Purpose
A setting-sensitive instrument for assessing Quality of Life (QoL) in Telemedicine (TM) was unavailable. To close this gap, a content-valid “add-on” measure was developed. In parallel, a brief index was derived featuring six items that summarise the main content of the multidimensional assessment. After pre- and pilot-testing, the psychometric performance of the final measures was investigated in an independent validation study.
Methods
The questionnaires were applied along with other standardised instruments of similar concepts as well as associated, yet disparate concepts for validation purposes. The sample consisted of patients with depression or heart failure, with or without TM (n = 200). Data analyses were aimed at calculating descriptive statistics and testing the psychometric performance on item, scale, and instrument level, including different types of validity and reliability.
Results
The proposed factor structure of the multidimensional Tele-QoL measure has been confirmed. Reliability coefficients for internal consistency, split-half, and test-retest reliability of the subscales and index reached sufficient values. The Tele-QoL subscales and the index demonstrated Rasch scalability. Validity of both instruments can be assumed. Evidence for discriminant construct validity was provided. Known-groups validity was indicated by respective score differences for various classes of disease severity.
Conclusion
Both measures show convincing psychometric properties. The final multidimensional Tele-QoL assessment consists of six outcome scales and two impact scales assessing (un-)intended effects of TM on QoL. In addition, the Tele-QoL index provides a short alternative for outcome assessment. The Tele-QoL measures can be used as complementary modules to existing QoL instruments capturing healthcare-related aspects of QoL from the patients’ perspective.
Hintergrund und Ziel
Um die präklinische Notfallversorgung zu optimieren und aktuelle Herausforderungen zu bewältigen, wurde im Landkreis Vorpommern-Greifswald im Jahr 2017 ein Telenotarzt-System eingeführt. Es sollte aus medizinischer und ökonomischer Sicht geprüft werden, ob dies, insbesondere im ländlichen Raum, eine effiziente Ergänzung der präklinischen Notfallversorgung darstellt.
Methodik
Es wurden ca. 250.000 Einsatzdaten, vor und nach Einführung des Systems, über die Jahre 2015 bis 2020 ausgewertet und ein Prä-Post-Vergleich über die Einsatzstruktur erstellt. Die 3611 Einsätze der Telenotärztinnen und -ärzte (TNA) wurden nach medizinischen Indikationen und zeitlichen Faktoren analysiert sowie mit Einsätzen ohne TNA verglichen. Zusätzlich erfolgten eine Analyse der Gesamtkosten des neuen Versorgungskonzeptes sowie eine Kostenanalyse der prä- und innerklinischen Behandlungskosten ausgewählter Erkrankungen.
Ergebnisse
Das Einsatzspektrum des TNA umfasste alle Altersstufen mit verschiedenen Meldebildern, die zu 48,2 % eine mittlere Erkrankungsschwere (stationäre Behandlung erforderlich) hatten. Von Patient*innen und Mitarbeitenden wurde das System gut angenommen. Die Einsatzdaten zeigten einen signifikanten Rückgang der Notarztbeteiligung bei telenotarztfähigen Einsatzfahrzeugen um 20 %. Die jährlichen Kosten des Systems belaufen sich auf ca. 1,7 Mio. €.
Schlussfolgerung
Die Ergebnisse belegen die Vorteilhaftigkeit des TNA-Systems, sodass es über die Projektdauer hinaus implementiert wurde. Das System ist medizinisch sinnvoll, funktionsfähig sowie effizient und steht als Innovation für die Umsetzung in ganz Deutschland bereit.
Introduction
In response to the COVID-19 pandemic, a general lockdown was enacted across Germany in March 2020. As a consequence, patients with mental health conditions received limited or no treatment in day hospitals and outpatient settings. To ensure continuity of care, the necessary technological preparations were made to enable the implementation of telemedical care via telephone or video conferencing, and this option was then used as much as possible. The aim of this study was to investigate the satisfaction and acceptance with telemedical care in a heterogeneous patient group of psychiatric outpatients in Germany during the first COVID-19 lockdown.
