Background: Stroke patients are at risk of acquiring secondary infections due to stroke-induced immune suppression (SIIS). Immunosuppressive cells comprise myeloid-derived suppressor cells (MDSCs) and immunosuppressive interleukin 10 (IL-10)-producing monocytes. MDSCs represent a small but heterogeneous population of monocytic, polymorphonuclear (or granulocytic), and early progenitor cells (“early” MDSC), which can expand extensively in pathophysiological conditions. MDSCs have been shown to exert strong immune-suppressive effects. The role of IL-10-producing immunosuppressive monocytes after stroke has not been investigated, but monocytes are impaired in oxidative burst and downregulate human leukocyte antigen—DR isotype (HLA-DR) on the cell surface.
Objectives: The objective of this work was to investigate the regulation and function of MDSCs as well as the immunosuppressive IL-10-producing monocytes in experimental and human stroke.
Methods: This longitudinal, monocentric, non-interventional prospective explorative study used multicolor flow cytometry to identify MDSC subpopulations and IL-10 expression in monocytes in the peripheral blood of 19 healthy controls and 27 patients on days 1, 3, and 5 post-stroke. Quantification of intracellular STAT3p and Arginase-1 by geometric mean fluorescence intensity was used to assess the functionality of MDSCs. In experimental stroke induced by electrocoagulation in middle-aged mice, monocytic (CD11b+Ly6G−Ly6Chigh) and polymorphonuclear (CD11b+Ly6G+Ly6Clow) MDSCs in the spleen were analyzed by flow cytometry.
Results: Compared to the controls, stroke patients showed a relative increase in monocytic MDSCs (percentage of CD11b+ cells) in whole blood without evidence for an altered function. The other MDSC subgroups did not differ from the control. Also, in experimental stroke, monocytic, and in addition, polymorphonuclear MDSCs were increased. The numbers of IL-10-positive monocytes did not differ between the patients and controls. However, we provide a new insight into monocytic function post-stroke since we can report that a differential regulation of HLA-DR and PD-L1 was found depending on the IL-10 production of monocytes. IL-10-positive monocytes are more activated post-stroke, as indicated by their increased HLA-DR expression.
Conclusions: MDSC and IL-10+ monocytes can induce immunosuppression within days after stroke.
Der aktuelle demografische Wandel in Deutschland zeigt eine erhöhte Lebenserwartung und damit einen Anstieg an altersassoziierten Erkrankungen wie dem Schlaganfall. Eine mögliche Folge ist die Armparese, welche eine gravierende Behinderung bei der Ausführung alltäglicher Handlungen darstellt. Dadurch kommt der motorischen Rehabilitation mit dem Ziel der Wiederherstellung der Alltagstauglichkeit eine besonders wichtige Rolle zu.
Unter zahlreichen Therapiekonzepten ist für das Arm-Fähigkeits-Training (AFT), welches einzeln verschiedene sensomotorische Armfähigkeiten anspricht und das motorische Lernen induziert, eine gute Wirksamkeit belegt.
In Studien konnte gezeigt werden, dass eine nicht-invasive Hirnstimulation in Form einer repetitiven transkraniellen Magnetstimulation (rTMS), genauer der intermittierenden Theta-Burst-Stimulation (iTBS), vorübergehend die lokal kortikale Erregbarkeit des stimulierten Areals erhöhen (Huang et al., 2005) und dadurch gegebenenfalls auch das nachfolgende trainingsinduzierte Lernen beeinflussen kann. Das Wissen über mögliche „Priming“-Effekte von iTBS auf das motorische Lernen bei Gesunden kann helfen, zielgerichtete therapeutische Anwendungen für Patienten nach einem Schlaganfall zu entwickeln.
Ziel dieser Untersuchung war es festzustellen, ob das exzitatorische „Priming“ mit iTBS über dem primären motorischen Kortex (M1) oder dem primären somatosensorischen Kortex (S1) unmittelbar vor einer täglichen Trainingseinheit mit AFT (über vier Tage) für den linken Arm bei gesunden rechtshändigen Probanden die sensomotorische Lerndynamik verbessern kann.
Zu diesem Zweck wurde ein Training des linken, nicht-dominanten Arms von 18 jungen und gesunden Probanden mithilfe von acht unterschiedlichen motorischen Aufgaben (AFT) einmal pro Tag für insgesamt fünf Tage durchgeführt. Mit Ausnahme des ersten Tages (Baseline) erfolgte das Training nach der Applikation einer exzitatorischen Form der repetitiven transkraniellen Magnetstimulation (iTBS). Die Stimulation wurde je nach randomisierter Gruppenzuordnung entweder über M1 oder S1 rechts oder als Sham-Stimulation, um einen möglichen Placebo-Effekt auszuschließen, über M1 rechts durchgeführt.
Die Hauptkomponentenanalyse der Daten zum motorischen Verhalten ergab acht
unabhängige motorische Komponenten, die den acht trainierten Aufgaben entsprachen. AFT induzierte motorisches Lernen über alle Fähigkeiten hinweg mit einem Generalisationseffekt auf eine nicht-trainierte Aufgabe der Fingergeschicklichkeitm(Nine-Hole-Peg-Test,ccNHPT).
Probanden, die iTBS (entweder über M1 oder S1) erhielten, zeigten im Vergleich zur Sham-Stimulation sowohl eine bessere Leistung bei den AFT-Aufgaben während der Trainingsdauer als auch eine größere Verbesserung der nicht-trainierten Fingergeschicklichkeitsaufgabe (NHPT) für den trainierten linken Arm nach Trainingsende.
Daraus resultiert, dass die exzitatorische repetitive transkranielle Magnetstimulation in Form von iTBS über M1 oder S1 das motorische Lernen über verschiedene sensomotorische Fähigkeiten hinweg verbessern kann.
