Neuroblastoma is the most common extracranial, malignant, solid tumor found in children. In more than one-third of cases, the tumor is in an advanced stage, with limited resectability. The treatment options include resection, with or without (neo-/) adjuvant therapy, and conservative therapy, the latter even with curative intent. Contrast-enhanced MRI is used for staging and therapy monitoring. Diffusion-weighted imaging (DWI) is often included. DWI allows for a calculation of the apparent diffusion coefficient (ADC) for quantitative assessment. Histological tumor characteristics can be derived from ADC maps. Monitoring the response to treatment is possible using ADC maps, with an increase in ADC values in cases of a response to therapy. Changes in the ADC value precede volume reduction. The usual criteria for determining the response to therapy can therefore be supplemented by ADC values. While these changes have been observed in neuroblastoma, early changes in the ADC value in response to therapy are less well described. In this study, we evaluated whether there is an early change in the ADC values in neuroblastoma under therapy; if this change depends on the form of therapy; and whether this change may serve as a prognostic marker. We retrospectively evaluated neuroblastoma cases treated in our institution between June 2007 and August 2014. The examinations were grouped as ‘prestaging’; ‘intermediate staging’; ‘final staging’; and ‘follow-up’. A classification of “progress”, “stable disease”, or “regress” was made. For the determination of ADC values, regions of interest were drawn along the borders of all tumor manifestations. To calculate ADC changes (∆ADC), the respective MRI of the prestaging was used as a reference point or, in the case of therapies that took place directly after previous therapies, the associated previous staging. In the follow-up examinations, the previous examination was used as a reference point. The ∆ADC were grouped into ∆ADCregress for regressive disease, ∆ADCstable for stable disease, and ∆ADC for progressive disease. In addition, examinations at 60 to 120 days from the baseline were grouped as er∆ADCregress, er∆ADCstable, and er∆ADCprogress. Any differences were tested for significance using the Mann–Whitney test (level of significance: p < 0.05). In total, 34 patients with 40 evaluable tumor manifestations and 121 diffusion-weighted MRI examinations were finally included. Twenty-seven patients had INSS stage IV neuroblastoma, and seven had INSS stage III neuroblastoma. A positive N-Myc expression was found in 11 tumor diseases, and 17 patients tested negative for N-Myc (with six cases having no information). 26 patients were assigned to the high-risk group according to INRG and eight patients to the intermediate-risk group. There was a significant difference in mean ADC values from the high-risk group compared to those from the intermediate-risk group, according to INRG. The differences between the mean ∆ADC values (absolute and percentage) according to the course of the disease were significant: between ∆ADCregress and ∆ADCstable, between ∆ADCprogress and ∆ADCstable, as well as between ∆ADCregress and ∆ADCprogress. The differences between the mean er∆ADC values (absolute and percentage) according to the course of the disease were significant: between er∆ADCregress and er∆ADCstable, as well as between er∆ADCregress and er∆ADCprogress. Forms of therapy, N-Myc status, and risk groups showed no further significant differences in mean ADC values and ∆ADC/er∆ADC. A clear connection between the ADC changes and the response to therapy could be demonstrated. This held true even within the first 120 days after the start of therapy: an increase in the ADC value corresponds to a probable response to therapy, while a decrease predicts progression. Minimal or no changes were seen in cases of stable disease.
Background
A bidirectional functional link between vestibular and fear-related disorders has been previously suggested.
Objective
To test a potential overlap of vestibular and fear systems with regard to their brain imaging representation maps.
Methods
By use of voxel-based mapping permutation of subject images, we conducted a meta-analysis of earlier functional magnetic resonance imaging (fMRI) studies applying vestibular stimulation and fear conditioning in healthy volunteers.
Results
Common clusters of concordance of vestibular stimulation and fear conditioning were found in the bilateral anterior insula cortex, ventrolateral prefrontal cortex and the right temporal pole, bilaterally in the adjacent ventrolateral prefrontal cortex, cingulate gyrus, secondary somatosensory cortex, superior temporal and intraparietal lobe, supplementary motor area and premotor cortex, as well as subcortical areas, such as the bilateral thalamus, mesencephalic brainstem including the collicular complex, pons, cerebellar vermis and bilateral cerebellar hemispheres. Peak areas of high concordance for activations during vestibular stimulation but deactivations during fear conditioning were centered on the posterior insula and S2.
Conclusions
The structural overlap of both networks allows the following functional interpretations: first, the amygdala, superior colliculi, and antero-medial thalamus might represent a release of preprogramed sensorimotor patterns of approach or avoidance. Second, the activation (vestibular system) and deactivation (fear system) of the bilateral posterior insula is compatible with the view that downregulation of the fear network by acute vestibular disorders or unfamiliar vestibular stimulation makes unpleasant perceived body accelerations less distressing. This also fits the clinical observation that patients with bilateral vestibular loss suffer from less vertigo-related anxiety.
Background and purpose
The insula has important functions in monitoring and integrating physiological responses to a personal experience of multimodal input. The experience of chills in response to auditory stimuli is an important example for a relevant arousing experience coupled with bodily response. A group study about altered chill experiences in patients with insula lesions is lacking.
Methods
Twenty-eight stroke patients with predominantly insula lesions in the chronic stage and 14 age-matched controls were investigated using chill stimuli of both valences (music, harsh sounds). Group differences were analyzed in subjective chill reports, associated bodily responses (skin conductance response), lesion mapping, diffusion-weighted imaging and functional magnetic resonance imaging. Other neuropsychological deficits were excluded by comprehensive testing. Diffusion-weighted imaging was quantified for four insula tracts using fractional anisotropy.
Results
The frequency of chill experiences was comparable between participant groups. However, bodily responses were decreased for the stroke group. Whereas there was no association of lesion location, a positive association was found for the skin conductance response during aversive sounds and the tract connecting anterior inferior insula and left temporal pole in the stroke group. Similarly, functional magnetic resonance imaging activation in areas hypothesized to compensate for damage was increased with bodily response.
Conclusions
A decoupling of felt arousal and bodily response after insula lesion was observed. Impaired bodily response was related to an impaired interaction of the left anterior insula and the temporal pole.
Background
Chronic pain of different aetiologies and localization has been associated with less grey matter volume (GMV) in several cortical and subcortical brain areas. Recent meta-analyses reported low reproducibility of GMV alterations between studies and pain syndromes.
