Background: Stroke patients are at risk of acquiring secondary infections due to stroke-induced immune suppression (SIIS). Immunosuppressive cells comprise myeloid-derived suppressor cells (MDSCs) and immunosuppressive interleukin 10 (IL-10)-producing monocytes. MDSCs represent a small but heterogeneous population of monocytic, polymorphonuclear (or granulocytic), and early progenitor cells (“early” MDSC), which can expand extensively in pathophysiological conditions. MDSCs have been shown to exert strong immune-suppressive effects. The role of IL-10-producing immunosuppressive monocytes after stroke has not been investigated, but monocytes are impaired in oxidative burst and downregulate human leukocyte antigen—DR isotype (HLA-DR) on the cell surface.
Objectives: The objective of this work was to investigate the regulation and function of MDSCs as well as the immunosuppressive IL-10-producing monocytes in experimental and human stroke.
Methods: This longitudinal, monocentric, non-interventional prospective explorative study used multicolor flow cytometry to identify MDSC subpopulations and IL-10 expression in monocytes in the peripheral blood of 19 healthy controls and 27 patients on days 1, 3, and 5 post-stroke. Quantification of intracellular STAT3p and Arginase-1 by geometric mean fluorescence intensity was used to assess the functionality of MDSCs. In experimental stroke induced by electrocoagulation in middle-aged mice, monocytic (CD11b+Ly6G−Ly6Chigh) and polymorphonuclear (CD11b+Ly6G+Ly6Clow) MDSCs in the spleen were analyzed by flow cytometry.
Results: Compared to the controls, stroke patients showed a relative increase in monocytic MDSCs (percentage of CD11b+ cells) in whole blood without evidence for an altered function. The other MDSC subgroups did not differ from the control. Also, in experimental stroke, monocytic, and in addition, polymorphonuclear MDSCs were increased. The numbers of IL-10-positive monocytes did not differ between the patients and controls. However, we provide a new insight into monocytic function post-stroke since we can report that a differential regulation of HLA-DR and PD-L1 was found depending on the IL-10 production of monocytes. IL-10-positive monocytes are more activated post-stroke, as indicated by their increased HLA-DR expression.
Conclusions: MDSC and IL-10+ monocytes can induce immunosuppression within days after stroke.
Der aktuelle demografische Wandel in Deutschland zeigt eine erhöhte Lebenserwartung und damit einen Anstieg an altersassoziierten Erkrankungen wie dem Schlaganfall. Eine mögliche Folge ist die Armparese, welche eine gravierende Behinderung bei der Ausführung alltäglicher Handlungen darstellt. Dadurch kommt der motorischen Rehabilitation mit dem Ziel der Wiederherstellung der Alltagstauglichkeit eine besonders wichtige Rolle zu.
Unter zahlreichen Therapiekonzepten ist für das Arm-Fähigkeits-Training (AFT), welches einzeln verschiedene sensomotorische Armfähigkeiten anspricht und das motorische Lernen induziert, eine gute Wirksamkeit belegt.
In Studien konnte gezeigt werden, dass eine nicht-invasive Hirnstimulation in Form einer repetitiven transkraniellen Magnetstimulation (rTMS), genauer der intermittierenden Theta-Burst-Stimulation (iTBS), vorübergehend die lokal kortikale Erregbarkeit des stimulierten Areals erhöhen (Huang et al., 2005) und dadurch gegebenenfalls auch das nachfolgende trainingsinduzierte Lernen beeinflussen kann. Das Wissen über mögliche „Priming“-Effekte von iTBS auf das motorische Lernen bei Gesunden kann helfen, zielgerichtete therapeutische Anwendungen für Patienten nach einem Schlaganfall zu entwickeln.
Ziel dieser Untersuchung war es festzustellen, ob das exzitatorische „Priming“ mit iTBS über dem primären motorischen Kortex (M1) oder dem primären somatosensorischen Kortex (S1) unmittelbar vor einer täglichen Trainingseinheit mit AFT (über vier Tage) für den linken Arm bei gesunden rechtshändigen Probanden die sensomotorische Lerndynamik verbessern kann.
Zu diesem Zweck wurde ein Training des linken, nicht-dominanten Arms von 18 jungen und gesunden Probanden mithilfe von acht unterschiedlichen motorischen Aufgaben (AFT) einmal pro Tag für insgesamt fünf Tage durchgeführt. Mit Ausnahme des ersten Tages (Baseline) erfolgte das Training nach der Applikation einer exzitatorischen Form der repetitiven transkraniellen Magnetstimulation (iTBS). Die Stimulation wurde je nach randomisierter Gruppenzuordnung entweder über M1 oder S1 rechts oder als Sham-Stimulation, um einen möglichen Placebo-Effekt auszuschließen, über M1 rechts durchgeführt.
Die Hauptkomponentenanalyse der Daten zum motorischen Verhalten ergab acht
unabhängige motorische Komponenten, die den acht trainierten Aufgaben entsprachen. AFT induzierte motorisches Lernen über alle Fähigkeiten hinweg mit einem Generalisationseffekt auf eine nicht-trainierte Aufgabe der Fingergeschicklichkeitm(Nine-Hole-Peg-Test,ccNHPT).
Probanden, die iTBS (entweder über M1 oder S1) erhielten, zeigten im Vergleich zur Sham-Stimulation sowohl eine bessere Leistung bei den AFT-Aufgaben während der Trainingsdauer als auch eine größere Verbesserung der nicht-trainierten Fingergeschicklichkeitsaufgabe (NHPT) für den trainierten linken Arm nach Trainingsende.
Daraus resultiert, dass die exzitatorische repetitive transkranielle Magnetstimulation in Form von iTBS über M1 oder S1 das motorische Lernen über verschiedene sensomotorische Fähigkeiten hinweg verbessern kann.
Auch wenn die verstärkenden Effekte eines exzitatorischen „Priming“ absolut gesehen klein waren, so geben sie dennoch Grund zur Annahme, dass darin auch ein therapeutisches Potenzial für die Armrehabilitation nach Schlaganfall liegt. Ob das so ist, wäre jedoch mit geeigneten klinischen Studien zu untersuchen.
Die Ergebnisse des Promotionsvorhabens wurden in einer Peer-Review-Zeitschrift publiziert (Platz et al., 2018a).
In der akuten Schlaganfallbehandlung nehmen die endovaskulären Revaskularisationsverfahren, insbesondere die intraarterielle Thrombolyse, einen wachsenden Stellenwert ein. Grundlage dafür ist die zentrumsspezifische Sicherheit und Durchführbarkeit, wobei nach den Zertifizierungskriterien der Deutschen Schlaganfallgesellschaft die intraarterielle Thrombolyse nur in zertifizierten überregionalen Stroke Units erfolgen sollte. Weiterhin existieren nur wenige Selektionsparameter zur Abschätzung des Risikos der intrakraniellen Blutung als wichtigste Komplikation gegenüber dem potenziellen Benefit der Behandlung. Ziel dieser Arbeit war 1. die Etablierung eines klinisch praktikablen Protokolls zur Entscheidung für eine intravenöse oder intraarterielle Thrombolyse sowie die systematische Untersuchung der zentrumsspezifischen Sicherheit und Durchführkeit der intraarteriellen Thrombolyse; 2. die Untersuchung des Einflusses der bekannten klinischen und radiologischen Faktoren, wie Zeit bis zur Thrombolyse oder Rekanalisation im vorliegenden Patientenkollektiv auf das neurologische Outcome; und 3. die Untersuchung der Perfusions-CT und der CT-Angiographie als Grundlage für weitere mögliche Selektionsparameter zur intraarteriellen Thrombolyse. Zwischen 02/2006 und 12/2008 konnten 26 Patienten eingeschlossen werden. Ein Patient verstarb innerhalb von 90 Tagen nach Schlaganfall aufgrund eines erneuten cerebrovaskulären Ereignisses. Die Rate der symptomatischen und asymptomatischen intrakraniellen Blutungen (8% bzw 8%) sowie die Rate der periprozeduralen Komplikationen sind trotz der eingeschränkten Vergleichbarkeit mit der Literatur als gleichwertig zu bewerten. Die Effektivitätsparameter Zeit bis Thombolyse (Durchschnitt +/- Standardabweichung: 4,03 ± 1,2), Rekanalisationsrate (73%) sowie das Ausmaß des guten funktionellen Outcome 90 Tage nach Schlaganfall (mRS <=2; 58%) sind ähnlich oder teilweise sogar besser als die in der Literatur veröffentlichen Ergebnisse für die intraarterielle und intravenöse Thrombolyse. Somit ist die intraarterielle Thrombolyse in dieser Studie als sicher und effektiv zu bewerten. Die Zeit bis zur Thrombolyse zeigte sich nicht signifikant mit dem neurologischen Outcome assoziiert, jedoch ergab sich ein Trend zu schlechterem Outcome mit größerer zeitlicher Latenz zwischen Symptom- und Therapiebeginn. Weiterhin konnte die Rekanalisaton als Prädiktor für das neurologische Outcome und die Infarktgröße bestätigt werden. Der prädiktive Wert des Ausmaßes der Kollateralen in der CT-Angiographie mittels eines von Tan et al. entwickelten Score für das finale Infarktvolumen konnte in diesem homogenen, hochselektionieren Patientengut bestätigt werden. Das Ausmaß der Kollateralen als ein neuer Selektionsparameter für die Thrombolyse sollte weiter detailiert untersucht werden. Weiterhin konnte nachgewiesen werden, dass mittels Perfusions-CT auf das kurzfristige Outcome mittels NIHSS zwischen Aufnahme und Entlassung geschlossen werden kann. Hierbei zeigte sich, dass der im Rahmen dieser Studie entwickelte Auswertealgorithmus der relativen Perfusions-CT-Parameter und der schon verwendete Algorithmus der ASPECTS-Perfusion, bezüglich der Vorhersagekraft des cerebralen Blutvolumens (CBV) gleichwertig sind, jedoch die relativen Perfusions-Parameter eine wesentliche bessere Interrater-Reliabiltät aufweisen. Bezüglich des cerebralen Blutflusses (CBF) ergab sich nach dem ASPECTS-Perfusions-Algorithmus keine Assoziation mit dem neurologischen Outcome. Bei den relativen Perfusionsparametern erreichte jedoch das CBF eine dem CBV vergleichbare Vorhersagefähigkeit gepaart mit einer besseren Interrater-Reliabilität als das CBV. Das aktuell weit verbreitete visuelle Abschätzen der Größe des tissue at risk zeigte weder eine Assoziation zum neurologischen Outcome nach intraarterieller Thrombolyse noch eine gute Interratervariabilität.
Objective: The study aimed to test the reliability of a semi-structured telephone interview for the classification of headache disorders according to the ICHD-3.
Background: Questionnaire-based screening tools are often optimized for single primary headache diagnoses [e.g., migraine (MIG) and tension headache (TTH)] and therefore insufficiently represent the diagnostic precision of the ICHD-3, which limits epidemiological research of rare headache disorders. Brief semi-structured telephone interviews could be an effective alternative to improve classification.
Methods: A patient population representative of different primary and secondary headache disorders (n = 60) was recruited from the outpatient clinic (HSA) of a tertiary care headache center. These patients completed an established population-based questionnaire for the classification of MIG, TTH, or trigeminal autonomic cephalalgia (TAC). In addition, they received a semi-structured telephone interview call from three blinded headache specialists individually. The agreement of diagnoses made either using the questionnaires or interviews with the HSA diagnoses was evaluated.
Results: Of the 59 patients (n = 1 dropout), 24% had a second-order and 5% had a third-order headache disorder. The main diagnoses were as follows: frequent primary headaches with 61% MIG, 10% TAC, 9% TTH, and 5% rare primary and 16% secondary headaches. Second-order diagnosis was chronic migraine throughout, and third-order diagnoses were medication overuse headache and TTH. Agreement between main headaches from the HSA was significantly better for the telephone interview than for the questionnaire (questionnaire: κ = 0.330; interview: κ = 0.822; p < 0.001). Second-order diagnoses were not adequately captured by questionnaires, while there was a trend for good agreement with the telephone interview (κ = 0.433; p = 0.074). Headache frequency and psychiatric comorbidities were independent predictors of HSA and telephone interview agreement. Male sex, headache frequency, severity, and depressive disorders were independently predictive for agreement between the questionnaire and HSA. The telephone interview showed high sensitivity (≥71%) and specificity (≥92%) for all primary headache disorders, whereas the questionnaire was below 50% in either sensitivity or specificity.
