Einleitung und Fragestellung:
Die dynamische kontrastmittelverstärkte MR-Mammografie besitzt eine hohe Sensitivität für die Entdeckung von Mammakarzinomen in frühen Stadien und wird zunehmend in der Diagnostik des Mammakarzinoms als auch als Screeningverfahren bei Hochrisikopatientinnen eingesetzt. Dabei kommt dem Kontrastmittelverhalten detektierter Läsionen als auch des Brustparenchyms eine zentrale Bedeutung zu.
In der vorliegenden Studie wurde die Variabilität der nativen Baseline T1w-Signalintensität (BSI) und des relativen Kontrastmittelenhancements (RCE) des gesundes Brustparenchyms in der dynamischen kontrastmittelgestützten MR-Mammographie innerhalb einer Bevölkerungsstudie untersucht und Prädiktoren auf deren Variabilität identifiziert. Es wurden der Einfluss anthropometrischer Parameter (Alter, Größe, Gewicht, BMI), des Menopausenstatus, exogener Hormone (orale Kontrazeptiva, postmenopausale Hormonersatztherapie) sowie der Einfluss früheren Stillverhaltens untersucht.
Methodik:
Im Rahmen eines Ganzkörper-MRT-Projektes innerhalb der Study of Health in Pomerania (SHIP) wurden 651 Frauen (mittleres Alter 48,96 Jahre ± 12,35 [Standardabweichung]) in eine kontrastmittelgestützte MR-Mammografie Substudie unter Verwendung einer T1-gewichteten, dreidimensionalen TWIST Sequenz vor und nach Injektion von 0,1 mmol/kg Körpergewicht Gadobutrol eingeschlossen. Nach Ausschluss von 192 (29,5%) Frauen aufgrund von früheren Brustkrebserkrankungen, Brustoperationen, vollständiger Parenchyminvolution oder Herdbefunden wurden die Daten von 459 (71,5%) Frauen analysiert. Davon waren 234 (51.0%) prämenopausal und 225 (49.0%) postmenopausal. 69 der prämenopausalen Frauen gaben an, orale Kontrazeptiva einzunehmen und 24 der postmenopausalen Frauen gaben an, eine Hormonersatztherapie anzuwenden.
Unter Verwendung der Mean-Curve Funktion der MRT-Workstation für die Mammadiagnostik wurde eine Freihand-Region-of-Interest (ROI) manuell so platziert, dass das gesamte fibroglanduläre Gewebe der Brust auf jeweils zwei Schichten oberhalb und unterhalb der Mamille eingeschlossen wurde. Eine Zeit-Signalintensitäts-Kurve wurde für die Zeitpunkte vor bzw. 1 bis 5 Minuten nach Gadobutrol-Injektion
generiert. Für die weiteren Analysen wurde das relative prozentuale Kontrastmittelenhancement (RCE) anhand der Formel RCE= [SI (t1-5) – SI (Baseline)] / SI (Baseline)* 100 berechnet, wobei SI (t1-5) die Signalintensität nach Gabe von Gadobutrol ist. Da der zeitliche Kontrastmittelverlauf in beiden Brüsten identisch war, wurde das mittlere RCE über beide Brüste in allen weiteren Analysen modelliert.
Ergebnisse:
Die BSI variierte erheblich zwischen den Probandinnen mit einem Mittelwert (Standardabweichung - SD) von 167,7 (±49,2) in der rechten Brust und einem Mittelwert (SD) von 175,9 (± 48,9) in der linken Brust (p < 0,001). Die BSI stieg linear mit dem Alter (p<0,001), dem Körpergewicht (p<0,001) und dem BMI (p<0,001) an und nahm linear mit der Körpergröße (p<0,001) ab. Postmenopausale Frauen hatten eine signi- fikant höhere BSI (p<0,001).
Die BSI und das RCE waren für alle Zeitpunkte in der rechten Brust signifikant nied- riger als in der linken Brust (p=0,001). Das RCE zeigte einen steigenden Verlauf, der mit zunehmender Zeit abschwächte. Es betrug zu den Zeitpunkten 1, 2, 3, 4 und 5 Minuten nach Kontrastmittelgabe 8,07%, 13,78%, 18,23%, 22,14% und 24,56%. Al- lerdings variierte das RCE stark zwischen den Probandinnen (p=0,001).
Das RCE nahm mit steigendem Körpergewicht signifikant zu (p=0,0001); je schwerer die Frau, desto steiler die Zunahme. Der Einfluss des Alters und der Menstruations-Zyklusphase auf das RCE des Brustparenchyms waren dagegen vernachlässigbar. Das RCE des gesunden Brustparenchyms war mit dem Menopausenstatus stark assoziiert (p=0,001). Das Brustparenchym prämenopausaler Frauen hatte initial eine niedrigere BSI (p=0,0001), aber anschließend ein im Mittel 30% steileres RCE (p<0,0001) im Vergleich zu dem postmenopausaler Frauen.
Während bei prämenopausalen Frauen das RCE des Brustparenchyms signifikant geringer unter der Einnahme oraler Kontrazeptiva (OC) im Vergleich zu Frauen ohne eine hormonelle Kontrazeption (p=0,01) war, hatte bei postmenopausalen Frauen eine Hormonersatztherapie keinen signifikanten Einfluss auf das RCE des Brustparenchyms HT (p=0,052).
Das RCE des Brustparenchyms war bei Frauen, die angaben gestillt zu haben signifikant höher als bei Frauen, die nicht gestillt haben (p=0,04), was sowohl bei prämenopausalen als auch bei postmenopausalen Frauen zu beobachten war.
Schlussfolgerung:
Die BSI und das RCE des gesunden Brustparenchyms variierten erheblich zwischen den Probandinnen einer populationsbasierten Kohortenstudie. Körpergewicht (p<0,001) und Alter (p<0,001) waren signifikante Prädiktoren der BSI; Menopausen- status (p<0,001) und Körpergewicht (p<0,001) signifikante Prädiktoren des RCE. Diese Prädiktoren könnten neben dem Kontrastmittelenhancement des gesunden Brustparenchyms auch die Analysen der Kontrastmittelkinetik von Brustläsionen beeinflussen. Daher sollten bei der Analyse dynamischer MR-Mammografien patientenbezogene Faktoren, insbesondere der Menopausenstatus und das Körpergewicht berücksichtigt werden.
Da die körpergewichtbezogene Erhöhung der absoluten Kontrastmittelmenge zu einer signifikanten Erhöhung des RCE im Brustparenchym führte, sollte die Kalkulation der Kontrastmitteldosis basierend auf dem absoluten Körpergewicht in der MR-Mammografie überdacht werden und alternative Berechnungen z.B. basierend auf dem mageren Körpergewicht (ohne Fettanteil) untersucht werden.
Da postmenopausale Hormonersatzpräparate keinen signifikanten Einfluss auf das RCE haben und prämenopausale orale Kontrazeptiva das RCE eher reduzieren, scheint ein Absetzen hormoneller Präparate vor einer MR-Mammografie-Untersuchung basierend auf den vorliegenden Daten nicht notwendig.
Zusammenfassung
Der Iran besitzt zwölf UNESCO-Biosphärenreservate, die reich an einmaligen Natur- und Kulturschätzen und hohem menschlichen Potenzial aus verschiedenen ethnischen Gruppen sind. Die ersten neun Biosphärenreservate wurden frühzeitig mit den ersten Biosphärenreser-vaten der Welt im Jahr 1976 gegründet, die auch gleichzeitig andere Kategorien der Schutz-gebiete im Iran wie Nationalparks, geschützte Lebensräume für Wildtiere und Naturschutzge-biete beinhalten und bis heute unter ihrem alten Status verwaltet werden. Damit entsprechen sie nicht den aktuellen internationalen Anforderungen an Biosphärenreservate und besteht die Gefahr, dass diese Gebiete in baldiger Zukunft ihre natürlichen und kulturellen Werte verlie-ren und irreversibel beschädigt werden.
Diese Studie untersucht und bewertet die zwei exemplarisch ausgewählten iranischen Bio-sphärenreservate Golestan und Dena unter Berücksichtigung der UNESCO-Kriterien, unter anderem die Ziele und Grundlagen der Sevilla-Strategie und der Internationalen Leitlinien für das Weltnetz der Biosphärenreservate (1995). Das Biosphärenreservat Golestan wurde im Jahr 1976 gegründet und ist somit eines der ältesten Biosphärenreservate des Irans. Bei dem im Jahre 2010 gegründeten Biosphärenreservat Dena, handelt es sich um das jüngste Biosphä-renreservat im Iran zu Beginn der Studie.
Beide Schutzgebiete sind gebirgig und beinhalten die wichtigsten Waldökosysteme mit einer großen Biodiversität. Das Biosphärenreservat Golestan befindet sich im Nordosten des Irans im östlichsten Teil des Elburs-Gebirge und Dena liegt im zentralen Zagros-Gebirge im Westiran.
Für den methodischen Ansatz dieser Studie wurde ein Methodenmix aus qualitativen Elemen-ten: Oral History, Interviews, offenen Fragen und Teilnehmender Beobachtung und quantita-tiven Elementen: SWOT-Analyse (engl. Akronym für Strengths (Stärken), Weaknesses (Schwächen), Opportunities (Chancen) und Threats (Bedrohungen) und Auswertung der Fra-gebögen mit Hilfe des statistischen Programms SPSS20 angewendet.
Die untersuchten Gruppen bestanden gemäß der jeweiligen Analyse aus Experten des De-partments für Umwelt (DoE) in Teheran, den Provinz-Umweltschutzbehörden von Golestan und Kohgiluye und Boyer Ahmad, Akademikern, der Nationale Commission for UNESCO in Teheran, Zeitzeugen, lokaler Bevölkerung, Rangern, Umwelt-NGOs (engl. Non-Governmental Organization), dem Tourismus-Sektor und den Umwelt-Medien.
Die Ergebnisse in dieser Studie zeigen, dass die Entwicklung der iranischen Biosphärenreser-vate seit ihrer Gründung 1976 bis heute von den Veränderungen der wirtschaftlichen, politi-schen und gesellschaftlichen Situation des Irans und demzufolge von den Veränderungen in der Organisationsstruktur des Departemants für Umwelt (DoE) und der Prioritätensetzung in Bezug auf die Gesetze zu Umwelt- und Naturschutz beeinflusst wurden.
Überdies stellen die Ergebnisse dar, dass in den beiden untersuchten Biosphärenreservaten Golestan und Dena hinsichtlich der internationalen UNESCO-Kriterien und Richtlinien ver-gleichsweise ähnliche Defizite und Mängel bestehen:
• fehlende nationale Rechtsstruktur für die Biosphärenreservate im Iran,
• fehlender Managementplan für Biosphärenreservate und somit auch schwaches Mana-gementsystem der Biosphärenreservate,
• Mangel an Kenntnissen über Biosphärenreservate,
• beschränkte Beteiligung an den Angelegenheiten der Biosphärenreservate seitens aller untersuchten Gruppen – von der lokalen Bevölkerung bis hin zu den staatlichen Ent-scheidungsträgern und
• ungenügende Zusammenarbeit zwischen Staat und Interessengruppen in diesen Gebie-ten.
Ebenso wurde in dieser Studie versucht, konkrete Lösungsansätze zur Verwirklichung der Ziele der Biosphärenreservate bzw. der Verbesserung ihrer aktuellen Situation zu empfehlen.
In diesem Zusammenhang ist es erforderlich, dass Gesetze für die Biosphärenreservate auf nationaler Ebene definiert und die vorhandenen Biosphärenreservate im Iran gründlich nach internationalen Kriterien untersucht und mit einem systematischen Managementplan auf wis-senschaftlicher Grundlage verwaltet werden. Des Weiteren benötigen diese Gebiete für ihre Funktionalität eine Erhöhung und Verbesserung der Kenntnisse über die Biosphärenreservate der aktiven Personen, sowie der Kooperation und Kommunikation zwischen allen zuständigen Behörden und Interessengruppen. Hiermit soll allen sozialen, kulturellen, geistigen und wirt-schaftlichen Anliegen der Interessengruppen, vor allem aber der lokalen Bevölkerung, Rech-nung getragen werden, entsprechend dem weltweiten Ansatz der UNESCO-Biosphärenreservate.
Ziel dieser Untersuchung sollte eine Validierung der Sehschärfeprüfung mit einem modifizierten Optotypen sein, um Simulation und Aggravation im gutachterlichen Sehtest aufzudecken. Dieses Testverfahren wurde hier nicht an simulierenden, sondern an wahrheitsgemäß antwortenden Probanden erprobt, um deren Verhalten auf besondere Optotypen bei einem Sehtest standardisiert zu evaluieren.
Transportproteine spielen für viele Arzneistoffe eine bedeutende Rolle bei der Absorption, Verteilung und Elimination und können folglich auch maßgeblich über die Anflutung eines Wirkstoffs am Ort der Wirkung und / oder Nebenwirkung(en) entscheiden. Ziel dieser Arbeit war es, die Affinität von Tigecyclin zu hepatischen Aufnahme- und Effluxtransportern zu bestimmen. Aus pharmakokinetischen Studien war bekannt, dass das Glycylcyclin hauptsächlich hepatisch ausgeschieden wird, wobei die zugrundeliegenden molekularen Transportmechanismen unbekannt waren, d.h. wie Tigecyclin von den Hepatozyten aufgenommen wird und wieder in die Galle bzw. in das Blut abgegeben wird. Die physikochemischen Eigenschaften der Substanz (logP-Wert: 0,8; pKs-Wert: 0,25/ 8,76) sprechen gegen eine rein passive Diffusion über biologische Membranen.
Um den hepatozellulären Aufnahmetransport aufzuklären, wurden Kompetitionsversuche und direkte Aufnahmeversuch mit Hilfe von stabil transfizierten HEK293-Zellmodellen, die Transportproteine der SLC- und SLCO-Familie stabil überexprimieren, durchgeführt. Unter Verwendung von inside-out-Vesikeln wurde der Effluxtransport von Tigecyclin über MRP2- und MRP3-Transporter untersucht. Um sicher zu gehen, dass die Zellmodelle funktionstüchtig sind, wurde deren Funktionalität vor jedem Versuch mittels bekannter Standardsubstrate und Standardinhibitoren bestätigt. Die direkten Aufnahmeversuche wurden mittels LC-MS/MS analysiert.
Tigecyclin zeigte im verwendeten Konzentrationsbereich und den angewendeten Inkubationszeiten keine zytotoxische Wirkung auf die Versuchszellen. Mit den Kompetitionsassays konnte nachgewiesen werden, dass Tigecyclin einen hemmenden Einfluss auf den Transport von bekannten OATP1B3- und OATP2B1-Referenzsubstraten hat, jedoch keine Beeinflussung von OATP1B1 und NTCP zeigte. Des Weiteren konnte in direkten Aufnahmeversuchen gezeigt werden, dass Tigecyclin ein Substrat des ubiquitär exprimierten Transporters OATP2B1 ist. Ein signifikanter Nachweis, dass Tigecyclin ein Substrat von OATP1B1-, OATP1B3- und NTCP-Transportern ist, gelang jedoch nicht.
Im Bezug auf Effluxtransporter konnte eine Kompetition von Tigecyclin mit dem MRP2- und MRP3-vermittelten Transport in inside-out-Vesikeln nachgewiesen werden. Ein direkter Transport von Tigecyclin über die Vesikel ließ sich jedoch nicht beweisen. Aufgrund methodischer Schwierigkeiten und der sehr begrenzten Bearbeitungszeit, konnten die Versuche nicht oft genug durchgeführt und keine entsprechend belastbaren Daten generiert werden.
Zusammenfassend kann aus den durchgeführten Untersuchungen abgeleitet werden, dass Tigecyclin ein Substrat von OATP2B1 ist und OATP2B1 somit möglicherweise für die hepatische Aufnahme des Arzneistoffes verantwortlich ist. Ob noch andere Transporter eine Rolle spielen, bleibt weiterhin unbekannt. Auch die konkrete Transportkinetik mit der Affinität (Km) und der Transportkapazität (vmax), bleibt nach diesen Versuchen noch unklar. Des Weiteren legen die Ergebnisse dieser Arbeit nahe, dass bei gleichzeitiger Tigecyclin-Gabe ein gewisses Risiko für unerwünschte pharmakokinetische Interaktionen mit Substraten der Transporter OATP1B3, OATP2B1, MRP2 und MRP3 besteht.
Als Fazit kann gesagt werden, dass mit dieser Arbeit ein Beitrag zur Aufklärung des Arzneimitteltransports und der Transporteigenschaften von Tigecyclin in den Hepatozyten geleistet werden konnte. Zusätzlich konnten weitere Erkenntnisse zur Kompetition gewonnen werden. In wie weit die in vitro Resultate auf in vivo Mechanismen übertragbar sind, ist unklar und bietet Möglichkeiten für weiterführende Untersuchungen.
Mit der Zielstellung einem Ärztemangel im nervenheilkundlichen Fachbereich entgegenzutreten ist die Ermittlung von Prädiktoren zur Vorhersage eines spezifischen Berufswunsches (einer Spezialisierungspräferenz) eines Medizinstudierenden von Interesse. Weiterhin erscheint es sinnvoll auch gerade die frühen praktischen Kontaktflächen zwischen Fachbereich und Medizinstudierendem
auf ihre Wahrnehmung zu betrachten. Gemeinsames Ziel der kumulativen Arbeiten war die Analyse von Angaben aus
studentischer Sicht zur späteren Fachbereichswahl in Neurologie und / oder Psychiatrie / Psychosomatik und im Bereich der Psychiatrie zu ersten Erfahrungen im
Pflegepraktikum. Es galt Aussagen zu Prädiktoren und Wahrnehmung zu treffen.
Three-year-olds and 4-year-olds have severe difficulties solving standard mental rotation tasks. Only 5-year-olds solve such tasks above chance reliably. In contrast studies relying on simplified mental rotation tasks indicate that infants discriminate between an object and its mirror image. Furthermore in another simplified mental rotation task with 3-year-olds, a linear relation between angular disparity and reaction time typical for mental rotation was revealed. Therefore it was assumed that 3-year-olds’ capabilities are underestimated. In the current study, 3-year-olds were trained in two isolated sessions to solve standard mental rotation tasks and were tested in a third session. Three-year-olds solved this test above chance as a group – a substantial number of them doing so on an individual level. However, a linear relation between angular disparity and reaction time, that would indicate an analog mental transformation, was not discernable. Nevertheless, these findings are in accordance with a continuous line describing mental rotation in infants and older children. And, these also indicate that children’s mental rotation capabilities might be underestimated.
Kardiovaskuläre Erkrankungen stellen seit Jahrzehnten in Industrienationen, wie Deutschland, die Haupttodesursache dar.1–4 Verschiedene Parameter nehmen Einfluss auf die Prävalenz und Inzidenz von kardiovaskulären Erkrankungen, sowie auf die Zunahme von Wanddicken großer arterieller Gefäße. Die Wanddicke der Arterien (MAWT) wird als ein Surrogatmarker für die Arteriosklerose verwendet, und diesbezüglich verschiedene Gefäßabschnitte untersucht.21,22 Auch MRT-Untersuchungen erfolgten, in welchem der bisherige Goldstandard die Dark-Blood Sequenz war.33,35–37 In der klinischen Routine wird jedoch im Rahmen eines Kardio-MRT eine cine steady-state free precession(SSFP)- Sequenz verwendet.38,39 Das Ziel unserer Arbeit war es die Dark-Blood (DB)- Sequenz mit der cine (SSFP)- Sequenz mittels Inter- sowie Intrarater Reliabilität zu vergleichen, sowie altersabhängige und geschlechtsspezifische Standardwerte der Wanddicke der Aorta ascendens und descendens in der Allgemeinbevölkerung anhand von Daten der Study of Health in Pomerania (SHIP- Studie) zu bestimmen.
Methode: Für den Vergleich der beiden Sequenzen wurden beide Sequenzen an 50 Nicht-SHIP-Probanden, welche ein Kardio- MRT erhielten, generiert. Diese wurden anschließend von zwei Untersuchern (AQ und TS) bezüglich Interratersowie Intrarater Reliabilität auf Höhe der rechten Pulmonalarterie vermessen und ausgewertet. In der zweiten Publikation erfolgte die Messung der Aortenwanddicke, sowie die Korrelation mit dem Alter und dem Geschlecht an 753 Personen aus der SHIPStudie.
Ergebnisse: Die cine SSFP- Sequenz konnte im Vergleich zu der DB- Sequenz, bei einer hohen Inter- und Intrarater Reliabilität, zuverlässige und vergleichbare Messwerte liefern. Insgesamt zeigte sich, dass die mittlere Aortenwanddicke der Aorta ascendens (1.46 mm bei Frauen, 1.56 mm bei Männern) im Vergleich zur Aorta descendens (1.26 mm bei Frauen, 1.36 mm bei Männern) größer war. Zusätzlich konnte ein Anstieg der Wanddicke mit zunehmendem Alter, sowie eine Geschlechtsabhängigkeit der Wanddicke nachgewiesen werden. Männer haben im Vergleich zu Frauen eine dickere Gefäßwand in beiden untersuchten Gefäßabschnitten auf (p<0.001). In weiteren Auswertungen stellte sich eine gering höhere Abhängigkeit der zunehmenden Aortenwanddicke mit zunehmendem Alter bei dem männlichen Geschlecht dar.
