Background
Signs of an inflammatory process have been described in major depression.
Methods
In a double-blind, randomized study of celecoxib or placebo add-on to reboxetine in 40 depressed patients, celecoxib treatment has beneficial effects. In order to evaluate the tryptophan/kynurenine metabolism and to identify predictors for remission, tryptophan (TRP), kynurenine (KYN), kynurenic acid (KYNA), and quinolinic acid (QUIN) were estimated in the serum of 32 patients before and after treatment and in a group of 20 healthy controls.
Results
KYN levels were significantly lower in patients (p = 0.008), and the QUIN/KYN ratios were significantly higher (p = 0.028). At baseline, the higher KYN/TRP ratio was predictive for remission during celecoxib add-on treatment (p = 0.04) as well as for remission in the overall patient group (p = 0.01). In the placebo group, remitters showed a higher KYNA/QUIN ratio (p = 0.032). In the overall group, remitters showed lower KYNA/KYN (p = 0.035) and QUIN/KYN (p = 0.011) ratios. The lower the formation of downstream metabolites, especially QUIN, the better the treatment outcome.
Conclusion
The high KYN/TRP ratio predicted remission after treatment with celecoxib in this small sample of depressed patients. Eventually, the KYN/TRP ratio might be a marker for those patients, which benefit from an additional anti-inflammatory treatment.
Existing literature evidences the association between adolescents’ school self-concept and engagement, both concepts being related to students’ perception of teachers and peers as motivators. However, few longitudinal studies explore the interplay of these factors. The present study aims to close this gap, applying latent cross-lagged panel design to two-wave data from German adolescent students [1088 8th grade students at T1 (Mage = 13.7, SD = 0.53; 53.9% girls) and 845 9th grade students at T2 (Mage = 14.86; SD = 0.57; 55% girls) from the initial sample]. Besides direct effects, three cross-lagged over-time paths were found to be significant: students’ perception of peers as positive motivators (PPMs) at the beginning of 8th grade (T1) positively predicts their behavioral school engagement at the end of 9th grade (T2), as well as emotional school engagement at the beginning of 8th grade positively predicts students’ perception of PPMs 1.5 years later. Furthermore, behavioral school engagement at T1 functions as a predictor of a student’s school self-concept at T2.
Longitudinal Effects of Student-Perceived Classroom Support on Motivation – A Latent Change Model

This two-wave longitudinal study examined how developmental changes in students’ mastery goal orientation, academic effort, and intrinsic motivation were predicted by student-perceived support of motivational support (support for autonomy, competence, and relatedness) in secondary classrooms. The study extends previous knowledge that showed that support for motivational support in class is related to students’ intrinsic motivation as it focused on the developmental changes of a set of different motivational variables and the relations of these changes to student-perceived motivational support in class. Thus, differential classroom effects on students’ motivational development were investigated. A sample of 1088 German students was assessed in the beginning of the school year when students were in grade 8 (Mean age = 13.70, SD = 0.53, 54% girls) and again at the end of the next school year when students were in grade 9. Results of latent change models showed a tendency toward decline in mastery goal orientation and a significant decrease in academic effort from grade 8 to 9. Intrinsic motivation did not decrease significantly across time. Student-perceived support of competence in class predicted the level and change in students’ academic effort. The findings emphasized that it is beneficial to create classroom learning environments that enhance students’ perceptions of competence in class when aiming to enhance students’ academic effort in secondary school classrooms.
Activation of trace amine-associated receptor 1 (TAAR1) in endocrine pancreas is involved in weight regulation and glucose homeostasis. The purpose of this study was the identification and characterization of potential TAAR1 variants in patients with overweight/obesity and disturbed glucose homeostasis. Screening for TAAR1 variants was performed in 314 obese or overweight patients with impaired insulin secretion. The detected variants were functionally characterized concerning TAAR1 cell surface expression and signaling properties and their allele frequencies were determined in the population-based Study of Health in Pomerania (SHIP). Three heterozygous carriers of the single nucleotide missense variants p.Arg23Cys (R23C, rs8192618), p.Ser49Leu (S49L, rs140960896), and p.Ille171Leu (I171L, rs200795344) were detected in the patient cohort. While p.Ser49Leu and p.Ille171Leu were found in obese/overweight patients with slightly impaired glucose homeostasis, p.Arg23Cys was identified in a patient with a complete loss of insulin production. Functional in vitro characterization revealed a like wild-type function for I171L, partial loss of function for S49L and a complete loss of function for R23C. The frequency of the R23C variant in 2018 non-diabetic control individuals aged 60 years and older in the general population-based SHIP cohort was lower than in the analyzed patient sample. Both variants are rare in the general population indicating a recent origin in the general gene pool and/or the consequence of pronounced purifying selection, in line with the obvious detrimental effect of the mutations. In conclusion, our study provides hints for the existence of naturally occurring TAAR1 variants with potential relevance for weight regulation and glucose homeostasis.
Resource and cost constraints in hospitals demand thorough planning of operating room schedules. Ideally, exact start times and durations are known in advance for each case. However, aside from the first case’s start, most factors are hard to predict. While the role of the start of the first case for optimal room utilization has been shown before, data for to-follow cases are lacking. The present study therefore aimed to analyze all elective surgery cases of a university hospital within 1 year in search of visible patterns. A total of 14,014 cases scheduled on 254 regular working days at a university hospital between September 2015 and August 2016 underwent screening. After eliminating 112 emergencies during regular working hours, 13,547 elective daytime cases were analyzed, out of which 4,346 ranked first, 3,723 second, and 5,478 third or higher in the daily schedule. Also, 36% of cases changed start times from the day before to 7:00 a.m., with half of these (52%) resulting in a delay of more than 15 min. After 7:00 a.m., 87% of cases started more than 10 min off schedule, with 26% being early and 74% late. Timeliness was 15 ± 72 min (mean ± SD) for first, 21 ± 84 min for second, and 25 ± 93 min for all to-follow cases, compared to preoperative day planning, and 21 ± 45, 23 ± 61, and 19 ± 74 min compared to 7:00 a.m. status. Start time deviations were also related to procedure duration, with cases of 61–90 min duration being most reliable (deviation 9.8 ± 67 min compared to 7:00 a.m.), regardless of order. In consequence, cases following after 61–90 min long cases had the shortest deviations of incision time from schedule (16 ± 66 min). Taken together, start times for elective surgery cases deviate substantially from schedule, with first and second cases falling into the highest mean deviation category. Second cases had the largest deviations from scheduled times compared to first and all to-follow cases. While planned vs. actual start times differ among specialties, cases of 61–90 min duration had the most reliable start times, with neither shorter nor longer cases seeming to improve timeliness of start times.
Background: We assessed the effect of the uniform donor questionnaire (UDQ) on deferral rates in first-time and repeat donors. We focused on the introduced question about unprotected sexual contact with a new partner. Another goal was a stratified comparison of the deferral rates of the donor questionnaire (DQ) and UDQ. Methods: Data on donors and deferrals using the DQ and UDQ were collected at four blood establishments. The comparison included a 2-year period by questionnaire version. For the comparison of the questionnaires, an adjusted multinomial logistic regression was performed. Results: The analysis included 260,848 donations. First-time (FTD) and repeat donations (RD) showed higher deferral rates with the UDQ (FTD +5.4%, RD +1.4%). Deferral due to a new partner was 3.0% in first-time and 0.4% in repeat donors. The majority of these occurred in the youngest age groups. The most frequent deferral criterion was ‘disease' (5.1%). Conclusion: The regression revealed stronger predictors for deferral than the questionnaire version. Especially younger age carried a higher and independent risk for deferral. The additional deferrals of mainly young first-time donors due to a new sexual partner may identify those donors with potential heterosexual risk behavior who would otherwise not be identified.
In healthy older adults, resveratrol supplementation has been shown to improve long-term glucose control, resting-state functional connectivity (RSFC) of the hippocampus, and memory function. Here, we aimed to investigate if these beneficial effects extend to individuals at high-risk for dementia, i.e., patients with mild cognitive impairment (MCI). In a randomized, double-blind interventional study, 40 well-characterized patients with MCI (21 females; 50–80 years) completed 26 weeks of resveratrol (200 mg/d; n = 18) or placebo (1,015 mg/d olive oil; n = 22) intake. Serum levels of glucose, glycated hemoglobin A1c and insulin were determined before and after intervention. Moreover, cerebral magnetic resonance imaging (MRI) (3T) (n = 14 vs. 16) was conducted to analyze hippocampus volume, microstructure and RSFC, and neuropsychological testing was conducted to assess learning and memory (primary endpoint) at both time points. In comparison to the control group, resveratrol supplementation resulted in lower glycated hemoglobin A1c concentration with a moderate effect size (ANOVARM p = 0.059, Cohen's d = 0.66), higher RSFC between right anterior hippocampus and right angular cortex (p < 0.001), and led to a moderate preservation of left anterior hippocampus volume (ANOVARM p = 0.061, Cohen's d = 0.68). No significant differences in memory performance emerged between groups. This proof-of-concept study indicates for the first-time that resveratrol intake may reduce glycated hemoglobin A1c, preserves hippocampus volume, and improves hippocampus RSFC in at-risk patients for dementia. Larger trials with longer intervention time should now determine if these benefits can be validated and extended to cognitive function.
Structural alterations in the corpus callosum (CC), the major white matter tract connecting functionally related brain regions in the two hemispheres, have been shown to be associated with emotional instability, impulsivity and suicidality in various mental disorders. To explore whether structural alterations of the CC would be similarly associated with emotional instability, impulsivity and suicidality in borderline personality disorder (BPD), we used diffusion tensor imaging (DTI) to assess the structural integrity of the CC in 21 BPD and 20 healthy control (HC) participants. Our hypothesis-driven analyses revealed a positive correlation between BPD participants’ suicidal behavior and fractional anisotropy (FA) in the splenium and genu of the CC and a negative correlation between BPD participants’ suicidal behavior and mean diffusivity (MD) in the splenium of CC. Our exploratory analyses suggested that suicidal BPD participants showed less FA and more MD in these regions than HC participants but that non-suicidal BPD participants showed similar FA and MD in these regions as HC participants. Taken together, our findings suggest an association between BPD participants’ suicidal behavior and structural alterations in regions of the CC that are connected with brain regions implicated in emotion regulation and impulse control. Structural alterations of the CC may, thus, account for deficits in emotion regulation and impulse control that lead to suicidal behavior in BPD. However, these findings should be considered as preliminary until replicated and extended in future studies that comprise larger samples of suicidal and non-suicidal BPD participants.
Background
There is a lack of data concerning socioeconomic outcome and quality of life (QoL) in patients after status epilepticus (SE) in Germany.
Patients and methods
Adult patients treated between 2011 and 2015 due to SE at the university hospitals in Frankfurt, Greifswald, and Marburg were asked to fill out a questionnaire regarding long-term outcome of at least 3 months after discharge. The SE cohort consisted of 25.9% patients with an acute symptomatic, 42% with a remote symptomatic and previous epilepsy, 22.2% with a new-onset remote symptomatic, and 9.9% with other or unknown etiology. A matched case–control analysis was applied for comparison with patients with drug refractory epilepsy and seizure remission, both not previously affected by SE.
Results
A total of 81 patients (mean age: 58.7 ± 18.0 years; 58% female) participated. A non-refractory course was present in 59.3%, while 27.2% had a refractory SE (RSE) and 13.6% had a superrefractory SE (SRSE). Before admission, a favorable modified Rankin Scale (mRS) of 0–3 was found in 82.7% (67/81), deteriorating to 38.3% (31/81) (p = 0.003) at discharge. The majority returned home [51.9% (42/81)], 32.1% entered a rehabilitation facility, while 12.3% were transferred to a nursing home and 3.7% to another hospital. The overall mRS at follow-up did not change; 61.8% (45/74) reached an mRS of 0–3. In RSE and SRSE, the proportion with a favorable mRS increased from 45.5% at discharge to 70% at follow-up, while QoL was comparable to a non-refractory SE course. Matched epilepsy controls in seizure remission were treated with a lower mean number of anticonvulsants (1.3 ± 0.7) compared to controls with drug refractory epilepsy (1.9 ± 0.8; p < 0.001) or SE (1.9 ± 1.1; p < 0.001). A major depression was found in 32.8% of patients with SE and in 36.8% of drug refractory epilepsy, but only in 20.3% of patients in seizure remission. QoL was reduced in all categories (QOLIE-31) in SE patients in comparison with patients in seizure remission, but was comparable to patients with drug refractory epilepsy.
