Many intrastate conflicts see more than one mediation effort. As the sequencing of mediation efforts in intrastate conflicts is neglected in existing research, this project addresses the question how and why previous mediation outcomes have an impact on subsequent mediation onset and subsequent mediation success. Drawing on bargaining theory, it is argued that governments and rebel groups engaged in intrastate conflicts account for previous mediation outcomes in their cost-benefit calculations on subsequent mediation onset, and, should subsequent talks set on, their behaviour during subsequent mediation efforts, which influences subsequent mediation success.
If mediation did not produce an agreement, the persistence of the private information problem is noted by the conflict parties. Yet, no new costs of mediation are uncovered, and hence the conflict parties will agree to subsequent mediation onset. Being aware of the necessity to overcome the private information and the commitment problem, the mediator will seek to account for the concerns of the conflict parties, and thereby work towards subsequent mediation success. If mediation produced a partial agreement, the benefits of mediation are underlined. The private information and the commitment problem seem solved with the assistance of the mediator. Subsequent mediation onset and eventually subsequent mediation success are observed. If a mediated agreement was reneged on by the rebel group, the government will refrain from further talks, pointing out the rebel group’s illegitimacy. If the government reneged on the agreement itself, it will also decide against subsequent mediation, as the previous mediation effort produced an agreement which did not mirror the power distribution in the dyad. Costs of mediation, which outweigh the benefits of it, were highlighted. Rebel groups will opt for mediation regardless which side reneged on an agreement. As both governments and rebel groups have to agree to subsequent mediation for talks to set on, subsequent mediation onset is unlikely if a mediated agreement was reneged on. Given the onset of subsequent mediation after a mediated agreement was reneged on, subsequent mediation success is unlikely to be observed, due to the previously underlined hazards of sharing private information and the persistence of the commitment problem.
The theoretical argument is tested with a mixed-methods approach. The quantitative analysis accounts for mediation efforts in African intrastate conflicts between 1993 and 2007. The qualitative analysis scrutinises the mediation efforts between the Government of Uganda and the Lord’s Resistance Army. The results of both parts of analysis largely go hand-in-hand, and show that partial mediation success and mediation which did not produce an agreement have a positive impact on subsequent mediation onset in particular, but also on subsequent mediation success. Reneged on mediated agreements have a severe negative impact on subsequent mediation onset and subsequent mediation success though.
By addressing the question which impact previous mediation outcomes have on subsequent mediation efforts, this research shows that mediation which does not produce an agreement is not the mediation outcome which needs to be feared by the international community. Instead, the deteriorating impact of short-lived agreements, a mediation outcome which is unaccounted for in existing research as an explanatory variable, becomes apparent. This research has important policy implications, especially for mediators, as it suggests that accepting mediation efforts to end without an agreement is more conducive for subsequent mediation efforts. Moreover, this research points towards the necessity of including reneged on agreements in mediation research as an explanatory variable more extensively, thereby shedding more light onto the dynamics at play in consecutive mediation efforts.
Background and Aims: Gastrointestinal stromal tumors (GISTs) are rare malignancies but the most common mesenchymal tumors of the digestive tract. Recent advances in diagnostic imaging and an increasing incidence will confront us more frequently with stromal tumors. This single center study aimed to characterize GIST patients in terms of tumor location, clinical presentation, metastasis formation, as well as associated secondary malignancies. Methods: In a retrospective study, 104 patients with a histologically confirmed diagnosis of GIST, collected between 1993 and 2011, were characterized for several clinical features. Results: The most common GIST location was the stomach (67.6%) followed by the small intestine (16.2%). Gastrointestinal bleeding (55.8%) and abdominal pain (38.5%) were the most frequently reported symptoms whereas about one-third of patients remained clinically asymptomatic (31.6%); 14.4% of patients had either synchronous or metachronous metastases and there was a significant prevalence also in the low risk group. The proportion of secondary malignant associated neoplasms was 31% in our GIST cohort, among which gastrointestinal, genitourinary tumors, and breast cancer were the most prevalent. Conclusion: There was a considerable risk for metastasis formation and the development of secondary neoplasias that should encourage discussion about the appropriate surveillance strategy after surgery for GIST.
Tuberculosis (TB) has tremendous public health relevance. It most frequently affects the lung and is characterized by the development of unique tissue lesions, termed granulomas. These lesions encompass various immune populations, with macrophages being most extensively investigated. Myeloid derived suppressor cells (MDSCs) have been recently identified in TB patients, both in the circulation and at the site of infection, however their interactions with Mycobacterium tuberculosis (Mtb) and their impact on granulomas remain undefined. We generated human monocytic MDSCs and observed that their suppressive capacities are retained upon Mtb infection. We employed an in vitro granuloma model, which mimics human TB lesions to some extent, with the aim of analyzing the roles of MDSCs within granulomas. MDSCs altered the structure of and affected bacterial containment within granuloma-like structures. These effects were partly controlled through highly abundant secreted IL-10. Compared to macrophages, MDSCs activated primarily the NF-κB and MAPK pathways and the latter largely contributed to the release of IL-10 and replication of bacteria within in vitro generated granulomas. Moreover, MDSCs upregulated PD-L1 and suppressed proliferation of lymphocytes, albeit with negligible effects on Mtb replication. Further comprehensive characterization of MDSCs in TB will contribute to a better understanding of disease pathogenesis and facilitate the design of novel immune-based interventions for this deadly infection.
Abstract
Background: Behavior management techniques (BMT) are essential in order to
achieve a successful dental treatment with a minimum amount of stress in paediatric
dentistry, but parents are not equally accepting different advanced BMT.
Purpose: To investigate the differences in parental acceptance of advanced
behavior management techniques between University of Greifswald/Germany and
Jordan University/Jordan.
Methods: Parents of the children treated in the pedodontic departments at the
University of Greifswald/Germany and Jordan University/Jordan rated their
acceptance level of four advanced behavior management techniques (passive
restraint, active restraint, nitrous oxide sedation and general anesthesia) for normal
treatment, and for urgent treatment using a five points Likert scale. 200 parents (100
in each university) completed the questionnaire forms for analysis.
Results: Nitrous oxide sedation was rated the most accepted technique in
Greifswald and Jordan (mean 3.78±1.34; 3.22±1.50, respectively). The least
acceptable technique in Greifswald was passive restraint (2.05±1.18) and in Jordan
general anesthesia (2.11±1.30). The parents in Greifswald are significantly more
accepting nitrous oxide sedation than parents in Jordan (p=0.010), while parents in
Jordan are significantly more willing to accept passive restraint (p=0.001). The
acceptance of all advanced behavior management techniques increased significantly
in both groups when the treatment is urgent (p≤0.05),
Conclusions: Parental culture and the urgency of the treatment affect the
acceptance to different behavior management techniques. Moreover, the parental
attitude to the pharmacological technique has changed, as nitrous oxide sedation
generally appears to be the most preferred advanced technique in both groups.
Significance of Hyperbaric Oxygenation in the Treatment of Fournier’s Gangrene: A Comparative Study

Introduction: Hyperbaric oxygenation (HBO), in addition to anti-infective and surgical therapy, seems to be a key treatment point for Fournier’s gangrene. The aim of this study was to investigate the influence of HBO therapy on the outcome and prognosis of Fournier’s gangrene. Patients and Methods: In the present multicenter, retrospective observational study, we evaluated the data of approximately 62 patients diagnosed with Fournier’s gangrene between 2007 and 2017. For comparison, 2 groups were distinguished: patients without HBO therapy (group A, n = 45) and patients with HBO therapy (group B, n = 17). The analysis included sex, age, comorbidities, clinical symptoms, laboratory and microbiological data, debridement frequency, wound dressing, antibiotic use, outcome and prognosis. The statistical analysis was performed with GraphPad Prism 7® (GraphPad Software, Inc., La Jolla, USA). Results: Demographic data showed no significant differences. The laboratory parameters C-reactive protein and urea were significantly higher in group B (group B: 301.7 vs. 140.6 mg/dL; group A: 124.8 vs. 54.7 mg/dL). Sepsis criteria were fulfilled in 77.8 and 100% of the patients in groups A and B respectively. Treatment in the intensive care unit (ICU) was therefore indicated in 69% of the patients in group A and 100% of the patients in group B. The mean ICU stay was 9 and 32 days for patients in groups A and B respectively. The wound debridement frequency and hospitalization stay were significantly greater in group B (13 vs. 5 debridement and 40 vs. 22 days). Initial antibiosis was test validated in 80% of the patients in group A and 76.5% of the patients in group B. Mortality was 0% in group B and 4.4% in the group A. Conclusion: The positive influence of HBO on the treatment of Fournier’s gangrene can be estimated only from the available data. Despite poorer baseline findings with comparable risk factors, mortality was 0% in the HBO group. The analysis of a larger patient cohort is desirable to increase the significance of the results.

Conclusion: The positive influence of HBO on the treatment of Fournier’s gangrene can be estimated only from the available data. Despite poorer baseline findings with comparable risk factors, mortality was 0% in the HBO group. The analysis of a larger patient cohort is desirable to increase the significance of the results.
Species of the genus Wolffia are traditionally used as human food in some of the Asian countries. Therefore, all 11 species of this genus, identified by molecular barcoding, were investigated for ingredients relevant to human nutrition. The total protein content varied between 20 and 30% of the freeze-dry weight, the starch content between 10 and 20%, the fat content between 1 and 5%, and the fiber content was ~25%. The essential amino acid content was higher or close to the requirements of preschool-aged children according to standards of the World Health Organization. The fat content was low, but the fraction of polyunsaturated fatty acids was above 60% of total fat and the content of n-3 polyunsaturated fatty acids was higher than that of n-6 polyunsaturated fatty acids in most species. The content of macro- and microelements (minerals) not only depended on the cultivation conditions but also on the genetic background of the species. This holds true also for the content of tocopherols, several carotenoids and phytosterols in different species and even intraspecific, clonal differences were detected in Wolffia globosa and Wolffia arrhiza. Thus, the selection of suitable clones for further applications is important. Due to the very fast growth and the highest yield in most of the nutrients, Wolffia microscopica has a high potential for practical applications in human nutrition.
In Deutschland gibt es ca. 3 Millionen Kontaktlinsenträger und pro Jahr ca. 12000 kontaktlinsen-assoziierte Keratitiden. Die Hauptursache dieser Keratitiden ist die mangelnde Compliance der Kontaktlinsenträger hinsichtlich einer adäquaten Kontaktlinsenhygiene. Leider sind jedoch die gängigen Reinigungsmethoden, besonders die am häufigsten angewandte chemische Reinigung mit den so genannten All-in-One-Lösungen besonders anfällig für Hygienefehler (Austauschrhythmen, Einwirkzeiten, mechanische Vorreinigung).
In dieser Arbeit wurde nun ein neues Desinfektionsverfahren getestet, das weitestgehend unabhängig von der Compliance der Anwender ist. Dazu setzten wir UVC-Strahlung ein und bestrahlten damit weiche Monatslinsen. Wir verwendeten zwei unterschiedlich dimensionierte Prototypen (UVC-Bestrahlungsgerät, LED-UVC-Bestrahlungsgerät). Wir testeten das Verfahren anhand der Vorschriften der Europäischen Norm EN ISO 14729. Diese Norm regelt die mikrobiellen Anforderungen und Prüfverfahren für Produkte und Systeme zum Hygienemanagement von Kontaktlinsen. Sie verlangt zwei verschiedene Test. Der Stand Alone Test ist die Pflegemitteldirektprüfung. Dabei wird das neue Desinfektionsverfahren an einer Keimaufschwemmung getestet. Der Regimen Test ist die Prüfung des Verfahrens anhand der eigenen Pflegeanleitung. Das Verfahren muss dazu an 5 Testkeimen (Staphylococcus aureus, Serratia marcescens, Pseudomonas aeruginosa, Candida albicans, Fusarium solani) getestet werden.
Parallel zur UVC-Bestrahlung testeten wir noch vier handelsübliche All-in-One-Lösungen (Perfect Aqua Plus (MPG&E), Opti-Free Replenish (Alcon), All-in-One light (Sauflon Pharmaceuticals), Solo Care Aqua (Ciba Vision)) und verglichen dann die Ergebnisse.
Die Versuche bewiesen, dass die UVC-Bestrahlung sehr gut geeignet ist, um weiche Kontaktlinsen zu desinfizieren. Es reichten nur 30 s (UVC-Bestrahlungsgerät) bzw. 2 Stunden (LED-UVC-Bestrahlungsgerät) Bestrahlung aus um die geforderte Log-Reduktion von 3-5 Logstufen bei allen 5 Testorganismen zu erreichen.
Leider mussten wir feststellen, dass die All-in-One-Lösungen deutlich schlechter abschnitten und die Norm somit zum größten Teil nicht erfüllten, wenn man auf den Schritt der manuellen Vorreinigung verzichtete.
Zusammenfassend ist also zu sagen, dass die UVC-Bestrahlung eine sehr gute Alternative für die mikrobielle Reinigung von Kontaktlinsen, im Vergleich zu den herkömmlichen Verfahren, darstellt und dass ihre Wirksamkeit dabei weitestgehend unabhängig von der Compliance der Anwender ist.
Ductal Mucus Obstruction and Reduced Fluid Secretion Are Early Defects in Chronic Pancreatitis

Objective: Defective mucus production in the pancreas may be an important factor in the initiation and progression of chronic pancreatitis (CP), therefore we aimed to (i) investigate the qualitative and quantitative changes of mucus both in human CP and in an experimental pancreatitis model and (ii) to correlate the mucus phenotype with epithelial ion transport function.
Design: Utilizing human tissue samples and a murine model of cerulein induced CP we measured pancreatic ductal mucus content by morphometric analysis and the relative expression of different mucins in health and disease. Pancreatic fluid secretion in CP model was measured in vivo by magnetic resonance cholangiopancreatography (MRCP) and in vitro on cultured pancreatic ducts. Time-changes of ductal secretory function were correlated to those of the mucin production.
Results: We demonstrate increased mucus content in the small pancreatic ducts in CP. Secretory mucins MUC6 and MUC5B were upregulated in human, Muc6 in mouse CP. In vivo and in vitro fluid secretion was decreased in cerulein-induced CP. Analysis of time-course changes showed that impaired ductal ion transport is paralleled by increased Muc6 expression.
Conclusion: Mucus accumulation in the small ducts is a combined effect of mucus hypersecretion and epithelial fluid secretion defect, which may lead to ductal obstruction. These results suggest that imbalance of mucus homeostasis may have an important role in the early-phase development of CP, which may have novel diagnostic and therapeutic implications.
Introduction
We retrospectively evaluated the efficacy and toxicity of gross tumor volume (GTV) mean dose optimized stereotactic body radiation therapy (SBRT) for primary and secondary lung tumors with and without robotic real-time motion compensation.
Materials and methods
Between 2011 and 2017, 208 patients were treated with SBRT for 111 primary lung tumors and 163 lung metastases with a median GTV of 8.2 cc (0.3–174.0 cc). Monte Carlo dose optimization was performed prioritizing GTV mean dose at the potential cost of planning target volume (PTV) coverage reduction while adhering to safe normal tissue constraints. The median GTV mean biological effective dose (BED)10 was 162.0 Gy10 (34.2–253.6 Gy10) and the prescribed PTV BED10 ranged 23.6–151.2 Gy10 (median, 100.8 Gy10). Motion compensation was realized through direct tracking (44.9%), fiducial tracking (4.4%), and internal target volume (ITV) concepts with small (≤5 mm, 33.2%) or large (>5 mm, 17.5%) motion. The local control (LC), progression-free survival (PFS), overall survival (OS), and toxicity were analyzed.
