Background
Adolescents and young adults (AYAs) with chronic conditions face a transfer, defined as an actual shift from paediatric to adult-oriented health care. Transition competence as the self-perceived knowledge, skills and abilities regarding the transition process was considered extremely useful.
Aim
This study was designed to investigate the impact of transition competence before and after the transfer on disease-specific quality of life (QoL) and health care satisfaction of AYAs with diabetes.
Results
In total, a sample of N = 90 AYAs with diabetes self-reported their transition competence, diabetes-specific QoL and satisfaction with care. Multiple linear regressions were used to analyse the impact of transition competence on satisfaction with care and QoL. Transition competence positively influenced the outcomes of satisfaction with care and QoL.
Conclusion
Young adults with diabetes showed higher transition competence scores than adolescents with diabetes.
Obstructive sleep apnea is known to be an overall public health problem that, among other things, increases morbidity and mortality. Risk factors as well as symptoms of this multidimensional sleep-related breathing disorder negatively affect quality of life. With our study we aimed to expose the association between obstructive sleep apnea and quality of life in the population of Pomerania, Germany. We utilized data from the population-based Study of Health in Pomerania (SHIP). Information on health status and risk factors about 4420 participants (2275 women) were gathered within the cohort SHIP-TREND, of which 1209 (559 women) underwent an overnight polysomnography and completed sleep questionnaires. The quality of life of the participants was measured using the Short-Form 12 questionnaire. For our study, an ordinal regression analysis with age, sex, body mass index and the Short-Form 12 health survey as predictors for apnea–hypopnea index was computed. The potential factors affecting quality of life are different between physical and mental dimensions of quality of life. Significant effects were found regarding age, sex, body mass index and the Short-Form 12 Mental Component Score, but not the Physical Component Score.
Parental cancer poses major challenges for families with minor children. Due to diagnosis and treatment family life is disrupted. To prevent long-term consequences in all family members and to design needs-oriented family-centered interventions, further understanding of the family's situation including the impact on the children, quality of life levels and the parental psychosocial needs is necessary. This study aims at investigating the impact of parental cancer on the minor children, family-specific psychosocial needs and quality of life levels of parents and children. Cancer patients parenting at least 1 minor child (<18 years) were eligible for study participation. In total, n=86 cancer patients under treatment participated in the study. After excluding participants without a minor child, 78 patients remained for analyses. We assessed children's quality of life using the parent proxy version of the KIDSCREEN-10 and parental quality of life using the EORTC QLQ C30 quality of life questionnaire. Additionally, the questionnaire comprised open questions about positive and negative changes parents perceived in their children as well as questions on specific family- and child-related psychosocial needs. The majority of participants were mothers (91%), mainly diagnosed with breast cancer (59%). The participating parents provided data on 117 minor children. Parents mentioned positive changes in 38% of the children (e.g., being more attentive and helpful). Negative changes were reported in 37% of the children (e.g., being more anxious and clingy). Parents reported family-specific psychosocial supportive care needs for themselves as a parent (e.g., support regarding parenting concerns), support needs for the partner or the children. Moreover, parents expressed family-related information needs and needs regarding practical aspects (e.g., childcare, household help). Global quality of life was M = 55.7 (SD = 23.4) for parents and M = 57.5 (SD = 15.5) for children. Pearson's correlation coefficient between parental and children's quality of life was 0.377 (p < 0.001). To identify parents with cancer and children in need for additional support, health care providers should proactively inquire about the impact of the disease on the children. In terms of a comprehensive cancer care, the direct assessment of family impact and family-specific support needs in cancer patients with minor children allows for needs-based allocation to support offers.
Quality of Life in Young Adults With Cerebral Palsy: A Longitudinal Analysis of the SPARCLE Study

Introduction: While most people with cerebral palsy (CP) will have a life expectancy similar to that of the general population, international research has primarily focused on childhood and adolescence; and knowledge about the quality of life (QoL) of young adults with CP, its trajectories, and associated factors remains scarce.