Bacteria are exposed to oxidative stress as an unavoidable consequence of their aerobic lifestyle. Reactive oxygen species (ROS) are generated in the stepwise one-electron reduction of molecular oxygen during the respiration. Pathogens encounter ROS during the oxidative burst of macrophages as part of the host immune defense. Besides ROS, bacteria also have to cope with reactive chlorine, electrophilic and nitrogen species (RCS, RES, RNS). To cope with these reactive species, bacteria have evolved different defense and repair mechanisms. To maintain the reduced state of the cytoplasm, they utilize low molecular weight (LMW) thiols. LMW thiols are small thiol-containing compounds that can undergo post-translational thiolmodifications with protein thiols, termed as S-thiolations. S-thiolations function as major redox regulatory and thiol-protection mechanism under oxidative stress conditions. In eukaryotes and Gram-negative bacteria, the tripeptide glutathione (GSH) functions as major LMW thiol, which is present in millimolar concentrations. The Actinomycetes, such as Mycobacterium and Corynebacterium species do not produce GSH and utilize instead mycothiol (MSH) as their alternative LMW thiol. In Firmicutes, including Bacillus and Staphylococcus species, bacillithiol (BSH) functions as the major LMW thiol. LMW thiols protect protein thiols against the irreversible overoxidation of cystein residues to sulfinic and sulfonic acids. In addition, LMW thiols contribute to the virulence and survival of pathogens, function in metal homeostasis and serve as enzyme cofactors for detoxification of xenobiotics and antibiotics. In this doctoral thesis, we aimed to investigate the roles of MSH and BSH in redox regulation of main metabolic enzymes under oxidative stress in the pathogens Corynebacterium diphtheriae and Staphylococcus aureus. Previous redox proteomics studies identified the glyceraldehyde-3-phosphate dehydrogenase GapDH and the aldehyde dehydrogenase AldA as S-thiolated in S. aureus and C. diphtheriae. Thus, we aimed to study the redox regulation of the metabolic enzyme GapDH in C. diphtheriae in response to NaOCl and H2O2 stress by S-mycothiolation, which is described in chapter 1. Moreover, we studied the involvement of the mycoredoxin-1 (Mrx1) and thioredoxin (Trx) pathways in reactivation of S-mycothiolated GapDH in vitro. Using shotgun proteomics, 26 S-mycothiolated proteins were identified under NaOCl stress in C. diphtheriae. These are involved in energy metabolism (Ndh, GlpD) and in the biosynthesis of amino acids (ThrA, LeuB), purines (PurA) and cell wall metabolites (GlmS). The glycolytic GapDH was identified as conserved target for S-thiolation across Gram-positive bacteria. GapDH was the most abundant protein, contributing with 0.75 % to the total cystein proteome. Moreover, GapDH is a conserved target for redox regulation and S-glutathionylation in response to oxidative stress in several prokaryotic and eukaryotic organisms. Treatment of GapDH with NaOCl and H2O2 in the absence of MSH resulted in irreversible enzyme inactivation due to overoxidation. Pretreatment of GapDH with MSH prior to H2O2 or NaOCl exposure resulted in reversible inactivation due to S-mycothiolation of the active site Cys153. Since S-mycothiolation is faster compared to overoxidation, S-mycothiolation efficiently protects the GapDH active site against overoxidation. The activity of S-mycothiolated GapDH could be restored by both, the Mrx1 and Trx pathway in vitro. Interestingly, the recovery of Smycothiolated GapDH by Mrx1 was faster compared to its reduction by the Trx pathway. In previous studies, the reactivation of S-mycothiolated Mpx and MrsA by the mycoredoxin pathway occurred also faster compared to the Trx pathway, which is consistent with our results. We were further interested to analyze the redox regulation of the glyceraldehyde-3phosphate dehydrogenase Gap of S. aureus under NaOCl and H2O2 stress, which is described in chapter 2. Using the quantitative redox proteomic approach OxICAT, 58 NaOCl-sensitive cystein residues with >10% thiol oxidation under NaOCl stress were identified. Gap and AldA showed the highest oxidation increase of 29% under NaOCl stress at their active site cystein residues. Using shotgun proteomics, five S-bacillithiolated proteins were identified, including Gap, AldA, GuaB, RpmJ and PpaC. Gap contributed with 4 % as most abundant cystein protein to the total cystein proteome. Our activity assays demonstrated that Gap of S. aureus is highly sensitive to overoxidation by H2O2 and NaOCl in vitro in the absence of BSH. The active site Cys151 of Gap was oxidized to the BSH mixed disulfide under H2O2 and NaOCl stress in the presence of BSH in vitro, which resulted in the reversible Gap inactivation. Moreover, inactivation of Gap by NaOCl and H2O2 due to S-bacillithiolation was faster compared to overoxidation, indicating that S-bacillithiolation protects the Gap active site against overoxidation in vitro. We further showed that the bacilliredoxin Brx catalyzes the reduction of S-bacillithiolated Gap in vitro. Molecular docking of BSH into the Gap active site revealed that S-bacillithiolation does not require major structural changes. Apart from Gap, the aldehyde dehydrogenase AldA was identified as S-bacillithiolated at its active site Cys279 under NaOCl stress in S. aureus previously. Thus, the expression, function, redox regulation and structural changes of AldA were analysed under NaOCl and aldehyde stress in S. aureus as summarized in chapter 3. AldA was S-bacillithiolated in the presence of H2O2 and BSH as demonstrated in BSH-specific Western blots in vitro. The expression of aldA was previously shown to be regulated by the alternative sigma factor SigmaB in S. aureus. Transcription of aldA was strongly increased in a SigmaB-independent manner under formaldehyde, NaOCl and diamide stress in S. aureus. Using an aldA deletion mutant, we demonstrated that aldA is required for growth and survival under NaOCl stress in S. aureus. The purified AldA enzyme was shown to catalyze the oxidation of various aldehyde substrates, including formaldehyde, methylglyoxal, glycolaldehyde and acetaldehyde in vitro. In addition, the function of the conserved Cys279 for AldA activity was investigated in vivo and in vitro. The purified AldAC279S mutant was shown to be inactive for aldehyde oxidation in vitro. Moreover, the aldAC279S mutant was very sensitive under NaOCl stress in vivo, and this phenotype could be reversed using the aldA complemented strain. These experiments demonstrate the function of Cys279 for AldA activity both in vitro and in vivo. AldA activity assays showed that AldA is sensitive to overoxidation and irreversible inactivation by H2O2 alone in vitro. In the presence of BSH, AldA is protected against overoxidation by reversible Sbacillithiolation in vitro. Molecular docking and molecular dynamics simulations revealed that BSH occupies two different positions in the Cys279 active site, which depend on the NAD+ cofactor. In the apoenzyme, BSH forms the disulfide with Cys279 in the “resting” state position, while Cys279 is S-bacillithiolated in the “attacking” state position in the holoenzyme in the presence of the NAD+ cofactor.
As the tree of life is populated with sequenced genomes ever more densely, the new challenge is the accurate and consistent annotation of entire clades of genomes. In my dissertation, I address this problem with a new approach to comparative gene finding that takes a multiple genome alignment of closely related species and simultaneously predicts the location and structure of protein-coding genes in all input genomes, thereby exploiting negative selection and sequence conservation. The model prefers potential gene structures in the different genomes that are in agreement with each other, or—if not—where the exon gains and losses are plausible given the species tree. The multi-species gene finding problem is formulated as a binary labeling problem on a graph. The resulting optimization problem is NP hard, but can be efficiently approximated using a subgradient-based dual decomposition approach.
I tested the novel approach on whole-genome alignments of 12 vertebrate and 12 Drosophila species. The accuracy was evaluated for human, mouse and Drosophila melanogaster and compared to competing methods. Results suggest that the new method is well-suited for annotation of a large number of genomes of closely related species within a clade, in particular, when RNA-Seq data are available for many of the genomes. The transfer of existing annotations from one genome to another via the genome alignment is more accurate than previous approaches that are based on protein-spliced alignments, when the genomes are at close to medium distances. The method is implemented in C++ as part of the gene finder AUGUSTUS.
For the last two decades, heparins have been widely used as anticoagulants. Besides
numerous advantages, up to 5% patients with heparin administration suffer from a major adverse
drug effect known as heparin-induced thrombocytopenia (HIT). This typical HIT can result in deep
vein thrombosis, pulmonary embolism, occlusion of a limb artery, acute myocardial infarct, stroke, and
a systemic reaction or skin necrosis. The basis of HIT may lead to clinical insights. Recent studies using
single-molecule force spectroscopy (SMFS)-based atomic force microscopy revealed detailed binding
mechanisms of the interactions between platelet factor 4 (PF4) and heparins of different lengths in
typical HIT. Especially, SMFS results allowed identifying a new mechanism of the autoimmune HIT
caused by a subset of human-derived antibodies in patients without heparin exposure. The findings
proved that not only heparin but also a subset of antibodies induce thrombocytopenia. In this review,
the role of SMFS in unraveling a major adverse drug effect and insights into molecular mechanisms
inducing thrombocytopenia by both heparins and antibodies will be discussed.
Die vorliegende Arbeit untersucht die Relevanz des Intermediärfilamentes Zytokeratin 15 (K15) als potentiellen epithelialen Stammzellmarker im Darm und den Einfluss K15 positiver Zellreihen auf die Krypta Homöostase.
Zwei Hauptstammzellpools regulieren den schnellen Zellumsatz im Darmepithel. Dies sind einerseits schnell proliferierende Lgr5 positive Stammzellen, welche zwischen den Paneth-Zellen an der Krypta-Basis vorgefunden werden. Anderseits gibt es vermutlich langsamer wachsende Bmi1 positive Zellen, welche sich an der +4-Position oberhalb der Krypta-Basis befinden. Im Haarfollikel und im Ösophagusepithel stellt das Intermediärfilament K15 einen Marker für Stammzellen dar, die zur Gewebereparatur beitragen. In dieser Arbeit haben wir gezeigt, dass K15 im Darm langlebige Kryptazellen mit Multipotenz- und Selbsterneuerungspotenzial markiert. K15 positive Krypta-Zellen sind resistent gegen hochdosierte ionisierende Strahlung und tragen zur Kryptaexpansion bei. Die hier vorgestellten Ergebnisse zeigen nun erstmals im Darm eine langlebige, multipotente K15 exprimierende Kryptazellpopulation, die eine Selbsterneuerungskapazität besitzt. Insbesondere führt der Verlust des Tumorsuppressor Gens Apc in K15 positiven Zellen zur Adenombildung, die potentiell zum Adenokarzinomen fortschreiten können. Wir erörtern die Hypothese, dass K15 eine Gruppe langlebiger, strahlenresistenter Stammzellen markiert, die die Homöostase der Krypta und die Regenerationsfähigkeit maßgeblich beeinflussen.
The Flavivirus genus (Flaviviridae family) comprises the most important arboviruses in the world such as dengue virus, West Nile virus (WNV), Zika virus (ZIKV), Japanese encephalitis virus and yellow fever virus (YFV). Every year, several outbreaks caused by flaviviruses are reported worldwide (i.e.: ZIKV and YFV outbreaks in South America) with a huge impact on economy and public health. In the last few decades, many aspects of the flavivirus biology and the interaction of flaviviruses with host cells have been elucidated. However, many underlying mechanisms concerning receptor usage, entry process and viral interaction with host cell factors are still not completely understood. Integrins, the major class of cell adhesion molecules have been implicated in the infectious cycle of different viruses including flaviviruses. A previous report proposed that a particular integrin, the αVβ3 integrin, might act as a cellular receptor for WNV. However, this hypothesis was not confirmed by other groups. In the present study, murine cell lines lacking the expression of one or more integrin subunits were used to evaluate the involvement of different integrins in the flavivirus infection cycle. Mouse fibroblasts lacking the expression of β1 integrin (MKF-β1-/-) or β3 integrin (MEF-β3-/-) subunits or αVβ3 integrin (MEF-αVβ3-/-) as well as their corresponding wild-type cells were utilized. A second model using Chinese hamster ovary cells (CHO-K1), a cell line that has been described to be refractory to some flaviviruses, were modified to express either αV (CHO-αV+/+) or β3 (CHO-β3+/+) integrin subunits. All cell lines were first characterized by confocal laser microscopy, flow cytometry and functional assays prior to infection to assess their integrin expression. The cell lines were then inoculated with different flaviviruses of public health relevance: WNV, YFV-17D, Usutu virus (USUV), Langat virus (LGTV) and ZIKV. Infection assays were designed in order to evaluate whether integrins influence i) cell susceptibility; ii) binding; iii) internalization and iv) replication of the investigated flaviviruses. Our findings clearly demonstrate that β1, β3 and αVβ3 integrins do not act as flavivirus cellular receptor or attachment factor since their ablation does not completely abrogate flavivirus infection in the investigated cell lines. Flavivirus binding to the cell surface of MEFs, MKFs and CHO cells was not disturbed by the genomic deletion of the above-mentioned integrins. The deletion of β1 and β3 integrin subunit did not affect internalization of any of the flaviviruses tested. In contrast to that, loss of αVβ3 integrin in the MEF-αVβ3-/- cells showed a statistically significant decrease in WNV and USUV internalization while ZIKV, YFV-17D and LGTV internalization remained unaffected suggesting that αVβ3 integrin might be involved in the internalization process of at least some flaviviruses. On the other hand, flavivirus replication was substantially impaired in the integrin-deficient cell lines in comparison to their corresponding wild-type cells. Both, MEF-β3-/- and MKF-β1-/- cells showed a statistically significant reduction on viral load for all flaviviruses tested in comparison to their respective wild-type cells. The MEF-αVβ3-/- cells in particular, showed a strong inhibition of flavivirus replication with a reduction of up to 99% on viral loads for all flaviviruses tested. Levels of flavivirus negative-strand RNA were substantially decreased in MEF-αVβ3-/- cells indicating that integrins might influence flavivirus RNA replication. The ectopic expression of either αV or β3 integrin subunits in CHO cells slightly increased the replication of all flaviviruses tested. Taken together, this is the first report highlighting the involvement of integrins in ZIKV, USUV, LGTV and YFV infection. The results strongly indicate that the investigated integrins play an important role in flavivirus infection and might represent a novel host cell factor that enhances flavivirus replication. Although the exact mechanism of interaction between integrins and flaviviruses is currently unknown, the results provided in this study deepen our insight into flavivirus - host cell interactions and open doors for further investigations.
Platelets transfusion is a safe process, but during or after the process, the recipient may experience an adverse reaction and occasionally a serious adverse reaction (SAR). In this review, we focus on the inflammatory potential of platelet components (PCs) and their involvement in SARs. Recent evidence has highlighted a central role for platelets in the host inflammatory and immune responses. Blood platelets are involved in inflammation and various other aspects of innate immunity through the release of a plethora of immunomodulatory cytokines, chemokines, and associated molecules, collectively termed biological response modifiers that behave like ligands for endothelial and leukocyte receptors and for platelets themselves. The involvement of PCs in SARs—particularly on a critically ill patient’s context—could be related, at least in part, to the inflammatory functions of platelets, acquired during storage lesions. Moreover, we focus on causal link between platelet activation and immune-mediated disorders (transfusion-associated immunomodulation, platelets, polyanions, and bacterial defense and alloimmunization). This is linked to the platelets’ propensity to be activated even in the absence of deliberate stimuli and to the occurrence of time-dependent storage lesions.
Glacitectonic deformation in the Quaternary caused the tectonic framework of large-scale folds and displaced thrust sheets of Maastrichtian (Upper Cretaceous) chalk and Pleistocene glacial deposits in the southwestern Baltic Sea area.
A wide spectrum of methods has been compiled to unravel the structural evolution of the Jasmund Glacitectonic Complex. The analyses of digital elevation models (DEM) suggest a division into two structural sub-complexes – a northern part with morphological ridges striking NW–SE and a southern part with SW–NE trending ridges. Geological cross sections from the eastern coast (southern sub-complex) were constructed and restored using the software Move™ and the complementary module 2D Kinematic Modelling™.
The final geometric model of the southern sub-complex shows a small-scale fold-and-thrust belt. It includes three different orders of architectural surfaces (see PEDERSEN, 2014): erosional surfaces and the décollement (1st order), thrust faults (2nd order), and beds outlining hanging-wall anticlines as well as footwall synclines (3rd order). Thrust faults of the southern structural sub-complex are mainly inclined towards south, which indicates a local glacier push from the S/SE.
The glacitectonic structures have a surface expression in form of sub-parallel ridges and elongated valleys in between. Geomorphological mapping and detailed landform analyses together with the structural investigations provide an insight into the chronology of sub-complexes formation. The northern part of the glacitectonic complex is suggested to have been formed before the southern one, considering the partly truncated northerly ridges and their superimposition by the southern sub-complex.
Although there is a high number of scientific publications on the glacitectonic evolution of Jasmund, these presented models often lack a consistent theory for the development integrating all parts of the 100 km2 large complex. Therefore, the combination of all results leads to a more self-consistent genetic model for the entire Jasmund Glacitectonic Complex.
Background: Magnetic resonance imaging (MRI) techniques are rarely used in the context of abdominal sepsis and in sepsis research. This study investigates the impact of MRI for monitoring septic peritonitis in an animal model (colon ascendens stent-induced peritonitis, CASP). The CASP model closely mimics that of human disease and is highly standardized. The most frequently employed readout parameter in mouse CASP studies is prolonged or decreased rate of survival. Monitoring the progression of peritonitis via MRI could provide a helpful tool in the evaluation of severity. The use of alternative readout systems could very well reduce the number of research animals. Perspectively, clinical improvement after certain treatment could be classified. Methods: This study describes for the first time MRI findings following the induction of septic peritonitis in mice using the CASP model. Two sublethal groups of mice with septic peritonitis were investigated. Each had received one of two differing stent diameters in order to control the leakage of feces into the abdominal cavity. Each mouse served as its own control. Imaging and analyses were performed blinded. Gut diameters, stomach volume, abdominal organ wall diameters, and volume of the adrenal glands were measured. Serum corticosterone levels were detected using ELISA. Serum IL-6, TNF-α, IL-1β, and IL-10 levels were screened by cytometric bead array. Statistical analysis was performed using the Mann-Whitney U test for nonparametric probes and the Kruskal-Wallis and t tests. Results: Using a 7-tesla MRI scanner 24 and 48 h after induction of septic peritonitis, interenteric fluid, organ swelling of spleen and adrenal glands, as well as dilatation of the stomach were compared to nonseptic conditions. Swelling of adrenal glands resulted in an increased serum corticosterone level. In addition, the wall of the intestine bowel was thickened. Based upon these findings, an MRI score (MRI sepsis score, MSS) for abdominal sepsis in mice was established. Reduced stent sizes led to reduced severity of the abdominal sepsis, which could be reproduced in the MSS, which is described here for the first time. Conclusions: Intraabdominal variations during septic peritonitis are detectable by MRI techniques. MRI methods should become a more important tool for the evaluation of abdominal peritonitis. MSS could provide an interesting tool for the evaluation of therapeutic strategies.

Aquaporins (AQPs) facilitate the transepithelial water flow involved in epithelial fluid secretion in numerous tissues; however, their function in the pancreas is less characterized. Acute pancreatitis (AP) is a serious disorder in which specific treatment is still not possible. Accumulating evidence indicate that decreased pancreatic ductal fluid secretion plays an essential role in AP; therefore, the aim of this study was to investigate the physiological and pathophysiological role of AQPs in the pancreas. Expression and localization of AQPs were investigated by real-time PCR and immunocytochemistry, whereas osmotic transmembrane water permeability was estimated by the dye dilution technique, in Capan-1 cells. The presence of AQP1 and CFTR in the mice and human pancreas were investigated by immunohistochemistry. Pancreatic ductal HCO3- and fluid secretion were studied on pancreatic ducts isolated from wild-type (WT) and AQP1 knock out (KO) mice using microfluorometry and videomicroscopy, respectively. In vivo pancreatic fluid secretion was estimated by magnetic resonance imaging. AP was induced by intraperitoneal injection of cerulein and disease severity was assessed by measuring biochemical and histological parameters. In the mice, the presence of AQP1 was detected throughout the whole plasma membrane of the ductal cells and its expression highly depends on the presence of CFTR Cl- channel. In contrast, the expression of AQP1 is mainly localized to the apical membrane of ductal cells in the human pancreas. Bile acid treatment dose- and time-dependently decreased mRNA and protein expression of AQP1 and reduced expression of this channel was also demonstrated in patients suffering from acute and chronic pancreatitis. HCO3- and fluid secretion significantly decreased in AQP1 KO versus WT mice and the absence of AQP1 also worsened the severity of pancreatitis. Our results suggest that AQP1 plays an essential role in pancreatic ductal fluid and HCO3- secretion and decreased expression of the channel alters fluid secretion which probably contribute to increased susceptibility of the pancreas to inflammation.
The Effect of the Patients Nutritional Status on Immune Alterations Induced by Ischemic Stroke

Ischemic stroke is one of the leading causes of death and disability throughout the world.
