Diese longitudinale populationsbasierte Studie im Rahmen der Study of Health in Pomerania (SHIP) untersuchte die Wechselwirkungen von sozioökonomischen Faktoren und systemischer Entzündung (CRP) auf die Parodontitisprogression und den Zahnverlust. Daten von 2566 Probanden innerhalb eines fünfjährigen Untersuchungszeitraumes wurden analysiert. Untersucht wurde der Einfluss von Bildung, Einkommen, Familienstand, Rauchverhalten und Übergewicht mittels multipler Regressionsanalysen auf Attachmentverlust und Poisson- Regressionsanalyen auf Zahnverlust. Resultierend ergab sich ein signifikanter Zusammenhang zwischen niedriger Bildung und geringem Einkommen auf das Zahnverlustrisiko (IRR 1.63, p < 0.001 und 1.25, p < 0.001) und auch auf die Progression des mittleren klinischen Attachmentverlusts (p < 0.010 und p = 0.046). Ein Anstieg der CRP- Werte war mit niedrigen sozioökonomischen Faktoren sowie mit Rauchen und Übergewicht assoziiert. Ebenso führten niedrige sozio-ökonomische Faktoren, besonders bei Vorhandensein hoher CRP- Werte, zu mehr Zahnverlust (p < 0.001). Übergewicht wirkte sich besonders nachteilig auf den Zahnverlust und den Attachmentverlust bei männlichen Probanden aus. Verheiratete männliche Probanden verzeichneten dagegen eine Abnahme des Attachments- und Zahnverlusts. Die Untersuchung bestätigte den Einfluss sozioökonomischer Faktoren auf die Parodontitisprogression und zeigt, dass hohe CRP-Werte den Einfluss niedriger sozioökonomischer Faktoren auf Zahn- und Attachmentverlust verstärken können.
Reactive oxygen species (ROS) can damage all cellular macromolecules and also produce secondary reactive intermediates, like reactive electrophilic species (RES) that include quinones or aldehydes. Low molecular weight (LMW) thiols are small thiol-containing compounds that play essential roles in the defense against ROS and RES in all organisms. The best studied LMW thiol is the tripeptide glutathione (GSH). Firmicutes bacteria including Bacillus und Staphylococcus species have been recently discovered to utilize the redox buffer bacillithiol (BSH). LMW thiols function as redox buffers to maintain the reduced state of the cytoplasm. Under conditions of oxidative stress, LMW thiols also react with protein thiols to form mixed LMW thiol – protein disulfides, termed S-thiolations, as major protection mechanism. Investigating the role of BSH in oxidative stress response and ROS-induced S-thiolations in Firmicutes bacteria was one subject of this PhD thesis. Specifically, the regulatory mechanisms and post-translational thiol-modifications in response to NaOCl stress were studied in the model bacterium for low-GC Gram-positive bacteria Bacillus subtilis. The transcriptome profile after NaOCl stress was indicative of disulfide stress and overlapped strongly with the response to diamide. NaOCl stress caused induction of the thiol- and oxidative stress-specific Spx, CtsR, PerR and OhrR regulons. Thiol redox proteomics identified only few NaOCl-sensitive proteins with reversible thiol-oxidations. Using mass spectrometry, eleven proteins were identified that were oxidized to mixed BSH protein disulfides (S-bacillithiolated) in B. subtilis cells after NaOCl-exposure. Methionine synthase MetE is the most abundant S-bacillithiolated protein in B. subtilis and other Bacillus species after NaOCl exposure. S-bacillithiolation of OhrR repressor leads to upregulation of the OhrA peroxiredoxin that confers together with BSH specific protection against NaOCl. S-bacillithiolation of MetE, YxjG, PpaC, and SerA causes hypochlorite-induced methionine starvation as supported by the induction of the S-box regulon. To further assess the conservation of targets for S-bacillithiolations in other Firmicutes bacteria, we studied the S-bacillithiolomes of Bacillus megaterium, Bacillus pumilus, Bacillus amyloliquefaciens, and Staphylococcus carnosus under NaOCl stress conditions. In total, 54 S-bacillithiolated proteins were identified, including 29 unique proteins and 8 conserved proteins involved in amino acid and cofactor biosynthesis, nucleotide metabolism, translation, protein quality control, redox and antioxidant functions. Together our data support a major role of BSH redox buffer in redox control and thiol protection of conserved and essential proteins against irreversible oxidation by S-bacillithiolations in Firmicutes bacteria. In response to ROS and RES, bacteria also activate the expression of antioxidant and detoxification enzymes, such as catalases, peroxidases, thiol-dependent peroxiredoxins and other specific oxidoreductases to detoxify ROS and RES. These defense mechanisms are often controlled by redox-sensitive transcription factors. B. subtilis encodes redox-sensing MarR-type regulators belonging to the OhrR and DUF24-families that are conserved among bacteria. Hence, we were further interested in this PhD thesis to study at the molecular and structural level the redox-sensing mechanisms of novel redox-sensing MarR/DUF24-type regulators in B. subtilis. We have characterized the regulatory mechanisms of HypR, YodB and CatR that sense and respond to hypochlorite, diamide and quinones stress. HypR is the first DUF24-family regulator whose crystal structure was resolved. HypR senses specifically disulfide stress and controls positively expression of the flavin oxidoreductase HypO after NaOCl and diamide stress. HypR resembles a 2-Cys-type regulator with a reactive nucleophilic N-terminal Cys14 and a second C-terminal Cys49. Besides HypR, B. subtilis encodes further MarR/DUF24-family members including the paralogous YodB and CatR repressors that sense quinones and diamide. YodB controls the azoreductase AzoR1, the nitroreductase YodC, and the Spx regulator. YodB resembles a 2-Cys-type MarR/DUF24-family regulator with three Cys residues (Cys6, Cys101, and Cys108) that form intermolecular disulfides in vivo under oxidative stress. YodB and its paralog CatR were further identified as repressors of the catDE operon encoding a catechol-2,3-dioxygenase that also contributes to quinone resistance. Although CatR is a 1-Cys-type regulator, our data showed that CatR also forms intermolecular disulfide in response to diamide and quinones in vitro. Thus, HypR, YodB and CatR are controlled by 2-Cys-type thiol-disulfide redox switches to sense disulfide and RES stress conditions, and to control specific RES detoxification enzymes.