Methods
In this observational study, patients in ongoing or newly initiated outpatient psychiatric therapy as well as those who had to be discharged from the day clinic ahead of schedule received telemedical treatment via telephone. Data collection to assess the patients’ and therapists’ satisfaction with and acceptance of the telemedical care was adjusted to the treatment setting.
Results
Of 60 recruited patients, 57 could be included in the analysis. 51.6% of the patients and 52.3% of their therapists reported that the discussion of problems and needs worked just as well over the phone as in face-to-face consultations. In the subgroup of patients who were new to therapy due to being discharged from hospital early, acceptance was higher and telemedicine was rated as equally good in 87.5% of contacts. Both patients and therapists felt that telemedicine care during lockdown was an alternative for usual therapy in the outpatient clinic and that the option of telemedicine care should continue for the duration of the coronavirus pandemic.
Discussion
The results show a clear trend towards satisfaction with and acceptance of telemedicine care in a heterogeneous group of unselected psychiatric patients. Although the number of patients is small, the results indicate that the mostly positive results of telemedicine concepts in research projects can probably be transferred to real healthcare settings.
Conclusions
Telemedicine can be employed in healthcare for psychiatric patients either an alternative treatment option to maintain continuity of care or as a potential addition to regular care.

Seit 2018 ist mit IDx-DR ein Verfahren auf dem Markt, welches den Grad der diabetischen Retinopathie (DR) mittels künstlicher Intelligenz (KI) bestimmt.
Methoden
Wir haben IDx-DR in die Sprechstunde an einer diabetologischen Schwerpunktklinik integriert und berichten über die Übereinstimmung zwischen IDx-DR (IDx Technologies Inc., Coralville, IA, USA) und Funduskopie sowie IDx-DR und ophthalmologischer Bildbeurteilung sowie über den Einfluss unterschiedlicher Kamerasysteme.
Ergebnisse
Mit der Topcon-Kamera (n = 456; NW400, Topcon Medical Systems, Oakland, NJ, USA) konnte im Vergleich zur Zeiss-Kamera (n = 47; Zeiss VISUCAM 500, Carl Zeiss Meditec AG, Jena, Deutschland) häufiger eine ausreichende Bildqualität in Miosis erreicht werden. Insgesamt war bei etwa 60 % der Patienten eine IDx-DR-Analyse in Miosis möglich. Alle Patienten, bei denen keine IDx-DR-Analyse in Miosis möglich war, konnten in Mydriasis funduskopiert werden. Innerhalb der Gruppe der auswertbaren Befunde zeigte sich eine Übereinstimmung zwischen IDx-DR und augenärztlicher Funduoskopie in ca. 55 %, ein Überschätzen des Schweregrads durch IDx-DR in ca. 40 % und ein Unterschätzen in ca. 4 %. Die Sensitivität (Spezifität) für das Erkennen einer schweren, behandlungsbedürftigen Retinopathie lag bei 95,7 % (89,1 %) für Fälle mit auswertbaren Fundusaufnahmen und bei 65,2 % (66,7 %), wenn alle Fälle betrachtet werden (inklusive derjeniger ohne verwertbare Aufnahme in Miosis). Der Kappa-Koeffizient zeigt mit 0,334 (p < 0,001) eine ausreichende Übereinstimmung zwischen IDx-DR und ärztlicher Bildauswertung anhand des Fundusfotos unter Berücksichtigung aller Patienten mit auswertbarer IDx-DR-Analyse. Der Vergleich zwischen IDx-DR mit der ärztlichen Funduskopie ergibt unter denselben Voraussetzungen eine geringe Übereinstimmung mit einem Kappa-Wert von 0,168 (p < 0,001).