Methods: This longitudinal study included young adults with CP living in five European regions and who had previously participated in the SPARCLE cohort as children and/or adolescents. Their QoL in the psychological well-being and social relationships domains was estimated using age-appropriate validated instruments (KIDSCREEN-52 in childhood/adolescence and WHOQOL-Bref in young adulthood). We used generalized linear mixed-effect models with random intercept to estimate long-term trajectories of QoL in both domains and to investigate whether severity of impairment, pain, and seizure influenced these trajectories. We sought to identify potentially different trajectories of QoL from childhood to adulthood using a shape-based clustering method.
Results: In total, 164 young adults with CP aged 22–27 years participated in the study. Psychological well-being linearly decreased by 0.78 points (scale 0–100) per year (95% confidence interval (CI) −0.99 to −0.56) from childhood to young adulthood, whereas QoL in the social relationships domain increased (β coefficient 1.24, 95% CI 0.92–1.55). Severity of impairment was associated with reduced QoL in all life periods of the study (childhood, adolescence, and young adulthood): motor impairment with social relationships, and more nuancedly intellectual disability with psychological well-being and social relationships. At all periods, frequent pain significantly reduced psychological well-being, and seizures were associated with lower QoL in the social relationships domain. In both domains, we identified a group of individuals with CP who presented a reverse trajectory compared with the general QoL trajectory.
Conclusion: Identification of QoL trajectories and their associated factors yields improved knowledge about the experience of individuals with CP until young adulthood. Further studies are needed to better understand the determinants that have the greatest influence on the different shapes of long-term trajectories of QoL.

A conceptual, operational, and psychometric elaboration of QoL in the context of TM is needed, because standardized instruments to assess QoL do not sufficiently represent essential aspects of intended outcomes of telemedical applications (TM). The overall aim is to develop an instrument that can adequately capture QoL in TM. For that purpose, an extended working model of QoL will be derived. Subsequently, an instrument will be developed and validated that captures those aspects of QoL that are influenced by TM. The initial exploratory study section includes (a) a systematic literature review, (b) a qualitative survey for concept elicitation, and (c) pre-testings using cognitive debriefings with patients and an expert workshop. The second quantitative section consists of an online expert survey and two patient surveys for piloting and validation of the newly developed instrument. The resulting questionnaire will assess central experiences of patients regarding telemedical applications and its impact on QoL more sensitively. Its use as adjunct instrument will lead to a more appropriate evaluation of TM and contribute to the improvement of care tailored to patients’ individual needs.