One important aspect of stroke pathophysiology are immunological changes after stroke, especially a combination of post stroke immunodepression, leading to
infectious complications after stroke and an activation of the immune system, leading to cerebral injury. Adipose tissue has several immunological functions and obesity
leads to immunological complications and is accompanied by a chronic immune activation.
To study the effects of body weight and obesity on the immune system and measure weight and fat tissue changes after ischemic stroke we conducted the LIPS Trial and enrolled 50 stroke patients and 16 control subjects between July 2015 and July 2016. On the day of admission and on the days 1, 2, 3, 4, 5, 7, 30, 90 and 180 after admission stroke patients were weighed with an in-bed scale, body composition was measured with BIA, the triceps-skin fold thickness was measured, the NIHSS scale was obtained and blood was drawn. FACS-analysis was performed and triglycerides,cholesterol, CRP and PCT were measured at the central laboratory facility of the Universitätsmedizin Greifswald. Luminex-multiplex analysis for multiple cyto- and chemokines was performed at the Multiplex Facility at the University Leiden. A cerebral MRI and an abdominal MRI were performed shortly after admission and on days 5-7 for most patients and the infarct volume, abdominal fat and hepatic fat percentage were measured. On days 30, 90 and 180 after stroke Bartel Index and mRS were obtained.
After stroke our patients showed the typical immunological changes described previously as stroke induced immune alterations, namely a post stroke immunodepression as well as signs of an activated immune system and an acute
phase response. Our patients lost weight, but only 1.7 ± 0.5 kg. Skinfold thickness did not change during the course of our trial and abdominal fat measurement did not change in stroke patients. Immunological parameters (leukocytes, neutrophils,CRP, PCT, IL-6) did not differ between BMI subgroups (normal weight: BMI < 25,overweight: BMI ≥ 25, < 30, obese: BMI ≥ 30) and in this trial we could not detect a
difference in patients with normal weight, overweight or obesity in the post stroke periode. In an additional analysis we could show that rapid clinical improvement
did result in a rapid improvement of post stroke immune alterations, especially for leukocytes, neutrophils, IL-6 and CRP.
Rabies virus (RABV) is an ancient, highly neurotropic rhabdovirus that causes lethal encephalitis. Most RABV pathogenesis determinants have been identified with laboratory-adapted or attenuated RABVs, but details of natural RABV pathogenesis and attenuation mechanisms are still poorly understood. To provide a deeper insight in the cellular mechanism of pathogenies of field RABV, this work was performed to assess virus strain specific differences in intra-neuronal virus transport, to identify cell culture adaptive mutations in recombinant field viruses and to explore shRNA-expressing RABVs as research tools for targeted host manipulation in infected cells.
Comparison of chimeric RABVs with glycoprotein (G) ecto-domains of different lyssaviruses, together with field RABVs from dog and fox in dorsal root ganglion (DRG) neurons revealed no detectable differences in the axonal accumulation of the viruses. This indicates that previously described G-dependent transport of newly formed RABV in axons can occur both in laboratory-adapted and field RABV. Moreover, partial overlap of nucleoprotein (N) and G protein particles in field virus infected DRG axons supported the hypothesis of the “separate model” for anterograde RABV transport.
Serial passages of recombinant dog and fox field clones in different cell lines led to the identification of general (D266N) and cell line specific (K444N) adaptive mutations in the G ecto-domain of both viruses. In BHK cells, synergistic effects of D226N, K444N and A417T on field dog virus G protein surface localization led to the loss of endoplasmic reticulum (ER) retention of G and increased virus titers in the supernatant, indicating that limited virus release by ER retention is a major bottleneck in cell culture adaptation. In addition, selection of mutations within the C-terminus of the RABV phosphoprotein (P) (R293H and R293C in fox and dog viruses, respectively) led to the hypothesis of altered binding affinities to nucleoprotein and RNP complexes. Identification of the above mentioned amino acid substitutions together with alterations in a suboptimal transcription stop signal in the P/M gene border indicated that adaptation to cell culture replication occurs on both levels, RNA transcription/replication and virus release.
To evaluate the possibility of an expression of a functional microRNA-adapted short-hairpin RNAs (miR-shRNA) expressing RABV, recombinant RABVs encoding miR-shRNAs against cellular Dynein Light Chain 1 (DYNLL1) and Acidic Nuclear Phosphoprotein 32 family member B (ANP32B) were generated. In spite of cytoplasmic transcription of the respective mRNAs, downregulation of DYNLL1 and ANP32B mRNA and respective protein levels in infected cells revealed correct processing to functional shRNAs. Specific downregulation of the cellular genes at 2, 3 and 4 days post infection further demonstrated feasibility of the approach in standard cell lines. However, it remained open whether miR-shRNA expressing RABV can be used to study neuro-infection in vivo. Since first attempts in primary rat neuron cultures failed, it has to be clarified in further experiments whether this strategy can be used in mature, non-dividing neurons or whether breakdown of the nucleus in the course of cell division is a requirement for the processing of cytoplasmically expressed miR-RNA by nuclear RNases.
By providing novel insights in axonal RABV transport and cell culture adaptive mutations this work extends the current understanding of RABV pathogenesis in natural and non-natural cell environments. Moreover, it provides a basis for further pathogenicity studies in which the impact of cell culture adaptation through increased virus release on RABV virulence can be investigated. With successful expression of functional miR-shRNAs from RABV vectors, this work also provides a tool for RABV gene targeting in infected cell lines and thus may contribute to the further investigation of RABV-host-cell-interactions.
The social context plays a decisive role in the formation of the academic self-concept (ASC) and has been widely studied as the big-fish-little-pond-effect (BFLPE). This effect describes that comparable talented students in high-achieving school settings have a lower ASC compared to equally talented students attending low-achieving settings. Past research has focused on students’ domain-specific ASC, while little is known about the relation of achievement-related classroom compositions and the various facets of ASC. Additionally, BFLPE-research has been critiqued to build its theoretical frame on social comparison theory, without providing sufficient empirical support. To address this gap, we analyzed how the single student’s social, criterial, absolute, and individual ASC relate to class-level achievement of 8th graders. Applying Multilevel Structural Equation Modeling (MLSEM) we found that all facets of ASC were significantly related to average-class achievement, while student’s social ASC revealed the strongest associated. The results reveal explicitly that average-class achievement is strongly related to social comparison processes.
Objectives: We aimed to update the 2010 evidence- and consensus-based national clinical guideline on the diagnosis and management of uncomplicated urinary tract infections (UTIs) in adult patients. Materials and Methods: An interdisciplinary group consisting of 17 representatives of 12 medical societies and a patient representative was formed. Systematic literature searches were conducted in MEDLINE, EMBASE, and the Cochrane Library to identify literature published in 2010–2015. Results: We provide 75 recommendations and 68 statements in the updated evidence- and consensus-based national clinical guideline. The diagnostics part covers practical recommendations on cystitis and pyelonephritis for each defined patient group. Clinical examinations, as well as laboratory testing and microbiological pathogen assessment, are addressed. Conclusion: In accordance with the global antibiotic stewardship initiative and considering new insights in scientific research, we updated our German clinical UTI guideline to promote a responsible antibiotic use and to give clear hands-on recommendations for the diagnosis and management of UTIs in adults in Germany for healthcare providers and patients.

This second publication focuses on treatment of acute episodes of cystitis and pyelonephritis as well as on prophylaxis of recurrent UTIs. Materials and Methods: An interdisciplinary group consisting of 17 representatives of 12 medical societies and a patient representative was formed. Systematic literature searches were conducted in MEDLINE, EMBASE, and the Cochrane Library to identify literature published in 2010–2015. Results: For the treatment of acute uncomplicated cystitis (AUC), fosfomycin-trometamol, nitrofurantoin, nitroxoline, pivmecillinam, and trimethoprim (depending on the local rate of resistance) are all equally recommended. Cotrimoxazole, fluoroquinolones, and cephalosporins are not recommended as antibiotics of first choice, for concern of an unfavorable impact on the microbiome. Mild to moderate uncomplicated pyelonephritis should be treated with oral cefpodoxime, ceftibuten, ciprofloxacin, or levofloxacin. For AUC with mild to moderate symptoms, instead of antibiotics symptomatic treatment alone may be considered depending on patient preference after discussing adverse events and outcomes. Primarily non-antibiotic options are recommended for prophylaxis of recurrent urinary tract infection. Conclusion: In accordance with the global antibiotic stewardship initiative and considering new insights in scientific research, we updated our German clinical UTI guideline to promote a responsible antibiotic use and to give clear hands-on recommendations for the diagnosis and management of UTIs in adults in Germany for healthcare providers and patients.
Abstract
Identifying materials with an efficient spin-to-charge conversion is crucial for future spintronic applications. In this respect, the spin Hall effect is a central mechanism as it allows for the interconversion of spin and charge currents. Spintronic material research aims at maximizing its efficiency, quantified by the spin Hall angle and the spin-current relaxation length . We develop an all-optical contact-free method with large sample throughput that allows us to extract and . Employing terahertz spectroscopy and an analytical model, magnetic metallic heterostructures involving Pt, W and Cu80Ir20 are characterized in terms of their optical and spintronic properties. The validity of our analytical model is confirmed by the good agreement with literature DC values. For the samples considered here, we find indications that the interface plays a minor role for the spin-current transmission. Our findings establish terahertz emission spectroscopy as a reliable tool complementing the spintronics workbench.
The rapid anthropogenic climate change that is projected for the 21st century is predicted to have severe impacts on ecosystems and on the provision of ecosystem services. With respect to the longevity of trees, forestry in particular has to adapt now to future climate change. This requires profound multidisciplinary knowledge on the direct and indirect climate sensitivity of forest ecosystems on various spatial scales. Predictions on growth declines due to increasing drought exposition during climate change are widely recognized for European beech (Fagus sylvatica L.), which is the major forest tree in European temperate deciduous forests. However, research from other continents or other biomes has shown that winter climate change may also affect forest growth dynamics due to declining snow cover and increased soil cooling. So far, this winter cold sensitivity is largely unexplored in Europe. Thus, particularly focussing on forest growth dynamics and winter cold sensitivity, the goal of this PhD-project was to explore how climate sensitivity of forest ecosystems differs regionally. By doing so, the project aimed to deliver insights about possibilities and limits of upscaling regional knowledge to a global understanding of climate sensitivity. To achieve these goals, this PhD-project integrated five studies (Manuscripts 1–5) that investigated the climate sensitivity of biogeochemical cycles, plant species composition in forests, and forest growth dynamics across spatial scales. In particular, a large-scale gradient-design field experiment simulated the influence of winter climate change on forest ecosystems by snow cover and soil temperature manipulations (Manuscript 1). This study indicated that soil cooling and decreased root nutrient uptake may indirectly reduce growth of adult forest trees. Moreover, this study indicated uniform ecological sensitivity to soil temperature changes across sites along a large winter temperature gradient (ΔT = 4 K across 500 km), irrespective of the site-specific history of snow cover conditions, which motivates upscaling from local winter climate change studies to the regional scale. Although regional climate drives growth of adult forest trees, local factors, such as site-specific edaphic conditions, might control plants in the forest understory. This assumption was tested by mapping the forest understory composition along the same winter temperature gradient as introduced above (Manuscript 2). Across sites, this study found that edaphic conditions explained the spatial turnover in the forest understory composition more than climate, which might moderate direct climate change impacts on the forest understory composition. However, edaphic conditions, forest structure, and climate are linked by triangular interactions. Thus, climate change might still indirectly affect the forest vegetation dynamics. Moreover, a dendroecological study focussed on the same winter temperature gradient from central to cold-marginal beech populations as above in order to identify gradual changes in summer drought and winter cold sensitivity in tree growth (Manuscript 3). Towards the cold distribution margin, the influence of drought on tree growth gradually decreased, while growth reductions were increasingly related to winter cold due to harsher winter climate. By a large-scale dendroecological network study assessed the relationship of growth dynamics to climate and reproductive effort in beech forests across Europe (Manuscript 4). Indeed, this study found the general pattern across the distribution range of beech that high temperature controlled growth indirectly via resource allocation to reproduction. However, the strong, direct drought signal that could be generally detected from dry-marginal to central populations vanished towards the cold-marginal populations, where the more focussed study of Manuscript 3 identified a stronger relationship of tree growth to winter cold. Further extending the scope of this PhD-thesis to global scales, litter decomposition rates were assessed across biomes (Manuscript 5). This study found a robust relationship between climate and decomposition rates, but it also demonstrated large within-biome variability on a local scale. These local scale differences might depend on habitat conditions that, in turn, could be modulated by climate change, which calls for a better exploration of indirect climate sensitivity. In conclusion, this PhD-thesis highlighted that multidisciplinary research can advance the understanding of ecological interactions in forest ecosystems under changing climate scenarios. In this PhD-project, a winter climate change experiment, where site-representative target trees were selected by means of dendroecology, contributed to a mechanistic understanding of winter cold sensitivity in forest growth dynamics. Dendroecological investigations then put the findings in a broader temporal and spatial context by describing local climate sensitivity of tree growth on different spatial scales. This thesis further shows that global generalizations about the relationship of climate and ecological processes in ecosystem models have to be critically reviewed for the need of local and regional adjustment because these processes might experience considerable regional- or local-scale variation. However, this thesis reports uniform sensitivity of ecological processes to altered winter soil temperature regimes across a large winter temperature gradient. Thus, upscaling from insights of previous winter climate change experiments to regional scales is encouraged.
Streptococcus pneumoniae is a commensal of the human upper respiratory tract and
the etiological agent of several life-threatening diseases. This pathogen is the model bacterium
for natural competence. Furthermore, the pneumococci played an important role in the
identification of DNA as the main molecule involved in bacterial transformation. As a result,
studies on the pneumococcal genome provided an initial overview of the genetic potential of
this pathogen. The pneumococcus is a highly versatile bacterium possessing a high rate of
uptake and recombination of exogenous DNA from neighboring bacteria. As such, a significant
diversity in the genome content among the different pneumococcal strains has been reported.
The capsular polysaccharide, an important pneumococcal virulence factor, is the best example
on the pneumococcal diversity. There are over 98 serotypes characterized to date presenting
differences in their capsule (cps) locus. Additional to the cps locus, the pneumococcus also
presents 13 genomic islets annotated as regions of diversity (RD) encoded in the auxiliary
genome. Remarkably, 8 of the pneumococcal RD studied so far have been associated with
virulence. Furthermore, the ongoing sequencing of over 4000 pneumococcal genomes have
shed light on the conservation level of well-known pneumococcal virulence factors.
Interestingly, important pneumococcal virulence determinants show variations in the gene and
protein sequence among the different strains. Prototypes are for example the pneumococcal
surface protein C (PspC) and pneumococcal adherence and virulence factor B (PavB).
Conversely, gene regulation in S. pneumoniae is carried out by highly conserved and genome-
wide distributed transcriptional factors. Overall, the pneumococci interplays with its
environment with 4 major regulatory systems: quorum sensing (QS), stand-alone
transcriptional regulators, small RNAs (sRNAs) and two-component regulatory systems (TCS).
Some of these systems are multifaceted and share more than one feature. Furthermore, there
is crosstalk among the different systems, requiring the activation of a signaling cascade to
function properly.
A comprehensive analysis of the distribution and conservation of pneumococcal
virulence factors and TCS was obtained in this study. The results are summarized as a
simplified variome in which 25 pneumococcal strains with a complete sequenced genome were
analyzed. Interestingly, the genes encoding the glycolytic protein enolase and the toxin
pneumolysin were the most conserved virulence determinants. Additionally, the high level of
conservation was confirmed for the pneumococcal TCS regulators, especially for WalKR,
CiaRH and TCS08.
The main focus of this study was on the regulatory functions of pneumococcal TCS.
With this in mind, an extensive and detailed systematic review of the 13 pneumococcal TCS
and its orphan RR was undertaken. For this purpose, every pneumococcal TCS was analyzed
for its reported functional and structural information along with its contribution to the main
pathophysiology of the pneumococci. In brief, S. pneumoniae can utilize its TCS for the
regulation of important cellular processes and the sensing of detectable signals in the
environment. Additionally, the role of TCS in pneumococcal processes and signal sensing can
be divided further. In the first place, pneumococcal TCS regulate competence and fratricide,
the production of bacteriocins and host-pathogen interaction processes, while the detectable
signals include cell-wall perturbations, environmental stress, and nutrients. As a conclusion
from this section, it is possible to analyze the pneumococcal TCS in a comprehensive manner.
There is a complex network among the different pneumococcal regulators and the TCS play
an important role. Moreover, these systems are highly conserved and essential for the proper
functioning of the pneumococcus as a pathogen.
Following up on pneumococcal TCS, this study focused especially on the TCS08.
Interestingly, the pneumococcal TCS08 has been previously associated with the regulation of the cellobiose metabolism. Furthermore, this system has also been reported to regulate the
expression of genes encoded in the RD4 (Pilus-1). Remarkably, the pneumococcal TCS08
was shown to be highly homologous to the SaeRS system of Staphylococcus aureus. Initially,
mutant strains lacking a single (Δrr08 or Δhk08) or both components (Δtcs08) of the TCS08
were generated in pneumococcal D39 and TIGR4 strains. Transcriptomics and functional
assays showed a downregulation of the PI-1 in the absence of the complete tcs08, while PavB
presented an upregulation in the Δhk08 knockout. Moreover, an important number of genes
coding for intermediary metabolism proteins were also found to be differentially expressed by
microarray analysis. As such, the TIGR4Δhk08 strain presented a downregulation for the
cellobiose operon (cel). In contrast, an upregulation was reported for the fatty acid biosynthesis
(fab) and arginine catabolism (arc) operons. Conversely, a decrease in gene expression was
seen in the TIGR4Δrr08 strain for the arc operon. Finally, in vivo murine pneumonia and sepsis
models highlighted an involvement of TCS08 in pneumococcal virulence. Remarkably, the
different TCS08 mutants presented a strain dependent effect on their virulence severity. The
TIGR4Δrr08, and all TCS08 mutants in D39 showed a decrease in virulence in the pneumonia
model, with no changes in sepsis. Conversely, the absence of HK08 in TIGR4 presented a
highly virulent phenotype in both pneumonia and sepsis models. To sum up, the pneumococcal
TCS08 influenced the expression of genes involved in fitness and colonization. Specifically,
those coding for the adhesins PavB and PI-1 and fitness proteins from the cel, arc and fab
operons. Remarkably, the highest changes in expression were observed in the strains lacking
the HK08. Additionally, TCS08 has a strain dependent impact on pneumococcal virulence as
showed by murine pneumonia and sepsis models when comparing the effects in D39 and
TIGR4.
This work study a monolayer of branched poly(ethyleneimine (PEI) adsorbed onto oppositely charged surfaces with iron chelates or iron ions in the absorption solution. The conformation of adsorbed PEI is explored in the dependence of the composition of the adsorption solution by measuring the surface forces using atomic force microscopy (AFM) with the colloidal probe (CP) at different ionic strengths (INaCl) in surrounding aqueous solution. The surface coverage of these layers is investigated using X-ray reflectivity.
PEI solutions show different pH values with iron chelates (pH = 3), iron ions (pH = 4.67) or pure water (pH = 9.3) at room temperature. Low surface coverage of PEI at pH = 3 adjusted by monovalent ions was also observed. However, adsorbing PEI with iron ions or iron chelates and washing with pure water shifts the pH, leading to an adsorbed PEI layer with high coverage. In our observation, the influence of iron ions and iron chelates on the surface coverage of PEI film is stronger than the pH effect. PEI adsorbed from a pure water solution shows flat conformation. Surface force measurements with CP show that PEI adsorbed from solutions containing iron chelates or iron ions cause almost identical steric forces. The thickness of the brush L is determined as a function of the ionic INaCl in the measuring solution. It scales as a polyelectrolyte brush.
The maximum number density of gold nanoparticles (AuNPs) adsorbed onto the PEI brushes was identical and larger than on flatly adsorbed PEI. On the PEI layer with the larger surface coverage, the AuNPs aggregate; on the PEI layer with the lower surface coverage they do not aggregate. Taken together, these results contribute to understanding the mechanisms determining surface coverage and conformation of PEI and demonstrate the possibility of controlling surface properties, which is highly desirable for potential future applications.
In this thesis, we also investigate the top layer (PSS and PDADMA) of polyelectrolyte multilayer (PEM) films. PEM films were prepared by sequential adsorption of oppositely charged PEs on solid substrates. PEM films consist of polydiallyldimethylammonium (PDADMA) as polycation and the polystyrene sulfonate (PSS) as polyanion. PDADMA has a smaller linear charge density than PSS. For this system, two different growth regimes are known: parabolic and linear. I studied the top layer (PSS and PDADMA) conformation of PEM films and how the structure of this top layer is affected by increasing the number of PDADMA/PSS layer pairs N and the addition of salt to the surrounding solution.