Hintergrund: Interspinöse Implantate werden in der Behandlung lumbaler Spinalkanalstenosen und segmentaler Instabilität im Bereich der Lendenwirbelsäule eingesetzt. Ein Vertreter ist der 1986 von Jaques Sénégas entwickelte Wallis®-Spacer (Abbott, Bordeaux, Frankreich). Durch Einschränkung des Bewegungsumfanges des betroffenen Bewegungssegmentes wird eine dynamische Stabilisierung erreicht. In Studien sind für verschiedene interspinöse Implantate kurzfristig gute Ergebnisse in der Schmerzreduktion und Funktionalität belegt. Neben Studien der Entwickler selbst, mangelt es an längerfristigen Ergebnissen nach Implantation interspinöser Implantate. Zielsetzung: Ziel der Arbeit war es, klinische und radiologische Ergebnisse nach einem Mindestbeobachtungszeitraum von 24 Monaten nach Implantation des Wallis®-Spacers zu evaluieren. Studiendesign: In einer retrospektiven Kohortenstudie wurden klinische und radiologische Daten von 64 Patienten der Klinik für Orthopädie und Unfallchirurgie des Asklepios Klinikum Uckermark in Schwedt/ Oder erhoben, bei denen im Zeitraum zwischen 01/2005 und 12/2007 ein Wallis®-Spacer implantiert wurde. Material/ Methodik: Zur Beurteilung der funktionellen Beeinträchtigung sowie von Rücken- und Beinschmerzen wurden der Oswestry Disability Index (ODI) und die visuelle Analogskala (VAS) verwendet. In konventionellen Röntgenaufnahmen wurden Gesamt- und segmentaler Lordosewinkel (nach Cobb), vordere (vBSH) und hintere Bandscheibenhöhe (hBSH) sowie Olisthesebildungen bestimmt. Außerdem erfolgte die Beurteilung der Grenzflächen zwischen Implantat und Knochen. Ausschlusskriterien waren Revisionseingriffe innerhalb von 24 Monaten und fehlende radiologische Daten. Vollständige Datensätze von 21 Patienten (8 weiblich, 13 männlich, mittleres Alter 59,8 Jahre [Min. 35, Max. 79]) mit einem durchschnittlichen Beobachtungszeitraum von 44,7 Monaten (Min. 24,3; Max. 64,7) wurden ausgewertet. Ergebnisse: Die Implantation erfolgte bei der Mehrzahl der Patienten in Höhe L4/5 (n=11). Es zeigte sich keine klinisch bedeutsame Verbesserung zum Zeitpunkt der Nachuntersuchung (NU) gegenüber der präoperativen Ausgangssituation (ODI präoperativ 40,9%; NU 31,1%; VAS-Besserung: Rückenschmerz 8% [präoperativ 6,6; NU 5,8], Beinschmerz 17% [präoperativ 6,5; NU 4,8]). Ein entlordosierender Effekt auf das operierte Bewegungssegment wurde nicht nachgewiesen (präoperativ 6,1°; postoperativ 6,1°; nach Mindestbeobachtungszeitraum 5,8°). Die Bandscheibenhöhen vergrößerten sich postoperativ und verringerten sich bis zur NU (vBSH/ hBSH: präoperativ 9,4 mm/ 6,0 mm; postoperativ 10,3 mm/ 6,8 mm; NU 8,3 mm/ 4,7 mm). Radiologisch wurden im Verlauf bei 18 Patienten (85,7%) deutliche Resorptionssäume um das Implantat nachgewiesen. Bei fünf Patienten (23,8%) bildeten sich radiologisch nachweisbare Ossifikationen um das Implantat. Angrenzend an das Implantat ereigneten sich bei zwei Patienten (9,5%) Dornfortsatzfrakturen. Bei 6 Patienten (28,6%) wurde zum Zeitpunkt der Nachuntersuchung die Indikation für eine Revisionsoperation gestellt. Diskussion: Die vorliegende Arbeit ist als retrospektive Studie mit fehlender Kontrollgruppe, kleiner Patientenkohorte, breitem Nachuntersuchungsintervall und Einschluss verschiedener Operationsverfahren in ihrer Aussagekraft limitiert. Es ließ sich im Nachuntersuchungszeitraum von mehr als 24 Monaten röntgenologisch kaum ein Einfluss auf das Bewegungssegment nachweisen. Die klinische Verbesserung blieb gering. Im Verlauf kam es zur Ausbildung von Resorptionssäumen und eine bedeutsame Anzahl an Revisionsoperationen wurde notwendig. Durch die vorliegende Arbeit bleibt ungeklärt, welche Beeinflussung von dem interspinösen Spacer ausgeht. Fazit: Für eine längerfristige Verwendung des Wallis®-Spacers sollte die Indikation unter Berücksichtigung der zeitlich limitierten Wirkung im Einzelfall kritisch diskutiert werden.
In terms of climate change and climate change mitigation, the quantitative knowledge of global carbon pools is important information. On the one hand, knowledge on the amount of carbon cycling among – and stored in – global pools (i.e. Atmosphere, Biosphere, Cryosphere, Hydrosphere, and Lithosphere) may improve the reliability of models predicting atmospheric CO2 concentrations in terms of fossil fuel combustion. On the other hand, the carbon sequestration potential of specific ecosystems allows for estimating their feasibility regarding carbon trade mechanisms such as the Clean Development Mechanism or the Reducing Emissions from Deforestation and Degradation Program (REDD+). However, up to date, the majority of terrestrial carbon assessments have focused on forests and peatlands, leaving a data gap open regarding the remaining ecosystems. This data gap is likely to be explained by the relatively high carbon densities and/or productivities of forests and peatlands. Nevertheless, to get a precise as possible global picture, information on carbon pools and sequestration of other ecosystems is needed. Although desert ecosystems generally express low carbon densities, they may absolutely store a remarkable amount of carbon due to their large areal extent. In this context, Central Asian Deserts (in particular within the Turanian Deserts, i.e. Karakum, Kysylkum, Muyunkum) likely inhibit comparably high carbon pools as they express a sparse vegetation cover due to an exceptionally high annual precipitation if compared to the World’s deserts. In this dissertation, three important woody plant species – Populus euphratica and Haloxylon aphyllum and Haloxylon persicum – of Central Asian Deserts were investigated for their carbon pools and carbon sequestration potential. These species were chosen as they I) locally express high carbon densities, II) are dominant species, III) have a rather large spatial distribution, and IV) have experienced a strong degradation throughout the 20th century. Thus, they likely show a remarkable potential for carbon re-sequestration through restoration and thus for an application of carbon trade mechanisms (CHAPTER I). P. euphratica was investigated in the nature reserve Kabakly at the Amu Darya, Turkmenistan and in Iminqak at the Tarim He, Xinjiang, China. The assessment of Haloxylon species was restricted to the Turanian deserts west of the Tain Shan. To achieve a first scientific basis for large scale estimates, different methodologies, ranging from allometric formulas, over dendrochronology to remote sensing were combined (CHAPTERS II-V). In CHAPTER II allometric formulas were successfully developed for Haloxylon aphyllum and Haloxylon persicum and applied to six study sites distributed over the Turanian Deserts to represent the allometric variability of Haloxylon species in Central Asia. CHAPTER III derives another allometric formula (only based on canopy area) for H. aphyllum and combines it with a remote sensing analysis from the nature reserve Repetek. Thereby, a first large scale estimate covering the Northeastern Karakum Desert of carbon pools related to mono specific H. aphyllum stands is achieved. CHAPTER IV describes the wood structure of Populus euphratica forests in the nature reserve Kabakly (Turkmenistan) and in Iminqak (Xinjiang, China). In CHAPTER V a dendrochronological approach derives models for predicting the Net Primary Productivity (NPP) and the age of P. euphratica in the nature reserve Kabakly. Thereby, a first feasibility assessment regarding remote sensing analyses and the upscaling of the obtained NPP results is carried out. First estimates based on these local studies (CHAPTER VI), reveal carbon densities ranging from 0.1 – 26.3 t C ha 1 for the three investigated species. Highest maximum and median carbon densities were found for P. euphratica, but Haloxylon aphyllum expressed remarkable maximum carbon densities (13.1 t C ha-1), too. The total carbon pools were estimated at 6480 kt C for P. euphratica, 520 kt C for H. aphyllum stands and 6900 kt C for Haloxylon persicum shrubland. Accounting for the extent of degraded areas, the total re-sequestration potentials of the respective species were estimated at 4320 kt C, 1620 kt C and 21900 kt C, this highlighting the remarkable absolute re-sequestration potential of H. persicum shrubland despite its low average carbon densities. In the end, the main results were put into a broader context (CHAPTER VI), discussing the general feasibility of reforestations both in ecological terms as well as in terms of carbon trade mechanisms. A short example highlights the strong connection between the feasibility of reforestations and the global carbon market. Finally, open research questions are brought forth revealing the yet large research potential of Central Asian Desert ecosystems in general and in terms of carbon sequestration.