Results
Median follow-up was 14.5 months (1–72 months). The 2-year actuarial LC, PFS, and OS rates were 93.1, 43.2, and 62.4%, and the median PFS and OS were 18.0 and 39.8 months, respectively. In univariate analysis, prior local irradiation (hazard ratio (HR) 0.18, confidence interval (CI) 0.05–0.63, p = 0.01), GTV/PTV (HR 1.01–1.02, CI 1.01–1.04, p < 0.02), and PTV prescription, mean GTV, and maximum plan BED10 (HR 0.97–0.99, CI 0.96–0.99, p < 0.01) were predictive for LC while the tracking method was not (p = 0.97). For PFS and OS, multivariate analysis showed Karnofsky Index (p < 0.01) and tumor stage (p ≤ 0.02) to be significant factors for outcome prediction. Late radiation pneumonitis or chronic rip fractures grade 1–2 were observed in 5.3% of the patients. Grade ≥3 side effects did not occur.
Conclusion
Robotic SBRT is a safe and effective treatment for lung tumors. Reducing the PTV prescription and keeping high GTV mean doses allowed the reduction of toxicity while maintaining high local tumor control. The use of real-time motion compensation is strongly advised, however, well-performed ITV motion compensation may be used alternatively when direct tracking is not feasible.

These wounds have a heterogeneous etiology, including diabetes and surgical trauma wounds. It is
therefore important to decipher molecular signatures that reflect the macroscopic process of wound
healing. To this end, we collected wound sponge dressings routinely used in vacuum assisted therapy
after surgical trauma to generate wound-derived protein profiles via global mass spectrometry.
We confidently identified 311 proteins in exudates. Among them were expected targets belonging to
the immunoglobulin superfamily, complement, and skin-derived proteins, such as keratins. Next to
several S100 proteins, chaperones, heat shock proteins, and immune modulators, the exudates
presented a number of redox proteins as well as a discrete neutrophil proteomic signature, including
for example cathepsin G, elastase, myeloperoxidase, CD66c, and lipocalin 2. We mapped over 200
post-translational modifications (PTMs; cysteine/methionine oxidation, tyrosine nitration, cysteine
trioxidation) to the proteomic profile, for example, in peroxiredoxin 1. Investigating manually
collected exudates, we confirmed presence of neutrophils and their products, such as microparticles
and fragments containing myeloperoxidase and DNA. These data confirmed known and identified
less known wound proteins and their PTMs, which may serve as resource for future studies on
human wound healing
Zusammenfassung
Die Wundheilung stellt einen komplexen und sensiblen Prozess dar, wobei neben Keratinozyten auch besonders Immunzellen eine wesentliche Rolle spielen. Besonders in der Medizin ist die Behandlung von Wunden eine zentrale Aufgabe und erfordert immer mehr Techniken, um diesen Prozess effizienter ablaufen zu lassen. Seit einigen Jahren steht die Behandlung von Wunden mittels Niedertemperatur Plasma immer mehr im Fokus der Wissenschaft. Dabei besteht Plasma aus zahlreichen Komponenten, wobei jede Komponente die Zelle unterschiedlich beeinflussen kann. Da auch jede Wunde einzigartig im Hinblick auf Beschaffenheit und Erregerspektrum ist, sollte dies auch mit einer individuellen Abstimmung der Plasamkomponenten einhergehen um so die Wundheilung noch effizienter zu gestallten. Ziel dieser Arbeit ist die Modifizierung der Plasma-Komponenten durch einen Gasmantel, um herauszufinden, welche Modifizierung am effektivsten auf Immunzellen und somit auch auf die Wundheilung wirkt.Dafür wurde in der vorliegenden Arbeit die Auswirkungen von fünf Gaszumischungen im Mantelgas eines argonbetriebenen kalten Atmosphärendruckplasmajets (kINPen 11) auf die Generierung von reaktiven Spezies in Flüssigkeiten und auf Monozyten in vitro untersucht. Der kINPen (ein Gerät der Firma neoplas,zur Erzeugung von Niedertemperatur-Plasma) war dabei auf 5slm(Standardliter pro Minute Argon) und Burstmodus eingestellt. Die Gaszusammensetzung variiert von 100% Stickstoff (N2) und 0% Sauerstoff (O2) in 25íger Schritten zu 100% O2 und 0% N2. Dabei entstehen abhängig von der O2- und N2-Zufuhr im Gasmantel unterschiedliche reaktive Spezies. Als repräsentativ für die Bildung von reaktiven Sauerstoffspezies (ROS) wird Wasserstoffperoxid (H2O2) und für reaktiven Stickstoffspezies (RNS) werden Nitrit (NO2–) und Nitrat (NO3–) erfasst. Die Experimente wurden nach Flüssigkeitsanalytik und zellphysiologischen Aspekten gegliedert. Es wurde die THP-1-Monozytenzelllinie verwendet. Zu den Methoden der Flüssigkeitsanalytik gehören die Bestimmung des pH-Werts, die Detektion der NO2–-, NO3–- und H2O2-Konzentration. Zellphysiologisch wurde die Zellviabillität,die Apoptoserate,der Wachstumsfaktor HB-EGF,sowie die Zytokinsekretion detektiert. Zudem wurde die direkte und indirekte Plasma-Behandlung verglichen.Bei derdirekten Plasma-Behandlung stehen die Zellen mit dem Plasmaeffluenten im direkten, kontinuierlich Kontakt, wohingegen bei der indirekten Behandlung nur das Zellkulturmedium behandelt wird und anschließend auf die Zellen gelangt. Die Zellen haben keinen Kontakt mit dem Plasmaeffluenten. Zu der Flüssigkeitsanalytik gehört die gezielte Messung der durch das Plasma entstehenden ROS und RNS in phosphatgepufferte Salzlösung (PBS) und Zellkulturmedium (RPMI-Medium, Roswell Park Memorial Institute). Es konnten keine pH-Wert-Veränderungen durch die Plasma-Behandlung gemessen werden, aber mit Zunahme der Behandlungszeit stieg die Menge an reaktiven Spezies. Der Gasmantel hat besonders Auswirkungen auf den Gehalt der Flüssigkeiten an reaktiven Spezies. Die niedrigsten Nitrat-Anion (NO3–)- und Nitrit-Anion (NO2–)-Konzentrationen ergaben sich bei der 100% und 0%N2-Mantelgaszumischung. Die75%N2-Mantelgaszumischung zeigte die höchste NO3–-Konzentration in PBS. Die H2O2-Konzentration nahm mit dem Anteil an O2 im Mantelgas zu und erreicht ihr Maximum bei Mantelgaszumischung gezeigt werden. Alle anderen Mantelgaszumischungen zeigten zwar bei der Flüssigkeitsanalytik deutliche Unterschiede, doch durch die Anwesenheit von THP-1-Zellen wurde der Einfluss deutlich geringer. Die Gaszumischungen hatten trotz unterschiedlicher Bildung von reaktiven Stickstoff- und Sauerstoffspezies keinen großen Effekt auf die Zelltoxizität. Die 50%-N2-Mantelgaszumischung bewirkte die höchsten Konzentrationen an reaktive Sauerstoff- und Stickstoffspezies (RONS), was mit einer erhöhten frühen und späten Apoptose einherging und mit niedrigen IL-8-Werten. Die 100% N2-Mantelgaszumischung zeigte die größten Effekte auf die Zytokinsekretion. Die IL-6-Konzentration sank sowohl bei den direkt als auch indirekt mit Plasma behandelten und mit Lipopolysaccharid (LPS)-stimulierten Proben. Gleichzeitig wurde für die unstimulierte, indirekte Plasma-Behandlung ein deutlicher Anstieg der IL-8-Konzentration gemessen. Neben den erwähnten Ergebnissen konnten weitere zelluläre Effekte unabhängig von den Mantelgaszumischungen gemessen werden. Zum Einen zeigten sich deutliche Unterschiede zwischen der direkten und indirekten Plasma-Behandlung. Die indirekte Plasma-Behandlung,d.h.diePlasma-Behandlung von Zellkulturmedium und die anschließende Inkubation der Zellen, zeigte eine starke Erhöhung der IL-8-Konzentration, die mit der N2-Konzentration im Mantelgas anstieg. Auch für die IL-6- und IL-8-Konzentrationen nach LPS-Stimulation zeigten sich Unterschiede zwischen der direkten und indirekten Behandlung. Die indirekte Plasma-Behandlung zeigte eine stark reduzierte Zellviabilität gegenüber der direkten Plasma-Behandlung (1 min), die eher aktivierend wirkte. Zum Zweiten zeigte sich nach der Plasma-Behandlung, dass die Zellen im Durchflusszytometer eine Erhöhung des Volumens und der Granularität nach langen Behandlungszeiten aufwiesen und scheinbar erhöhte Zellviabilität. Eine Detektion des Wachstumsfaktors HB-EGF mit Hilfe der FACS-Analyse konnte nicht gezeigt werden. Drittens, konnte durch Zugabe von Katalase ein Anstieg der NO2–-Konzentration im RPMI im Vergleich zur Behandlung ohne Katalase gezeigt werden. Somit spiegelt die Flüssigkeitsanalytik nicht die Stituation in Gegenwart von Zellen in vitro wieder. Erstmalig konnte in dieser Arbeit gezeigt werden, dass die Modifizierung der Umgebungsgase bei der Plasma-Behandlung einen Einfluss auf die Bildung von ROS, RNS und Zytokinen hat. Besonders die 50% N2-Mantelgaszumischung hat mit der Reduktion der IL-8-Konzentration und damit der Reduktion eines für die Angionese relevanten Faktors möglicherweise einen negativen Effekt auf die Wundheilung. Im Gegensatz dazu bewirkt die 100% N2-Mantelgaszumischung durch Abnahme von IL-6 eine Verringerung der Entzündungsmediatoren und durch die Erhöhung von IL-8 eine Zunahme eines für die Angiogenese in Wunden wichtigen Parameters. Gleichzeitig konnte gezeigt werden, dass der Einfluss durch die Behandlungszeit und der direkten und indirekten Behandlung einen weitaus größeren Effekt auf Monozyten haben könnte als bisher angenommen. Somit leistet diese Arbeit einen zusätzlichen Beitrag für das weitere Verständnis bei der Aufklärung der zellulären Effekte durch die Plasma-Behandlung. Es kamen aber auch neue Fragen auf, die noch nicht hinreichend geklärt wurden: Erstens, welcher Mechanismus steht hinter der Zunahme der Granularität und des Volumens nach langen Plasma-Behandlungen. Zweitens, in wie weit sind die ohne Katalase-Behandlung gemessenen NO2–-Konzentrationen aussagekräftig. Drittens, haben die indirekte und direkte Plasma-Behandlung den gleichen Effekt in vivo wie in vitro. Um diese Fragen weiter zu klären ist weitere Forschung nötig.
Currently, plastic materials are an integral part of our lives, but their production mostly bases on fossil fuels or derivatives, which resources are decreasing. Extraction and processing of non-renewable resources have also negative impact on environment. One of the most promising and environmentally friendly approaches is use of microorganism. This PhD dissertation presents the non-conventional yeast Arxula adeninivorans as a host for production of bio-based and biodegradable poly(hydroxyalkanoates) plastics poly(hydroxybutyrate) and co-polymer poly(hydroxybutyrate-co-hydroxyvalerate). Additionally, the constructed yeast strain was able to secrete enantiomerically pure (R)-3-hydroxybutyric acid.
The production of PHAs requires three enzymes: β-ketothiolase, acetoacetyl-CoA reductase and PHA synthase. The strategy followed in this project was divided into two parts. While all three enzymes are responsible for intracellular production of PHA polymer, first two only lead to secretion of (R)-3-HB into culture media, which was used in a first stage of work to establish and optimize polymer production. Both, different bacterial strains and yeast A. adeninivorans were taken into account in screening of the genes encoding aforementioned enzymes. Bacterial genes were chemically synthesized using codon optimization pattern and endogenous genes were obtained using PCR and genomic DNA template from A. adeninivorans LS3 wild-type strain. Each gene was cloned into Xplor2 vector between TEF1 constitutive promoter and PHO5 terminator. Vector containing both thiolase and reductase genes was used for A. adeninivorans transformation.
The best combination of heterologous genes was overexpression of β-ketothiolase gene from Clostridium acetobutylicum and acetoacetyl-CoA reductase gene from Cupriavidus necator which led to secretion of 4.84 g L−1 (R)-3-HB, at a rate of 0.023 g L−1 h−1 over 214 h in shaking flask cultivation. Further optimization by fed-batch culturing with glucose as a carbon source did not improve (R)-3-HB secretion, but the rate of production was doubled to 0.043 g L−1 h−1 [3.78 g L−1 of (R)-3-HB at 89 h].
The product of acetoacetyl-CoA reductase is (R)-3-HB-CoA and further removing of CoA moiety is needed for acid secretion into culture media. A. adeninivorans is able to conduct this process without any additional modification but the conversion rate is unknown. Two thioesterases, cytosolic TesBp encoded by TesB gene from E. coli and mitochondrial ATes1p encoded by ATES1 gene from A. adeninivorans, were analysed to enhance secretion process. Additionally, a cytosolic version of ATES1 gene (ATES1cyt) was tested. All three genes were expressed in A. adeninivorans cells under TEF1 constitutive promoter together with thiolase and reductase genes. Despite detected enzymatic activity the yield of (R)-3-HB synthesis and secretion was not increased. Moreover, overexpressed thioesterases negatively influenced cell growth, indicating that they act on other metabolic components. The results provided two sets of information, first, the endogenous secretion system is sufficient for (R)-3-HB production; second, further screening of suitable genes needs to be performed.
Based on optimization of (R)-3-HB synthesis, thiolase gene (thl) from C. acetobutylicum and reductase gene (phaB) from C. necator were chosen to combine with PHA synthase gene (phaC) for creating the PHB-V producing strain. The PHA synthase expression module, containing TEF1 promoter and PHO5 terminator, was cloned into Xplor2 vector together with thiolase and reductase expression modules and used for A. adeninivorans transformation. The engineered strain accumulated up to 7.47% PHB of dcw. During the set of cells passaging A. adeninivorans lost the ability to accumulate polymer with maximal 23.1 % of primary accumulation level. Additionally, use of a vector including hygromycin B antibiotic resistance marker (instead of auxotrophic marker in Xplor2) did not improve polymer accumulation and stability.
To counteract the effect of loss of accumulation stability, phasin gene (phaP1), originated from C. necator, was introduce together with PHA pathway genes. First screening cultivations resulted in stabilizing of polymer production reaching 9.58 % PHB of dcw and only 12.0 % loss of production ability. Further experiments increased PHB content with 19.9% PHB of dcw (3.85 g L-1) after 180 h of cultivation using rich medium. Use of another thiolase gene, the second thiolase from C. necator (bktB), which theoretically should induce production of PHBV copolymer, led to accumulation only 11.4% PHB of dcw after 139 h and no PHV fraction was detected.
Variation of the ratio between flask volume and amount of media influences the level of aeration. Importantly, decrease of aeration level significantly increased polymer synthesis. Additionally, PHB-V copolymer accumulation has been induced by use of different carbon source co-substrates. Use of rich media supplemented with ethanol allow the strain with thl thiolase to accumulate up to 42.9 % PHB of dcw without PHV fraction and with bktB thiolase to 30.5 % PHB of dcw. Nevertheless, despite of lower total amount of polymer, supplementation with 1-propanol allow both strains to accumulate PHB-V copolymer with 7.30 %mol and 22.5 %mol of PHV for thl and bktB strains, respectively.