Schlussfolgerung
Die vorliegende Studie zeigt Möglichkeiten und Grenzen des KI-gestützten DR-Screenings auf. Eine wesentliche Einschränkung liegt in der Tatsache, dass bei ca. 40 % der Patienten keine ausreichenden Aufnahmen in Miosis gewonnen werden konnten. Wenn ausreichende Aufnahmen vorlagen, stimmten IDx-DR und augenärztliche Diagnose in über 50 % der Fälle überein. Ein Unterschätzen des Schweregrades durch IDx-DR kam selten vor. Für die Integration in augenärztlich unterstützten Sprechstunden erscheint uns das System grundsätzlich geeignet. Die hohe Rate an fehlenden Aufnahmen in Miosis stellt allerdings eine Limitation dar, die einen Einsatz ohne augenärztliche Kontrollmöglichkeit schwierig erscheinen lässt.
Background
Pregnancy and the postpartum period are times when women are at increased risk for depression and mental problems. This may also negatively affect the foetus. Thus, there is a need for interventions with low-threshold access and care. Telemedicine interventions are a promising approach to address these issues. This systematic literature review examined the efficacy of telemedicine interventions for pregnant women and/or new mothers to address mental health-related outcomes. The primary objective was to analyse whether telemedicine interventions can reduce mental health problems in pregnant women and new mothers. The secondary aim was to clarify the impact of type of interventions, their frequency and their targets.
Methods
Inclusion criteria: randomized controlled trials, with participants being pregnant women and/or new mothers (with infants up to twelve months), involving telemedicine interventions of any kind (e.g. websites, apps, chats, telephone), and addressing any mental health-related outcomes like depression, postnatal depression, anxiety, stress and others. Search terms were pregnant women, new mothers, telemedicine, RCT (randomised controlled trials), mental stress as well as numerous synonyms including medical subject headings. The literature search was conducted within the databases PubMed, Cochrane Library, Web of Science and PsycINFO. Screening, inclusion of records and data extraction were performed by two researchers according to the PRISMA guidelines, using the online tool CADIMA.
Results
Forty four articles were included. A majority (62%) reported significantly improved mental health-related outcomes for participants receiving telemedicine interventions compared to control. In particular (internet-delivered) Cognitive Behavioural Therapy was successful for depression and stress, and peer support improved outcomes for postnatal depression and anxiety. Interventions with preventive approaches and interventions aimed at symptom reduction were largely successful. For the most part there was no significant improvement in the symptoms of anxiety.
Conclusion
Telemedicine interventions evaluated within RCTs were mostly successful. However, they need to be designed to specifically target a certain mental health issue because there is no one-size-fits-all approach. Further research should focus on which specific interventions are appropriate for which mental health outcomes in terms of intervention delivery modes, content, target approaches, etc. Further investigation is needed, in particular with regard to anxiety.
Background: To analyze the relation and distribution of mean, systolic and diastolic ocular perfusion pressure (OPP) in telemedical homemonitoring of patients with primary open-angle glaucoma (POAG). Methods: 70 patients with POAG measured intraocular pressure (IOP) and blood pressure at home for a period of 6 months with the Goldmann applanation self-tonometer Ocuton S and the blood pressure device boso medicus PC. Twenty-four-hour profiles were taken every 4 weeks in addition to single measurements in the morning and evening once a week. All measured values were transmitted to an electronic patient record, which calculated OPP by taking systolic, diastolic and mean arterial blood pressure and subtracting IOP. Results: We analyzed 3,282 values of mean, systolic and diastolic OPP. The quantity of values below the risk levels of the Barbados Eye Studies was calculated. We found values lower than the risk levels for LE: 49 (1.5%)/RE: 60 (1.8%) systolic OPP, LE: 1,623 (49.5%)/RE: 1,761 (53.7%) diastolic OPP and LE: 687 (20.9%)/RE: 794 (24.2%) mean OPP. The individual average OPP levels of all 70 patients below the risk levels showed the following distribution: LE: 4 (5.7%)/RE: 6 (8.6%) systolic OPP, LE: 19 (27.1%)/RE: 20 (28.6%) diastolic OPP and LE: 10 (14.3%)/RE: 10 (14.3%) mean OPP. Conclusion: The individual distribution of different OPP values in POAG patients is not easy to interpret for clinical ophthalmologists. Precise practicable guidelines for clinical use still have to be determined.