The INaCl was changed during the force-distance measurements. PSS terminated films always show electrostatic forces at INaCl < 0.1 M and flat conformation. The surface charge density is always negative at INaCl < 0.1 M. The surface charge of the PSS top layer starts to turn from negative to positive at N ≥ 14. At N between 13 and 15, adsorbed PSS cannot compensate all the excess PDADMA charge. This leads to an accumulation of the positive extrinsic sites within the PSS terminated film beyond a specific N. At INaCl ≈ 0.1 M, an exponential decaying force was measured. This is an indication of unusual long-ranged hydration force (decay length λ-1 ≈ 0.2-0.5 nm), and PSS terminated film shows zwitterionic or neutral surface. At INaCl > 0.1 M, a non-electrostatic action occurs and the PSS terminated film reswells in solution.
PDADMA terminated surface consisting of few layers show a flat conformation and the electrostatic forces were measured. For N ≥ 9 and INaCl ≤ 0.1 M, steric forces were measured. The force-distance profiles are well-explained by Alexander and de Gennes theory. PDADMA chains show a maximum L that is around 40-45 % of the contour length. For INaCl ≈ 0.1 M, and N > 9, a flat, neutral or zwitterionic surface is found (λ-1 ≈ 0.3-0.9 nm). For N = 9 and INaCl > 0.1 M, a strong screening of electrostatic interaction and attractive forces are observed. For N > 9 and INaCl > 0.1 M, the ion adsorption into the PE chains leads to an increase in the monomer size and as a result, the L increases and PDADMA brushes reswell again into the solution.
These data show that by varying N and INaCl, different surface forces can be obtained: Electrostatic forces (flat chains) both positive and negative, steric forces (brush), hydration force (flat, neutral or zwitterionic surface), and effects not yet explained (reswelling brush).
Zusammenfassung
Die Wundheilung stellt einen komplexen und sensiblen Prozess dar, wobei neben Keratinozyten auch besonders Immunzellen eine wesentliche Rolle spielen. Besonders in der Medizin ist die Behandlung von Wunden eine zentrale Aufgabe und erfordert immer mehr Techniken, um diesen Prozess effizienter ablaufen zu lassen. Seit einigen Jahren steht die Behandlung von Wunden mittels Niedertemperatur Plasma immer mehr im Fokus der Wissenschaft. Dabei besteht Plasma aus zahlreichen Komponenten, wobei jede Komponente die Zelle unterschiedlich beeinflussen kann. Da auch jede Wunde einzigartig im Hinblick auf Beschaffenheit und Erregerspektrum ist, sollte dies auch mit einer individuellen Abstimmung der Plasamkomponenten einhergehen um so die Wundheilung noch effizienter zu gestallten. Ziel dieser Arbeit ist die Modifizierung der Plasma-Komponenten durch einen Gasmantel, um herauszufinden, welche Modifizierung am effektivsten auf Immunzellen und somit auch auf die Wundheilung wirkt.Dafür wurde in der vorliegenden Arbeit die Auswirkungen von fünf Gaszumischungen im Mantelgas eines argonbetriebenen kalten Atmosphärendruckplasmajets (kINPen 11) auf die Generierung von reaktiven Spezies in Flüssigkeiten und auf Monozyten in vitro untersucht. Der kINPen (ein Gerät der Firma neoplas,zur Erzeugung von Niedertemperatur-Plasma) war dabei auf 5slm(Standardliter pro Minute Argon) und Burstmodus eingestellt. Die Gaszusammensetzung variiert von 100% Stickstoff (N2) und 0% Sauerstoff (O2) in 25íger Schritten zu 100% O2 und 0% N2. Dabei entstehen abhängig von der O2- und N2-Zufuhr im Gasmantel unterschiedliche reaktive Spezies. Als repräsentativ für die Bildung von reaktiven Sauerstoffspezies (ROS) wird Wasserstoffperoxid (H2O2) und für reaktiven Stickstoffspezies (RNS) werden Nitrit (NO2–) und Nitrat (NO3–) erfasst. Die Experimente wurden nach Flüssigkeitsanalytik und zellphysiologischen Aspekten gegliedert. Es wurde die THP-1-Monozytenzelllinie verwendet. Zu den Methoden der Flüssigkeitsanalytik gehören die Bestimmung des pH-Werts, die Detektion der NO2–-, NO3–- und H2O2-Konzentration. Zellphysiologisch wurde die Zellviabillität,die Apoptoserate,der Wachstumsfaktor HB-EGF,sowie die Zytokinsekretion detektiert. Zudem wurde die direkte und indirekte Plasma-Behandlung verglichen.Bei derdirekten Plasma-Behandlung stehen die Zellen mit dem Plasmaeffluenten im direkten, kontinuierlich Kontakt, wohingegen bei der indirekten Behandlung nur das Zellkulturmedium behandelt wird und anschließend auf die Zellen gelangt. Die Zellen haben keinen Kontakt mit dem Plasmaeffluenten. Zu der Flüssigkeitsanalytik gehört die gezielte Messung der durch das Plasma entstehenden ROS und RNS in phosphatgepufferte Salzlösung (PBS) und Zellkulturmedium (RPMI-Medium, Roswell Park Memorial Institute). Es konnten keine pH-Wert-Veränderungen durch die Plasma-Behandlung gemessen werden, aber mit Zunahme der Behandlungszeit stieg die Menge an reaktiven Spezies. Der Gasmantel hat besonders Auswirkungen auf den Gehalt der Flüssigkeiten an reaktiven Spezies. Die niedrigsten Nitrat-Anion (NO3–)- und Nitrit-Anion (NO2–)-Konzentrationen ergaben sich bei der 100% und 0%N2-Mantelgaszumischung. Die75%N2-Mantelgaszumischung zeigte die höchste NO3–-Konzentration in PBS. Die H2O2-Konzentration nahm mit dem Anteil an O2 im Mantelgas zu und erreicht ihr Maximum bei Mantelgaszumischung gezeigt werden. Alle anderen Mantelgaszumischungen zeigten zwar bei der Flüssigkeitsanalytik deutliche Unterschiede, doch durch die Anwesenheit von THP-1-Zellen wurde der Einfluss deutlich geringer. Die Gaszumischungen hatten trotz unterschiedlicher Bildung von reaktiven Stickstoff- und Sauerstoffspezies keinen großen Effekt auf die Zelltoxizität. Die 50%-N2-Mantelgaszumischung bewirkte die höchsten Konzentrationen an reaktive Sauerstoff- und Stickstoffspezies (RONS), was mit einer erhöhten frühen und späten Apoptose einherging und mit niedrigen IL-8-Werten. Die 100% N2-Mantelgaszumischung zeigte die größten Effekte auf die Zytokinsekretion. Die IL-6-Konzentration sank sowohl bei den direkt als auch indirekt mit Plasma behandelten und mit Lipopolysaccharid (LPS)-stimulierten Proben. Gleichzeitig wurde für die unstimulierte, indirekte Plasma-Behandlung ein deutlicher Anstieg der IL-8-Konzentration gemessen. Neben den erwähnten Ergebnissen konnten weitere zelluläre Effekte unabhängig von den Mantelgaszumischungen gemessen werden. Zum Einen zeigten sich deutliche Unterschiede zwischen der direkten und indirekten Plasma-Behandlung. Die indirekte Plasma-Behandlung,d.h.diePlasma-Behandlung von Zellkulturmedium und die anschließende Inkubation der Zellen, zeigte eine starke Erhöhung der IL-8-Konzentration, die mit der N2-Konzentration im Mantelgas anstieg. Auch für die IL-6- und IL-8-Konzentrationen nach LPS-Stimulation zeigten sich Unterschiede zwischen der direkten und indirekten Behandlung. Die indirekte Plasma-Behandlung zeigte eine stark reduzierte Zellviabilität gegenüber der direkten Plasma-Behandlung (1 min), die eher aktivierend wirkte. Zum Zweiten zeigte sich nach der Plasma-Behandlung, dass die Zellen im Durchflusszytometer eine Erhöhung des Volumens und der Granularität nach langen Behandlungszeiten aufwiesen und scheinbar erhöhte Zellviabilität. Eine Detektion des Wachstumsfaktors HB-EGF mit Hilfe der FACS-Analyse konnte nicht gezeigt werden. Drittens, konnte durch Zugabe von Katalase ein Anstieg der NO2–-Konzentration im RPMI im Vergleich zur Behandlung ohne Katalase gezeigt werden. Somit spiegelt die Flüssigkeitsanalytik nicht die Stituation in Gegenwart von Zellen in vitro wieder. Erstmalig konnte in dieser Arbeit gezeigt werden, dass die Modifizierung der Umgebungsgase bei der Plasma-Behandlung einen Einfluss auf die Bildung von ROS, RNS und Zytokinen hat. Besonders die 50% N2-Mantelgaszumischung hat mit der Reduktion der IL-8-Konzentration und damit der Reduktion eines für die Angionese relevanten Faktors möglicherweise einen negativen Effekt auf die Wundheilung. Im Gegensatz dazu bewirkt die 100% N2-Mantelgaszumischung durch Abnahme von IL-6 eine Verringerung der Entzündungsmediatoren und durch die Erhöhung von IL-8 eine Zunahme eines für die Angiogenese in Wunden wichtigen Parameters. Gleichzeitig konnte gezeigt werden, dass der Einfluss durch die Behandlungszeit und der direkten und indirekten Behandlung einen weitaus größeren Effekt auf Monozyten haben könnte als bisher angenommen. Somit leistet diese Arbeit einen zusätzlichen Beitrag für das weitere Verständnis bei der Aufklärung der zellulären Effekte durch die Plasma-Behandlung. Es kamen aber auch neue Fragen auf, die noch nicht hinreichend geklärt wurden: Erstens, welcher Mechanismus steht hinter der Zunahme der Granularität und des Volumens nach langen Plasma-Behandlungen. Zweitens, in wie weit sind die ohne Katalase-Behandlung gemessenen NO2–-Konzentrationen aussagekräftig. Drittens, haben die indirekte und direkte Plasma-Behandlung den gleichen Effekt in vivo wie in vitro. Um diese Fragen weiter zu klären ist weitere Forschung nötig.
Durch die Promotionsschrift werden die Auswirkungen der verschiedenen, von gravierenden gesellschaftlichen, politischen und wirtschaftlichen Wandlungsprozessen gekennzeichneten geschichtlichen und kunstgeschichtlichen Zeitabschnitten auf den städtebaulichen Entwicklungsprozess und die Architektur der mecklenburgischen Mittelstadt Güstrow im Vergleich zu der etwas kleineren Stadt Parchim dargestellt.
Die Arbeit ist in vier Hauptkapitel gegliedert: die Phase von den sog. Gründerjahren bis zum Ende des I. Weltkriegs (1871-1918), die Zeit der Weimarer Republik (1919-1933), die Phase der Herrschaft des Nationalsozialismus (1933-1945) und daran anschließend die Veränderungen der Bautätigkeit während der sowjetischen Besatzungszeit ab 1945 sowie in der Zeitspanne von der Gründung der DDR bis zur Wiedervereinigung (1949-1990). Ein letzter Blick auf das Baugeschehen bis in die unmittelbare Vergangenheit und die Gegenwart (1990-2015/16) schließt sich an.
Die Binnenstruktur jedes Hauptkapitels umfasst nach einer knappen Einleitung zu den jeweiligen geschichtlichen Umständen die stadtplanerischen internationalen und nationalen Tendenzen in diesem Zeitabschnitt. In weiteren Teilkapiteln werden Entwicklungen der beiden vorgestellten Städte im Verkehrssystem, den industriellen Strukturen und Versorgungseinrichtungen dargestellt, bevor auf kommunale und militärische Bauten, sowie dann speziell auf die Entwicklung im Wohnungs- und Siedlungsbau der genannten Städte eingegangen wird.
Planungsvorhaben und Baurealisierungen in den einzelnen Quartieren der Mittelstädte werden betrachtet, exemplarisch verhandelt und einer vergleichenden Analyse unterzogen. Anhänge mit Abbildungen, Landkarten, Stadtplänen, Bau- und Planzeichnungen sowie Fotos tragen zu einer greifbaren und transparenten komplexen Darstellung bei.
Prägnante Gebäude beider Städte sowie realisierte Typenbauten werden genau beschrieben.
Ein Resümee beantwortet die Frage nach Gemeinsamkeiten und Spezifika in der stadtplanerischen und architektonischen Entwicklung der Mittelstädte Mecklenburgs und beweist die Erkennbarkeit eines charakteristischen Schemas.
Auf der Grundlage der populationsbezogenen Daten der Study of Health in Pomerania wurden in dieser Studie a) die Häufigkeit einer dynamischen Überblähung und b) prädiktive Faktoren für das Auftreten dieses Phänomens analysiert.
Es wurden Daten von 6753 Probanden (3510 weiblich) aus 2 Kohorten der Study of Health in Pomerania (SHIP-2 und SHIP-TREND) untersucht.
Eine dynamische Überblähung wurde mit Hilfe der IC-Manöver, welche im Rahmen der Spiroergometrie durchgeführt wurden, als Konstanz oder Anstieg des EELV unter Belastung definiert.
Analysiert wurden die Ergebnisse anhand von 2148 Probanden (1052 weiblich) mit einem vollständigen kardiopulmonalen Untersuchungskomplexes ohne gröbere Fehler, mindestens 3 IC-Manövern der Spiroergometrie und einer Variabilität des EELV von weniger als 200 ml.
Aus dieser Gesamtgruppe wurde durch Ausschluss von aktiven Rauchern und Probanden mit kardiopulmonalen Vorerkrankungen und Vormedikation eine gesunde Studienpopulation extrahiert. Mit Hilfe einer detaillierten Qualitätsanalyse ergab sich aus der Gesamtpopulation zusätzlich eine Studienpopulation mit fehlerfreien dynamischen Fluss-Volumen-Kurven.
Alle Ergebnisse wurden als Absolut- und Relativwerte angegeben. Die Relativwerte wurden anhand der vorgestellten Normwertformeln berechnet.
Bei einem mittleren Alter von 54 und 52 Jahren und einem mittleren BMI von 27 kg/m2 wiesen die untersuchten Studienpopulationen lungenfunktionelle Werte im Normbereich auf. Zudem war eine normale Belastbarkeit bei normwertiger peakVO2 und VO2@AT gegeben.
Die Datenlage zu Häufigkeiten und Prädiktoren der dynamischen Überblähung war bisher noch gering. So konnten wir mit Hilfe unserer Studie zeigen, dass eine dynamische Überblähung bei Probanden einer epidemiologischen Studie mit einer Häufigkeit von bis zu 23 % vorkommt. Zusätzlich war es möglich, die Prävalenz der dynamischen Überblähung über alle Altersgruppen getrennt für Männer und Frauen anzugeben.
Signifikante Unterschiede zwischen den Gruppen der Probanden mit und ohne dynamische Überblähung zeigten sich in allen drei Studienpopulationen hinsichtlich Alter (Jahren), BMI (kg/m2) und Größe (cm) sowie bezüglich der spirometrischen Parameter FEV (l), PEF (l/s) und MEF 75 (l/s) und der bodyplethysmografischen Parameter Rtot (kPa*s/l und %pred) und ITGV (l).
Weitere signifikante Unterschiede waren in den spiroergometrischen Parametern peakVO2 (ml/min), Vtin_max (l) und peakVt (l und %pred) zu verzeichnen.
Als bester Prädiktor einer dynamischen Überblähung erwies sich in unserer Studie der Atemwegswiderstand Rtot in %pred, gefolgt von Alter, Größe und Gewicht.
Unsere Studie stellt die Bedeutung der dynamischen Fluss-Volumen-Kurven als ein wichtiges diagnostisches Verfahren im Rahmen der Spiroergometrie heraus und zeigt Möglichkeiten einer qualitativen Bewertung dieser Untersuchungsmethode auf. Sie ist zudem eine der ersten Studien, welche die lungenfunktionellen und spiroergometrischen Parameter hinsichtlich ihrer prädiktiven Potenz zur Feststellung einer dynamischen Überblähung untersuchte.
Um die Bedeutung für den klinischen Alltag herauszuarbeiten, werden weitere Studien notwendig sein.
Growth corridors have been an instrument of
economic development for decades but have gained new
attention in regional economic development policies in recent years, e.g., in Sub-Saharan Africa or Southeast Asia.
They are seen by policy makers and private businesses as
catalysts of regional economic integration, pushing traditional businesses into increasingly complex international
value chains. However, the outcomes of such development
initiatives are still barely understood. Critics argue that development policies are based on simplified models that are
unable to sufficiently address the complexity of regional
development. Policies on value-chain development, for
example, can lead to conflicts, external dependencies,
land rush, and a polarization of wealth. Growth corridors
often go hand-in-hand with socio-economic transformations and land-use conflicts. This paper first discusses the
theoretically possible desired and undesired regional socio-economic effects of modern corridors. Second, we illustrate the potential and challenges to realize integrative
(or inclusive) development by contrasting three growth
corridors: the SAGCOT growth corridor in Tanzania, the
Walvis Bay-Ndola-Lubumbashi Development Corridor
(WBNLDC) in Namibia, Zambia and Zimbabwe, and the
growth corridors in the Greater Mekong Subregion (GMS)
Die differentialdiagnostische Einordnung der Polyneuropathie (PNP), insbesondere die Frage nach dem Vorliegen einer inflammatorisch bedingten PNP, die prinzipiell einer immunmodulatorischen Therapie zugängig ist, kann im Alltag eine große Herausforderung darstellen. Neben der Standarddiagnostik mit klinischer, laborchemischer und
elektrophysiologischer Untersuchung hat sich im Laufe der letzten Jahre die Ultraschalldiagnostik peripherer Nerven als zusätzliches Diagnostikum etabliert. In den zu Beginn der Datenerhebung der vorliegenden Arbeit publizierten Studien zur Sonographie peripherer Nerven bei PNP konnte gezeigt werden, dass besonders inflammatorisch bedingte demyelinisierende PNP zu einer Vergrößerung der Nervenquerschnittsfläche (CSA) der Nerven der oberen Extremität führen. Die Daten zur Veränderung der CSA der Nerven der unteren Extremität, die in Anbetracht des häufigen Auftretens längenabhängiger PNP mit klinischer Manifestation an den Beinen besonders praxisrelevant ist, sind jedoch bislang spärlich. In der vorliegenden Fall-Kontroll-Studie sollte daher untersucht werden, ob die CSA des N. suralis bei Patienten mit PNP größer ist als bei Gesunden und ob Zusammenhänge zwischen der CSA des N. suralis und dem Schädigungsmuster, der Ätiologie und der Neurographie bestehen. Untersucht wurden insgesamt 36 Patienten mit PNP unterschiedlicher Ätiologien und 18 gesunde Probanden. Von den 36 Patienten wiesen 25 Patienten ein axonales und 11 Patienten ein demyelinisierendes Schädigungsmuster auf. Die sonographische Messung der CSA des N. suralis erfolgte an 3 zuvor definierten Punkten (Anastomose des N. cutaneus surae medialis und des N. cutaneus surae lateralis, Beginn der Sehne des M. triceps surae und im Bereich des Retinaculum musculorum peroneorum) an beiden Beinen. Um den Einfluss möglicher Messfehler gering zu halten, wurden für die weiteren Analysen die mittlere CSA (Mean CSA) als arithmetisches Mittel aus den 6 Messwerten sowie die Mean of the Max der CSA (MMax der CSA) als Mittelwert aus dem jeweiligen Maximalwert der CSA des rechten und linken N. suralis verwendet. Durch wiederholte Untersuchung 5 weiterer Probanden im Abstand von 24 Stunden konnte eine sehr gute Reproduzierbarkeit der Mean CSA und der MMax der CSA sichergestellt werden(Intraclass- Korrelationskoeffizient jeweils 0,98). Die wichtigsten Ergebnisse der Studie sind, dass der N. suralis bei Patienten mit inflammatorischer PNP eine größere CSA aufweist als bei Patienten mit CIAP und Vitamin- Mangel- Polyneuropathien (2,2 mm2 vs. 1,7 mm2, p=0,03) und dass die Patienten mit einem demyelinisierendem Schädigungsmuster ebenfalls eine höhere CSA des N. suralis aufweisen als jene mit einem axonalen Schädigungsmuster. Diese Ergebnisse stehen im Einklang mit der aktuellen Literatur, sodass die sonographische Untersuchung des N. suralis als einziger rein sensibler Nerv und Vertreter der kleinen Nerven der unteren Extremität heute einen wichtigen Bestandteil von standardisierten). Untersuchungsprotokollen (beispielsweise Ultrasound pattern sum score, UPSS)darstellt.Ein signifikanter Unterschied zwischen der der CSA des N. suralis bei PNP-Patienten und Gesunden besteht nicht. Weiterhin konnte wie auch an anderen peripheren Nerven zuvor schon gezeigt, auch für den N. suralis eine große Streuung der CSA des N. suralis bei Patienten mit diabetischer PNP nachgewiesen werden, sodass kein signifikanter Unterschied zu PNP anderer Ätiologien oder zu Gesunden besteht. Stärken der vorliegenden Arbeit sind die hohe Standardisierung der Untersuchungen sowie insbesondere die Vielfalt der verglichenen Ätiologien. Limitierend ist die nicht gewährleistete Verblindung der Untersucherin bezüglich des Status des zu Untersuchenden als Patient oder Proband und der der Erkrankung zugrunde liegenden Ätiologie. Insgesamt ist damit nicht nur die Sonographie der peripheren Nerven der oberen Extremität sondern auch des N. suralis ein sinnvolles Add-On zur Standarddiagnostik und erleichtert die Differenzierung zwischen inflammatorischen PNP und längenabhängigen axonalen PNP. Die Sonographie ist leicht zu erlernen, bietet eine Darstellung in Echtzeit, ist kostengünstig und im Gegensatz zu anderen bildgebenden Verfahren wie der Magnetresonanztomographie gibt es keine Kontraindikationen für die Untersuchung. Gegenstand zukünftiger Studien sollten die Heterogenität morphologischer Veränderungen der Nerven bei diabetischen PNP sowie der Einsatz der Sonographie bei seltenen Kompressionssyndromen des N. suralis sein.