Nasenseptumperforationen stellen ein seltenes Krankheitsbild dar, welches jedoch mit erheblichen physischen und psychischen Beeinträchtigungen einhergehen kann. Eine mögliche Therapieform ist der chirurgische Verschluss. Dieser stellt hinsichtlich der OP-Methode und ihrer Erfolgschancen eine große Herausforderung dar. Ziel dieser Studie war es, anhand standardisierter Fragebögen und objektiver Messungen postoperative Langzeitergebnisse zu gewinnen, um das Abwägen für und wider eine Operation zu erleichtern. Hierfür wurden 57 Patienten aus der Klinik für Hals-Nasen-Ohrenheilkunde, Kopf- und Halschirurgie der Universitätsmedizin Greifswald befragt, die zwischen 1993 und 2009 mittels Brückenlappentechnik nach Schultz-Coulon operiert worden waren. Es handelte sich um 24 weibliche und 33 männliche Patienten mit einem Durchschnittsalter von 48 Jahren. Das Patientengut wurde in erfolgreich verschlossene (n= 35) und reperforierte Fälle (n= 22) geteilt. Es wurde die Lebensqualität beider Gruppen eruiert und bewertet. Dabei konnten Erkenntnisse durch Ermitteln aus Patientenakten, Befragung mittels standardisierter Fragebögen (SNOT 20-D, Lund McKay, NOSE-D, SF-36) und objektiver Messmethoden (Sniffin‘ Sticks, Akustische Rhinometrie, Rhinomanometrie, Rhinoresistometrie) gewonnen werden. Zur klinischen Nachuntersuchung erschienen 25 der Patienten. Die Auswertung der Fragebögen SNOT 20-D, Lund McKay und NOSE-D lieferte keine signifikanten Ergebnisse bezüglich der spezifischen rhinologischen Lebensqualität. Im SF-36 konnte hingegen eine signifikant verbesserte allgemeine Lebensqualität der Patienten mit erfolgreich verschlossenem Septum nachgewiesen werden. Sowohl die psychischen als auch physischen Skalenwerte waren höher als bei Patienten mit Reperforationen. Diese Spätrezidive traten bis zu 16 Jahre postoperativ auf. Spezifische Riechtests zeigten ein besseres Riechvermögen bei erfolgreicher Operation. Die Akustische Rhinometrie ergab keine signifikanten Unterschiede im Gruppenvergleich. Rhinoresistometrie und Rhinomanometrie deuteten eine leichtgradige Obstruktion der Nase nach erfolgreichem Septumverschluss an. Daraus wird eine Beziehung zwischen der anatomischen Beschaffenheit der Nase und einer Nasenatmungsbehinderung vermutet. Es bestand ein Zusammenhang zwischen objektiver Messung und subjektiver Einschätzung. So korrelierte die objektive Funktionsdiagnostik der Nase signifikant mit der Erhebung der allgemeinen Lebensqualität (Ergebnis des SF-36). Bei älteren Patienten war die Wahrscheinlichkeit einer erfolgreichen Operation geringer. Es konnte gezeigt werden, dass eine Septumperforation eine relevante Erkrankung ist, die mit dem Leidensdruck von Patienten mit schwerwiegenden chronischen Erkrankungen (Diabetes mellitus, atrophische Rhinitis) gleichzusetzen ist. Nach einem erfolgreichen Verschluss einer Septumperforation konnte eine physische und psychische Verfassung ähnlich der eines Gesunden hergestellt werden. Die postoperative Zufriedenheit war bei 77% der Patienten so groß, dass diese sich einer erneuten Operation unterziehen würden. Zusammenfassend ist es innerhalb der vorliegenden Arbeit gelungen, neue Erkenntnisse über die psychische und physische Verfassung von Septumperforationspatienten vor und nach einer Verschlussoperation zu ermitteln. Diese sollen empfehlend in der klinischen Praxis Anwendung finden. Wünschenswert wäre es, in zukünftigen multizentrischen Studien verschiedene Operationstechniken eines größeren Patientenkollektivs zu vergleichen. Nach unseren Erkenntnissen stellt der SF-36 hierfür einen validen Test dar. Eine Befragung der Patienten könnte sowohl kurz vor und nach der Operation erfolgen, um vergleichbare Daten zu erhalten. Zur Verbesserung der postoperativen Betreuung wäre ferner ein Recallsystem zu entwickeln.