Background: A telemedicine care concept based on telephone contacts and individualized text messages was developed for patients with mental disorders to continue treatment after therapy in a psychiatric day hospital. The primary objective of this study was to evaluate the effectiveness of the telemedicine interventions. Methods: The study had a 3-armed, randomized design with 2 intervention arms (intervention 1: telephone contacts; intervention 2: telephone contacts and short text messages; both took place over a period of 6 months and in addition to usual care), and a control group with usual care. Primary outcomes were 18-item Brief Symptom Inventory (BSI-18) scores for anxiety, depression and somatization. All participants were recruited from psychiatric day hospitals. The study was registered in the German Clinical Trials Register (DRKS00000662). Results: 113 participants were analyzed 6 months after starting the intervention. The average BSI-18 anxiety score after 6 months was -2.04 points lower in intervention group 2 than in the control group (p value: 0.042). The difference in BSI depression score between these two groups was marginally significant (p value: 0.1), with an average treatment effect of -1.73. In an exploratory sensitivity analysis restricted to the 75% of patients with the highest symptom scores at baseline, intervention group 1 yielded a significant effect for anxiety and depression compared to the control group (p = 0.036 and 0.046, respectively). Conclusions: Telemedicine provides a novel option in psychiatric ambulatory care with statistically significant effects on anxiety. A positive tendency was observed for depression, especially in cases with higher symptom load at baseline.
Aufgrund steigender Patientenzahlen gewinnt die Palliativmedizin in den Industrienationen immer mehr an Bedeutung. Häufig wünschen die Patienten, in der Häuslichkeit gepflegt zu werden und auch dort zu versterben. Eine Versorgung der Palliativpatienten in der Häuslichkeit ist möglich und muss der Versorgung im Krankenhaus um nichts nachstehen. Die ambulante Versorgung dieser Patienten stößt, gerade in dünn besiedelten Gebieten mit starken strukturassoziierten Veränderungen wie im Landkreis Ostvorpommern, auf Grenzen hinsichtlich regelmäßiger Hausbesuche von medizinischem Personal, insbesondere durch niedergelassene Ärzte. Durch eine randomisierte, prospektive Studie wurde geprüft, ob sich durch eine telemedizinische Intervention mit Hilfe regelmäßiger Telefonanrufe ein Vorteil der Versorgung für diese Patienten ergibt. Außerdem wurde geprüft, ob das Studiendesign durchführbar und machbar war. In der vorliegenden Pilotstudie konnte die Patientenzielgruppe erreicht werden. Die Patientenrekrutierung erwies sich als schwierig. Außerdem zeigte sich eine hohe Schmerz- und Symptombelastung der Projektpopulation. Die zusätzlichen Telefonanrufe wurden von den Patienten positiv bewertet. Es konnte kein signifikanter Unterschied zwischen der Patientengruppe, die regelmäßige Telefonanrufe erhalten hat, und der Gruppe, die nur die übliche palliativmedizinische Betreuung erhalten hat, hinsichtlich der Entwicklung der Schmerzen, der Lebensqualität, der Symptome, der Mobilität, der Aktivität, der Arztbesuche oder der Krankenhausverweildauer zur Base- Line und zur Follow- Up Befragung festgestellt werden. Bei einigen Interventionspatienten konnte eine temporäre Schmerzreduktion nach einer Intervention in Form einer telefonischen Umstellung der Schmerzmedikation erreicht werden. Es gibt Hinweise darauf, dass die Patienten, die regelmäßige Telefonanrufe erhalten haben, die Schmerzbedarfsmedikation als Selbstmedikation gezielt einsetzen konnten. Nach heutigem Kenntnisstand ist dies die erste Studie, die den Effekt von regelmäßigen Telefonanrufen zur Betreuung von in der Häuslichkeit gepflegten Palliativpatienten untersucht hat. Eine analoge Studie, durchgeführt bei chronischen Schmerzpatienten, bestätigt diese Ergebnisse. Auch hier erwiesen sich regelmäßige Telefonanrufe als wirkungsvoll, da eine signifikante Schmerzreduktion in der telemedizinisch betreuten Gruppe erreicht werden konnte.