Auch wenn die verstärkenden Effekte eines exzitatorischen „Priming“ absolut gesehen klein waren, so geben sie dennoch Grund zur Annahme, dass darin auch ein therapeutisches Potenzial für die Armrehabilitation nach Schlaganfall liegt. Ob das so ist, wäre jedoch mit geeigneten klinischen Studien zu untersuchen.
Die Ergebnisse des Promotionsvorhabens wurden in einer Peer-Review-Zeitschrift publiziert (Platz et al., 2018a).
In der akuten Schlaganfallbehandlung nehmen die endovaskulären Revaskularisationsverfahren, insbesondere die intraarterielle Thrombolyse, einen wachsenden Stellenwert ein. Grundlage dafür ist die zentrumsspezifische Sicherheit und Durchführbarkeit, wobei nach den Zertifizierungskriterien der Deutschen Schlaganfallgesellschaft die intraarterielle Thrombolyse nur in zertifizierten überregionalen Stroke Units erfolgen sollte. Weiterhin existieren nur wenige Selektionsparameter zur Abschätzung des Risikos der intrakraniellen Blutung als wichtigste Komplikation gegenüber dem potenziellen Benefit der Behandlung. Ziel dieser Arbeit war 1. die Etablierung eines klinisch praktikablen Protokolls zur Entscheidung für eine intravenöse oder intraarterielle Thrombolyse sowie die systematische Untersuchung der zentrumsspezifischen Sicherheit und Durchführkeit der intraarteriellen Thrombolyse; 2. die Untersuchung des Einflusses der bekannten klinischen und radiologischen Faktoren, wie Zeit bis zur Thrombolyse oder Rekanalisation im vorliegenden Patientenkollektiv auf das neurologische Outcome; und 3. die Untersuchung der Perfusions-CT und der CT-Angiographie als Grundlage für weitere mögliche Selektionsparameter zur intraarteriellen Thrombolyse. Zwischen 02/2006 und 12/2008 konnten 26 Patienten eingeschlossen werden. Ein Patient verstarb innerhalb von 90 Tagen nach Schlaganfall aufgrund eines erneuten cerebrovaskulären Ereignisses. Die Rate der symptomatischen und asymptomatischen intrakraniellen Blutungen (8% bzw 8%) sowie die Rate der periprozeduralen Komplikationen sind trotz der eingeschränkten Vergleichbarkeit mit der Literatur als gleichwertig zu bewerten. Die Effektivitätsparameter Zeit bis Thombolyse (Durchschnitt +/- Standardabweichung: 4,03 ± 1,2), Rekanalisationsrate (73%) sowie das Ausmaß des guten funktionellen Outcome 90 Tage nach Schlaganfall (mRS <=2; 58%) sind ähnlich oder teilweise sogar besser als die in der Literatur veröffentlichen Ergebnisse für die intraarterielle und intravenöse Thrombolyse. Somit ist die intraarterielle Thrombolyse in dieser Studie als sicher und effektiv zu bewerten. Die Zeit bis zur Thrombolyse zeigte sich nicht signifikant mit dem neurologischen Outcome assoziiert, jedoch ergab sich ein Trend zu schlechterem Outcome mit größerer zeitlicher Latenz zwischen Symptom- und Therapiebeginn. Weiterhin konnte die Rekanalisaton als Prädiktor für das neurologische Outcome und die Infarktgröße bestätigt werden. Der prädiktive Wert des Ausmaßes der Kollateralen in der CT-Angiographie mittels eines von Tan et al. entwickelten Score für das finale Infarktvolumen konnte in diesem homogenen, hochselektionieren Patientengut bestätigt werden. Das Ausmaß der Kollateralen als ein neuer Selektionsparameter für die Thrombolyse sollte weiter detailiert untersucht werden. Weiterhin konnte nachgewiesen werden, dass mittels Perfusions-CT auf das kurzfristige Outcome mittels NIHSS zwischen Aufnahme und Entlassung geschlossen werden kann. Hierbei zeigte sich, dass der im Rahmen dieser Studie entwickelte Auswertealgorithmus der relativen Perfusions-CT-Parameter und der schon verwendete Algorithmus der ASPECTS-Perfusion, bezüglich der Vorhersagekraft des cerebralen Blutvolumens (CBV) gleichwertig sind, jedoch die relativen Perfusions-Parameter eine wesentliche bessere Interrater-Reliabiltät aufweisen. Bezüglich des cerebralen Blutflusses (CBF) ergab sich nach dem ASPECTS-Perfusions-Algorithmus keine Assoziation mit dem neurologischen Outcome. Bei den relativen Perfusionsparametern erreichte jedoch das CBF eine dem CBV vergleichbare Vorhersagefähigkeit gepaart mit einer besseren Interrater-Reliabilität als das CBV. Das aktuell weit verbreitete visuelle Abschätzen der Größe des tissue at risk zeigte weder eine Assoziation zum neurologischen Outcome nach intraarterieller Thrombolyse noch eine gute Interratervariabilität.
Objective: The study aimed to test the reliability of a semi-structured telephone interview for the classification of headache disorders according to the ICHD-3.
Background: Questionnaire-based screening tools are often optimized for single primary headache diagnoses [e.g., migraine (MIG) and tension headache (TTH)] and therefore insufficiently represent the diagnostic precision of the ICHD-3, which limits epidemiological research of rare headache disorders. Brief semi-structured telephone interviews could be an effective alternative to improve classification.
Methods: A patient population representative of different primary and secondary headache disorders (n = 60) was recruited from the outpatient clinic (HSA) of a tertiary care headache center. These patients completed an established population-based questionnaire for the classification of MIG, TTH, or trigeminal autonomic cephalalgia (TAC). In addition, they received a semi-structured telephone interview call from three blinded headache specialists individually. The agreement of diagnoses made either using the questionnaires or interviews with the HSA diagnoses was evaluated.