Optimization based on genetic engineering further enhanced polymer production yield led to exceeding of 50 % PHB-V of dcw. For doubling the gene dosage, PHA synthesizing strains of A. adeninivorans were again transformed with Xplor2 vector containing PHA pathway genes. Resulting strains exhibited twice the level of enzymatic activities of thiolase and reductase compared with strains transformed once with expression vector. In a shaking flask experiment the strain transformed twice with vector containing bktB thiolase reached after 240 h 52.1% PHB-V of dcw (10.8 g L-1) with 12.3 %mol of PHV fraction which is the highest level found in yeast. As another genetic approach, a fusion strain has been created. Two different strains have been established and merged using protoplast fusion technique. Doubling of genetic material resulted in similar level of copolymer produced by Arxula as in former experiments (50.2% of dcw, 10.7 g L-1).
Culture conditions were optimized in controllable cultivation using fed-batch mode. Although optimal oxygen and pH level and continuous carbon source and nitrogen feeding were maintained, final polymer level in % of dry mass was around three times lower than for shaking flask experiment. Nevertheless, efficient growth of Arxula in fed-batch mode led to increase of total copolymer level in g L-1 (16.5 g L-1 compare to 10.8 g L-1 for shaking flasks) showing the feasibility of using Arxula strain for up-scaling production of copolymer.
Acetyl-CoA is a main precursor in synthesis of PHB-V copolymer and change of its pool was investigated. ATP citrate lyase is a cytosolic enzyme converting citrate into oxaloacetate and acetyl-CoA, supporting the biosynthesis of fatty acids. Two genes encoding Acl subunits from Aspergillus nidulans (AnAcl1 and AnAcl2) were again cloned into Xplor2 vector and transformed into A. adeninivorans PHA producing strain. Despite of higher enzymatic activity of AnAclp, accumulation of polymer was around three times higher for control without expression of lyase genes. Expectedly, the strain expressing AnAcl1/2 genes accumulated larger amount of each stearic, palmitic and oleic acid in both standard and fatty acid inducing conditions (lower nitrogen level). Thus, overexpression of AnAcl1/2 genes in A. adeninevorans cells may improve biosynthesis of fatty acids but is ineffective for PHB polymer accumulation.
The aim of the project was use of starch-based media, manufactured as by-products, for polymer production. Genetically engineered Arxula strains were cultivated using these media instead of glucose-based media. Although yeast cells were both able to secrete (R)-3-HB and to accumulate PHB, the yield was lower than for previous media. Additionally, only trace of PHV was found at the end of cultivation time when 1-propanol was supplemented. Obtained results showed that use of cheaper media is a promising approach to decrease production costs but further optimization needs to be performed especially for extended scale of production.
Determination of produced copolymer has been done based on microscopic analysis and studies of physical and chemical properties. Results revealed that Arxula accumulated PHA polymer in cytosolic granules with a similar size range compared to the ones produced by bacteria. The physicochemical study showed that produced polymer exhibited slightly different properties in comparison to bacterial polymer with similar content of PHV, i.e. very-low molecular mass, higher melting and glass transition temperature.
All above results showed that A. adeninivorans is a promising host for PHB-V production. Expression of phasin greatly increased production and stability of polymer, which led to an accumulation level never found before in yeast. Further optimization in higher production scale using cheap starch-based media may establish Arxula strain as a valuable tool for industrial production of PHB-V copolymer.
Streptococcus pneumoniae (pneumococci) and Staphylococcus aureus (S. aureus) are human-specific commensals of the upper respiratory tract. Every individual is asymptomatically colonized with both bacteria at least once in their life-time. The opportunistic pathogens can affect further organs and invade into deeper tissue. The occupation of normally sterile niches of the human body with the bacteria can lead to local infections such as sinusitis, otitis media and abscesses, or to life-threatening diseases like pneumonia, meningitis or sepsis. A strong interaction between the bacterium and the respiratory epithelial cells is a prerequisite for a successful colonization. This interaction is ensured by bacterial surface proteins, so called adhesins. The binding of the adhesins to the epithelial lineage occurs predominantly indirectly via components of the extracellular matrix (ECM), but also directly to cellular receptors. Pneumococci and S. aureus bind to various ECM glycoproteins, amongst others: fibronectin, fibrinogen, vitronectin, and collagen. Also binding of both pathogens to human thrombospondin-1 has been described. Thrombospondin-1 is mainly stored in the α-granula of thrombocytes (platelets) and released into the circulation upon activation. However, thrombospondin-1 is also produced and secreted by other cell types like endothelial cells, macrophages, and fibroblasts, which gets subsequently incorporated as component into the ECM. So far, no thrombosponin-1-binding adhesins of pneumococci were identified. PspC, Hic, and PavB are important surface-localized virulence factors, which were shown to interact with human ECM and plasma proteins. PspC and Hic bind to vitronectin and factor H, which inhibits the complement cascade of the human immune system. PavB interacts with fibronectin and plasminogen, and a pavB-deficient mutant of S. pneumoniae showed diminished capacity in colonization in a mouse model. Among the surface proteins of S. aureus, only Eap was identified as thrombospondin-1-binding adhesin. Beyond colonization, pneumococci and S. aureus can enter the blood circulation, interact with platelets, and cause their activation. The aggregation of platelets, especially initiated by S. aureus, plays an important role in the clinic, because most of the septic patients develop thrombocytopenia. Surface localized factors of
S. pneumoniae triggering platelet activation are unknown to date. In contrast, few proteins of S. aureus with potential to activate platelets, including Eap, were identified previously.
This study identified the surface proteins PavB, PspC, and Hic of S. pneumoniae as specific ligands of the human thrombospondin-1. Flow cytometric, surface plasmon resonance spectroscopic and immunological analyses revealed interactions between the pneumococcal proteins and soluble as well as immobilized thrombospondin-1. The use of specific pneumococcal deletion mutants verified the importance of the three virulence factors as binding partners of soluble thrombospondin-1. The results suggest that pneumococci are capable of acquiring soluble thrombospondin-1 from blood as well as utilizing immobilized glycoprotein of the ECM as substrate for adhesion. Furthermore, the thrombospondin-1-binding domain within the pneumococcal proteins was analyzed by use of recombinant fragments of PavB, PspC, and Hic. The binding capacity of thrombospondin-1 increased proportionally with the amount of repetitive sequences in PavB and PspC, and the length of the α-helical region within the Hic molecule. The binding behavior of thrombospondin-1 towards PavB and PspC is comparable with that of the ECM proteins vitronectin and fibronectin, but is unique towards Hic.
The localization of the binding domain of the adhesins within the thrompospondin-1 molecule occurred via use of glycosaminoglycans as competitive inhibitors for the interaction. The results suggest that the pneumococcal proteins Hic and PspC target the identical binding region within thrombospondin-1, which differs from the binding domain for PavB. However, all three virulence factors seem to bind in the N-terminal part of thrombospondin-1.
Two-dimensional gel electrophoresis, thrombospondin-1 overlay assay and subsequent mass spectrometric analysis identified AtlA of S. aureus as a surface localized interaction partner of human thrombospondin-1. Moreover, a vitronectin binding activity for AtlA was determined. Immunological and surface plasmon resonance binding studies with recombinant AtlA fragments revealed that interactions with both matrix proteins is mediated via the C-terminal located repeats R1R2 of the AtlA amidase domain. Binding of thrombospondin-1 and vitronectin occurred not simultaneously, due to a competitive inhibition.
The second part of the study focused on the activation of human platelets by recombinant pneumococcal and staphylococcal proteins. In total, 28 proteins of S. pneumoniae and 52 proteins of S. aureus were incubated with human platelets. The activation of the cells was detected by flow cytometry using the activation markers P-selectin and the dimerization of the integrin αIIbβIII. The proteins CbpL, PsaA, PavA, and SP_0899 of S. pneumoniae induced platelet activation, however, the detailed mechanism has to be deciphered in further studies. Furthermore, the secreted proteins CHIPS, FLIPr, and AtlA of S. aureus were discovered as inductors for the activation of platelets. In addition, the domains of AtlA and Eap, crucial for platelet activation, were narrowed down. Interestingly, CHIPS, FLIPr, and Eap were described as inhibitors of neutrophil recruitment. Platelets are recently recognized as immune cells, due to the expression of immune receptors. The data obtained in this study highlight a comprehensive spectrum of effects of the S. aureus proteins towards different type of immune cells. Besides the activation of platelets in suspension buffer and plasma, the aggregation of platelets in whole blood was triggered by the proteins CHIPS, AtlA, and Eap. These results suggest a contribution of the proteins during the S. aureus-induced infectious endocarditis. Secretion of the platelet activating virulence factors, which were identified within this study, might represent a pathogenic strategy during S. aureus infection in which a direct contact between S. aureus and platelets is not required or even avoided.
In conclusion, PavB, PspC, and Hic of S. pneumoniae and AtlA of S. aureus were identified as interaction partners of human thrombospondin-1. Furthermore, CHIPS, FLIPr, AtlA, and Eap were characterized as platelet activators. This study provides candidates for the development of protein-based vaccines, to prevent bacterial colonization and to neutralize secreted pathogenic factors.
Der Einsatz von Glukose Point-Of-Care-Testung- (POCT) Geräten zur Diagnose von Diabetes mellitus (DM) ist derzeit nicht allgemein akzeptiert. In der Patientenversorgung werden bei einzelnen Patienten unterschiedliche Messsysteme mit verschiedenen prä-analytischen Prozessen regelhalft parallel angewandt. POCT-Glukose aus Kapillarblut wird oft zur Verlaufskontrolle, venöses Plasma im Zentrallabor eher zu diagnostischen Zwecken verwendet. Behandelnde Ärzte müssen dementsprechend Messwerte verschiedener Probetypen und Messsysteme einordnen können. In dieser Arbeit wurden Glukosemessungen in Kapillarblut sowie venösem Blut mittels POCT- und Zentrallaborgeräten durchgeführt mit dem Ziel, typische Szenarien der Patientenversorgung nachzubilden. Die resultierenden Diagnoseraten von gestörter Glukosetoleranz (IGT) und DM wurden bestimmt und verglichen. Dafür wurden zwei Kohorten wurden einem OGTT unterzogen. In den Proben wurde die P-Glukosekonzentration mit zwei POCT-Methoden untersucht: einem Handgerät mit Teststreifen sowie einem kleinen Tischgerät mit Batch-Reagenzien, das oft patientennah eingesetzt wird. Daneben wurden zwei Messsysteme im Zentrallabor eingesetzt. Es wurden Vollblutproben sowie Kapillarproben untersucht. Zusätzlich wurde glykolisiertes Hämoglobin (HbA1c) aus Vollblut bestimmt.
Insgesamt wurden 231 Probanden untersucht. Die Zweistunden-OGTT-Glukosekonzentrationen der Kapillarproben wiesen eine systematische positive Messabweichung von 0,8 mmol/L auf, verglichen mit den Messungen an Plasmaproben im Kernlabor. Daraus resultierte eine höhere Diagnoserate von IGT und DM wenn die Messergebnisse der Kapillarproben zugrunde gelegt werden. Die Messabweichung reduzierte sich auf 0,2 mmol/L, wenn an allen Messsystemen venöses Blut untersucht wurde. Ein Methodenwechsel im Kernlabor hingegen führte zu einer negativen systematischen Messabweichung von 0,5 mmol/L und damit verbunden zu geringeren Diagnoseraten.
Venöse Proben, die patientennah gemessen werden, sind am besten geeignet zur Diagnose von DM im Rahmen eines OGTT. Die untersuchte Teststreifen-basierte POCT-Methode erscheint geeignet zur Diagnose von IGT und DM, wenn venöse Proben eingesetzt werden. Wechsel von Kernlabormethoden können systematische Messunterschiede in der P-Glukose-Messung verursachen, die die Diagnoserate von IGT und DM beeinflussen. Dies unterstreicht die Notwendigkeit zur Engerfassung der Kriterien für die externe Qualitätssicherung.
Myxomycetes are protists belonging to the super-group Amoebozoa. The traditional taxonomic system, which is now largely outdated by molecular studies, recognizes five orders: Liceales, Trichiales, Physarales, Stemonitales and Echinosteliales. Molecular phylogenies revealed two basal clades: Physarales and Stemonitales (the so-called dark-spored myxomycetes) are the first; the other above-mentioned orders form the second (the bright-spored myxomycetes). However, except for Echinosteliales none of the traditional orders appears to be monophyletic in the traditionally used delimitation. The dark-spored myxomycetes encompass the majority of the described morphospecies. Due to the high genetic divergence in DNA sequences between the bright- and dark-spored myxomycetes, only the latter are considered in this dissertation. Historically myxomycetes have been described as fungi, due to their macroscopically visible fructifications which, though considerably smaller, resemble those of fungi. These fruit bodies provide enough morphological traits to support a morphological species concept with currently ca. 1000 species described. Therefore diversity studies of myxomycetes have been conducted for over 200 years and a substantial body of data on ecology and distribution of these fructifications exist. From these studies myxomycetes are known to form often distinct communities across terrestrial ecosystems with highly specific habitat requirements, such as snowbanks (nivicolous), herbivore dung (coprophilous) or decaying wood (xylophilous). However knowledge on the myxamoebae – the trophic life stage of the myxomycetes – is very scarce. Only recent advances in molecular techniques such as direct species identification based on DNA sequences from environmental samples (ePCR), have made studies of myxamoebae (and other microbes) possible. From these first molecular based studies myxomycetes are currently estimated to account for between 5 to almost 50% of all soil amoebae, and have been shown to be present in a wide variety of soils. To fully take advantage of these new methods, a molecular DNA marker needs to be established as well as a reference sequence database. The usability of a DNA marker gene depends on its ability to separate species by a distinction between intra- and interspecific divergence between sequences of the same and related species, the so-called ‘barcoding gap’.
The first part of this thesis (article I and II) deals with the subject of establishing such a DNA marker and database, and in doing so touches upon the subject of ‘what is a myxomycetes species?’
A total of 1 200 specimens were compiled into a reference database (the largest database to date of dark-spored myxomycetes). The genetic distance from sequence-to-sequence was used to assess genetic clade structures within morphospecies and putative biospecies (sexually isolated linages) were identified. The result was an estimate of hidden diversity, exceeding that of described morphospecies by 99%. The optimum sequence similarity threshold for OTU-picking (genetic species differentiation, denoted Operational Taxonomic Unit) with the used SSU marker was identified as 99.1% similarity.
The second part of this thesis (article III and IV) presents ecological studies conducted with NGS (ePCR) in which the established threshold and database are applied and are demonstrated to provide reliable and novel insights into the soil myxamoebae community. It is investigated whether the occurrence of fruit bodies reflects the distribution of soil myxamoebae, and the research questions ‘do myxomycetes show broader realized niches as soil amoebae than as fructifications?’ and ‘are myxamoebae distributions correlated to potential prey organisms (fungi and bacteria)?’ are investigated.