Discussion
Patients after SE show substantial impairments in their QoL and daily life activities. However, in the long term, patients with RSE and SRSE had a relatively favorable outcome comparable to that of patients with a non-refractory SE course. This underlines the need for efficient therapeutic options in SE.
Background: The association of polyomaviruses BK and JC with other opportunistic infections and graft-versus-host disease (GvHD) in allogeneic stem cell transplantation is controversially discussed. Methods: We conducted a retrospective study of 64 adult patients who received their first allogeneic stem cell transplantation between March 2010 and December 2014; the follow-up time was 2 years. Results: Acute leukemia was the most frequent underlying disease (45.3%), and conditioning included myeloablative (67.2%) and nonmyeloablative protocols (32.8%). All patients received 10 mg of alemtuzumab on day -2 (20 mg in case of mismatch) as GvHD prophylaxis. Twenty-seven patients (41.5%) developed cytomegalovirus (CMV) reactivation. BKPyV-associated hemorrhagic cystitis was diagnosed in 10 patients (15.6%). Other opportunistic infections caused by viruses or protozoa occurred rarely (<10%). There was no association of BKPyV or JCPyV with CMV reactivation, Epstein-Barr virus reactivation, human herpes virus 6, or parvovirus B19 infection requiring treatment. There was a significant correlation of BKPyV-associated hemorrhagic cystitis with toxoplasmosis (p = 0.013). Additionally, there was a significant link of simultaneous BKPyV and JCPyV viruria with toxoplasmosis (p = 0.047). BKPyV and JCPyV were not associated with GvHD, relapse, or death. Conclusion: We found no association of BKPyV or JCPyV with viral infections or GvHD. Only the correlation of both polyomaviruses with toxoplasmosis was significant. This is a novel and interesting finding.
Objective: We performed a pilot RCT to prove the hypothesis that a controlled ingestion of polyphenol-rich beverages (soy drink, decaffeinated black tea) in nutritive dosages by nursing women has an effect on the composition (flavonoid concentration, total antioxidant capacity) of breast milk. Methods: Healthy nursing women were supplemented with either 250 mL of a soy drink (12 mg isoflavones; n = 18), 300 mL decaffeinated black tea (67 mg catechins; n = 18), or 300 mL water (n = 8, control) for 6 days. Milk samples were collected before, during, and after intervention. Flavonoid content (isoflavones/catechins, HPLC) and total antioxidant capacity of milk and test drinks in milk specimens were assessed. Results: Isoflavone content (genistein and daidzein) in breast milk increased up to 12 nmol/L after soy drink consumption; the major flavonoids constituents of black tea (catechin, epicatechin, and respective conjugates) could not be detected in milk samples. With both interventions, the total antioxidant capacity of breast milk was not affected. Conclusions: Mothers' daily consumption of a soy drink considerably increases isoflavone content of breast milk resulting in an estimated daily exposure of 9.6 nmol isoflavones in a 4-month-old suckling infant. Luminal flavanol uptake from black tea consumed by the nursing mother may be too low to affect flavanol concentrations in breast milk.
Less invasive caries management techniques for treating cavitated carious primary teeth, which involve the concept of caries control by managing the activity of the biofilm, are becoming common. This study aimed to compare the clinical efficacy (minor/major failures) and survival rates (successful cases without any failures) of 3 carious lesion treatment approaches, the Hall Technique (HT), non-restorative caries treatment (NRCT), and conventional restorations (CR), for the management of occlusoproximal caries lesions (ICDAS 3-5) in primary molars. Results at 2.5 years are presented. A total of 169 children (3- to 8-year-olds) were enrolled in this secondary care-based, 3-arm parallel-group, randomised controlled trial. Participants were allocated to: HT (n = 52; sealing caries with stainless-steel crowns without caries removal), NRCT (n = 52; opening up the cavity and applying fluoride varnish), CR (n = 65; control arm, complete caries removal and compomer restoration). Statistical analyses were: non-parametric Kruskal-Wallis analysis of variance, Mann-Whitney U test and Kaplan-Meier survival analyses. One hundred and forty-two participants (84%; HT = 40/52; NRCT = 44/52; CR = 58/65) had follow-up data of 1-33 months (mean = 26). Overall, 25 (HT = 2, NRCT = 9, CR = 14) of 142 participants (17.6%) presented with at least 1 minor failure (reversible pulpitis, caries progression, or secondary caries; p = 0.013, CI = 0.012-0.018; Mann-Whitney U test). Ten (HT = 1, NRCT = 4, CR = 5) of 142 participants (7.04%) experienced at least 1 major failure (irreversible pulpitis, abscess, unrestorable tooth; p = 0.043, CI = 0.034-0.045). Independent comparisons between 2 samples found that NRCT-CR had no statistically significant difference in failures (p > 0.05), but for CR-HT (p = 0.037, CI = 0.030-0.040) and for NRCT-HT (p = 0.011, CI = 0.010-0.016; Kruskal-Wallis test) significant differences were observed. Cumulative survival rates were HT = 92.5%, NRCT = 70.5%, and CR = 67.2% (p = 0.012). NRCT and CR outcomes were comparable. HT performed better than NRCT and CR for all outcomes. Streptococcus pneumoniae infections can lead to severe complications with excessive immune activation and tissue damage. Interleukin-37 (IL-37) has gained importance as a suppressor of innate and acquired immunity, and its effects have been therapeutic as they prevent tissue damage in autoimmune and inflammatory diseases. By using RAW macrophages, stably transfected with human IL-37, we showed a 70% decrease in the cytokine levels of IL-6, TNF-α, and IL-1β, and a 2.2-fold reduction of the intracellular killing capacity of internalized pneumococci in response to pneumococcal infection. In a murine model of infection with S. pneumoniae, using mice transgenic for human IL-37b (IL-37tg), we observed an initial decrease in cytokine expression of IL-6, TNF-α, and IL-1β in the lungs, followed by a late-phase enhancement of pneumococcal burden and subsequent increase of proinflammatory cytokine levels. Additionally, a marked increase in recruitment of alveolar macrophages and neutrophils was noted, while TRAIL mRNA was reduced 3-fold in lungs of IL-37tg mice, resulting in necrotizing pneumonia with augmented death of infiltrating neutrophils, enhanced bacteremic spread, and increased mortality. In conclusion, we have identified that IL-37 modulates several core components of a successful inflammatory response to pneumococcal pneumonia, which lead to increased inflammation, tissue damage, and mortality.
Background: Mild-to-moderate chronic kidney disease (CKD G3a) is prevalent in older adults. Substantial evidence suggests that individuals with advanced CKD face a high risk for common geriatric conditions, like functional impairment and cognitive decline, whereas the relationships between mild-to-moderate CKD and functional impairment and cognitive decline, but also poor nutritional status and mood disorders, are still unclear. Objective: The aim of this study was to explore associations between mild-to-moderate CKD and impairments in the core domains of geriatric assessment (GA) in a large cohort of community-dwelling older adults. Methods: This was a cross-sectional analysis of 1,476 participants of the Berlin Aging Study II. Study participants were stratified as to presence or absence of CKD G3a (estimated glomerular filtration rate [eGFR] 45-59 mL/min/1.73 m<sup>2</sup> vs. eGFR ≥60 mL/min/1.73 m<sup>2</sup>). GA comprised the following instruments: the Activities of Daily Living Scale (ADL), the Timed up and Go (TUG), the Tinetti test (Tinetti), the Mini-Mental-State Examination (MMSE), the Geriatric Depression Scale (GDS), and the Mini Nutritional Assessment (MNA). We used logistic regression models to estimate multivariable-adjusted associations between CKD G3a and impairments in the respective domains. Results: A total of 282 subjects with mild-to-moderate CKD (CKD G3a) were identified (19.1%). Overall, the prevalence of impairments identified was higher among subjects with compared to without CKD G3a (21 vs. 15.9%, p = 0.043). In multivariable-adjusted models, CKD G3a was consistently associated with increased odds of an impaired gait performance as to the TUG (adjusted odds ratio 2.06, 95% CI 1.04-4.09). In contrast, on average, individuals with and without CKD G3a did not differ as to their results in the MMSE, the ADL, the MNA, and the GDS. Conclusion: GA identified impairments in 21 versus 15.9% of older adults with and without mild-to-moderate CKD, respectively. However, except for an increased likelihood of impaired gait performance (TUG) with mild-to-moderate CKD, we did not find independent associations between mild-to-moderate CKD and geriatric conditions.
Background: Securing future blood supply is a major issue of transfusion safety. In this prospective 10-year longitudinal study we enrolled all blood donation services and hospitals of the federal state Mecklenburg-Western Pomerania. Methods and Results: From 2005 to 2015 (time period with major demographic effects), whole blood donation numbers declined by 18%. In male donors this paralleled the demographic change, while donation rates of females declined 12.4% more than expected from demography. In parallel, red cell transfusion rates/1,000 population decreased from 2005 to 2015 from 56 to 51 (-8.4%), primarily due to less transfusions in patients >60 years. However, the transfusion demand declined much less than blood donation numbers: -13.5% versus -18%, and the population >65 years (highest transfusion demand) will further increase. The key question is whether the decline in transfusion demand observed over the previous years will further continue, hereby compensating for reduced blood donation numbers due to the demographic change. The population structure of Mecklenburg-Western Pomerania reflects all Eastern German federal states, while the Western German federal states will reach similar ratios of age groups 18-64 years / ≥65 years about 10 years later. Conclusions: Regular monitoring of age- and sex-specific donation and transfusion data is urgently required to allow transfusion services strategic planning for securing future blood supply.
Wound antisepsis has undergone a renaissance due to the introduction of highly effective wound-compatible antimicrobial agents and the spread of multidrug-resistant organisms (MDROs). However, a strict indication must be set for the application of these agents. An infected or critically colonized wound must be treated antiseptically. In addition, systemic antibiotic therapy is required in case the infection spreads. If applied preventively, the Wounds-at-Risk Score allows an assessment of the risk for infection and thus appropriateness of the indication. The content of this updated consensus recommendation still largely consists of discussing properties of octenidine dihydrochloride (OCT), polihexanide, and iodophores. The evaluations of hypochlorite, taurolidine, and silver ions have been updated. For critically colonized and infected chronic wounds as well as for burns, polihexanide is classified as the active agent of choice. The combination 0.1% OCT/phenoxyethanol (PE) solution is suitable for acute, contaminated, and traumatic wounds, including MRSA-colonized wounds due to its deep action. For chronic wounds, preparations with 0.05% OCT are preferable. For bite, stab/puncture, and gunshot wounds, polyvinylpyrrolidone (PVP)-iodine is the first choice, while polihexanide and hypochlorite are superior to PVP-iodine for the treatment of contaminated acute and chronic wounds. For the decolonization of wounds colonized or infected with MDROs, the combination of OCT/PE is preferred. For peritoneal rinsing or rinsing of other cavities with a lack of drainage potential as well as the risk of central nervous system exposure, hypochlorite is the superior active agent. Silver-sulfadiazine is classified as dispensable, while dyes, organic mercury compounds, and hydrogen peroxide alone are classified as obsolete. As promising prospects, acetic acid, the combination of negative pressure wound therapy with the instillation of antiseptics (NPWTi), and cold atmospheric plasma are also subjects of this assessment.
Der vorliegende Übersichtsartikel belegt zunächst, warum das Extinktionslernen als ein zentraler Wirkmechanismus der Expositionsbehandlung angesehen wird. Nach Darstellung der lerntheoretischen Grundlagen wird ein Modell präsentiert, das die Grundlagen der Ausformung, Konsolidierung und des Abrufs des Extinktionsgedächtnisses beschreibt. Dieses Gedächtnismodell der Extinktion liefert die Basis für die Diskussion der aktuellen neurowissenschaftlichen Erkenntnisse zum Extinktionslernen. Durch diese Befunde im Tier-, aber auch im Humanbereich ist es gelungen, die neuronalen Schaltkreise der Extinktion relativ gut zu beschreiben. Der Übersichtsartikel gibt eine Zusammenfassung dieser aktuellen Befunde und geht außerdem auf einige Neurotransmittersysteme dieser Schaltkreise ein, zumindest in dem Maße, wie sie für die Befunde zur pharmakologischen Unterstützung des Extinktionslernens relevant sind. Anschließend wird ein integratives Modell vorgestellt, das den Ausgangspunkt für die Optimierung der Extinktion in der Expositionstherapie liefert. Den Abschluss bildet eine Kasuistik, in der diese Optimierungsstrategien nochmals am Beispiel der Behandlung einer Patientin mit Emetophobie für die Praxis verdeutlicht werden.