Methods
To investigate GMV in common chronic pain conditions defined by body location (chronic back pain, n = 174; migraine, n = 92; craniomandibular disorder, n = 39) compared to controls (n = 296), we conducted voxel-based morphometry and determined GMV from high-resolution cranial MRIs obtained in an epidemiologic survey. Mediation analyses were performed between the presence of chronic pain and GMV testing the mediators stress and mild depression. The predictability of chronic pain was investigated with binomial logistic regression.
Results
Whole-brain analyses yielded reduced GMV within the left anterior insula and the anterior cingulate cortex, for a ROI approach additionally the left posterior insula and left hippocampus showing less GMV across all patients with chronic pain. The relationship of pain with GMV in the left hippocampus was mediated by self-reported stressors in the last 12 months. Binomial logistic regression revealed a predictive effect for GMV in the left hippocampus and left anterior insula/temporal pole for the presence of chronic pain.
Conclusions
Chronic pain across three different pain conditions was characterized by less GMV in brain regions consistently described for different chronic pain conditions before. Less GMV in the left hippocampus mediated by experienced stress during the last year might be related to altered pain learning mechanisms in chronic pain patients.
Significance
Grey matter reorganization could serve as a diagnostic biomarker for chronic pain. In a large cohort, we here replicated findings of less grey matter volume across three pain conditions in the left anterior and posterior insula, anterior cingulate and left hippocampus. Less hippocampal grey matter was mediated by experienced stress.
Chills are emotional peaks especially in response toward acoustic stimuli. In the present study, we examined facial expressions associated with pleasant and unpleasant chill experiences during music and harsh sounds by measuring electromyographic activity from facial corrugator and zygomatic muscles. A rubber bulb could be pressed by the participants to report chill intensities. During harsh sounds, increased activation of both corrugator and zygomatic muscle was observed. Zygomatic muscle activity was even more pronounced when a chill experience was reported during such sounds. In contrast, pleasant chill experiences during music were associated with slightly increased corrugator activity compared with absent chills. Our data suggest that harsh sounds produce a painful facial expression that is even intensified when a chill experience is reported. Increased corrugator activity during chills toward music might refer to states of being moved. The results are discussed in the light of a proposed role of the chill in regulating social behavior. Our results suggest that recording facial muscle activity can be a valuable method for the examination of pleasant and unpleasant peak emotions induced by acoustic stimuli.
To evaluate the influence of the blood–brain barrier on neuronal gadolinium deposition in a mouse model after multiple intravenous applications of the linear contrast agent gadodiamide. The prospective study held 54 mice divided into three groups: healthy mice (A), mice with iatrogenic induced disturbance of the blood–brain barrier by glioblastoma (B) or cerebral infarction (C). In each group 9 animals received 10 iv-injections of gadodiamide (1.2 mmol/kg) every 48 h followed by plain T1-weighted brain MRI. A final MRI was performed 5 days after the last contrast injection. Remaining mice underwent MRI in the same time intervals without contrast application (control group). Signal intensities of thalamus, pallidum, pons, dentate nucleus, and globus pallidus-to-thalamus and dentate nucleus-to-pons ratios, were determined. Gadodiamide complex and total gadolinium amount were quantified after the last MR examination via LC–MS/MS and ICP-MS. Dentate nucleus-to-pons and globus pallidus-to-thalamus SI ratios showed no significant increase over time within all mice groups receiving gadodiamide, as well as compared to the control groups at last MR examination. Comparing healthy mice with group B and C after repetitive contrast administration, a significant SI increase could only be detected for glioblastoma mice in globus pallidus-to-thalamus ratio (p = 0.033), infarction mice showed no significant SI alteration. Tissue analysis revealed significantly higher gadolinium levels in glioblastoma group compared to healthy (p = 0.013) and infarction mice (p = 0.029). Multiple application of the linear contrast agent gadodiamide leads to cerebral gadolinium deposition without imaging correlate in MRI.
An aneurysmal fibrous histiocytoma is a rare cutaneous soft-tissue tumour which accounts for approximately 0.06% of all dermatopathologies. Metastasis is exceedingly uncommon, to the point that there have only been eight reported cases in the scientific literature. We present the case of a 25-year-old male with a primary aneurysmal fibrous histiocytoma located in the nuchal region which exhibited rapid growth and abrupt ulceration over a short time span and showed signs of locoregional aggressive infiltration. A subsequent histopathological analysis confirmed the presence of diffuse solid and cystic pulmonary metastases. Further genetic sequencing verified LAMTOR1-PRKCD fusion. This case report seeks to review the existing literature on aneurysmal fibrous histiocytoma, discuss the challenges of differential diagnosis and propose standardised diagnostic criteria.
Structural integrity of the insula and emotional facial recognition performance following stroke

The role of the human insula in facial emotion recognition is controversially discussed, especially in relation to lesion-location-dependent impairment following stroke. In addition, structural connectivity quantification of important white-matter tracts that link the insula to impairments in facial emotion recognition has not been investigated. In a case–control study, we investigated a group of 29 stroke patients in the chronic stage and 14 healthy age- and gender-matched controls. Lesion location of stroke patients was analysed with voxel-based lesion-symptom mapping. In addition, structural white-matter integrity for tracts between insula regions and their primarily known interconnected brain structures was quantified by tractography-based fractional anisotropy. Our behavioural analyses showed that stroke patients were impaired in the recognition of fearful, angry and happy but not disgusted expressions. Voxel-based lesion mapping revealed that especially lesions centred around the left anterior insula were associated with impaired recognition of emotional facial expressions. The structural integrity of insular white-matter connectivity was decreased for the left hemisphere and impaired recognition accuracy for angry and fearful expressions was associated with specific left-sided insular tracts. Taken together, these findings suggest that a multimodal investigation of structural alterations has the potential to deepen our understanding of emotion recognition impairments after stroke.
Thermal ablation offers a minimally invasive alternative in the treatment of hepatic tumours. Several types of ablation are utilised with different methods and indications. However, to this day, ablation size remains limited due to the formation of a central non-conductive boundary layer. In thermal ablation, this boundary layer is formed by carbonisation. Our goal was to prevent or delay carbonisation, and subsequently increase ablation size. We used bovine liver to compare ablation diameter and volume, created by a stand-alone laser applicator, with those created when utilising a spacer between laser applicator and hepatic tissue. Two spacer variants were developed: one with a closed circulation of cooling fluid and one with an open circulation into hepatic tissue. We found that the presence of a spacer significantly increased ablation volume up to 75.3 cm3, an increase of a factor of 3.19 (closed spacer) and 3.02 (open spacer) when compared to the stand-alone applicator. Statistical significance between spacer variants was also present, with the closed spacer producing a significantly larger ablation volume (p < 0.001, MDiff = 3.053, 95% CI[1.612, 4.493]) and diameter (p < 0.001, MDiff = 4.467, 95% CI[2.648, 6.285]) than the open spacer. We conclude that the presence of a spacer has the potential to increase ablation size.