In the ecological study presented in article III parallel metabarcoding of bacteria, fungi and dark-spored myxomycete was used for the first time in a joint approach to analyze the communities from an elevational transect in the northern limestone German Alps (48 soil samples). Illumina sequencing of the soil samples revealed 1.68 Mio sequences of a section of the rRNA gene, which were assigned to 578 operational taxonomic units (OTU) from myxomycetes. These show a high similarity (>98%) to 42 different morphospecies (the respective figures for bacteria and fungi were 2.16/5710/215 and 3.68/6133/260, respectively). Multivariate analyses were carried out to disentangle microbial interplay and to identify the main environmental parameters determining the distribution of myxamoebae and thus setting the boundaries for their ecological niches. Potential interactions between the three target organisms were analysed by integrating community composition and phylogenetic diversity with environmental parameters. We identified niche differentiation for all three communities (bacteria, fungi and myxamoebae) which was strongly driven by the vegetation. Bacteria and fungi displayed similar community responses, driven by symbiont species and plant substrate quality. Myxamoebae showed a more patchy distribution, though still clearly stratified among genera, which seemed to be a response to both structural properties of the habitat and specific bacterial taxa. In addition we find an altitudinal species turn-over for all three communities, most likely explained by adaptation to harsh environmental conditions. Finally a high number of myxomycetes OTUs (associated with the genus Lamproderma) not currently represented in our reference database were found, representing potentially novel species. This study is the first to report niche differentiation between the guild of nivicolous (“snowbank”) myxomycetes and thus fine-scale niche differentiation among a predatory soil protist; identifying both potential food preferences and antagonistic interactions with specific bacterial taxa.
Finally, the second ecological study (article IV) focuses on comparing the distribution of myxamoebae revealed by ePCR of soil samples with fructifications collected from the same area (714 specimens determined to 30 morphospecies, which form 70 unique ribotypes that can be assigned to 45 ribotype clusters using a 99.1% similarity threshold). The study found a strong coherency between the two inventories, though with species specific relative differences in abundance, which can in part be attributed to the visibility of the fructifications. In addition, a year to year comparison of fructification records gives support to the hypothesis that the abundance of fructifications depends strongly on the onset of snowfall in the previous autumn and the soil temperature regime throughout the winter.
Die krankhafte Fettleibigkeit (Adipositas) wird in weiten Teilen der Welt zunehmend zum bestimmenden Gesundheitsproblem. Die Datenerhebungen der Weltgesundheits-organisation (WHO) sowie der Organisation zur wirtschaftlichen Zusammenarbeit und Entwicklung (OECD) zeigen einen deutlichen Anstieg der Adipositasprävalenz über die letzten Jahrzehnte. In vielen OECD Ländern gilt heute über die Hälfte der Bevölkerung als übergewichtig oder adipös (WHO: Website der WHO, zuletzt geprüft am 02.09.2017; OECD: Fettleibigkeit und Übergewicht nehmen in den OECD-Ländern weiter zu, zuletzt geprüft am 02.09.2017). Dies wird zur immer größeren Belastung für das Gesundheitssystem, da Adipositas mit vielen Sekundärkrankheiten wie Herz-Kreislauferkrankungen, Bluthochdruck und bestimmten Krebsarten assoziiert wird (Bray 2004; Després et al. 2001; Malnick und Knobler 2006). Für das Jahr 2003 wurde für das deutsche Gesundheitssystem dadurch ein finanzieller Aufwand in Höhe von 11 Milliarden Euro für die Behandlung von Adipositas oder durch Adipositas verursachte Komorbiditäten veranschlagt (Knoll 2010). Hinzu kommen „emotionale Kosten“ der Betroffenen, die unter sozialer Ausgrenzung und Stigmatisierung leiden (Latner und Stunkard 2003; Neumark-Sztainer et al. 1998; Sobal et al. 1995; Brewis et al. 2011; Brewis 2014).
Neben diesen klar Adipositas-assoziierten gesundheitlichen Beeinträchtigungen hat es immer wieder Untersuchungen zu einem möglichen Zusammenhang von Adipositas und Kognition gegeben. Dieser wurde in vielen Studien am Menschen untersucht und die bisherigen Ergebnisse sehr anschaulich von Anna Dahl und Linda Hassing 2013 beziehungsweise Christina Prickett und Kollegen 2015 analysiert (Dahl und Hassing 2013; Prickett et al. 2015). Diese Übersichtsarbeiten zeigen auf, dass es durchaus Belege für einen Zusammenhang von Adipositas und Kognition gibt, allerdings ist die Datenlage zu diesem Thema durchaus ambivalent.
In dieser Arbeit sollte deshalb der Einfluss von Adipositas auf die Kognition mithilfe eines etablierten Mausmodells für Adipositas untersucht werden. Zu diesem Zweck wurden adulte, vier bis sechs Monate alte, Leptin-defiziente Mäuse (ob) und deren Wildtypkontrollen (wt) vergleichend untersucht. Unsere Daten zeigen, dass Adipositas im Mausmodell nicht mit einer kognitiven Beeinträchtigung einher geht. Sowohl im Verhaltensexperiment (hippocampusabhängiges Lernen, Morris water maze) als auch auf zellulärer Ebene in der Verbindungsdichte der Nervenzellen untereinander (Dichte dendritischer Dornen) zeigten sich zwischen Leptin-defizienten und Wildtyptieren keine signifikanten Unterschiede.
Allerdings wiesen Leptin-defiziente Tiere ein kleineres Hirnvolumen als Wildtypkontrolltiere auf, ein Ergebnis, das mit anderen Publikationen übereinstimmt (Ahima et al. 1999; Steppan und Swick 1999). Detaillierte Analysen der Volumenverhältnisse im Gehirn von Leptin-defizienten und Wildtypmäusen in dieser Arbeit ergaben, dass sich die relativen Größenverhältnisse im Gehirn von ob‑Tieren zugunsten des Hippocampus verschieben. Diese Ergebnisse widersprechen damit Befunden in adipösen Menschen, die kleinere Hippocampusvolumina aufwiesen (Isaac et al. 2011).
Die adulte hippocampale Neurogenese selbst, also die Bildung neuer, funktionaler Neuronen im adulten Gehirn, war im Gyrus dentatus von Leptin-defizienten Mäusen signifikant vermindert. Zusammen mit den Analysen von Proliferation und Apoptose von Hirnzellen im Gyrus dentatus, konnte diese eingeschränkt Neurogenese auf eine geringere Proliferation neuronaler Vorläuferzellen zurückgeführt werden. Die Überlebens-wahrscheinlichkeit schien dabei nicht beeinflusst, da keine erhöhte Apoptose im Gyrus dentatus ermittelt werden konnte.
Die hier durchgeführten Experimente konnten keine direkte, negative Auswirkung von Adipositas auf Kognition im Mausmodell belegen. Wenngleich ein Einfluss auf Aspekte der neuronalen Plastizität durch eine verminderte adulte Neurogenese sowie das Gehirngesamtvolumen bestätigt werden konnte, waren Veränderungen des Verhaltens der ob‑Tiere unter Berücksichtigung ihrer motorischen Defizite nicht nachweisbar.
One of the major problems in the study of the dynamics of proteins is the visualization of changing conformations that are important for processes ranging from enzyme catalysis to signaling. A protein exhibiting conformational dynamics is the soluble blood protein beta 2-glycoprotein I (beta2GPI), which exists in two conformations: the closed (circular) form and the open (linear) form. It is hypothesized that an increased proportion of the open conformation leads to the autoimmune disease antiphospholipid syndrome (APS). A characteristic feature of beta2GPI is the high content of lysine residues. However, the potential role of lysine in the conformational dynamics of beta2GPI has been poorly investigated. Here, we report on a strategy to permanently open up the closed protein conformation by chemical acetylation of lysine residues using acetic acid N-hydroxysuccinimide ester (NHS-Ac). Specific and complete acetylation was demonstrated by the quantification of primary amino groups with fluoraldehyde o-phthalaldehyde (OPA) reagent, as well as western blot analysis with an anti-acetylated lysine antibody. Our results demonstrate that acetylated beta2GPI preserves its secondary and tertiary structures, as shown by circular dichroism spectroscopy. We found that after lysine acetylation, the majority of proteins are in the open conformation as revealed by atomic force microscopy high-resolution images. Using this strategy, we proved that the electrostatic interaction of lysine residues plays a major role in stabilizing the beta2GPI closed conformation, as confirmed by lysine charge distribution calculations. We foresee that our approach will be applied to other lysine-rich proteins (e.g. histones) undergoing conformational transitions. For instance, conformational dynamics can be triggered by environmental conditions (e.g. pH, ion concentration, post-translational modifications, and binding of ligands). Therefore, our study may be relevant for investigating the equilibrium of protein conformations causing diseases.
For the last two decades, heparins have been widely used as anticoagulants. Besides
numerous advantages, up to 5% patients with heparin administration suffer from a major adverse
drug effect known as heparin-induced thrombocytopenia (HIT). This typical HIT can result in deep
vein thrombosis, pulmonary embolism, occlusion of a limb artery, acute myocardial infarct, stroke, and
a systemic reaction or skin necrosis. The basis of HIT may lead to clinical insights. Recent studies using
single-molecule force spectroscopy (SMFS)-based atomic force microscopy revealed detailed binding
mechanisms of the interactions between platelet factor 4 (PF4) and heparins of different lengths in
typical HIT. Especially, SMFS results allowed identifying a new mechanism of the autoimmune HIT
caused by a subset of human-derived antibodies in patients without heparin exposure. The findings
proved that not only heparin but also a subset of antibodies induce thrombocytopenia. In this review,
the role of SMFS in unraveling a major adverse drug effect and insights into molecular mechanisms
inducing thrombocytopenia by both heparins and antibodies will be discussed.
β-Phenylalanine Ester Synthesis from Stable β-Keto Ester Substrate Using Engineered ω-Transaminases

Hintergrund: Patientennahe und klinische Erhebungsverfahren zur Dokumentation des Therapieverlaufs und des Behandlungserfolgs in der Ambulanten Muskuloskeletalen Rehabilitation (AMR) wurden bisher nur selten im direkten Vergleich hinsichtlich ihrer methodischen Eigenschaften, speziell der Änderungssensitivität untersucht. Für die angemessene Beurteilung medizinischer Behandlungsergebnisse sind begründete Empfehlungen für die Auswahl und Beurteilung geeigneter Outcomeparameter erforderlich.
Ziel: Die methodischen Eigenschaften verschiedener klinischer Messverfahren (Bestimmung der Gelenkbeweglichkeit und des Fingerbodenabstands mit der Neutral-Null-Methode, manuelle Muskelkraftmessung, Gehstreckenbestimmung mit dem Laufband, Staffelsteinscore Version Hüfte und Knie, Harris Hip Score und Knee Society Score) werden ermittelt und verglichen. Es wird untersucht, ob und wie gut diese Erhebungsmethoden mit generischen (SF-36) und spezifischen (WOMAC® bzw. Funktionsfragebogen Hannover-Rückenschmerz) patientennahen Erhebungsinstrumenten korrespondieren. Neben den klassischen Haupt- und Nebentestgütekriterien und den Verteilungseigenschaften wird die Änderungssensitivität bestimmt, um zu zeigen, wie gut die Instrumente in der Lage sind, Veränderungen über die Zeit abzubilden.
Methoden: In die prospektive unkontrollierte Längsschnittstudie mit indirekter Ergebnismessung im Vorher-Nachher-Design werden alle Patienten konsekutiv eingeschlossen, welche vom 1.1.2013 bis zum 1.5.2014 eine durchschnittlich dreiwöchige Ambulante Muskuloskeletale Rehabilitation nach Hüft- oder Knie-TEP-Implantation sowie nach operativer oder konservativer Behandlung der Claudicatio spinalis im Zentrum für Rehabilitation und Physikalische Medizin am SRH Zentralklinikum Suhl absolviert haben. Bei Aufnahme (T1) und bei Entlassung (T2) werden die o.g. klinischen Erhebungsverfahren, anamnestische Daten und bei den Patienten mit Claudicatio spinalis der Funktionsfragebogen Hannover-Rückenschmerz, bei den Endoprothese-Patienten der WOMAC® und bei allen Patienten der SF-36 bestimmt. Die Datenerhebung für WOMAC® und SF-36 erfolgt per Selbstauskunft, mit Hilfe oder als Interview. Die Reliabilität der patientennahen Instrumente wird für T1 mit Cronbach's alpha ermittelt. Für die Änderungssensitivität werden Effektstärken und Standardisierte Mittelwertdifferenzen berechnet und miteinander verglichen. Mit der Pearson-Korrelation wird gemessen, ob und wie gut die Ergebnisse der verschiedenen klinischen Befunde untereinander und mit den patientennahen Erhebungsverfahren korrelieren.
Ergebnisse: Die Untersuchung umfasst 175 Patienten (40 nach Hüft- und 60 nach Knie-TEP-Implantation sowie 75 mit Claudicatio spinalis, davon 24 nach einer Operation). Im Verlauf der Ambulanten Muskuloskeletalen Rehabilitation zeigen sich unabhängig von Alter, Geschlecht und Bildungsstand positive Veränderungen. Die heterogene Datenerhebung mittels schriftlicher Befragung bzw. durch ein persönliches Interview für SF-36 und WOMAC® hat keinen Einfluss auf das Untersuchungsergebnis. Wegen der unterschiedlichen Ergebnisse für die beiden Indikatoren zur Bestimmung der Änderungssensitivität (Effektstärke und Standardisierte Mittelwertdifferenz) ist es empfehlenswert, in Studien grundsätzlich beide Kennziffern zu berechnen und zu vergleichen. Die höchste Änderungssensitivität aller Instrumente erzielt der Staffelsteinscore in beiden Versionen (Knie stärker als Hüfte). Die Änderungssensitivität der anderen klinischen Erhebungsverfahren und des Funktionsfragebogen Hannover-Rückenschmerz fällt schwächer aus, ist aber der Änderungssensitivität der körperlichen Domänen des SF-36 überlegen, die wiederum eine höhere Änderungssensitivität aufweisen als die WOMAC® Subscores. Am niedrigsten ist die Änderungssensitivität für die Psychischen Domänen des SF-36. Es gibt keine relevanten Antwortausfälle. Nennenswerte Boden- und Deckeneffekte zeigen testimmanent nur der WOMAC® und für die Items "Körperliche Rollenfunktion", "Emotionale Rollenfunktion" und "Soziale Funktionsfähigkeit" der SF-36. Der Funktionsfragebogen Hannover-Rückenschmerz, der WOMAC® und mit Einbußen der SF-36 für die Items "Schmerz" und "Allgemeine Gesundheitswahrnehmung" sind zum Zeitpunkt T1 hoch reliabel (Cronbach's alpha). Mit der Pearson-Korrelation wird außer für die Hüft-TEP-Patienten in T1 die Konstruktvalidität für den SF-36 und den WOMAC® bestätigt. Als mögliche Fehlerursache kann nur die geringe Anzahl von 40 Hüft-TEP-Patienten identifiziert werden. Staffelsteinscore, Harris Hip Score und Knee Society Score korrelieren miteinander hochsignifikant. Der Staffelsteinscore ist in beiden Versionen bezüglich der Nebentestgütekriterien und der Korrelation mit dem SF-36 bzw. dem WOMAC® dem Harris Hip Score und dem Knee Society Score insgesamt leicht überlegen. Bei Patienten mit Claudicatio spinalis korrelieren Gehstrecke, Funktionsfragebogen Hannover-Rückenschmerz und mit Abstrichen für T1 der Fingerbodenabstand hochsignifikant miteinander und auch akzeptabel mit dem SF-36. Die Gehstrecke der Totalendoprothese-Patienten korreliert eher weniger gut mit allen Kenngrößen.