Conclusion: The semi-structured telephone interview appears to be a more reliable tool for accurate diagnosis of headache disorders than self-report questionnaires. This offers the potential to improve epidemiological headache research and care even in underserved areas.
The combination of repeated behavioral training with transcranial direct current stimulation (tDCS) holds promise to exert beneficial effects on brain function beyond the trained task. However, little is known about the underlying mechanisms. We performed a monocenter, single-blind randomized, placebo-controlled trial comparing cognitive training to concurrent anodal tDCS (target intervention) with cognitive training to concurrent sham tDCS (control intervention), registered at ClinicalTrial.gov (Identifier NCT03838211). The primary outcome (performance in trained task) and secondary behavioral outcomes (performance on transfer tasks) were reported elsewhere. Here, underlying mechanisms were addressed by pre-specified analyses of multimodal magnetic resonance imaging before and after a three-week executive function training with prefrontal anodal tDCS in 48 older adults. Results demonstrate that training combined with active tDCS modulated prefrontal white matter microstructure which predicted individual transfer task performance gain. Training-plus-tDCS also resulted in microstructural grey matter alterations at the stimulation site, and increased prefrontal functional connectivity. We provide insight into the mechanisms underlying neuromodulatory interventions, suggesting tDCS-induced changes in fiber organization and myelin formation, glia-related and synaptic processes in the target region, and synchronization within targeted functional networks. These findings advance the mechanistic understanding of neural tDCS effects, thereby contributing to more targeted neural network modulation in future experimental and translation tDCS applications.
Introduction
Given rapid global population aging, developing interventions against age-associated cognitive decline is an important medical and societal goal. We evaluated a cognitive training protocol combined with transcranial direct current stimulation (tDCS) on trained and non-trained functions in non-demented older adults.
Methods
Fifty-six older adults (65–80 years) were randomly assigned to one of two interventional groups, using age and baseline performance as strata. Both groups performed a nine-session cognitive training over 3 weeks with either concurrent anodal tDCS (atDCS, 1 mA, 20 minutes) over the left dorsolateral prefrontal cortex (target intervention) or sham stimulation (control intervention). Primary outcome was performance on the trained letter updating task immediately after training. Secondary outcomes included performance on other executive and memory (near and far transfer) tasks. All tasks were administered at baseline, post-intervention, and at 1- and 7-month follow-up assessments. Prespecified analyses to investigate treatment effects were conducted using mixed-model analyses.
Results
No between-group differences emerged in the trained letter updating and Markov decision-making tasks at post-intervention and at follow-up timepoints. Secondary analyses revealed group differences in one near-transfer task: Superior n-back task performance was observed in the tDCS group at post-intervention and at follow-up. No such effects were observed for the other transfer tasks. Improvements in working memory were associated with individually induced electric field strengths.
Discussion
Cognitive training with atDCS did not lead to superior improvement in trained task performance compared to cognitive training with sham stimulation. Thus, our results do not support the immediate benefit of tDCS-assisted multi-session cognitive training on the trained function. As the intervention enhanced performance in a near-transfer working memory task, we provide exploratory evidence for effects on non-trained working memory functions in non-demented older adults that persist over a period of 1 month.
Most children use their fingers when learning to count and calculate. These sensorimotor experiences were argued to underlie reported behavioral associations of finger gnosis and counting with mathematical skills. On the neural level, associations were assumed to originate from overlapping neural representations of fingers and numbers. This study explored whether finger-based training in children would lead to specific neural activation in the sensorimotor cortex, associated with finger movements, as well as the parietal cortex, associated with number processing, during mental arithmetic. Following finger-based training during the first year of school, trained children showed finger-related arithmetic effects accompanied by activation in the sensorimotor cortex potentially associated with implicit finger movements. This indicates embodied finger-based numerical representations after training. Results for differences in neural activation between trained children and a control group in the IPS were less conclusive. This study provides the first evidence for training-induced sensorimotor plasticity in brain development potentially driven by the explicit use of fingers for initial arithmetic, supporting an embodied perspective on the representation of numbers.
202 Patienten (113 Frauen und 89 Männer im Alter von 25 bis 95 Jahren) mit einem ischämischen, supratentoriellem und territorialen Schlaganfall vorwiegend im Mediastromgebiet, wurden 9/2007 bis 6/2012 aus dem Patientengut der Stroke Unit der Neurologischen Klinik der Universitätsmedizin Greifswald für die monozentrische Studie zur Häufigkeit epileptischer Anfälle sowie ihrer Provokationsfaktoren rekrutiert.
Bei einem durchschnittlichen Beobachtungszeitraum von 36,5 Monaten erlitten 39 Patienten (19,3%) mindestens einen epileptischen Anfall. Davon hatten 12 Patienten einen akut symptomatischen Anfall und 27 einen unprovozierten Anfall. Bei 21 Patienten traten rezidivierende Anfälle auf. Wendet man die aktualisierte Definition der ILAE für Epilepsie an, so hatten 31 (15,3%) Patienten eine Epilepsie und 8 (4%) einen einzelnen akutsymptomatischen Anfall. Damit hatte unsere Studie die bisher höchste dokumentierte Rate an Epilepsie nach einer Ischämie. Der erste epileptische Anfall trat dabei überwiegend im 1. Jahr nach erlittenem Schlaganfall auf. Es zeigte sich, dass Patienten mit einer geringeren Schwere des Schlagfalls (erfasst mittels NIHSSS und mRS) ein geringeres Risiko für die Entwicklung eines epileptischen Anfalls hatten. Alter und Geschlecht zeigten keine Korrelation zum Auftreten eines epileptischen Anfalls.
96 Patienten (53 Frauen und 43 Männer) erhielten innerhalb der ersten 6 h nach dem Schlaganfall eine auswertbare PCT, davon hatten 17 (17,7%) mindestens einen epileptischen Anfall. Mit Hilfe der PCT wurden für die einzelnen Perfusionsparameter CBF, CBF und TTP der ASPECTS, das Perfusionsdefizit und die relativen Perfusionsparameter bestimmt. Bei dem ASPECTS CBF und ASPECTS CBV zeigte sich, dass die vorderen Mantelregionen M1 und M4 bzw. nur M1 bei den Patienten mit einem epileptischen Anfall signifikant häufiger einen geringeren CBF oder ein geringeres CBV hatten. Ein signifikanter Unterschied ergab sich auch beim Betrachtung der Perfusionsdefizite in Bezug auf den CBF und das CBV: Patienten mit einem epileptischer Anfall zeigten ein größeres Perfusionsdefizit als die Patienten ohne epileptischen Anfall. In die relativen Perfusionsparameter fließt neben dem Perfusionsdefizit noch die Infarktgröße ein. Zwar ergab sich kein signifikanter Unterschied zwischen den beiden Gruppen in Bezug auf die Infarktgröße, trotzdem konnte eine Korrelation zwischen einem erniedrigten R[CBF] bzw. einem erniedrigten R[CBV] und dem Auftreten von epileptischen Anfällen nach einem ischämischen Schlaganfall feststellt werden.
Studies of stroke in experimental animals have demonstrated the neuroprotective efficacy of a variety of interventions; however, most such strategies have failed to show clinical benefits in aged humans. One possible explanation for this discrepancy between animal and clinical studies may be the role that age plays in the recovery of the brain following insult. For example, the poor functional recovery of aged rats after stroke may be caused by a decline in brain plasticity. Although the incidence of ischemic stroke increases dramatically with advancing age, relatively few studies have been conducted on aged animals, which would mimic most closely the context in which stroke occurs in humans. We have shown that, at one week following stroke, there was vigorous expression of MAP1B and its mRNA, as well as MAP2 protein, in the border zone adjacent to the infarct of 3 month- and 20 month-old male Sprague Dawley rats. Hypothesis: The decline in brain plasticity is caused by an age-related decline in the upregulation of factors promoting brain plasticity (MAP1B, ßAPP) and an age-related increase in astroglial scaring and in the expression of neurotoxins such as beta amyloid. Methods: Focal cerebral ischemia was produced by reversible occlusion of the right middle cerebral artery in 3- and 20-month-old male Sprague Dawley rats. The functional outcome was assessed in neurobehavioral tests at 3, 7, 14, and 28 days post-stroke. At these time points, brains were removed and analyzed for markers of (i) brain plasticity (microtubule-associated protein 1B, MAP1B, secreted forms of fi-amyloid precursor protein); (ii) neurogenesis (BrdU-positive cells, doublecortin, nestin); (iii) neurotoxicity (B-amyloid aggregates); (iv) inflammation (microglia, astrocytes, oligodendrocytes, endothelial cells). Results: (1) There was a non-significant tendency for blood pressure to be higher in old than in young rats. By post-stroke day 3 the infarct volume covered about 15% of the cortical neurons in young and 28% in aged rats. By day 7, infarct volumes were roughly equal in the two age groups. (2) Cell counting showed increases in the number of BrdU-positive cells in the infarcted area of old rats at day 3 post-stroke. This increase became even more dramatic at day 7 post-stroke in aged rats. There was no significant contribution of apoptosis to cell death. (3) Behaviorally, young rats recovered gradually and reached a maximum of 90% of baseline performance at day 14, post-stroke while the aged rats recovered only to a maximum of 70% of pre-surgery performance by week 2 post-stroke, and remained at that level. (4) The temporal pattern of recovery correlated well with the expression of growth-associated phenotype of ßAPP as well as with MAP1B accumulation in varicosities along axons (an indicator of growth) in cortical areas affected by stroke and was at maximum between days 14 to 28 in young rats. In contrast, aged rats showed delayed (day 28) and reduced axonal remodelling as well as a delayed (day 28) expression of growth-associated ßAPP. Instead, the neurotoxic carboxy-terminal form of ßAPP steadily accumulated over time and reached a maximum at day 14 in aged rats as compared to 28d for the young rats. Nestin, a marker for immature neurons, overlapped with BrdU-labelled cells at day 7 post-stroke in corpus callosum and at the infarct border in both young and aged rats, suggesting increased stroke-induced neurogenesis. (5) In young rats there was a gradual activation of both microglia and astrocytes that peaked by days 14 to 28 with the formation of a glial scar. In contrast, aged rats showed an accelerated astrocytic and microglial reaction that peaked in week 1 post-stroke. We also noted a strong activation of oligodendrocytes at early stages of infarct development in all rats that persisted in aged rats. Evolution of astrocytic and microglial reactivity closely paralled the time course of scar formation in both young and aged rats and coincided with the stagnation in the recovery rate of aged rats. Conclusions: The time course of functional recovery in young rats correlated well with the expression of plasticity proteins such as MAP1B and ßAPP while an early and persistent expression of the neuro toxic fragment AB in conjunction with a delayed expression of MAP1B and ßAPP may impede functional recovery in aged rats. The results also suggest that a temporally anomalous glial reaction to cerebral ischemia in aged rats leads to the premature formation of scar tissue that impedes functional recovery to stroke.
In dieser Arbeit wird die Hypothese getestet, dass durch H2S-induzierte Hibernation eine Reduktion des Infarktvolumens nach Schlaganfall, sowie eine Verbesserung der funktionellen Erholung erreicht wird. Hierzu wird im Post-Akut-Tiermodell des Schlaganfalls an alten Tieren eine 48-stündige Ganzkörper-Hypothermie induziert. An diesen Tieren wird dann die funktionelle Erholung nach einem Schlaganfall ermittelt sowie das Infarktvolumen gemessen. Daraus könnten sich neue Behandlungsstrategien für Schlaganfallpatienten ableiten lassen. Wir können demonstrieren, dass gealterte Ratten nach einem Schlaganfall eine 50%ige Reduktion der Infarktgröße aufweisen, wenn ihre Körpertemperatur für 48 Stunden mittels H2S gesenkt wird ohne offensichtliche zusätzliche neurologische Defizite oder physiologische Nebenwirkungen zu produzieren. Gleichzeitig verbessert sich die Leistung der Tiere in Versuchen, die komplex-motorische Funktionen erfordern. Des weiteren zeigt sich in unseren Experimenten, dass die Anzahl der Makrophagen-ähnlichen Zellen, sowie die mRNA Expression der Marker für Inflammation und Apoptose NF kappa B, grp78 und caspase 12 in den Versuchstieren deutlich niedriger ist als in den Kontrolltieren und dass bei den Hypothermie-Tieren der Infarktkern deutlich weniger apoptotische Zellen aufweist als in den Kontrollen. Unsere Ergebnisse zeigen, dass die Hypothermie signifikantes Potential bei der Behandlung des Schlaganfalls im Nager hat und weitere Untersuchungen zu diesem Thema in klinischen Studien durchgeführt werden sollten.