In this thesis, size-sensitive phenomena of three-dimensional dust crystals emerged in a low temperature plasma are presented. Depending on the number of particles in the system phase transitions, collective vortex motions and large-scaled expansions can be observed. To investigate these fascinating effects an advanced experimental setup as well as new evaluation methods have been developed. This thesis will present these new techniques and the gained insights.
Significance of Hyperbaric Oxygenation in the Treatment of Fournier’s Gangrene: A Comparative Study

Introduction: Hyperbaric oxygenation (HBO), in addition to anti-infective and surgical therapy, seems to be a key treatment point for Fournier’s gangrene. The aim of this study was to investigate the influence of HBO therapy on the outcome and prognosis of Fournier’s gangrene. Patients and Methods: In the present multicenter, retrospective observational study, we evaluated the data of approximately 62 patients diagnosed with Fournier’s gangrene between 2007 and 2017. For comparison, 2 groups were distinguished: patients without HBO therapy (group A, n = 45) and patients with HBO therapy (group B, n = 17). The analysis included sex, age, comorbidities, clinical symptoms, laboratory and microbiological data, debridement frequency, wound dressing, antibiotic use, outcome and prognosis. The statistical analysis was performed with GraphPad Prism 7® (GraphPad Software, Inc., La Jolla, USA). Results: Demographic data showed no significant differences. The laboratory parameters C-reactive protein and urea were significantly higher in group B (group B: 301.7 vs. 140.6 mg/dL; group A: 124.8 vs. 54.7 mg/dL). Sepsis criteria were fulfilled in 77.8 and 100% of the patients in groups A and B respectively. Treatment in the intensive care unit (ICU) was therefore indicated in 69% of the patients in group A and 100% of the patients in group B. The mean ICU stay was 9 and 32 days for patients in groups A and B respectively. The wound debridement frequency and hospitalization stay were significantly greater in group B (13 vs. 5 debridement and 40 vs. 22 days). Initial antibiosis was test validated in 80% of the patients in group A and 76.5% of the patients in group B. Mortality was 0% in group B and 4.4% in the group A. Conclusion: The positive influence of HBO on the treatment of Fournier’s gangrene can be estimated only from the available data. Despite poorer baseline findings with comparable risk factors, mortality was 0% in the HBO group. The analysis of a larger patient cohort is desirable to increase the significance of the results.

Human cytomegalovirus (HCMV) latency is typically harmless but reactivation can be largely detrimental to immune compromised hosts. We modeled latency and reactivation using a traceable HCMV laboratory strain expressing the Gaussia luciferase reporter gene (HCMV/GLuc) in order to interrogate the viral modulatory effects on the human adaptive immunity. Humanized mice with long-term (more than 17 weeks) steady human T and B cell immune reconstitutions were infected with HCMV/GLuc and 7 weeks later were further treated with granulocyte-colony stimulating factor (G-CSF) to induce viral reactivations. Whole body bio-luminescence imaging analyses clearly differentiated mice with latent viral infections vs. reactivations. Foci of vigorous viral reactivations were detectable in liver, lymph nodes and salivary glands. The number of viral genome copies in various tissues increased upon reactivations and were detectable in sorted human CD14+, CD169+, and CD34+ cells. Compared with non-infected controls, mice after infections and reactivations showed higher thymopoiesis, systemic expansion of Th, CTL, Treg, and Tfh cells and functional antiviral T cell responses. Latent infections promoted vast development of memory CD4+ T cells while reactivations triggered a shift toward effector T cells expressing PD-1. Further, reactivations prompted a marked development of B cells, maturation of IgG+ plasma cells, and HCMV-specific antibody responses. Multivariate statistical methods were employed using T and B cell immune phenotypic profiles obtained with cells from several tissues of individual mice. The data was used to identify combinations of markers that could predict an HCMV infection vs. reactivation status. In spleen, but not in lymph nodes, higher frequencies of effector CD4+ T cells expressing PD-1 were among the factors most suited to distinguish HCMV reactivations from infections. These results suggest a shift from a T cell dominated immune response during latent infections toward an exhausted T cell phenotype and active humoral immune response upon reactivations. In sum, this novel in vivo humanized model combined with advanced analyses highlights a dynamic system clearly specifying the immunological spatial signatures of HCMV latency and reactivations. These signatures can be merged as predictive biomarker clusters that can be applied in the clinical translation of new therapies for the control of HCMV reactivation.
Zielstellung: In dieser prospektiven Arbeit überprüften wir, ob ein sequenzbasierter Softwarealgorithmus zur Geräuschreduktion in der Magnetresonanztomographie (MRT) einen Einfluss auf die Bildqualität und das Signal-zu-Rauschverhältnis (SNR) besitzt.
Material und Methoden: Einunddreißig Patienten, 18 Männer, 13 Frauen, Durchschnittsalter 58,3 ± 14,5 Jahren, unterzogen sich einer indizierten, kranialen MRT-Bildgebung. Das Sequenzprotokoll schloss FLAIR-Sequenzen ein, welche mit identischen Scanparametern jeweils mit und ohne Anwendung des Softwarealgorithmus Quiet Suite (QS) (Siemens Healthcare) angefertigt wurden. Mit Hilfe eines Schalldruckpegelmessgerätes (Testo, Typ 815) wurden die maximalen Lautstärkepegel für beide Sequenzen im Untersuchungsraum ermittelt. Es erfolgte die subjektive Beurteilung der Bildqualität auf einer Fünf-Punkte-Skala (1 sehr gut - 5 inadäquat) durch zwei Radiologen unabhängig voneinander. Separat wurde das SNR für die graue Hirnsubstanz, weiße Hirnsubstanz und den Liquor für beide Sequenzen ermittelt. Zusätzlich erfolgte die Auswertung von Detektionsraten pathologischer Läsionen innerhalb der weißen Hirnsubstanz jeweils mit und ohne Verwendung von QS. Die statistische Analyse mit dem Vergleich beider Sequenzen erfolgte mittels Mann-Whitney-U-Test.
Ergebnisse: Unsere Ergebnisse zeigen, dass die Verwendung von QS die Schalldruckpegelspitzen signifikant reduziert (83,5 ± 7,3 dB(A) versus 81,5 ± 7,5 dB(A) mit QS, p ≤ 0,017). Hinsichtlich der Beurteilung der subjektiven Bildqualität wurden keine signifikanten Unterschiede zwischen beiden Befundern ermittelt (Befunder 1: 2,03/2,07, p = 0,730; Befunder 2: 1,98/2,10, p = 0,362). QS führte für ausgewählte Bereiche zu einem Anstieg des SNR (graue Substanz, p ≤ 0,001; weiße Substanz p = 0,006). Die Detektionsraten von suspekten Läsionen innerhalb der weißen Substanz zeigten keine signifikanten Unterschiede unter Verwendung von QS (Befunder 1, p = 0,944; Befunder 2, p = 0,952).
Schlussfolgerungen: Sequenzbasierte Softwarealgorithmen, wie QS, besitzen das Potential die Schalldruckpegelspitzen in der MRT signifikant zu reduzieren ohne die subjektive Bildqualität zu kompromittieren oder die diagnostische Genauigkeit zu gefährden.
Sphingosine-1-phosphate (S1P) regulates the migration of follicular B cells
(B2 cells) and directs the positioning of Marginal zone B cells (MZ B cells) within the spleen. The
function of S1P signalling in the third B cell lineage, B1 B cells, mainly present in the pleural and
peritoneal cavity, has not yet been determined. Methods: S1P receptor expression was analysed
in peritoneal B cells by real-time polymerase chain reaction (qPCR). The chemotactic response to
S1P was studied in vitro. The role of S1P signalling was further explored in a s1p4
−/− mouse
strain. Results: Peritoneal B cells expressed considerable amounts of the S1P receptors 1 and 4
(S1P1 and S1P4, respectively). S1P1 showed differential expression between the distinct peritoneal B
cell lineages. While B2 cells showed no chemotactic response to S1P, B1 B cells showed a migration
response to S1P. s1p4
−/− mice displayed significant alterations in the composition of peritoneal
B cell populations, as well as a significant reduction of mucosal immunoglobulin A (IgA) in the
gut. Discussion: S1P signalling influences peritoneal B1 B cell migration. S1P4 deficiency alters the
composition of peritoneal B cell populations and reduces secretory IgA levels. These findings suggest
that S1P signalling may be a target to modulate B cell function in inflammatory intestinal pathologies.
Quantitative reconstructions of past vegetation cover commonly require pollen productivity estimates (PPEs). PPEs are calibrated in extensive and rather cumbersome surface-sample studies, and are so far only available for selected regions. Moreover, it may be questioned whether present-day pollen-landcover relationships are valid for palaeo-situations. We here introduce the ROPES approach that simultaneously derives PPEs and mean plant abundances from single pollen records. ROPES requires pollen counts and pollen accumulation rates (PARs, grains cm−2 year−1). Pollen counts are used to reconstruct plant abundances following the REVEALS approach. The principle of ROPES is that changes in plant abundance are linearly represented in observed PAR values. For example, if the PAR of pine doubles, so should the REVEALS reconstructed abundance of pine. Consequently, if a REVEALS reconstruction is “correct” (i.e., “correct” PPEs are used) the ratio “PAR over REVEALS” is constant for each taxon along all samples of a record. With incorrect PPEs, the ratio will instead vary. ROPES starts from random (likely incorrect) PPEs, but then adjusts them using an optimization algorithm with the aim to minimize variation in the “PAR over REVEALS” ratio across the record. ROPES thus simultaneously calculates mean plant abundances and PPEs. We illustrate the approach with test applications on nine synthetic pollen records. The results show that good performance of ROPES requires data sets with high underlying variation, many samples and low noise in the PAR data. ROPES can deliver first landcover reconstructions in regions for which PPEs are not yet available. The PPEs provided by ROPES may then allow for further REVEALS-based reconstructions. Similarly, ROPES can provide insight in pollen productivity during distinct periods of the past such as the Lateglacial. We see a potential to study spatial and temporal variation in pollen productivity for example in relation to site parameters, climate and land use. It may even be possible to detect expansion of non-pollen producing areas in a landscape. Overall, ROPES will help produce more accurate landcover reconstructions and expand reconstructions into new study regions and non-analog situations of the past. ROPES is available within the R package DISQOVER.
Action comprehension that is related to language or gestural integration has been shown to engage the motor system in the brain, thus providing preliminary evidence for the gestural-verbal embodiment concept. Based on the involvement of the sensorimotor cortex (M1) in language processing, we aimed to further explore its role in the cognitive embodiment necessary for gestural-verbal integration. As such, we applied anodal (excitatory) and sham transcranial direct current stimulation (tDCS) over the left M1 (with reference electrode over the contralateral supraorbital region) during a gestural-verbal integration task where subjects had to make a decision about the semantic congruency of the gesture (prime) and the word (target). We used a cross-over within-subject design in young subjects. Attentional load and simple reaction time (RT) tasks served as control conditions, applied during stimulation (order of three tasks was counterbalanced). Our results showed that anodal (atDCS) compared to sham tDCS (stDCS) reduced RTs in the gestural-verbal integration task, specifically for incongruent pairs of gestures and verbal expressions, with no effect on control task performance. Our findings provide evidence for the involvement of the sensorimotor system in gestural-verbal integration performance. Further, our results suggest that functional modulation induced by sensorimotor tDCS may be specific to gestural-verbal integration. Future studies should now evaluate the modulatory effect of tDCS on semantic congruency by using tDCS over additional brain regions and include assessments of neural connectivity.
Humanity is constantly confronted with the emergence and reemergence of infectious diseases. Many of them produce large or devastating epidemics, like AIDS (HIV) and Ebola. Others have been long neglected, yet pose immediate threats to global public health as evidences the abrupt emergence of Zika virus in South America and its association with microcephaly in babies. The examples illustrate, that many of these diseases are provoked by RNA viruses. One of the first steps in understanding and eliminating those threats is the development of sensitive and rapid diagnostic methods. A general and relatively rapid method is the direct detection and examination of the agent’s genome. However, the nature of (re)emerging RNA viruses poses a series of very specific problems for the design of such methods. Therefore, a systematic approach was proposed for the design of DNA-hybridization-base methods to detect and characterize RNA viruses that will have both a high sensitivity and a specificity sufficiently broad to detect, per reaction, down to a single copy of any of the possible variants of the viral genome.
Following this approach a series of assays were designed, developed or adapted and put into use for detection and characterization of important RNA viruses. One of those viruses is West Nile virus (WNV), which after its explosive introduction into USA become the most widespread flavivirus throughout the world and, consequently, many countries began an intensive monitoring. While existing assay detected predominantly the Lineage 1, in Europa Lineage 2 was expected. Two new RT-qPCR for the detection of both lineages were developed, and reportedly used by independent laboratories. Due to more than 50000 associated deaths per year, the Hepatitis E virus also received an increasing attention to elucidate novel routes of transmission. This virus (especially genotype 3) has the zoonotic potential of transmission from pigs and wild boar to humans. RT-qPCR and nested qPCR for detection and characterization of this virus as well as a methodology for subtyping were developed and the first detected case of subtype 3b in a German wild animal was documented. In addition a novel assay for flaviviruses conformed by a RT-qPCR coupled with a low density DNA microarray was developed, which enabled the identification of WNV in mosquitoes from Greece. A RT-qPCR suitable for surveillance and diagnostic of all known variants of Venezuelan equine encephalitis virus was developed too. A causative agent of hemorrhagic infections, the Ngari virus, was detected and characterized in animal samples from Mauritania. These achievements were supported by the development of software applications for selection and visualization of primers and probes from aligned DNA sequences and for modeling of DNA hybridizations using unaligned sequences.
In conclusion a general methodology for rapid development of sensitive diagnostic methods based in DNA-hybridization technics (PCR, sequencing and microarray) was stablished and successful applications are reported.
Der Zahnverlust, überwiegend bedingt durch Karies und Parodontitis, ist eine der häufigsten Erkrankungen unserer Gesellschaft. Herausnehmbarer Zahnersatz spielt eine große Rolle bei der Wiederherstellung von Kau- und Sprechfunktion, sowie der Ästhetik, daher ist das Ziel dieser Studie, die klinischen Parameter einer rein konuskronengetragenen oder kombiniert konuskronen- und klammergetragenen Prothese zu untersuchen und Folgeleistungen darzustellen. Weiterhin wurde die Zufriedenheit der Patienten mit diesem Zahnersatz in Abhängigkeit von der Art des Zahnersatzes überprüft.
Von April 2011 bis Juni 2012 wurden 24 Frauen und 25 Männer mit einem Durchschnittsalter von 68,9 Jahren untersucht. 43 Ober- und 37 Unterkieferprothesen befanden sich im Mittel 5,1 Jahre in situ. Es wurden 158 Konuskronen und 38 Klammerzähne klinisch untersucht. Dabei wurden Eckzähne und Prämolaren vor allem zur Konuskronenverankerung und Molaren häufig zur Klammerretention verwendet.
Es konnte gezeigt werden, dass im Unterkiefer durchschnittlich weniger Zähne (2,97) zur Retention der Prothesen benötigt werden als im Oberkiefer (3,29). Die Sondierungstiefen waren mesial durchschnittlich am höchsten. Rund 72 % der Pfeilerzähne wiesen einen Lockerungsgrad von 0 – 1 auf. Ein kariöser Defekt im Randbereich der Innenkoni trat selten auf (1,1 %).
Am zufriedensten waren die Teilnehmer mit der Ästhetik und dem Sprechen mit den Prothesen. Am schlechtesten schnitten die Prothesen beim sicheren Halt bzw. der Lagestabilität ab.
In den verfügbaren Patientenakten wurde am häufigsten die Druckstellenentfernung dokumentiert, gefolgt von den Rezementierungen der Primärkronen und der Friktionsverminderung.
Im Durchschnitt wurden bei den rein konuskronengetragenen Prothesen Kosten von insgesamt 344,97 € fällig (Eigenanteil (EA) der Patienten: 112,85 €; Krankenkassen-Anteil (KK): 218,32 €). Die kombiniert konuskronen- und klammergetragenen Konstruktionen waren bei der Instandhaltung etwas teurer: insgesamt 386,21 € (EA: 138,64 €; KK: 282,57 €).
Diese Arbeit kann als Orientierung zur Planung von prothetischen Behandlungsmaßnahmen und zur Aufklärung der Patienten dienen. Es kann abgeschätzt werden, welche Nachsorgeleistungen für den Zahnarzt und ggf. für das Dentallabor, sowie Folgekosten für den Patienten entstehen könnten. Die Analyse der Zufriedenheit mit dem doppelkronengetragenem Zahnersatz in Bezug auf Lage, Stabilität und Ästhetik könnte für Zahnärzte ebenfalls zur Argumentation bei der Beratung zu einer Neuversorgung mittels konuskronengetragenem Zahnersatz hinzugezogen werden.
Weiterhin ist diese Arbeit Grundlage für weiter klinische Studien zur Abschätzung klinischer Parameter in Bezug auf den prothetischen Erhaltungsaufwand und die Patientenzufriedenheit von anderen prothetischen Versorgungskonzepten Zahnersatz und deren Folgekosten.
Submerged macrophytes play a key role in north temperate shallow lakes by stabilizing clear-water conditions. Eutrophication has resulted in macrophyte loss and shifts to turbid conditions in many lakes. Considerable efforts have been devoted to shallow lake restoration in many countries, but long-term success depends on a stable recovery of submerged macrophytes. However, recovery patterns vary widely and remain to be fully understood. We hypothesize that reduced external nutrient loading leads to an intermediate recovery state with clear spring and turbid summer conditions similar to the pattern described for eutrophication. In contrast, lake internal restoration measures can result in transient clear-water conditions both in spring and summer and reversals to turbid conditions. Furthermore, we hypothesize that these contrasting restoration measures result in different macrophyte species composition, with added implications for seasonal dynamics due to differences in plant traits. To test these hypotheses, we analyzed data on water quality and submerged macrophytes from 49 north temperate shallow lakes that were in a turbid state and subjected to restoration measures. To study the dynamics of macrophytes during nutrient load reduction, we adapted the ecosystem model PCLake. Our survey and model simulations revealed the existence of an intermediate recovery state upon reduced external nutrient loading, characterized by spring clear-water phases and turbid summers, whereas internal lake restoration measures often resulted in clear-water conditions in spring and summer with returns to turbid conditions after some years. External and internal lake restoration measures resulted in different macrophyte communities. The intermediate recovery state following reduced nutrient loading is characterized by a few macrophyte species (mainly pondweeds) that can resist wave action allowing survival in shallow areas, germinate early in spring, have energy-rich vegetative propagules facilitating rapid initial growth and that can complete their life cycle by early summer. Later in the growing season these plants are, according to our simulations, outcompeted by periphyton, leading to late-summer phytoplankton blooms. Internal lake restoration measures often coincide with a rapid but transient colonization by hornworts, waterweeds or charophytes. Stable clear-water conditions and a diverse macrophyte flora only occurred decades after external nutrient load reduction or when measures were combined.

Einige Arten von Verständnisproblemen und Scheindissensen können überwunden oder verhindert werden, indem solche Ausdrücke zur Klärung herangezogen oder bevorzugt braucht werden.
So können etwa die Rezipienten von missverständlichen Äußerungen höherstellige Prädikate verwenden, um klärende Interpretationen zu erstellen. Autoren von Äußerungen können ihrerseits höherstellige Prädikate gebrauchen, um das Risiko von Verständnisproblemen zu minimieren.
Es werden philosophische Methoden entwickelt, die auf dieser gebrauchssprachlichen Praxis des Einsatzes höherstelliger Prädikate aufbauen. Dazu gehören die Interpretation durch Stelligkeitserhöhung, mit der verständnisbefördernde Paraphrasen erstellt werden können, und Methoden der Stelligkeitsrevision, durch die inkonsistente Sprachen in konsistente überführt werden können.