Background: ‘Quality in medicine' is a term used in a broad sense. In this work the definition and dimensions of quality in medicine and the implementation of a measurement and reporting system in Germany are discussed. Existing applications are described and possible future effects are pointed out. Methods: The ongoing process of implementing a quality reporting system into the German healthcare system is studied by publicly available legal texts, published reactions of stakeholders and publications of G-BA and IQTIG. Definitions of quality, dimensions of quality and quality measurement in medicine are studied by using textbooks as well as the world wide web and PubMed search. Results: Donabedian‘s ‘dimensions of quality' are fundamental in dealing with quality in medicine. Existing measurement and reporting systems have immanent strengths and weaknesses, as the definition of quality is affected by one‘s point of view. The legislator will have to decide which ‘dimension of quality' is mandatory and how to measure it. Conclusion: Quality has become a control instrument with unforeseeable consequences. A clear definition of the used quality concept is as essential as the use of feasible measurement and reporting systems. The use of routine data could be an interesting option.
Background: The plasminogen activator system plays a key role in ovarian cancer (OC) tumor progression. The plasminogen activator inhibitor type 1 (PAI-1) and the recently identified PAI-1 RNA binding protein 1 (PAI-RBP1) are primary regulators of plasminogen activation and thus are putative biomarkers for OC progression. Methods: One hundred fifty six OC patients were analyzed to identify the presence of PAI-1 and PAI-RBP1 and subsequently correlated to clinicopathological parameters. Primary cells obtained from OC patient samples were applied in fluorescence microscopy analysis for examination of PAI-1 and PAI-RBP1 distribution. Results: PAI-1 and PAI-RBP1 have been found to be predictive markers for OC patients' outcome. PAI-1 levels significantly correlated with volume of ascites, FIGO staging, and lymph node status. PAI-RBP1 expression significantly correlated with age at first diagnosis, histological tumor type, presence of distant metastasis (pM), and recurrence. PAI-1 showed a trend toward association and PAI-RBP1 was significantly associated with progression-free survival. Notably, PAI-1 protein in recurrent OC tissues was exclusively localized in the nucleus. Conclusion: This study has shown that a combination of PAI-1 and PAI-RBP1 may represent novel prognostic factor for OC. Prospective trials are needed.
Osteoporose und Atherosklerose wurden lange als unabhängige Erkrankungen mit
gemeinsamen Risikofaktoren betrachtet, jedoch wurde in den letzten Jahren auch ein
unabhängiger Zusammenhang zwischen beiden Erkrankungen postuliert. Bisherige Studien
zur Assoziation zwischen Osteoporose und Atherosklerose sind oft auf postmenopausale
Frauen beschrankt und es gibt es nur wenige Studien, die die Knochengesundheit anhand von Osteosonographiemesswerten beurteilen. Ziel der vorliegenden Arbeit war zu untersuchen, ob es eine unabhängige Assoziation zwischen den Steosonographiemesswerten an der Ferse und der IMT der A. carotis, dem Vorhandensein von Plaques der A. carotis und dem ABI gibt. Grundlage für die Analyse bilden die Daten von SHIP-2 und SHIP-Trend. Multivariate logistische Regressionsmodelle zeigten einen von klassischen kardiovaskularen Risikofaktoren (Hypertonie, Dyslipidamie, Diabetes mellitus) unabhängigen Zusammenhang zwischen den Osteosonographiemesswerten und dem Vorkommen von Plaques der A. carotis bei Männern. Bei Frauen konnte jedoch keine entsprechende Assoziation detektiert werden. Darüber hinaus konnten keine Assoziationen zwischen den Steosonographiemesswerten und der IMT der CCA oder dem ABI in vollständig adjustierten Modellen detektiert werden. IMT und Plaques sind Marker für Atherosklerose, die sich sowohl in ihrer Pathogenese, im Risikofaktorprofil sowie in ihrer Vorhersagefähigkeit fur kardiovaskulare Ereignisse unterscheiden. Plaques stellen dabei den härteren Endpunkt der Atherosklerose dar, was erklären konnte, warum unabhängige Assoziationen zwischen den Osteosonographiemesswerten und Plaques beobachtet wurden aber nicht zur IMT.
Geschlechtsspezifische Unterschiede im Knochenstoffwechsel und der kardiovaskularen
Gesundheit sind denkbare Ursachen für die ausschließlich bei den Männern beobachteten
Assoziationen. Der pathophysiologische Zusammenhang zwischen Osteoporose und
Atherosklerose ist Gegenstand aktueller Forschung. Diskutiert werden u.a. Einflüsse von
Vitamin D, RANK/RANK-L und OPG, MGP, FGF-23 und Klotho, Fetuin A und Cathepsin K.
Während sich die vorliegende Studie durch die große Anzahl an Probanden beider
Geschlechter, einem breiten Altersbereich und einer umfassenden Berücksichtigung von
Confoundern auszeichnet, lasst das querschnittliche Studiendesign keine Ruckschlusse auf
Kausalität der beobachteten Assoziation zu. Weitere Limitationen stellen u.a. die Einnahme
von kardioprotektiver Medikation, die eingeschränkte Vergleichbarkeit mit früheren Studien
aufgrund unterschiedlicher Messmethoden und die Beschränkung der Analysen zum ABI auf
eine Subgruppe dar. Die beobachtete signifikante Assoziation zwischen sinkendem
QUS-basierten SI und dem Vorkommen von Plaques bei Männern weist darauf hin, dass
Männer, die klinische Zeichen einer Osteoporose aufweisen, ein erhöhtes Risiko für
atherosklerotische Erkrankungen, unabhängig von kardiovaskularen Risikofaktoren, wie Alter, BMI, Rauchen, Bewegungsmangel, Diabetes mellitus, Hypertonie und Dyslipidamie haben, sodass diese von einer frühzeitigen Evaluation des atherosklerotischen Risikos profitieren konnten. Insgesamt ist der Zusammenhang zwischen Osteoporose und Atherosklerose komplex und in nachfolgenden Studien wäre es von großem Interesse speziell das männliche Geschlecht in den Fokus zu rucken. Darüber hinaus ist die Betrachtung einzelner Signalstoffe von großem Interesse, um die gegenseitige Beeinflussung beider Erkrankungen tiefgreifender zu verstehen.
Enzalutamid ist seit 2012 ein zugelassenes Medikament für die Hormontherapie nach Docetaxel-Behandlung bei kastrationsresistenten Prostatakarzinom (CRPC). Als bekannter Wirkmechanismus gilt der Antagonismus am Androgenrezeptor (AR) und die Verhinderung der Translokation in den Nukleus, sowie die Inhibition der Transkription am AR.
In dieser Arbeit wurde der Wirkmechanismus von Enzalutamid auf zellulärer und molekularer Ebene präzisiert. In der Wachstumskinetik zeigt sich eine Wachstumshemmung sowohl der AR-positiven LNCaP Zellen, als auch der AR- negativen PC-3 Zellen. Zur Untersuchung des antiproliferativen Effektes von Enzalutamid betrachtet diese Arbeit AR-assoziierte Wachstumsfaktoren, den AR selbst und Östrogenrezeptoren. Als Einflussgrößen gelten die AR-assoziierten Proteine, wie Hitzeschockprotein (HSP) 27, HSP70, HSP90α/β und die Co-Chaperone HSP40 und HSP 70/90 organizing Protein (HOP). Hierbei zeigte vor allem HSP27 aber auch HSP90β eine deutliche Suppression der Protein-Expression in beiden Zelllinien. Die Untersuchung der Proteinexpression der Co-Chaperone, HSP40 in beiden Zelllinien und HOP in LNCaP Zellen, ergab ebenfalls eine deutliche Supression. Diese Proteine, insbesondere HSP27, haben aufgrund ihrer zellulären Funktion Einfluss auf die Proliferation der PC-Zellen, somit kann ein supprimierender Effekt unter Enzalutamid die Erklärung für das eingeschränkte Wachstum der LNCaP Zellen aber auch der hormonresistenten PC-3 Zellen sein. In LNCaP Zellen konnte der supprimierende Effekt von Enzalutamid auf die AR-Expression bestätigt werden. Als weiteren möglichen Proliferationsfaktor wurde in dieser Arbeit der Östrogenrezeptor (ER) mit seinen Isoformen ERα, ERβ1 und ERβ2 untersucht. Es zeigte sich dabei eine deutliche Suppression des ERβ1, welcher in einem möglichen Zusammenhang mit dem AR stehen kann. Die Suppression der ERβ1-Expression zeigte sich aber auch deutlich in AR-negativen PC-3 Zellen, was auch als antiproliferativer Effekt gewertet werden kann. Die Untersuchung der weiteren ER-Isoformen unter Einwirkung von Enzalutamid zeigten keine Effekte.
Das Resultat dieser Arbeit ist, dass Enzalutamid nicht nur ein Hormonrezeptorantagonist ist, sondern auch auf AR-unabhängige Signalkaskaden Einfluss hat. Enzalutamid zeigt im Zellmodell einen deutlich antiproliferativen Effekt, was mit seiner effektiven Wirkung bei CRPC korreliert und auch an den in dieser Arbeit betrachteten Faktoren liegen kann.
Background: Despite optimized medical therapy, severe idiopathic pulmonary arterial hypertension (IPAH) is a devastating disease with a poor outcome. Autoantibodies have been detected in IPAH that can contribute to worsening of the disease. Objectives: The objective of this prospective, open-label, single-arm, multicenter trial was to evaluate the safety and efficacy of immunoadsorption (IA) as an add-on to optimized medical treatment for patients with IPAH. Methods: A total of 10 IPAH patients received IA over 5 days. Their clinical parameters, including hemodynamics measured by right heart catheter, were assessed at baseline and after 3 and 6 months. The primary endpoint was the change in pulmonary vascular resistance (PVR). Secondary endpoints included the change in 6-min walking distance, quality of life, safety, and plasma levels of IgG and autoantibodies. Results: The evaluation of the 10 IPAH patients (75% female; 51 ± 12 years; 166 ± 10 cm; WHO functional class III; 53% on combination therapy) revealed that IA was a safe procedure that efficiently removed IgG and autoantibodies from the circulation. After 3 months, the mean PVR improved significantly by 13.2% (p = 0.03) and the cardiac index improved by 13.1%, but no significant changes were found in 6-min walking distance. The quality of life physical functioning subscale score significantly improved after 6 months. The serious adverse events in 3 patients were possibly related to IA and included pneumonia, temporary disturbance in attention, and thrombocytopenia. Conclusions: IA as an add-on to targeted medical treatment for IPAH is a safe procedure with beneficial effects on hemodynamics, especially in patients with high levels of autoantibodies. Larger-scale controlled studies are needed to assess its efficacy in IPAH and to identify responders.
The M protein of Streptococcus canis (SCM) is a virulence factor and serves as a surface-associated receptor with a particular affinity for mini-plasminogen, a cleavage product of the broad-spectrum serine protease plasmin. Here, we report that SCM has an additional high-affinity immunoglobulin G (IgG) binding activity. The ability of a particular S. canis isolate to bind to IgG significantly correlates with a scm-positive phenotype, suggesting a dominant role of SCM as an IgG receptor. Subsequent heterologous expression of SCM in non-IgG binding S. gordonii and Western Blot analysis with purified recombinant SCM proteins confirmed its IgG receptor function. As expected for a zoonotic agent, the SCM-IgG interaction is species-unspecific, with a particular affinity of SCM for IgGs derived from human, cats, dogs, horses, mice, and rabbits, but not from cows and goats. Similar to other streptococcal IgG-binding proteins, the interaction between SCM and IgG occurs via the conserved Fc domain and is, therefore, non-opsonic. Interestingly, the interaction between SCM and IgG-Fc on the bacterial surface specifically prevents opsonization by C1q, which might constitute another anti-phagocytic mechanism of SCM. Extensive binding analyses with a variety of different truncated SCM fragments defined a region of 52 amino acids located in the central part of the mature SCM protein which is important for IgG binding. This binding region is highly conserved among SCM proteins derived from different S. canis isolates but differs significantly from IgG-Fc receptors of S. pyogenes and S. dysgalactiae sub. equisimilis, respectively. In summary, we present an additional role of SCM in the pathogen-host interaction of S. canis. The detailed analysis of the SCM-IgG interaction should contribute to a better understanding of the complex roles of M proteins in streptococcal pathogenesis.