Vaskuläre Erkrankungen stellen in Deutschland und in anderen industrialisierten Ländern die häufigsten Todesursachen dar. Neben traditionellen Risikofaktoren der Atherosklerose wie Hypertonus, Diabetes mellitus, Hypercholesterinämie und Nikotinabusus, rückten in den vergangenen Jahren zunehmend genetische Faktoren der Atherosklerose in den Mittelpunkt des wissenschaftlichen Interesses. Pathophysiologisch spielt das Renin-Angiotensin-System (RAS) eine wichtige Rolle bei der Entstehung der Atherosklerose. Die Aktivität des zirkulierenden und wahrscheinlich auch des gewebsständigen ACE steht unter einer deutlichen genetischen Kontrolle. Dabei variiert die Enzymaktivität zwischen den einzelnen Genotypen um den Faktor 20 bis 40. Das menschliche ACE-Gen ist auf Chromosom 17q23 lokalisiert und weist einen Insertions/Deletions-Polymorphismus auf. Das Ziel der vorliegenden Arbeit war es festzustellen, ob in der Basisstudie der Study of Health in Pomerania (SHIP-0), einer populationsbezogenen Querschnittsstudie im Nordosten Deutschlands, eine Assoziation zwischen dem ACE I/D-Polymorphismus und atherosklerotischen Gefäßwandläsionen besteht. Hierzu wurden sowohl subklinische Risikomarker wie die Intima-Media-Dicke (IMT) in der A. carotis als auch manifeste atherosklerotische Veränderungen wie Plaques und Stenosen untersucht. Von den 4310 Teilnehmern der SHIP-0-Studie wurde bei 4262 Personen (98,9%) der ACE-Polymorphismus bestimmt. Die gentechnische Analyse der DNA ergab, dass 1031 Personen (23,9%) den Genotyp II besitzen, 2131 Personen (49,4%) haben den Genotyp ID und 1100 (25,5%) der Untersuchten zeigen den Genotyp DD. Insgesamt konnten in der Gesamtpopulation keine signifikanten Assoziationen zwischen dem ACE-Polymorphismus und der IMT nachgewiesen werden. Es fanden sich ebenfalls keine signifikanten Zusammenhänge zwischen dem Polymorphismus und klinischen Endpunkten wie vaskulären Erkrankungen. In einem weiteren Schritt der Studie wurden Subgruppenanalysen durchgeführt. Ausgeschlossen wurden Diabetiker und Raucher, um zu verhindern, dass diese Risikofaktoren der Atherosklerose den Effekt des ACE-Polymorphismus maskieren. Dabei zeigte sich, dass Stenosen der A. carotis (> 50%) bei männlichen Teilnehmern mit dem DD-Genotyp signifikant häufiger nachweisbar waren als bei Teilnehmern mit dem II-Genotyp. Die Regressionsanalyse in dieser Subgruppe zeigte eine positive Assoziation zwischen dem D-Allel und dem Auftreten von Stenosen der A. carotis, mit einem 2,9fach (homozygote D-Allel-Träger) bzw. 1,2fach (heterozygote D-Allel-Träger) erhöhtem Risiko. Einschränkend ist jedoch anzumerken, dass diese Subgruppenanalysen als explorative Pilotstudien zu verstehen sind, die das Ziel besitzen, Hypothesen zu formulieren. Sie sind aufgrund nicht ausreichender statistischer Trennschärfe (so genannte statistische Power) nicht geeignet, potentielle Assoziationen zu überprüfen. Die Ergebnisse der SHIP-Studie stehen somit im Einklang mit vier weiteren mittlerweile publizierten großen epidemiologischen Studien, die ebenfalls keine Assoziationen zwischen dem ACE-Genpolymorphismus und der IMT bzw. atherosklerotischen Plaques der A. carotis nachweisen konnten. Dennoch scheint eine, allerdings nur schwache Assoziation zwischen dem homozygoten DD-Genotyp des ACE-Polymorphismus und subklinischen atherosklerotischen Gefäßwandveränderungen zu bestehen, die ähnlich der Assoziation zwischen ACE I/D-Polymorphismus und Myokardinfarkten bzw. ischämischen Schlaganfällen nur in Meta-Analysen nachgewiesen werden kann.
Abstract
Head motion during magnetic resonance imaging (MRI) induces image artifacts that affect virtually every brain measure. In parallel, cross‐sectional observations indicate a correlation of head motion with age, psychiatric disease status and obesity, raising the possibility of a systematic artifact‐induced bias in neuroimaging outcomes in these conditions, due to the differences in head motion. Yet, a causal link between obesity and head motion has not been tested in an experimental design. Here, we show that a change in body mass index (BMI) (i.e., weight loss after bariatric surgery) systematically decreases head motion during MRI. In this setting, reduced imaging artifacts due to lower head motion might result in biased estimates of neural differences induced by changes in BMI. Overall, our finding urges the need to rigorously control for head motion during MRI to enable valid results of neuroimaging outcomes in populations that differ in head motion due to obesity or other conditions.
Background: Primary angiitis of the central nervous system (PACNS) is a rare but serious condition. A fraction of patients suffering from PACNS concurrently exhibit pronounced cerebral amyloid angiopathy (CAA) which is characterized by deposits of amyloid-β (Aβ) in and around the walls of small and medium-sized arteries of the brain. PACNS with CAA has been identified as a distinct disease entity, termed Aβ-related angiitis (ABRA). Evidence points to an immune reaction to vessel wall Aβ as the trigger of vasculitis. Objective: To investigate whether the inflammatory response to Aβ has (1) any effect on the status of immune activation in the brain parenchyma and (2) leads to clearance of Aβ from brain parenchyma. Methods: We studied immune activation and Aβ load by quantitative immunohistochemical analysis in brain parenchyma adjacent to affected vessels in 11 ABRA patients and 10 matched CAA controls. Results: ABRA patients showed significantly increased immune activation and decreased Aβ loads in the brain parenchyma adjacent to affected vessels. Conclusion: Our results are in line with the hypothesis of ABRA being the result of an excessive immune response to Aβ and show that this can lead to enhanced clearance of Aβ from the brain parenchyma by immune-mediated mechanisms.
Background: Intact socio-cognitive abilities, such as theory of mind (ToM), facial emotion recognition (FER), social decision making (SDM) and visual perspective taking (VPT), are essential for human well-being and quality of life. Impairment in social cognition can have major implications for health in affected individuals and society as a whole. Evidence for changes in social cognition in healthy and pathological aging processes, such as subjective cognitive decline (SCD) and mild cognitive impairment (MCI), is currently either sparse or inconclusive. It is important to determine how social cognition changes in healthy and pathological aging and provide grounds for targeted and early assessment and intervention. The aims of this thesis were to investigate social cognition across four domains, in particular, ToM, FER, SDM and VPT, in healthy young and older individuals, as well as in individuals with cognitive deficits, such as SCD and MCI. In the case of a decline, further goals were to investigate the degree of impairment and the domains affected.
Methods: A systematic literature search was conducted in four major academic databases, MEDLINE, Web of Science Core Collection, CENTRAL, and PsycInfo, for studies investigating social cognition in healthy young and old individuals as well as individuals affected by SCD and MCI which met the inclusion criteria. The primary outcome was ToM and secondary outcomes were FER, SDM and VPT. After a systematic review was performed, studies eligible for meta-analysis were divided according to comparison groups and outcomes. Random-effects meta-analyses were conducted using standardized mean differences (SMD). Risk of Bias was assessed using the “Tool to assess risk of bias in cohort studies” modified for the present study design.
Results: After a thorough systematic literature search, 86 studies containing 88 comparisons were included in the systematic review, of which 47 were eligible for quantitative analysis. The meta-analysis revealed a progressive decline in ToM and FER abilities from young adulthood to MCI. Varying effect sizes demonstrated different trajectories of change for specific domains. Due to a lack of research, data investigating SDM and VPT, as well as SCD were insufficient for quantitative analysis.
Conclusion: ToM and FER decline gradually from healthy to pathological aging. Therefore, assessment of social cognition is important and should be incorporated in routine neurocognitive testing, so that targeted interventions can be introduced when needed. With this information in mind, future research should focus on the development of new assessment tools, as well as preventive and treatment strategies. This review also identified research gaps in certain populations (e.g. SCD, middle age, MCI-subtypes) as well as domains (VPT and SDM) that need to be addressed in the future.
Polypharmacy in patients with multiple sclerosis and the impact on levels of care and therapy units

Background: The aim of this study was to examine the societal costs of polypharmacy in patients with multiple sclerosis (MS). We therefore focused on the association between the number of medications on the level of care (LOC), the German classification of the need for care, and the number of therapy sessions (TTU).
Methods: In addition to demographic information and medication, 101 MS patients performed the Multiple Sclerosis Health Resource Utilization Survey (MS-HRS). Medications were subdivided into a total number of medications (TD), MS-related medication [MSD, i.e., disease-modifying drugs (DMDs) and symptomatic treatment (SD)], and medication for comorbidities (CDs). Multivariate linear regression models were performed to estimate if the amount of each medication type affects LOC or TTU.
Results: Polypharmacy appeared in 54 patients at the time of the survey. The relative risk (RR) of LOC 1 increased significantly by 2.46 (p = 0.001) per TD and by 2.55 (p = 0.004) per MSD, but not per CD (RR 1.44; p = 0.092). The effect of RR on MSD was driven by SD (RR 2.2; p = 0.013) but not DMD (RR 2.6; p = 0.4). RR of MSD remained significant for LOC 2 (1.77; p = 0.009) and LOC 3/4 (1.91; p = 0.015), with a strong trend in RR of SD, but not DMD. TTU increased significantly per MSD (p = 0.012), but not per TD (p = 0.081) and CD (p = 0.724).
Conclusion: The number of MSDs is related to the likelihood of a higher level of care and the number of therapy sessions and is therefore a good indication of the extent of the societal costs.
Neurodegenerative Erkrankungen sind durch einen progressiven Verlust von Neuronen oder Gliazellen im Gehirn oder Rückenmark gekennzeichnet. Der Ersatz dieser zu Grunde gegangen Zellen, zum Beispiel durch neurale Stammzellen (NSCs) stellt dabei einen möglichen kurativen Behandlungsansatz dar, bei dem alternativ zur stereotaktisch-chirurgischen Zelltransplantation die Injektion der NSCs in den Liquorraum als atraumatische Transplantationsmethode in Frage kommt. In vorangegangenen Studien hierzu wird allerdings über eine sehr geringe Migration der intrathekal injezierten Zellen ins Hirnparenchym berichet, zurückzuführen möglicherweise auf einen zu niedrigen Nährstoffgehalt des Liquors. Andererseits ist bekannt, dass Liquor ein wichtiges Milieu für das Überleben NSCs und deren Zellpfaddetermination im sich entwickelnden Gehirn darstellt. In unserer Studie haben wir deshalb den Einfluss von Liquor auf das Verhalten adulter humaner (ahNSCs) und fetaler muriner neuronaler Stammzellen (fmNSCs)in vitro untersucht. Die zentralen Ergebnisse unserer Studie sind: (i) sowohl während der Expansion als auch während der Differenzierung führt Liquor zu einer erhöhten Überlebensrate ahNSCs und fmNSCs im Vergleich zum standard Expansions-/DIfferenzierungsmedium (ii) Liquor fördert das Auswachsen von Zellfortsätzen im Vergleich zum Standardmedium und führt so zu einem schnelleren Verlust der Teilungsfähigkeit (iii) Liquor förder die Astrogliogenese in ahNSCs und fmNSCs und hemmt die Neurogenese in fmNSCs. Eine nierige Überlebensrate der intrathekal injezierten Zellen auf Grund eines zu niedrigen Nährstoffgehaltes scheint auf Grund unserer Ergebnisse somit unwahrscheinlich. Stattdessen könnte das schnelle Auswachsen der Zellförtsätze nach intrathekaler Injektion zu einer Adhäsion injezierter Stammzellen an der Ventrikelwand führen und somit eine ausreichende intraparenchymale Migration verhindern. Als zusätzlich negativen Effekt des Liquors zeigte sich in unserer Studie eine Förderung der Astrogliogenese und Hemmung der Differenzierung in benötigte Neurone. Vor der praktischen Umsetzung der intrathekalen Stammzellinjektion als alternative Transplantationsmethode sind deshalb weitere Studien zu den Inhaltsstoffen des Liquors und Möglichkeiten der Beeinflussung dieser notwendig.
