Ziel dieser Arbeit war es, die Rolle des sympathischen Nervensystems in der Entstehung der Schlaganfall-induzierten Immunveränderungen zu beleuchten. Dabei charakterisierten wir den Immunphänotyp der Lymphozyten mit Hilfe der DBH-transgenen Mauslinie. In einem Vorversuch wurde die Kontrollgruppe zu den DBH-Knock-Out-Mäusen aus Wildtypen und DBH-heterozygoten Tieren definiert. Der experimentelle Schlaganfall induzierte eine vielseitige Immunsuppression auch in Abwesenheit des sympathischen Nervensystems. Dabei waren periphere Immunveränderungen durch eine Milzatrophie, reduzierte Keimzentrumsreaktion, Lymphozytopenie und T-Zell-Aktivierung gekennzeichnet. Die Keimzentrumsreaktion war hoch signifikant in Fläche und Anzahl zu frühen Zeitpunkten über Tage hinweg erniedrigt, was für eine reduzierte humorale Immunreaktion verantwortlich sein könnte. Das könnte wiederum eine erhöhte Infektanfälligkeit, welche bei Schlaganfallpatienten und auch im Mausmodell beschrieben wurde, begründen. Zusätzlich zur gestörten humoralen Infektabwehr kam es zur frühen Aktivierung von T-Lymphozyten, was durch eine vermehrte Expression von CTLA4 auf T-Effektor-Zellen sichtbar wurde. Die CTLA4-Expression nahm auch auf regulatorischen FoxP3+ T-Zellen zu. Das könnte ein Äquivalent zu der erhöhten suppressorischen Funktion von Tregs sein, welche somit einen Beitrag zur Herausbildung der peripheren Immunsuppression leisten könnten. In der post-akuten Phase des experimentellen Schlaganfalls war eine tendenziell schnellere Erholung der Keimzentrumsreaktion in Katecholamin-depletierten Mäusen erkennbar. Des Weiteren zeigten regulatorische T-Zellen ein relatives Überleben nach experimentellem Schlaganfall in Katecholamin-Abwesenheit. Die Lymphozytopenie und Milzatrophie war ebenfalls in Abwesenheit der Katecholamine weniger stark ausgeprägt. Diese Ergebnisse lassen einen geringen Stellenwert der Katecholamine in der Entstehung peripherer Immunveränderungen erkennen. Die zuvor postulierte Schlüsselfunktion des sympathischen Nervensystems in der Herausbildung der Immunsuppression sollte kritisch betrachtet werden. Vielmehr sollte ein größeres Augenmerk auf die vielseitigen in Wechselwirkung stehenden neuroendokrinen Regelkreise wie HHN-Achse und PNA und immunodulatorische Transmitter wie Substanz P, vasoaktives intestinales Peptid und Neuropeptid Y als mögliche Ursache der Immunsuppression gelegt werden. Weitere Forschung ist nötig, um die detaillierten Vorgänge der Immunsuppression und vor allem ihre Ursachen zu analysieren.
Forests are and have been of major importance to cover a variety of societal needs. Today the demands on forests are ever increasing with sequestering carbon and balancing the climate, to name only a few. To cover those requirements forests need vital, productive, and sustainable. A difficult concept as such as the understanding of a healthy forest varies greatly. Nevertheless, forests still have to produce a sufficient amount of yield while threatened by changing climate conditions. These are predicted to bring extended and more intense drought periods as well as a higher frequency of storms and the promotion of secondary disturbances like insects calamities to also rise. In this complex situation of high and versatile demand the focus is on the allocation of the “right” forest. Forest management is requested to balance the needs of humans against those of wildlife against those of the trees themselves. To gain the respective knowledge on species responses and provenance growth, now and in the future research gaps need to be closed. All factors influencing tree growth and therefore ultimately yield need to be understood and special focus needs to be on the interactions within the forest ecosystem. One of the parameters in understanding aggregated tree growth is the dynamic of growth. This can be visualised by dendroecological methods, providing a picture of growth within the individual years. Growth dynamics are dependent on multiple factors, some, like soil being preconditioning and other like climate causing short-term responses. In this thesis I focus on the influence of climate on annual tree growth using a new approach of daily climate data to calculate climate-growth correlations. This method has the advantage of representing tree processes better than the former approach of using monthly means. Furthermore the program enables the user to feed climate scenarios and therefore estimate future growth. To gather information on species as well as provenance differences to provide advice to foresters I used different trials in Britain and Germany. On the British site different oak species were planted while the German sites are stocked with various spruce provenances. For the latter we additionally used stable carbon isotope analysis to calculate intrinsic water use efficiency. The climate-growth correlations revealed differences between the oak species with a generally higher linkage to precipitation than temperature. While the differences are clear, the question of thresholds and the role of extreme events became apparent in this work. VII Abstract Assessing the impact of extreme events using dendrometer data revealed little differences in the response to short term events of the three investigated species, oak, beech, and pine. We were able to pick up stimulus-response-relationships and as a novel result no species-specific responses were found when focusing on such a small time frame. The provenance trials offered the opportunity to investigate the potential of the use of daily climate data more closely. The two contrasting sites planted with six spruce provenances each gave an insight on the adaptive potential of provenances as well as an indication on the response times. Depending on the proceeding environmental and the local climate conditions decisions have to be made on the species or provenance selection. This thesis provides a method as well as insight on the behaviour of the important European species beech, oak, pine, and spruce. It, however, highlights the limitations such methods have for large scale estimates. While general trends on the response to specific soil factors can be used, the climatic responses, be it thresholds or climate-growth correlations can only be seen within the ecological context of their sampling region.