Introduction: Germany has established a national mammography screening program (MSP). Despite extensive awareness campaigns, the participation rate is only 54%, which is considerably below the European guidelines’ recommendation of at least 70%. Several reasons why women do not participate are already known. Telephone consultations along with invitation letters have improved the participation rate. Here, we analyzed the reasons for non-participation and offered barrier-specific counseling to examine which impediments can be overcome to improve participation. Study Design: In a randomized controlled trial, women who had not attended their proposed screening appointment in the MSP after a written invitation were contacted by telephone and asked why they did not attend. Barrier-specific counseling via telephone was then offered. Participation in the MSP was rechecked 3 months after counseling. Setting: 1772 women, aged 50–69 years, who had not scheduled a mammography screening after a written invitation were contacted by telephone and asked for their reasons for non-participation. Intervention: The reasons were recorded by the calling consultant and categorized either during the call or later based on their recorded statements. Afterward, the women received counseling specific to their statements and were given general information about the MSP. Main outcome measures: We categorized the reasons given, calculated their frequency, and analyzed the probabilities to which they could be successfully addressed in individual counseling. Participation rates were determined post-consultation according to the reason(s) indicated. Results: The data were analyzed in 2022. After exclusions, 1494 records were analyzed. Allowing for multiple reasons to be stated by every individual 3280 reasons for not attending were abstracted. The most frequent reason was participation in “gray screening” (51.5%), which included various breast cancer prevention measures outside the national MSP. Time problems (26.6%) and health reasons (17.3%) were also important. Counseling was most effective when women had not participated for scheduling reasons. Conclusion: Several reasons prevented women from participating in the MSP. Some reasons, such as time-related issues, could be overcome by telephone counseling, but others, like barriers resulting from fear of the examination procedure or its result, could not.
Sensorimotor representations of swallowing in pre- and postcentral gyri of both cerebral hemispheres are interconnected by callosal tracts. We were interested in (1) the callosal location of fibers interconnecting the precentral gyri (with the primary motor cortex; M1) and the postcentral gyri (with the primary somatosensory cortex; S1) relevant for swallowing, and (2) the importance of their integrity given the challenges of swallowing compliance after recovery of dysphagia following stroke. We investigated 17 patients who had almost recovered from dysphagia in the chronic stage following stroke and age-matched and gender-matched healthy controls. We assessed their swallowing compliance, investigating swallowing of a predefined bolus in one swallowing movement in response to a ‘go’ signal when in a lying position. A somatotopic representation of swallowing was mapped for the pre- and postcentral gyrus, and callosal tract location between these regions was compared to results for healthy participants. We applied multi-directional diffusion-weighted imaging of the brain in patients and matched controls to calculate fractional anisotropy (FA) as a tract integrity marker for M1/S1 callosal fibers. Firstly, interconnecting callosal tract maps were well spatially separated for M1 and S1, but were overlapped for somatotopic differentiation within M1 and S1 in healthy participants’ data (HCP: head/face representation; in house dataset: fMRI-swallowing representation in healthy volunteers). Secondly, the FA for both callosal tracts, connecting M1 and S1 swallowing representations, were decreased for patients when compared to healthy volunteers. Thirdly, integrity of callosal fibers interconnecting S1 swallowing representation sites was associated with effective swallowing compliance. We conclude that somatosensory interaction between hemispheres is important for effective swallowing in the case of a demanding task undertaken by stroke survivors with good swallowing outcome from dysphagia.
Das Neuroblastom, ist der häufigste extrakranielle, maligne, solide, kindliche Tumor und in etwa 37 % der Fälle ist das Tumorleiden bei der Erstdiagnose bereits in einem fortgeschrittenen Stadium mit eingeschränkter Resektabilität oder metastatischer Ausbreitung. Das Therapieregime umfasst je nach Stadium: Tumorresektion, zum Teil auch nach neoadjuvanter Therapie; insbesondere bei unvollständiger Resektion folgt dann meist eine weitere adjuvante Therapie; bei fehlender Resektabilität können konservative Therapieformen mit kurativem Ansatz zum Einsatz kommen.
Für Staging und Therapiemonitoring werden Kontrastmittel-MRT genutzt. Oft werden dabei auch diffusionsgewichtete Aufnahmen (DWI) durchgeführt, welche die Bestimmung mittlerer ADC-Werte (apparant diffusion coefficient) zur quantitativen Beurteilung von Diffusion in einem Tumors ermöglichen. Hieraus lassen sich unter anderem Rückschlüsse auf histologische Tumoreigenschaften ziehen. Derartige Erkenntnisse finden auch zunehmend Beachtung im Monitoring von Therapieansprechen maligner Läsionen: U.a. wurde ein Anstieg des ADC-Werts bei Ansprechen von Karzinomen auf Chemotherapie beschrieben. Die Veränderungen des ADC-Werts waren dabei zum Teil bereits messbar, bevor eine Größenreduktion des Tumors beobachtet werden konnte. Übliche Kriterien zur Bestimmung von Therapieansprechen können also durch ADC-Werte ergänzt werden, um so möglicherweise eine frühzeitigere Aussage bezüglich des Therapieansprechens zu treffen. Bisherige Erkenntnisse zur DWI bei Neuroblastomen zeigen ein vergleichbares Bild. Allerdings konnte keine Aussage über eine frühzeitige Änderung des ADC-Werts bei Therapieansprechen getroffen werden.
In der vorliegenden Arbeit sollten drei Fragestellungen beantwortet werden:
Kommt es bei Neuroblastomen unter Therapie zu einer (frühzeitigen) Veränderung des ADC-Werts als Ausdruck therapie-assoziierter Veränderungen? Falls ja, ist sie abhängig von der Therapieform und können die ADC-Veränderungen als prognostische Marker dienen?