Diskussion: Im Ergebnis der Studie kann zur Evaluation und Dokumentation der Prozess- und Ergebnisqualität in der Ambulanten Muskuloskeletalen Rehabilitation für Endoprothese-Patienten die Verwendung des Staffelsteinscores in beiden Versionen und für Patienten mit Claudicatio spinalis die Bestimmung des Funktionsfragebogen Hannover-Rückenschmerz, der Gehstrecke und des Fingerbodenabstandes empfohlen werden. Dabei sind die differierenden Ergebnisse der Hüft-TEP-Patienten zum Zeitpunkt T1 und die heterogene Form der Datenerhebung für SF-36 und WOMAC® zu berücksichtigen. Die Kombination des Staffelsteinscores mit dem Harris Hip Score für die Hüft-TEP- und mit dem Knee Society Score für die Knie-TEP-Patienten bringt keinen Vorteil. Der Einsatz des SF-36 und des WOMAC® in der Routine ist nicht sinnvoll, genau wie das Messen der Gehstrecke mit dem Laufband bei den Endoprothese-Patienten. Weitere Studien an größeren Patientengruppen, im Einrichtungsvergleich, mit praktikableren patientenberichteten Endpunkten für die Endoprothese-Patienten und mit spezifischeren patientennahen Erhebungsverfahren für die Patienten mit Claudicatio spinalis sind genauso notwendig wie Untersuchungen mit anderen Assessmentinstrumenten der Mobilität sowie zur Bestimmung der Gehstrecke in anderer Art und Weise.
GH und sein Hauptmediator IGF-I spielen eine wichtige Rolle im Knochenstoffwechsel. Der Zusammenhang zwischen IGF-I und verminderter BMD bei Osteoporose wurde bereits vielfach untersucht. Insbesondere in Bezug auf eine mögliche geschlechtsspezifische Assoziation wurden allerdings widersprüchliche Ergebnisse gefunden. Zudem gibt es wenige Daten zu QUS-Messungen zur Bestimmung der Knochenqualität und zum IGFBP-3, welches die Bioverfügbarkeit von IGF-I reguliert. Das Ziel der vorliegenden Arbeit war daher, diese Zusammenhänge in der Allgemeinbevölkerung im Nordosten Deutschlands zu untersuchten. Grundlage für die Analysen der vorliegenden Studie bildeten die Daten von 3.543 Teilnehmern (1.759 Männer und 1.784 Frauen im Alter von 20-79 Jahren) von SHIP-Trend, einer bevölkerungsbasierten Studie in der Region Vorpommern. Von allen diesen Probanden lagen Serum IGF-I und IGFBP-3 Werte ebenso wie Messwerte einer Quantitativen Ultraschallmessung am Calcaneus vor. Geschlechtsspezifische lineare und logistische Regressionsanalysen, adjustiert für Alter, Taillenumfang, Sport, Diabetes mellitus, hsCRP und Menopausenstatus (nur bei Frauen) wurden berechnet. Bei Männern wurden positive lineare Assoziationen zwischen IGF-I, der IGF-I/IGFBP-3 Ratio und den QUS Parametern SOS, BUA und dem Steifigkeitsindex beobachtet, während bei Frauen keine klaren, statistisch signifikanten Assoziationen gezeigt werden konnten. Weiterhin wurde in der logistischen Regressionsanalyse eine inverse Assoziation zwischen den IGF-I-Spiegeln und dem Risiko für eine osteoporotische Fraktur bei beiden Geschlechtern gefunden. Bei Männern konnte nach dem Abfall des IGF-I-Spiegels um eine SD eine um 18% erhöhte Wahrscheinlichkeit für ein mittleres Frakturrisiko, sowie eine um 37% höhere Wahrscheinlichkeit für ein hohes (gegenüber einem niedrigen) Frakturrisiko festgestellt werden. Bei Frauen stieg beim Absinken des IGF-I Spiegels um eine SD die Wahrscheinlichkeit für ein hohes (gegenüber einem niedrigen) osteoporotisches Frakturrisiko um 45%. Für das IGFBP-3 konnten keine Assoziationen mit den QUS Parametern oder dem Risiko für eine osteoporotische Fraktur bei beiden Geschlechtern gezeigt werden.
Insbesondere bei Männer bleibt Osteoporose oft unentdeckt. Die hier aufgezeigten Assoziationen von IGF-I mit dem Steifigkeitsindex und dem QUS-basierten Frakturrisiko könnten helfen, die Anzahl der nicht diagnostizierten Patienten mit Osteoporose, besonders bei Männern, zu senken.
Charakterisierung und Behandlung von Patienten mit neuroendokrinen Tumoren (NeT) in Vorpommern

Ziel der Arbeit war es, einerseits Daten zu spezifischen Charakteristika und zur Therapie bei Patienten mit neuroendokrinen Tumoren (NeT) in Vorpommern zu erstellen und Hinweise zur Verbesserung zu geben und diese andererseits mit den 2006 und 2008 erschienenen, 2012 aktualisierten Leitlinien der ENETS zu vergleichen sowie, wo möglich, Hinweise zur Verbesserung der Versorgung zu geben.
Hierbei fiel auf, dass sich die Inzidenzen in etwa mit denen der ENETS decken, die Tumoren aber teils in einem späteren Stadium gefunden wurden. Wenn internationale Inzidenzen abwichen, wie z.B. bei kleinen Rektum-NeT, muss diskutiert werden, ob eventuell eine geringere Annahme von Vorsorgekoloskopien und anderen präventiven Untersuchungen bzw. ein späterer Arztbesuch von Seiten des Patienten in Vorpommern eine Ursache sein könnte. Im internationalen Vergleich spielt zum Beispiel bei Rektum-NeT auch die Ethnie eine Rolle. Im deutschen NeT-Register erscheinen Appendix-, Rektum-NeT ebenfalls unterrepräsentiert, jedoch auch aufgrund des zentrenbetonten Patientenkollektivs.
Die fortgeschrittenen Tumorstadien des Jejunums/Ileums und des Kolons können möglicherweise auf erst spät auftretende oder unspezifische Symptome zurückzuführen sein. Beim Jejunum/Ileum ist auch abhängig von der Lage des Tumors die diagnostische Erreichbarkeit eingeschränkt.
Wenn möglich kann auch im Rahmen von CUP-Syndromen eine Kapselendoskopie erwogen werden, um NeT des Jejunums und Ileums zu detektieren. Eine SRS kann ebenfalls oft erfolgreich durchgeführt werden, wobei hier als Tracer bei Verfügbarkeit eher HTP oder DOPA anstatt dem sonst üblichen FDG empfohlen werden.
Klinischerseits sollte auf vollständige Anamnesen, vor allem bezüglich der Flush-Symptomatik, geachtet werden, hierzu fanden sich teilweise nur ungenaue Angaben. Außerdem sollte beachtet werden, dass falsch positive Werte von CgA und 5-HIES z.B. durch Vorerkrankungen, Medikamente oder Nahrungsmittel verursacht werden können.
Bei Gastroskopien ist es wichtig, auch ein bioptisches Mapping makroskopisch unauffälliger Schleimhaut des Fundus, Korpus und Antrum durchzuführen, damit die histologische Diagnose einer chronisch atrophischen Gastritis gestellt werden kann bzw. der Ausschluss anderer Erkrankungen möglich ist.
Im Vergleich mit den ENETS-Leitlinien fiel auf, dass die Therapiearten und -ziele oft aufgrund der Seltenheit der Erkrankung auch in Kombination mit schweren Vorerkrankungen individuell abgestimmt werden mussten. Weiterhin wurde der Vergleich durch öfters unvollständige Datenlage z.B. aufgrund eines Wegzugs erschwert. Zudem wurden Daten ab 1999 betrachtet, wohingegen Teile der Leitlinien der ENETS frühestens ab 2006 zur Verfügung standen.
Zur prophylaktischen Cholezystektomie unter Sandostatintherapie finden sich gemischte Aussagen, sodass auch hier individuell auf Grundlage der Art und Schwere der Vorerkrankungen (als Risikofaktor Cholezystolithiasis, Zustand nach Cholezystitis) entschieden werden sollte.
Weiterhin kann bei einem Becherzellkarzinoid der Appendix vermiformis bei Frauen eine zusätzliche Entfernung der Adnexe aufgrund eines möglicherweise gehäuften Auftretens von Metastasen erwogen werden.
Auf Seiten der Pathologie kann eine Angabe der Eindringtiefe vor allem von NeT der Appendix vermiformis eventuell hilfreich sein, da ab einer Invasion des Mesenteriolums von über 3 mm möglicherweise mit einem aggressiveren Krankheitsverlauf und einem erhöhten Risiko für Lymphknotenmetastasen zu rechnen ist. Zusätzlich sollten neben der Standardimmunhistochemie mit CgA, SP und Ki67 Grading und TNM-Klassifizierung einheitlich durchgeführt werden. Zum Grading ist gerade bei niedrigem Proliferationsindex bei Verfügbarkeit auch eine zugelassene digitale Auswertung empfehlenswert. Eine Grenzwertdefinition bei einem Ki67-Index zwischen 2 und 3 wäre dann allerdings wünschenswert.
Medikamentös sind sowohl Sandostatin als auch Interferon eher zur Symptomkontrolle als zur Progresshemmung geeignet, wobei Interferon häufiger gegeben werden muss und verstärkt Nebenwirkungen aufweist, sodass Sandostatin als erste Wahl in Betracht gezogen werden sollte.
Bei multiplen Tumoren des Endokriniums bzw. gehäuftem Auftreten innerhalb von Familien empfiehlt sich eine genetische Beratung und gegebenenfalls eine Mutationsanalyse, um MEN-Syndrome oder auch das von Hippel-Lindau-Syndrom diagnostizieren zu können.
Zweitmalignome werden in Studien mit einer Häufigkeit von 17-55 % angegeben. Bei uns betrug die Häufigkeit 24 %. Auch bei großer Schwankungsbreite der Daten besteht dennoch ein etwa doppelt so häufiges Auftreten von syn- oder metachronen Malignomen bei NeT-Patienten im Vergleich zu Patienten mit nicht endokrinen Malignomen, sodass möglicherweise eine lebenslange regelmäßige Kontrolle vor allem des Gastrointestinal- und Urogenitaltraktes erwogen werden sollte, da besonders in diesen Bereichen auch bei uns eine Häufung der Zweitkarzinome vorlag. Andererseits könnte es auch bedeuten, dass NeT bei synchronen Karzinomen aufgrund besserer Diagnostik in zunehmender Zahl gefunden werden. Als möglicher Erklärungsansatz kann die Feldeffekt-Theorie herangezogen werden, die aufgrund gemeinsamer karzinogener Effekte das gleichzeitige Wachstum neuroendokriner und epithelialer Zellen stimuliert. Eine andere Theorie beruht auf einem gemeinsamen genetischen Hintergrund, ohne dass bisher eine verursachende somatische Mutation gefunden werden konnte. Hier bleiben weitere Untersuchungen abzuwarten.
Die Atherosklerose ist eine weit verbreitete, mit dem Alter zunehmende Erkrankung der Gefäße, die schwerwiegende Folgeerkrankungen auslösen kann. Zu den Hauptrisikofaktoren gehören Störungen im Lipidstoffwechsel. Insbesondere erhöhte Cholesterin- und Triglyceridspiegel im Blut fördern die Entstehung von atherosklerotischen Läsionen in der Gefäßwand.
Protease-aktivierte Rezeptoren (PAR) spielen in vielen verschiedenen entzündlichen Prozessen unseres Körpers eine große Rolle, dabei ist der PAR-2 aktuell weniger gut erforscht. Der PAR-2 wird durch die Serinprotease Faktor Xa aktiviert und vermittelt proinflammatorische Antworten. Eine wesentliche Rolle des PAR-2 im Lipidstoffwechsel ist derzeit nicht bekannt. Aus der Arbeit von Frau Dr. Flößer war eine Zunahme des Körpergewichts, sowie erhöhte Triglycerid- und Cholesterinspiegel, bedingt durch den Knockout des PAR-2 und des ApoE-Gens bekannt. Dadurch wurde ein Einfluss des PAR-2 auf den Lipidstoffwechsel vermutet.
Das LIGHT-Protein wurde sowohl im Zusammenhang mit atherosklerotischen Läsionen, als auch im Rahmen des Lipidstoffwechsels beschrieben. Ebenso ist eine Interaktion des LIGHT-Proteins mit dem PAR-2 bekannt. Demnach ergab sich die Vermutung, dass der Einfluss des PAR-2 auf den Lipidstoffwechsel über das LIGHT-Protein erfolgt.
Die Analyse der Expression der mRNA in vier verschiedenen Mauslinien ergab eine signifikante Reduktion der Expression der mRNA des LIGHT-Proteins und der Expression der mRNA seiner beiden Rezeptoren HVEM und LTβ-Rezeptor. Da ebenfalls bereits ein Einfluss des LIGHT-Proteins auf die Expression der hepatischen Lipase bekannt war und die hepatische Lipase eine wesentliche Rolle im Lipidstoffwechsel spielt, wurde auch die mRNA-Expression der hepatischen Lipase bestimmt. Es konnte eine erniedrigte Expression der mRNA der hepatischen Lipase festgestellt werden, was die erhöhten Triglycerid- und Cholesterinspiegel begründet. Gegensprüchlich in dieser Arbeit war jedoch, dass das LIGHT-Protein in anderen Arbeiten als negativer Regulator der hepatischen Lipase beschrieben wurde und somit eine verminderte Expression von LIGHT eher zu einer vermehrten Expression der hepatischen Lipase führen würde. Da aber ebenfalls beide Rezeptoren des LIGHT-Proteins vermindert exprimiert wurden und die Interaktion mit der hepatischen Lipase über den LTβ-Rezeptor erfolgt, wird davon ausgegangen, dass die LIGHT-LTβ-Rezeptor Interaktion durch den Knockout des PAR-2 weniger beeinflusst wird und dadurch weiterhin aktiv stattfand.
Zusammenfassend kann festgehalten werden, dass der PAR-2 eine Rolle im Lipidstoffwechsel spielt und zu einer verminderten Expression des LIGHT-Proteins und seiner Rezeptoren führt.