Less invasive caries management techniques for treating cavitated carious primary teeth, which involve the concept of caries control by managing the activity of the biofilm, are becoming common. This study aimed to compare the clinical efficacy (minor/major failures) and survival rates (successful cases without any failures) of 3 carious lesion treatment approaches, the Hall Technique (HT), non-restorative caries treatment (NRCT), and conventional restorations (CR), for the management of occlusoproximal caries lesions (ICDAS 3-5) in primary molars. Results at 2.5 years are presented. A total of 169 children (3- to 8-year-olds) were enrolled in this secondary care-based, 3-arm parallel-group, randomised controlled trial. Participants were allocated to: HT (n = 52; sealing caries with stainless-steel crowns without caries removal), NRCT (n = 52; opening up the cavity and applying fluoride varnish), CR (n = 65; control arm, complete caries removal and compomer restoration). Statistical analyses were: non-parametric Kruskal-Wallis analysis of variance, Mann-Whitney U test and Kaplan-Meier survival analyses. One hundred and forty-two participants (84%; HT = 40/52; NRCT = 44/52; CR = 58/65) had follow-up data of 1-33 months (mean = 26). Overall, 25 (HT = 2, NRCT = 9, CR = 14) of 142 participants (17.6%) presented with at least 1 minor failure (reversible pulpitis, caries progression, or secondary caries; p = 0.013, CI = 0.012-0.018; Mann-Whitney U test). Ten (HT = 1, NRCT = 4, CR = 5) of 142 participants (7.04%) experienced at least 1 major failure (irreversible pulpitis, abscess, unrestorable tooth; p = 0.043, CI = 0.034-0.045). Independent comparisons between 2 samples found that NRCT-CR had no statistically significant difference in failures (p > 0.05), but for CR-HT (p = 0.037, CI = 0.030-0.040) and for NRCT-HT (p = 0.011, CI = 0.010-0.016; Kruskal-Wallis test) significant differences were observed. Cumulative survival rates were HT = 92.5%, NRCT = 70.5%, and CR = 67.2% (p = 0.012). NRCT and CR outcomes were comparable. HT performed better than NRCT and CR for all outcomes. Zusammenfassung
Zielsetzung: Ziel dieser Studie war es einerseits zu ermitteln, ob Frauen mit hohen Testosteron-Konzentrationen oder niedrigen Sexualhormon-bindendes GlobulinKonzentrationen vermehrt kardiovaskuläre Krankheiten (KVK) entwickeln und andererseits mögliche Assoziationen von Androgenen und Gesamt- und KVKspezifischer Mortalität in der weiblichen Allgemeinbevölkerung zu untersuchen.
Studiendesign und Methodik: Aus der Study of Health in Pomerania (SHIP) wurden Daten von 2129 Frauen (mittleres Alter 49,0 Jahre) mit einem mittleren Follow-up von 10,9 Jahren verwendet. Die Assoziationen von Sexualhormon-Konzentrationen (totales Testosteron (TT), Sexualhormon-bindendes Globulin (SHBG), Androstendion (ASD), freiem Testosteron (fT), freiem Androgene Index (FAI)) und kardiovaskulären Krankheiten (KVK), sowie KVK-spezifischer und Gesamtmortalität wurden im Quer- und Längsschnittdesign mittels multivariabel-adjustierten Regressionsmodellen analysiert.