Untersuchung von 183 Bundeswehrsoldaten, die alle eine unversorgte Schaltlücke im Seitenzahnbereich aufweisen. Es sollte überprüft werden, ob unversorgte Schaltlücken einen Einfluss auf die mundbezogene Lebensqualität haben. Diese wurde anhand des OHIP G 14 Fragebogens ermittelt. Zusätzlich haben die Probanden 17 Fragen aus dem zahnärztlichen Interview der SHIP Studie beantwortet. Die Probanden wurden entsprechend der Lokalisation der Lücke in 8 Gruppen unterteilt, die Modelle sind mit einem Cementographen vermessen worden. Referenzwert war die gesunde, kontralaterale Seite des jeweiligen Probanden. Das mundbezogene Verhalten von Probanden mit Ein- und Mehrzahnverlust war nicht unterschiedlich. Die Mundbezogene Lebensqualität war bei persistierender Seitenzahnlücke nicht reduziert. Es konnte keine Korrelation zwischen Grad der Lückenverengung und OHIP Score festgestellt werden. Es waren Zahnstellungsveränderungen vorhanden, auch Elongationen, Kippungen und Funktionsstörungen
Inflammation is an adaptive response that is triggered by noxious stimuli and conditions, such as infection and tissue injury. Neutrophils, eosinophils, monocytes, tissue macrophages and dendritic cells can all ingest bacteria, tissues debris and apoptotic cells after injury or infection. These cells derived from bone marrow progenitors, circulate in the blood and migrate to peripheral tissues. Macrophages produce and secrete a cascade of pro-inflammatory and anti-inflammatory cytokines, such as interleukin-6 (IL-6), IL-10, and IL-12 that are trafficked and secreted by constitutive exocytosis. IL-10 and IL-6 are known to be rapidly induced during infection and / or injury, which make them possible mediators of early phagocyte recruitment. This thesis work aimed at detailed investigation of role of these cytokines in peritoneal inflammation. Under normal physiological conditions peritoneal cavity of normal BALB/c mice contains mainly CD45+ lymphocytes and CD11b+ myeloid cells with typical macrophage phenotype. The resident peritoneal cells play an important role in organismal homeostasis by taking part in innate and adaptive immunity. To explore this in detail, the physiological properties of peritoneal resident macrophage populations were studied under steady state and during inflammation conditions. Upon rapid induction of sterile inflammation by thioglycollate or lipopolysaccharide, the resident peritoneal cells could no longer be recovered in a peritoneal wash 6h after treatment. During ceacal content (CC) peritonitis, these cells were lost even more rapidly. Neutrophils, monocytes and lymphocytes replace the resident peritoneal phagocyte populations. During sepsis the absence of peritoneal macrophages decreases neutrophils recruitment to the inflammatory site and subsequently increases sepsis. Upon peritoneal wash cell transfer, total peritoneal cells could be recovered from the peritoneum of non infected mice, whereas these cells disappeared after CC infection in mice. The fate of resident peritoneal cells and their migration into lymphoid organs such as omentum and parathymic lymph nodes was further studied following induction of peritoneal infection. The CC infection induced lost cells from peritoneum were emigrated into omentum and parathymic lymph nodes but not in mesenteric lymph nodes. R1 cells were mostly observed in parathymic lymph nodes after 72h of infection but not after 1h, whereas, R2 cells were selectively observed in omentum just 1h after infection and 72h as well. These results were further confirmed by adoptive transfer showing emigration of R2 cells into omentum 1h after infection. Additionally, analysis of cytokine production after CC peritonitis showed early production of IL-10 and IL-6, which is in agreement with earlier findings and further supports the importance of these cytokines in phagocyte recruitment. The role of IL-10, IL-6 and other cytokines as possible mediators of early inflammation and in the recruitment of monocytes, neutrophils or eosinophils to the peritoneum during inflammation was determined by cytokine application. The intraperitoneal application of IL-10 recruited monocytes, neutrophils, T cells, B cells and eosinophils to the peritoneum. However, IL-10 knockout mice showed even increased recruitment of leucocytes to the peritoneal cavity in CC infection suggesting their IL-10 independent recruitment with the exception of eosinophils. Even though eosinophils are effector cells which are recruited to the site of inflammation; during homeostasis eosinophils constitute an abundant leukocyte population in the gastrointestinal tract. Therefore, possible role of eosinophils in bacterial infection was further studied using Δdbl GATA mice which lack mature eosinophils. In the absence of eosinophils, the monocyte and neutrophil recruitment was unaffected after CC infection, while there was increased T and B cell recruitment at the same time. The Δdbl GATA mice also showed reduced production of IL-4, 18h after infection. The eosinophils secrete IL 4 which may induce alternative macrophage activation. These results together with cytokine administration and IL-10 ko mouse data suggest a novel and major role of IL-10 in attracting and in recruiting eosinophils after peritoneal infection. Altogether, present thesis work demonstrates a new aspect of IL-10 interaction with eosinophils in mouse peritoneal environment during peritonitis. It gives a new insight for understanding the possible role of eosinophils in modulating the peritoneal environment in resolution of bacterial infection and can be useful in designing new approaches for therapeutic strategies in combating sepsis and peritoneal inflammation.
