and healing seems possible. Paternalistic colonial mindsets continue to patronize Indigenous knowledges (as unreliable and unscientific) until Western “discoveries” prove what has been known for decades.
The thesis underscores dreams as an essential part of Indigenous Knowledges, i.e. as knowledge sources. Surmounting Western dream perceptions and instead valorizing Indigenous knowledges, the characters in the texts discussed in my thesis, unremittingly follow their dreams’ instruction and eventually achieve reconciliation and healing. In the fictional texts discussed, nightmares represent homelessness, trauma, stagnation, and a disconnection to one’s (Native) background, whereas dreams represent continuity through the restoration of identity, finding home, and a sense of belonging. The notion of a dream reality and a waking reality influencing and informing each other relies on sharing dreams with the community, eventually leading to an enactment of the dream or vision. Dreaming and identity are significantly linked and foster processes of intellectual self-determination. The characters’ inability to externalize their internal wishes, desires, and needs results in further denial and consequential bitterness that feed into the spiral of alcohol and drug abuse as well as metaphorical and literal homelessness. The dreams’ semantic field strongly alludes to ceremonial traditions and provides the prospect of a rooted Indigeneity. At the turning points in the lives of the characters, when dreams and visions start to appear, they are lost in translation. The characters’ own illiteracy towards Native epistemology and spirituality has them trapped in the inability to read and act on their dream messages. Strong (often female) Indigenous presences that go hand in hand with the appearance of dreams provide the protagonists with guidance and lead the way back to the “Old Ways.” Through dreaming, the spiral of colonialism is disrupted and replaced by the circle of reconciliation and relationality.
Das Ziel dieser Studie war die Untersuchung der biomechanischen Eigenschaften von einigen Prototypen im Vergleich zueinander und zu einigen kommerziellen MDIs. Die Untersuchung wurde durchgeführt mit Hilfe der Finite Elemente Methoden und konzentrierte sich auf die Ermittlung von simulierten Verformungen und Vergleichsspannungen in den Prototypen und im Knochen. Aufgrund der besonders vorteilhaften Geometrien und Festigkeitswerte, wurden die Prototypen aus der Serie S2 ausgewählt und durch eine Modifikation des Kopfes zu einem marktreifen Produkt weiterentwickelt. Als Ergebnis dieser Studie wurde von der Firma Dentaurum das CITO mini® Implantatsystem auf den Markt gebracht (siehe Produktinformationen unter www.dentaurum.de ).
In dieser retrospektiven Arbeit wurde untersucht, inwiefern Einflussfaktoren (im Folgenden Risikofaktoren) wie Diabetes mellitus, Adipositas, rheumatoide Arthritis sowie invasive Voreingriffe am Kniegelenk das klinische Outcome der Patienten bei bereits festgestellter periprothetischer Infektion beeinflussen. Das Patientenkollektiv hierzu bildeten 74 Personen, welche in der Orthopädischen Klinik der Universitätsmedizin Greifswald zwischen 01.01.2006 - 30.06.2013 wegen einer infizierten Knie-TEP behandelt wurden. Die Infektionsrate lag dabei insgesamt bei 2,3%. Nach entsprechender Datengewinnung wurden innerhalb des Patientenkollektivs vier Gruppenpaare gebildet, wobei pro untersuchtes Paar eine Gruppe mit jeweiligem Risikofaktor von einer entsprechenden Kontrollgruppe ohne den untersuchten Risikofaktor separiert wurde. Diese wurden durch statistische Tests auf signifikante Unterschiede bezüglich des objektiven und subjektiven Outcomes hin untersucht. Bei der Betrachtung des objektiven Outcomes, worin alle Patienten eingeschlossen waren, bestätigte sich der Verdacht, dass Diabetes mellitus sowie Adipositas den klinischen Verlauf bei periprothetischer Infektion negativ beeinflussen, was auch im Vergleich zur Literatur bestätigt werden konnte. Die primär durchgeführte Therapie zum Erhalt der einliegenden Prothese vor allem durch Inlay-Wechsel war in diesen beiden Risikofaktoren weniger erfolgreich als in den jeweiligen Kontrollgruppen. Sogar der TEP-Wechsel als primäre Infektsanierung war in der Gruppe der Diabetiker signifikant erfolgloser als in der jeweiligen Kontrollgruppe. Dies führte insgesamt zu vermehrten operativen Eingriffen bis zum Ende des Beobachtungszeitraumes. Das subjektive Outcome wurde durch den SF-36 Score mittels Fragebogen und telefonischer Kontaktaufnahme ermittelt, wobei 50% aller Patienten