Results: Of the 59 patients (n = 1 dropout), 24% had a second-order and 5% had a third-order headache disorder. The main diagnoses were as follows: frequent primary headaches with 61% MIG, 10% TAC, 9% TTH, and 5% rare primary and 16% secondary headaches. Second-order diagnosis was chronic migraine throughout, and third-order diagnoses were medication overuse headache and TTH. Agreement between main headaches from the HSA was significantly better for the telephone interview than for the questionnaire (questionnaire: κ = 0.330; interview: κ = 0.822; p < 0.001). Second-order diagnoses were not adequately captured by questionnaires, while there was a trend for good agreement with the telephone interview (κ = 0.433; p = 0.074). Headache frequency and psychiatric comorbidities were independent predictors of HSA and telephone interview agreement. Male sex, headache frequency, severity, and depressive disorders were independently predictive for agreement between the questionnaire and HSA. The telephone interview showed high sensitivity (≥71%) and specificity (≥92%) for all primary headache disorders, whereas the questionnaire was below 50% in either sensitivity or specificity.
Conclusion: The semi-structured telephone interview appears to be a more reliable tool for accurate diagnosis of headache disorders than self-report questionnaires. This offers the potential to improve epidemiological headache research and care even in underserved areas.
The combination of repeated behavioral training with transcranial direct current stimulation (tDCS) holds promise to exert beneficial effects on brain function beyond the trained task. However, little is known about the underlying mechanisms. We performed a monocenter, single-blind randomized, placebo-controlled trial comparing cognitive training to concurrent anodal tDCS (target intervention) with cognitive training to concurrent sham tDCS (control intervention), registered at ClinicalTrial.gov (Identifier NCT03838211). The primary outcome (performance in trained task) and secondary behavioral outcomes (performance on transfer tasks) were reported elsewhere. Here, underlying mechanisms were addressed by pre-specified analyses of multimodal magnetic resonance imaging before and after a three-week executive function training with prefrontal anodal tDCS in 48 older adults. Results demonstrate that training combined with active tDCS modulated prefrontal white matter microstructure which predicted individual transfer task performance gain. Training-plus-tDCS also resulted in microstructural grey matter alterations at the stimulation site, and increased prefrontal functional connectivity. We provide insight into the mechanisms underlying neuromodulatory interventions, suggesting tDCS-induced changes in fiber organization and myelin formation, glia-related and synaptic processes in the target region, and synchronization within targeted functional networks. These findings advance the mechanistic understanding of neural tDCS effects, thereby contributing to more targeted neural network modulation in future experimental and translation tDCS applications.
Introduction
Given rapid global population aging, developing interventions against age-associated cognitive decline is an important medical and societal goal. We evaluated a cognitive training protocol combined with transcranial direct current stimulation (tDCS) on trained and non-trained functions in non-demented older adults.
Methods
Fifty-six older adults (65–80 years) were randomly assigned to one of two interventional groups, using age and baseline performance as strata. Both groups performed a nine-session cognitive training over 3 weeks with either concurrent anodal tDCS (atDCS, 1 mA, 20 minutes) over the left dorsolateral prefrontal cortex (target intervention) or sham stimulation (control intervention). Primary outcome was performance on the trained letter updating task immediately after training. Secondary outcomes included performance on other executive and memory (near and far transfer) tasks. All tasks were administered at baseline, post-intervention, and at 1- and 7-month follow-up assessments. Prespecified analyses to investigate treatment effects were conducted using mixed-model analyses.
Results
No between-group differences emerged in the trained letter updating and Markov decision-making tasks at post-intervention and at follow-up timepoints. Secondary analyses revealed group differences in one near-transfer task: Superior n-back task performance was observed in the tDCS group at post-intervention and at follow-up. No such effects were observed for the other transfer tasks. Improvements in working memory were associated with individually induced electric field strengths.
Discussion
Cognitive training with atDCS did not lead to superior improvement in trained task performance compared to cognitive training with sham stimulation. Thus, our results do not support the immediate benefit of tDCS-assisted multi-session cognitive training on the trained function. As the intervention enhanced performance in a near-transfer working memory task, we provide exploratory evidence for effects on non-trained working memory functions in non-demented older adults that persist over a period of 1 month.
Most children use their fingers when learning to count and calculate. These sensorimotor experiences were argued to underlie reported behavioral associations of finger gnosis and counting with mathematical skills. On the neural level, associations were assumed to originate from overlapping neural representations of fingers and numbers. This study explored whether finger-based training in children would lead to specific neural activation in the sensorimotor cortex, associated with finger movements, as well as the parietal cortex, associated with number processing, during mental arithmetic. Following finger-based training during the first year of school, trained children showed finger-related arithmetic effects accompanied by activation in the sensorimotor cortex potentially associated with implicit finger movements. This indicates embodied finger-based numerical representations after training. Results for differences in neural activation between trained children and a control group in the IPS were less conclusive. This study provides the first evidence for training-induced sensorimotor plasticity in brain development potentially driven by the explicit use of fingers for initial arithmetic, supporting an embodied perspective on the representation of numbers.
202 Patienten (113 Frauen und 89 Männer im Alter von 25 bis 95 Jahren) mit einem ischämischen, supratentoriellem und territorialen Schlaganfall vorwiegend im Mediastromgebiet, wurden 9/2007 bis 6/2012 aus dem Patientengut der Stroke Unit der Neurologischen Klinik der Universitätsmedizin Greifswald für die monozentrische Studie zur Häufigkeit epileptischer Anfälle sowie ihrer Provokationsfaktoren rekrutiert.
Bei einem durchschnittlichen Beobachtungszeitraum von 36,5 Monaten erlitten 39 Patienten (19,3%) mindestens einen epileptischen Anfall. Davon hatten 12 Patienten einen akut symptomatischen Anfall und 27 einen unprovozierten Anfall. Bei 21 Patienten traten rezidivierende Anfälle auf. Wendet man die aktualisierte Definition der ILAE für Epilepsie an, so hatten 31 (15,3%) Patienten eine Epilepsie und 8 (4%) einen einzelnen akutsymptomatischen Anfall. Damit hatte unsere Studie die bisher höchste dokumentierte Rate an Epilepsie nach einer Ischämie. Der erste epileptische Anfall trat dabei überwiegend im 1. Jahr nach erlittenem Schlaganfall auf. Es zeigte sich, dass Patienten mit einer geringeren Schwere des Schlagfalls (erfasst mittels NIHSSS und mRS) ein geringeres Risiko für die Entwicklung eines epileptischen Anfalls hatten. Alter und Geschlecht zeigten keine Korrelation zum Auftreten eines epileptischen Anfalls.