Because Moringa is rich in secondary metabolites and phenolics, we faced a challenge in extracting a pure DNA required for AFLP (the first proposed genotyping method). Later, different DNA isolation methods were tested to overcome the obstacles caused by phenolics and sugars, an AFLP protocol that worked well with the cultivated seedlings at the botanical garden in Greifswald. The markers for the Internal Transcribed Spacer (ITS) were as well tested that showed a monomorphic structure between all samples. Finally, SSR (microsatellite) markers were established. To optimize DNA extraction, the method of Doyle and Doyle was modified and optimized. This is an ideal method for obtaining a non-fragmented DNA that could be used for AFLP. In addition, two other DNA extraction methods; (KingFisher Flex robot using Omega M1130 extraction Kits, and spin columns and 96-plates using Stratec kits). Although we achieved similar results for both Robot kits (Omega) and Stratec kits, the amplification for most of the samples extracted with Robot did not work, therefore the Stratec kit was the method of choice as it has also a lower cost, combined with a high quality of DNA. For ITS, no polymorphism was found for 28 samples of M. peregrina from Sinai (sequences submitted to GenBank). However, since microsatellite markers of M. peregrina were not known, it was a challenge to try a cross amplification from other species with well-known microsatellite primers. Cross-amplification of 16 primers known from the related species M. oleifera was tested, and three multiplex PCR reactions were established after testing different annealing temperatures and different primers concentrations. This included 13 primers out of the 16 investigated markers which gave a reliable band. All methods used for genetic assessments for the different Moringa species are compiled in a comparative review to look for connections between the different Moringa species. For Moringaceae, M. oleifera and M. peregrina are closely related to each other. Both species have slender trunks, with thick, tough bark and tough roots and bilaterally symmetrical flowers with a short hypanthium. All but one SSR markers used in this study are highly informative However, the degree of polymorphy varied considerably within the 13 markers used. The Probability of Identity (PI) for all loci was 2.6 x 10-9 with high resolution. The percentage of polymorphic loci for all populations was 88.5±2.2; figures for single populations were 92.3%, 84.6%, 84.6%, and 92.3% for the wadis WM, WA, WF, and WZ, respectively. The genotype accumulation curve as well demonstrated that 7–8 markers were necessary to discriminate between 100% of the multilocus genotypes. Significant departures from HWE were detected for eight loci (P < 0.001), probably due a high degree of inbreeding within population. The observed (HO) and expected (HE) heterozygosities ranged from 0 to 0.86 and from 0 to 0.81, respectively. However, for the pooled population, excluding the monomorphic locus MO41, HO and HE ranged from 0.069 to 0.742 and from 0.126 to 0.73 with averages of 0.423 and 0.469, respectively. The mean of FST was 0.133, indicating that, due to the long generation time of M. peregrina, there is still relatively little differentiation between the four remaining populations. An analysis of molecular variance (AMOVA) revealed that the old populations of M. peregrina are still genetically diverse where 75% of variance was recorded within individuals and 83% within populations. An analysis with STRUCTURE, varying the parameter K between 1 and 7, revealed the most pronounced genetic structure for K=3, thus uniting the populations from two neighboured wadis (W. Agala and W. Feiran). The three groups seem to be now genetically isolated. (They may be remainders of a formerly contiguous population, especially when considering the change towards a drier climate in Northern Africa within the last 6000 years). Six clones of each two individuals collected from the same wadi were found, pointing to vegetative dispersal via broken twigs, which may have rooted after flash floods. It may be an alternative mode of reproduction under harsh conditions. Our data reveal a low gene flow between three of the four wadis, suggesting that the trees are relictual populations. In general, conservation of populations from the three genetically most diverse wadis and cross-breeding of trees within a reforestation program is recommended as an effective strategy to ensure the survival of M. peregrina at Sinai, Egypt.
Growth corridors have been an instrument of
economic development for decades but have gained new
attention in regional economic development policies in recent years, e.g., in Sub-Saharan Africa or Southeast Asia.
They are seen by policy makers and private businesses as
catalysts of regional economic integration, pushing traditional businesses into increasingly complex international
value chains. However, the outcomes of such development
initiatives are still barely understood. Critics argue that development policies are based on simplified models that are
unable to sufficiently address the complexity of regional
development. Policies on value-chain development, for
example, can lead to conflicts, external dependencies,
land rush, and a polarization of wealth. Growth corridors
often go hand-in-hand with socio-economic transformations and land-use conflicts. This paper first discusses the
theoretically possible desired and undesired regional socio-economic effects of modern corridors. Second, we illustrate the potential and challenges to realize integrative
(or inclusive) development by contrasting three growth
corridors: the SAGCOT growth corridor in Tanzania, the
Walvis Bay-Ndola-Lubumbashi Development Corridor
(WBNLDC) in Namibia, Zambia and Zimbabwe, and the
growth corridors in the Greater Mekong Subregion (GMS)
Amine transaminases are versatile biocatalysts for the production of pharmaceutically and agrochemically relevant chiral amines. They represent an environmentally benign alternative to waste intensive transition metal catalysed synthesis strategies, especially because of their high stereoselectivity and robustness. Therefore, they have been frequently used in the (chemo)enzymatic synthesis of amines and/or became attractive targets for enzyme engineering especially in the last decade, mainly in order to enlarge their substrate scope. Certainly, one of the most notable examples of amine transaminase engineering is the
manufacturing of the anti-diabetic drug Sitagliptin in large scale after several rounds of protein engineering. Thereby, the target amine was produced in asymmetric synthesis mode which is the most convenient and favored route to a target chiral amine, starting from the corresponding ketone. The choice of the amine donor is highly relevant for reaction design in terms of economical and thermodynamic considerations. For instance, the use of alanine as the natural amine donor is one of the most common strategies for the amination of target ketones but needs the involvement of auxiliary enzymes to shift the reaction equilibrium towards product formation. In fact, isopropylamine is probably one of the most favored donor molecules since it is cheap and achiral but it is supposed to be accepted only by a limited number of amine transaminases.
This thesis focusses on the optimization and application of amine transaminases for asymmetric synthesis reactions en route to novel target chiral amines using isopropylamine as the preferred amine donor.
Background: Magnetic resonance imaging (MRI) techniques are rarely used in the context of abdominal sepsis and in sepsis research. This study investigates the impact of MRI for monitoring septic peritonitis in an animal model (colon ascendens stent-induced peritonitis, CASP). The CASP model closely mimics that of human disease and is highly standardized. The most frequently employed readout parameter in mouse CASP studies is prolonged or decreased rate of survival. Monitoring the progression of peritonitis via MRI could provide a helpful tool in the evaluation of severity. The use of alternative readout systems could very well reduce the number of research animals. Perspectively, clinical improvement after certain treatment could be classified. Methods: This study describes for the first time MRI findings following the induction of septic peritonitis in mice using the CASP model. Two sublethal groups of mice with septic peritonitis were investigated. Each had received one of two differing stent diameters in order to control the leakage of feces into the abdominal cavity. Each mouse served as its own control. Imaging and analyses were performed blinded. Gut diameters, stomach volume, abdominal organ wall diameters, and volume of the adrenal glands were measured. Serum corticosterone levels were detected using ELISA. Serum IL-6, TNF-α, IL-1β, and IL-10 levels were screened by cytometric bead array. Statistical analysis was performed using the Mann-Whitney U test for nonparametric probes and the Kruskal-Wallis and t tests. Results: Using a 7-tesla MRI scanner 24 and 48 h after induction of septic peritonitis, interenteric fluid, organ swelling of spleen and adrenal glands, as well as dilatation of the stomach were compared to nonseptic conditions. Swelling of adrenal glands resulted in an increased serum corticosterone level. In addition, the wall of the intestine bowel was thickened. Based upon these findings, an MRI score (MRI sepsis score, MSS) for abdominal sepsis in mice was established. Reduced stent sizes led to reduced severity of the abdominal sepsis, which could be reproduced in the MSS, which is described here for the first time. Conclusions: Intraabdominal variations during septic peritonitis are detectable by MRI techniques. MRI methods should become a more important tool for the evaluation of abdominal peritonitis. MSS could provide an interesting tool for the evaluation of therapeutic strategies.
Background: Magnetic resonance imaging (MRI) techniques are rarely used in the context of abdominal sepsis and in sepsis research. This study investigates the impact of MRI for monitoring septic peritonitis in an animal model (colon ascendens stent-induced peritonitis, CASP). The CASP model closely mimics that of human disease and is highly standardized. The most frequently employed readout parameter in mouse CASP studies is prolonged or decreased rate of survival. Monitoring the progression of peritonitis via MRI could provide a helpful tool in the evaluation of severity. The use of alternative readout systems could very well reduce the number of research animals. Perspectively, clinical improvement after certain treatment could be classified. Methods: This study describes for the first time MRI findings following the induction of septic peritonitis in mice using the CASP model. Two sublethal groups of mice with septic peritonitis were investigated. Each had received one of two differing stent diameters in order to control the leakage of feces into the abdominal cavity. Each mouse served as its own control. Imaging and analyses were performed blinded. Gut diameters, stomach volume, abdominal organ wall diameters, and volume of the adrenal glands were measured. Serum corticosterone levels were detected using ELISA. Serum IL-6, TNF-α, IL-1β, and IL-10 levels were screened by cytometric bead array. Statistical analysis was performed using the Mann-Whitney U test for nonparametric probes and the Kruskal-Wallis and t tests. Results: Using a 7-tesla MRI scanner 24 and 48 h after induction of septic peritonitis, interenteric fluid, organ swelling of spleen and adrenal glands, as well as dilatation of the stomach were compared to nonseptic conditions. Swelling of adrenal glands resulted in an increased serum corticosterone level. In addition, the wall of the intestine bowel was thickened. Die arterielle Hypertonie (aHT) stellt einen wichtigen kardio- und zerebrovaskulären Risikofaktor dar. Durch eine nicht diagnostizierte, unbehandelte aHT kommt es zu einer Reihe von Endorganschäden und damit verbunden zu einem Anstieg der Morbidität und Mortalität. In früher durchgeführten Studien ergab sich der Anhalt dafür, dass eine überschießende Blutdruckregulation während eines Belastungstests als Prädiktor für eine sich in Zukunft entwickelnde aHT darstellen könnte. Jedoch existiert für den Belastungshypertonus aktuell weder eine einheitliche Definition noch ein standardisiertes Untersuchungsprotokoll.
In der vorliegenden Arbeit wurden erstmalig zwölf unterschiedliche, in der Literatur häufig verwendete, Definitionen für einen Belastungshypertonus auf eine 662 Probanden (388 Frauen, 274 Männer) umfassende Stichprobe einer bevölkerungs-basierten Kohorte angewendet, der Study of Health in Pomerania (SHIP). Es wurden die Prävalenzen sowie assoziierte Risikofaktoren und subklinische kardiovaskuläre Risikomarker der unterschiedlichen Definitionen miteinander verglichen. Zusätzlich konnten mögliche Determinanten eines Belastungshypertonus identifiziert werden.
Durch diese Arbeit konnte ein Beitrag zum weiteren Verständnis der Belastungshypertonie erbracht werden. Es konnte gezeigt werden, dass sich die mit einem Belastungshochdruck assoziierten Risikofaktorenprofile und subklinische kardiovaskuläre Risikomarker abhängig von der angewandten Definition stark voneinander unterschieden. Bei der in dieser Arbeit untersuchten Kohorte wiesen insbesondere das Alter, der BMI und eine vergrößerte IMT signifikante Unterschiede bei den Definitionen auf, die sich auf die Entwicklung des systolischen Blutdrucks bezogen. Die nachgewiesene, vom Geschlecht abhängige, ungleiche Blutdruckentwicklung unter Belastung, insbesondere unter einer submaximalen, unterstützt die These der Wichtigkeit von geschlechtsspezifischen Referenzwerten und Belastungsstufen.
Damit der Belastungshypertonus einen möglichen Einzug in den praktischen Klinikalltag halten kann, sollte weiter an einer Standardisierung eines Untersuchungsprotokolls und an geschlechtsspezifischen und nach dem Alter abgestuften Referenzwerten gearbeitet werden.

Die vorliegende Studie befasste sich mit der Morphologie des Canalis nasolacrimalis (CN) von Japanischen Makaken (Macaca fuscata) basierend auf der Untersuchung koronaler computertomographischer Schichtaufnahmen. Im ersten Teil der Studie wurde das geschlechtsspezifische Wachstum des CN anhand von 29 Schädeln in unterschiedlichen postnatalen Altersgruppen analysiert. Der zweite Teil befasste sich mit dem Einfluss geographischer und klimatischer Faktoren auf die Morphologie des Tränenkanals adulter Japanischer Makaken, die definierten lokalen Populationen entstammen. Im dritten Teil der Arbeit wurde schließlich die Morphologie des CN von Macaca fuscata mit der anderer Spezies der Faszikularis-Gruppe der Makaken (Macaca cyclopis, Macaca fascicularis, Macaca mulatta) verglichen.
Zur Darstellung der Morphologie des CN wurden anhand der koronalen CT-Schichtaufnahmen mit Hilfe der WinSurf Version 4.0® - Software 3D-Rekonstruktionen des CN angefertigt und die entsprechenden Volumina ermittelt. Darüber hinaus wurde die Länge des CN sowie der Winkel zwischen der Achse des CN und der Horizontalebene mit dem Programm OsiriX® Version 5.8.2 vermessen. Die ermittelten Maße des CN wurden mit externen linearen Schädelmaßen in Beziehung gesetzt, um den Einfluss der Schädelmorphologie auf den CN zu untersuchen.
Männliche und weibliche Japanische Makaken wiesen zahlreiche Übereinstimmungen in der Morphologie des CN auf. So erstreckte sich der CN vom orbitalen Eingang nach kaudal und lateral. Dabei befand sich der orbitale Eingang weiter medial als die Mündungsstelle in der Nasenhöhle. Erst ab der Altersgruppe 2 erschien der Sinus maxillaris gleichzeitig auf einem Schichtbild mit dem CN. Die Maße des CN vergrößerten sich mit zunehmendem Alter, wobei das Volumen des CN adulter männlicher Tiere größer war als das der weiblichen Tiere. Bis auf den linken CN adulter Tiere wiesen weibliche Individuen in allen anderen Altersgruppen einen längeren CN auf als männliche Individuen. Die größte Volumen- und Längenzunahme zeigten sich bei Männchen zwischen der Altersgruppe 3 und 4, bei Weibchen zwischen der Altersgruppe 1 und 2. Die Achse des CN war bei infantilen Tieren nahezu senkrecht zur Horizontalebene ausgerichtet und verlagerte sich mit zunehmenden Alter schräg nach vorne.
Für die Volumen-, Längen- und Winkelmessungen des CN konnten signifikante geschlechtsspezifische Unterschiede beobachtet werden. Jedoch waren die Unterschiede nicht einheitlich und betrafen nicht alle Altersgruppen. Die vorliegende Studie zeigte für Macaca fuscata einen statistisch signifikanten Zusammenhang zwischen dem Wachstum des CN und externen Schädelmaßen. Darüber hinaus zeigten die Vergleiche der Anstiege der Regressionsgeraden zwischen beiden Geschlechtern keine signifikanten Unterschiede. Somit ist anzunehmen, dass sich die Tränennasenkanäle beider Geschlechter mit einem ähnlichen Wachstumsmuster vergrößern.
Der Einfluss klimatischer und geographischer Faktoren auf den CN wurde an 37 Schädeln adulter Japanischer Makaken beiderlei Geschlechts von fünf definierten geographischen Regionen Japans untersucht. Dazu wurden die ermittelten Maße des CN in Relation zu Klimadaten gesetzt. Für das Volumen ergaben sich keine statistisch signifikanten Unterschiede im Populationsvergleich. Jedoch unterschieden sich die Messwerte der lokalen Gruppen hinsichtlich der Länge und der Achsenneigung des CN signifikant voneinander. Zudem konnten für den linken CN signifikante Beziehungen zur geographischen Lage und der kleinsten Januartemperatur herausgearbeitet werden. Da dieser Zusammenhang nur für eine Seite beobachtet werden konnte, besteht weiterer Untersuchungsbedarf.
Schließlich wurden die vier Spezies der Faszikularis-Gruppe bezüglich der Morphologie des CN anhand von 34 adulten Schädeln beiderlei Geschlechts untersucht. Statistisch signifikante Unterschiede ergaben sich für das Volumen und die Länge des CN. Während für den CN von Macaca fuscata bezüglich Volumen und Länge die größten Messwerte bestanden, zeigte Macaca mulatta die kleinsten Messwerte. Hinsichtlich der Ausrichtung des CN zur Horizontalebene bestanden keine Speziesunterschiede.