Es wurden alle hausinternen und externen MRT-Untersuchungen von Patienten mit Neuroblastomerkrankung der Klinik für Kinder- und Jugendmedizin der Universität Greifswald aus dem Zeitraum von 06.2007 bis 08.2014 ausgewertet. Die MRT-Untersuchungen wurden eingeteilt in: Prestaging = Untersuchung vor Start der jeweiligen Therapieform; Zwischenstaging = Untersuchung während laufender Therapie; Abschlussstaging = Untersuchung nach Abschluss der jeweiligen Therapieform; Follow-Up = Folgeuntersuchung nach Abschlussstaging ohne Therapiebezug. Zur Bewertung des Krankheitsverlaufs wurde eine Einteilung in „Progress“ und „stable disease“ bzw. „Regress“ anhand der jeweiligen Abschlussstagings vorgenommen. Zur Bestimmung der jeweiligen ADC-Werte wurde eine Region of Interest (ROI) entlang der Grenzen aller Tumormanifestation eingezeichnet.
Zur Berechnung von ADC-Veränderungen (∆ADC) wurde, wenn vorhanden, als baseline und Bezugspunkt dabei immer das jeweilige MRT des Prestaging bzw. bei Therapien, welche direkt im Anschluss an vorherige Therapien erfolgten, das zugehörige, vorhergegangene Staging genutzt (n = 45). Bei follow-up Untersuchungen wurde als Bezugspunkt die jeweils vorhergegangene Untersuchung herangezogen (n=25). Die ∆ADC teilen sich auf in ∆ADC-Regress bei Regress (n = 5); ∆ADC-Stable bei stable disease (n = 50) und ∆ADC bei Progress (n = 15). Außerdem wurden solche zusätzlich ausgewählt, deren Errechnung eine Untersuchung unter Therapie nutzte und sie zwischen 60 und 120 d von der jeweiligen baseline entfernt stattfand: er∆ADC-Regress (n = 4), er∆ADC-Stable (n = 11) und er∆ADC-Progress (n = 7).
Unterschiede wurden mittels Mann-Whitney-Test auf Signifikanz getestet (Signifikanzniveau p < 0.05).
Das Neuroblastomregister der Klinik für Kinder und Jugendmedizin der Universitätsmedizin Greifswald umfasste zum Studienzeitpunkt 113 Patienten von denen 79 Patienten anhand Ausschlusskriterien ausschieden, konnten 34 Patienten mit insgesamt 40 auswertbaren Tumormanifestationen und 121 diffusionsgewichteten MRT-Untersuchungen eingeschlossen wurden. Bei 27 Patienten lag eine Neuroblastomerkrankung im Stadium IV nach INSS vor und in 7 Fällen eine Neuroblastomerkrankung im Stadium III nach INSS. Bei 11 Tumorerkrankungen lag eine positive N-MYC-Expression vor, 17 Patienten wurden auf N-MYC negativ getestet (6 Fälle keine Angaben). 26 Patienten wurden der high-risk-Gruppe nach INRG zugeordnet und 8 Patienten der intermediate-risk-Gruppe.
Es zeigte sich ein signifikanter Unterschied der ADC-Mittelwerte aus der high-risk gegenüber der intermediate-risk Gruppe nach INRG.
Bei Betrachtung des Krankheitsverlaufs kam es insgesamt bei Regress zu einem deutlichen Anstieg und beim Progress zu einem deutlichen Abfall des ADC-Werts. Die Unterschiede zwischen den mittleren ∆ADC-Werten (absolut und prozentual) nach Krankheitsverlauf waren dabei signifikant: zwischen ∆ADC-Regress und ∆ADC-Stable; zwischen ∆ADC-Progress und ∆ADC-Stable sowie zwischen ∆ADC-Regress und ∆ADC-Progress. Auch waren die Unterschiede zwischen den mittleren er∆ADC-Werten (absolut und prozentual) nach Krankheitsverlauf teils signifikant: zwischen er∆ADC-Regress und er∆ADC-Stable sowie zwischen er∆ADC-Regress und er∆ADC-Progress. In Abhängigkeit von der Therapieform, N-MYC-Status und Risiko-Gruppen konnten keine zusätzlichen signifikanten Unterschiede der ∆ADC gesehen werden.
Die hier vorliegenden Ergebnisse konnten einen klaren Zusammenhang der ADC-Veränderungen mit dem Therapieansprechen, auch bereits innerhalb der ersten 120 Tagen nach Therapiebeginn, demonstrieren. Derartige Zusammenhänge sind auf einer Linie mit den bisherigen Veröffentlichungen zu diesem Thema und passen zur theoretischen Überlegung welche sich aus Histopathologie und physikalischer Grundlage ergibt. So korrespondiert ein Anstieg des ADC-Werts mit einer hören Wahrscheinlichkeit eines Therapieansprechens bzw. ein Abfall mit einer größeren Wahrscheinlichkeit eines Progresses. Geringe Veränderungen / ein Gleichbleiben des Wertes zeigten sich dagegen besonders oft bei stabilen Krankheitsverläufen. Diese Korrelationen sind dabei auch bereits schon nach maximal 120 Tagen nach Therapiebeginn signifikant. Klassische bildmophologische Kriterien sind in diesem Zeitraum oft nicht ausreichend um eine sichere Aussage bezüglich Therapieansprechen zu tätigen. Hier kann der ADC bzw. seine Veränderung Auskunft geben und so frühzeitig eine Prognose bezüglich des Therapieerfolges liefern.
Dynamische MR- Mammografie – Prävalenz und Follow- Up von Brustläsionen in der Normalbevölkerung

Brustkrebs ist die häufigste Tumorerkrankung bei Frauen in Deutschland. Hohe Inzidenzraten und die Erkrankung junger Frauen führen zu wichtigen medizinischen und sozioökonomischen Auswirkungen dieser Krankheit. Ergebnisse prospektiver randomisierter Studien zeigen, dass eine frühzeitige Diagnose zu einer reduzierten Sterblichkeit durch Brustkrebs führt. Daher wurde auch in Deutschland ein Mammografie- Screening- Programm (MSP) eingeführt, das Frauen im Alter von 50 bis 69 Jahren zur Teilnahme an einer Mammografie alle zwei Jahre berechtigt. Zahlreiche Studien belegen eine reduzierte Sensitivität der Mammografie bei Frauen mit dichtem Brustgewebe, wodurch Intervallkarzinome mit einer Häufigkeit von 30- 50 % im MSP auftreten. Die begrenzte Sensitivität der Mammographie und die damit verbundene relativ hohe Intervallkarzinomrate haben das Interesse an alternativen Screening- Methoden wie die Tomosynthese und Sonografie geweckt. Die MR- Mammografie (MRM) wurde bisher zum Screening von Frauen mit hohem Brustkrebsrisiko eingesetzt. In diesem Setting weist sie im Vergleich zur Mammographie und Sonografie eine durchweg überlegene Sensitivität auf. Inzwischen gibt es erste Hinweise darauf, dass sich ein MRM- Screening bei Frauen mit familiärem Brustkrebs auch in einem Überlebensvorteil niederschlägt. Zu Beginn dieser Untersuchungen existierte keine Studie, die den Nutzen der MRM zum Screening bei Frauen mit durchschnittlichem Brustkrebsrisiko untersucht hat.