96 Patienten (53 Frauen und 43 Männer) erhielten innerhalb der ersten 6 h nach dem Schlaganfall eine auswertbare PCT, davon hatten 17 (17,7%) mindestens einen epileptischen Anfall. Mit Hilfe der PCT wurden für die einzelnen Perfusionsparameter CBF, CBF und TTP der ASPECTS, das Perfusionsdefizit und die relativen Perfusionsparameter bestimmt. Bei dem ASPECTS CBF und ASPECTS CBV zeigte sich, dass die vorderen Mantelregionen M1 und M4 bzw. nur M1 bei den Patienten mit einem epileptischen Anfall signifikant häufiger einen geringeren CBF oder ein geringeres CBV hatten. Ein signifikanter Unterschied ergab sich auch beim Betrachtung der Perfusionsdefizite in Bezug auf den CBF und das CBV: Patienten mit einem epileptischer Anfall zeigten ein größeres Perfusionsdefizit als die Patienten ohne epileptischen Anfall. In die relativen Perfusionsparameter fließt neben dem Perfusionsdefizit noch die Infarktgröße ein. Zwar ergab sich kein signifikanter Unterschied zwischen den beiden Gruppen in Bezug auf die Infarktgröße, trotzdem konnte eine Korrelation zwischen einem erniedrigten R[CBF] bzw. einem erniedrigten R[CBV] und dem Auftreten von epileptischen Anfällen nach einem ischämischen Schlaganfall feststellt werden.
Studies of stroke in experimental animals have demonstrated the neuroprotective efficacy of a variety of interventions; however, most such strategies have failed to show clinical benefits in aged humans. One possible explanation for this discrepancy between animal and clinical studies may be the role that age plays in the recovery of the brain following insult. For example, the poor functional recovery of aged rats after stroke may be caused by a decline in brain plasticity. Although the incidence of ischemic stroke increases dramatically with advancing age, relatively few studies have been conducted on aged animals, which would mimic most closely the context in which stroke occurs in humans. We have shown that, at one week following stroke, there was vigorous expression of MAP1B and its mRNA, as well as MAP2 protein, in the border zone adjacent to the infarct of 3 month- and 20 month-old male Sprague Dawley rats. Hypothesis: The decline in brain plasticity is caused by an age-related decline in the upregulation of factors promoting brain plasticity (MAP1B, ßAPP) and an age-related increase in astroglial scaring and in the expression of neurotoxins such as beta amyloid. Methods: Focal cerebral ischemia was produced by reversible occlusion of the right middle cerebral artery in 3- and 20-month-old male Sprague Dawley rats. The functional outcome was assessed in neurobehavioral tests at 3, 7, 14, and 28 days post-stroke. At these time points, brains were removed and analyzed for markers of (i) brain plasticity (microtubule-associated protein 1B, MAP1B, secreted forms of fi-amyloid precursor protein); (ii) neurogenesis (BrdU-positive cells, doublecortin, nestin); (iii) neurotoxicity (B-amyloid aggregates); (iv) inflammation (microglia, astrocytes, oligodendrocytes, endothelial cells). Results: (1) There was a non-significant tendency for blood pressure to be higher in old than in young rats. By post-stroke day 3 the infarct volume covered about 15% of the cortical neurons in young and 28% in aged rats. By day 7, infarct volumes were roughly equal in the two age groups. (2) Cell counting showed increases in the number of BrdU-positive cells in the infarcted area of old rats at day 3 post-stroke. This increase became even more dramatic at day 7 post-stroke in aged rats. There was no significant contribution of apoptosis to cell death. (3) Behaviorally, young rats recovered gradually and reached a maximum of 90% of baseline performance at day 14, post-stroke while the aged rats recovered only to a maximum of 70% of pre-surgery performance by week 2 post-stroke, and remained at that level. (4) The temporal pattern of recovery correlated well with the expression of growth-associated phenotype of ßAPP as well as with MAP1B accumulation in varicosities along axons (an indicator of growth) in cortical areas affected by stroke and was at maximum between days 14 to 28 in young rats. In contrast, aged rats showed delayed (day 28) and reduced axonal remodelling as well as a delayed (day 28) expression of growth-associated ßAPP. Instead, the neurotoxic carboxy-terminal form of ßAPP steadily accumulated over time and reached a maximum at day 14 in aged rats as compared to 28d for the young rats. Nestin, a marker for immature neurons, overlapped with BrdU-labelled cells at day 7 post-stroke in corpus callosum and at the infarct border in both young and aged rats, suggesting increased stroke-induced neurogenesis. (5) In young rats there was a gradual activation of both microglia and astrocytes that peaked by days 14 to 28 with the formation of a glial scar. In contrast, aged rats showed an accelerated astrocytic and microglial reaction that peaked in week 1 post-stroke. We also noted a strong activation of oligodendrocytes at early stages of infarct development in all rats that persisted in aged rats. Evolution of astrocytic and microglial reactivity closely paralled the time course of scar formation in both young and aged rats and coincided with the stagnation in the recovery rate of aged rats. Conclusions: The time course of functional recovery in young rats correlated well with the expression of plasticity proteins such as MAP1B and ßAPP while an early and persistent expression of the neuro toxic fragment AB in conjunction with a delayed expression of MAP1B and ßAPP may impede functional recovery in aged rats. The results also suggest that a temporally anomalous glial reaction to cerebral ischemia in aged rats leads to the premature formation of scar tissue that impedes functional recovery to stroke.