Die Ergebnisse dieser Untersuchung beschreiben nicht nur den Tränennasengang innerhalb einer Wachstumsreihe von Macaca fuscata, sondern liefern erstmals Erkenntnisse über die Morphologie des CN bei verschiedenen geographischen Populationen der Rotgesichtsmakaken sowie der Faszkularis-Gruppe. Angesichts der bekannten Unterschiede zwischen Catarrhini und Platyrrhini bezüglich der Morphologie der Nasenhöhle sind weiterführende Untersuchungen notwendig um die Faktoren, welche die Morphologie des Tränennasengangs beeinflussen, weiter zu ergründen.
Ion thrusters are Electric Propulsion systems used for satellites and space missions. Within
this work, the High Efficient Multistage Plasma Thruster (HEMP-T), patented by the
THALES group, is investigated. It relies on plasma production by magnetised electrons.
Since the confined plasma in the thruster channel is non-Maxwellian, the near-field plume
plasma is as well. Therefore, the Particle-In-Cell method combined with a Monte-Carlo
Collision model (PIC-MCC) is used to model both regions. In order to increase the sim-
ulated near-field plume region, a non-equidistant grid is utilised, motivated by the lower
plasma density in the plume. To minimise artificial self-forces at grid points bordered by
cells of different size a modified method for the electric field calculation was developed in
this thesis. In order to investigate the outer plume region, where electric field and collisions
are negligible, a ray-tracing Monte-Carlo model is used. With these simulation methods,
two main questions are addressed in this work.
What are the basic mechanisms for plasma confinement, plasma-wall-interaction
and thrust generation?
For the HEMP-T the plasma is confined by magnetic fields in the thruster channel, generated
by cylindrical permanent magnets with opposite polarity. Due to different Hall parameters,
electrons are magnetised, while ions are not. Therefore, the dominating electron transport
is parallel to the magnetic field lines. In the narrow cusp regions, the magnetic mirror effect
reduces the electron flux towards the wall and confines the electrons like in a magnetic
bottle. At the anode, propellant gas streams into the thruster channel, which gets ionised
by the electrons creating the plasma. As a result of the electron oscillation between the two
cusp regions, ionisation of the propellant gas is efficient.
The magnetic field configuration of the HEMP-T also influences the plasma potential inside
the thruster channel. Close to the symmetry axis, the mainly axial magnetic field results in
a flat potential. At the inner wall, the field configuration reduces the plasma wall interaction
to only the narrow cusp regions. Here, the floating potential of the dielectric channel wall
and its plasma sheath result in a rather low radial potential drop compared to the applied
anode potential. As a result, the electric potential is rather flat and impinging ions at the
thruster channel wall have energies below the sputter threshold energy of the wall material.
Therefore, no sputtering appears at the dielectric wall. At the thruster exit the confinement
by the magnetic field is weakened and the potential drops with nearly the full anode voltage.
The resulting electric field accelerates the generated ions into the plume and generate the
thrust, but they are also able to sputter surfaces. During terrestrial testing, sputteringat vacuum vessel walls leads to the production of impurities. The amount of back-flux
towards the channel exit is determined by the sputter yield of the vacuum chamber wall. A
large distance between thruster exit and vessel wall reduces the back-flux and smooths the
pattern of deposition inside the thruster channel. Dependent on their material, the evolving
deposited layers can get conductive, modify by this the potential distribution and reduce
the thrust.
For the HEMP-T, ions are mainly generated at high potential close to the applied anode
potential. Therefore, the accelerated ions producing the thrust gain the maximum energy
as observed in experiment. Ions emitted from the thruster into different angles in the
plume contribute mainly to the ion current at angles between 30 ◦ and 90 ◦ . They mainly
originate from ionisation at the thruster exit. The resulting angular distribution of the
ejected ion current is close to the one of the experiment, slightly shifted by about ten
degrees to higher emission angles. In front of the thruster exit, electrons are trapped by
electrostatics forces. This enhanced density allows ionisation and an additional electron
density structure establishes.
What are possible physics based ideas for optimisation of an ion thruster?
An optimised thruster should have a high ionisation rate inside the thruster channel, low
erosion and an ion angular distribution with small contributions at high angles for min-
imised thruster satellite interactions. In experiments, the HEMP-T satisfies already quite
nicely these requests. In the simulations, low erosion inside the thruster channel and angular
ion distributions close to the experimental data are demonstrated. However, the ionisation
efficiency is lower and radial ion losses are larger than in experiment. A possible explanation
of these differences is an underestimated transport perpendicular to the magnetic field lines,
well known for magnetised plasmas.
A successful example for an optimisation using numerical simulations is the reduction of
back-flux of sputtered impurities during terrestrial experiments by an improved set-up of
the vacuum vessel. The implementation of baffles reduces the back-flux towards the thruster
exit and therefore deposition inside the channel. These improvements were successfully im-
plemented in the experiment and showed a reduction of artefacts during long time measure-
ments. This leads to a stable performance, as it is expected in space.
Das Cochlea-Implantat (CI)-System stellt eine etablierte Versorgungsmöglichkeit einer Taubheit oder an Taubheit grenzenden Schwerhörigkeit dar, die in vielen Fällen den Betroffenen ein gutes bis sehr gutes Sprachverstehen ermöglicht. Jedoch wird in einigen Fällen das erwünschte Ergebnis einer CI-Versorgung nur zum Teil oder nicht erreicht. Außerdem kann sich im Verlauf einer CI-Versorgung ein anfänglich guter Hörgewinn trotz anscheinend regelrechter Funktion des Implantats verschlechtern. In diesen Fällen kann eine retrocochleäre Störung vorliegen, deren Nachweis allein mit bildgebenden Verfahren schwierig ist. Die Kombination von elektrisch evozierten Summenaktionspotentialen (ECAP) und elektrisch evozierten Hirnstammpotentialen (EAEP) kann den Nachweis einer retrocochleären Störung bei Patienten mit CI verbessern. Ziel der vorliegenden Studie war es, Referenzwerte für elektrisch evozierte frühe Hirnstammpotentiale (ECAP und EBERA) von CI-Trägern zu ermitteln, deren Rehabilitationsverlauf ein durchschnittliches Versorgungsergebnis erwarten ließ.
An 19 Probanden wurden insgesamt 20 Messreihen durchgeführt. Dabei wurden an diesen Probanden, unter Verwendung eines optimierten, klick-äquivalenten Stimulus, elektrisch evozierte Potentiale gemessen. Die verwendeten Stimulations- und Messparameter wurden ausführlich erläutert. Auch die Durchführung der Messreihen wurde umfassend beschrieben.
Die Ergebnisse aller Messreihen zur Erstellung der Referenzwerte wurden dargestellt und analysiert. Bei der Darstellung und Diskussion wurde besonders auf die Dynamik, die Amplituden, die Absolut-Latenzen und Latenz-Differenzen der gemessenen Potentiale eingegangen. Die Referenzwerte wurden für ipsilaterale und kontralaterale Ableitung in Wertetabellen dargestellt. Besonders die von der Stimulationsintensität abhängigen Absolut-Latenzen der Potentiale eJ3 und eJ5 zeigten unterschiedliche Ausprägungen. Im Anschluss wurden fünf EAEP-Messungen aus der klinischen Routine mit jeweils unterschiedlichen Fragestellungen vorgestellt und gegen die Referenzwerte verglichen.
Im letzten Abschnitt wurden sowohl die Referenzwerte als auch die Messungen aus der klinischen Routine diskutiert. Aus der Dynamik der gemessenen ECAP konnte eine Durchführungsempfehlung für EBERA-Messungen an Patienten, die die subjektiv empfundenen Lautheiten der Stimulation nicht eindeutig angeben können, abgeleitet werden. Abschließend wurden die Grenzen der Methode diskutiert und Ausblicke auf zukünftige Forschungsvorhaben gegeben.
Bei der Untersuchung verschiedener chronischer Nierenerkrankungen in den letzten Jahrzehnten zeigte sich, dass Podozyten als Bestandteil der glomerulären Filtrationsbarriere häufig in die Pathomechanismen involviert sind, wobei ein Verlust ihrer besonderen Architektur aus interdigitierenden Fußfortsätzen (effacement) und auch die Ablösung einzelner Podozyten von der glomerulären Basalmembran beobachtet werden können. Da es sich um postmitotische Zellen handelt, kann ein Verlust nur durch eine Hypertrophie verbleibender Zellen ausgeglichen werden. Nach wie vor ist nicht eindeutig geklärt, ob ein Ersatz stattfindet und wenn ja, welche Zellen dafür verantwortlich sind. Seit einigen Jahren sind die PECs in den Fokus der Aufmerksamkeit gerückt und es konnte gezeigt werden, dass sie sich unter bestimmten Bedingungen zu Podozyten-ähnlichen Zellen differenzieren, aber auch einen negativen Einfluss im vorgeschädigten Glomerulum durch ein profibrotisches Potential ausüben können.
In dieser Arbeit wurden drei Transkriptionsfaktoren (Dach1, MafB und Foxc2) in PECs transfiziert. Für alle drei konnte in der Vergangenheit eine Bedeutung in der Nieren-bzw. Podozytenentwicklung gezeigt werden. Es sollte untersucht werden, ob einer der drei Transkriptionsfaktoren eine Differenzierung der PECs zu Podozyten in vitro induzieren könnte. Auch der Einfluss der jeweiligen Faktoren untereinander wurde untersucht. Dabei zeigte sich, dass die Transfektion mit pMafB-tGFP einen signifikanten Dach1-Anstieg in PECs bedingte. Hinsichtlich der Expression von F Aktin, α-Tubulin und dem Podozyten-spezifischen Transkriptionsfaktor WT-1 in PECs zeigte sich bildmorphologisch kein Hinweis für einen möglichen Einfluss von Dach1, MafB und Foxc2 in PECs. Allerdings konnte durch die Überexpression von Dach1 in PECs ein signifikanter Anstieg des Podozyten-spezifischen Proteins Synaptopodin beobachtet werden. Des Weiteren kam es zu einer Herunterregulation von Pax-2, was insofern bedeutsam ist, als dass auch Podozyten eine verminderte Pax-2-Expression aufweisen. Es zeigte sich außerdem eine verminderte Expression von Caveolin-1 und β-Catenin. Während Ersteres eher als ein Zeichen des profibrotischen und somit negativen Potentials gewertet werden könnte, ähnelt die Herunterregulation von β-Catenin wiederum dem Status von Podozyten.
Zusammenfassend bestätigt die Arbeit das Differenzierungspotential von PECs zu Podozyten und zeigt, dass Dach1 in vitro ein entscheidender Faktor dafür ist.
Functional characterization of a novel protease isolated from a mouse-adapted S. aureus strain

Background: The high incidence of methicillin-resistant Staphylococcus aureus
(MRSA) strengthens the need for new effective antibiotics and a protective vaccine. Up till now, mainly human-adapted Staphylococcus aureus strains were used to study S. aureus pathogenicity in mouse models. However, it is known that S. aureus is highly host-specific. Recently, a mouse-adapted S. aureus strain, JSNZ, was identified. This strain could be a promising tool in developing more appropriate infection models. JSNZ produces high amounts of a putative extracellular protease, named JSNZ extracellular protease (Jep). Since the jep gene was only detected in S. aureus isolates from laboratory mice and wild small rodents and shrews, we hypothesize that Jep is important for colonization and infection in mice. The jep deletion mutant previously created by our collaborators from the University of Auckland, New Zealand, intriguingly showed a reduced survival and growth fitness in murine serum and whole blood as compared to the JSNZ wild type (WT) strain.
Objective: To elucidate the role of Jep in the interaction between S. aureus and its
host by comparing the impact of JSNZ WT with a mutant and a complement strain on the murine immune system. In addition, the elucidation of possible genetic factors behind host-adaptation of S. aureus strains isolated from wild rodents and shrews.
Methods: A jep complemented strain was generated by chromosomal replacement.
JSNZ WT, the jep mutant and the complement strain were subjected to functional
assays (whole blood survival assay, coagulation assay). In addition, the genetic
background that might confer host specificity was tested by staph array genotyping.
Results: The mutant strain JSNZDjep was successfully complemented with the jep
gene using a chromosomal integration approach. The WT strain and the
complemented strain produced the Jep protein in comparable amounts.
Unexpectedly, the complemented strains did not behave like the WT strain but rather like the mutant in a series of in vitro assays. Firstly, the growth of both the deletion mutant and the complemented strains was slightly reduced in TSB as compared to the WT strain. Secondly, the jep knockout strain showed a strongly reduced survival in murine whole blood compared to its wild type counterpart, but so did the complemented strain. Finally, the coagulation of murine plasma was less pronounced for the jep deletion mutant and the complemented strain as compared to the JSNZ WT. To exclude a defect in jep gene expression, we compared the amount of Jep expressed during growth in TSB medium for the three strains. The complemented strain produced Jep in a manner similar to the WT strain in a growth-phase dependent manner, suggesting that Jep expression was not affected during the creation of the complemented strain.
The array data showed some differences in the genetic makeup between animal
isolated strains and matched human strains. For example, while all animal isolates of the CC88 lacked the resistance mecA gene it was found in some human isolates of the same strain.
Conclusion: In conclusion, our unidentified mutation created during the generation
of the jep knock-out strain rather than the jep gene itself manipulated the murine
immune response. The responsible gene and the underlying mechanisms remain to
be clarified. Genetic profiling of S. aureus strains allowed us to obtain some valuable information including data about CC49, the most frequently isolated lineage in wild rodents and shrews where compared to the human isolates the murine strains showed clear signs of host adaptation. However, the analysis had several limitations including the small sample size.
Background/Aim: Laparoscopic single-port surgery has emerged as a growing trend in minimally invasive surgery. Single-port access is preferred among women undergoing gynecologic surgery who have cosmetic concerns about scarring. Furthermore, this approach results in comparable clinical outcomes to standard laparoscopic surgery and perioperative morbidity rates have been reported to be low. The hypothesis is that a single-port technique might offer such advantages over the standard multi-port laparoscopy as less postoperative pain and better cosmetic results by decreasing abdominal wall tissue trauma. The potential disadvantages of single-port approaches are the larger umbilical incision and the technical difficulties. There are only a few randomized studies in the literature that investigate the value and safety of single-incision laparoscopic surgery in gynecological surgery. The aim of this study was to compare the safety and quality of life in patients who undergo single-incision laparoscopic assisted vaginal hysterectomy and those who undergo conventional laparoscopic assisted vaginal hysterectomy.
Methods: In a prospective randomized trial, 64 patients from three different centers in Germany were randomized (1:1) to conventional laparoscopic assisted vaginal hysterectomy (n=32) or single-incision laparoscopic assisted vaginal hysterectomy (n=30). Data was collected on 60 patients who fulfilled the criteria.
Results: The baseline characteristics of patients were similar in both groups. The mean operative time was comparable in both groups (68.2 vs 73.6 min., p = 0.409). Within the two groups, no differences were seen regarding estimated blood loss (p = 0.915), intra- and postoperative complications (p = 0.944), and wound infection rates (p = 0.944). Patients within the single-incision laparoscopic surgery group experienced significantly less pain in the first 24 hours postoperatively (p = 0.006), while pain scores at days 3, 5, 7 and 2 months postoperatively were comparable.
Conclusion: This study demonstrates that single-incision laparoscopic assisted vaginal hysterectomy is a reliable and safe setup in gynecologic surgery. Compared to conventional laparoscopic assisted vaginal hysterectomy, Notably, patients undergoing single-incision laparoscopic assisted vaginal hysterectomy experienced less pain postoperatively.