Ziel der vorliegenden Studie war es daher, den Einsatz der MRM für die Detektion des Mammakarzinoms innerhalb einer Normalbevölkerung zu evaluieren. Die Study of Health in Pomerania (SHIP)-Studie ist eine populationsbasierte Bevölkerungsstudie und repräsentativ für eine deutsche Normalbevölkerung. Im Rahmen der SHIP-Studie durchgeführte, kontrastmittelgestützte, dynamische MRM wurden unter Verwendung des MR-BI-RADS Atlas des American College of Radiology standardisiert ausgewertet. Ziel war es, zu klären mit welcher Prävalenz unterschiedliche Brustläsionen in der MRM in der Normalbevölkerung auftreten, wie hoch die Raten an abklärungsbedürftigen Befunden (Recall- Rate), die Biopsierate und die Karzinomrate sind und wie sich diese Parameter im Vergleich zu aktuellen Daten aus dem MRM- Hochrisikoscreening und dem deutschen MSP verhalten.
In der vorliegenden populationsbasierten Beobachtungsstudie haben sich im Zeitraum von Juni 2008 bis Oktober 2012 774 Frauen einer kontrastmittelgestützten MRM unterzogen. Das mittlere Alter betrug 51 ± 12,9 Jahre (Umfang 20 - 83 Jahre). 320 (41,3 %) Frauen waren prämenopausal und 454 (58,7 %) postmenopausal.
Die standardisierte Auswertung der Bilddaten anhand des modifizierten MR-BI-RADS Lexikons führte zu einer exzellenten Inter- Reader- Übereinstimmung für die abschließende BI-RADS- Klassifikation (gewichteter kappa 0,828 bzw. 0,857).
113/ 774 Frauen (Detektionsrate 14,6 %) hatten einen mitteilungsbedürftigen Befund der Kategorie BI-RADS IV (N=106, 13,7 %) oder BI-RADS V (N=7, 0,9 %). Von den insgesamt 128 suspekten Läsionen waren 84 (65,6 %) Herdbefunde, 29 NML (22,7 %) und 15 Foci (11,8 %). Die Mehrzahl der kontrastmittelanreichernden Läsionen (825/ 953, 86,6 %) wurde dagegen als gutartig (BI-RADS II) beschrieben. Da eine Direktzuweisung von Probandinnen mit abklärungsbedürftigen Befunden an die Kliniken und Institute der Universitätsmedizin Greifswald zur weiteren Abklärung seitens der Ethikkommission untersagt wurde, erfolgte diese über die Haus- und Fachärzte. Die Ergebnisse der Abklärungsdiagnostik wurden mittels Fragebogen bei den Haus- und Fachärzten und bei den Probandinnen selbst angefragt. Die gesamte Nachverfolgungsrate betrug 111/113 (98,2 %) Frauen bzw. 124/128 (96,9 %) Läsionen. Bei 21/111 (18,9 %) Frauen bzw. 23/124 (18,5 %) Läsionen erfolgte eine histologische Sicherung (Biopsierate 21/ 774, 2,7 %). Ein Malignom wurde bei 7/774 Frauen (Karzinomrate 0,9 %) histologisch gesichert: sechs invasive Mammakarzinome (zweimal Stadium T1b, dreimal Stadium T1c, einmal Stadium T2) und ein duktales Carcinoma in situ (DCIS). Bei einer Probandin (0,13 %) wurde eine NML fälschlicherweise als benigne eingestuft (falsch negativ).
Zusammengefasst zeigen die Ergebnisse dieser prospektiven Beobachtungsstudie, dass eine standardisierte Auswertung von MR- Mammografien mittels MR-BI-RADS Lexikon untersucherunabhängig auch in einer Normalbevölkerung gut durchführbar ist. Dabei muss eine hohe Prävalenz verschiedener Brustläsionen, vor allem benigner Läsionen, antizipiert werden. Erwartungsgemäß zeigen unsere Ergebnisse eine höhere Abklärungsrate, Biopsierate und Karzinomrate im Vergleich zum deutschen MSP, was die Annahme einer höheren Sensitivität der kontrastmittelgestützten MRM nicht nur bei Frauen mit erhöhtem Brustkrebsrisiko, sondern auch bei Frauen in einer Normalpopulation mit durchschnittlichem Brustkrebsrisiko bestätigt. Analog zu den Ergebnissen aus dem MRM- Risikoscreening wurden in unserer Studie kleine invasive Tumoren und Vorstufen detektiert. Weitere Studien sind erforderlich, um den Nutzen der MRM zum Screening von Frauen mit durchschnittlichem Brustkrebsrisiko zu evaluieren. Die Mehrzahl der neu diagnostizierten Karzinome in dieser Studie wurden bei Frauen im Alter von 40 bis 49 bzw. 70 Jahren gefunden, welche nach den aktuellen gesetzlichen Regelungen des deutschen MSP nicht anspruchsberechtigt zur Teilnahme an einer Früherkennungs- Mammografie sind. Diese Beobachtung unterstützt die Forderungen nach einer Ausweitung der Altersgrenzen im deutschen MSP für Frauen ab 45 Jahre und Frauen bis 74 Jahre, so wie es von den Europäischen Richtlinien für das Mammografie- Screening bereits empfohlen wird.
In zwei populationsbasierten randomisierten Studien wurde untersucht, ob telefonische Beratung ein geeignetes Mittel zur Steigerung der Teilnahmequote am Mammographie-Screening-Programm ist. Es wurden die Gründe für eine Nichtteilnahme am Mammographiescreening von Nichtteilnehmerinnen quantitativ erfasst und bestimmt, inwiefern sich diese Gründe durch eine barriere-spezifische Beratung beeinflussen lassen.