Whether mice are an appropriate model for S. aureus infection and vaccination studies is a matter of debate, because they are not considered as natural hosts of S. aureus. We previously identified a mouse-adapted S. aureus strain, which caused infections in laboratory mice. This raised the question whether laboratory mice are commonly colonized with S. aureus and whether this might impact on infection experiments. Publicly available health reports from commercial vendors revealed that S. aureus colonization is rather frequent, with rates as high as 21% among specific-pathogen-free mice. In animal facilities, S. aureus was readily transmitted from parents to offspring, which became persistently colonized. Among 99 murine S. aureus isolates from Charles River Laboratories half belonged to the lineage CC88 (54.5%), followed by CC15, CC5, CC188, and CC8. A comparison of human and murine S. aureus isolates revealed features of host adaptation. In detail, murine strains lacked hlb-converting phages and superantigen-encoding mobile genetic elements, and were frequently ampicillin-sensitive. Moreover, murine CC88 isolates coagulated mouse plasma faster than human CC88 isolates. Importantly, S. aureus colonization clearly primed the murine immune system, inducing a systemic IgG response specific for numerous S. aureus proteins, including several vaccine candidates. Phospholipase C emerged as a promising test antigen for monitoring S. aureus colonization in laboratory mice. In conclusion, laboratory mice are natural hosts of S. aureus and therefore, could provide better infection models than previously assumed. Pre-exposure to the bacteria is a possible confounder in S. aureus infection and vaccination studies and should be monitored.
Purpose
Due to the demographic change morbidity raises the demand for medical hospital services as well as a need for medical specialization, while economic and human resources are diminishing. Unlike other industries hospitals do not have sufficient data and adequate models to relate growing demands and increasing performance to growth in staff capacity and to increase in staff competences.
Method
Based on huge medical data sample covering the years from 2010 to 2014 with more than 150,000 operations of the Department for Anesthesiology at the University Hospital Muenster, Germany, comparisons are drawn between the development of medical services and the development of personnel capacity and expertise.
Results
The numbers of surgical operations increased by 21% and “skin incision to closure” time by 17%. Simultaneously, personnel capacity grew by 16% largely resting upon recruiting first-time employees. Expertise measured as “years of professional experience” dwindled from 10 years to 5.4 years on average and staff turnover accelerated.
Conclusion
Static benchmark data collected at fixed reference dates do not sufficiently reflect the nexus between capacity and competence and do not reflect the dynamic changes in a hospital’s requirements for expertise and specialization, at all. Staff turnover leads to a loss of experience, which jeopardizes patient safety and hampers medical specialization. In consequence of the dramatic shortage of medical specialists, drop-off rates must be reduced and retention rates must be increased. To that end, working conditions need to be fundamentally converted for a multigeneration, multicultural, and increasingly female workforce.
Introduction
Primary immunodeficiency disorders (PIDs) are a heterogeneous group of more than 200 rare diseases. Timely diagnosis is of uttermost importance. Therefore, we aimed to develop a diagnostic questionnaire with computerized pattern-recognition in order to support physicians to identify suspicious patient histories.
Materials and methods
Standardized interviews were conducted with guardians of children with PID. The questionnaire based on parental observations was developed using Colaizzis’ framework for content analysis. Answers from 64 PID patients and 62 controls were analyzed by data mining methods in order to make a diagnostic prediction. Performance was evaluated by k-fold stratified cross-validation.
Results
The diagnostic support tool achieved a diagnostic sensitivity of up to 98%. The analysis of 12 interviews revealed 26 main phenomena observed by parents in the pre-diagnostic period. The questions were systematically phrased and selected resulting in a 36-item questionnaire. This was answered by 126 patients with or without PID to evaluate prediction. Item analysis revealed significant questions.
Discussion
Our approach proved suitable for recognizing patterns and thus differentiates between observations of PID patients and control groups. These findings provide the basis for developing a tool supporting physicians to consider a PID with a questionnaire. These data support the notion that patient’s experience is a cornerstone in the diagnostic process.
For the normal development of pregnancy, a balance between immune tolerance and defense is crucial. However, the mechanisms mediating such a balance are not fully understood. CD83 is a transmembrane protein whose expression has been linked to anti-inflammatory functions of T and B cells. The soluble form of CD83, released by cleavage of the membrane-bound protein, has strong anti-inflammatory properties and was successfully tested in different mouse models. It is assumed that this molecule contributes to the establishment of immune tolerance. Therefore, we postulated that the expression of CD83 is crucial for immune tolerance during pregnancy in mice. Here, we demonstrated that the membrane-bound form of CD83 was upregulated in T and B cells during allogeneic murine pregnancies. An upregulation was also evident in the main splenic B cell subtypes: marginal zone, follicular zone, and transitional B cells. We also showed that there was an augmentation in the number of CD83+ cells toward the end of pregnancy within splenic B and CD4+ T cells, while CD83+ dendritic cells were reduced in spleen and inguinal lymph nodes of pregnant mice. Additionally, B lymphocytes in late-pregnancy presented a markedly higher sensitivity to LPS in terms of CD83 expression and sCD83 release. Progesterone induced a dosis-dependent upregulation of CD83 on T cells. Our data suggest that the regulation of CD83 expression represents a novel pathway of fetal tolerance and protection against inflammatory threats during pregnancy.

Mittels der vorliegenden Dissertation sollten, anhand eines größeren Patientinnenkollektivs, uterine Leiomyosarkome (LMS) umfassend charakterisiert werden um als Vergleichsmaterial zu regelhaften Leiomyomen (LM) für die Errechnung eines präoperativen LMS-Risiko-Scores zu dienen.
Zu diesem Zweck wurden insgesamt 240 LMS-Beratungsfälle des Deutschen klinischen Kompetenzzentrums für genitale Sarkome und Mischtumoren der Universitätsmedizin Greifswald (DKSM) aus dem Zeitraum August 2009 – September 2017 retro- und prospektiv erfasst und analysiert. Von diesen wurden 154 Fälle einer vorangegangenen Promotion (126) des DKSM entnommen. Letztere wurden mit erweiterten bzw. neu hinzugekommenen Kriterien aktualisiert. Um eine vergleichende Auswertung mit LM zu ermöglichen, war es erforderlich, die relevanten Kriterien in ein Schema zu integrieren, das simultan bei den LM-Analysen der anderen Arbeitsgruppen des DKSM zur Anwendung kam.
Das mediane Alter von 52 Jahren und die höchste Inzidenz zwischen 50 – 59 Jahren decken sich mit Angaben aus der Literatur. Der Anteil postmenopausaler Frauen lag mit 60,8 % über den ermittelten Werten anderer Autoren.
Die anamnestischen Angaben, wie eine Tamoxifenexposition, eine vorausgegangene medikamentöse Therapie mit Ulipristalacetat (UPA) und vorausgegangene Operationen wegen Varianten des LM und eines STUMP (glattmuskulärer Tumor mit unsicherem malignem Potential) spiegeln die Literaturangaben nur bedingt wider. Ein Therapieversagen von UPA bei symptomatischen LM scheint ein Hinweis auf ein uterines Sarkom zu sein. Allerdings wurden in dieser Untersuchung insgesamt nur 8 entsprechende Fälle gefunden. Eine vorausgegangene Operation wegen Varianten des LM bzw. eines STUMP fand sich nur in 2,1 % der Fälle.
Betreffs der Blutungsstörungen wurden bei 44 % der prämenopausalen Frauen intermenstruelle Zusatzblutungen und bei 45,2 % Blutungen in der Postmenopause beschrieben. Zudem wurden tumorbezogene Symptome ohne jegliche Blutungsstörungen, wie uterine oder Unterbauchschmerzen, ein vermehrter Harndrang sowie ein Druckgefühl oder eine Zunahme des Abdominalumfanges in 46,3 % der Fälle beobachtet.
In mehr als der Hälfte der Fälle (53,4 %) lag ein schnelles Tumorwachstum vor. Der Wert lag in der Postmenopause sogar bei 55,5 %. In 45,5 % wurde das LMS als weich bzw. auffällig weich beschrieben.
In 99,6 % aller Fälle wurde eine diagnostische Sonografie durchgeführt. Dabei zeigte sich in 82 % ein als auffällig bezeichneter Befund.
Der Durchmesser der beobachteten LMS betrug median 9 cm. Am häufigsten wiesen die LMS eine Größe zwischen 6 – 10 cm auf. Der Anteil der solitären LMS (keine zusätzlichen LM) lag bei 51,7 %. Der mittlere Durchmesser der solitären LMS betrug 10,3 cm.
Bei 73 der Fälle (30 %) wurde eine diagnostische Abrasio oder eine direkte Biopsie durchgeführt. Unter diesen wurde nur in 38,4 % primär die richtige Diagnose gestellt. Die Literaturangaben zur Stadienverteilung der LMS decken sich nicht mit eigenen Ergebnissen, nach denen sich 79,6 % der Fälle im Stadium I und nur 0,4 % im Stadium IV befanden. Ebenfalls widersprechen die eigenen Daten den Angaben bezüglich einer Lymphknotenbeteiligung. In insgesamt 80,4 % (n = 189 Fälle) wurde keine Lymphonodektomie (LNE) durchgeführt. Insgesamt 10,9 % der LMS mit einer LNE (n = 46 Fälle) wiesen positive Lymphknoten auf.
Die Literaturangaben zur Netz- und Zervixbeteiligung konnten bestätigt werden. Von 235 operierten Frauen fand nur in 21 Fällen (13,2 %) eine Netzresektion statt. In 4 dieser Fälle war das Netz vom Tumor befallen. In 15,1 % der Fälle der vorliegenden Arbeit wurde ein Zervixbefall im Sinne einer Infiltration durch einen korporalen Tumor bzw. einer primären Lokalisation in der Zervix gefunden.
In insgesamt 67,7 % aller Fälle wurde das LMS primär nicht erkannt und konsekutiv unter der falschen Indikation operiert. In 66,0 % bildete ein LM die Operationsindikation bei der Enddiagnose LMS. Dementsprechend wurden nur 35,7 % der LMS mittels totaler Hysterektomie ohne Uterus- und/oder Tumorverletzung operiert.
In 71,3 % der Fälle lagen Daten zur Mitosenzahl je 10 HPF vor. Der errechnete Mittelwert aller LMS betrug 20,7 M/10 HPF und speziell die der spindelzelligen LMS 21,7 M/10 HPF. Diese Zahlen decken sich mit den Angaben aus der Literatur. Das gilt gleichermaßen für die Expression des Ki67-Antigens (im Mittel 37,2 %). Die 5-Jahres- Überlebenswahrscheinlichkeit (Nomogramm Overall Survival Probability nach Zivanovic) lag bei 144 berechenbaren Fällen median bei 48 %. In den vorliegenden Fällen konnten in 8,3 % primäre Fernmetastasen nachgewiesen werden. Diese fanden sich mit 65,1 bzw. 25,3 % am häufigsten in der Lunge und/oder der Leber. Eine Ausbreitung in die Knochen sowie in die Knochen plus simultan in jegliche anderweitigen Strukturen lag nur in jeweils 6,0 % vor.
Die gewonnenen Ergebnisse wurden in der Zentrale des DKSM zur Berechnung des LMS-Risiko-Scores mittels Vergleich mit 670 prospektiv analysierten LMS aus 2 weiteren Promotionen verwendet. Der Score ist zwischenzeitlich publiziert worden.
Assoziation zwischen Thyreotropin und dem Lipidprofil bei Kindern und Jugendlichen in Deutschland

Zielstellung:
Es gibt noch keine populationsbezogene Studie, die den Zusammenhang zwischen Thyreotropin (TSH) und den Lipidparametern Gesamtcholesterin, Low-Density-Lipoprotein-(LDL)-Cholesterin, High-Density-Lipoprotein-(HDL)-Cholesterin und Triglyzeriden bei Kindern und Jugendlichen unter-sucht hat, trotz der möglichen Auswirkungen eines erhöhten Lipidprofils auf die Entstehung von kardiovaskulären Erkrankungen.