Die Polyarteriitis nodosa ist eine seltene nekrotisierende Vaskulitis der mittelgroßen Arterien, die erstmalig Kussmaul und Maier 1866 beschrieben haben. Peters et al. konnten in ihrem Tiermodell die Polyarteriitis nodosa in Cyp1a1ren2 transgenen Ratten reproduzierbar induzieren. Notwendige Faktoren sind die proreninabhängige Aktivierung des Renin-Angiotensin-Systems und die Implantation eines telemetrischen Blutdrucksenders in die Aorta. Die orale Darbietung von I3C führt zur Aktivierung des Cyp1a1-Promoters und daraus resultierend zu einer dosisabhängigen Expression von Prorenin aus dem murinen ren2-Transgen. Die Implantation des telemetrischen Blutdrucksenders stimuliert das Immunsystem, was über den Nachweis von undefinierten Autoantikörpern sichtbar wird. Nur die Kombination all dieser Faktoren ist ausreichend, um eine PAN zu induzieren. Die Studie untersucht, ob die Polyarteriitis nodosa der Cyp1a1ren2 transgenen Ratte abhängig vom AT1-Rezeptor ist, ob sich ihr Verlauf beeinflussen lässt und ob die Vaskulitisschäden teilweise oder sogar ganz heilbar sind. Nach einer Krankheitsinduktionsphase von 6 Wochen erfolgte bei einem Teil der Ratten die Therapie mit dem AT1-Rezeptorblocker Losartan. Losartan konnte das Fortschreiten der PAN stoppen. Infolge der Therapie war bei keiner dieser Ratten histologisch eine aktive PAN mehr nachweisbar. Diese Studie liefert starke Hinweise darauf, dass Losartan eine bestehende PAN bei der Cyp1a1ren2 transgenen Ratte zur Abheilung bringt. Dies begleitet eine Reendothelialisierung. Residual findet sich häufig eine Fibrose. Die immunhistochemische Untersuchung der Organe zeigte, dass sich im Bereich der von aktiver PAN betroffenen Blutgefäße der nicht therapierten Gruppe eine vermehrte Anzahl an CD4-positiven Immunzellen insbesondere Makrophagen/Monozyten befinden; diese können pathognomonisch bedeutsam sein. Die vorliegende Arbeit liefert starke Hinweise darauf, dass die Polyarteriitis nodosa der Cyp1a1ren2 transgenen Ratte AT1-Rezeptor vermittelt ist und eine Losartantherapie zur Regression der Erkrankung führen kann.
Sowohl Kaubewegungen als auch deren Kaumuster erweisen sich als sehr komplexe und variantenreiche Vorgänge, welche durch zahlreiche Faktoren beeinflusst werden können. Bei 271 vollbezahnten Probanden, davon 109 männliche und 162 weibliche, aus der bevölkerungsrepräsentativen Basisstudie SHIP-0 wurden die Kaubewegungen mit dem Jaw Motion Analyser (JMA) der Firma Zebris Medical (D-Isny) aufgezeichnet. Als viskoelastisches Kaugut wurde jeweils ein Gummibärchen der Firma Haribo (D-Bonn) verwendet. Die Ergebnisse dieser Studie zeigen keine eindeutigen Unterschiede der Variabilität der Öffnungs- und Schließbewegungen des Inzisalpunktes in okklusaler Nähe in Bezug auf Geschlecht, Alter, bevorzugter Kauseite oder Dysfunktionsgrad. Lediglich die Schließbewegungen weisen eine tendenzielle Abnahme der Variabilität bei steigendem Dysfunktionsgrad auf. Wie auch bei den Kaubewegungen besteht bei den Kaumusterfeldverteilungen eine hohe Ähnlichkeit zwischen angewiesenem Rechts- und Linkskauen. Die Auswertung zeigt, dass weder die Bevorzugung einer Kauseite noch das Geschlecht einen erkennbaren Einfluss haben. In Abhängigkeit vom Alter hingegen nimmt tendenziell mit steigendem Alter die relative Anzahl der Fälle mit zahngeführten Anteilen (KMF A) zu. Die Auswertung der Kaumusterfeldverteilung nach allgemeinen Dysfunktionsgraden lässt keine eindeutige Zuordnung einzelner Kaumusterfelder (KMF) zu. Das tropfenförmige KMF C tritt bei allen Schweregraden verhältnismäßig am häufigsten auf. Lediglich im konkreten Fall der Gelenkdysfunktion ließ sich für beide angewiesenen Kauseiten eine charakteristische Zunahme der prozentualen Anzahl der Fälle mit Dysfunktionen im KMF D feststellen. Im Einzelfall hat eine Kaumusterklassifizierung somit keine gesicherte Aussagekraft über das Vorhandensein oder gar die Art einer Dysfunktion.