Es konnte gezeigt werden, dass die telefonische Beratung ein geeignetes Mittel darstellt, die Teilnahmequote am Mammographie-Screening zu verbessern. Es existieren, außer individuellen Gründen, auch häufige spezifizierbare Gründe die von Nichtteilnehmerinnen genannt werden. Unter diesen ist die Teilnahme am sogenannten „grauen Screening“ der häufigste, der Frauen von einer Teilnahme am Programm abhält. Frauen, die angaben, die Einladung nicht erhalten zu haben oder den in der Einladung angegebenen Termin als unpassend ansahen, konnten am meisten von der telefonischen Beratung profitieren. Die Kenntnis dieser Gründe könnte in Zukunft helfen, im Rahmen der Öffentlichkeitsarbeit spezifische Vorbehalte zu adressieren und im Rahmen des Einladungsverfahrens eine bessere Akzeptanz der Screeninguntersuchung erzeugen. Eine weitere Möglichkeit, die Teilnahmequote zu steigern ist die Reduktion des grauen Screenings, wobei eine individuelle telefonische Beratung hierzu nicht geeignet ist.
Introduction
The concept of thermal ablation has proven a minimally invasive alternative or accompaniment to conventional tumour therapy. Patients with hepatic primary tumours or metastases are able to profit from it. Several modalities of thermal ablation exist, including radiofrequency ablation, microwave ablation and laser ablation. They differ in regards to their indications and their physical backgrounds, yet they all share the same aim: the hyperthermic ablation of tumorous target tissue.
At this point in time the maximum ablation diameter attained in a singular session using a singular applicator is about 30 mm. The maximum attainable volume is about 23 cm3. However, the mean and median of hepatic lesions exceed that number with about 50 mm. Most hepatic tumours therefore cannot be easily ablated in toto.
One of the main limitations of thermal ablation is the periprocedural transformation of vital tissue into a boundary layer. This boundary layer prevents efficient energy transmission into peripheral tissue and thus limits the potential of thermal ablation. The boundary layer is usually located centrally around the ablation applicator. In laser ablation the formation of this boundary layer is called carbonisation.
A technically simple, yet potentially effective approach to delay or prevent the formation of this boundary layer is the usage of a spacer. This perfused spacer cools the central zone surrounding the applicator. Therefore central temperatures remain beyond the point of carbonisation.
Methods
The development of two spacer prototypes took place in cooperation with the AG “Experimentelle Radiologie” of the University Clinic Charité. The first fully closed prototype featured an internal circulation of cooling fluid without tissue perfusion. The second open prototype perfused into tissue through an opened tip.
The conduct of this study included ex vivo experiments on bovine livers (n = 15) by means of laser ablation. Ablation diameter and ablation volume were recorded through MR-guided volumetry and manual displacement volumetry. The mean values of diameter and volume that were recorded when the stand-alone applicator system was used were then compared to the mean values recorded when using the closed spacer-supported applicator system and the open spacer-supported applicator system. The difference in values between the three applicator types were then examined for statistical significance using SPSS.
To exclude covariates a preliminary experiment was conducted which aimed to maximise power input of the laser and time interval while minimising the chance of carbonisation. For that, one of the variables was increased in intervals and the ablation diameter of all three applicator types was measured until carbonisation occurred.
Results
In the preliminary experiment it was found that following the increase of the pre-set power input of the laser a proportional increase of ablation diameter followed. However when increasing power input above 25 Watt almost instantaneous carbonisation of the central tissue occurred. This was the same for all three applicator types.
When increasing the time interval > 10 minutes the stand-alone applicator system showed central carbonisation, which was not the case when using the closed spacer-supported applicator system or the open spacer-supported applicator system. The two spacer prototypes only experienced carbonisation when a time interval of > 25 minutes was set. Thus the comparison of all three applicator types was conducted at 25 Watt and 10 minutes, whereas the comparison between the closed spacer-supported applicator system and the open spacer-supported applicator system was conducted at 25 Watt and 25 minutes.
During the first experiment the stand-alone applicator system achieved mean values of 37.50 mm ablation diameter and 23.61 cm3 ablation volume. This was a statistically significant (p < 0.001) increase to the values either spacer was able to attain: the closed spacer-supported applicator system recorded a mean value of 28.67 mm ablation diameter and 18.12 cm3 ablation volume, whereas the open spacer-supported applicator system recorded a mean value of 31.00 mm ablation diameter and 18.49 cm3 ablation volume. However, setting a longer time interval was not possible when the stand-alone applicator system was used for ablation. Due to this, a second experiment comparing mean ablation diameter and volume between the two spacer prototypes followed.
During the second experiment with a time interval of 25 minutes the closed spacer-supported applicator system attained a mean value of 52.07 mm ablation diameter and 75.25 cm3 ablation volume. These values showed a statistically significant (p < 0.001) difference in comparison to the open spacer-supported applicator system with mean values of 47.60 mm ablation diameter und 72.20 cm3 ablation volume.
Discussion
Within the framework of this study it was proven that the presence of a spacer between laser applicator and hepatic tissue was able to achieve a significant increase in ablation diameter and ablation volume. By using a closed spacer an increase in volume by a 3.19 factor of change was possible. The open spacer obtained an increase in volume by a 3.06 factor of change. The concept of using a spacer in thermal ablation as a proof of concept study is therefore valid and suitable for further pre-clinical studies.
Klassifikation perirenaler Septenbildung und ihre Korrelation mit potentiellen Einflussfaktoren

Zusammenfassung
Ziel: Ziel dieser Arbeit war es, Prävalenz, Ausprägung und Einflussfaktoren von
perirenalen Septen in der Allgemeinbevölkerung zu untersuchen.
Material und Methoden: Von den per Zufallsstichprobe ausgewählten 4420
Probanden der populationsbasierten SHIP-TREND Kohorte wurden die T2-
gewichteten Sequenzen der MRT-Ganzkörperuntersuchungen nach einem
festgelegten Septenklassifikationsschema beurteilt. Eine Durchmesserbestimmung
des Peri- und posterioren Pararenalraumes wurde anhand von T1-gewichteten
Sequenzen vorgenommen. Nach Prüfung von Ausschlusskriterien umfasste die
endgültige Stichprobe 910 Frauen und 842 Männer (N= 1752) im Alter von 21 bis 79
Jahren. Als zu betrachtende Risikofaktoren wurden aus den Erhebungen der SHIPTREND Studie der Raucherstatus, der BMI, die glomeruläre Filtrationsrate und
Vorerkrankungen wie Diabetes mellitus und Hypertonie herangezogen.
