Background: Men and women with valvular heart disease have different risk profiles for clinical endpoints. Non-esterified fatty acids (NEFA) are possibly involved in cardio-metabolic disease. However, it is unclear whether NEFA concentrations are associated with physical performance in patients undergoing transcatheter aortic valve implantation (TAVI) and whether there are sex-specific effects. Methods: To test the hypothesis that NEFA concentration is associated with sex-specific physical performance, we prospectively analysed data from one hundred adult patients undergoing TAVI. NEFA concentrations, physical performance and anthropometric parameters were measured before and 6 and 12 months after TAVI. Physical performance was determined by a six-minute walking test (6-MWT) and self-reported weekly bicycle riding time. Results: Before TAVI, NEFA concentrations were higher in patients (44 women, 56 men) compared to the normal population. Median NEFA concentrations at 6 and 12 months after TAVI were within the reference range reported in the normal population in men but not women. Men but not women presented with an increased performance in the 6-MWT over time (p = 0.026, p = 0.142, respectively). Additionally, men showed an increased ability to ride a bicycle after TAVI compared to before TAVI (p = 0.034). NEFA concentrations before TAVI correlated with the 6-MWT before TAVI in women (Spearman’s rho −0.552; p = 0.001) but not in men (Spearman’s rho −0.007; p = 0.964). No association was found between NEFA concentrations and physical performance 6 and 12 months after TAVI. Conclusions: NEFA concentrations improved into the reference range in men but not women after TAVI. Men but not women have an increased physical performance after TAVI. No association between NEFA and physical performance was observed in men and women after TAVI.
Long-chain fatty acids (LCFAs) serve as energy sources, components of cell membranes, and precursors for signaling molecules. Uremia alters LCFA metabolism so that the risk of cardiovascular events in chronic kidney disease (CKD) is increased. End-stage renal disease (ESRD) patients undergoing dialysis are particularly affected and their hemodialysis (HD) treatment could influence blood LCFA bioaccumulation and transformation. We investigated blood LCFA in HD patients and studied LCFA profiles in vivo by analyzing arterio–venous (A–V) LFCA differences in upper limbs. We collected arterial and venous blood samples from 12 ESRD patients, before and after HD, and analyzed total LCFA levels in red blood cells (RBCs) and plasma by LC–MS/MS tandem mass spectrometry. We observed that differences in arterial and venous LFCA contents within RBCs (RBC LCFA A–V differences) were affected by HD treatment. Numerous saturated fatty acids (SFA), monounsaturated fatty acids (MUFA), and polyunsaturated fatty acids (PUFA) n-6 showed negative A–V differences, accumulated during peripheral tissue perfusion of the upper limbs, in RBCs before HD. HD reduced these differences. The omega-3 quotient in the erythrocyte membranes was not affected by HD in either arterial or venous blood. Our data demonstrate that A–V differences in fatty acids status of LCFA are present and active in mature erythrocytes and their bioaccumulation is sensitive to single HD treatment.
Einleitung
Die Ausscheidung zahlreicher Medikamente über die Nieren erfordert bei Patienten mit CKD (chronic kidney disease) eine sorgfältige Dosisanpassung um Nebenwirkungen zu vermeiden. Ziel der Studie war es zu untersuchen, wie gut die verordnete Medikation bei Patienten mit CKD an die Nierenfunktion angepasst ist, welche Medikamente häufig fehlverordnet werden und Prädiktoren für Fehlverordnungen zu identifizieren.
Methode
In einer Querschnittstudie in 34 Hausarztpraxen in Vorpommern wurde die Medikation von Patienten mit einer CKD ≥ Stadium 3 auf Grundlage der Fachinformation und zusätzlich unter Berücksichtigung von Empfehlungen von Fachgesellschaften ausgewertet. Die verordneten Medikamente wurden mittels ATCCode erfasst. Es wurden kontraindizierte und überdosierte Verordnungen unterschieden. Prädiktive Faktoren für Fehlverordnungen wurden mittels logistischer Regressionsanalyse untersucht.
Ergebnisse
589 Patienten (Ø 78 Jahre, 63 % weiblich) mit einer CKD ≥ Stadium 3 aus 34 Hausarztpraxen wurden eingeschlossen. Sie konnten in 52 % dem CKD Stadium 3a, 37 % dem CKD Stadium 3b und 10 % CKD Stadium 4 und 5 zugeordnet werden. Insgesamt wurden in den Medikationsplänen 5102 Verordnungen erfasst (94,6 % Dauermedikationen, 5,4 % Bedarfsmedikationen). Im Mittel nahm jeder Patient fast 9 verschiedene Wirkstoffe ein (Ø=8,66; SD=3,6). 4,2 % aller Verordnungen waren nach Auswertung der Fachinformationen in der gegebenen Dosierung kontraindiziert (2,1 %) oder überdosiert (2,1 %). Bei 173 Patienten (29 %) war mindestens eine Verordnung betroffen. Der Anteil der Fehlverordnungen sank unter Berücksichtigung von neueren Empfehlungen auf 3,5 %. Metformin, Methotrexat und Kaliumpräparate wurden häufig trotz Kontraindikation verordnet. Ramipril, Sitagliptin und Simvastatin in Kombination mit Ezetimib wurden häufig überdosiert. Wichtigste Prädiktoren für Fehlverordnungen waren CKD Stadium ≥ 3b und die Anzahl der verordneten Dauermedikamente.
Schlussfolgerung
Der Anteil der unangepassten Verordnungen war in Bezug auf die Gesamtmenge aller Verordnungen gering, trotzdem war ein Viertel aller Patienten von mindestens einer unangepassten Verordnung betroffen. Unter zusätzlicher Berücksichtigung von Empfehlungen von Fachgesellschaften sank die Zahl der Fehlverordnungen. Zur klinischen Relevanz der Fehlverordnungen fehlen aussagekräftige Daten und hier besteht weiterer Forschungsbedarf. Für ein besseres Monitoring der Verordnungsqualität von Medikamenten bei CKD in der Hausarztpraxis wird ein Konsens benötigt, der auf Fachinformationen, Empfehlungen von Fachgesellschaften und klinischer Relevanz basiert. Wegen des hohen Aufwands des Einzelabgleichs sollten sich Maßnahmen zur Qualitätssicherung auf Patienten mit CKD Stadium ≥ 3b, Patienten mit Polypharmazie und auf problematische Wirkstoffe konzentrieren.
Given the increasing prevalence of chronic kidney disease (CKD) and its impact on health care, it is important to better understand the multiple factors influencing health-related quality of life (HRQOL), particularly since they have been shown to affect CKD outcomes. Determinants of HRQOL as measured by the validated Kidney Disease Quality of Life questionnaire (KDQOL) and the Patient Health Questionnaire depression screener (PHQ-9) were assessed in a routine CKD patient sample, the Greifswald Approach to Individualized Medicine (GANI_MED) renal cohort (N = 160), including a wide range of self-reported data, sociodemographic and laboratory measures. Compared to the general population, CKD patients had lower HRQOL indices. Dialysis was associated with (1) low levels of physical functioning, (2) increased impairments by symptoms and problems, and (3) more effects and burden of kidney disease. HRQOL is seriously affected in CKD patients. However, impairments were found irrespective of eGFR decline and albuminuria. Rather, the comorbid conditions of depression and diabetes predicted a lower HRQOL (physical component score). Further studies should address whether recognizing and treating depression may not only improve HRQOL but also promote survival and lower hospitalization rates of CKD patients.