This thesis aims at improving the current representation of adaptation in economic frameworks of climate change by a) accounting for the time-dependent evolution of the adaptive capacities of countries and b) quantifying unwelcome feedbacks of the adaptation process. In this context, it is proposed that economic assessments of climate change incorporate adaptation as a cyclic and phase-dependent process while devising their cost methodologies. A phase-dependent process acknowledges the existence of adaptation barriers while a cyclic process accounts for potential unwanted feedbacks of adaptation. By analyzing economic assessments against this framework, it is shown that dependencies between phases of adaptation and phases altogether are often disregarded. Furthermore, potential negative consequences associated with adaptation are rarely considered and adaptation is generally assumed to be unconstrained. The assumption of unconstrained adaptation is only acceptable in the context of high adaptive capacity. This concept was further investigated through a review of vulnerability assessments regarding their operation of the adaptive capacity component. It was found that adaptive capacity is mostly equated to proxies that reflect the knowledge, financial and livelihood capacities of the system under analysis. With this theoretical considerations in mind, a dynamic representation of adaptive capacity was elaborated at a country-level. The Human Development Index (HDI) was used as a proxy of the adaptive capacity of countries and its evolution in time extrapolated. The time required for countries to achieve developed world standards of human development was then estimated. The results indicate that between 2005 and 2020, half of the world population will live in countries with low adaptive capacity. This percentage is then progressively reduced to 15% in the year 2050, with marked regional differences. The time required for a country to achieve an appropriate level of development sets a clear constraint on when, and to what extent, the country can engage on climate change adaptation. This does not imply that adaptation will not take place before development occurs. Rather, it calls for adaptation options to be tailored in order to t the current and future adaptive capacities of countries. Obtaining higher levels of adaptive capacity is likely to be associated with negative consequences for the climatic system. The statistical relation between HDI and per-capita emissions of countries was established and future projections made. Between 2010 and 2050 approx. 300 Gt of CO2 are estimated to be associated with the increase of adaptive capacities of current developing countries. This value represents about 30% of the allowed CO2-budgets to restrict global temperatures to an increase of 2 degrees by 2100 compared to pre-industrial times - conditional to a 25% risk of failing to meet the target. For the case of sea-level rise, the modelling framework DIVA (Dynamic Interactive Vulnerability Assessment) was used in order to illustrate the drawbacks of a simplistic representation of adaptation. The results show that adaptation via the construction of protective infrastructure might be economically feasible for particular countries. For others, modeled results fail to provide a clear choice between adaptation or inaction. The assumption of unconstrained adaptation resulted in the valuation of costly protection options whose financial and knowledge requirements can be at odds with the capacities of some coastal countries - namely developing countries. Further, infrastructural protection as adaptive measure to prevent coastal damages can have the counter-productive effect of raising the amount and value of assets at risk. This is a direct result of DIVA disregarding the potential unwelcome feedbacks of adaptation itself. In conclusion, the full potential of economic assessments of climate adaptation is likely to remain unlocked as long as adaptation continues to be misrepresented. The methodologies discussed in this work provide a way forward to alleviate this deficiency in forthcoming assessments. For the case of sea-level rise, the modeling framework DIVA (Dynamic Interactive Vulnerability Assessment) was used in order to illustrate the drawbacks of a simplistic representation of adaptation. The results show that adaptation via the construction of protective infrastructure might be economically feasible for particular countries. Objective: The purpose of this study was to determine the accuracy and reliability of Frankfort horizontal plane identification using displays of multi-planar reconstructed MRI images, and propose it as a sufficiently stable and standardized reference plane for craniofacial structures Materials and Methods: MRI images of 43 adolescent randomly selected subjects were obtained from the longitudinal population based cohort study SHIP-2 using a T1-weighted 3D sequence. Five examiners independently identified the three landmarks that form FH plane. Intra-examiner reproducibility and inter-examiner reliability, correlation coefficients (ICC), coefficient of variability and Bland-Altman plots were obtained for all landmarks coordinates to assess reproducibility. Intra-examiner reproducibility and inter-examiner reliability in terms of location and plane angulation were also assessed. Results: Intra- and inter-examiner reliabilities for X, Y and Z coordinates of all three landmarks were excellent with ICC values ranging from 0.914 to 0.998. Differences among examiners were more in X and Z than in Y dimensions. The Bland–Altman analysis demonstrated excellent intra- as well as inter-examiner agreement between examiners in all coordinates for all landmarks. Intra-examiner reproducibility and inter-examiner reliability of the three landmarks in terms of distance showed mean differences between 1.3 to 2.9 mm, Mean differences in plane angulation were between 1.0° to 1.5° among examiners. Conclusion: This study revealed excellent intra-examiner and inter-examiner reproducibility of Frankfort Horizontal plane through 3D landmark identification in MRI. Sufficiently stable landmark-based reference plane could be used for different treatments and studies.
The development of innovative coatings with multifunctional properties is an ambitious task in modification of material surfaces. A novel approach is a hybrid method combining the non-thermal plasma processing with nanotechnology for the development of multifunctional surface coatings. The conception of the hybrid coating process is based on three steps: the preparation of a suspension consisting of an organic liquid and functional nanoparticles, the deposition of the suspension as a thin liquid film on the material surface, and the plasma modification of the liquid organic film to achieve a thin solid composite film with embedded nanoparticles demonstrating multifunctional properties and good adherence on the substrate material. In this work the liquid polydimethylsiloxane (PDMS) was applied as a model system, and the experimental investigations were focused on the PDMS plasma modification. In particular, the specific role of the different plasma components and the influence of the plasma and processing parameters on the PDMS modification were studied. The applied capacitively coupled radio frequency (CCRF) plasma was analyzed by electric probe measurements and optical emission spectroscopy, whereas the molecular changes in PDMS due to plasma-induced chemical reactions were studied by the Fourier transform infrared reflection absorption spectroscopy. Additionally, the photocatalytic activity of thin composite films consisting of plasma cross-linked PDMS with embedded TiO2 nanoparticles was demonstrated. During the investigation it was found that the CCRF discharge modifies efficiently thin liquid PDMS films to solid coatings. The samples were positioned in the plasma bulk at floating potential. The penetration depth of particles like neutrals, ions, electrons and radicals in the film is strongly limited. The heating of samples in the CCRF discharge is weak to modify PDMS by itself and only the plasma radiation is able to transform the liquid bulk to solid one. It is known that the absorption onset of PDMS lies in the VUV region (below 200 nm). The energetic VUV radiation penetrates into the PDMS film on a thickness from several hundred nanometers to few micrometers and initiates photochemical reactions there. Thus, different gases like Ar, Xe, O2, H2O, air and H2 were tested to provide the strongest VUV emission intensity of the CCRF discharge. Discharge pressure and power were varied for all these gases and it was found that at all conditions the H2 plasma demonstrates drastically stronger emission. Thus, H2 gas was selected for the plasma treatment of liquid PDMS films. The IRRAS analysis revealed the transformation process of PDMS with the degradation of CH3 groups, the formation of new groups like SiOH, CH2 and SiH, the formation of the SiOx material and crosslinking. It was found that the modification effect is not uniform across the film thickness. The top region with an initial thickness up to 100 nm loses all CH3 groups, in the underlying region the CH3 concentration increases gradually from zero to the value for PDMS, if the film was thick enough. The methyl-free SiOx top layer contains also SiOH and SiH groups. Furthermore, the SiH groups are concentrated only in a very thin layer with a thickness below 10 nm. The presence of the unscreened polar SiOSi and SiOH groups on the surface causes the adsorption of H2O from the atmosphere, which was also observed by IRRAS. By means of the spectroscopic ellipsometry it was found out that all above described regions experience a shrinking. The reason is the crosslinking and loss of material. The most shrunken layer is the top SiOx layer with the shrinking ratio (final thickness/initial thickness) of 0.55 - 0.60. Further, this ratio gradually rise up to the value of 0.95 in the deeper region, which has the concentration of CH3 groups of about that for PDMS. After the analysis of all results the depth of effective modification was estimated at 300 400 nm for the most optimal conditions. The optimization of the plasma VUV intensity was realized by variation of discharge pressure and power. The strongest plasma emission at studied conditions provided the irradiance of the sample of ca. 13 mW/cm2. However, such strong radiation causes very strong production rate of the gases. These products leave the modifying film slower as they are produced, what causes their accumulation in there. Their pressure grows up leading to formation of bubbles, which later explode. Finally, the film becomes heavily damaged. To avoid this effect the pressure and the RF power were changed to reduce the irradiance to 6 - 7 mW/cm2. This resulted in the absence of any damages.