Ergebnis: Die Prävalenz eines perirenalen Septenstadiums von ≥ 1 lag unter allen
Studienteilnehmern bei 40,7%. Ein deutlicher Unterschied zwischen den
Geschlechtern war für beide Nieren sichtbar. Die Männer hatten an der linken Niere
(Septensstudium ≥ 1) eine Prävalenz von 66% im Vergleich zu 17% bei den Frauen.
Im Seitenvergleich zeigte sich mit einer Signifikanz von p < 0,001 eine Häufigkeit von
17% für Septenstadium 3 in der linken Niere und von 13% in der rechten Niere. Bei
den Älteren wurde ebenfalls eine höhere Prävalenz deutlich, mit 77% bei den über
70-Jährigen und 3% bei den 20 bis 29 Jährigen (p < 0,001). Als signifikant
unabhängige Risikofaktoren für perirenale Septen der linken Niere zeigten sich
männliches Geschlecht (OR = 13, 2; 95%CI 9,7; 17,9; p<0,001), ein höheres Alter
(OR = 1,09; 95%CI 1,07; 1,10; p<0,001), ein erhöhter BMI (OR = 1,06; 95% CI
1,03; 1,10), aktives Rauchen (OR = 1,89, 95%CI 1,35; 2,64; p<0,001), ein Diabetes
mellitus (OR= 1,79; 95%CI 1,15; 2,78; p<0,001) und eine erhöhte Dicke des
posterioren Pararenalraums (OR = 1,20; 95%CI 1,17; 1,23; p<0,001). Für die rechte
Niere ergaben sich bis auf den Diabetes mellitus die gleichen Risikofaktoren.
Schlussfolgerung: Insgesamt sind perirenale Septen in der Allgemeinbevölkerung
häufig, mit höherer Prävalenz für die linke Niere. Risikofaktoren für eine stärkere
Ausprägung sind männliches Geschlecht, höheres Alter, hoher BMI, aktives Rauchen
und erhöhte Dicke des posterioren Pararenalraums
Zielsetzung: CT-gesteuerte perkutane transthorakale Lungenbiopsien werden zur Charakterisierung von unklaren pulmonalen Läsionen eingesetzt, da diese weniger Komplikationen hervorrufen als operative Explorationen. Die hierbei am häufigsten auftretenden Komplikationen sind Pneumothorax und pulmonalen Blutungen. Um diese zu verringern, konzentrierten sich Studien vermehrt auf mögliche Einflussfaktoren. Hierbei zeigte sich, dass Komplikationen überwiegend von Läsionsgröße, Stich-kanallänge und pleuralem Kontakt des Tumors abhingen. Weitere einflussnehmen-de Faktoren waren das Auftreten von Lungenemphysemen und die Lokalisation des Tumors. Nach unserem Wissen wurde der potentielle Einfluss von Berufserfahrung und Persönlichkeitsmerkmalen von interventionellen Radiologen bei CT-gesteuerten Lungenbiopsien bislang nicht untersucht.
Folglich war die Motivation zur vorliegenden Studie, mithilfe psychologischer Frage-bögen Rückschlüsse auf die Interventionsergebnisse und Ausbildung im Bereich der Interventionellen Radiologie zu ziehen. Konkret wurde untersucht inwiefern Berufserfahrung und Persönlichkeitsmerkmale von interventionellen Radiologen den technischen Erfolg und die Komplikationsrate CT-gesteuerter perkutaner transthorakaler Lungenbiopsien beeinflussen.
Methodisches Vorgehen: Auf Grundlage einer retrospektiven Datenanalyse von 1056 CT-gesteuerten Lungenbiopsien, durchgeführt von 38 interventionellen Radio-logen im Zeitraum 2006 bis 2014, wurden für die vorliegende Studie 445 Eingriffe herangezogen, die von 14 Radiologen durchgeführt wurden (Ausschlusskriterien: Fehlende Zustimmung der Radiologen; < 20 Interventionen pro Interventionalist; Punktionen mit unvollständigen Daten etc., Vgl. Abbildung 1).
Die Interventionen wurden bewertet in Bezug auf die Häufigkeit von auftretenden Komplikationen, technischer Erfolgsrate und interventionsbezogenen Einflussgrößen. Persönlichkeitsmerkmale der interventionellen Radiologen wurden auf Basis der psychologischen Fragebögen NEO-FFI (Big Five) und State-Trait Angst-Inventar beurteilt. Die Ergebnisse der CT-gesteuerten Lungenbiopsien wurden den interventionsbezogenen Einflussgrößen zugeordnet und mit den Persönlichkeitsmerkmalen und der Berufserfahrung der 14 Radiologen abgeglichen.
Resultat: Es wurden keine signifikanten Unterschiede zwischen den 14 Interventionalisten bezüglich des technischen Erfolgs (Variationsbreite: 75.0 - 95.5%, p = 0.406), Komplikationsrate insgesamt (Variationsbreite: 44.7 - 85.7%, p = 0.088) beziehungsweise Major- (Variationsbreite: 15.6 - 40.9%, p = 0.679) und Minor-Komplikationen (Variationsbreite: 31.6 - 71.4%, p = 0.074) identifiziert. Die Berufserfahrung hatte keine Aus-wirkung auf den technischen Erfolg (p = 0.254) und die Komplikationsrate (p = 0.470). Die Persönlichkeitsmerkmale hatten keinen Einfluss auf den technischen Erfolg. Drei der sechs Persönlichkeitsmerkmale besaßen jedoch einen Einfluss auf die Komplikationsrate.
Fazit: Die Berufserfahrung besitzt keinen Einfluss auf den technischen Erfolg und die Komplikationsrate von CT-gesteuerten transthorakalen Lungenbiopsien. Auch die Persönlichkeitsmerkmale Angstgefühl, Offenheit, Gewissenhaftigkeit, Extraversion, Verträglichkeit und Neurotizismus nehmen keinen signifikanten Einfluss auf den technischen Erfolg. Jedoch haben die drei Persönlichkeitsmerkmale Angstgefühl, Offenheit und Gewissenhaftigkeit einen signifikanten Einfluss auf das Auftreten von Komplikationen.