Die Dissertation befasst sich in vier Studien mit den Auswirkungen von Flucht und Vertreibung infolge des Zweiten Weltkrieges auf die Lebensqualität und die psychische Gesundheit im Alter. Basierend auf der Theorie der Kumulativen Ungleichheit wird postuliert, dass frühe Lebensereignisse über die Kumulation von Risiken, aber auch Ressourcen einen Einfluss auf die Lebensqualität und psychische Gesundheit im Alter haben. Die Kohorte ehemals Vertriebener erfuhr eine Vielzahl von Benachteiligungen und ein erhöhtes Ausmaß an Traumata während der Flucht und Vertreibung, was mit einer Kumulation weiterer Risiken im Lebensverlauf einhergeht. Als gesundheitspsychologische Variablen werden die subjektive globale sowie gesundheitsbezogene Lebensqualität untersucht, welche sowohl aus aktueller als auch retrospektiver Sicht betrachtet werden. Es wird angenommen, dass Vertreibung mit einer Kumulation von Risiken im Lebensverlauf einhergeht und die subjektive globale Lebensqualität sowie die gesundheitsbezogene Lebensqualität beeinflusst. Hinsichtlich der psychischen Gesundheit weisen ehemals Vertriebene eine erhöhte psychische Belastungssymptomatik im Alter auf. Es wird angenommen, dass der Vertreibungsstatus als Prädiktor für das Vorliegen psychischer Belastung im Alter dient. Die Folgen der Vertreibung manifestieren sich in einer reduzierten gesundheitsbezogenen Lebensqualität und einer erhöhten psychischen Belastung im Alter. Laut der Theorie der Kumulativen Ungleichheit hat der Zeitpunkt der Vertreibung infolge des Zweiten Weltkrieges Auswirkungen auf den Lebensverlauf einer Person. Aufgrund der unterschiedlichen kognitiven und emotionalen Entwicklung von Kindern und Jugendlichen wird angenommen, dass die psychische Belastung im Alter abhängig vom Alter zum Zeitpunkt der Vertreibung ist. Es konnte gezeigt werden, dass Vertriebene trotz der reduzierten gesundheitsbezogenen Lebensqualität keine Unterschiede in der subjektiven Lebensqualität mit Vergleichsgruppen aufweisen, was den Annahmen des disability paradox bzw. dem Wohlbefindensparadox entspricht. Die subjektive und die gesundheitsbezogene Lebensqualität basieren auf unterschiedlichen Kriterien, zwischen denen aufgrund von Adaptionsprozessen zunächst kein Zusammenhang besteht. Über das Prinzip der Homöostase wird eine hohe subjektive Lebensqualität aufrechterhalten, auch wenn körperliche Einschränkungen die gesundheitsbezogene Lebensqualität beeinträchtigen. Die Vertriebenen der untersuchten Stichproben verfügen über sehr gute Anpassungsprozesse und eine hohe Lebensqualität. Die Theorie der Kumulativen Ungleichheit postuliert neben kumulierenden Nachteilen auch Wendepunkte, persönliche Entscheidungen und Handlungen sowie mobilisierbare Ressourcen, die den Lebenslauf positiv beeinflussen. Das Fehlen von Unterschieden kann auf eine relativ hohe Lebensqualität im Alter, stetige Anpassungsprozesse, soziale und temporale Vergleiche, positive Verzerrung von Einschätzungen im Alter oder der veränderten Populationszusammensetzung aufgrund erhöhter Mortalität besonders vulnerabler Individuen zurückgeführt werden. Die Mobilisierung von Ressourcen in Form von Anpassungsprozessen oder Lebenseinstellungen können die negativen Auswirkungen von Vertreibung im frühen Lebensalter kompensieren. Im Sinne des erfolgreichen Alterns kann eine hohe Lebensqualität als Maß für die Anpassung an Schwierigkeiten während des Lebensverlaufs sowie die zunehmenden Einschränkungen im Alterungsprozess betrachtet werden. Vertreibung infolge des Zweiten Weltkrieges ist ein Risikofaktor, der im Kontext multipler, über den Lebensverlauf wirkender Einfluss- und Risikofaktoren untersucht werden sollte. Der Lebensverlauf sowie der Alterungsprozess sind komplex, inter- und intraindividuell verschieden und müssen im Kontext historischer Ereignisse gesehen werden. Die zukünftige Forschung zur Förderung und Aufrechterhaltung der Gesundheit über den Lebensverlauf ist herausgefordert, Risiken sowie Ressourcen und Anpassungsprozesse über den Lebensverlauf zu identifizieren, den Beitrag einzelner Prädiktoren zu bestimmen und deren Wechselwirkungen zu untersuchen.