Das Cochlea-Implantat (CI)-System stellt eine etablierte Versorgungsmöglichkeit einer Taubheit oder an Taubheit grenzenden Schwerhörigkeit dar, die in vielen Fällen den Betroffenen ein gutes bis sehr gutes Sprachverstehen ermöglicht. Jedoch wird in einigen Fällen das erwünschte Ergebnis einer CI-Versorgung nur zum Teil oder nicht erreicht. Außerdem kann sich im Verlauf einer CI-Versorgung ein anfänglich guter Hörgewinn trotz anscheinend regelrechter Funktion des Implantats verschlechtern. In diesen Fällen kann eine retrocochleäre Störung vorliegen, deren Nachweis allein mit bildgebenden Verfahren schwierig ist. Die Kombination von elektrisch evozierten Summenaktionspotentialen (ECAP) und elektrisch evozierten Hirnstammpotentialen (EAEP) kann den Nachweis einer retrocochleären Störung bei Patienten mit CI verbessern. Ziel der vorliegenden Studie war es, Referenzwerte für elektrisch evozierte frühe Hirnstammpotentiale (ECAP und EBERA) von CI-Trägern zu ermitteln, deren Rehabilitationsverlauf ein durchschnittliches Versorgungsergebnis erwarten ließ.
An 19 Probanden wurden insgesamt 20 Messreihen durchgeführt. Dabei wurden an diesen Probanden, unter Verwendung eines optimierten, klick-äquivalenten Stimulus, elektrisch evozierte Potentiale gemessen. Die verwendeten Stimulations- und Messparameter wurden ausführlich erläutert. Auch die Durchführung der Messreihen wurde umfassend beschrieben.
Die Ergebnisse aller Messreihen zur Erstellung der Referenzwerte wurden dargestellt und analysiert. Bei der Darstellung und Diskussion wurde besonders auf die Dynamik, die Amplituden, die Absolut-Latenzen und Latenz-Differenzen der gemessenen Potentiale eingegangen. Die Referenzwerte wurden für ipsilaterale und kontralaterale Ableitung in Wertetabellen dargestellt. Besonders die von der Stimulationsintensität abhängigen Absolut-Latenzen der Potentiale eJ3 und eJ5 zeigten unterschiedliche Ausprägungen. Im Anschluss wurden fünf EAEP-Messungen aus der klinischen Routine mit jeweils unterschiedlichen Fragestellungen vorgestellt und gegen die Referenzwerte verglichen.
Im letzten Abschnitt wurden sowohl die Referenzwerte als auch die Messungen aus der klinischen Routine diskutiert. Aus der Dynamik der gemessenen ECAP konnte eine Durchführungsempfehlung für EBERA-Messungen an Patienten, die die subjektiv empfundenen Lautheiten der Stimulation nicht eindeutig angeben können, abgeleitet werden. Abschließend wurden die Grenzen der Methode diskutiert und Ausblicke auf zukünftige Forschungsvorhaben gegeben.
Die vorliegende Studie befasste sich mit der Morphologie des Canalis nasolacrimalis (CN) von Japanischen Makaken (Macaca fuscata) basierend auf der Untersuchung koronaler computertomographischer Schichtaufnahmen. Im ersten Teil der Studie wurde das geschlechtsspezifische Wachstum des CN anhand von 29 Schädeln in unterschiedlichen postnatalen Altersgruppen analysiert. Der zweite Teil befasste sich mit dem Einfluss geographischer und klimatischer Faktoren auf die Morphologie des Tränenkanals adulter Japanischer Makaken, die definierten lokalen Populationen entstammen. Im dritten Teil der Arbeit wurde schließlich die Morphologie des CN von Macaca fuscata mit der anderer Spezies der Faszikularis-Gruppe der Makaken (Macaca cyclopis, Macaca fascicularis, Macaca mulatta) verglichen.
Zur Darstellung der Morphologie des CN wurden anhand der koronalen CT-Schichtaufnahmen mit Hilfe der WinSurf Version 4.0® - Software 3D-Rekonstruktionen des CN angefertigt und die entsprechenden Volumina ermittelt. Darüber hinaus wurde die Länge des CN sowie der Winkel zwischen der Achse des CN und der Horizontalebene mit dem Programm OsiriX® Version 5.8.2 vermessen. Die ermittelten Maße des CN wurden mit externen linearen Schädelmaßen in Beziehung gesetzt, um den Einfluss der Schädelmorphologie auf den CN zu untersuchen.
Männliche und weibliche Japanische Makaken wiesen zahlreiche Übereinstimmungen in der Morphologie des CN auf. So erstreckte sich der CN vom orbitalen Eingang nach kaudal und lateral. Dabei befand sich der orbitale Eingang weiter medial als die Mündungsstelle in der Nasenhöhle. Erst ab der Altersgruppe 2 erschien der Sinus maxillaris gleichzeitig auf einem Schichtbild mit dem CN. Die Maße des CN vergrößerten sich mit zunehmendem Alter, wobei das Volumen des CN adulter männlicher Tiere größer war als das der weiblichen Tiere. Bis auf den linken CN adulter Tiere wiesen weibliche Individuen in allen anderen Altersgruppen einen längeren CN auf als männliche Individuen. Die größte Volumen- und Längenzunahme zeigten sich bei Männchen zwischen der Altersgruppe 3 und 4, bei Weibchen zwischen der Altersgruppe 1 und 2. Die Achse des CN war bei infantilen Tieren nahezu senkrecht zur Horizontalebene ausgerichtet und verlagerte sich mit zunehmenden Alter schräg nach vorne.
Für die Volumen-, Längen- und Winkelmessungen des CN konnten signifikante geschlechtsspezifische Unterschiede beobachtet werden. Jedoch waren die Unterschiede nicht einheitlich und betrafen nicht alle Altersgruppen. Die vorliegende Studie zeigte für Macaca fuscata einen statistisch signifikanten Zusammenhang zwischen dem Wachstum des CN und externen Schädelmaßen. Darüber hinaus zeigten die Vergleiche der Anstiege der Regressionsgeraden zwischen beiden Geschlechtern keine signifikanten Unterschiede. Somit ist anzunehmen, dass sich die Tränennasenkanäle beider Geschlechter mit einem ähnlichen Wachstumsmuster vergrößern.
Der Einfluss klimatischer und geographischer Faktoren auf den CN wurde an 37 Schädeln adulter Japanischer Makaken beiderlei Geschlechts von fünf definierten geographischen Regionen Japans untersucht. Dazu wurden die ermittelten Maße des CN in Relation zu Klimadaten gesetzt. Für das Volumen ergaben sich keine statistisch signifikanten Unterschiede im Populationsvergleich. Jedoch unterschieden sich die Messwerte der lokalen Gruppen hinsichtlich der Länge und der Achsenneigung des CN signifikant voneinander. Zudem konnten für den linken CN signifikante Beziehungen zur geographischen Lage und der kleinsten Januartemperatur herausgearbeitet werden. Da dieser Zusammenhang nur für eine Seite beobachtet werden konnte, besteht weiterer Untersuchungsbedarf.
Schließlich wurden die vier Spezies der Faszikularis-Gruppe bezüglich der Morphologie des CN anhand von 34 adulten Schädeln beiderlei Geschlechts untersucht. Statistisch signifikante Unterschiede ergaben sich für das Volumen und die Länge des CN. Während für den CN von Macaca fuscata bezüglich Volumen und Länge die größten Messwerte bestanden, zeigte Macaca mulatta die kleinsten Messwerte. Hinsichtlich der Ausrichtung des CN zur Horizontalebene bestanden keine Speziesunterschiede.
Die Ergebnisse dieser Untersuchung beschreiben nicht nur den Tränennasengang innerhalb einer Wachstumsreihe von Macaca fuscata, sondern liefern erstmals Erkenntnisse über die Morphologie des CN bei verschiedenen geographischen Populationen der Rotgesichtsmakaken sowie der Faszkularis-Gruppe. Angesichts der bekannten Unterschiede zwischen Catarrhini und Platyrrhini bezüglich der Morphologie der Nasenhöhle sind weiterführende Untersuchungen notwendig um die Faktoren, welche die Morphologie des Tränennasengangs beeinflussen, weiter zu ergründen.
Molybdopterin spielt in der Natur eine wesentliche Rolle, da es gebunden an Molybdän den Molybdän-Cofaktor bildet, der einer Reihe verschiedener Enzyme als katalytisches Zentrum dient. Der Molybdän-Cofaktor kann zwar aus dem Protein freigesetzt werden, erweist sich dann aber als instabil. Trotz langjähriger Bemühungen konnten der Molybdän-Cofaktor und seine biologischen Vorstufen bisher nicht auf chemischem Wege synthetisiert werden. Daher konnten die bisher gewonnenen Kenntnisse über diese Verbindung nicht anhand von Untersuchungen an dem freien Cofaktor gewonnen werden. Um den Cofaktor in seiner Chemie zu verstehen, beschäftigt sich diese Arbeit mit der chemischen Synthese von Modellverbindungen, die die Aufgaben des natürlichen Cofaktors nachbilden können. Um den Einfluss der verschiedenen Struktureinheiten auf die Stabilität oder die katalytische Aktivität zu verstehen und so ein tieferes Verständnis über Molybdopterin und mögliche Struktur-/Funktionsbeziehungen des natürlichen Cofaktors zu entwickeln, werden einzelne Strukturabschnitte untersucht. Im Rahmen dieser Arbeit war der Fokus das Verständnis der Chemie des Pyrazin-Pyran-Dithiolen-Strukturabschnittes und nach Möglichkeit die Entwicklung alternativer Modellverbindungen, die in der Lage sind Sauerstoff-Transport-Reaktionen zu katalysieren und/oder mit dem Apoenzym verbunden werden können. Im besten Falle kann so eine Modellverbindung als Behandlungsmöglichkeit der Molybdän-Cofaktor-Defizienz eingesetzt werden, bei Bindung mit dem Apoenzym für iSOD (isolierte Sulfitoxidase-Defizienz) oder bei Nichtbindung für MoCo-Defizienz Typ B. Für die Synthese der Pyrazin-Pyran-Dithiolen-Liganden sollten bereits literaturbekannte Syntheserouten insbesondere von Garner modifiziert und optimiert werden. Vergleichsweise sollten auch Ligandensysteme mit einer CH2-Gruppe anstelle der Sauerstofffunktion des Pyrans synthetisiert werden. Des Weiteren sollten neue Synthesewege zu strukturell und elektronisch ähnlichen Verbindungen entwickelt werden. Die so gewonnenen Ligandensysteme sollten anschließend mit vorzugsweise Molybdän, aber auch Wolfram komplexiert werden.
The highly oncogenic alphaherpesvirus Marek’s disease virus (MDV) causes immense economic losses in the poultry industry. The main targets of in vivo MDV infection are primary B and T lymphocytes. The cytolytic infection of B cells leads to depletion of lymphoid cells results in severe immunosuppression. Infected B cells recruit and activate T cells. The close interaction between B cells and T cells enables efficient intercellular transfer of MDV. During infection of T cells, the virus enters a latent state. Infection of T cells can lead to transformation of these cells and formation of lymphoma, which manifest in various visceral organs. This study aimed at the characterization of the proteomes of MDV-infected lymphocytes during the lytic and latent phases of infection.
Previous in vitro studies concerning the MDV pathogenesis and host-virus interactions have been mainly conducted with primary fibroblasts or kidney cells, due to the short lifespan of primary lymphocytes in cell culture. Recently, a cultivation system has been established that extents the lifespan of primary lymphocytes through the addition of cytokines to the growth medium. This allowed the infection of B cells in vitro and to conduct quantitative proteomic analysis of primary lymphocytes. Infection with GFP labelled virus recombinants allowed the isolation of infected cells by FACS for the proteome analysis of MDV infected B lymphocytes. An efficient quantitative proteomic workflow was developed, which consisted of a filter-aided (FASP) digest of the extracted proteins, followed by differential dimethyl chemical labeling of the peptides for quantitative evaluation prior to LC-MALDI TOF/TOF mass spectrometry. Only few alterations of the protein and transcript expression profiles were observed after infection of primary B cells with the very virulent RB-1B and the live-attenuated vaccine strain CVI988/Rispens. Relevant changes in relative protein levels were found for only twelve and six interesting host proteins after RB1B and CVI988 infection, respectively. However, the regulations were confirmed by inspection of the spectra from all experiments. The identified candidates play a role in immune response, translation and inflammatory response.
To confirm the potential infection markers, RNA-seq analysis of three biological replicates of each RB-1B -, CVI988- and mock-infected B cells was performed. Eighty expressed MDV transcripts could be identified, which were associated with lytic infection. The same MDV proteins were identified after infection with RB-1B or CVI988. However, transcriptome and proteome analysis of MDV-infected primary B cells showed only poor correlation. This indicates that the changes in protein expression profiles are mostly due to posttranscriptional events. Infection marker candidates were identified by the RNA-seq analysis, for which the gene expression was altered by MDV infection. Although almost 12,000 transcripts were identified, only few transcript levels changed markedly after MDV infection. The biological processes immune response, apoptotic process, signal transduction, cell migration and response to virus were enriched after MDV infection. The RNA-seq results confirm the observation that alterations of protein levels early after MDV infection are rare.
Most notably, MDV induces transformation of lymphocytes leading to malignant T-cell lymphomas in visceral organs with mortalities of up to 100 %. While several factors involved in MDV tumorigenesis have been identified, the transformation process is not fully understood. Therefore, we set out to fill this knowledge gap using proteome analysis of transformed T-cells ex vivo. In addition, the role of the viral telomerase RNA during transformation was assessed by comparison of tumors that had formed after infection with WT-virus or a telomerase RNA negative mutant. A major obstacle for tumor proteome analyses is the preparation of sufficient amounts of homogenous tumor tissue, as tumors appear with a dispersed morphology in the affected organs. The quantitation of cell types within the tumors indicated varying portions of hepatocytes, connective tissue, and CD3+ lymphocytes even with the same virus strain in different animals. However, the ∆vTR-induced tumors contained lower levels of hepatocytes and higher levels of CD3+ lymphocytes compared to WT tumors in all tested tumor samples. Thus, ∆vTR tumors were chosen for determination of differences in protein expression profiles of tumors and naïve T cells for their lower content of liver cells. We developed a workflow for the proteome analysis of T cell tumors from livers of MDV-infected chickens. Samples included laser capture micro-dissected tissue cuts from tumors and surrounding healthy liver tissue as well as naïve T-cells prepared from thymus. To enable quantitative proteome analysis, samples were digested using the FASP protocol and peptides were isotope-coded by differential dimethyl labeling. To improve proteome analysis peptides were fractionated by preparative isoelectric focusing prior to nano-HPLC MALDI/TOF-TOF mass- spectrometric analysis.
Proteomic analyses of LCM dissected ΔvTR tumor compared to naïve T cells, the main targets of transformation, identified nineteen potential transformation markers but again only minor changes in relative levels were observed. Several of the identified markers could also be verified by RT-qPCR on transcript level. The identified transformation candidates were associated with nucleosome assembly, regulation of transcription, inflammatory response, immune response and oxidation-reduction process.
However, further functional analyses are necessary to fully elucidate the role of the identified markers during MDV infection and transformation.
Background/Aim: Laparoscopic single-port surgery has emerged as a growing trend in minimally invasive surgery. Single-port access is preferred among women undergoing gynecologic surgery who have cosmetic concerns about scarring. Furthermore, this approach results in comparable clinical outcomes to standard laparoscopic surgery and perioperative morbidity rates have been reported to be low. The hypothesis is that a single-port technique might offer such advantages over the standard multi-port laparoscopy as less postoperative pain and better cosmetic results by decreasing abdominal wall tissue trauma. The potential disadvantages of single-port approaches are the larger umbilical incision and the technical difficulties. There are only a few randomized studies in the literature that investigate the value and safety of single-incision laparoscopic surgery in gynecological surgery. The aim of this study was to compare the safety and quality of life in patients who undergo single-incision laparoscopic assisted vaginal hysterectomy and those who undergo conventional laparoscopic assisted vaginal hysterectomy.
Methods: In a prospective randomized trial, 64 patients from three different centers in Germany were randomized (1:1) to conventional laparoscopic assisted vaginal hysterectomy (n=32) or single-incision laparoscopic assisted vaginal hysterectomy (n=30). Data was collected on 60 patients who fulfilled the criteria.
Results: The baseline characteristics of patients were similar in both groups. The mean operative time was comparable in both groups (68.2 vs 73.6 min., p = 0.409). Within the two groups, no differences were seen regarding estimated blood loss (p = 0.915), intra- and postoperative complications (p = 0.944), and wound infection rates (p = 0.944). Patients within the single-incision laparoscopic surgery group experienced significantly less pain in the first 24 hours postoperatively (p = 0.006), while pain scores at days 3, 5, 7 and 2 months postoperatively were comparable.
Conclusion: This study demonstrates that single-incision laparoscopic assisted vaginal hysterectomy is a reliable and safe setup in gynecologic surgery. Compared to conventional laparoscopic assisted vaginal hysterectomy, Notably, patients undergoing single-incision laparoscopic assisted vaginal hysterectomy experienced less pain postoperatively.
Production-Integrated Compensation in Environmental Offsets—A Review of a German Offset Practice

Background: Biomarkers for gains of evidence based interventions for upper limb motor training in the subacute stage following stroke have rarely been described. Information about these parameters might help to identify patients who benefit from specific interventions and to determine individually expected behavioral gains for a certain period of therapy.
Objective: To evaluate predictors for hand motor outcome after arm ability training in the subacute stage after stroke selected from known potentially relevant parameters (initial motor strength, structural integrity of the pyramidal tract and functional motor cortex integrity).
Methods: We applied the arm ability training (AAT) over 3 weeks to a subpopulation of stroke patients with mild arm paresis, i.e., in 14 patients on average 4 weeks after stroke. The following biomarkers were measured before therapy onset: grip strength on the affected hand, transcranial magnetic stimulation recruitment curve steepness over the primary motor hand area [slope ratio between the ipsilesional hemisphere (IH) and contralesional hemisphere (CH)], and diffusion weighted MRI fractional anisotropy (FA) in the posterior limb of the internal capsule (PLIC; determined as a lateralization index between IH and CH). Outcome was assessed as the AATgain (percentage improvement over training). The “Test d'Evaluation des Membres Supérieurs de Personnes Âgées” (TEMPA) was assessed before and after training to test for possible associations of AAT with activity of daily living.
Results: A stepwise linear regression identified the lateralization index of PLIC FA as the only significant predictor for AAT-gain (R2 = 0.519; P = 0.029). AAT-gain was positively associated (r = 0.59; P = 0.028) with improvement in arm function during daily activities (TEMPA).
Conclusions: While all mildly affected patients achieved a clinically relevant therapeutic effect, pyramidal tract integrity nevertheless had a modifying role for clinical benefit.
Dendrochronology, the science of tree-rings is a tool which has been widely used for many years for understanding changes in the environment, as trees react to environmental changes over time. In the contemporary situation, where climate warming in the Arctic is unequivocal and its effects on the Alpine and tundra ecosystems are seen pronouncedly in the past decade, the role of dendro-studies and the use of trees and shrubs alike as proxies of change has become critical. Studies clearly indicate that warming in the Arctic and Alpine tundra has resulted in increased vegetation in recent years. Shrubs, in these sensitive ecosystems, have proven to be highly instrumental as they likely benefit from this warming and hence are good indicators and auditees of this change. Therefore, in this study, we investigate the potential of shrubs in the evolving field of dendro-ecology/climatology.
Studies from classical dendrochronology used annual rings from trees. Further, because of shrub sensitivity to contemporary change, shrub-based dendrochronological research has increased at a notable scale in the last decade and will likely continue. This is because shrubs grow even beyond the tree line and promise environmental records from areas where tree growth is very limited or absent. However, a common limitation noted by most shrub studies is the very hard cross-dating due to asynchronous growth patterns. This limitation poses a major hurdle in shrub-based dendrochronological studies, as it renders weak detection of common signals in growth patterns in population stands. This common signal is traced by using a ‘site-chronology’.
In this dissertation, I studied shrub growth through various resolutions, starting from understanding radial growth within individuals along the length of the stem, to comparison of radial growth responses among male and female shrubs, to comparing growth responses among trees and shrubs to investigation of biome-wide functional trait responses to current warming. Apart from Chapter 4 and Chapter 6, I largely used Juniperus communis sp. for investigations as it is the most widely distributed woody dioecious species often used in dendro-ecological investigations in the Northern Hemisphere.
Primarily, we investigated radial growth patterns within shrubs to better understand growth within individuals by comparing different stem-disks from different stem heights within individuals. We found significant differences in radial growth from different stem-disks with respect to stem heights from same individuals. Furthermore, we found that these differences depending on the choice of the stem-disk affect the resulting site-chronology and hence climate-sensitivity to a substantial extent and that the choice of a stem-disk is a crucial precursor which affects climate-growth relationships.
Secondly, we investigated if gender difference – often reported causing differential radial growth in dioecious trees – is an influential factor for heterogeneous growth. We found that at least in case of Juniperus communis. L and Juniperus communis ssp nana. WILLD there is no substantial gender biased difference in radial growth which might affect the site-chronology. We did find moderate differences between sexes in an overall analysis and attribute this to reproductive effort in females.
In our study to test the potential of shrubs for reconstruction, we used a test case of Alnus viridis ssp crispa. We found a strong correlation between ring-width indices and summer temperature. Initially, the model failed the stability tests when we tested the stability of this relation using a response function model. However, using wood-anatomical analysis we discovered that this was because of abnormal cell-wall formation resulting in very thin rings in the year 2004. Pointer year analysis revealed that the thin rings were caused because of a moth larval outbreak and when corrected for these rings the model passed all stability tests.