Aged rats recover poorly after unilateral stroke, whereas young rats recover readily possibly with the help from the contralateral, healthy hemisphere. We asked whether anomalous, age-related changes in the transcriptional activity in the brains of aged rats could be one underlying factor contributing to reduced functional recovery. We analysed gene expression in the periinfarct and contralateral areas of 3-month- and 18-month-old Sprague Dawley rats. Our experimental end-points were cDNA arrays containing genes related to hypoxia signalling, DNA damage and apoptosis, cellular response to injury, axonal damage and regrowth, cell lineage differentiation, dendritogenesis and neurogenesis. The major transcriptional events observed were: (i) Early up-regulation of DNA damage and down-regulation of anti-apoptosis-related genes in the periinfarct region of aged rats after stroke; (ii) Impaired neurogenesis in the periinfarct area, especially in aged rats; (iii) Impaired neurogenesis in the contralateral (unlesioned) hemisphere of both young and aged rats at all times after stroke and (iv) Marked up-regulation, in aged rats, of genes associated with inflammation and scar formation. These results were confirmed with quantitative real-time PCR. Conclusion I: reduced transcriptional activity in the healthy, contralateral hemisphere of aged rats in conjunction with an early up-regulation of DNA damage-related genes and pro-apoptotic genes and down-regulation of axono- and neurogenesis in the periinfarct area are likely to account for poor neurorehabilitation after stroke in old rats. Efficient neuroprotection after stroke requires long-term, regulated lowering of whole body temperature. After stroke, exposure of aged rats to hydrogen sulfide resulted in sustained, deep hypothermia (30.8 ± 0.7°C) that led to a 50% reduction in infarct size with a concomitant reduction in the number of phagocytic cells. At the transcription level, we found an overall decrease in the transcriptional activity related to inflammation and apoptosis. Behaviorally, hypothermia was associated with better performance on tests that require complex sensorimotor skills, in the absence of obvious neurological deficits or physiological side-effects, in aged rats. Conclusion II: Prolonged hypothermia is a simple and efficacious method to limit damage inflicted by stroke in aged rats.
Die sonographische Diagnostik nimmt in vielen Bereichen der Medizin einen hohen Stellenwert ein. Durch den technischen Fortschritt können heutzutage auch die peripheren Nerven inklusive verschiedenster pathologischer Veränderungen dargestellt werden. Die Messung der Nervenquerschnittsfläche (nerve cross sectional area = NCSA) im transversalen Bild hat sich in vielen Studien als geeigneter Parameter zur Beurteilung bzw. zum Vergleich des Nervenkalibers herausgestellt. Polyneuropathien (PNP) sind Erkrankungen des peripheren Nervensystems. Man differenziert zwischen primär demyelinisierenden, axonalen sowie gemischten PNP. Ätiologisch lassen sich u. a. metabolische, toxische und immunologische PNP unterscheiden. In den bisher publizierten Studien zur Sonographie bei PNP konnte beobachtet werden, dass besonders demyelinisierende PNP zu einer Vergrößerung der NCSA führen. Informationen zum sonographischen Erscheinungsbild der in der Gesellschaft häufigen PNP (diabetisch, Alkohol assoziierte, PNP bei Vitamin B12 Defizit) sind jedoch bisher limitiert. In der vorliegenden explorativen Pilotstudie wurde geprüft, ob die NCSA bei PNP größer ist als bei gesunden Nerven und ob Zusammenhänge zwischen der NCSA und dem Schädigungstyp, der Ätiologie oder dem elektrophysiologisch bestimmten Schweregrad bestehen. Um diese Fragestellungen zu beantworten, wurde die NCSA an insgesamt 12 fest definierten, außerhalb physiologischer Engpassstellen liegenden, Messpositionen unterschiedlicher Nerven (N. medianus, N. ulnaris, N. tibialis, Spinalwurzel C6 / C7, Trunci des Plexus brachialis) bestimmt. Die Reliabilität der Ergebnisse wurde bei 5 Probanden durch eine Wiederholung der Messungen im Abstand von mehr als 24 h geprüft. Dabei konnte gezeigt werden, dass eine gute Reproduzierbarkeit der Messungen besteht, da bei sämtlichen Messpunkten, ausgenommen dem Truncus inferior, ein Variationskoeffizienten von weniger als 20 % vorgelegen hat. Die Untersuchung bei 27 gesunden Probanden ergab, dass keine Abhängigkeit der NCSA von der Körperseite besteht, sodass der Untersuchungsumfang bei den 56 Patienten mit elektrophysiologisch gesicherten PNP auf eine Körperseite reduziert werden konnte. Die wichtigsten Ergebnisse der Studie sind, dass Patienten mit PNP größere NCSA aufweisen als Gesunde. Die Verdickung der Nerven ist sowohl in proximalen als auch in distalen Abschnitten nachweisbar und ist sowohl bei demyelinisierenden als auch bei axonalen PNP vorhanden. Die vorliegende Arbeit liefert weiterhin Hinweise dafür, dass demyelinisierende PNP eine deutlichere Vergrößerung der NCSA aufweisen als primär axonale Schädigungen und dass sie sich dadurch sonographisch voneinander unterscheiden. Außerdem ist die NCSA bei immunvermittelten Neuropathien wie dem GBS, der CIDP und den paraproteinämischen PNP besonders in den proximalen Abschnitten größer als bei PNP anderer Ätiologie. Eine Assoziation zwischen dem Schweregrad und der NCSA konnte nicht beobachtet werden. Die Ergebnisse der vorliegenden Studie stehen im Einklang mit dem Grundtenor der Literatur. Anzumerken ist jedoch, dass die bisherigen Publikationen überwiegend Einzelvergleiche beinhalten, in denen meist immunvermittelte PNP im Fokus stehen, oder dass sie sich häufig auf die oberflächlicher gelegenen Nerven der oberen Extremitäten beschränkt haben. Stärken der vorliegenden Studie sind die Vielfältigkeit der ausgewählten Nervensegmente und der Verzicht auf Messungen an Engpassstellen sowie die Mannigfaltigkeit der verglichenen Ätiologien. Ein limitierender Faktor der vorliegenden Studie ist die nicht immer gewährleistete Verblindung des Untersuchers bezüglich der Ätiologien und die teilweise geringen Subgruppengrößen. Aus diesem Grund wurde die vorliegende Studie als explorative Pilotstudie angelegt, mit dem Ziel, Hypothesen zu generieren und die Berechnung eines Stichprobenumfangs zukünftiger konfirmatorischer Studien zu ermöglichen, an Hand derer dann mit ausreichender statistischer Trennschärfe (Power) die aufgestellten Hypothesen überprüft werden können.
Abstract: Ischemic stroke is an aging disease and causes high mortality or long-term disability. The reduced neurological recovery in aging is possibly associated with impairment of angiogenesis and non-specific enhancement of inflammatory reaction. To check this hypotheses, those events were compared within young and elder animals brain at day 14 following focal ischemic stroke. Moreover, it is of importance to investigate also the potential therapies of indomethacin for prolonging the therapeutic window using aged animal models. The focus of present study was on neurobiological and neurological differences between young and old rats modulated by indomethacin daily treatment beginning at four hours post-ischemic episode. The effectiveness of indomethacin treatment in young and elder rats was probed using immunohistochemistry, oligonucleotide microarray, Real Time PCR and neurological evaluation. Our results provide insight of several age-independent positive consequences of Cox non-specific inhibition by indomethacin including increased NeuN positive surviving neurons, reduced infarct volume and enhanced neuroprotective response of innate immune system evidenced by increased Iba1 and Anx3 immunoreactivities in moderately activated microglia in periinfarction. From gene level we observed in both age groups downregulation of Mdk and Cxcl1 chemokines, and Id3 transcription factor which might modulate inflammatory response and facilitate repair. Other several findings showed age-dependent drug effect. Indomethacin had reduced efficacy in aged ischemic brain. From a total of 34 genes differential regulated, we observed 43% in young and only 28% of genes in aged have tendency toward age-matched sham expression level. In aged rats, indomethacin is ineffective in inhibiting phagocytic activity which is probably due to no expression changes of several cytokines like Tnfá and Cxcl4. Also, at protein level we observed no change of lysosomal ED1 immunoreactivity under treatment. On the other hand aging is characterized by no expression changes of Plau, Timp1, Timp2 and Col18a1 after treatment resulting in no improvement of angiogenesis. In young rats, conversely, drug administration decreased phagocytic activity by downregulating several cytotoxic cytokines such as Tnfá and Cxcl4. Moreover, the observable decrease of proteases like MMP10, Plau and MMP inhibitor Timp2 employed in matrix remodeling together with downregulation of Col18a1 expression after treatment might sustain angiogenesis in young rat ischemic brain. Indomethacin improves the motor-sensory performance in ischemic stroke rats as compared with age-matched untreated animals. Young rats fully recovered while aged showed important recuperation but did not achieve the preoperative level. In view of all this, indomethacin treatment might be consider as adjuvant therapy following ischemic stroke, even if aging blunts the positive effect of indomethacin on altered angiogenic-related gene expression. Because of the small number of rats, the results obtained from this study show only a tendency to significance and that further studies with more animals need to be statistically validated before firmly conclusions can be drawn. Juvenile Myoclonic Epilepsy Shows Potential Structural White Matter Abnormalities: A TBSS Study

Results on gray matter alterations in complex regional pain syndrome (CRPS) showed heterogeneous findings. Since CRPS is a rare disease, most studies included only small and heterogeneous samples resulting in a low reliability of findings between studies. We investigated 24 CRPS patients with right upper limb affection in the chronic stage of disease using structural MRI and clinical testing. We focused on gray matter volume (GMV) alterations of the brain in comparison to 33 age matched healthy controls, their association to clinical characteristics (duration of pain syndrome and pain intensity ratings) and sensorimotor performance (finger dexterity and spatiotactile resolution). When applying an explorative whole brain analysis CRPS patients showed lower GMV in the bilateral medial thalamus. No other areas showed a relevant GMV difference for the group comparisons. When applying a region of interest driven approach using anatomical masks of the thalamus, ACC/mPFC, putamen, and insula we found relevant associations of clinical and behavioral data in ACC and insula. Whereas, the GMV in ACC showed negative associations with pain intensity and CRPS duration, the GMV of the left posterior insula was negatively associated with sensorimotor performance of the affected hand side. Overall, our results are in accordance to results of others describing a thalamic reduction of GMV in patients with neuropathic pain and are also in accordance with associations of pain intensity and duration with reduced ACC in general in patients with chronic pain syndromes. Sensorimotor performance seems to be related to posterior insula GMV reduction, which has not been described yet for other patient groups.
In der Vergangenheit ergaben sich aus zahlreichen Untersuchungen vermehrt Hinweise für eine wichtige Rolle von Insulin-like Growth Factor-1 (IGF-1) für das Überleben, den Erhalt, sowie das Differenzierungsverhalten von verschiedenen Zelltypen. Hierbei wurden Effekte von IGF-1 auf Zellen unterschiedlicher Gewebearten, u. a. auch des Zentralen Nervensystems, festgestellt. Für viele der beobachteten IGF-1-Wirkungen konnte eine Vermittlung über den PI3-Kinase/Akt/NF-κB-Signalweg bestätigt werden. Aber auch der ERK-Signal- weg scheint an der IGF-1-vermittelten Signaltransduktion beteiligt zu sein.
Aus früheren Untersuchungen der eigenen Arbeitsgruppe mit dem auch in der vorliegenden Arbeit verwendeten Zellsystem ist bekannt, dass der PI3-Kinase/Akt/NF-κB-Signalweg die astrogliale Differenzierung der fetalen mesenzephalen neuralen Precursorzellen („fetal mesencephalic precursor cells“ [fmNPCs]) maßgeblich beeinflusst.
Hingegen wird die durch Interleukin-1β (IL-1β) induzierte dopaminerge Differenzierung von fmNPCs über den ERK/MAP-Kinase-Signalweg vermittelt und durch die Hemmung des PI3-Kinase/Akt/NF-κB-Signalweges erleichtert.
Die vorliegende Arbeit beschäftigte sich vor diesem Hintergrund mit dem Einfluss von IGF-1 auf langzeitexpandierte fmNPCs der Ratte. Dazu wurde das in vitro-Modell der primären Dissoziationskultur aus mesenzephalem Gewebe 14 Tage alter embryonaler Ratten herangezogen. Es wurde der Frage nachgegangen, inwieweit IGF-1 die Überlebensfähigkeit, das Proliferationspotenzial, die Expression des IGF-1-Rezeptors, sowie das neuronale, astrogliale und oligodendrogliale Differenzierungsverhalten dieser fmNPCs der Ratte beeinflussen kann. Weiterhin sollten an der Differenzierung beteiligte intrazelluläre Signaltransduktionsmechanismen näher charakterisiert werden.