Ergebnisse: Die Ausgangsmessung zeigte eine KVK-Prävalenz von 17,8% (378 Frauen). Im Follow-up war die KVK-Inzidenz 50,9 pro 1000 Personenjahre. Im altersadjustierten Model zeigte sich eine inverse Assoziation von SHBG und prävalenten KVK (Relatives Risiko pro Standardabweichungsanstieg: 0,83; 95% Konfidenzintervall: 0,74-0,93), diese Assoziation verschwand jedoch im multivariabeladjustierten Modell. KVK-Inzidenz und den Sexualhormon-Konzentrationen waren sowohl im alters-, als auch im multivariabel-adjustierten Regressionsmodell nicht signifikant assoziierte. Für keines der untersuchten Sexualhormone konnte eine Assoziation mit der Gesamtmortalität aufgezeigt werden.
Fazit: Zusammenfassend erbrachte diese populationsbasierte Kohortenstudie keine konsistenten Assoziationen zwischen Androgenen und KVK-Inzidenz oder Mortalität.
Streptococcus pneumoniae infections can lead to severe complications with excessive immune activation and tissue damage. Interleukin-37 (IL-37) has gained importance as a suppressor of innate and acquired immunity, and its effects have been therapeutic as they prevent tissue damage in autoimmune and inflammatory diseases. By using RAW macrophages, stably transfected with human IL-37, we showed a 70% decrease in the cytokine levels of IL-6, TNF-α, and IL-1β, and a 2.2-fold reduction of the intracellular killing capacity of internalized pneumococci in response to pneumococcal infection. In a murine model of infection with S. pneumoniae, using mice transgenic for human IL-37b (IL-37tg), we observed an initial decrease in cytokine expression of IL-6, TNF-α, and IL-1β in the lungs, followed by a late-phase enhancement of pneumococcal burden and subsequent increase of proinflammatory cytokine levels. Additionally, a marked increase in recruitment of alveolar macrophages and neutrophils was noted, while TRAIL mRNA was reduced 3-fold in lungs of IL-37tg mice, resulting in necrotizing pneumonia with augmented death of infiltrating neutrophils, enhanced bacteremic spread, and increased mortality. In conclusion, we have identified that IL-37 modulates several core components of a successful inflammatory response to pneumococcal pneumonia, which lead to increased inflammation, tissue damage, and mortality.
Die Antiseptika-Forschung ist ein wichtiges Gebiet, aber die Testmethoden sind nur bis zur Prüfstufe des Suspensionstests einheitlich geregelt. Daher sind die Ergebnisse sich in der Testhierarchie anschließender Studien schwierig zu vergleichen und zwingen den Praktiker, Therapieentscheidungen oft auf Basis empirischer Erfahrungen zu treffen. In der vorliegenden Arbeit wurde das Verhalten verschiedener in der klinischen Praxis gebräuchlicher Antiseptika in einem Prüfkörpertest untersucht, der an die Wirksamkeit höhere Anforderungen als der klassische Suspensionstest stellt. Dabei sollte auf dem Prüfkörper zugleich eine für Wunden realitätsnahe Belastung eingesetzt werden. Für die Testung wurden Metallträger mit verschiedenen Erregersuspensionen und Belastungen kontaminiert, angetrocknet und die Wirksamkeit von PVP-Iod und Chlorhexidindigluconat bei verschiedenen Einwirkungszeiten ermittelt. Das erfolgte mit der Zielsetzung festzustellen, ob sich einer der beiden Wirkstoffe als Referenz für künftige Prüfkörpertests eignet. PVP-Iod zeigte in allen Versuchen unabhängig vom Testorganismus (S. aureus, P. aeruginosa und E. faecium) und von der verwendeten Belastung (ohne, MEM + 10 % FBS, 10 bzw. 30 % Vollblut) eine rasch einsetzende, stabile und ausreichende antiseptische Wirkung (ohne Belastung RF > 5, mit Belastung RF > 3 [Pitten et al. 2003]). Das bildete einen deutlichen Kontrast zu den mit Chlorhexidindigluconat erzielten Ergebnissen. Die mit PVP-Iod und Chlorhexidindigluconat gewonnenen Ergebnisse wurden mit den Ergebnissen einer vorangegangenen Studie [Brautferger 2010] zu Octenidindihydrochlorid und Polihexanid verglichen. Auch als Schlussfolgerung dieses Vergleiches kann PVP-Iod als Referenzsubstanz für die Antiseptikatestung empfohlen werden. Obwohl PVP-Iod in Fragen von Verträglichkeit, Remanenz etc. von den neueren Antiseptika übertroffen wird, zeigte kein anderer Wirkstoff in unseren Testungen ein vergleichbar stabiles Wirkungsverhalten. In Verbindung mit der jahrzehntelangen Erfahrung im Einsatz von PVP-Iod kann daher PVP-Iod als Referenzsubstanz für die Antiseptikatestung empfohlen werden.