In aktuellen Studien aus den letzten Jahren mehren sich die Hinweise auf metabolische Einflüsse
von Osteocalcin. Neben dem Skelettsystem scheint besonders der Energiestoffwechsel,
speziell auf der Ebene der Distribution und des Verbrauchs von Glucose mit Osteocalcin
zu interagieren. In der vorliegenden Studie wurden Blutplasma- und Urinproben von 931
gesunden Probanden mittels Massenspektrometrie (Tandem-Massenspektroskopie mit vorgeschalteter
Flüssigkeitschromatographie) auf niedermolekulare Substanzen untersucht, um
dann auf systemische Effekte von Osteocalcin zu schließen. Diese Methodik erlaubt eine
breite Untersuchung von Wirkungen von Osteocalcin in allen Organsystemen, auch in jenen,
für die bisher noch keine Interaktionen mit Osteocalcin bekannt sind. Die Berechnung eines
ersten Modells zeigte viele Zusammenhänge. Diese waren jedoch stark durch die Nierenfunktion
beeinflusst. Nach Adjustierung für die Nierenfunktion blieben insgesamt 29 signifikante
Ergebnisse erhalten. Zu diesen Ergebnissen zählten zuvorderst Zwischenprodukte des
Kollagenstoffwechsels, besonders Prolinderivate, was die Bedeutung von Osteocalcin im
Knochenstoffwechsel unterstreicht. Die weiteren Ergebnisse umfassten eine Assoziation mit
Kynurenin, ein Hinweis auf die Möglichkeit, dass Entzündungen Einflüsse auf zirkulierendes
Osteocalcin haben könnten. Weitere Hinweise auf die bereits bekannte Verknüpfung zwischen
dem Energiestoffwechsel und Osteocalcin bietet die vorliegende Studie durch die Detektion
einer Assoziation zwischen Osteocalcin und Abbauprodukten von verzweigtkettigen
Aminosäuren. Auch scheint Osteocalcin vom Lebensstil, wie beispielsweise dem Tabakrauchen,
beeinflusst zu werden. Zusammenfassend bietet die vorliegende Studie einen umfassenden
Überblick über die metabolischen Einflüsse von Osteocalcin. Darin war eine Vielzahl
von Assoziationen nachweisbar, die jedoch insgesamt für eine eher geringe Rolle von Osteocalcin
im menschlichen Stoffwechsel sprechen.
Im Rahmen einer prospektiven Studie von Mai 2012 bis Juni 2013 an 206 Patienten mit lumbalem Facettensyndrom an der Klinik für Orthopädie und orthopädische Chirurgie wurde die Symptomlinderung durch eine stationäre Komplextherapie mit Bildwandler verstärkten Facetteninfiltrationen anhand standardisierter Fragebögen (im Folgenden: Funktionsfragebogen Hannover Rücken, Oswestry Disability Index, Numerische Rating-Skala für Bein- und Rückenschmerz, Center for Epidemiological Studies - Depression Scale, Hospital Anxiety and Depression Scale - deutsche Version, Fragebogen zur Lebenszufriedenheit-Module, SF-12 Health Survey) zu den drei Zeitpunkten vor Infiltration (T1), nach Abschluss der Infiltration – „kurzfristig“ (T2), sowie vier bis sechs Wochen nach Infiltration – „mittelfristig“ (T3) erhoben. Die Patienten wurden entsprechend des Vorhandenseins einer Deformität (in Form einer degenerativen [adulten] Skoliose/ Spondylolisthese) in zwei Gruppen unterteilt und deren Ergebnisse verglichen. Bei 115 Patienten bestand zum Untersuchungszeitpunkt keine Wirbelsäulendeformität, bei 91 Patienten bestand eine Wirbelsäulendeformität. Als statistisches Verfahren kam der Kruskal-Wallis-Test zum Einsatz, mit dem getestet wurde, ob zwischen den zwei Gruppen zu den drei Befragungszeitpunkten signifikante Unterschiede bestanden. Innerhalb einer jeden Gruppe wurden die drei Befragungszeitpunkte untereinander mithilfe des Vorzeichen-Rang Tests auf signifikante Unterschiede getestet. Betrachtet man die wesentlichen Ergebnisse, so zeigt sich, dass das subjektive Schmerzempfinden (NAS-Rücken, NAS-Bein) durch die Therapie signifikant kurz- und mittelfristig sinkt. Hierbei spielt das Vorliegen einer Wirbelsäulendeformität keine Rolle. Die körperliche Funktionsfähigkeit (FFbH-R, ODI) steigert sich signifikant kurz und mittelfristig bei Patienten ohne Deformität, die Gruppe mit Deformität profitiert nur kurzfristig und weist zu beiden Nachuntersuchungszeitpunkten signifikant schlechtere Funktionswerte auf. Die Fragebögen zur depressiven Symptomatik weisen einen kurz- und mittelfristigen Rückgang der depressiven Symptome ohne Gruppenunterschied auf (CES-D), wobei ein Fragebogen (HADS-D) Hinweise auf eine Unterlegenheit der Patienten mit Deformität zum Untersuchungszeitpunkt T3 gibt. Keine signifikanten Veränderungen und Gruppenunterschiede kann der Score zur allgemeinen Lebenszufriedenheit ausmachen, während die Zufriedenheit mit der Gesundheit sich bei allen Patienten kurzfristig und bei Patienten ohne Deformität auch mittelfristig verbessert, wobei diese nur präinterventionell eine signifikant bessere Zufriedenheit zeigen.
Aufgrund dessen sind bei lumbaler Infiltrationstherapie eines Facettensyndroms, insbesondere zur Prognose der Entwicklung der körperlichen Funktionsfähigkeit, die biomechanischen Veränderungen zu beachten. Unabhängig vom Vorliegen einer Deformität hingegen ist die durch die Therapie signifikant verminderte subjektive Schmerzwahrnehmung. Weiterhin gibt das Vorhandensein einer Deformität, in verschiedener Ausprägung im jeweiligen Score, Hinweise auf die Dauer der Symptomverbesserung.
Die Zusammenhänge zwischen der Aktivität Epigenetik-assoziierter Enzyme und Krankheitsgeschehen neoplastischer Art konnten nicht nur experimentell bestätigt, sondern auch spätestens mit der Zulassung von Inhibitoren der DNA-Methyltransferase und Histon-Desacetylasen kausal bewiesen werden. Die weniger gut untersuchten 2-Oxoglutarat- und Eisen(II)-abhängigen JumonjiC-Domänen enthaltenden Histon-Demethylasen (KDMs) werden aufgrund von Überexpression in einigen Tumorzellen ebenfalls als mögliches epigenetisches Target in der Tumortherapie angesehen.
Die in dieser Arbeit synthetisierten KDM4-Inhibitoren basierten zum einen auf Tetrazol- und Hydrazid-haltigen Motiven, für welche synthetische Routen in weitergehenden Untersuchungen entwickelt wurden, um das aliphatische Rückgrat der Moleküle mit aromatischen Resten zu modifizieren oder komplett durch eines auf Basis der Anthranilsäure zu ersetzen. Zum anderen sollten Synthesewege für verschiedenartige (1H-Tetrazol-5 yl)pyridine gefunden werden. Dafür wurde ein Tetrazol in Position vier des Pyridinrings eingefügt, um anschließend systematische Untersuchungen der Seitenkette in Position zwei des Pyridinrings vorzunehmen. Neben direkten Heteroatomverknüpfungen wurden auch Carbonylverbindungen wie Carbonsäureamide oder eine Hydroxamsäure dargestellt. Über zahlreiche Wege konnten ebenfalls Methylengruppen-haltige Verbindungen erfolgreich synthetisiert werden.
Die biologische Testung der Verbindungen erfolgte mit einem Antikörper-basierten LANCE-Assay gegen KDM4A. Zusätzlich konnten vier der Substanzen röntgenkristallographisch in der Isoform KDM4D vermessen werden. Eine in Position zwei des Pyridinrings eingefügte Hydroxamsäure zeigte relevante Interaktionen im Enzym und veranschaulichte zugleich, dass (1H-Tetrazol-5 yl)pyridine zu einer hohen Affinität bestimmter Isoformen der Histon-Demethylasen neigen können.
Zusätzlich zu den bereits ansatzweise verstandenen Zusammenhängen zwischen Epigenetik und neoplastischen Krankheiten, weisen Experimente im Nagermodell darauf hin, dass epigenetisch modifizierende Enzyme auch an der Entstehung von depressionsartigen Phänotypen beteiligt sind. Die grundsätzliche Erforschung neuer Therapieoptionen abseits der Monoaminhypothese für die Indikation Depression ist jedoch auch deshalb notwendig, weil zugelassene medikamentöse Behandlungsoptionen zum Teil zu starken Nebenwirkungen führen und bis zur Entfaltung der vollen Wirkung oftmals mehrere Wochen vergehen können. Mit dem NMDA-Rezeptorantagonisten Ketamin behandelte depressive Patienten zeigten häufig eine rasche Symptomlinderung bei vergleichsweise günstigem Nebenwirkungsprofil. Welche Moleküle für diese Effekte ursächlich sind, ist noch Gegenstand aktueller Forschung, da neben Ketamin selber auch Metaboliten wie 2R,6R-Hydroxynorketamin mit antidepressiven Wirkungen assoziiert werden. Für die qualitativen und quantitativen Analyse von R- und S-Ketamin sowie den Metaboliten R- und S-Norketamin, R- und S-Dehydronorketamin und 2R,6R- und 2S,6S Hydroxynorketamin in menschlichem Urin wurde in der vorliegenden Arbeit eine chromatographische Methode entwickelt und validiert, welche die Techniken der überkritischen Fluidchromatographie und Massenspektrometrie nutzt (SFC-MS). Die für die SFC charakteristischen Eigenschaften wie hohe Flussraten und Trenneffizienz sowie die Nutzung von Kohlenstoffdioxid als mobile Phase konnten ausgenutzt werden, um das Trennproblem in nur einer Methode mit kurzer Laufzeit zu lösen. Gleichzeitig wurde der Verbrauch von organischen Lösungsmitteln reduziert. Damit kann die erarbeitete Methode einen nachhaltigen Beitrag für zukünftige Ketaminforschung leisten.
Depressive Störungen führen zu den größten individuellen und sozialgesellschaftlichen Kosten weltweit. In der vorliegenden Arbeit, basierend auf der epidemiologischen bevölkerungsrepräsentativen SHIP-LEGENDE Studie, betrachteten wir den Einfluss kritischer Lebensereignisse bezüglich Anzahl, Zeitpunkt des Eintretens und subjektiver sowie objektivierter Bewertung als Risikovariablen und sozialer Unterstützung als potentiell protektivem Faktor auf das Auftreten von Depressivität im Lebensverlauf. Unser Ziel war es, auf dieser Grundlage ätiologische Modelle und adäquate therapeutische Interventionsstrategien zu prüfen und weiterzuentwickeln. Die Stralsunder Ereignisliste (SEL) konnte dabei als ein potentielles Standardverfahren zur Erfassung kritischer Lebensereignisse erprobt und im Weiteren validiert werden. Herausragend ist bei diesem strukturierten Interviewverfahren insbesondere die Bezugnahme auf subjektive und objektive Bewertungen von Lebensereignissen und die zeitliche Einordnung der Lebensereignisse.
Hintergrund und Ziele:
Nach Schätzung des Bundesamtes für Strahlenschutz erhielt in Deutschland im Jahr 2012 jeder Einwohner ca. 1,6 Röntgenuntersuchungen bzw. eine Strahlendosis von 1,8 mSv. Etwa ein Drittel aller Untersuchungen entfiel dabei auf den Skelettapparat. Auch die Hüftdiagnostik wird bisher primär anhand konventioneller Röntgen- oder CT-Bilder durchgeführt. Daher existieren bislang nur hierfür validierte Norm- und Referenzwerte. Da in Deutschland inzwischen fast flächendeckend die Option einer Diagnostik über die Magnetresonanztomographie (MRT) vorhanden ist, besteht die Möglichkeit, Patienten ohne Strahlenbelastung zu untersuchen. Das Ziel der vorliegenden Arbeit ist es daher eine wissenschaftliche Grundlage zur Auswertung von MRT-Bildern der Hüfte zu schaffen.
Methodik:
Zur Referenzwertbestimmung wurden der Center-Edge-Winkel (CE), der Centrum-Collum-Diaphysen-Winkel (CCD), der modifizierte Alpha-Winkel (AA), der Trianguläre Index (TI) und der Femurkopfdurchmesser (HD) an 3.226 coronalen MRT-Bildern (1.587 Männer, 1.639 Frauen) der SHIP-Studie untersucht. Des Weiteren wurden Abhängigkeiten der Winkel untereinander sowie zu somatometrischen Daten (Geschlecht, Alter, BMI, Körpergewicht, Körpergröße und Taillenumfang) analysiert. Aufgrund dieser Abhängigkeiten konnten zudem adjustierte Referenzwerte ermittelt werden. Zusätzlich wurden die Prävalenzen verschiedener Schwellenwertüberschreitungen in der Region Vorpommern bestimmt.
Ergebnisse:
Die Readerzertifizierung wurde anhand von 25 MRT-Datensätzen durchgeführt. Die nach der Bland-Altman-Methode bestimmten Qualitätsanforderungen an die Intra- und Interreader-Reliabilität wurden erfüllt.
Der aus den Messungen ermittelte durchschnittliche CE lag bei 31,1 Grad (± 7,02), der abgeleitete Normbereich, entsprechend dem 95 %-Referenzbereich (Mittelwert ± 1,96 * SD), bei 17,6 – 44,9 Grad. Für den CCD ergab sich ein Mittelwert von 126,9 Grad (± 6,66), der abgeleitete Normbereich betrug 113,9 – 140,0 Grad. Beim modifizierten AA wurde ein mittlerer Wert von 54,5 Grad (± 8,15) sowie ein Referenzbereich von 38,5 – 70,5 Grad gefunden. Im Mittel konnte für den TI -2,29 mm (± 0,667) errechnet werden, der HD betrug durchschnittlich 44,6 mm (± 3,94).
Unter den gemessenen Hüftparametern fanden sich, außer zwischen CE und AA bzw. TI sowie zwischen HD und CCE bzw. TI, auffällige Zusammenhänge. Die dabei ermittelten Effekte zwischen den Winkeln waren jedoch überwiegend geringer als die von somatometrischen Parametern. Hierbei waren die Effekte von Geschlecht und Alter auf die ermittelten Hüftparameter am größten. Deshalb wurden für CE, CCD und AA darauf adjustierte Referenzwerte ermittelt.
Die hier ermittelten Prävalenzen der Schwellenwertüberschreitungen umfassen einen großen Bereich, da derzeit unterschiedlichste Schwellenwerte Anwendung finden.
Schlussfolgerung:
In der Hüftdiagnostik an MRT-Daten können der CE, der CCD, der modifizierte AA, der TI, und der HD verlässlich bestimmt werden. Die hier ermittelten Messwerte der MRT-Diagnostik stimmen im Wesentlichen mit den etablierten Normwerten der röntgenologischen und computertomographischen Bildgebung überein. Die erwarteten Abhängigkeiten der ermittelten Parameter untereinander sowie mit somatometrischen Parametern sind zum großen Teil gegeben. Zur genaueren Interpretation der Werte sollte aufgrund der Abhängigkeiten adjustierte Referenzwerte verwendet werden. Die Ergebnisse gelten für eine norddeutsche Population. Für allgemeingültige Normwerte sollte die Bevölkerung weiterer Regionen und Ethnien untersucht werden.