Teilnehmer und Methoden:
Es wurden Eigenschaften und Laborparameter von insgesamt 6 622 drei- bis zehnjährigen Kindern und 6 134 elf- bis 17-jährigen Heranwachsenden der Studie zur Gesundheit von Kindern und Jugendlichen in Deutschland (KiGGS) analysiert. Primäre Expositionsvariable dieser Querschnittsstudie war der mit einer Elektroluminiszenz-Methode gemessene TSH-Spiegel im Serum. Primäre Outcomevariablen waren die Gesamtcholesterin-, LDL-Cholesterin-, HDL-Cholesterin- und Triglyzeridspiegel im Serum, die über enzymatische Farbanalysen bestimmt wurden. Die Variablen wurden mithilfe von altersspezifischen Grenzwerten kategorisiert.
Ergebnisse:
Signifikante positive Assoziationen lagen bei Kindern zwischen Serum-TSH-Spiegeln und allen Nicht-HDL-Cholesterinspiegeln (Gesamtcholesterin-, LDL-Cholesterin- und Triglyzeridspiegeln) vor (β = 0,90, 95 %-Konfidenzintervall [KI]: 0,53 – 1,27; β = 0,78, 95 %-KI: 0,44 – 1,13; und β = 0,90, 95 %-KI: 0,52 – 1,27), gleichermaßen bei Jugendlichen (β = 0,90, 95 %-KI: 0,47 – 1,32; β = 0,67, 95 %-KI: 0,29 – 1,05; und β = 0,92, 95 %-KI: 0,49 – 1,35; p < 0,05). Stratifizierte Modelle deuteten darauf hin, dass diese Zusammenhänge besonders stark bei übergewichtigen und adipösen Kindern waren. Kinder mit erhöhten TSH-Spiegeln hatten im Vergleich zu Kindern mit TSH-Spiegeln im Referenzbereich höhere Nicht-HDL-Cholesterinspiegel.
Fazit:
Wie schon bei Erwachsenen bekannt, zeigt diese Studie bei Kindern und Jugendlichen einen positiven Zusammenhang zwischen den Serumspiegeln von TSH und Nicht-HDL-Lipiden. Längsschnittstudien könnten klären, ob ein erhöhter Serum-TSH-Spiegel, der von einem unvorteilhaften Lipidprofil begleitet wird, im Erwachsenenalter fortdauert.
Untersuchungen zum Mechanismus der oralen Absorption von Trospiumchlorid an gesunden Probanden

In Deutschland leiden ca. 15 % der über 40-jährigen am Syndrom der überaktiven Blase (overactive bladder, OAB), welches durch plötzlich auftretenden, nicht aufhaltbaren Harndrang definiert wird. Trospiumchlorid (TC) ist ein kationischer, wasserlöslicher, antimuskarinerger Arzneistoff mit einer stark variablen Bioverfügbarkeit von ca. 10 %, der häufig zur Behandlung der OAB eingesetzt wird. Aufgrund seiner quartären Ammoniumstruktur überwindet er die Hirnschranke nicht und löst somit keine kognitiven Nebenwirkungen aus, was einen entscheidenden Vorteil gegenüber anderen Anticholinergika darstellt. Zielsetzung dieser Arbeit war die Optimierung seines schlechten oralen Absorptionsverhaltens.
TC überwindet die Enterozytenmembran als Substrat des Efflux-Carriers P-glycoprotein (P-gp) und des Aufnahmetransporters organic cation transporter 1 (OCT1). Durch die geringe Expression von P-gp in den proximalen Dünndarmabschnitten und einer gleichmäßigen Expression von OCT1 im gesamten Darm vermuteten wir ein „Absorptionsfenster“ für TC in diesem proximalen Dünndarmareal.
In zwei nach ähnlichem Design durchgeführten kontrollierten, randomisierten, cross-over Studien der Phase I versuchten wir dieses vermutete „Absorptionsfenster“ mit der Simulation gastroretentiver Darreichungsformen für TC gezielt zu bedienen. Das TC sollte dabei mit Hilfe der Antrum-Motilität über einen längeren Zeitraum, in portionierten Mengen aus dem Magen in den Dünndarm befördert werden. In der offenen, vier-armigen GI-Studie (gastric infusion) benutzten wir dafür eine Magensonde über die 30 mg in Wasser gelöstes TC in einem Zeitraum von 6 h in den Magen infundiert wurden (GI). Im Vergleich dazu stand die orale Einnahme einer 30 mg schnell freisetzenden TC-Filmtablette (immediate release, IR). Die Applikationen erfolgten jeweils im nüchternen Zustand (fasted) und nach dem Verzehr einer standardisierten fettreichen Mahlzeit (FDA; fed).
In der NaHCO3-Studie (Natriumbikarbonat) simulierten wir unter Ausnutzung der verzögerten Magenentleerung nach Verzehr einer fettreichen Mahlzeit eine physiologische Form der gastroretentiven Darreichung von TC. Durch Zugabe einer NaHCO3-Kapsel zu TC (IR-TC + NaHCO3) als Brausesubstanz in Kontakt mit Magensäure, sollte TC gleichmäßig mit dem Mageninhalt vermischt werden, um interindividuelle Unterschiede in der TC-Bioverfügbarkeit zu verringern. Im Vergleich standen die intravenöse Gabe von TC (IV-TC) und die Komedikation eines NaHCO3-Placebos (IR-TC + NaHCO3-Placebo).
Die geplanten Ansätze zur Verbesserung des Absorptionsverhaltens von TC gelangen in beiden Studien leider nicht.
Mit Hilfe von pharmakokinetischem modelling der aus der GI-Studie gewonnenen Daten, postulierten wir mögliche Gründe. So fanden wir heraus, dass es im menschlichen Darm zwei „Absorptionsfenster“ für TC geben muss. Ein schmaleres mit geringerer Permeabilität im Jejunum und ein breiteres mit höherer Permeabilität im Caecum/Colon ascendens. Ursächlich hierfür könnten die lokale Häufigkeit und das Wechselspiel der in diesen Arealen vorkommenden Transportproteine P-gp und OCT1 sein. Der Versuch durch Gastroretention ein proximales „Absorptionsfenster“ zu bedienen erwies sich daher nach pharmakokinetischer Modellanalyse als Fehlkonzept. In zukünftigen Studien sollten weitere Darreichungsformen erprobt werden, die insbesondere auf eine Freisetzung des TCs im zweiten „Absorptionsfenster“ mit einer höheren Permeabilität für TC abzielen.
In vorangegangenen Studien wurde ein Zusammenhang der Sexualhormone mit dem Frakturrisiko nahegelegt. Wir haben darauf aufbauend untersucht in welchem Zusammenhang die Sexualhormone mit den quantitativen Ultraschallparametern der Ferse in Frauen und Männern Vorpommerns stehen. Für die vorliegende Studie wurden Testosteron, Estron und Androstendion mittels LC/MS-Verfahren und darüber hinaus Dehydroepiandrosteron-Sulphat und das Sexualhormon-bindende-Globulin in Immunoassays gemessen. Unter Einbezug relevanter Confounder wurden die Assoziation der Sexualhormone zu den quantitativen Ultraschallparametern der Ferse geprüft.
Interessanterweise zeigte sich neben der positiven Assoziation von freiem Testosteron zur frequenzabhängigen Ultraschalldämpfung, eine inverse Assoziation von Dehydroepiandrosteron-Sulphat zum Stiffness Index des Knochens, die sich weder mit Blick auf das aktuelle biochemische Verständnis noch auf die Studienlage eindeutig erklären lässt.
CAP-Behandlungen bieten neue Möglichkeiten in der Krebstherapie. Die Wirkungen von CAP sind bei verschiedenen Krebsentitäten nachgewiesen worden und zeigen sich hauptsächlich als Hemmung der Proliferation. In dieser Arbeit konnte gezeigt werden, dass CAP antiproliferativ auf OC-Zellen wirkt. Dieser Antitumoreffekt betraf alle vier untersuchten OC-Zelllinien und war abhängig von der Behandlungsdauer.
Weiterhin wurde nachgewiesen, dass der antiproliferative Effekt von CAP auch indirekt über CAP-behandeltes Medium übertragen werden konnte. Beim Medium RPMI/1640 zeigte sich der signifikante antiproliferative Effekt ebenfalls abhängig von der Behandlungsdauer des Mediums.
Ferner wurde der Einfluss von CAP auf die Zellbeweglichkeit untersucht. Wiederum konnte eine Hemmung der Zellmotilität in allen vier OC-Zelllinien nachgewiesen werden, obgleich sich hierbei leichte Unterschiede zwischen den Linien andeuteten.
Als weiterer molekularer Mechanismus wurde die Modulation von intra- und extrazellulären Konzentrationen des zytoprotektiven Faktors HSP27 ausgemacht. Dieses Protein vermittelt pro-onkogene Effekte und spielt eine wichtige Rolle bei der Therapieresistenz. Wie gezeigt werden konnte, sank nach CAP-Behandlung die intrazelluläre Konzentration von HSP27 während die extrazelluläre Konzentration anstieg. Die zellulären Funktionen dieser HSP27-Freisetzung sind jedoch bisher noch unklar und sicherlich sehr interessant für zukünftige Projekte.
Insgesamt kann festgestellt werden, dass CAP geeignet ist, um OC-Zellen zu inaktivieren. Die Behandlung führt zu antiproliferativen und antimetastatischen Effekten sowie zu einem intrazellulären Absinken von proonkogenem HSP27. Vorbehaltlich weiterer Untersuchungen, insbesondere auch zu der Funktion von sekretiertem HSP27, und vorbehaltlich der Evaluierung der CAP-Behandlung im klinische Kontext ist ein Einsatz von CAP in der gynäkologischen Onkologie sehr vielversprechend. Hierbei wäre sowohl eine direkte CAP-Behandlung von Tumorgewebe als auch eine indirekte Behandlung durch CAP-aktiviertes Medium denkbar.
Eisen als wichtigstes Spurenelement im menschlichen Körper bedingt, dass für nahezu alle Erkrankungen, insbesondere chronische und akute Entzündungsreaktionen eine fein regulierte Eisenhomöostase von besonderer Bedeutung ist. Ziel dieser Arbeit war es den Einfluss von zwei neuen Eisenchelatoren, DIBI und MAHMP auf die intestinale Mikrozirkulation bei experimenteller Sepsis zu evaluieren.
Im Rahmen einer Sepsis können Eisenchelatoren eine mögliche neue Therapieoption darstellen, indem sie zum einen in verschiedenen Kompartimenten freies Eisen binden können, welches folglich nicht mehr zur Generierung reaktiver Sauerstoffspezies zur Verfügung steht, und zum anderen die Bioverfügbarkeit freien Eisens für Stoffwechselprozesse und Zellteilung pathogener Keime reduzieren.
Hierzu führten wir intravitalmikroskopische Untersuchungen des Darmes an zwei murinen Modellen der experimentellen Sepsis durch.
Wir konnten zeigen, dass bei Endotoxinämie eine Behandlung mit MAHMP die Leukozytenaktivierung reduziert, die intestinale Kapillarperfusion verbessert und vor einen histologischen Mukosaschaden schützt und somit anti-inflammatorisch wirkt.
Im CASP-Modell führte die Behandlung einer polybakteriellen Peritonitis mit DIBI zur signifikanten Reduktion der Leukozytenaktivierung. Dies begründen wir hauptsächlich durch eine antibakterielle Wirkung von DIBI, welche durch extrazelluläre Eisenrestriktion quantitativ das Bakterienwachstum reduziert, und folglich über eine Reduktion Toxin-vermittelter, intrazellulärer Signaltransduktionskaskaden sekundär antiinflammatorisch wirkt.