In dieser Arbeit wird die Hypothese getestet, dass durch H2S-induzierte Hibernation eine Reduktion des Infarktvolumens nach Schlaganfall, sowie eine Verbesserung der funktionellen Erholung erreicht wird. Hierzu wird im Post-Akut-Tiermodell des Schlaganfalls an alten Tieren eine 48-stündige Ganzkörper-Hypothermie induziert. An diesen Tieren wird dann die funktionelle Erholung nach einem Schlaganfall ermittelt sowie das Infarktvolumen gemessen. Daraus könnten sich neue Behandlungsstrategien für Schlaganfallpatienten ableiten lassen. Wir können demonstrieren, dass gealterte Ratten nach einem Schlaganfall eine 50%ige Reduktion der Infarktgröße aufweisen, wenn ihre Körpertemperatur für 48 Stunden mittels H2S gesenkt wird ohne offensichtliche zusätzliche neurologische Defizite oder physiologische Nebenwirkungen zu produzieren. Gleichzeitig verbessert sich die Leistung der Tiere in Versuchen, die komplex-motorische Funktionen erfordern. Des weiteren zeigt sich in unseren Experimenten, dass die Anzahl der Makrophagen-ähnlichen Zellen, sowie die mRNA Expression der Marker für Inflammation und Apoptose NF kappa B, grp78 und caspase 12 in den Versuchstieren deutlich niedriger ist als in den Kontrolltieren und dass bei den Hypothermie-Tieren der Infarktkern deutlich weniger apoptotische Zellen aufweist als in den Kontrollen. Unsere Ergebnisse zeigen, dass die Hypothermie signifikantes Potential bei der Behandlung des Schlaganfalls im Nager hat und weitere Untersuchungen zu diesem Thema in klinischen Studien durchgeführt werden sollten.
Vaskuläre Erkrankungen stellen in Deutschland und in anderen industrialisierten Ländern die häufigsten Todesursachen dar. Neben traditionellen Risikofaktoren der Atherosklerose wie Hypertonus, Diabetes mellitus, Hypercholesterinämie und Nikotinabusus, rückten in den vergangenen Jahren zunehmend genetische Faktoren der Atherosklerose in den Mittelpunkt des wissenschaftlichen Interesses. Pathophysiologisch spielt das Renin-Angiotensin-System (RAS) eine wichtige Rolle bei der Entstehung der Atherosklerose. Die Aktivität des zirkulierenden und wahrscheinlich auch des gewebsständigen ACE steht unter einer deutlichen genetischen Kontrolle. Dabei variiert die Enzymaktivität zwischen den einzelnen Genotypen um den Faktor 20 bis 40. Das menschliche ACE-Gen ist auf Chromosom 17q23 lokalisiert und weist einen Insertions/Deletions-Polymorphismus auf. Das Ziel der vorliegenden Arbeit war es festzustellen, ob in der Basisstudie der Study of Health in Pomerania (SHIP-0), einer populationsbezogenen Querschnittsstudie im Nordosten Deutschlands, eine Assoziation zwischen dem ACE I/D-Polymorphismus und atherosklerotischen Gefäßwandläsionen besteht. Hierzu wurden sowohl subklinische Risikomarker wie die Intima-Media-Dicke (IMT) in der A. carotis als auch manifeste atherosklerotische Veränderungen wie Plaques und Stenosen untersucht. Von den 4310 Teilnehmern der SHIP-0-Studie wurde bei 4262 Personen (98,9%) der ACE-Polymorphismus bestimmt. Die gentechnische Analyse der DNA ergab, dass 1031 Personen (23,9%) den Genotyp II besitzen, 2131 Personen (49,4%) haben den Genotyp ID und 1100 (25,5%) der Untersuchten zeigen den Genotyp DD. Insgesamt konnten in der Gesamtpopulation keine signifikanten Assoziationen zwischen dem ACE-Polymorphismus und der IMT nachgewiesen werden. Es fanden sich ebenfalls keine signifikanten Zusammenhänge zwischen dem Polymorphismus und klinischen Endpunkten wie vaskulären Erkrankungen. In einem weiteren Schritt der Studie wurden Subgruppenanalysen durchgeführt. Ausgeschlossen wurden Diabetiker und Raucher, um zu verhindern, dass diese Risikofaktoren der Atherosklerose den Effekt des ACE-Polymorphismus maskieren. Dabei zeigte sich, dass Stenosen der A. carotis (> 50%) bei männlichen Teilnehmern mit dem DD-Genotyp signifikant häufiger nachweisbar waren als bei Teilnehmern mit dem II-Genotyp. Die Regressionsanalyse in dieser Subgruppe zeigte eine positive Assoziation zwischen dem D-Allel und dem Auftreten von Stenosen der A. carotis, mit einem 2,9fach (homozygote D-Allel-Träger) bzw. 1,2fach (heterozygote D-Allel-Träger) erhöhtem Risiko. Einschränkend ist jedoch anzumerken, dass diese Subgruppenanalysen als explorative Pilotstudien zu verstehen sind, die das Ziel besitzen, Hypothesen zu formulieren. Sie sind aufgrund nicht ausreichender statistischer Trennschärfe (so genannte statistische Power) nicht geeignet, potentielle Assoziationen zu überprüfen. Die Ergebnisse der SHIP-Studie stehen somit im Einklang mit vier weiteren mittlerweile publizierten großen epidemiologischen Studien, die ebenfalls keine Assoziationen zwischen dem ACE-Genpolymorphismus und der IMT bzw. atherosklerotischen Plaques der A. carotis nachweisen konnten. Dennoch scheint eine, allerdings nur schwache Assoziation zwischen dem homozygoten DD-Genotyp des ACE-Polymorphismus und subklinischen atherosklerotischen Gefäßwandveränderungen zu bestehen, die ähnlich der Assoziation zwischen ACE I/D-Polymorphismus und Myokardinfarkten bzw. ischämischen Schlaganfällen nur in Meta-Analysen nachgewiesen werden kann.