In dieser retrospektiven Arbeit wurde untersucht, inwiefern Einflussfaktoren (im Folgenden Risikofaktoren) wie Diabetes mellitus, Adipositas, rheumatoide Arthritis sowie invasive Voreingriffe am Kniegelenk das klinische Outcome der Patienten bei bereits festgestellter periprothetischer Infektion beeinflussen. Das Patientenkollektiv hierzu bildeten 74 Personen, welche in der Orthopädischen Klinik der Universitätsmedizin Greifswald zwischen 01.01.2006 - 30.06.2013 wegen einer infizierten Knie-TEP behandelt wurden. Die Infektionsrate lag dabei insgesamt bei 2,3%. Nach entsprechender Datengewinnung wurden innerhalb des Patientenkollektivs vier Gruppenpaare gebildet, wobei pro untersuchtes Paar eine Gruppe mit jeweiligem Risikofaktor von einer entsprechenden Kontrollgruppe ohne den untersuchten Risikofaktor separiert wurde. Diese wurden durch statistische Tests auf signifikante Unterschiede bezüglich des objektiven und subjektiven Outcomes hin untersucht. Bei der Betrachtung des objektiven Outcomes, worin alle Patienten eingeschlossen waren, bestätigte sich der Verdacht, dass Diabetes mellitus sowie Adipositas den klinischen Verlauf bei periprothetischer Infektion negativ beeinflussen, was auch im Vergleich zur Literatur bestätigt werden konnte. Die primär durchgeführte Therapie zum Erhalt der einliegenden Prothese vor allem durch Inlay-Wechsel war in diesen beiden Risikofaktoren weniger erfolgreich als in den jeweiligen Kontrollgruppen. Sogar der TEP-Wechsel als primäre Infektsanierung war in der Gruppe der Diabetiker signifikant erfolgloser als in der jeweiligen Kontrollgruppe. Dies führte insgesamt zu vermehrten operativen Eingriffen bis zum Ende des Beobachtungszeitraumes. Das subjektive Outcome wurde durch den SF-36 Score mittels Fragebogen und telefonischer Kontaktaufnahme ermittelt, wobei 50% aller Patienten teilnahmen. Hierbei zeigte sich ausschließlich bei Diabetes mellitus eine signifikant geringere körperliche Funktion im Vergleich zur Kontrollgruppe. Bezüglich der Risikofaktoren "rheumatoide Arthritis" und "Voreingriffe am Kniegelenk" hat sich generell keine signifikante Beeinflussung des objektiven und subjektiven Outcomes gezeigt. Zusammenfassend kann Diabetes mellitus und Adipositas als Risikofaktor für ein schlechteres Outcome bei festgestellter Knie-TEP Infektion angesehen werden. Vor allem bei der Auswahl der passenden Primärtherapie nach festgestellter Infektion muss die klinische Gesamtsituation betrachtet werden. Dabei ist der TEP-Erhalt vor allem durch Inlay-Wechsel bei Adipositas und Diabetes mellitus weniger erfolgsversprechend ebenso wie der TEP-Wechsel bei Diabetikern, was bei der Therapieplanung bedacht werden sollte, um zukünftig multiple operative Eingriffe bei adipösen Patienten und körperliche Funktionseinschränkungen bei Diabetikern zu vermeiden.