Background: Fetal growth failure has been associated with an increased risk of hypertension, cardiovascular disease and diabetes in adulthood. Exploring the mechanisms underlying this association should improve our understanding of these common adult diseases. Patients and Methods: We investigated 225 SNPs in 10 genes involved in growth and glucose metabolism (GH1, GHR, IGF1, IGF1R, STAT5A, STAT5B, MAPK1, MAPK3, PPARγ and INS) in 1,437 children from the multinational NESTEGG consortium: 345 patients born small for gestational age who remained short (SGA-S), 288 who showed catch-up growth (SGA-Cu), 410 idiopathic short stature (ISS) and 394 controls. We related genotype to pre- and/or postnatal growth parameters, response to growth hormone (if applicable) and blood pressure. Results: We found several clinical associations for GH1, GHR, IGF1, IGF1R, PPARγ and MAPK1. One SNP remained significant after Bonferroni's correction: IGF1R SNP rs4966035's minor allele A was significantly more prevalent among SGA and associated with smaller birth length (p = 0.000378) and birth weight (weaker association), independent of gestational age. Conclusion:IGF1R SNP rs4966035 is significantly associated with birth length, independent of gestational age. This and other associations suggest that polymorphisms in these genes might partly explain the phenotype of short children born SGA and children with ISS.
Ein Mythos ist weder eine Denkweise noch eine Erzählung, er ist ein Prinzip, das sich im Text als unbewusster Subtext unabhängig von Inhalt und Plot zeigt. Dieses Prinzip ist charakterisiert durch seine drei Merkmale Narrativität, Fiktionalität und Funktionalität. Politische Mythen tragen zur Nationenbildung bei. Im Falle Litauens ist die Literatur der Zwischenkriegszeit sehr geeignet für die Untersuchung politischer Mythen. Diese Dissertation behandelt 24 Romane aus der litauischen Zwischenkriegszeit von Gabrielė Petkevičaitė, Vincas Mykolaitis, Ieva Simonaitytė, Vladas Andriukaitis, Liudas Dovydėnas, Jurgis Jankus, Sofija Kymantaitė-Čiurlionienė, Juozas Tumas, Juozas Paukštelis, Juozas Grušas, Antanas Žukauskas, Jonas Marcinkevičius, Ignas Šeinius, Kazys Puida, Kazys Boruta, Petronėlė Orintaitė-Janutienė, Šatrijos Ragana, Antanas Vaičiulaitis und Petras Cvirka. Die Romane beinhalten politische Mythen, die sich auf das Großfürstentum Litauen, auf das litauische Dorf und auf den Widerstand gegen Gegner der litauischen Unabhängigkeit beziehen. Interessanter Weise entspricht die so konstruierte litauische nationale Identität nicht immer den Werten, die zur Gründung des litauischen Nationalstaats geführt haben.
Bei dieser Studie wurden die röntgenologischen Veränderungen des Knochenniveaus nach systematischer Parodontalbehandlung sowohl auf Probanden- als auch auf Zahngruppenebene (Incisivi, Canini, Prämolaren, Molaren) untersucht. Die Auswertung erfolgte im zeitlichen Abstand von etwa 5 Jahren anhand von Einzelzahnaufnahmen, bei denen der Knochenabbau in mm vermessen wurde. Bei 10 von insgesamt 47 Patienten lag nach der Parodontalbehandlung eine exzellente klinische Heilung vor. Diese Patienten erfüllten nicht die Eingangskriterien für die weitere Studienteilnahme und wurden somit in der röntgenologischen Auswertung nicht berücksichtigt, jedoch verdeutlichen sie den klinischen Behandlungserfolg. Der Effekt der Parodontalbehandlung ist besonders am röntgenologischen Knochenzuwachs bei vertikalen Knochenkratern erkennbar, welcher auf Probandenebene durchschnittlich 1,1 mm beträgt und statistisch signifikant ist. Insgesamt haben sich 22,3% aller Knochendefekte von einem vertikalen in einen horizontalen Knochenabbau entwickelt bzw. sind „ausgeheilt“. Die Parodontalbehandlung bei dieser Studie erfolgte nur bei motivierten Patienten unter „optimalen Bedingungen“ ohne Zeitdruck. Aufgrund von ökonomischen Aspekten und dem breiten Patientenspektrum einer allgemeinen zahnärztlichen Praxis kann der Behandlungserfolg nicht verallgemeinert werden. Jedoch bleibt festzuhalten, dass der klinische Behandlungserfolg einer sorgfältig durchgeführten Parodontaltherapie sich nach 5 Jahren radiologisch dokumentieren lässt.
Background: Hepatitis E virus (HEV) is the etiological agent of an acute self-limiting hepatitis in humans worldwide. The main route of infection is by ingestion of food or water contaminated with the virus. In Germany, several hundred human cases are reported each year, while preliminary studies suggest a high infestation rate of herds of domestic pig (Sus scrofa domesticus) and sounders of wild boar (Sus scrofa). Autochthonous cases are originating mainly from zoonotic transmission from domestic pig and wild boar, but other animals may also be involved. Recently, a novel strain of HEV (ratHEV) had been found in Norway rats (Rattus norvegicus) in Germany, that could contribute to human epidemiology. Therefore, the aim of this study was to assess the seroprevalence of both HEV and the novel ratHEV in human, domestic pig and rat. For each of the three mammal species, an indirect immunoglobulin G (IgG) enzyme-linked immunosorbent assay (ELISA) was established, that based on an Escherichia coli-expressed carboxy-terminal segment (GT3-Ctr, amino acid (aa) 326–608) of the capsid protein of the autochthonous genotype 3 (GT3), derived from a wild boar from Germany. In parallel, a segment from ratHEV homologous to GT3-Ctr was also expressed in E. coli (ratHEV-Ctr, aa315–599) and was used in the ELISA. Hence, the established tests detect antibodies directed against HEV GT3 when using GT3-Ctr as antigen and ratHEV when using ratHEV-Ctr. Results: The GT3-based in-house human IgG test was validated using a commercial assay and showed high specificity and sensitivity. The average human population (represented by a panel of blood donors from Berlin and Brandenburg) reached a seroprevalence of 12.3% (37/301) with the in-house ELISA. A panel of forestry workers from Brandenburg had an even higher seroprevalence of 21.4% (119/555). Furthermore, ratHEV-specific antibodies could be detected in several sera of forestry workers. The novel ratHEV-based rat IgG ELISA could not be compared to similar tests, however, parallel testing with GT3-Ctr and statistical inference allowed conclusion of a seroprevalence. Rats trapped from several sites in Germany had an overall seroprevalence of 24.5% (36/147). The sera were reactive exclusively with ratHEV-Ctr. As with the in-house ELISA for human sera, the porcine IgG test was validated using a commercial assay, yielding high specificity and sensitivity. A panel of domestic pigs from ten federal states of Germany showed a seroprevalence of 42.7% (383/898) when tested with the in-house ELISA. Reactivity with ratHEV was present, but seemed to be caused mostly by cross-reactivity to GT3-Ctr. Conclusion: The HEV seroprevalence observed for human sera of the average population of Germany is among the highest in Europe and has been confirmed recently by other authors. The high seroprevalence found in forestry workers suggests that they should be counted as a risk group for HEV infection. Populations of rats have been shown to be infested heavily with ratHEV, as rats from all trapping sites situated within cities had a high prevalence for ratHEV exclusively and no serum reacted exclusively with GT3-Ctr. Seroprevalence in domestic pigs was demonstrated to be distributed evenly across federal states and districts. However, a vast difference of infestation could be detected in different herds, suggesting either differences in husbandry conditions, or an external source of infection that acts locally only. The rare but exclusive reactivity of human sera with ratHEV as well as the high cross-reactivity of swine sera with ratHEV suggests that viral strains other than the ones already known may contribute to cases of hepatitis E.