Bei der Untersuchung der Überlebensfähigkeit der Zellen zeigte sich, dass die Behandlung mit IGF-1 (50 ng/ml) sowohl während der Expansion als auch während der Differenzierung über 24 h und 72 h im Vergleich zur unbehandelten Kontrollreihe zu einer signifikanten Steigerung der Überlebensrate führte. Dieser überlebenssteigernde Effekt von IGF-1 auf die fmNPCs konnte durch die gleichzeitige Behandlung mit AG 1024 – einem Inhibitor des IGF-1-Rezeptors – während der Differenzierung, nicht jedoch während der Expansion, aufgehoben werden. Dies spricht für eine besondere Rolle von IGF-1 für den Differenzierungsprozess der fmNPCs, welche folglich während ihrer Differenzierung in höherem Maße auf die Expression des IGF-1-Rezeptors angewiesen sind als während der Expansion. Übereinstimmend mit diesem Ergebnis zeigte sich bei den Untersuchungen zur Expression des IGF-1-Rezeptors eine signifikante Hochregulierung der IGF-1-Rezeptor-Expression während der Differenzierung im Vergleich zur Expansion der Zellen. Weiterhin konnte unter IGF-1-Behandlung und Differenzierung der Zellen wiederum eine signifikante Hochregulierung der IGF-1-Rezeptor-Expression im Vergleich zur Differenzierung unter Kontrollbedingungen beobachtet werden.
Bei den Untersuchungen zum Einfluss von IGF-1 auf den Erhalt des Proliferationspotentials der fmNPCs konnte bezüglich der Ki-67-markierten Zellen nach der Expansion über 72 h ein signifikanter Effekt von IGF-1 beobachtet werden, mit einer höheren Anzahl Ki-67-positiver Zellen im Vergleich zur Kontrollreihe. Allerdings konnte dieser, das Proliferationspotential steigernde, Effekt nach der Expansion über 24 h und anschließender Markierung der Zellen mit BrdU nicht beobachtet werden. Trotzdem zeigten sich im Vergleich zur Kontrolle mehr Zellen mit Proliferationspotential (BrdU-positiv) und somit eine Tendenz in Richtung des Ergebnisses nach der Expansion über 72 h.
Ein weiterer Fokus der vorliegenden Arbeit lag auf der Untersuchung des Einflusses von IGF-1 auf das neuronale und gliale Differenzierungsverhalten von fmNPCs. Hierbei zeigte sich, dass die Behandlung der Zellen mit IGF-1 über einen Differenzierungszeitraum von 7 d zu signifikant mehr Zellen mit neuronalem Phänotyp (MAP2-positiv) im Vergleich zur Kontrollreihe führte. Bezüglich der astroglialen und oligodendroglialen Differenzierung der fmNPCs konnte eine Behandlung mit IGF-1 keine signifikanten Ergebnisse zeigen.
Schließlich beschäftigte sich die vorliegende Arbeit auch damit, die an der Differenzierung beteiligten intrazellulären Signaltransduktionsmechanismen näher zu charakterisieren. Dazu wurden ELISA-Experimente durchgeführt, um die Aktivierung ausgewählter Kinasen und Transkriptionsfaktoren in der Frühphase der Differenzierung der fmNPCs zu bestimmen. Hierbei zeigte sich unter IGF-1-Behandlung, im Vergleich zur unbehandelten Kontrollreihe, ein Abfall der Aktivität der PI3-Kinase, Akt und von NF-κB mit signifikanten Werten nach 0 h, 3 h und 6 h für die PI3-Kinase, nach 1 h und 6 h für Akt und nach 0,5 h für NF-κB. Im Gegensatz dazu konnte die Aktivität der ERK unter IGF-1-Behandlung nach 1 h, 3 h und 6 h signifikant gesteigert werden. Für die p38 MAP-Kinase zeigte sich nach 3 h Behandlung mit IGF-1 eine signifikante Reduktion der Aktivität im Vergleich zur Kontrollreihe. Auf die JNK-Aktivität zeigte die Behandlung mit IGF-1 hingegen keinen signifikanten Effekt.
Die Ergebnisse der vorliegenden Arbeit lassen folgende Schlussfolgerungen zu:
• IGF-1 erhöht die Überlebensrate von fmNPCs während deren Expansion und Differenzierung in vitro über 24 h und 72 h.
• AG 1024 inhibiert diesen überlebenssteigernden Effekt während der Differenzierung von fmNPCs, nicht aber während deren Expansion.
• IGF-1 steigert das Proliferationspotenzial von fmNPCs in vitro.
• IGF-1 induziert die Neurogenese von fmNPCs in vitro.
• Effekte von IGF-1 auf die fmNPCs werden vermittelt über den IGF-1-Rezeptor, welcher während der Differenzierung durch die Behandlung mit IGF-1 hochreguliert wird.
• IGF-1 führt in der weiteren zellulären Signaltransduktion während der Differenzierung von fmNPCs zu einer Herunterregulierung des PI3-Kinase/Akt/NF-κB-Signalwegs und zu einer Hochregulierung von ERK1/2 des MAP-Kinase-Signalwegs.
Separating EEG correlates of stress: Cognitive effort, time pressure, and social‐evaluative threat

Abstract
The prefrontal cortex is a key player in stress response regulation. Electroencephalographic (EEG) responses, such as a decrease in frontal alpha and an increase in frontal beta power, have been proposed to reflect stress‐related brain activity. However, the stress response is likely composed of different parts such as cognitive effort, time pressure, and social‐evaluative threat, which have not been distinguished in previous studies. This distinction, however, is crucial if we aim to establish reliable tools for early detection of stress‐related conditions and monitoring of stress responses throughout treatment. This randomized cross‐over study (N = 38) aimed to disentangle EEG correlates of stress. With linear mixed models accounting for missing values in some conditions, we found a decrease in frontal alpha and increase in beta power when performing the Paced Auditory Serial Addition Test (PASAT; cognitive effort; n = 32) compared to resting state (n = 33). No change in EEG power was found when the PASAT was performed under time pressure (n = 29) or when adding social‐evaluative threat (video camera; n = 29). These findings suggest that frontal EEG power can discriminate stress from resting state but not more fine‐grained differences of the stress response.
Background: Many regions worldwide reported a decline of stroke admissions during the early phase of the coronavirus disease 2019 (COVID-19) pandemic. It remains unclear whether urban and rural regions experienced similar declines and whether deviations from historical admission numbers were more pronounced among specific age, stroke severity or treatment groups.
Methods: We used registry datasets from (a) nine acute stroke hospitals in Berlin, and (b) nine hospitals from a rural TeleNeurology network in Northeastern Germany for primary analysis of 3-week-rolling average of stroke/TIA admissions before and during the COVID-19 pandemic. We compared course of stroke admission numbers with regional cumulative severe acute respiratory syndrome coronavirus 2 (Sars-CoV-2) infections. In secondary analyses, we used emergency department logs of the Berlin Charité University hospital to investigate changes in age, stroke severity, and thrombolysis/thrombectomy frequencies during the early regional Sars-CoV-2 spread (March and April 2020) and compared them with preceding years.
Results: Compared to past years, stroke admissions decreased by 20% in urban and 20-25% in rural hospitals. Deviations from historical averages were observable starting in early March and peaked when numbers of regional Sars-CoV-2 infections were still low. At the same time, average admission stroke severity and proportions of moderate/severe strokes (NIHSS >5) were 20 and 20–40% higher, respectively. There were no relevant deviations observed in proportions of younger patients (<65 years), proportions of patients with thrombolysis, or number of thrombectomy procedures. Stroke admissions at Charité subsequently rebounded and reached near-normal levels after 4 weeks when the number of new Sars-CoV-2 infections started to decrease.
Conclusions: During the early pandemic, deviations of stroke-related admissions from historical averages were observed in both urban and rural regions of Northeastern Germany and appear to have been mainly driven by avoidance of admissions of mildly affected stroke patients.
Objective
To evaluate individual and group long-term efficacy and safety of erenumab in individuals with episodic migraine (EM) for whom 2–4 prior preventatives had failed.
Methods
Participants completing the 12-week double-blind treatment phase (DBTP) of the LIBERTY study could continue into an open-label extension phase (OLEP) receiving erenumab 140 mg monthly for up to 3 years. Main outcomes assessed at week 112 were: ≥50%, ≥75% and 100% reduction in monthly migraine days (MMD) as group responder rate and individual responder rates, MMD change from baseline, safety and tolerability.
Results
Overall 240/246 (97.6%) entered the OLEP (118 continuing erenumab, 122 switching from placebo). In total 181/240 (75.4%) reached 112 weeks, 24.6% discontinued, mainly due to lack of efficacy (44.0%), participant decision (37.0%) and adverse events (AEs; 12.0%). The ≥50% responder rate was 57.2% (99/173) at 112 weeks. Of ≥50% responders at the end of the DBTP, 36/52 (69.2%) remained responders at ≥50% and 22/52 (42.3%) at >80% of visits. Of the non-responders at the end of the DBTP, 60/185 (32.4%) converted to ≥50% responders in at least half the visits and 24/185 (13.0%) converted to ≥50% responders in >80% of visits. Change from baseline at 112 weeks in mean (SD) MMD was −4.2 (5.0) days. Common AEs (≥10%) were nasopharyngitis, influenza and back pain.
Conclusions
Efficacy was sustained over 112 weeks in individuals with difficult-to-treat EM for whom 2–4 prior migraine preventives had failed. Erenumab treatment was safe and well tolerated, in-line with previous studies.

Die ex-vivo Wirkung von Spermin und Spermidin auf T-Lymphozyten bei Patienten mit kognitivem Defizit

Demenzerkrankungen stellen die Medizin und die Wissenschaft vor eine der größten Herausforderungen des 21. Jahrhunderts. Aufgrund des Fehlens kausaler Therapien werden neue Therapieansätze dringend für die Bewältigung dieser medizinisch herausfordernden Erkrankung benötigt. Vielversprechende Ergebnisse im Hinblick auf ein neues Therapeutikum liefert eine Klinische Studie (Smart-Age) mit einem polyaminreichen Pflanzenextrakt als Nahrungsergänzungsmittel, eingesetzt bei Patienten mit Subjektiver kognitiver Verschlechterung (engl. Subjective Cognitive Decline).
Da die genauen Wirkmechanismen der Polyamine Spermin und Spermidin auf die kognitive Gesundheit noch unzureichend verstanden sind und das Immunsystem eine zentralen Rolle in der Pathogenese der Alzheimer-Demenz einnimmt, war es das Ziel in der vorliegenden Dissertation die Wirkung der beiden genannten Polyamine auf die Aktivierung und Autophagie von T-Lymphozyten und die Zytokinsekretion von PBMC durchflusszytometrisch, anhand einer Studienkohorte kognitiv beeinträchtigter Patienten (Subjektive kognitive Verschlechterung SCD, milde kognitive Beeinträchtigung MCI, milde Alzheimer-Demenz) (n=22) und gesunder, altersäquivalente Kontrollprobanden (n=12), zu untersuchen.
Die Ergebnisse der Arbeit zeigen, dass Spermin und Spermidin die Aktivierung und die Autophagie von T-Lymphozyten dosisabhängig in der Patienten- und der Kontrollkohorte steigert. Spermin führte zu einer dosisabhängig verminderten Zytokinexpression aller 11 untersuchten Zytokine in der Patienten- und der Kontrollkohorte. Spermidin hingegen führte sowohl zu einer vermehrten als auch einer verminderten Expression einzelner Zytokine im Zellkulturüberstand.
In der vorliegenden explorativen Studie konnte somit erstmals prospektiv der Einfluss der Polyamine Spermin und Spermidin auf die T-Lymphozyten Aktivierung, Autophagie und die Zytokinexpression von Patienten im Frühstadium kognitiver Erkrankungen in in-vitro Experimenten gezeigt werden. Verglichen mit einer Kontrollkohorte kognitiv gesunder, altersentsprechender Probanden zeigten sich ähnliche Effekte von Spermin und Spermidin auf die untersuchten Parameter, wobei für einzelne Parameter der Untersuchung eine höhere Sensitivität der Polyaminbehandlung von T-Lymphozyten kognitiv erkrankter Patienten nachzuvollziehen war.