Abstract
Head motion during magnetic resonance imaging (MRI) induces image artifacts that affect virtually every brain measure. In parallel, cross‐sectional observations indicate a correlation of head motion with age, psychiatric disease status and obesity, raising the possibility of a systematic artifact‐induced bias in neuroimaging outcomes in these conditions, due to the differences in head motion. Yet, a causal link between obesity and head motion has not been tested in an experimental design. Here, we show that a change in body mass index (BMI) (i.e., weight loss after bariatric surgery) systematically decreases head motion during MRI. In this setting, reduced imaging artifacts due to lower head motion might result in biased estimates of neural differences induced by changes in BMI. Overall, our finding urges the need to rigorously control for head motion during MRI to enable valid results of neuroimaging outcomes in populations that differ in head motion due to obesity or other conditions.
Background: Primary angiitis of the central nervous system (PACNS) is a rare but serious condition. A fraction of patients suffering from PACNS concurrently exhibit pronounced cerebral amyloid angiopathy (CAA) which is characterized by deposits of amyloid-β (Aβ) in and around the walls of small and medium-sized arteries of the brain. PACNS with CAA has been identified as a distinct disease entity, termed Aβ-related angiitis (ABRA). Evidence points to an immune reaction to vessel wall Aβ as the trigger of vasculitis. Objective: To investigate whether the inflammatory response to Aβ has (1) any effect on the status of immune activation in the brain parenchyma and (2) leads to clearance of Aβ from brain parenchyma. Methods: We studied immune activation and Aβ load by quantitative immunohistochemical analysis in brain parenchyma adjacent to affected vessels in 11 ABRA patients and 10 matched CAA controls. Results: ABRA patients showed significantly increased immune activation and decreased Aβ loads in the brain parenchyma adjacent to affected vessels. Conclusion: Our results are in line with the hypothesis of ABRA being the result of an excessive immune response to Aβ and show that this can lead to enhanced clearance of Aβ from the brain parenchyma by immune-mediated mechanisms.
Background: Intact socio-cognitive abilities, such as theory of mind (ToM), facial emotion recognition (FER), social decision making (SDM) and visual perspective taking (VPT), are essential for human well-being and quality of life. Impairment in social cognition can have major implications for health in affected individuals and society as a whole. Evidence for changes in social cognition in healthy and pathological aging processes, such as subjective cognitive decline (SCD) and mild cognitive impairment (MCI), is currently either sparse or inconclusive. It is important to determine how social cognition changes in healthy and pathological aging and provide grounds for targeted and early assessment and intervention. The aims of this thesis were to investigate social cognition across four domains, in particular, ToM, FER, SDM and VPT, in healthy young and older individuals, as well as in individuals with cognitive deficits, such as SCD and MCI. In the case of a decline, further goals were to investigate the degree of impairment and the domains affected.
Methods: A systematic literature search was conducted in four major academic databases, MEDLINE, Web of Science Core Collection, CENTRAL, and PsycInfo, for studies investigating social cognition in healthy young and old individuals as well as individuals affected by SCD and MCI which met the inclusion criteria. The primary outcome was ToM and secondary outcomes were FER, SDM and VPT. After a systematic review was performed, studies eligible for meta-analysis were divided according to comparison groups and outcomes. Random-effects meta-analyses were conducted using standardized mean differences (SMD). Risk of Bias was assessed using the “Tool to assess risk of bias in cohort studies” modified for the present study design.
Results: After a thorough systematic literature search, 86 studies containing 88 comparisons were included in the systematic review, of which 47 were eligible for quantitative analysis. The meta-analysis revealed a progressive decline in ToM and FER abilities from young adulthood to MCI. Varying effect sizes demonstrated different trajectories of change for specific domains. Due to a lack of research, data investigating SDM and VPT, as well as SCD were insufficient for quantitative analysis.
Conclusion: ToM and FER decline gradually from healthy to pathological aging. Therefore, assessment of social cognition is important and should be incorporated in routine neurocognitive testing, so that targeted interventions can be introduced when needed. With this information in mind, future research should focus on the development of new assessment tools, as well as preventive and treatment strategies. This review also identified research gaps in certain populations (e.g. SCD, middle age, MCI-subtypes) as well as domains (VPT and SDM) that need to be addressed in the future.
Polypharmacy in patients with multiple sclerosis and the impact on levels of care and therapy units

Background: The aim of this study was to examine the societal costs of polypharmacy in patients with multiple sclerosis (MS). We therefore focused on the association between the number of medications on the level of care (LOC), the German classification of the need for care, and the number of therapy sessions (TTU).
Methods: In addition to demographic information and medication, 101 MS patients performed the Multiple Sclerosis Health Resource Utilization Survey (MS-HRS). Medications were subdivided into a total number of medications (TD), MS-related medication [MSD, i.e., disease-modifying drugs (DMDs) and symptomatic treatment (SD)], and medication for comorbidities (CDs). Multivariate linear regression models were performed to estimate if the amount of each medication type affects LOC or TTU.
Results: Polypharmacy appeared in 54 patients at the time of the survey. The relative risk (RR) of LOC 1 increased significantly by 2.46 (p = 0.001) per TD and by 2.55 (p = 0.004) per MSD, but not per CD (RR 1.44; p = 0.092). The effect of RR on MSD was driven by SD (RR 2.2; p = 0.013) but not DMD (RR 2.6; p = 0.4). RR of MSD remained significant for LOC 2 (1.77; p = 0.009) and LOC 3/4 (1.91; p = 0.015), with a strong trend in RR of SD, but not DMD. TTU increased significantly per MSD (p = 0.012), but not per TD (p = 0.081) and CD (p = 0.724).
Conclusion: The number of MSDs is related to the likelihood of a higher level of care and the number of therapy sessions and is therefore a good indication of the extent of the societal costs.