Staphylococcus (S.) aureus nimmt dem Menschen gegenüber eine ambivalente Rolle ein, da er zum einen häufiger Kommensale ist, aber auch zum Pathogen werden kann. Zu den bedeutendsten Gegnern von S. aureus zählen die neutrophilen Granulozyten. Sie haben eine kurze Lebensspanne und weisen hochspezialisierte Zelltodstrategien, wie Apoptose oder NETose auf. Auffälligerweise besitzen S. aureus-Isolate, die Furunkulose auslösen, überproportional häufig das Gen für das Panton-Valentine-Leukozidin (PVL), welches hochspezifisch humane neutrophile Granulozyten lysiert. Zu den Interaktionen zwischen S. aureus und Neutrophilen sind noch viele Fragen ungeklärt. Die Ziele dieser Arbeit waren deshalb (1) die Charakterisierung der Wirkung von S. aureus auf das Zelltodverhalten von Neutrophilen und (2) der Vergleich dieser Mechanismen bei kommensalen S. aureus-Isolaten und Isolaten von Patienten mit chronisch rekurrenter Furunkulose. Dazu wurde der Zelltod von Neutrophilen mit einem DNA-Freisetzungstest (Sytox-Assay) und mittels Immunfluoreszenzfärbung analysiert. Auffällig waren dabei folgende Beobachtungen: (1) Hohe Konzentrationen der S. aureus-Kulturüberstände führten zur Nekrose der Neutrophilen. In sublytischen Konzentrationen bewirkten Überstände der stationären Wachstumsphase dagegen eine Verzögerung der natürlichen Apoptose der Neutrophilen in Kultur, deren Fähigkeit zur proinflammatorischen Aktivierung dabei erhalten blieb. Es ist vorstellbar, dass sich S. aureus, von dem inzwischen bekannt wurde, dass er auch intrazellulär persistieren kann, auf diese Weise in den Neutrophilen eine Überlebensnische schafft. Bei Stimulation mit lebenden Bakterienzellen konnten konzentrationsabhängig Nekrose, Apoptose und geringfügig NETose der Neutrophilen beobachtet werden. (2) Der Vergleich von S. aureus-Isolaten aus nasaler Besiedlung und Furunkuloseinfektion zeigte, dass bakterielle Überstände von Furunkulose-Isolaten, die die PVL-kodierenden Gene besaßen, eine deutlich stärkere Lyse der Neutrophilen verursachten. Dieser Effekt war nur dann zu beobachten, wenn die Bakterien tatsächlich PVL bildeten, wozu sie nur in besonders reichhaltigen Medien in der Lage waren. Dies ist ein starker Hinweis darauf, dass der Zelltod durch PVL verursacht wurde. Mit lebenden Bakterien konnten zwischen beiden Gruppen keine Unterschiede in der Zelltodinduktion der Neutrophilen festgestellt werden. Die Ergebnisse dieser Arbeit zeigen, dass S. aureus neutrophile Granulozyten auf verschiedene Weise beeinflussen kann: Neben der Induktion von Zelltod können die Bakterien auch Substanzen freisetzen, die die Lebensspanne der Abwehrzellen verlängern. Außerdem stützen die Befunde das Konzept einer zentralen Rolle für PVL bei Haut- und Weichteilinfektionen durch S. aureus, das bisher vor allem auf epidemiologischen Befunden basierte.
Diese Arbeit untersucht den Zusammenhang zwischen 10 verschiedenen Höckerneigungen vom Eckzahn bis zum zweiten Molar von Ober- und Unterkiefer und ihre Bedeutung für die funktionelle Okklusion. Dafür wurden Abdrücke von 18 Probanden genommen und eingescannt. Die generelle Verteilung der Höckerneigungen unterstützt das Konzept der sequenziellen Okklusion mit Eckzahndominanz. Die statistische Analyse erbrachte mathematische Zusammenhänge zwischen den verschiedenen Winkeln. Es wurde eine Art Formelwerk erstellt, das im Vergleich zu CAD-/CAM-Systemen ungeeignet für die Rekonstruktion des individuellen Zahns ist. Es könnte jedoch einen Ansatz zur Optimierung von Prothesenzähnen darstellen.
Der Naturstoff Splitomicin war als selektiver Inhibitor (der Aktivität) der NAD+- abhängigen Histon-Desacetylase des Sir2-Proteins Ausgangspunkt der Untersuchungen, ist aber an humanen Enzymen inaktiv und aufgrund der im menschlichen Körper zu erwartenden Hydrolyse der Verbindung weder als Arzneistoff noch als Grundgerüst für Analoga geeignet. Da die Stabilität der Verbindung Voraussetzung dafür ist, dass die Verbindungen ihre Wirkung entfalten können, und eine Verschiebung der Selektivität hin zu humanen Enzymen beabsichtigt war, wurde im Rahmen dieser Arbeit Splitomicin zwar als Leitstruktur übernommen aber das heterocyclische Ringsystem auf der Grundlage von Struktur-Aktivitätsbeziehungen modifiziert. Der in Splitomicin enthaltene Sechsring des Pyranons wurde durch einen Fünfring ersetzt. Neben anellierten Imidazolonen wurden Imidazolthione und Chinolinimidazolthione dargestellt, um die Stabilität der Verbindungen gegenüber Hydrolyseeinflüssen zu erhöhen.