Die Daten dieser Dissertation liefern somit einen ersten Beitrag zur umfassenden Beleuchtung zellulärer Wirkmechanismen einer Polyaminsubstitution in humanen, peripheren Immunzellen bezüglich kognitiver Gesundheit, um hierdurch neue therapeutische Ansätze zur Behandlung dementieller Erkrankungen zu entwickeln.
Background:
Post-stroke delirium (PSD) is a modifiable predictor for worse outcome in stroke. Knowledge of its risk factors would facilitate clinical management of affected patients, but recently updated national guidelines consider available evidence insufficient.
Aims:
The study aimed to establish risk factors for PSD incidence and duration using high-frequency screening.
Methods:
We prospectively investigated patients with ischemic stroke admitted within 24 h. Patients were screened twice daily for the presence of PSD throughout the treatment period. Sociodemographic, treatment-related, and neuroimaging characteristics were evaluated as predictors of either PSD incidence (odds ratios (OR)) or duration (PSD days/unit of the predictor, b), using logistic and linear regression models, respectively.
Results:
PSD occurred in 55/141 patients (age = 73.8 ± 10.4 years, 61 female, National Institutes of Health Stroke Scale (NIHSS) = 6.4 ± 6.5). Age (odds ratio (OR) = 1.06 (95% confidence interval (CI): 1.02–1.10), b = 0.08 (95% CI = 0.04–0.13)), and male gender (b = 0.99 (95% CI = 0.05–1.93)) were significant non-modifiable risk factors. In a multivariable model adjusted for age and gender, presence of pain (OR < sub > mvar </sub >= 1.75 (95% CI = 1.12–2.74)), urinary catheter (OR < sub > mvar </sub > = 3.16 (95% CI = 1.10–9.14)) and post-stroke infection (PSI; OR < sub > mvar </sub > = 4.43 (95% CI = 1.09–18.01)) were predictors of PSD incidence. PSD duration was impacted by presence of pain (b < sub > mvar </sub >= 0.49 (95% CI = 0.19–0.81)), urinary catheter (b < sub > mvar </sub > = 1.03 (95% CI = 0.01–2.07)), intravenous line (b < sub > mvar </sub >= 0.36 (95% CI = 0.16–0.57)), and PSI (b < sub > mvar </sub >= 1.60 (95% CI = 0.42–2.78)). PSD (OR = 3.53 (95% CI = 1.48–5.57)) and PSI (OR = 5.29 (95% CI = 2.92–7.66)) independently predicted inferior NIHSS at discharge. Insular and basal ganglia lesions increased the PSD risk about four- to eight-fold.
Discussion/Conclusion:
This study identified modifiable risk factors, the management of which might reduce the negative impact PSD has on outcome.
Objectives: Navigated transcranial magnetic stimulation (nTMS) provides significant benefits over classic TMS. Yet, the acquisition of individual structural magnetic resonance images (MRIindividual) is a time-consuming, expensive, and not feasible prerequisite in all subjects for spatial tracking and anatomical guidance in nTMS studies. We hypothesize that spatial transformation can be used to adjust MRI templates to individual head shapes (MRIwarped) and that TMS parameters do not differ between nTMS using MRIindividual or MRIwarped.
Materials and Methods: Twenty identical TMS sessions, each including four different navigation conditions, were conducted in 10 healthy subjects (one female, 27.4 ± 3.8 years), i.e., twice per subject by two researchers to additionally assess interrater reliabilities. MRIindividual were acquired for all subjects. MRIwarped were obtained through the spatial transformation of a template MRI following a 5-, 9-and 36-point head surface registration (MRIwarped_5, MRIwarped_9, MRIwarped_36). Stimulation hotspot locations, resting motor threshold (RMT), 500 μV motor threshold (500 μV-MT), and mean absolute motor evoked potential difference (MAD) of primary motor cortex (M1) examinations were compared between nTMS using either MRIwarped variants or MRIindividual and non-navigated TMS.
Results: M1 hotspots were spatially consistent between MRIindividual and MRIwarped_36 (insignificant deviation by 4.79 ± 2.62 mm). MEP thresholds and variance were also equivalent between MRIindividual and MRIwarped_36 with mean differences of RMT by −0.05 ± 2.28% maximum stimulator output (%MSO; t(19) = −0.09, p = 0.923), 500 μV-MT by −0.15 ± 1.63%MSO (t(19) = −0.41, p = 0.686) and MAD by 70.5 ± 214.38 μV (t(19) = 1.47, p = 0.158). Intraclass correlations (ICC) of motor thresholds were between 0.88 and 0.97.
Conclusions: NTMS examinations of M1 yield equivalent topographical and functional results using MRIindividual and MRIwarped if a sufficient number of registration points are used.
Objectives: The significance of pre-motor (PMC) corticospinal projections in a frontoparietal motor network remains elusive. Temporal activation patterns can provide valuable information about a region's engagement in a hierarchical network. Navigated transcranial magnetic stimulation (nTMS)-induced virtual lesions provide an excellent method to study cortical physiology by disrupting ongoing activity at high temporal resolution and anatomical precision. We use nTMS-induced virtual lesions applied during an established behavioral task demanding pre-motor activation to clarify the temporal activation pattern of pre-motor corticospinal projections.
Materials and Methods: Ten healthy volunteers participated in the experiment (4 female, mean age 24 ± 2 years, 1 left-handed). NTMS was used to map Brodmann areae 4 and 6 for primary motor (M1) and PMC corticospinal projections. We then determined the stimulator output intensity required to elicit a 1 mV motor evoked potential (1 mV-MT) through M1 nTMS. TMS pulse were randomly delivered at distinct time intervals (40, 60, 80, 100, 120, and 140 ms) at 1 mV-MT intensity to M1, PMC and the DLPFC (dorsolateral pre-frontal cortex; control condition) before participants had to perform major changes of their trajectory of movement during a tracing task. Each participant performed six trials (20 runs per trial). Task performance and contribution of regions under investigation was quantified through calculating the tracing error induced by the stimulation.
Results: A pre-motor stimulation hotspot could be identified in all participants (16.3 ± 1.7 mm medial, 18.6 ± 1.4 mm anterior to the M1 hotspot). NTMS over studied regions significantly affected task performance at discrete time intervals (F(10, 80) = 3.25, p = 0.001). NTMS applied over PMC 120 and 140 ms before changes in movement trajectory impaired task performance significantly more than when applied over M1 (p = 0.021 and p = 0.003) or DLPFC (p = 0.017 and p < 0.001). Stimulation intensity did not account for error size (β = −0.0074, p = 1).
Conclusions: We provide novel evidence that the role of pre-motor corticospinal projections extends beyond that of simple corticospinal motor output. Their activation is crucial for task performance early in the stage of motor preparation suggesting a significant role in shaping voluntary movement. Temporal patterns of human pre-motor activation are similar to that observed in intracortical electrophysiological studies in primates.
BACKGROUND Acute disseminated encephalomyelitis (ADEM) is a rare, acquired demyelination syndrome that causes cognitive impairment and
focal neurological deficits and may be fatal. The potentially reversible disease mainly affects children, often after vaccination or viral infection, but may
be seen rarely in adults.
OBSERVATIONS A 50-year-old woman presented with loss of visual acuity of the left eye. Magnetic resonance imaging (MRI) revealed an intra- and
suprasellar mass, which was removed successfully. On postoperative day 1, MRI showed gross total resection of the lesion and no surgery-related
complications. On postoperative day 2, the patient presented with a progressive left-sided hemiparesis, hemineglect, and decline of cognitive
performance. MRI showed white matter edema in both hemispheres. Cerebrospinal fluid analysis revealed mixed pleocytosis (355/mL) without further
evidence of infection. In synopsis of the findings, ADEM was diagnosed and treated with intravenous immunoglobulins. Shortly thereafter, the patient
recovered, and no sensorimotor deficits were detected in the follow-up examination.
LESSONS Pituitary gland pathologies are commonly treated by transsphenoidal surgery, with only minor risks for complications. A case of ADEM after
craniopharyngioma resection has not been published before and should be considered in case of progressive neurological deterioration with multiple
white matter lesions.
Multimorbidität ist die besondere Herausforderung der älter werdenden Gesellschaft. Der ältere Patient mit neu diagnostizierter Epilepsie trägt nicht nur die Bürde seiner Epilepsie, sondern ist mit zunehmendem Lebensalter dem Risiko komorbider chronischer Erkrankungen ausgesetzt. Die Übersichtsarbeit fokussiert auf kardiovaskuläre Erkrankungen bei Epilepsie im höheren Lebensalter und ihren Beitrag zur vorzeitigen Mortalität. Es werden aktuelle Arbeiten zu medikamentösen Interaktionen bei Komedikation von Antiepileptika (AED) mit direkten oralen Antikoagulanzien (DOAK) und kardiovaskulären Medikamenten zusammengefasst.
Der Blepharospasmus ist eine Erkrankung, bei der es zu häufig wiederkehrenden Kontraktionen der Lidmuskulatur beider Augen kommt und gehört zu den fokalen Dystonien. Die Pathophysiologie dieser Bewegungsstörung ist noch nicht vollständig geklärt. Die Ursachenforschung und damit ein besseres Verständnis des Blepharospasmus ist wichtig, da der Blepharospasmus für die Patienten sehr behindernd sein kann. Die bisherigen Therapien des Blepharospasmus sind sicher, aber aktuell lediglich symptomorientiert. Auch deshalb sind Untersuchungen zur Pathophysiologie des Blepharospasmus sinnvoll. Derzeitige Vorstellungen zur Pathophysiologie des Blepharospasmus legen nahe, dass Veränderungen in einem Netzwerk vorliegen, welches aus den Basalganglien, dem Kleinhirn, dem Thalamus und dem sensomotorischen Kortex besteht. Zudem sind wahrscheinlich andere Strukturen wie die peripheren Schmerzrezeptoren und der visuelle Kortex beim Blepharospasmus betroffen. Während die Basalganglien, das Kleinhirn, der Thalamus und den sensomotorischen Kortex vorrangig für die Verarbeitung der sensorischen und motorischen Informationen verantwortlich sind, deuten mehrere Hinweise darauf hin, dass diese Strukturen auch bei der Verarbeitung von Informationen im Zusammenhang mit dem Riechen und dem Schmecken von Bedeutung sind. Basierend auf diesen Überschneidungen der beteiligten neuroanatomischen Strukturen stellt sich die Frage, ob Veränderungen des Riech- und Schmeckvermögens bei Patienten mit Blepharospasmus gefunden werden. Ferner wurde im Rahmen dieser Studie der Frage nachgegangen, ob die Behandlung mit Botulinumtoxin eine Auswirkung auf das Riechen und Schmecken bei Patienten mit Blepharospasmus hat.
Im Rahmen dieser Arbeit wurden 19 Patienten mit einem Blepharospasmus (Alter 66.6 ± 8.7 Jahren, 14 Frauen und 5 Männer) und 15 gesunde Kontrollpersonen (Alter 65.9 ± 8.4 Jahren, 9 Frauen und 6 Männer) untersucht. Davon wurden zehn Patienten mit und ohne die Wirkung vom Botulinumtoxin untersucht und dazu passend zehn Kontrollpersonen im Abstand von vier Wochen zweimal untersucht. Die Erfassung der Riechschwelle, Geruchsidentifikation und -diskrimination fand mit den Sniffin‘ Sticks statt. Zudem wurden die Taste Strips verwendet, um den Geschmackssinn der Probanden zu bewerten. Des Weiteren wurden kognitive Funktionen und psychiatrische Veränderungen als bekannte Komorbiditäten des Blepharospasmus erfasst und mit den Ergebnissen beim Riechen und Schmecken korreliert.
Patienten mit einem Blepharospasmus hatten eine signifikant geringere Riechschwelle als die Kontrollpersonen. Ferner war der Anteil der Patienten, die eine Hyposmie aufwiesen, höher als bei den Kontrollpersonen. Die motorischen, die nicht-motorischen sowie die psychiatrischen Symptome korrelierten nicht mit den Defiziten bei der Riechschwelle. Es fand sich keine Schmeckstörungen und auch die Geruchsdiskriminierung sowie die Geruchsidentifikation waren bei den Patienten mit Blepharospasmus unauffällig. Auch fand sich kein wesentlicher Unterschied beim Riechen oder Schecken bei der Behandlung der Patienten mit Botulinumtoxin.