Neurodegenerative Erkrankungen sind durch einen progressiven Verlust von Neuronen oder Gliazellen im Gehirn oder Rückenmark gekennzeichnet. Der Ersatz dieser zu Grunde gegangen Zellen, zum Beispiel durch neurale Stammzellen (NSCs) stellt dabei einen möglichen kurativen Behandlungsansatz dar, bei dem alternativ zur stereotaktisch-chirurgischen Zelltransplantation die Injektion der NSCs in den Liquorraum als atraumatische Transplantationsmethode in Frage kommt. In vorangegangenen Studien hierzu wird allerdings über eine sehr geringe Migration der intrathekal injezierten Zellen ins Hirnparenchym berichet, zurückzuführen möglicherweise auf einen zu niedrigen Nährstoffgehalt des Liquors. Andererseits ist bekannt, dass Liquor ein wichtiges Milieu für das Überleben NSCs und deren Zellpfaddetermination im sich entwickelnden Gehirn darstellt. In unserer Studie haben wir deshalb den Einfluss von Liquor auf das Verhalten adulter humaner (ahNSCs) und fetaler muriner neuronaler Stammzellen (fmNSCs)in vitro untersucht. Die zentralen Ergebnisse unserer Studie sind: (i) sowohl während der Expansion als auch während der Differenzierung führt Liquor zu einer erhöhten Überlebensrate ahNSCs und fmNSCs im Vergleich zum standard Expansions-/DIfferenzierungsmedium (ii) Liquor fördert das Auswachsen von Zellfortsätzen im Vergleich zum Standardmedium und führt so zu einem schnelleren Verlust der Teilungsfähigkeit (iii) Liquor förder die Astrogliogenese in ahNSCs und fmNSCs und hemmt die Neurogenese in fmNSCs. Eine nierige Überlebensrate der intrathekal injezierten Zellen auf Grund eines zu niedrigen Nährstoffgehaltes scheint auf Grund unserer Ergebnisse somit unwahrscheinlich. Stattdessen könnte das schnelle Auswachsen der Zellförtsätze nach intrathekaler Injektion zu einer Adhäsion injezierter Stammzellen an der Ventrikelwand führen und somit eine ausreichende intraparenchymale Migration verhindern. Als zusätzlich negativen Effekt des Liquors zeigte sich in unserer Studie eine Förderung der Astrogliogenese und Hemmung der Differenzierung in benötigte Neurone. Vor der praktischen Umsetzung der intrathekalen Stammzellinjektion als alternative Transplantationsmethode sind deshalb weitere Studien zu den Inhaltsstoffen des Liquors und Möglichkeiten der Beeinflussung dieser notwendig.
Aged rats recover poorly after unilateral stroke, whereas young rats recover readily possibly with the help from the contralateral, healthy hemisphere. We asked whether anomalous, age-related changes in the transcriptional activity in the brains of aged rats could be one underlying factor contributing to reduced functional recovery. We analysed gene expression in the periinfarct and contralateral areas of 3-month- and 18-month-old Sprague Dawley rats. Our experimental end-points were cDNA arrays containing genes related to hypoxia signalling, DNA damage and apoptosis, cellular response to injury, axonal damage and regrowth, cell lineage differentiation, dendritogenesis and neurogenesis. The major transcriptional events observed were: (i) Early up-regulation of DNA damage and down-regulation of anti-apoptosis-related genes in the periinfarct region of aged rats after stroke; (ii) Impaired neurogenesis in the periinfarct area, especially in aged rats; (iii) Impaired neurogenesis in the contralateral (unlesioned) hemisphere of both young and aged rats at all times after stroke and (iv) Marked up-regulation, in aged rats, of genes associated with inflammation and scar formation. These results were confirmed with quantitative real-time PCR. Conclusion I: reduced transcriptional activity in the healthy, contralateral hemisphere of aged rats in conjunction with an early up-regulation of DNA damage-related genes and pro-apoptotic genes and down-regulation of axono- and neurogenesis in the periinfarct area are likely to account for poor neurorehabilitation after stroke in old rats. Efficient neuroprotection after stroke requires long-term, regulated lowering of whole body temperature. After stroke, exposure of aged rats to hydrogen sulfide resulted in sustained, deep hypothermia (30.8 ± 0.7°C) that led to a 50% reduction in infarct size with a concomitant reduction in the number of phagocytic cells. At the transcription level, we found an overall decrease in the transcriptional activity related to inflammation and apoptosis. Behaviorally, hypothermia was associated with better performance on tests that require complex sensorimotor skills, in the absence of obvious neurological deficits or physiological side-effects, in aged rats. Conclusion II: Prolonged hypothermia is a simple and efficacious method to limit damage inflicted by stroke in aged rats.