teilnahmen. Hierbei zeigte sich ausschließlich bei Diabetes mellitus eine signifikant geringere körperliche Funktion im Vergleich zur Kontrollgruppe. Bezüglich der Risikofaktoren "rheumatoide Arthritis" und "Voreingriffe am Kniegelenk" hat sich generell keine signifikante Beeinflussung des objektiven und subjektiven Outcomes gezeigt. Zusammenfassend kann Diabetes mellitus und Adipositas als Risikofaktor für ein schlechteres Outcome bei festgestellter Knie-TEP Infektion angesehen werden. Vor allem bei der Auswahl der passenden Primärtherapie nach festgestellter Infektion muss die klinische Gesamtsituation betrachtet werden. Dabei ist der TEP-Erhalt vor allem durch Inlay-Wechsel bei Adipositas und Diabetes mellitus weniger erfolgsversprechend ebenso wie der TEP-Wechsel bei Diabetikern, was bei der Therapieplanung bedacht werden sollte, um zukünftig multiple operative Eingriffe bei adipösen Patienten und körperliche Funktionseinschränkungen bei Diabetikern zu vermeiden.
Streptococcus pneumoniae infections can lead to severe complications with excessive immune activation and tissue damage. Interleukin-37 (IL-37) has gained importance as a suppressor of innate and acquired immunity, and its effects have been therapeutic as they prevent tissue damage in autoimmune and inflammatory diseases. By using RAW macrophages, stably transfected with human IL-37, we showed a 70% decrease in the cytokine levels of IL-6, TNF-α, and IL-1β, and a 2.2-fold reduction of the intracellular killing capacity of internalized pneumococci in response to pneumococcal infection. In a murine model of infection with S. pneumoniae, using mice transgenic for human IL-37b (IL-37tg), we observed an initial decrease in cytokine expression of IL-6, TNF-α, and IL-1β in the lungs, followed by a late-phase enhancement of pneumococcal burden and subsequent increase of proinflammatory cytokine levels. Additionally, a marked increase in recruitment of alveolar macrophages and neutrophils was noted, while TRAIL mRNA was reduced 3-fold in lungs of IL-37tg mice, resulting in necrotizing pneumonia with augmented death of infiltrating neutrophils, enhanced bacteremic spread, and increased mortality. In conclusion, we have identified that IL-37 modulates several core components of a successful inflammatory response to pneumococcal pneumonia, which lead to increased inflammation, tissue damage, and mortality.
Background: Mild-to-moderate chronic kidney disease (CKD G3a) is prevalent in older adults. Substantial evidence suggests that individuals with advanced CKD face a high risk for common geriatric conditions, like functional impairment and cognitive decline, whereas the relationships between mild-to-moderate CKD and functional impairment and cognitive decline, but also poor nutritional status and mood disorders, are still unclear. Objective: The aim of this study was to explore associations between mild-to-moderate CKD and impairments in the core domains of geriatric assessment (GA) in a large cohort of community-dwelling older adults. Methods: This was a cross-sectional analysis of 1,476 participants of the Berlin Aging Study II. Study participants were stratified as to presence or absence of CKD G3a (estimated glomerular filtration rate [eGFR] 45-59 mL/min/1.73 m<sup>2</sup> vs. eGFR ≥60 mL/min/1.73 m<sup>2</sup>). GA comprised the following instruments: the Activities of Daily Living Scale (ADL), the Timed up and Go (TUG), the Tinetti test (Tinetti), the Mini-Mental-State Examination (MMSE), the Geriatric Depression Scale (GDS), and the Mini Nutritional Assessment (MNA). We used logistic regression models to estimate multivariable-adjusted associations between CKD G3a and impairments in the respective domains. Results: A total of 282 subjects with mild-to-moderate CKD (CKD G3a) were identified (19.1%). Overall, the prevalence of impairments identified was higher among subjects with compared to without CKD G3a (21 vs. 15.9%, p = 0.043). In multivariable-adjusted models, CKD G3a was consistently associated with increased odds of an impaired gait performance as to the TUG (adjusted odds ratio 2.06, 95% CI 1.04-4.09). In contrast, on average, individuals with and without CKD G3a did not differ as to their results in the MMSE, the ADL, the MNA, and the GDS. Conclusion: GA identified impairments in 21 versus 15.9% of older adults with and without mild-to-moderate CKD, respectively. However, except for an increased likelihood of impaired gait performance (TUG) with mild-to-moderate CKD, we did not find independent associations between mild-to-moderate CKD and geriatric conditions.