Background: High resolution three-dimensional (3-D) magnetic resonance imaging (MRI) is widely used to predict the neurovascular anatomy within the cerebellopontine angle. Objective: To assess value of 3-D three-dimensional Steady-state free precession imaging (SSFP) and Time-of-flight magnetic resonance angiography (TOF MRA) and in detecting the offending vessels in hemifacial spasm by comparison to intraoperative endoscopic visualization. Methods: 42 patients underwent endoscope-assisted microvascular decompression (MVD). All available preoperative 3-D SSFP and TOF MRA images were checked. Intraoperative videos were captured by a high definition endoscopic camera attached to endoscopes while exploring the area of facial nerve root exit zone (REZ). Evaluation of the 3-D images was performed by two independent groups of observers and compared with the operative findings. Results: 3-D MRI had an average positive predictive value (PPV) of 89.1% in differentiating between simple and complex compression. Mean accuracy of the before mentioned images in detection of the offending vessels was 83.3% and 77% according to the first and second group of observers respectively. Averaged inter-observer agreement between the two groups of observers was substantial with an averaged Kappa coefficient (K) of 0.56. In simple compression group, mean accuracy was 97% and 89.4% according to the first and second group of observers respectively. Averaged K for agreement was substantial (K=0.65). Conclusion: According to endoscopic visualisation, 3-D SSFP and TOF MRA images are accurate in detecting the offending vessels in simple compression of the facial nerve, and in predicting presence of a complex compression with variable sensitivity in identifying all offending vessels.
Moderne Arzneistoffentwicklungsprogramme erbringen in zunehmendem Maße schwer wasserlösliche Arzneistoffe. Dies bringt pharmazeutisch-technologische und biopharmazeutische Probleme für deren Formulierung mit sich. Eine ausreichende Löslichkeit ist notwendig für die Herstellung von intravenös zu applizierenden Zubereitungen und die Durchführung von in vitro Untersuchungen z.B. im Rahmen der Arzneistoffentwicklung. Eine schlechte Löslichkeit kann die Resorption verzögern und so die Bioverfügbarkeit von oral verabreichten Arzneistoffen beeinträchtigen. Lösungsvermittler bieten eine Möglichkeit, die Wasserlöslichkeit von Arzneistoffen zu verbessern und finden breite Anwendung in vielen zugelassenen Arzneimitteln und v.a. in der präklinischen und klinischen Entwicklung. Trotz des Anspruchs der pharmakologischen Inaktivität wurde in der Literatur für verschiedene Lösungsvermittler jedoch ein Einfluss auf die Pharmakokinetik von Arzneistoffen beschrieben. Die Absorption, Verteilung und Elimination eines Arzneistoffes wird zu großen Teilen von Transportproteinen und metabolisierenden Enzymen bestimmt. Als Ursache für die pharmakokinetischen Veränderungen wurde eine Interaktion der Hilfsstoffe mit dem Effluxtransporter P-Glykoprotein (ABCB1) und dem metabolisierenden Enzym Cytochrom P450 (CYP) 3A4 erkannt. Zum Einfluss auf Aufnahmetransporter gab es bisher nur wenige Erkenntnisse für Cremophor EL. Da sie dem Metabolismus und Efflux vorgelagert sind, spielen Aufnahmetransporter eine besondere Rolle. Daher gab es einen speziellen Bedarf an Wissen über den Einfluss von Lösungsvermittlern auf Aufnahmetransporter. In der vorliegenden Arbeit wurde der Einfluss der Lösungsvermittler Polyethylenglykol (PEG) 400, Hydroxypropyl-β-cyclodextrin (HPCD), Solutol HS15 (SOL) und Cremophor EL (CrEL) auf die Aufnahmetransporter organic anion transporting polypeptide (OATP) 1A2, OATP1B1, OATP1B3, OATP2B1 und Na+ / taurocholate cotransporting polypeptide (NTCP) an zellulären Transportermodellen untersucht. PEG 400 hemmte selektiv OATP1A2. HPCD hemmte bei allen Transportern nur die Aufnahme von Substraten mit Sterangrundgerüst vermutlich durch Komplexbildung, stimulierte jedoch die NTCP-abhängige Aufnahme von Bromosulfophthalein (kein Steran). SOL und CrEL hemmten alle Transporter. Für OATP1B1 und NTCP (Hemmung oberhalb der kritischen Mizellbildungskonzentration (CMC)) ist ein mizellares trapping als Ursache wahrscheinlich. Für OATP1A2, OATP1B3 und OATP2B1 (Hemmung unterhalb der CMC) müssen spezifische Mechanismen involviert gewesen sein. Pharmakokinetische Interaktionen mit Hilfsstoffen können also auch auf Ebene der Aufnahmetransporter stattfinden. Im zweiten Teil der Arbeit wurde die in vivo Relevanz der Befunde überprüft. In einer Studie wurde der Einfluss von PEG 400, HPCD und SOL auf die Pharmakokinetik der Modellarzneistoffe Paracetamol, Talinolol, Colchicin und Ciclosporin A nach intravenöser Applikation an Ratten untersucht. Dabei erwies sich keiner der Hilfsstoffe als vollständig inert. Es wurde eine Vielzahl unterschiedlicher Effekte beobachtet. Häufig traten Veränderungen in der AUC, der Halbwertzeit und der Verteilung auf. Grundsätzlich schienen alle in Frage kommenden Mechanismen auch in vivo eine Rolle zu spielen. Die in der Literatur beschriebene Hemmung von Effluxtransport und Phase I Metabolismus konnte bestätigt werden. Die in vitro beobachtete Hemmung von Aufnahmetransportern konnte in vivo belegt werden. Auch unspezifische Mechanismen wie Komplexbildung und mizellares trapping schienen in vivo relevant zu sein. Durch die Überlagerung verschiedener Mechanismen ergab sich jedoch ein sehr komplexes Bild, das sowohl von den Eigenschaften des jeweiligen Hilfsstoffes als auch von denen des Arzneistoffes geprägt war. Daher konnten in den meisten Fällen nur allgemeine Hypothesen zu den möglichen Ursachen der Interaktion aufgestellt werden. Aus demselben Grund ist keine Extrapolation der Daten auf andere Lösungsvermittler und Arzneistoffe im Sinne einer Vorhersage möglich, da die wenigsten Substanzen ausreichend gut charakterisiert sind. Dennoch lässt sich schlussfolgern, dass Lösungsvermittler ein gewisses Interaktions-potenzial besitzen, das sich nicht nur auf ABCB1 und CYP3A4 beschränkt. Damit können sie an Arzneimittelinteraktionen beteiligt sein. Diese Effekte sollten somit auch in der Arzneimittelentwicklung berücksichtigt werden.