Es zeigt sich, dass Persönlichkeitsmerkmale zumindest teilweise einen Einfluss auf die Komplikationsrate besitzen und daher eine verbesserte individuelle Weiterbildung helfen kann, individuelle Fehlerquellen zu minimieren und das Leistungsniveau zu erhöhen. Ein verstärktes Augenmerk sollte analog anderer Berufe auch in der Medizin darauf gelegt werden, bereits bei der Kandidatenauswahl stärker persönlichkeitsmerkmalbezogene Aspekte zu integrieren und während der Berufslaufbahn immer wieder zu erheben.
Temporomandibulärer Schmerz ist ein häufiges Symptom bestehend aus Schmerzen lokalisiert um den gelenknahen Anteil des Kiefers mit einer starken Abhängigkeit von Stressoren. Chronische Schmerzen wurden bereits mit Veränderungen des Volumens der grauen Hirnsubstanz (GMV) assoziiert. Frühere Studien zum Thema haben bis jetzt jedoch widersprüchliche Ergebnisse geliefert. Dies könnte unter anderem den unterschiedlichen Zusammensetzungen der Studienpopulationen sowie den zugrundeliegenden Studienmethoden geschuldet sein.
Um die Auswirkungen chronischer Schmerzen auf die Gehirnstruktur mit hohen Fallzahlen zu untersuchen, wurden in der vorliegenden Arbeit drei Gruppen klinischer Patienten, insgesamt 47 Individuen, und eine Studienpopulation, bestehend aus insgesamt 57 Patienten, die Gesichtsschmerzen über die letzten 6 Monate angaben, analysiert. Die Patienten der Studienpopulation wurden aus der Study of Health in Pomerania (SHIP) rekrutiert, die Patienten der klinischen Probandengruppen in Zusammenarbeit mit der Zahnklinik Greifswald. Das Volumen der grauen Hirnsubstanz dieser Patienten wurde jeweils mit dem einer gesunden Kontrollgruppe ohne chronische Schmerzen verglichen.
Die Kontrollgruppe bestand aus 60 Individuen für den Vergleich mit den klinischen Patienten sowie 381 Individuen für den Vergleich mit den Patienten der Studienpopulation. Beide Kontrollgruppen waren jeweils für Alter und Geschlecht entsprechend korrigiert.
Zur Gehirnstrukturanalyse kam Voxelbasierte Morphometrie als populäres In-vivo-Verfahren zum Einsatz. Hiefür erhielten sowohl Patienten als auch die Kontrollpersonen ein MRT. In einer Region of Interest (Bereich von Interesse) Analyse zeigte nur die klinische Patientengruppe ein verringertes Volumen der grauen Substanz in den Bereichen des anteromedialen Gyrus cinguli bis in den medialen präfrontalen Cortex reichend, bei denen aus der Literatur bereits eine besonde Anfälligkeit gegenüber chronischen Schmerzen und einer damit verbundenen Reduktion der grauen Substanz bekannt ist.
Introduction: Patients who are overweight or obese have an increased risk of developing type 2 diabetes mellitus (T2DM). Weight loss can have a positive effect on glycemic control. Objective: We aimed to investigate glycemic control in patients with T2DM and overweight or obesity during a structured weight-loss program. Methods: This was a prospective, interventional study. We recruited 36 patients (14 men and 22 women) with a median age of 58.5 years and median body mass index (BMI) of 34.1, to a 15-week structured weight-loss program with a low-calorie (800 kcal) formula diet for 6 weeks. The primary end point, HbA<sub>1c</sub> level, and secondary end points, anthropometric data, medication, and safety, were assessed weekly. Laboratory values and quality of life were assessed at baseline and after 15 weeks. Results: HbA<sub>1c</sub> decreased from 7.3% at baseline to 6.5% at 15 weeks (p < 0.001), median body weight by 11.9 kg (p < 0.001), median BMI by 4.3 (p < 0.001) and median waist circumference by 11.0 cm (p < 0.001). Two participants discontinued insulin therapy, 4 could reduce their dosage of oral antidiabetic agents, and 6 completely discontinued their antidiabetic medication. Insulin dose decreased from 0.63 (0.38–0.89) to 0.39 (0.15–0.70) units/kg body weight (p < 0.001). No patient experienced hypoglycemic episodes or hospital emergency visits. Triglycerides and total cholesterol decreased as well as surrogate markers of liver function. However, the levels of high-density and low-density lipoprotein cholesterol (HDL-C and LDL-C) as well as uric acid remain unchanged. Regarding quality of life, the median physical health score increased from 44.5 (39.7–51.4) at baseline to 48.0 (43.1–55.3; p = 0.007), and the median mental health score decreased from 42.1 (36.1–46.7) to 37.4 (30.3–43.7; p = 0.004). Conclusions: A structured weight-loss program is effective in the short term in reducing HbA<sub>1c</sub>, weight, and antidiabetic medication in patients with T2DM who are overweight or obese. Levels of HDL-C and LDL-C were not affected by short-term weight loss. The decline in mental health and the long-term effects of improved glycemic control require further trials.

focusing on the viscerocranium rather than the neurocranium is underrepresented. Methods: Retrospective cohort study (2006–2014) at an urban level 1 trauma center assessing the frequency and
typical patterns of orbital injuries on whole-body computed tomography (WBCT) with maxillofacial
multi-slice CT (MSCT) after severe trauma. (1) Screening of consecutive WBCT cases for dedicated
maxillofacial MSCT. (2) Examination by two independent experts’ radiologists for (peri-/)orbital
injuries. (3) Case review for trauma mechanisms. Results: 1061 WBCT were included revealing 250
(23.6%) patients with orbital injuries. Less than one-quarter (23.3%) of patients showed osseous
and 9.5% showed soft tissue injuries. Combined osseous and soft tissue lesions were present in
39.2% of orbital injuries, isolated soft tissue injuries were rare. Single- or two-wall fractures of the
orbit were prevalent, and the orbital floor was affected in 67% of fractures. Dislocated extraocular
muscles (44.6%), deformation of the ocular globe (23.8%), and elongation of the optic nerve (12.9%)
were the most frequently soft tissue findings. Vascular trauma was suspected in 15.8% of patients.
Conclusions: Orbital trauma was confirmed in 23.6% of cases with suspected facial injuries after severe
trauma. Concomitant soft tissue injuries should be excluded explicitly in cases with orbital fractures
to prevent loss of vision or ocular motility.