Gegenwärtig gilt eine präoperative Differenzierung zwischen Leiomyomen (LM), den LM-Varianten/STUMP und den uterinen Sarkomen, insbesondere den Leiomyosarkomen (LMS) und low-grade endometriale Stromasarkomen als schwierig. Konsekutiv wird ein hoher Anteil von uterinen Sarkomen unter der Indikation von LM inadäquat operiert. Die Zielsetzung dieser Studie bestand darin anhand eines größeren Patientinnenkollektivs histologisch gesicherte regelhafte LM anamnestisch, klinisch und paraklinisch zu charakterisieren. Des Weiteren wurden die in diesem Rahmen gefundenen LM-Varianten/STUMP und uterine Sarkome gesondert analysiert. Die Ergebnisse dieser Arbeit wurden mit den unter weitgehend identischen Erhebungs- bzw. Analyseverfahren erhobenen LM-Daten von Krichbaum aus Münster und den durch weitere Promovenden der Promotions- und Forschungsgruppe für genitale Sarkome (PFGS) rekrutierten Sarkomdaten verglichen. Diese Daten sollten als Basis für die Errechnung eines durch das DKSM (Deutsche klinisches Kompetenzzentrum für genitale Sarkome und Mischtumoren) entwickelten Sarkom-Scores dienen. Zusätzlich erfolgte eine Analyse verschiedener Laborwerte (LDH, Neutrophilen/Lymphozyten-Ratio: NLR, Lymphozyten/Monozyten-Ratio: LMR) und der Ultraschallbefunde einschließlich der Dopplersonographien. Insgesamt sollten Charakteristika gefunden werden, die zur Diskriminierung von LM und uterinen Sarkomen geeignet sind. Es wurden insgesamt 526 vollständige Datensätze von LM-Operationen, die im Zeitraum von Dezember 2015 bis Oktober 2016 in der Frauenklinik des Krankenhauses Sachsenhausen/Frankfurt durchgeführt worden sind, prospektiv erfasst. Es wurden 470 Fälle mit histologisch gesicherten, regulären LM, sowie 36 zellreiche LM, 4 mitotisch aktive LM, 5 LM mit bizarren Kernen und 1 Lipoleiomyom, 1 Angioleiomyom und 1 myxoides LM in dieser Arbeit ausgewertet. Zusätzlich wurden 8 uterine Sarkome (3 LMS, 3 low grade endometriale Stromasarkome, 1 undifferenziertes uterines Sarkom und 1 Adenosarkom) analysiert. Von den 526 Patientinnen wurden 521 (470 LM, 46 LM-Varianten und 5 uterine Sarkome) unter der Diagnose LM aufgenommen und operiert. Bei den Parametern Alter, Tumorgröße, Menopausestatus, intermenstruelle Blutung, auffällige Sonographie, schnelles Tumorwachstum und Solitärtumor zeigte sich ein hochsignifikanter Unterschied (p < 0,001) zwischen den LM-Pat. der vorliegenden Promotion und von Krichbaum auf der einen und den 236 LMS der Promovenden der PFGS auf der anderen Seite. Es ergaben sich Hinweise, dass LMS eine höhere LDH und NLR sowie eine niedrigere LMR aufweisen. Erhöhte LDH-Werte sowie eine erhöhte NLR und eine erniedrigte LMR kamen auch bei den LM vor. So wiesen die Patientinnen mit einer Tumorgröße > 5 cm, aber auch mit stark degenerativ veränderten LM signifikant höhere LDH-Werte, als Frauen mit einer Tumorgröße ≤ 5 cm (p = 0,04334) und LM ohne degenerative Veränderungen (p= 0,0052) auf. Die NLR von Frauen ≥ 35 Jahre war signifikant höher, als bei denen mit < 35 Jahren (p = 1,4 x 10-5) und die LMR war bei ≥ 35 Jahren signifikant niedriger, als bei den Frauen < 35 Jahren (p = 0,0007). Bei der Thrombozytenzahl ergaben sich gleichfalls Hinweise, dass LMS (n = 3; Mittelwert: 398 Tsd/μl) und LM (n = 469; Mittelwert: 279,42 Tsd/μl) unterschiedlich hohe Werte aufweisen. Aufgrund der geringen Fallzahl der Sarkome ließen sich die Ergebnisse statistisch nicht sichern. Die Sonographie ist die wichtigste diagnostische Untersuchung zur Unterscheidung zwischen LM und LMS. 9,79 % der LM und 82,63 % der LMS des DKSM wiesen einen auffälligen Sonographiebefund auf. Dopplersonographisch ließen sich bei den uterinen Sarkomen bzw. LMS wegen der niedrigen Zahl der uterinen Sarkome lediglich Trends erkennen. Die uterinen Sarkome zeigten eine eher zentrale Durchblutung und ein LMS (1 von 2 Pat.) hatte eine stärker ausgeprägte Durchblutung, während die LM (n = 290) eher peripher (81,03 %) und weniger stark durchblutet waren. Nur 5,17 % der LM zeigten eine auffallend starke Durchblutung. Im Rahmen dieser Studie wurde der RI zentral und peripher im Tumor und in der A. uterina gemessen. Der zentrale RI der uterinen Sarkome (4 Messwerte) lag mit einem Mittelwert von 0,43 unter dem der LM (162 Messwerte) mit 0,58. Bei den LMS (2 Messwerte) wurden mit einem mittleren RI zentral von 0,35 sehr niedrige Werte gemessen. Zum Teil wiesen auch LM einen niedrigen RI auf. Der RI zentral (p = 0,0002) und der RI peripher (p= 1,59 x 10-7) waren bei LM mit einer Tumorgröße > 5 cm hochsignifikant niedriger, als bei einer Tumorgröße ≤ 5 cm. Dies traf bei dem RI der A. uterina nicht zu (p= 0,3805). Auch der RI zentral (p = 0,0411) und der RI peripher (p = 1,74 x 10-5) der solitären LM war signifikant niedriger, als der RI, der im größten Tumor bei multiplen LM gemessen wurde. Die Daten dieser Analyse mit 470 LM und die Daten der Promotion Krichbaum mit 199 LM wurden direkt für die Berechnung des präoperativen LMS-Risiko-Scores verwendet. Letztere erfolgte unter Hinzufügung der LMS-Daten des DKSM im Institut für Bioinformatik der Universitätsmedizin Greifswald. Dieser Score wurde zwischenzeitlich zur Publikation eingereicht.
The aims of this study were to quantify the key mineralization processes and the resulting nutrient release potentials of different sediment types, their ranges of extent and dependency on varying environmental conditions such as seasonal variations or shifts in oxygen availability. Benthic phosphate fluxes and flux potentials were of particular interest as P is an essential nutrient for algal growth in marine systems and phosphate is often limiting primary production, hence strongly promotes the production of new biomass. A major P source in marine environments are mineralization products of early diagenetic processes.
To gain insight into the pathways of organic matter mineralization and subsequent secondary reactions, key reactants in the solid and the dissolved phase were considered in typical sediments of the Baltic Sea and the Black Sea. Seven study sites in the German Baltic Sea region, representative of the major depositional environments, including coarse and fine grained sediments, both rich or poor in organic carbon, were intensively studied. The investigations were conducted on a seasonal basis during ten ship-based expeditions between July 2013 and March 2016, covering spring- and autumn algal blooms, stagnation periods with bottom water hypoxia and winter dormancy. Hypoxic conditions frequently developed in the bottom waters of the Bay of Mecklenburg, Stoltera and the Arkona Basin sites, shallower stations like the Tromper Wiek, the Oder Bank and the Darss Sill were usually not affected by hypoxia. Increased nutrient concentrations in the bottom waters coincided with oxygen depletion. High salinity dynamics were observed in the bottom waters above the studied sediments, which were due to frequent salt water inflows from the North Sea. Bottom water temperature variability was seasonally conditioned.
The studied sands showed 2-3 orders of magnitude higher permeability values and about one order of magnitude lower organic matter contents compared to the studied muds. Occasionally, strongly increased organic matter contents were observed in the sands, likely induced by downward mixing of plankton bloom derived particles. The organic matter was found to be essentially supplied by marine phytoplankton, indicated by its elemental composition and isotopic signature.
Additionally, an adapted approach of the Keeling plot method was made to characterize the source material of organic matter mineralization. In marine environments, dissolved inorganic carbon concentrations often increase with depth together with an isotopic carbon signature shift to lighter values due to organic matter mineralization. The Keeling plot method was commonly used to determine the isotopic signature of carbon sources for ecosystem respiration. Conventionally, the influence of respiratory depletion of 13CO2 on the isotopic composition of the atmosphere was studied in terrestrial and limnic biogeochemistry. The same approach was applied on organic matter mineralization in sediments and the water column during this study. Mixing of bottom water derived background DIC and DIC released into the environment during organic carbon decomposition was assumed. In a modification of previous approaches, where changes in concentration and δ13C of DIC were followed over time, vertical profiles were analyzed in this study, which represent time-dependent variations superimposed by transport processes . DI13C gradients in the water column of the Black Sea and pore water profiles in the Black Sea and the Baltic Sea were used to estimate the 13C signature of the mineralized organic carbon via Keeling plot analysis. The Black Sea water column revealed a δ13C signature of the organic matter source close to the signature of typical particulate organic matter in the ocean and previously reported values for the Black Sea euphotic zone. In the pore waters of Black Sea sediments (from short and long sediment cores), Keeling plot analysis clearly demonstrates that the released DIC at depth can be derived from different sources. An isotopically very light carbon source (< –60 ‰) was associated with anaerobic oxidation of methane in the Black Sea. Marine organic matter was the principal source for DIC in the deeps of Baltic Sea basins, while the calculated carbon source isotopic signature in sediments of sand flats and bays was shifted to heavier δ13C signatures compared to marine organic matter. These shifts were attributed to potential dissolution of sedimentary carbonates or organic inputs of terrestrial C4 vascular plants like maize and other agricultural plants. The carbon source isotopic composition calculated via Keeling plot analysis correlated well with directly measured δ13C signatures of surface sediments POM.
Organic matter mineralization activity in the southern Baltic Sea sediments was studied via gross sulfate reduction rate analysis and total oxygen uptake measurements in sands and muds. Oxygen penetration depths were less than 4 mm in both, muddy and sandy sediments. Oxygen uptake rates were similar in muds (10.2 mmol m–2 d–1) and sands (10.7 mmol m–2 d–1), while significantly higher rates were measured in the coastal near sites of the Bay of Mecklenburg (about 12 mmol m–2 d–1) than in the deeper Arkona Basin (about 9 mmol m–2 d–1). Substantial sulfate reduction was measured in the muddy (about 4 mmol m–2 d–1) and the sandy (about 1 mmol m–2 d–1) study sites. Highest sulfate reduction rates (4.4 – 5 mmol m–2 d–1) were detected in the muds of the Bay of Mecklenburg during summer, about twice as high as in the Arkona Basin muds. Increased mineralization activity of the coastal near muds of the Bay of Mecklenburg is attributed to enhanced input of fresh organic matter during algal blooms.
About twofold higher oxygen consumption and sulfate reduction rates were measured in summer compared to winter in sandy and muddy sediments.
The studied sites were usually characterized by a typical biogeochemical zonation with oxic, suboxic and sulfidic zones. The concentration profiles in the muds reflected sulfate reduction and secondary redox-reactions, liberating dissolved carbon, nitrogen, phosphorus and hydrogen sulfide into the interstitial waters. Orders of magnitude lower concentrations were detected in the sands, while their top centimeters were mostly irrigated and mineralization products only accumulated below.
A several centimeter thick suboxic zone was sustained by active downward transport of oxidized material in the southern Baltic Sea coastal sediments, presumably mainly through bioturbation. Especially sulfur and iron species were involved in the secondary reactions between the metabolites of early diagenetic processes occurring in the suboxic zone. Partly high temporal variability was observed in the form of vertical migration of the sulfidic zone and a corresponding expansion and shrinking of the suboxic zone. The Arkona Basin site showed the most stable geochemical zonation over time, while the Luebeck Bight site and especially the Mecklenburg Bight site showed striking dynamics. The consequent redox-regime shifts within the surface sediments might promote mineralization of organic matter as higher sulfate reduction rates and higher total oxygen uptake were measured in these more dynamic muds. The vertical shifts of the redox-gradients can largely be explained by temporal and spatial variability of bioturbation activity, but also anthropogenic activities may play a role. Bottom trawling, may be the dominant mixing process at the Mecklenburg Bight site.
In the sands, less reactive iron and manganese contents were available compared to the studied muds, which may be due to frequent irrigation of the top centimeters leading to a loss of pore water reservoirs like dissolved Fe and Mn.
Another mixing process, storm induced sediment resuspension, was suggested to be important for the sediments in the study area. A severe sediment resuspension event at the silty Tromper Wiek site was indicated by steep gradients in pore water concentration and particulate mater content profiles in the top ∼ 5 cm sediments. Carrying out non steady state modeling (Bo Liu, IOW), the successive re-development of the pore water profiles towards a steady-state was simulated and the time span necessary to establish measured pore water profiles after the sediment disturbance event was approximated. In the predicted time span, daily average wind speeds reached an annual high of the category ‘gale’ which was probably sufficient to resuspend and irrigate the surface silt-type sediments.