Furthermore, to see if trees and shrubs growing in same biomes react to environmental changes similarly, a network analysis with sites ranging from the Mediterranean biome to the Ural Mountains in Russia was carried out. We found that shrubs react better to the current climate warming and have a decoupled divergent temperature response as compared to coexisting trees. This outcome reiterated the importance of shrub studies in relation to contemporary climate change. Even though trees and shrubs are woody forms producing annual rings, they have very different growth patterns and need different methods for analysis and data treatment.
Finally, in a domain-wide network analysis from plant-community vegetation survey, we investigated functional relationships between plant traits (leaf area, plant height, leaf nitrogen content, specific leaf area (SLA), and leaf dry matter content (LDMC)) and abiotic factors viz. temperature and soil moisture. We found a strong relation between summer temperature and community height, SLA and LDMC on a spatial scale. Contrarily, the temporal-analysis revealed SLA and LDMC lagged and did not respond to temperature over the last decade. We realized that there are complex interactions between intra-specific and inter-specific plant traits which differ spatially and temporally impacting Arctic ecosystems in terms of carbon turn over, surface albedo, water balance and heat-energy fluxes. We found that ecosystem functions in the Arctic are closely linked with plant height and will be indicative of warming in the short term future becoming key factors in modelling ecosystem projections.
Die Bedeutung des (postoperativen) Schmerzes sowie dessen adäquate Behandlung nimmt im klinischen Alltag eine zentrale Stellung ein.
Zur Verbesserung der Schmerztherapie wurde im Juli 2009 das Akutschmerzzertifizierungsprogramm „Qualitätsmanagement Akutschmerz“ in der Universitätsmedizin Greifswald eingeführt. Die Intention dieser Maßnahme war, mit Hilfe der Implementierung der S3-Leitlinie zur „Behandlung akuter perioperativer und post- traumatischer Schmerzen“ das Schmerzmanagement zu optimieren. Dies sollte durch folgende Grundpfeiler erreicht werden: Durch die Zunahme der interdisziplinären Zusammenarbeit seitens aller am Schmerzmanagement beteiligten Fachrichtungen, der Einführung von standardisierten verfahrensspezifischen Behandlungsalgorithmen mit definierten Interventionsgrenzen, einer umfangreichen Schmerzanamnese, -erfassung und -dokumentation sowie der Weiterbildung der am Schmerzmanagement beteiligten Berufsgruppen.
Ziel der vorliegenden Arbeit war es, die postoperative Schmerzsituation am ersten Tag nach einer Wirbelsäulenoperation vor und nach Einführung des „Qualitätsmanagements Akutschmerz“ in der Klinik und Poliklinik für Orthopädie und orthopädische Chirurgie der Universitätsmedizin Greifswald zu untersuchen und zu prüfen, ob eine Verbesserung des postoperativen Schmerzmanagements durch die Einführung des Zertifizierungsprogramms „Qualitätsmanagement Akutschmerz“ gelungen ist. Des Weiteren wurde analysiert, ob Faktoren wie Operationsart, Geschlecht, Rauchen, psychische Komorbiditäten und chronische Schmerzen das postoperative Schmerzempfinden nach Eingriffen an der Wirbelsäule beeinflussen.
Hierzu wurden die Daten retrospektiv von 1322 Patienten ausgewertet, die sich im Zeitraum vom 01.01.2008 bis zum 31.12.2013 in der Klinik und Poliklinik für Orthopädie und orthopädische Chirurgie der Universitätsmedizin Greifswald einer Wirbelsäulenoperation unterzogen hatten. Zur Evaluierung des postoperativen Schmerzempfindens wurde die Schmerzstärke (n=893) mittels Numerischer Ratingskala (NRS) und die Zufriedenheit mit der aktuellen Schmerzsituation (Schmerzzufriedenheit) (n=438) anhand eines 5-stufigen Skalierungssystems erhoben.
In dem untersuchten Zeitraum konnte eine kontinuierliche Verbesserung der postoperativen Schmerzstärke festgestellt werden. Die Ergebnisse bezüglich der postoperativen Schmerzzufriedenheit zeigten, abgesehen von einer einmaligen Verbesserung zwischen 2009 und 2010 zum Zeitpunkt der Einführung des „Qualitätsmanagements Akutschmerz“, eine stetige Verschlechterung der Schmerzzufriedenheit. Daraus lassen sich zwei Schlussfolgerungen ableiten:
1. Durch die Einführung des „Qualitätsmanagements Akutschmerz“ wurde eine Verbesserung des postoperativen Schmerzmanagements erreicht.
2. Die Schmerzstärke und Schmerzzufriedenheit sind nicht miteinander assoziiert.
Die Analyse der Faktoren, die möglicherweise Schmerzstärke und Schmerzzufriedenheit beeinflussen, ergab bei chronischen Schmerzpatienten eine signifikant höhere Schmerzstärke im Vergleich zu Patienten ohne chronische Schmerzen. Die Resultate der Schmerzzufriedenheit bezüglich chronischer Schmerzen zeigten keinen signifikanten Unterschied. Bei der Untersuchung der Operationsart zeigten sich signifikante Unterschiede mit der höchsten Schmerzstärke sowie geringsten Schmerzzufriedenheit bei offenen Operationen, gefolgt von perkutanen Eingriffen und kombinierten perkutanen Operationen mit Mini-open-Synovektomien. Bei den Ergebnissen hinsichtlich des Geschlechts, Rauchens sowie psychischer Komorbiditäten kristallisierten sich bei der Schmerzstärke und Schmerzzufriedenheit jeweils unterschiedliche Tendenzen heraus; es ließen sich jedoch keine signifikanten Unterschiede feststellen.
Die Ergebnisse bezüglich der Schmerzstärke im gesamten untersuchten Zeitraum und der Schmerzzufriedenheit zwischen 2009 und 2010 bestätigen die Empfehlungen der S3-Leitlinie zur „Behandlung akuter perioperativer und posttraumatischer Schmerzen“, dass eine optimale postoperative Schmerztherapie durch ihre Integration in ein multimodales perioperatives Gesamtkonzept erreicht wird. Dabei haben neben den oben genannten Grundpfeilern der Leitlinie die Integration des Patienten in das perioperative Schmerzmanagement und die Berücksichtigung dessen individueller Bedürfnisse einen hohen Stellenwert.
Die Einführung von standardisierten Behandlungsalgorithmen haben sich im klinischen Alltag bewährt. Allerdings zeigen die Ergebnisse an der Universitätsmedizin Greifswald, dass für chronische Schmerzpatienten zusätzlich eine individuell angepasste Schmerztherapie notwendig ist. Neben den positiven Ergebnissen hinsichtlich der Schmerzstärke im gesamten untersuchten Zeitraum, bedarf es zukünftig weiterer Untersuchungen, um der festgestellten abnehmenden Schmerzzufriedenheit ab 2010 entgegenzuwirken.
Postoperative Therapien, rezidivfreies Intervall und Gesamtüberleben von uterinen Adenosarkomen

Uterine Adenosarkome (AS) sind eine sehr seltene Variante der uterinen Sarkome. Sie bestehen aus einer benignen, epithelialen und einer malignen, mesenchymalen Komponente. Ein wichtiges histopathologisches Merkmal ist das sarcomatous overgrowth (SO), welches bei den für gewöhnlich low-grade Tumoren mit einer aggressiveren Verlaufsform assoziiert ist. Aktuell existieren für Deutschland praktisch keine Daten zu wirksamen Therapieregimen und zum Langzeitüberleben.
In diese Studie konnten 53 AS, eine für diese Entität hohe Fallzahl, aus der Bundesrepublik Deutschland eingebunden werden. Die Datenerhebung erfolgte gemischt retro- und prospektiv über neun Jahre. Mittels der Kaplan-Meier-Methode und des multivariaten Cox-Regression-Modells wurden mögliche Prognosefaktoren für das rezidivfreie Intervall (RFI) und das Gesamtüberleben (OS) identifiziert. Zum Vergleich der einzelnen Überlebenskurven wurde der log-Rank-Test verwendet. Das Signifikanzniveau aller Tests lag bei p=0,005.
Von den 53 beobachteten AS konnten 33 (62,3%) als AS ohne SO und 20 (37,7%) als AS mit SO (AS-SO) diagnostiziert werden. Unser medianes Follow-up beträgt 19 Monate (Spanne 1-144 Monate). Insgesamt verstarben elf Frauen (20,8%), wobei in neun Fällen das AS todesursächlich war. Es wurde ein medianes OS von 68 Monaten bzw. ein tumorspezifisches Überleben von 34 Monaten ermittelt. Das SO ist sowohl hinsichtlich des OS (p=0,195) als auch des tumorspezifischen Gesamtüberlebens (p=0,083) mit einer schlechteren Prognose assoziiert. Während das 1- und 2- Jahresüberleben beider Gruppen sehr ähnlich liegt, ist das Langzeitüberleben nach fünf Jahren für AS-SO drastisch reduziert, sodass insbesondere hier eine lange Nachbeobachtungszeit erforderlich ist.
Insgesamt entwickelten 15 Frauen (28,3%) nach median 79 Monaten (95%-KI) ein Rezidiv, welches einer Rezidivrate von 27,3% (n=9) für AS ohne SO und 30% (n=6) für AS-SO entspricht. Zudem kann näherungsweise ein negativer Einfluss für das Vorliegen eines SO auf das RFI registriert werden (p=0,098).
Eine R1/2-Resektion und eine G3-Klassifizierung sind mit einem signifikant schlechteren Outcome (RFI und OS) verbunden. Zudem zeigen AS mit einem Ki67-Proliferationsindex ≥ 30 einen Trend früher Rezidive zu entwickeln (p=0,164). Prognostisch vorteilhaft, mit längerem RFI und OS, ist das Fehlen eines Lymphknotenbefalls zu werten. Jedoch führt eine systematische Lymphonodektomie nicht zu einem Überlebensvorteil. Im Gegenteil: Die Lymphonodektomie führt sogar zu einem 5,6-fach höheren Sterberisiko als ein Unterlassen der selbigen. Eine BSO erbringt ebenfalls keinen Überlebensvorteil.
Zusammenfassend ist die Therapie der Wahl die totale Hysterektomie. Eine R0-Resektion ist bei ausgedehnten AS anzustreben. Eine Lymphonodektomie bzw. bilaterale Salpingoophorektomie hingegen erbringen keinen prognostischen Vorteil. Grundsätzlich führt die generelle Anwendung einer postoperativen Therapie zu keiner Verbesserung des RFI und OS. Bei ausgeprägtem postoperativen Therapiewunsch oder fortgeschrittenen Stadien kann am ehesten eine Radiotherapie zur Reduktion von Lokalrezidiven angewendet werden.
Sieben (n=7/15; 46,6%) Frauen mit einem Rezidiv verstarben (medianes Überleben 64 Monate), wobei auch hier das prognostisch schlechtere AS-SO mit einem kürzeren Überleben einhergeht. Prozentual wurden mehr Fernmetastasen (60,9%) gegenüber Lokalrezidiven (39,1%) registriert, wobei am häufigsten eine Fernmetastasierung in die Lunge stattfand. Die Rezidivlokalisation zeigt keinen Einfluss auf das Outcome der Frauen. Analog zur Primärtherapie ist das maximale Tumordebulking mit einer Resektion aller Tumorherde die beste Rezidivtherapie. Multimodale Therapiekonzepte, die ein maximales Tumordebulking beinhalten, erzielen ähnlich gute Ergebnisse. Chemotherapeutika waren nur in Einzelfällen wirksam und haben insgesamt den gleichen Effekt wie eine reine Beobachtung. Häufig musste eine Chemotherapie aufgrund der Toxizität und starken Nebenwirkungen, bei meist unter Therapie progredienter Krankheit, abgebrochen werden. Die längste partielle Remission konnte mit einer Kombination aus Carboplatin und Paclitaxel bei einer Patientin mit AS ohne SO für 13 Monate erreicht werden.
Zukünftig werden noch größere Studien benötigt um konsequente Therapiestrategien, insbesondere postoperative Therapien nach inadäquat erfolgter Operation, zur Verbesserung des OS und des RFI abzuleiten oder zu entwickeln. Um auch verlässlichere Aussagen bezüglich des Rezidivverhaltens und –therapie machen zu können, muss ein langes Follow-up möglichst vieler Frauen gewährleistet werden. Insbesondere sollte auch ein stadienabhängige Betrachtung der einzelnen Therapieverfahren erfolgen, um einen besseren Vergleich zu ermöglichen.

Hintergrund und Zielsetzung: Mecklenburg-Vorpommern (MV) ist eine Region, die mit landschaftlichen Vorzügen punkten kann. Allerdings gilt es auch hier – wie in Gesamtdeutschland – sich dem Fachkräftemangel in den Gesundheits- und Pflegeberufen zu stellen. Deshalb war es die Zielsetzung der Arbeit, die Gegenstandsbereiche Stimmungen, Erwartungsbilder und Zukunftspläne von jungen Erwachsenen während ihrer Ausbildungszeit im noch bestehenden Bildungsgang Gesundheits- und Krankenpflege vor dem Start der generalistischen Pflegeausbildung zu untersuchen.
Methode: Die Studie orientierte sich am Forschungsansatz der Triangulation, bei dem sowohl quantitative als auch qualitative Erhebungsmethoden verwendet werden. An der schriftlichen Befragung nahmen im Jahr 2015 78 Schüler (70,5% weiblich) im 3. Ausbildungsjahr der Beruflichen Schule an der Universitätsmedizin Greifswald und der Außenstelle Wolgast teil. Von ihnen erklärten sich wiederum 35 Probanden zur Durchführung eines problemzentrierten Interviews bereit.
Ergebnisse: Die mittels exploratorischer Hauptkomponentenanalysen identifizierten Skalen Stimmung, Erwartungserfüllung, Anforderungen, Erwartungshaltung und Zukunftspläne spiegeln zusammenfassend die Ergebnisse auf Variablenebene wider und ergeben insgesamt recht positive Mittelwerte zwischen 1,29 und 2,31 („1“ positivster Wert zugunsten und „4“ negativster Wert zuungunsten der betreffenden Skalen). Die Erwartungserfüllung – bezogen auf die Berufsausbildung und das Berufsbild – weist mit einem Mittelwert von 2,31 die deutlichsten Potenziale auf. Durch eine Schülerklassifikation wird, mehr als auf der Variablenebene, deutlich, dass am Ende der Ausbildungszeit 52,6% der Schüler ihre Zukunft in der Pflege und in MV sehen. Bei den Interviews verringert sich dieser Anteil jedoch auf 22,8% (Pflege direkt am Patienten in MV, ohne Angabe von Studienwünschen). Mit der Pearson-Korrelation lassen sich auf Skalenebene positive signifikante Zusammenhänge (p<0,05) zwischen den Gegenstandsbereichen nachweisen, wobei sich die stärksten Zusammenhänge zwischen Stimmung – Erwartungserfüllung (r=0,707, p<0,001) und Stimmung – Zukunftsplänen (r=0,663, p<0,001) zeigen.
Fazit und Ausblick: Trotz vieler positiver Ergebnisse sollten die Anteile im Negativbereich berücksichtigt und nicht verkannt werden, denn die Absicherung der Pflege in MV stellt sich weiter kritisch dar. „Pflege pflegen“ und „Gesundheit leben“ bleiben wichtige Themen. Neben den konstatierten Pflegebedingungen führt aber auch der Wunsch, sich weiterqualifizieren zu wollen, zu der Pflegefluktuation. Bemerkenswert ist die spürbare Verbundenheit zum Land. MV sollte dennoch die Wünsche von jungen Erwachsenen nicht aus den Augen verlieren: ausbaufähige Berufs- und Freizeitattraktivität sowie Verkehrsanbindungen zwischen den Städten bei Naturerhalt. Dies sind vermutlich langfristig entscheidende Investitionen, die positiv zurückwirken können.
The present study seeks to determine potential associations between viral infections and neuropsychiatric diseases. To address this issue, we investigated the peptide commonalities between viruses that have been related to psychiatric and neurological disorders—such as rubella, human immunodeficiency virus, and herpesviruses—and human distal-less homeobox (DLX) proteins expressed in developing brain—namely, DLX1, DLX2, DLX5, and DLX6. Peptide matching analyses revealed a high degree of pentapeptide sharing. From an immunological perspective, this overlap is relevant because pentapeptides are endowed with immunogenicity and antigenicity—that is, they are immune determinants. Moreover, infection-induced immune cross-reactions might have functional, spatial, and temporal implications related to the functions and expression patterns of DLX1 and DLX5 in the fetal and adult human brain. In sum, our data support the hypothesis that viral infections may be linked to neuropsychiatric diseases through autoimmune cross-reactions caused by molecular mimicry between viral proteins and brain-specific DLX self-antigens.
Abstract
Background: Behavior management techniques (BMT) are essential in order to
achieve a successful dental treatment with a minimum amount of stress in paediatric
dentistry, but parents are not equally accepting different advanced BMT.
Purpose: To investigate the differences in parental acceptance of advanced
behavior management techniques between University of Greifswald/Germany and
Jordan University/Jordan.
Methods: Parents of the children treated in the pedodontic departments at the
University of Greifswald/Germany and Jordan University/Jordan rated their
acceptance level of four advanced behavior management techniques (passive
restraint, active restraint, nitrous oxide sedation and general anesthesia) for normal
treatment, and for urgent treatment using a five points Likert scale. 200 parents (100
in each university) completed the questionnaire forms for analysis.
Results: Nitrous oxide sedation was rated the most accepted technique in
Greifswald and Jordan (mean 3.78±1.34; 3.22±1.50, respectively). The least
acceptable technique in Greifswald was passive restraint (2.05±1.18) and in Jordan
general anesthesia (2.11±1.30). The parents in Greifswald are significantly more
accepting nitrous oxide sedation than parents in Jordan (p=0.010), while parents in
Jordan are significantly more willing to accept passive restraint (p=0.001). The
acceptance of all advanced behavior management techniques increased significantly
in both groups when the treatment is urgent (p≤0.05),
Conclusions: Parental culture and the urgency of the treatment affect the
acceptance to different behavior management techniques. Moreover, the parental
attitude to the pharmacological technique has changed, as nitrous oxide sedation
generally appears to be the most preferred advanced technique in both groups.
Ein Mangel an spezifischen Symptomen und diagnostischen Möglichkeiten verhindert häufig eine frühe Diagnosestellung und damit eine effektive Therapie des OC. Das Hitzeschockprotein HSP27 ist in die Progression vieler maligner Krebserkrankungen involviert und nachweislich in einer Vielzahl solider Tumoren hochreguliert. HSP27-Serumspiegel korrelieren mit der Aggressivität und Chemotherapieresistenz des OC. Daher stellt HSP27 einen potentiellen Biomarker für die Diagnostik, Prognose und Therapie des OC dar.
Die aktive Sekretion von HSP27 wurde bereits für diversen Zelltypen beschrieben und findet auch in Zellen ovariellen Ursprungs statt. Daher wurde die HSP27-Sekretion in dieser Arbeit im Zellkulturmodell mit den Ovarialkarzinomzelllinien OVCAR-3, SK-OV-3 und TOV-21G untersucht. Die Überexpression von HSP27 wurde sowohl gentechnisch mittels des DNA-Vektors pHSP27, als auch mittels Hitzeschock-Behandlung simuliert. Western Blot und ELISA Messungen dienten der Bestimmung der intra- und extrazellulären HSP27-Konzentration. Die Inhibition des intrazellulären Proteintransportes durch das ER wurde durch BFA realisiert. Der Transfektion der Zellen mit dem Vektor pHSP27 folgte eine Steigerung der intrazellulären HSP27-Expression. Diese war assoziiert mit signifikant erhöhten extrazellulären HSP27-Konzentrationen. Diese Ergebnisse konnten durch die Hitzeschock-Versuche bestätigt werden. Die Inhibition des ER-abhängigen intrazellulären Transportmechanismus durch BFA lässt auf eine aktive Sekretion von HSP27 über einen alternativen Mechanismus schließen.
In der vorliegenden Arbeit wurde gezeigt, dass die HSP27-Sekretion in OVCAR-3, SK-OV-3 und TOV-21G Zellen von der intrazellulären HSP27-Konzentration abhängig, aber unabhängig von ER-Transportmechanismen in den Ovarialkarzinomzellen ist. Die Anwesenheit von HSP27 im Serum von Patientinnen deutet darauf hin, dass die HSP27-Sekretion eine Funktion bzgl. der Tumorprogression des OC besitzt. Daher könnte die Konzentration an sekretiertem HSP27 als Biomarker für Diagnostik und Therapieverlauf sowie intrazelluläres HSP27 als Zielmolekül für die Therapie des Ovarialkarzinoms von Bedeutung sein.
Unstable environments and habitats changing due to climate change force individuals to either respond by genetic adaptation, phenotypic plasticity or by dispersal to suitable environments. Theodoxus fluviatilis (Linneaus, 1758) is a good study organisms when researching phenotypic plasticity and genetic adaptation as it naturally appears in freshwater (FW) as well as brackish water (BW) and thus inhabits a wide range of environmental salinities (0-18‰). It is a euryhaline snail that can be found in shallow waters with stony ground or on Fucus spp. and has formed regional subgroups. The brackish water and the freshwater subgroups are spatially separated and the species cannot be found in areas inbetween, e.g. estuaries.