Die gefundenen Riechstörung sind weniger ausgeprägt als bei der zervikalen Dystonie, können aber zum besseren Verständnis der Erkrankung des Blepharospasmus beitragen. Vergleichend mit Befunden von Läsionsstudien deuten die Ergebnisse darauf hin, dass der Thalamus und das Kleinhirn eventuell in der Pathophysiologie des Blepharospasmus beteiligt sein könnten. Eine genaue Benennung der Ursachenlokalisation ist schwierig, da es sich beim Blepharospasmus wahrscheinlich um eine Netzwerkerkrankung handelt und somit verschiedene Strukturen betroffen sein können. Die Behandlung der Patienten mit Blepharospasmus mit Botulinumtoxin beeinflusst wahrscheinlich nicht das Riechen und Schmecken und ist damit auch aus dieser Sicht als eine eher sichere Behandlungsmöglichkeit anzusehen. Weitere Untersuchungen auf diesem Gebiet, zum Beispiel mittels neurophysiologischer Untersuchungsverfahren zum Riechen und Schmecken oder bildgebenden Untersuchungsverfahren, sind mit Spannung zu erwarten.
Der Langzeitverlauf und damit die langfristige Prognose von Patienten mit juveniler myoklonischer Epilepsie wird nach wie vor kontrovers diskutiert. Ziel dieser Arbeit war es zum einen, den Langzeitverlauf von Patienten mit juveniler myoklonischer Epilepsie nach einem Follow-up von wenigstens 25 Jahren zu untersuchen und zum anderen, Prädiktoren für ein günstiges als auch ungünstiges Langzeit-Outcome zu finden. Hierzu wurden zunächst die Daten von 71 Patienten mit JME und Diagnosestellung vor dem 01.01.1986 retrospektiv analysiert. 31 Patienten, davon 19 Frauen, konnten in die Studie eingeschlossen werden. Von den übrigen 40 Patienten waren zwei aus nicht bekannten Gründen verstorben und 38 konnten auf Grund geänderter Namen oder Adressen nicht kontaktiert werden. Der Beobachtungszeitraum betrug mindestens 25 Jahre, im Mittel lag dieser bei 39,1 Jahren. Von den 31 Patienten wurden 21 Patienten (67,7%) anfallsfrei. 6 Patienten (28,6%) waren zum Zeitpunkt des Follow-up ohne antikonvulsive Medikation seit mindestens 5 Jahren anfallsfrei. Als Prädiktoren für ein ungünstiges Outcome konnten das Auftreten von generalisiert tonisch-klonischen Anfällen, denen bilateral myoklonische Anfälle unmittelbar vorausgingen und eine lange Epilepsiedauer mit insuffizienter Therapie identifiziert werden. Ferner zeigte sich eine antikonvulsive Polytherapie als Prädiktor für ein schlechtes Outcome. Die komplette Remission von generalisiert tonisch-klonischen Anfällen unter antikonvulsiver Therapie hingegen erhöhte die Chance auf komplette Anfallsfreiheit signifikant. Das Auftreten einer photoparoxysmalen Reaktion erhöhte signifikant das Risiko für ein Anfallsrezidiv nach Absetzen der antikonvulsiven Medikamente. Zusammenfassend zeigte diese Studie erstmalig, dass bei diesem heterogenen idiopathisch generalisiertem Epilepsiesyndrom eine lebenslange medikamentöse Therapie nicht notwendigerweise erforderlich ist, um Anfallsfreiheit zu erreichen. Die oben genannten Prädiktoren können helfen, eine geeignete Therapieentscheidung für Patienten mit juveniler myoklonischer Epilepsie zu finden.
Ziel: Bis heute liegt keine endgültige Einschätzung zur Bedeutung des EEG im Hinblick auf prognostische Aussagen bei fokalen Epilepsien vor. Deshalb versuchten wir der Frage nachzugehen, ob das EEG bei kryptogen fokaler Epilepsie (KFE) eine prognostische Relevanz hat. Methoden: Hierzu wurden 32 Patienten mit KFE, davon 16 pharmakoresistente und 16 anfallsfreie, mit Beginn einer adäquaten antiepileptischen Medikation prospektiv von 1985 - 1992 untersucht. Wir analysierten 166 EEG, abgeleitet zu definierten Zeitpunkten im 10-20-System einschließlich Hyperventilation und Photostimulation. Erfasst und statistisch ausgewertet wurden die Hintergrundaktivität, Wellenparoxymen, fokale Verlangsamungen und epileptiforme Muster (EM), Unser besonderes Augenmerk galt den EM. Die Ausmessung erfolgte semiquantitativ in ihrer Dauer und Amplitude des jeweils ersten Spitzenpotentials. Ergebnisse: Insgesamt 22 Frauen und 10 Männer wurden untersucht. Patienten beider Gruppen zeigten im Krankheitsverlauf teils unauffällige, teils pathologische EEG. Erst die Gegenüberstellung aller 166 EEG und ihre Analyse im Krankheitsverlauf ließen statistische Zusammenhänge zwischen EEG-Veränderungen und Epilepsieverlauf erkennen. Von Bedeutung erwiesen sich die EM hinsichtlich ihrer Lokalisation (p = .0001), Dauer (p = .003) und Dichte (p =.0004) sowie die Provokation durch Hyperventilation (p = .007). Bei den nicht anfallsfreien Patienten traten EM in kurzer Dauer, hoher Dichte, rechtshemisphärieller, extratemporaler Lokalisation sowie bevorzugt im Ruhe-EEG auf. Die EM hingegen bei den anfallsfreien Patienten zeigten eine längere Dauer, eine niedrigere Dichte, eine linkshemisphärielle, temporale Lokalisation und ein vermehrtes Auftreten unter Hyperventilation. Zusammenfassung: Für den einzelnen Patienten mit kryptogen fokaler Epilepsie und normalem EEG im gesamten Epilepsie verlauf sind keine Prädiktoren zur Prognose abzuleiten. Bei Patienten mit EM lassen die Lokalisation der EM, ihre Amplitude, Dauer und Dichte eine Aussage zur Prognose vermuten. Nicht das Vorhandensein oder Fehlen von EM scheint also bedeutsam, sondern die Ausprägung einzelner EM-Charakteristika.
Funktionelle Bildgebung zur Perzeption affektiver Gesichter bei Patienten mit Multipler Sklerose

Multiple Sklerose, eine der häufigsten neurologischen Erkrankungen, führt neben physischen auch zu neuropsychologischen Defiziten. Dabei wurden in der vorliegenden Arbeit die beeinträchtigten Fähigkeiten hinsichtlich der Wahrnehmung emotionaler Gesichter dargestellt. MS-Patienten, die schon im Vorfeld als beeinträchtigt in der Verarbeitung emotionaler Gesichter eingestuft wurden, wiesen in einer fMRT-Untersuchung im Vergleich zu unbeeinträchtigten Patienten und gesunden Kontrollen eine signifikant niedrigere Aktivierung in verschiedenen Hirnarealen auf. Es zeigte sich, dass die Fähigkeit, negativ valente Expressionen zuzuordnen, mit einer Aktivierung im linken, ventrolateralen Präfrontalcortex korrelierte. Gleichzeitig wiesen die beeinträchtigten Patienten eine Läsion in der weißen Substanz am linken Temporalhorn des Seitenventrikels auf. Eine klinische Beeinträchtigung in der Perzeption von Gesichtsexpressionen kann somit mit einer gestörten Signalübertragung von primär wahrnehmenden zu höher verarbeitenden Arealen erklärt werden, die zu einer verminderten Aktivierung im linken ventrolateralen Präfrontalcortex führt. Diese Ergebnisse sind der Inhalt der Publikation „Prefrontal function associated with impaired emotion recognition in patients with multiple sclerosis“, erschienen 2009 in Behavioural brain research 205, 280-285, welche die Grundlage für die veröffentlichungsbasierte Dissertation darstellt.
Background:
Epileptic seizures can occur throughout the course of multiple sclerosis (MS) and are associated with increasing disability progression over time. However, there are no data on whether epileptic seizures at the onset of MS also lead to increasing disability.
Objective:
To examine disease progression over time for MS patients with epileptic seizures at onset.
Methods:
We analyzed the data of 30,713 patients on the German Multiple Sclerosis Register in a case–control study for more than 15 years. MS patients with seizures at onset were further divided into subgroups with polysymptomatic and monosymptomatic onset to assess the impact of additional symptoms on disease progression.
Results:
A total of 46 patients had seizures as onset symptoms. Expanded Disability Status Scale (EDSS) within the first year was lower in the group with seizures at onset compared to controls (0.75 versus 1.6, p < 0.05), which changed until the last reported visit (3.11 versus 3.0). Both subgroups revealed increased EDSS progression over time compared to controls.
Conclusion:
Epileptic seizures at MS onset are associated with a higher amount of disability progression over time. Additional longitudinal data are needed to further clarify the impact of seizures on the pathophysiology of MS disease progression.
Background:
Epilepsy development during the course of multiple sclerosis (MS) is considered to be the result of cortical pathology. However, no long-term data exist on whether epilepsy in MS also leads to increasing disability over time.
Objective:
To examine if epilepsy leads to more rapid disease progression.
Methods:
We analyzed the data of 31,052 patients on the German Multiple Sclerosis Register in a case–control study.
Results:
Secondary progressive disease course (odds ratio (OR) = 2.23), age (OR = 1.12 per 10 years), and disability (OR = 1.29 per Expanded Disability Status Scale (EDSS) point) were associated with the 5-year prevalence of epilepsy. Patients who developed epilepsy during the course of the disease had a higher EDSS score at disease onset compared to matched control patients (EDSS 2.0 vs 1.5), progressed faster in each dimension, and consequently showed higher disability (EDSS 4.4 vs 3.4) and lower employment status (40% vs 65%) at final follow-up. After 15 years of MS, 64% of patients without compared to 54% of patients with epilepsy were not severely limited in walking distance.
Conclusion:
This work highlights the association of epilepsy on disability progression in MS, and the need for additional data to further clarify the underlying mechanisms.
Background
Fatigue is a common symptom in patients with multiple sclerosis. Several studies suggest that outdoor temperature can impact fatigue severity, but a systematic study of seasonal variations is lacking.
Methods
Fatigue was assessed with the Fatigue Scale for Motor and Cognitive Functions (FSMC) in a temperate climatic zone with an average outdoor temperature of 8.8°C. This study included 258 patients with multiple sclerosis from 572 visits temporally distributed over the year. The data were adjusted for age, sex, cognition, depression, disease severity, and follow-up time. Linear regression models were performed to determine whether the temporal course of fatigue was time-independent, linearly time dependent, or non-linearly time dependent.
Results
Fatigue was lowest during January (mean FSMC: 49.84) and highest during August (mean FSMC: 53.88). The regression analysis showed the best fit with a model that included months + months2, which was a non-linear time dependency. Mean FSMC per month correlated significantly with the average monthly temperature (ρ = 0.972; p < 0.001).
Conclusion
In multiple sclerosis, fatigue showed a natural temporal fluctuation. Fatigue was higher during summer compared to winter, with a significant relationship of fatigue with outdoor temperature. This finding should be carefully taken into account when clinically monitoring patients over time to not interpret higher or lower scores independent of seasonal aspects.
Background
The Symbol Digit Modalities Test (SDMT) is most frequently used to test processing speed in patients with multiple sclerosis (MS). Functional imaging studies emphasize the importance of frontal and parietal areas for task performance, but the influence of frontoparietal tracts has not been thoroughly studied. We were interested in tract-specific characteristics and their association with processing speed in MS patients.
Methods
Diffusion tensor imaging was obtained in 100 MS patients and 24 healthy matched controls to compare seed-based tract characteristics descending from the superior parietal lobule [Brodman area 7A (BA7A)], atlas-based tract characteristics from the superior longitudinal fasciculus (SLF), and control tract characteristics from the corticospinal tract (CST) and their respective association with ability on the SDMT.
Results
Patients had decreased performance on the SDMT and decreased white matter volume (each p < 0.05). The mean fractional anisotropy (FA) for the BA7A tract and CST (p < 0.05), but not the SLF, differed between MS patients and controls. Furthermore, only the FA of the SLF was positively associated with SDMT performance even after exclusion of the lesions within the tract (r = 0.25, p < 0.05). However, only disease disability and total white matter volume were associated with information processing speed in a linear regression model.
Conclusions
Processing speed in MS is associated with the structural integrity of frontoparietal white matter tracts.