This study showed that sediment mixing processes have a strong influence on early diagenetic reactions, solute release potentials and actual fluxes from the sediments. Also the sediment mixing via bottom trawl fishing can be regarded as an event-like disturbance. The newly developed approach of non-steady-state modeling of pore water sets (Bo Liu; IOW) can help to answer the question, how long it takes to reach a steady-state after a sediment disturbance event.
The concentration profiles in interstitial waters were used to calculate net transformation rates via the transport-reaction models REC and PROFILE. The calculations revealed net release of H4SiO4 and PO4 within the top 20 cm of the studied muds, while net DIC and NH+4 consumption and sulfate production was clearly evident within the top ∼ 5 cm. Intensive reoxidation of sulfide that was produced via sulfate reduction was also indicated by large deviations between the modeled net sulfate transformation rates and experimentally derived gross sulfate reduction rates. These surface near transformations were probably associated with microbial chemosynthesis.
The studied sands of the southern Baltic Sea were usually characterized by very low pore water gradients, probably due to their frequent irrigation. Accordingly, calculated transformation rates were one to two orders of magnitude lower in the studied sands compared to the muds of the southern Baltic Sea. However, during a situation with a stratified water column, substantial pore water reservoirs with the typical concentration trends were present in the sandy Darss Sill sediments. Integrated production rates of PO4, Mn2+ and H4SiO4 derived from these concentration profiles were in the same order of magnitude as in the muddy Arkona Basin.
Intensive pore water irrigation is also capable to transport fresh organic matter into the sediment as was occasionally indicated by strongly increased surface near TOC in the sands. Considerable gross sulfate reduction rates and total oxygen uptake rates were measured in the sandy Oder Bank sediments, where pore water concentration profiles rather suggested absence of diagenetic processes. The studied sands were, hence, not unreactive substrates but usually rather unable to preserve the mineralization products.
This reflects the often limited significance of pore water evaluations in irrigated sandy sites but also the high mineralization potentials of coastal sands. Early diagenetic processes and the impact of intense benthic-pelagic exchange in such shallow marine environments is still poorly understood, as they were rarely investigated in the past and are methodologically more difficult to investigate.
The Baltic Sea deeps Gotland Deep and Landsort Deep were mainly controlled by sulfate reduction and shallow anaerobic oxidation of methane. Calculated rates of net sulfate reduction and net sulfide production were equivalent, indicating a lack of sulfide re-oxidation reactions. Phosphate liberation rates were low in the surface sediments, but strong linear concentration gradients indicated liberation at depth.
The euxinic Black Sea sediments were purely controlled by sulfate reduction via anaerobic oxidation of methane at the sulfate-methane transition zone in sediment depths of up to several meters. Subsequent diffusive concentration gradients clearly dominated pore water profiles in the surface sediments.
Benthic solute reservoirs of the top 10 cm pore waters were generally higher in the muds than in the sands of the southern Baltic Sea. The smaller reservoirs in the sands were cause by intensive exchange between pore water and bottom water in these permeable sediments. The three studied muddy sites of the southern Baltic Sea showed great dissimilarities with respect to their pore water compositions. Large observed pools of dissolved Fe2+ and PO4 clearly point to reduction of reactive Fe and release of adsorbed P pools. Multi-dimensional scaling analysis showed that seasonal variability played only a minor role for the observed variability of the benthic solute reservoirs. Principal component analysis revealed that the studied sediments can be characterized by essentially two factors based on their pore water reservoirs of the top 10 cm: 1) their mineralization and accumulation efficiency and 2) their secondary reactions in the suboxic zone, reflecting fundamental differences in their sedimentation conditions and mixing processes. While the sands were similar to each other due to their overall low reservoirs, sands, silts and muds mainly differed in their mineralization and accumulation efficiency. Large variability was also observed within the studied muds regarding their predominating redox metabolites. Highest dissimilarities were evident between the neighboring sites Mecklenburg Bight (mostly suboxic) and Luebeck Bight (mostly sulfidic). Therefore, the biogeochemical state in the studied sediments were shown to be mainly controlled by their sediment type and the transport of reactive iron into the sediments. The supply of organic matter to the sea floor controlled the overall mineralization activity, while the sediment permeability determined the accumulation efficiency of the sediment. Mixing of surface sediments together with the complex relation of oxygen, sulfur, iron and phosphorus in the solid and aqueous phase is controlling benthic nutrient fixation/liberation reactions. Salinity variability showed no noticeable effects on early diagenetic processes. During four sampling occasions in the Arkona Basin, bottom water salinity showed strong variability which was also effecting pore water concentration profiles in more than 15 cm depth. However, concentration profiles of typical organic matter mineralization products remained remarkably stable. Also gross sulfate reduction rates seemed to be unaffected by the variable pore water sulfate concentrations.
Nutrient fluxes across the sediment-water interface were obtained by the interpretation of vertical pore water concentration profiles via different models (diffusive fluxes) and via incubation of intact sediment cores (total fluxes). Benthic diffusive fluxes represent potential release of solutes into the water column. They were often strongly associated with the benthic reservoirs, thus fluxes were highest in the muds, considerably lower in the organic-poorer silts, and generally close to zero in the sands. The dissimilarities between diffusive fluxes in the different sediments were mainly controlled by the sediment type. Highest variability was observed within the muds, controlled by their different diagenetic pathways due to different sediment mixing intensities.
In the muddy sediments, diffusive PO4 fluxes were much higher than mineralization of organic matter with the common element ratios of marine organic matter can provide, indicating active recycling of phosphorus within the muds due to recurring adsorption and re-release on reactive iron oxyhydroxide phases. Especially in the strongly mixed Mecklenburg Bight sediments, pore water dissolved PO4 was primarily controlled by the release of adsorbed P. Actual PO4 release into the bottom waters (determined via core incubation experiments) was only measured under extended bottom water oxygen deficiency conditions. The same applied to the redox-sensitive solutes Fe2+, Mn2+ and sulfide.
For the less redox-sensitive solutes, diffusion usually accounted only for a fraction of the total interfacial flux. The proportion of advective to diffusive transport was estimated with different methods. Bioturbation induced sediment mixing was quantified by comparing the diffusive H4SiO4 fluxes derived from pore water modeling with the total fluxes derived from the core incubations. The studied muds showed infauna induced advection proportions of about 35 – 100 %. Only when infauna was absent, diffusion became the dominant transport process across the sediment-water interface. In the studied sands, advection was the dominating transport process, since their generally low surface near reservoirs lead to diffusive fluxes close to zero. However, it remains unclear whether bioturbation or hydrodynamic irrigation are responsible for that. An attempt was made to estimate the effect of hydrodynamic irrigation for a range of reasonable bottom water velocities from the sediment bedform geometry after Neumann et al. (2017). However, resulting potential hydrodynamic irrigation was found to be rather low compared to reported values from the literature and may significantly underestimate actual irrigation of the sandy sediments.
In a new non steady-state multi-element diagenetic modeling approach (Bo Liu, IOW), vertical δ13C pore water profiles were used to estimate the significance of advective transport processes. Mixing processes at the sediment-water interface were expressed as multiple of pure diffusion (ε). By adjusting this mixing coefficient as boundary condition for best fits of predicted to measured pore water profiles, the degree of these mixing processes was estimated. In the Arkona Basin surface sediments, this approach revealed best fits assuming a mixing depth of 3.5 cm with a tenfold higher total mixing degree based on diffusional transport. The calculated mixing depths and intensities were similar to bioturbation depths and rates obtained via traditional methods (Morys, 2016). The novel modeling approach is a promising method to evaluate surface sediment mixing processes.
Increased mineralization activity during productive seasons lead to increased oxygen consumption and therefore frequent bottom water hypoxia. The effects of hypoxic bottom water conditions on the early diagenetic processes in the sediments were studied via prolonged core incubation experiments. Shifts to bottom water oxygen deficiency had various consequences for benthic reservoirs and fluxes. The oxygen consumption decreased during hypoxic incubation phases. Decreased bioturbation activity diminished advective transport so that total fluxes of redox-insensitive solutes (e.g. H4SiO4) were decreased. Reactive iron and manganese oxides act as barrier (“iron curtain”) in the suboxic zone, preventing redox-sensitive solutes from their release into the water column. After a shift to bottom water hypoxia, these reactive oxides are re-dissolved and liberated into the water column. Prolonged incubation experiments suggested that the Mn-oxide reservoirs were depleted first before the Fe-oxides with the adsorptively bound phosphate were liberated.
The release of nutrients (especially phosphorus), dissolved inorganic carbon, and redox- sensitive compounds (e.g. hydrogen sulfide) strongly varied in the different studied environments, covering coastal-near oxic, temporary hypoxic and euxinic conditions.
In environments, where advection through hydrodynamic irrigation or bioturbation do not occur, like in anoxic or euxinic systems, surface-near pore water gradients reflect total solute interfacial fluxes. They depend on the supply of organic matter to the sea floor, the mineralization rates in the sediments and the composition of the overlying bottom water. In the Black Sea, much of the organic matter mineralization was performed already in the water column and not in the sediments, leading to a decreasing export of organic matter to the sediment and increasing concentrations of mineralization products in the bottom water with increasing water depth. Accordingly, benthic fluxes across the sediment-water interface of the deep Black Sea sites were the lowest of the entire study, essentially reflecting the low mineralization rates of AOM in deeper layers. Highest fluxes in the Black Sea were observed at the continental slope at intermediate water depths, but were still lower than in the Baltic Sea deeps.
In contrast, highest DIC fluxes were detected in the oxygen depleted Baltic Sea deeps Gotland Deep and Landsort Deep. These basins are shallow enough that reactive organic matter reaches the sea floor, where it is mineralized via sulfate reduction close to the sediment surface. Strong concentration gradients and therefore high diffusive interfacial fluxes across the sediment-water interface were evident. As bioturbation was absent, these diffusive fluxes were representative for total interfacial fluxes unlike in sediments with additional advective flux components.
In more complex environments, like coastal oxic sediments, inhabited by macrofauna, overflowed by currents and affected by resuspension events, the sediment surface represents an interface between turbulent and calm conditions, high and low concentrations and/or different redox-states. Such gradients are the basis for intensive exchange processes. The surface sediments in the coastal sites of the southern Baltic Sea were characterized by active organic matter mineralization via sulfate reduction and mixing induced secondary reactions taking place in the suboxic zone. This included the removal of dissolved sulfide due to iron oxide reduction with simultaneous liberation of PO4 into the interstitial waters. High surface near P reservoirs existed due to internal P cycling within the sediments, which was in turn driven by continuous re-oxidation of the reduced iron by downwards transported oxygen. These reservoirs were only actually released into the water column during bottom water oxygen deficiency situations, when the iron re-oxidation was inhibited.
This also applies for sediments of the Gulf of Finland and the deeper Baltic Sea basins Bornholm Basin and Gdanks basin, where temporary hypoxic conditions are responsible for recurrent benthic phosphate release.
The widespread occurring phosphate adsorption on sedimentary solid iron phases is a much debated ecosystem service of marine sediments. As discussed in this work, the sedimentary P liberation rates and pools of readily bio-available dissolved phosphate can be substantial in Baltic Sea muddy sediments. Actual phosphate fluxes across the sediment-water interface, though, are relatively small because phosphate is scavenged by adsorption on iron oxyhydroxides that are usually an integral part of coastal marine sediments if overlain by oxic bottom waters. Although muddy sediments often show only an oxic layer of only few millimeters thickness, adsorption capacities of iron oxyhydroxides are large enough to substantially retain P from being liberated into the water column. Falling under reducing conditions, iron oxyhydroxides are re-dissolved, liberating high amounts of PO4 into the surrounding waters. While permanently oxic sediments will preserve adsorbed P, permanently anoxic sediments steadily release mineralized P into the water column. However, sedimentary environments of oscillating redox conditions are predestined for high, burst-like benthic P fluxes. Especially in a quasi isolated environment like the Baltic Sea, with high nutrient inputs but only few sinks, these internal recycling processes promote eutrophication in the long-term. Further expanding hypoxia and anoxia in the Baltic Sea with a subsequent loss of benthic fauna and altered nutrient dynamics in the surface sediments may be the consequence.