Encephalitides induced by lyssa-, borna- and astroviruses: molecular detection and characterization

Encephalitis is a severe inflammatory disease of the brain which often has a fatal outcome or can lead to subsequent damages. In around two-thirds of all human encephalitis cases, the causative agent is, despite improved diagnostics, unknown today. Aim of this work was the development, improvement and validation of diagnostic methods, improvement of sampling strategies and the development of optimized systems for characterization of three viruses causing viral encephalitis. The main burden of RABV lies in developing countries, were standard diagnostic tools are often not realizable. Therefore, simple and rapid diagnostic tests for the use under resource limited settings, so called point-of-care tests (POCT), are favorable. Commercially available lateral flow device (LFD) based immunodiagnostic tests were analyzed and failed in terms of sensitivity compared to the standard FAT and RT-qPCR (Paper I). Therefore, molecular RABV alternative targeting genome tests were developed and combined with rapid nucleic acid extraction methods. The new HighSpeed RT-qPCR and RPA assays together with magnetic bead based automated or manual extraction methods delivered a specificity between 100% and 97.2% and a limit of detection of 10 or 1,000 genome copies per reaction, respectively and seem suitable as novel POCT (Paper II). Recently, a novel zoonotic VSBV-1, responsible for fatal encephalitis of three squirrel breeders, was detected. For further investigations of this new virus, methods for an in-vivo sampling approach of squirrels were established. They were useful to identify animals harboring this dangerous virus, and new sequence data could be obtained from the VSBV-1 positive animals. Until now, 3.5% of all investigated squirrels were VSBV-1 RNA positive and two subfamilies (Sciurinae and Callosciurinae) are affected. The pathogen occurs not only in Germany, but also squirrel holdings and zoological gardens in the Netherlands and Croatia were tested positive, indicating a serious human health threat of this virus (Paper III and IV). With the help of a metagenomic approach, astroviruses were detected to be associated to encephalitis in cattle and sheep. These viruses were detected in a cow in Germany (Paper V), and in brain samples from two sheep in the United Kingdom (Paper VI). In both cases, the sequences generated by high-throughput-sequencing (HTS) were confirmed by specific RT-qPCRs, which could be used for subsequent screening approaches. Together, methods for the detection of three different encephalitis viruses were developed, validated and applied for different sample material.
Multiple evidence in animal models and in humans suggest a beneficial role of cold physical
plasma in wound treatment. Yet, risk assessment studies are important to further foster therapeutic
advancement and acceptance of cold plasma in clinics. Accordingly, we investigated the long-term
side effects of repetitive plasma treatment over 14 consecutive days in a rodent full-thickness ear
wound model. Subsequently, animals were housed for 350 days and sacrificed thereafter. In blood,
systemic changes of the pro-inflammatory cytokines interleukin 1β and tumor necrosis factor α
were absent. Similarly, tumor marker levels of α-fetoprotein and calcitonin remained unchanged.
Using quantitative PCR, the expression levels of several cytokines and tumor markers in liver,
lung, and skin were found to be similar in the control and treatment group as well. Likewise,
histological and immunohistochemical analysis failed to detect abnormal morphological changes
and the presence of tumor markers such as carcinoembryonic antigen, α-fetoprotein, or the neighbor
of Punc 11. Absence of neoplastic lesions was confirmed by non-invasive imaging methods such as
anatomical magnetic resonance imaging and positron emission tomography-computed tomography.
Our results suggest that the beneficial effects of cold plasma in wound healing come without apparent
side effects including tumor formation or chronic inflammation.
Die offene Dekompression des N. ulnaris beim Kubitaltunnelsyndrom über eine kleine Schnittführung von 3 - 4 cm ist eine suffiziente Technik, die der endoskopischen Dekompression weder im Hinblick auf postoperative Schmerzen und damit auch die Länge der Arbeitsunfähigkeit, noch in Bezug auf das neurologische Outcome unterlegen ist. Bereits im frühen Follow-up konnte in der offen operierten Gruppe dieselbe signifikante Verbesserung wie in der endoskopischen Behandlungsgruppe festgestellt werden. Allerdings stellt die offene Dekompression mit signifikant weniger postoperativen Hämatomen die komplikationsärmere Methode dar. Die von einigen Autoren beschriebenen, weit im M. flexor carpi ulnaris gelegenen, faszienähnlichen Bänder, die zur Nervenkompression beitragen sollen, konnten bei unseren Patienten der endoskopischen Behandlungsgruppe nicht dargestellt werden. Es fanden sich keine den Nerven komprimierenden Strukturen weiter als 4 cm distal von der Mitte des knöchernen Sulcus ulnaris ausgehend. In der Gesamtheit zeigen die Ergebnisse dieser Arbeit, dass eine weit nach distal gerichtete Nervendekompression komplikationsträchtiger, aber in Routinefällen in der Regel unnötig ist und keine Vorteile bezüglich des klinischen Outcomes bietet. Daher wird die offene Dekompression über eine kleine Hautinzision als die Therapie der Wahl beim Kubitaltunnelsyndrom bewertet.