Der Fokus meiner Dissertation lag auf der genauen Charakterisierung epileptischer Anfälle eines klar definierten Patientenkollektivs mit spät manifestierender (≥ 55. Lebensjahr) nicht-läsioneller Epilepsie (nonlesional late-onset epilepsy, NLLOE). Erstmalig wurden semiologische Merkmale der epileptischen Anfälle in dieser Kohorte systematisch untersucht und entsprechend des neusten Klassifikationssystems der Internationalen League Against Epilepsy von 2017 eingeordnet. Die Kohorte umfasste 54 Patienten, von denen zusätzlich EEG- und Liquordaten im Rahmen ihrer Behandlung am Epilepsiezentrum der Universitätsmedizin Greifswald erhoben und mit einer Kontrollkohorte verglichen wurden. Diese setzte sich aus 58 Patienten mit früh beginnender nicht-läsioneller Epilepsie (nonlesional early-onset epilepsy, NLEOE) zusammen. Ein weiterer wesentlicher Aspekt meiner Dissertation war die Überprüfung des Behandlungserfolges nach 12-monatiger antikonvulsiver Therapie in der NLLOE-Kohorte.
Die durchgeführte Studie zeigte bei Patienten mit spät manifestierender nicht-läsioneller Epilepsie ein im Vergleich zur Kontrollkohorte signifikant häufigeres Auftreten von focal onset impaired awareness non-motor seizures. Diese manifestierten sich insbesondere in Form von passageren Beeinträchtigungen der Kognition und sensorischen Auren. Im Liquor der NLLOE-Patienten konnten bei drei Patienten oligoklonale Banden nachgewiesen werden. Bei einem dieser Patienten wurde im Nachhinein eine autoimmune Enzephalitis als Ursache der Epilepsie postuliert. Ferner waren 70% der NLLOE-Patienten nach einem Beobachtungszeitraum von 12 Monaten anfallsfrei. Den besten Behandlungserfolg, jedoch auch die höchste Nebenwirkungsrate, wurde mit dem Antikonvulsivum Levetiracetam erzielt.
Diese Ergebnisse machen deutlich, dass bei NLLOE-Patienten durch das Fehlen charakteristischer und leicht erkennbarer motorischer Symptome eine genaue Charakterisierung auch anderer semiologischer Merkmale unabdingbar ist, um gerade in dieser Patientengruppe epileptische Anfälle sicher identifizieren und eine adäquate Therapie einleiten zu können. Die Liquoranalyse deutet zudem daraufhin, dass systematische Antikörpertestungen im Liquor und Serum als weiterführende diagnostische Maßnahme zum Ausschluss einer autoimmunen Genese einer Epilepsie berücksichtigt werden sollten. Darüber hinaus belegen die Resultate ein gutes therapeutisches Ansprechen der medikamentösen antikonvulsiven Therapie bei Patienten mit nicht-läsioneller Epilepsie im höheren Lebensalter.
Manual sleep scoring for research purposes and for the diagnosis of sleep disorders is labor-intensive and often varies significantly between scorers, which has motivated many attempts to design automatic sleep stage classifiers. With the recent introduction of large, publicly available hand-scored polysomnographic data, and concomitant advances in machine learning methods to solve complex classification problems with supervised learning, the problem has received new attention, and a number of new classifiers that provide excellent accuracy. Most of these however have non-trivial barriers to use. We introduce the Greifswald Sleep Stage Classifier (GSSC), which is free, open source, and can be relatively easily installed and used on any moderately powered computer. In addition, the GSSC has been trained to perform well on a large variety of electrode set-ups, allowing high performance sleep staging with portable systems. The GSSC can also be readily integrated into brain-computer interfaces for real-time inference. These innovations were achieved while simultaneously reaching a level of accuracy equal to, or exceeding, recent state of the art classifiers and human experts, making the GSSC an excellent choice for researchers in need of reliable, automatic sleep staging.
A New Laboratory Workflow Integrating the Free Light Chains Kappa Quotient into Routine CSF Analysis

We performed this cohort study to test whether further analysis of intrathecal inflammation can be omitted if the free light chain kappa (FLCκ) quotient is within the reference range in the corresponding quotient diagram. FLCκ concentrations were measured in serum and cerebrospinal fluid (CSF) samples. The intrathecal fraction (IF) of FLCκ was calculated in relation to the hyperbolic reference range. 679 patient samples were used as a discovery cohort (DC). The sensitivity and negative predictive value (NPV) of the FLCκ-IF for the detection of an intrathecal humoral immune response (CSF-specific OCB and/or IF IgG/A/M > 0%) was determined. Based on these data, a diagnostic algorithm was developed and prospectively validated in an independent validation cohort (VC, n = 278). The sensitivity of the FLCκ-IF was 98% in the DC and 97% in the VC with a corresponding NPV of 99%. The use of the FLCκ-IF as a first line analysis would have reduced the Ig and OCB analysis by 62% in the DC and 74% in the VC. The absence of a FLCκ-IF predicts the absence of a humoral intrathecal immune response with a very high NPV of 99%. Thus, integration of our proposed algorithm into routine CSF laboratory analysis could help to reduce analytical efforts.
Die prospektive Studie beschäftigt sich mit den Auswirkungen epilepsiechirurgischer Eingriffe auf medizinischer, neuropsychologischer und psychosozialer/sozioökonomischer Ebene. Ziel war es, Prädiktoren für ein insgesamt gutes Outcome zu identifizieren. Einbezogen wurden insgesamt 43 Patienten mit fokalen Epilepsien, von denen 35 operiert werden konnten. Die Datenerhebung erfolgte jeweils zur prächirurgischen Diagnostik (t1) und zum 12-Monats-follow-up (t2). Verwendet wurden weitgehend standardisierte Verfahren wie kognitive Leistungstests, Selbstbeurteilungsverfahren zur Erfassung von Persönlichkeitseigenschaften, klinisch-psychologischer Parameter und Lebensqualität sowie ein strukturiertes Interview für psychosoziale Aspekte. Zu t1 unterschieden sich operierte und konservativ weiterbehandelte Patienten nicht voneinander. Kognitive Beeinträchtigungen betrafen hauptsächlich das Gedächtnis, gut ein Drittel der Patienten war davon betroffen. Psychische Beeinträchtigungen fanden sich insgesamt bei etwa 50% in Form eines erhöhten Angstniveaus, bei 25% als Depression. Etwa die Hälfte der Patienten war arbeitslos oder epilepsiebedingt berentet. Versuche zur beruflichen Rehabilitation gab es nur für jeden zweiten dieser Patienten. Etwa 40% aller Patienten schätzten ihre Lebensqualität insgesamt als eher schlecht ein. Zu t2 hatten sich die operierten Patienten hinsichtlich ihrer kognitiven Leistungsfähigkeit insgesamt eher verbessert. Auch das Ausmaß emotional-affektiver Beeinträchtigungen war deutlich geringer. Bei den konservativ weiterbehandelten Patienten fanden sich solche positiven Veränderungen nicht. Nur noch 20% der operierten, dagegen etwa 40% der konservativ behandelten Patienten beurteilten ihre Lebensqualität als eher schlecht. Hinsichtlich der Erwerbssituation fanden sich Verschlechterungen nur in der Gruppe operierter Patienten. Besonders auffällig war eine Verschiebung von der Arbeitslosigkeit hin zur Berentung. Nicht Anfallsfreiheit, sondern die Berufstätigkeit zu t1 sowie Verbesserungen in kognitiven Bereichen und die Abwesenheit emotional-affektiver Beeinträchtigungen zu t2 erwiesen sich als stärkste Prädiktoren für eine gute postoperative Lebensqualität.
Action comprehension that is related to language or gestural integration has been shown to engage the motor system in the brain, thus providing preliminary evidence for the gestural-verbal embodiment concept. Based on the involvement of the sensorimotor cortex (M1) in language processing, we aimed to further explore its role in the cognitive embodiment necessary for gestural-verbal integration. As such, we applied anodal (excitatory) and sham transcranial direct current stimulation (tDCS) over the left M1 (with reference electrode over the contralateral supraorbital region) during a gestural-verbal integration task where subjects had to make a decision about the semantic congruency of the gesture (prime) and the word (target). We used a cross-over within-subject design in young subjects. Attentional load and simple reaction time (RT) tasks served as control conditions, applied during stimulation (order of three tasks was counterbalanced). Our results showed that anodal (atDCS) compared to sham tDCS (stDCS) reduced RTs in the gestural-verbal integration task, specifically for incongruent pairs of gestures and verbal expressions, with no effect on control task performance. Our findings provide evidence for the involvement of the sensorimotor system in gestural-verbal integration performance. Further, our results suggest that functional modulation induced by sensorimotor tDCS may be specific to gestural-verbal integration. Future studies should now evaluate the modulatory effect of tDCS on semantic congruency by using tDCS over additional brain regions and include assessments of neural connectivity.
Age-related deterioration in white and gray matter is linked to cognitive deficits. Reduced microstructure of the fornix, the major efferent pathway of the hippocampus, and volume of the dentate gyrus (DG), may cause age-associated memory decline. However, the linkage between these anatomical determinants and memory retrieval in healthy aging are poorly understood. In 30 older adults, we acquired diffusion tensor and T1-weighted images for individual deterministic tractography and volume estimation. A memory task, administered outside of the scanner to assess retrieval of learned associations, required discrimination of previously acquired picture-word pairs. The results showed that fornix fractional anisotropy (FA) and left DG volumes were related to successful retrieval. These brain-behavior associations were observed for correct rejections, but not hits, indicating specificity of memory network functioning for detecting false associations. Mediation analyses showed that left DG volume mediated the effect of fornix FA on memory (48%), but not vice versa. These findings suggest that reduced microstructure induces volume loss and thus negatively affects retrieval of learned associations, complementing evidence of a pivotal role of the fornix in healthy aging. Our study offers a neurobehavioral model to explain variability in memory retrieval in older adults, an important prerequisite for the development of interventions to counteract cognitive decline.
Die zervikale Dystonie ist charakterisiert durch eine gesteigerte Muskelkontraktion, welche zu
einer Fehlstellung des Kopfes führt. Neben den motorischen Auffälligkeiten haben Patienten
auch nicht-motorische Störungen, wie neuropsychologische Defizite oder psychiatrische
Komorbiditäten. Eine genaue Krankheitsursache ist bis heute noch nicht abschließend geklärt.
Unter anderem könnten eine fehlende laterale Hemmung, sensorische Abweichungen oder eine
gestörte neuronale Plastizität in einem Netzwerk unter Einbeziehung von Basalganglien,
sensomotorischen Kortex und Kleinhirn ursächlich für die Bewegungsstörung bei der Dystonie
sein. Diese Strukturen sind auch beim Riechen und Schmecken beteiligt. Daher wurde
angenommen, dass Dystonie-Patienten schlechter riechen und schmecken können als Gesunde
Kontrollpersonen.
Es wurden 40 Patienten und 40 Kontrollpersonen untersucht. Das Riechvermögen wurde mit den Sniffin‘ Sticks beurteilt.
Die Schmecktestung erfolgte mittels Taste-Strips.
Zur neuropsychologischen Testung wurden der MoCA, der Trail-Making-Test, der Digit-Span-Test sowie der F-A-S-Test eingesetzt; zur Erfassung von Ängstlichkeit und Depressionen wurden
Abschnitte aus dem BSI verwendet.
Es fand sich eine Beeinträchtigung der Riechschwelle, der Riechidentifikation und des Gesamtwertes aus Riechschwelle, Diskriminations- und Identifikationsfähigkeit bei Patienten mit
einer zervikalen Dystonie. Dies stimmt mit den Ergebnissen anderer, unabhängiger Arbeiten
überein. Die Beeinträchtigung der Riechfunktion bei der zervikalen Dystonie ist möglicherweise
bedingt durch eine Netzwerkstörung, insbesondere unter Einbeziehung des Kleinhirns. Auch
beim Schmecken zeigten sich schlechtere Werte bei den Patienten als bei den gesunden
Kontrollpersonen, was möglicherweise auf eine Veränderung eines Netzwerks unter
Einbeziehung des sensomotorischen Kortex oder auf die nachgewiesene Riechstörung
zurückzuführen ist. Vergleichbare Publikationen zum Schmecken bei zervikaler Dystonie gibt es
noch nicht. Die Korrelationsanalyse legt nahe, dass das Alter der Patienten und das Ausmaß der
Schmerzen im Rahmen der zervikalen Dystonie das Riechen beeinflusst. Die allgemeine kognitive
Leitungsfähigkeit wie sie im MoCA erfasst wird, beeinflusst demgegenüber das Schmecken bei
den Patienten. Die gefundenen Veränderungen beim Riechen und Schmecken sind nur gering
ausgeprägt, können aber zum besseren Verständnis der Pathophysiologie der zervikalen
Dystonie beitragen. Die Ergebnisse stützen die Annahme, dass die zervikale Dystonie eine
Netzwerkerkrankung ist.