Als Folge des demographischen Wandels in Deutschland erkranken immer mehr Menschen an chronischen Erkrankungen und Tumorleiden, wobei über 70.000 Frauen pro Jahr die Diagnose Mammakarzinom erhalten. Diese Sachlage erklärt, warum sich aktuell viele Studien mit molekularen Pathomechanismen der Mammakarzinom-Entstehung sowie der Optimierung und Weiterentwicklung medikamentöser Therapieregime bei Mammakarzinom-Patientinnen befassen. Aber auch die Verbesserung der diagnostischen Möglichkeiten ist wesentlicher Bestandteil derzeitiger Forschung.
Während die Mammografie über einen langen Zeitraum als die Methoden der Wahl zur Mammakarzinom-Diagnostik angesehen wurde, wird der kontrastmittelgestützten Magnetresonanzmammografie (ceMRM) derzeit ein immer höherer Stellenwert beigemessen. Da alle Untersuchungsmodalitäten in Abhängigkeit von individuellen anatomischen und physiologischen Gegebenheiten der weiblichen Brust in ihrer Sensitivität und Spezifität deutlich eingeschränkt sein können, ist es umso wichtiger individuelle Untersuchungskonzepte zu finden. Vor diesem Hintergrund wurden im Rahmen der vorliegenden Arbeit bei 461 Probandinnen die Parameter Brustdichte (BD) und Brustparenchymenhancement (BPE) mittels ceMRM qualitativ und quantitativ bestimmt, da diese bekanntermaßen erheblichen Einfluss auf die am häufigsten verwendeten Modalitäten, Mammografie und ceMRM, nehmen können. Des Weiteren wurde der Zusammenhang zwischen BD und BPE sowie der potenzielle Einfluss des Menopausen-Status auf beide Parameter untersucht. Zur Validierung der Auswertung und qualitativen Einschätzung der Ergebnisse wurden im Voraus beide Parameter hinsichtlich ihres Inter- und Intra-Reader-Agreements anhand von 49 Probandinnen untersucht.
Basierend auf den initial durchgeführten Analysen zu anthropometrischen und anamnestisch erhobenen Parametern konnte beurteilt werden, inwieweit das Probandinnenkollektiv eine repräsentative Stichprobe aus der weiblichen Bevölkerung darstellt und ob somit eine Übertragbarkeit der Ergebnisse auf die weibliche Bevölkerung sinnvoll und möglich erscheint. Zusätzlich konnten potenzielle Confounder ausgeschlossen werden, was insofern relevant ist, als dass sich damit das Risiko fälschlicherweise angenommener Zusammenhänge minimiert.
Übereinstimmend mit der aktuellen Literatur zeigte sich in den qualitativen und quantitativen Analysen sowohl die BD als auch das BPE bei postmenopausalen Frauen verglichen mit prämenopausalen Frauen signifikant vermindert; eine Konstellation, die unter anderem dem Einfluss des postmenopausalen Absinkens der Sexualhormone geschuldet ist. Ferner ergaben sich intraindividuell signifikante Unterschiede in der Brustzusammensetzung mit höheren rechtsseitigen Brustdichten, für die derzeit noch keine abschließende pathophysiologische Erklärung gefunden werden konnte und die im Widerspruch zu dem bekanntermaßen vermehrt linksseitigen Auftreten von Mammakarzinomen stehen.
Als Kernpunkt dieser Arbeit können die im Anschluss durchgeführten Analysen bezüglich eines Zusammenhanges von BD und BPE in Abhängigkeit vom Menopausen-Status angesehen werden. Hierbei ergab sich weder in der qualitativen noch in der quantitativen Auswertung ein signifikanter Zusammenhang oder eine Übereinstimmung von BD und BPE. Die Ergebnisse der bisher lediglich sehr vereinzelt zu dem Thema veröffentlichten Studien decken sich nur bedingt mit den hier beschriebenen, wobei sich die Probandenkollektive allerdings auch in erheblichen Maß von dem hier untersuchten unterscheiden. Sowohl das Studiendesign, die Art der Probandinnen-Rekrutierung als auch die Ein- und Ausschlusskriterien und die endgültige Probandenzahl unterscheiden sich hierbei in erheblichem Maß, was demzufolge einen Vergleich mit dieser Arbeit nur eingeschränkt möglich macht.
Das Fehlen einer Assoziation zwischen BD und BPE in der ceMRM bedeutet, dass beim Vorliegen einer mammografisch hohen BD nicht automatisch von einem zusätzlich starken BPE in der ceMRM ausgegangen werden kann und umgekehrt. Andere Einflussfaktoren, wie z. B. der Menopausen-Status, die Zyklusphase, Körpergewicht und BMI, oder auch die Einnahme von Hormonpräparaten sind für die starke interindividuelle Variabilität des BPE ebenfalls verantwortlich. Weitere Studien sind zukünftig erforderlich, um den Einfluss dieser Confounder auf das BPE näher zu untersuchen und um weitere Confounder zu identifizieren.
Unsere Daten sind insbesondere für Hochrisikopatientinnen und Patientinnen mit einem intermediären Risiko für die Entwicklung eines Mammakarzinoms von Bedeutung, da für diese in den aktuellen Leitlinien die jährliche ceMRM in der intensivierten Vorsorge empfohlen wird. Dabei handelt es sich mehrheitlich um junge, prämenopausale Frauen mit gehäuft mammografisch hoher BD und gehäuft ausgeprägtem BPE, wodurch eine erschwerte diagnostische Beurteilbarkeit resultieren kann. Um zusätzliche diagnostische Sicherheit zu erlangen, erscheint es daher im klinischen Kontext sinnvoll, individuell einen Modalitäten-Wechsel bzw. die Kombination mehrerer Modalitäten in der Diagnostik in Erwägung zu ziehen, wenn sich in einem Untersuchungsverfahren deutliche Beurteilungseinschränkungen der radiologischen Aufnahmen ergeben. Der individualisierte Einsatz einer oder mehrerer geeigneter Modalitäten könnte somit entscheidenden Einfluss auf eine frühzeitige Diagnosestellung nehmen und eine Verbesserung der Prognose erkrankter Frauen durch einen Therapiebeginn in einem früheren Tumorstadium ermöglichen.
Die neonatale sympathische Denervierung steigert die Agonist-induzierte Gefäßkontraktion. Dieser Effekt kommt möglicherweise durch modifizierte Signaltransduktionsmechanismen zustande. Dazu könnten Veränderungen in Rho-Kinase(ROCK)- und L-Typ-Ca2+-Kanalabhängigen Signalwegen gehören, welche den Rezeptoren der vasoaktiven Agonisten nachgeschaltet sind. In der vorliegenden Studie wurde untersucht, ob die verstärkte
Aktivierung der ROCK und/oder L-Typ-Ca2+-Kanäle zur gesteigerten
Noradrenalin(NA)-induzierten Gefäßkontraktion bei renalen Widerstandsgefäßen neonatal sympathektomierter Ratten beitragen. Für die experimentellen Untersuchungen wurden normotensive männliche Wistar-Ratten (Crl:Wi) verwendet. Diese wurden neonatal sympathektomiert oder scheinsympathektomiert. Es folgte die Untersuchung isolierter renaler
Widerstandsarterien 9 bis 12 Wochen alter Tiere mittels Small-Vessel-Myographie. Die Expression der L-Typ-Ca2+-Kanäle isolierter renaler Widerstandsgefäße wurde mittels Western-Blot untersucht. Zusätzlich wurde das Ruhemembranpotential der glatten Gefäßmuskelzellen mittels Mikroelektroden registriert. Die neonasale sympathische Denervierung führte im Vergleich zur Scheinsympathektomie zu einer gesteigerten NA-Sensitivität bei Gefäßen normotensiver Ratten. Sowohl die Inhibition der ROCK als auch die Blockade von L-Typ-Ca2+-Kanälen führte zur Rechtsverschiebung der NA-Konzentrations-Wirkungs-Kurve. Diese Effekte waren bei Gefäßen sympathektomierter Tiere im Vergleich zu scheinbehandelten Kontrollen deutlicher ausgeprägt. L-Typ-Ca2+-Kanalaktivierung mittels
S-(-)-BayK8644 löste bei Nierenarterien sympathektomierter Ratten starke
Gefäßkontraktionen aus. Die Gefäße der Kontrollen reagierten hingegen nur schwach auf S-(-)-BayK8644. Der Proteingehalt der a1-Untereinheit der L-Typ-Ca2+-Kanäle glatter Gefäßmuskelzellen unterschied sich nicht zwischen beiden Gruppen. Das Ruhemembranpotential glatter Gefäßmuskelzellen unterschied sich statistisch signifikant zwischen beiden Gruppen (p < 0,05) und betrug –57,5 ± 2,2 mV bei renalen Widerstandsgefäßen sympathektomierter Tiere und –64,3 ± 0,3 mV bei Kontrollen. Die Depolarisation der glattmuskulären Zellmembran durch kaliumreiche Organbadlösung steigerte die S-(-)-BayK8644-induzierte Kontraktion der Gefäße sympathektomierter Tiere und löste Kontraktionen bei Gefäßen der Kontrolltiere aus. Die Aktivierung von KATP-Kanälen führte zum vollständigen Verschwinden der S-(-)-BayK8644-induzierten Kontraktion bei Gefäßen
sympathektomierter Tiere. Diese Befunde zeigen, dass die sympathische Denervierung renaler Gefäße zu einer Depolarisation des Membranpotentials der Gefäßmuskelzellen führt, die zu einer gesteigerten Aktivierbarkeit L-Typ-Ca2+-Kanal-abhängiger Signalwege beiträgt.
Im Rahmen der durchgeführten Arbeit wurde die Schnittstelle Schockraum bei der Traumaversorgung aus Sicht der beteiligten Einrichtungen im Bereich des TraumaNetzwerkes Berlin evaluiert.
Die Versorgung von schwerverletzten Patienten bedarf eines gut funktionierenden und eng aufeinander abgestimmten Netzwerkes aus präklinischer Versorgung durch den Rettungsdienst und weiterer klinischer Versorgung des Patienten in Traumazentren. Die entscheidende Schnittstelle zwischen den Bereichen Präklinik und Klinik stellt der Schockraum eines Traumazentrums dar. Aufgrund der hohen Komplexität der Abläufe und Strukturen an eben dieser Schnittstelle sind zwingend Maßnahmen zur Qualitätssicherung mit dem Ziel der Optimierung von Abläufen und Strukturen erforderlich. Bisherige Mechanismen zum Qualitätsmanagement für Rettungsdienste und Kliniken erfassen die wesentliche Schnittstelle zwischen beiden Bereichen nur ungenügend.
Das gewählte Studiendesign stellt Screeningtool dar, um Schwachstellen an der Schnittstelle zwischen Präklinik und Klinik aufzudecken. Detaillierte Aussagen zu konkreten Prozeduren sind mit Hilfe des genutzten Tools aufgrund der hierfür benötigten deutlich umfangreicheren Datenerhebung nicht möglich gewesen, zumal die einfache Anwendbarkeit im notfallmedizinischen Kontext im Vordergrund steht.
Aus den gewonnenen Ergebnissen konnten verschiedene qualitätssichernde Maßnahmen abgeleitet werden. Allen beteiligten Einrichtungen wurde nach Abschluss der Studie ein individueller Ergebnisbericht zur Verfügung gestellt.
Zusammenfassend konnte festgestellt werden, dass die Abläufe an der Schnittstelle Schockraum insbesondere unter dem Aspekt der zeitlichen Abläufe und der Kooperation zwischen den verschiedenen Bereichen im Wesentlichen gut funktionierten. Die präklinische Versorgung durch den Rettungsdienst wurde von den Kliniken insgesamt als gut eingeschätzt, wobei in einzelnen Bereichen der Patientenversorgung Defizite mit einem sich daraus ableitbaren Nachschulungsbedarf aufgedeckt werden konnten. Als unproblematisch und weitgehend korrekt stellte sich die Auswahl der Zielklinik dar. Auch mit Blick auf die Notarztbeteiligung und die Voranmeldung von Patienten in der Zielklinik konnten keine wesentlichen Defizite aufgedeckt werden.
Als limitierende Faktoren dieser Studie ist die Zahl der eingeschlossenen Fälle zu sehen, so dass bei deutlich höheren Fallzahlen belastbarere Aussagen zu erwarten wären. Auch Aussagen über das Outcome der Patienten lässt die Studie nicht zu.
Das genutzte Screeningtool ist dafür geeignet, um auch in anderen Rettungsdienstbereichen in anderen Bundesländern angewendet zu werden.