Wound antisepsis has undergone a renaissance due to the introduction of highly effective wound-compatible antimicrobial agents and the spread of multidrug-resistant organisms (MDROs). However, a strict indication must be set for the application of these agents. An infected or critically colonized wound must be treated antiseptically. In addition, systemic antibiotic therapy is required in case the infection spreads. If applied preventively, the Wounds-at-Risk Score allows an assessment of the risk for infection and thus appropriateness of the indication. The content of this updated consensus recommendation still largely consists of discussing properties of octenidine dihydrochloride (OCT), polihexanide, and iodophores. The evaluations of hypochlorite, taurolidine, and silver ions have been updated. For critically colonized and infected chronic wounds as well as for burns, polihexanide is classified as the active agent of choice. The combination 0.1% OCT/phenoxyethanol (PE) solution is suitable for acute, contaminated, and traumatic wounds, including MRSA-colonized wounds due to its deep action. For chronic wounds, preparations with 0.05% OCT are preferable. For bite, stab/puncture, and gunshot wounds, polyvinylpyrrolidone (PVP)-iodine is the first choice, while polihexanide and hypochlorite are superior to PVP-iodine for the treatment of contaminated acute and chronic wounds. For the decolonization of wounds colonized or infected with MDROs, the combination of OCT/PE is preferred. For peritoneal rinsing or rinsing of other cavities with a lack of drainage potential as well as the risk of central nervous system exposure, hypochlorite is the superior active agent. Silver-sulfadiazine is classified as dispensable, while dyes, organic mercury compounds, and hydrogen peroxide alone are classified as obsolete. As promising prospects, acetic acid, the combination of negative pressure wound therapy with the instillation of antiseptics (NPWTi), and cold atmospheric plasma are also subjects of this assessment.
Background: ‘Quality in medicine' is a term used in a broad sense. In this work the definition and dimensions of quality in medicine and the implementation of a measurement and reporting system in Germany are discussed. Existing applications are described and possible future effects are pointed out. Methods: The ongoing process of implementing a quality reporting system into the German healthcare system is studied by publicly available legal texts, published reactions of stakeholders and publications of G-BA and IQTIG. Definitions of quality, dimensions of quality and quality measurement in medicine are studied by using textbooks as well as the world wide web and PubMed search. Results: Donabedian‘s ‘dimensions of quality' are fundamental in dealing with quality in medicine. Existing measurement and reporting systems have immanent strengths and weaknesses, as the definition of quality is affected by one‘s point of view. The legislator will have to decide which ‘dimension of quality' is mandatory and how to measure it. Conclusion: Quality has become a control instrument with unforeseeable consequences. A clear definition of the used quality concept is as essential as the use of feasible measurement and reporting systems. The use of routine data could be an interesting option.