Die sonographische Diagnostik nimmt in vielen Bereichen der Medizin einen hohen Stellenwert ein. Durch den technischen Fortschritt können heutzutage auch die peripheren Nerven inklusive verschiedenster pathologischer Veränderungen dargestellt werden. Die Messung der Nervenquerschnittsfläche (nerve cross sectional area = NCSA) im transversalen Bild hat sich in vielen Studien als geeigneter Parameter zur Beurteilung bzw. zum Vergleich des Nervenkalibers herausgestellt. Polyneuropathien (PNP) sind Erkrankungen des peripheren Nervensystems. Man differenziert zwischen primär demyelinisierenden, axonalen sowie gemischten PNP. Ätiologisch lassen sich u. a. metabolische, toxische und immunologische PNP unterscheiden. In den bisher publizierten Studien zur Sonographie bei PNP konnte beobachtet werden, dass besonders demyelinisierende PNP zu einer Vergrößerung der NCSA führen. Informationen zum sonographischen Erscheinungsbild der in der Gesellschaft häufigen PNP (diabetisch, Alkohol assoziierte, PNP bei Vitamin B12 Defizit) sind jedoch bisher limitiert. In der vorliegenden explorativen Pilotstudie wurde geprüft, ob die NCSA bei PNP größer ist als bei gesunden Nerven und ob Zusammenhänge zwischen der NCSA und dem Schädigungstyp, der Ätiologie oder dem elektrophysiologisch bestimmten Schweregrad bestehen. Um diese Fragestellungen zu beantworten, wurde die NCSA an insgesamt 12 fest definierten, außerhalb physiologischer Engpassstellen liegenden, Messpositionen unterschiedlicher Nerven (N. medianus, N. ulnaris, N. tibialis, Spinalwurzel C6 / C7, Trunci des Plexus brachialis) bestimmt. Die Reliabilität der Ergebnisse wurde bei 5 Probanden durch eine Wiederholung der Messungen im Abstand von mehr als 24 h geprüft. Dabei konnte gezeigt werden, dass eine gute Reproduzierbarkeit der Messungen besteht, da bei sämtlichen Messpunkten, ausgenommen dem Truncus inferior, ein Variationskoeffizienten von weniger als 20 % vorgelegen hat. Die Untersuchung bei 27 gesunden Probanden ergab, dass keine Abhängigkeit der NCSA von der Körperseite besteht, sodass der Untersuchungsumfang bei den 56 Patienten mit elektrophysiologisch gesicherten PNP auf eine Körperseite reduziert werden konnte. Die wichtigsten Ergebnisse der Studie sind, dass Patienten mit PNP größere NCSA aufweisen als Gesunde. Die Verdickung der Nerven ist sowohl in proximalen als auch in distalen Abschnitten nachweisbar und ist sowohl bei demyelinisierenden als auch bei axonalen PNP vorhanden. Die vorliegende Arbeit liefert weiterhin Hinweise dafür, dass demyelinisierende PNP eine deutlichere Vergrößerung der NCSA aufweisen als primär axonale Schädigungen und dass sie sich dadurch sonographisch voneinander unterscheiden. Außerdem ist die NCSA bei immunvermittelten Neuropathien wie dem GBS, der CIDP und den paraproteinämischen PNP besonders in den proximalen Abschnitten größer als bei PNP anderer Ätiologie. Eine Assoziation zwischen dem Schweregrad und der NCSA konnte nicht beobachtet werden. Die Ergebnisse der vorliegenden Studie stehen im Einklang mit dem Grundtenor der Literatur. Anzumerken ist jedoch, dass die bisherigen Publikationen überwiegend Einzelvergleiche beinhalten, in denen meist immunvermittelte PNP im Fokus stehen, oder dass sie sich häufig auf die oberflächlicher gelegenen Nerven der oberen Extremitäten beschränkt haben. Stärken der vorliegenden Studie sind die Vielfältigkeit der ausgewählten Nervensegmente und der Verzicht auf Messungen an Engpassstellen sowie die Mannigfaltigkeit der verglichenen Ätiologien. Ein limitierender Faktor der vorliegenden Studie ist die nicht immer gewährleistete Verblindung des Untersuchers bezüglich der Ätiologien und die teilweise geringen Subgruppengrößen. Aus diesem Grund wurde die vorliegende Studie als explorative Pilotstudie angelegt, mit dem Ziel, Hypothesen zu generieren und die Berechnung eines Stichprobenumfangs zukünftiger konfirmatorischer Studien zu ermöglichen, an Hand derer dann mit ausreichender statistischer Trennschärfe (Power) die aufgestellten Hypothesen überprüft werden können.
Abstract: Ischemic stroke is an aging disease and causes high mortality or long-term disability. The reduced neurological recovery in aging is possibly associated with impairment of angiogenesis and non-specific enhancement of inflammatory reaction. To check this hypotheses, those events were compared within young and elder animals brain at day 14 following focal ischemic stroke. Moreover, it is of importance to investigate also the potential therapies of indomethacin for prolonging the therapeutic window using aged animal models. The focus of present study was on neurobiological and neurological differences between young and old rats modulated by indomethacin daily treatment beginning at four hours post-ischemic episode. The effectiveness of indomethacin treatment in young and elder rats was probed using immunohistochemistry, oligonucleotide microarray, Real Time PCR and neurological evaluation. Our results provide insight of several age-independent positive consequences of Cox non-specific inhibition by indomethacin including increased NeuN positive surviving neurons, reduced infarct volume and enhanced neuroprotective response of innate immune system evidenced by increased Iba1 and Anx3 immunoreactivities in moderately activated microglia in periinfarction. From gene level we observed in both age groups downregulation of Mdk and Cxcl1 chemokines, and Id3 transcription factor which might modulate inflammatory response and facilitate repair. Other several findings showed age-dependent drug effect. Indomethacin had reduced efficacy in aged ischemic brain. From a total of 34 genes differential regulated, we observed 43% in young and only 28% of genes in aged have tendency toward age-matched sham expression level. In aged rats, indomethacin is ineffective in inhibiting phagocytic activity which is probably due to no expression changes of several cytokines like Tnfá and Cxcl4. Also, at protein level we observed no change of lysosomal ED1 immunoreactivity under treatment. On the other hand aging is characterized by no expression changes of Plau, Timp1, Timp2 and Col18a1 after treatment resulting in no improvement of angiogenesis. In young rats, conversely, drug administration decreased phagocytic activity by downregulating several cytotoxic cytokines such as Tnfá and Cxcl4. Moreover, the observable decrease of proteases like MMP10, Plau and MMP inhibitor Timp2 employed in matrix remodeling together with downregulation of Col18a1 expression after treatment might sustain angiogenesis in young rat ischemic brain. Indomethacin improves the motor-sensory performance in ischemic stroke rats as compared with age-matched untreated animals. Young rats fully recovered while aged showed important recuperation but did not achieve the preoperative level. In view of all this, indomethacin treatment might be consider as adjuvant therapy following ischemic stroke, even if aging blunts the positive effect of indomethacin on altered angiogenic-related gene expression. Because of the small number of rats, the results obtained from this study show only a tendency to significance and that further studies with more animals need to be statistically validated before firmly conclusions can be drawn.