Der vorliegende Übersichtsartikel belegt zunächst, warum das Extinktionslernen als ein zentraler Wirkmechanismus der Expositionsbehandlung angesehen wird. Nach Darstellung der lerntheoretischen Grundlagen wird ein Modell präsentiert, das die Grundlagen der Ausformung, Konsolidierung und des Abrufs des Extinktionsgedächtnisses beschreibt. Dieses Gedächtnismodell der Extinktion liefert die Basis für die Diskussion der aktuellen neurowissenschaftlichen Erkenntnisse zum Extinktionslernen. Durch diese Befunde im Tier-, aber auch im Humanbereich ist es gelungen, die neuronalen Schaltkreise der Extinktion relativ gut zu beschreiben. Der Übersichtsartikel gibt eine Zusammenfassung dieser aktuellen Befunde und geht außerdem auf einige Neurotransmittersysteme dieser Schaltkreise ein, zumindest in dem Maße, wie sie für die Befunde zur pharmakologischen Unterstützung des Extinktionslernens relevant sind. Anschließend wird ein integratives Modell vorgestellt, das den Ausgangspunkt für die Optimierung der Extinktion in der Expositionstherapie liefert. Den Abschluss bildet eine Kasuistik, in der diese Optimierungsstrategien nochmals am Beispiel der Behandlung einer Patientin mit Emetophobie für die Praxis verdeutlicht werden.
Background: Securing future blood supply is a major issue of transfusion safety. In this prospective 10-year longitudinal study we enrolled all blood donation services and hospitals of the federal state Mecklenburg-Western Pomerania. Methods and Results: From 2005 to 2015 (time period with major demographic effects), whole blood donation numbers declined by 18%. In male donors this paralleled the demographic change, while donation rates of females declined 12.4% more than expected from demography. In parallel, red cell transfusion rates/1,000 population decreased from 2005 to 2015 from 56 to 51 (-8.4%), primarily due to less transfusions in patients >60 years. However, the transfusion demand declined much less than blood donation numbers: -13.5% versus -18%, and the population >65 years (highest transfusion demand) will further increase. The key question is whether the decline in transfusion demand observed over the previous years will further continue, hereby compensating for reduced blood donation numbers due to the demographic change. The population structure of Mecklenburg-Western Pomerania reflects all Eastern German federal states, while the Western German federal states will reach similar ratios of age groups 18-64 years / ≥65 years about 10 years later. Conclusions: Regular monitoring of age- and sex-specific donation and transfusion data is urgently required to allow transfusion services strategic planning for securing future blood supply.
Less invasive caries management techniques for treating cavitated carious primary teeth, which involve the concept of caries control by managing the activity of the biofilm, are becoming common. This study aimed to compare the clinical efficacy (minor/major failures) and survival rates (successful cases without any failures) of 3 carious lesion treatment approaches, the Hall Technique (HT), non-restorative caries treatment (NRCT), and conventional restorations (CR), for the management of occlusoproximal caries lesions (ICDAS 3-5) in primary molars. Results at 2.5 years are presented. A total of 169 children (3- to 8-year-olds) were enrolled in this secondary care-based, 3-arm parallel-group, randomised controlled trial. Participants were allocated to: HT (n = 52; sealing caries with stainless-steel crowns without caries removal), NRCT (n = 52; opening up the cavity and applying fluoride varnish), CR (n = 65; control arm, complete caries removal and compomer restoration). Statistical analyses were: non-parametric Kruskal-Wallis analysis of variance, Mann-Whitney U test and Kaplan-Meier survival analyses. One hundred and forty-two participants (84%; HT = 40/52; NRCT = 44/52; CR = 58/65) had follow-up data of 1-33 months (mean = 26). Overall, 25 (HT = 2, NRCT = 9, CR = 14) of 142 participants (17.6%) presented with at least 1 minor failure (reversible pulpitis, caries progression, or secondary caries; p = 0.013, CI = 0.012-0.018; Mann-Whitney U test). Ten (HT = 1, NRCT = 4, CR = 5) of 142 participants (7.04%) experienced at least 1 major failure (irreversible pulpitis, abscess, unrestorable tooth; p = 0.043, CI = 0.034-0.045). Independent comparisons between 2 samples found that NRCT-CR had no statistically significant difference in failures (p > 0.05), but for CR-HT (p = 0.037, CI = 0.030-0.040) and for NRCT-HT (p = 0.011, CI = 0.010-0.016; Kruskal-Wallis test) significant differences were observed. Cumulative survival rates were HT = 92.5%, NRCT = 70.5%, and CR = 67.2% (p = 0.012). NRCT and CR outcomes were comparable. HT performed better than NRCT and CR for all outcomes.