The species shows great variability in shell patterning and shell size and there is still debate whether the subgroups are distinguishable by these traits or not. The mitochdrial RNA marker cytochrome c subunit I did not show differences between the subgroups indicating that they must be closely related, but salinity tolerance has been observed to be higher in BW snails. This might be caused by the different protein expression patterns and osmolyte accumulation (measured as ninhydrin-positive substances) observed in this species in previous studies. The exact mechanisms regulating protein expression and osmolyte accumulation, however, are not fully understood yet.
Data collected for this thesis shows differences in shell size and suggests a less strict grouping of FW and BW individuals as shell sizes of one FW site are more similar to BW individuals than the other FW ones. A better salinity tolerance towards high salinities and a higher physiological salinity limit of BW snails was confirmed and extended by demonstrating an expanded tolerance range through slow acclimation to challenging salinities in snails from both subgroups. This was achieved by a shift in the slope of their reaction norms that was much more pronounced in BW snails than FW ones. S3 individuals showed a shift similar to that of BW individuals. The data for the salinity tolerance indicates that the underlying mechanism for these tolerances are a combination of phenotypic plasticity and genetic adaptation. Despite an acclimation and shift in the slope of the reaction norms and therefore an increased tolerance towards high salinities (plasticity) FW individuals from two collection sites were not able to cope with salinities as high as BW individuals (local adaptation). The general ability to mobilise free amino acids (FAA) as organic osmolytes was not the reason for this tolerance difference. Individuals from BW and FW sites were capable of accumulating quantities of FAAs equally well. Proline, alanine and urea were the most important components of the accumulated cocktail of organic osmolytes. Even though the total amount of FAAs accumulated under hyperosmotic conditions was the same in both subgroups, there were differences in the metabolic pathways involved in osmolyte accumulation in the foot muscle. The data indicates that the hydrolysis of storage proteins and the synthesis of proline and alanine are the main processes to avoid detrimental body volume shrinkage in T. fluviatilis. While FW individuals seemed to rely on the degradation of proteins and synthesis of alanine, BW individuals depended on newly synthesising proline and alanine and accumulating urea as a side product of transamination. The accumulation of urea is a new finding in aquatic living snails and has not been reported as a mechanism to avoid cell volume shrinkage in these animals.
Differing protein expression patterns were observed under control conditions across all collection sites. 9 spots showed volume changes in BW snails opposite to those of FW snails from collection sites S1 and S2. For 6 of those spots, S3 individuals showed patterns similar to those of BW individuals and for the remaining 3 they showed patterns similar to those of FW animals. The patterns observed when exposing snails to hypo- or hyperosmotic stress were not conclusive in relation to pinpointing individual spots that show the same pattern in all collection sites, but revealed the heterogeneity of protein expression in snails from the different collection sites and in the process of osmoregulation. It also showed the general tendency of protein reduction when snails where under osmotic stress of either kind (hypo- or hyperosmotic), which supports the hypothesis of storage protein degradation.
The investigation of an ANP-receptor showed two variations of the encoding sequence expressed in T. fluviatilis. S3 individuals as well as BW individuals were found to express one type, while FW individuals, with the exception of one sample expressed the other type. This showed that the FW subgroup of T. fluviatilis seems to be more heterogeneous than the BW subgroup, but also raises the question of the dispersal history of this species. The collected data indicates that T. fluviatilis individuals are firstly capable of surviving the acidity of a duck's gizzard and secondly can tolerate acute salinity changes to 16‰ when introduced into a new environment. Hence, if snails from the FW were to be transported to waters with a salinity of up to 16‰ by man, bird, drifting plants or some other means of transport, they would most likely survive and possibly be able to thrive and spread.
Cerebral palsy (CP) remains one of the most common debilitating diseases in the world.
Factors such as neuromuscular incoordination, use of soft diet and delayed oral
clearance of food make maintenance of optimum oral hygiene a challenging task in
these children. The compromised oral hygiene in turn make these children highly
vulnerable to dental disease. Maintaining optimal oral/dental health in CP children is of
utmost importance as these children usually suffer from several associated general
health problems in addition to their primary condition. Poor dental health further
compromises their general health. Nevertheless, CP children often suffer from dental
diseases including dental caries more than healthy children. This underscores the need
for improvement in oral health of the CP children. The CP children are dependent on
those around them for their oral hygiene maintenance and dietary intake. CP children
spend most of their time with health care workers in special centers for them or with
their parents/care takers at home. Therefore, the attitude of special health care workers
and parents towards their dental health is of prime importance. The purpose of this
project included to determine the attitude of special health care workers towards their
dental health and oral health comprehension by parents of CP children. In addition, a
study on caries experience of CP children and associated risk factors was also completed.
The final objective was to identify various potential areas of improvement with ultimate
goal of improving dental health of the CP children.
The study involving special health care workers in a Disabled Children’s Center revealed
that oral health knowledge and practices among the workers could generally be labeled
as satisfactory, however some weak areas (such as trend towards symptom-oriented
utilization of oral health care and lack of knowledge about fluoridated water) were
identified. There is a need to enhance the workers’ knowledge in these areas.
The study involving parents of the CP children showed mixed results. Some areas of
strong oral health comprehension (such as importance of good dental health, harmful
effects of sweetened foods), others satisfactory (such as need for routine dental visits,
use of fluoride) and some weak areas (use of flavored fizzy drinks, bottled/canned juices
and sweetened/flavored milks, seeking early treatment) were identified among parents
of CP children. The results strongly indicated a need for enhanced efforts towards
improvement of oral health comprehension in the parents of the CP children.
The study about caries experience and risk factors in CP children has provided useful
information in an area where literature has been scarce. Almost all the studied children
had clinical dental caries, and most of the caries were untreated. Very few children in
the present study had optimal oral hygiene. This study confirmed the strong correlation
between high dental caries experience and poor oral hygiene. Routine dental check-up
visits and topical fluoride application clearly resulted in lower caries experience. Bottled
juices and crispy potato chips were the two predictors for high caries experience.
As stated before, the goal of the project was to identify various potential areas of
improvement in oral health attitude of special health care workers, oral health
comprehension of the parents of CP children and risk factors for dental caries in CP
children. It is contemplated that by addressing the identified weak areas, an
improvement in dental health and care of the CP children is expected. It is also expected
that the collected information will help in improvement of clinical preventive as well as
restorative services for the CP children. CP children and their parents lead a challenging
life. This project was an effort to assist these children and their parents/care takers with
ultimate goal of improving their oral health and in turn their overall quality of life.
Objektive Eingruppierung sequenzierter Tollwutisolate mithilfe des Affinity Propagation Clusterings.

Das International Committee on Taxonomy of Viruses (ICTV) reguliert die Nomenklatur von Viren sowie die Entstehung neuer Taxa (dazu gehören: Ordnung, Familie, Unterfamilie, Gattung und Art/Spezies). Dank dieser Anstrengungen ist die Einteilung für verschiedenste Viren in diese Kategorien klar und transparent nachvollziehbar. In den vergangenen Jahrzehnten sind insgesamt mehr als 21.000 Datensätze der Spezies „rabies lyssavirus“ (RABV) sequenziert worden. Eine weiterführende Unterteilung der sequenzierten Virusisolate dieser Spezies ist bislang jedoch nicht einheitlich vorgeschlagen. Die große Anzahl an sequenzierten Isolaten führte auf Basis von phylogenetischen Bäumen zu uneindeutigen Ergebnissen bei der Einteilung in Cluster. Inhalt meiner Dissertation ist daher ein Vorschlag, diese Problematik mit der Anwendung einer partitionierenden Clusteringmethode zu lösen. Dazu habe ich erstmals die Methodik des affinity propagation clustering (AP) für solche Fragestellungen eingesetzt. Als Datensatz wurden alle verfügbaren sequenzierten Vollgenomisolate der Spezies RABV analysiert. Die Analysen des Datensatzes ergaben vier Hauptcluster, die sich geographisch separieren ließen und entsprechend als „Arctic“, „Cosmopolitain“, „Asian“ und „New World“ bezeichnet wurden. Weiterführende Analysen erlaubten auch eine weitere Aufteilung dieser Hauptcluster in 12-13 Untercluster. Zusätzlich konnte ich einen Workflow generieren, der die Möglichkeit bietet, die mittels AP definierten Cluster mit den Ergebnissen der phylogenetischen Auswertungen zu kombinieren. Somit lassen sich sowohl Verwandtschaftsverhältnisse erkennen als auch eine objektive Clustereinteilung vornehmen. Dies könnte auch ein möglicher Analyseweg für weitere Virusspezies oder andere vergleichende Sequenzanalysen sein.
Species of the genus Wolffia are traditionally used as human food in some of the Asian countries. Therefore, all 11 species of this genus, identified by molecular barcoding, were investigated for ingredients relevant to human nutrition. The total protein content varied between 20 and 30% of the freeze-dry weight, the starch content between 10 and 20%, the fat content between 1 and 5%, and the fiber content was ~25%. The essential amino acid content was higher or close to the requirements of preschool-aged children according to standards of the World Health Organization. The fat content was low, but the fraction of polyunsaturated fatty acids was above 60% of total fat and the content of n-3 polyunsaturated fatty acids was higher than that of n-6 polyunsaturated fatty acids in most species. The content of macro- and microelements (minerals) not only depended on the cultivation conditions but also on the genetic background of the species. This holds true also for the content of tocopherols, several carotenoids and phytosterols in different species and even intraspecific, clonal differences were detected in Wolffia globosa and Wolffia arrhiza. Thus, the selection of suitable clones for further applications is important. Due to the very fast growth and the highest yield in most of the nutrients, Wolffia microscopica has a high potential for practical applications in human nutrition.
Together with endothelial cells and the glomerular basement membrane, podocytes form the size-specific filtration barrier of the glomerulus with their interdigitating foot processes. Since glomerulopathies are associated with so-called foot process effacement—a severe change of well-formed foot processes into flat and broadened processes—visualization of the three-dimensional podocyte morphology is a crucial part for diagnosis of nephrotic diseases. However, interdigitating podocyte foot processes are too narrow to be resolved by classic light microscopy due to Ernst Abbe's law making electron microscopy necessary. Although three dimensional electron microscopy approaches like serial block face and focused ion beam scanning electron microscopy and electron tomography allow volumetric reconstruction of podocytes, these techniques are very time-consuming and too specialized for routine use or screening purposes. During the last few years, different super-resolution microscopic techniques were developed to overcome the optical resolution limit enabling new insights into podocyte morphology. Super-resolution microscopy approaches like three dimensional structured illumination microscopy (3D-SIM), stimulated emission depletion microscopy (STED) and localization microscopy [stochastic optical reconstruction microscopy (STORM), photoactivated localization microscopy (PALM)] reach resolutions down to 80–20 nm and can be used to image and further quantify podocyte foot process morphology. Furthermore, in vivo imaging of podocytes is essential to study the behavior of these cells in situ. Therefore, multiphoton laser microscopy was a breakthrough for in vivo studies of podocytes in transgenic animal models like rodents and zebrafish larvae because it allows imaging structures up to several hundred micrometer in depth within the tissue. Additionally, along with multiphoton microscopy, lightsheet microscopy is currently used to visualize larger tissue volumes and therefore image complete glomeruli in their native tissue context. Alongside plain visualization of cellular structures, atomic force microscopy has been used to study the change of mechanical properties of podocytes in diseased states which has been shown to be a culprit in podocyte maintenance. This review discusses recent advances in the field of microscopic imaging and demonstrates their currently used and other possible applications for podocyte research.
Neurosteroids, comprising pregnane, androstane, and sulfated steroids can alter neuronal excitability through interaction with ligand-gated ion channels and other receptors and have therefore a therapeutic potential in several brain disorders. They can be formed in brain cells or are synthesized by an endocrine gland and reach the brain by penetrating the blood–brain barrier (BBB). Especially sulfated steroids such as pregnenolone sulfate (PregS) and dehydroepiandrosterone sulfate (DHEAS) depend on transporter proteins to cross membranes. In this review, we discuss the involvement of ATP-binding cassette (ABC)- and solute carrier (SLC)-type membrane proteins in the transport of these compounds at the BBB and in the choroid plexus (CP), but also in the secretion from neurons and glial cells. Among the ABC transporters, especially BCRP (ABCG2) and several MRP/ABCC subfamily members (MRP1, MRP4, MRP8) are expressed in the brain and known to efflux conjugated steroids. Furthermore, several SLC transporters have been shown to mediate cellular uptake of steroid sulfates. These include members of the OATP/SLCO subfamily, namely OATP1A2 and OATP2B1, as well as OAT3 (SLC22A3), which have been reported to be expressed at the BBB, in the CP and in part in neurons. Furthermore, a role of the organic solute transporter OSTα-OSTβ (SLC51A/B) in brain DHEAS/PregS homeostasis has been proposed. This transporter was reported to be localized especially in steroidogenic cells of the cerebellum and hippocampus. To date, the impact of transporters on neurosteroid homeostasis is still poorly understood. Further insights are desirable also with regard to the therapeutic potential of these compounds.
Neurosonographischer und klinischer Verlauf von distalen extra- und intrakraniellen Stenosen des vertebrobasilären Stromgebietes in einem ambulanten Patientenkollektiv.
Hintergrund: Ca. 26% der Schlaganfälle sind im vertebrobasilären Stromgebiet lokalisiert. Stenookklusive Veränderungen der intrakraniellen Arteria vertebralis und basilaris bergen nach vorliegenden Studien ein hohes Risiko für ein ischämisches Ereignis. Empirisch hat sich jedoch in einem ambulanten Setting unserer neurovaskulären Sprechstunde trotz einer relativ hohen Dynamik in den neurosonographischen Befunden eine eher geringe Konversionsrate asymptomatischer Stenosen in ein manifestes zerebrovaskuläres Ereignis beobachten lassen.
Methoden: Patienten aus der Neurovaskulären Ambulanz mit seit ≥24 Monaten bekannten vertebrobasilären stenookklusiven Veränderungen wurden prospektiv in die Studie eingeschlossen. Über mindestens ein Jahr erfolgte die Verlaufsbeobachtung der neurovaskulären Befunde mittels extra- und intrakranieller Doppler- sowie Duplexsonographie sowie des neurologischen und funktionellen Status mittels NIHSS und MRS. Demographische Basisdaten, kardiovaskuläre Risikofaktoren, Komorbiditäten und therapeutische Maßnahmen wurden anhand von standardisierten Fragebögen gewonnen. Eine kognitive Leistungserfassung erfolgte mittels MMST, die Erfassung der Lebensqualität mittels EuroQol und SF-36 v.2.0.
Ergebnisse: 94 Patienten (mittleres Alter 69,7 Jahre, 55% männlich) mit einem retrospektiven Beobachtungszeitraum von im Mittel 5,5 Jahre (2-12,9 Jahre) wurden eingeschlossen. An Komorbiditäten war bei 97% eine Hypertonie, 83% eine Dyslipidämie, 67% ein Nikotinabusus, 35% ein Diabetes mellitus, 38% eine koronare Herzkrankheit und 27% eine periphere arterielle Verschlusskrankheit bekannt. Ein zerebrovaskuläres Ereignis hatte vor Einschluss bereits 75% der Probanden, 28% im vertebrobasilären Stromgebiet. Im Follow-Up traten bei 2,4% Schlaganfälle ausschließlich im Carotis-Stromgebiet auf. Bei den stenookklusiven Veränderungen handelte es sich um 63% basiläre (61% leicht-, 25% mittel-, 12% hochgradige, 2% okklusive) und 56% distale vertebrale Läsionen (48%, 15%, 31%, 8%). Bei 21% der Patienten konnten im Follow-Up sonographische Veränderungen beobachtet werden, im hinteren Stromgebiet bei 11% (78% Stenosegrad-Reduktion, 22% -Zunahme) und im vorderen Stromgebiet bei 12% (50%, 60%). Die funktionelle Beeinträchtigung wurde durch den medianen MRS mit 1 (Spanne 0-4; MRS ≤2 88%), den NIHSS mit 0 (0-11) und den MMST mit 29 (22-30) angegeben, im Follow-Up mit leichtem Anstieg des MRS auf 1 (0-6; p=0,038) und des NIHSS auf 1 (0-15; p=0,058). Die klinische Veränderung ging bei 29% mit einer sonographischen Veränderung einher. Die für dieses Kollektiv gute Lebensqualität zeigte im Follow-Up trotz einer signifikanten Besserung der Vitalität eine Reduktion des allgemeinen Gesundheitszustands und der sozialen Funktionalität.
Fazit: In dieser Studie konnten wir trotz eines hohen vaskulären Risikoprofils und einer niedrigen Rate an Sekundärprophylaxe eine Tendenz zur Regredienz der Stenosen im vertebrobasilären Stromgebiet sowie eine sehr niedrige Schlaganfall-Inzidenz beobachten. Im longitudinalen Verlauf zeigte sich ein insgesamt gutes klinisches Outcome mit einer nur leichten, aber signifikanten Verschlechterung. Ursächlich für diese Entwicklung vertebrobasilärer Stenosen sahen wir den im Vergleich zum vorderen Stromgebiet bekannten abweichenden Pathomechanismus, so dass die Schlussfolgerung nahe liegt, dass vertebrobasiläre Stenosen weniger gefährlich sind. Im Weiteren ist zu klären, wie die Entwicklung der vertebrobasilären Stenosen unter verschiedenen Therapieregimen verläuft.
The synthesis of pterin-dithiolene ligands was achieved by employing the radical nucleophilic substitution, i.e. the so-called “Minisci- Reaction”1. This protocol was used for the first time by Professor W. Pfleiderer on pterin substrates2 and proved a powerful method for the preparation of 6 acyl-pterins in course of this work. Subsequent construction of the dithiolene ring facilitates the synthesis of pterin-dithiolene ligands with completely unprotected pterin moieti.
The molybdenum cofactor is probably one of the most relevant discoveries in the recent history of pterin chemistry and biochemistry. Many efforts have been made for the preparation of compounds able to mimic the features of the Moco ligand system called "Molybdopterin". In fact, the study of MPT models enables a deeper understanding of the “mechanism of function” of this cofactor and most importantly, lays the foundation for a potential treatment for the Moco related diseases MoCOD and iSOD.
In der vorliegenden Arbeit wurden verschiedene grundlegende Ansätze zur Prävention der Afrikanischen Schweinepest entwickelt und teilweise evaluiert. Die ersten beiden befassten sich mit der Impfstoffentwicklung, wobei einerseits eine vektorbasierte Strategie verfolgt und zum anderen auf eine attenuierte ASPV Lebendvakzine hingearbeitet wurde. Die dritte Herangehensweise war auf die Inhibierung der viralen Replikation in Schweinezellen mittels CRISPR/Cas9 fokussiert, um Hinweise auf die generelle Anwendbarkeit des Systems in transgenen Schweinen zu gewinnen.
Der Pseudorabies-Virus Impfstamm Bartha (PrV-Ba) wurde als potenzieller Vektor für die Expression von ASPV-Antigenen im Schwein gewählt. Dazu wurde zunächst eine Methode etabliert, mit der PrV-Rekombinanten effizient generiert werden konnten. Dabei erwies sich die Verwendung eines artifiziellen bakteriellen Chromosoms (BAC), welches das infektiöse PrV-Genom enthielt, als hilfreich. In diesem wurde ein essentielles PrV-Gen inaktiviert, dass dann durch homologe Rekombination mit einem Transferplasmid in co-transfizierten Säugerzellen rekonstituiert werden konnte. Durch gleichzeitiges CRISPR/Cas9-vermitteltes Schneiden der BAC-DNA am gewünschten Insertionsort konnte die Rekombinationsrate weiter gesteigert werden. Diese Strategie wurde anschließend für die Insertion verschiedener ASPV Gene in das PrV-Genom genutzt, wobei unterschiedliche Promotoren und Kodonoptimierungen getestet wurden. In den meisten Fällen konnten durch Verwendung des CAG-Promotors und eine Kodonadaptation an porcine Gene die höchsten Expressionsraten erreicht werden. Somit wurde eine Methode entwickelt, mit der prinzipiell alle ASPV Gene im PrV Vektorsystem exprimiert und in Impfstudien getestet werden könnten.
In der zweiten Untersuchung wurden zwei Proteine des ASPV mittels monospezifischer Antiseren näher charakterisiert und ASPV-Deletionsmutanten generiert, um Hinweise auf die Funktionen der Proteine zu erhalten. Bei diesen Proteinen handelte es sich um p285L und pK145R. Sie wurden anhand von Daten aus einer vorhergehenden Proteomanalyse von Keßler et al. (2018) ausgewählt, da sie in großen Mengen in ASPV infizierten Zellen nachgewiesen wurden und deshalb wichtige Funktionen (beispielweise auch Virulenz-determinierende Funktionen) haben könnten. Bei p285L handelt es sich um ein früh exprimiertes Virionprotein, das zunächst in den sogenannten Virusfabriken infizierter Wildschweinlungenzellen (WSL) akkumuliert. pK145R ist ein spätes ASPV Protein, das in Virionen nicht nachweisbar ist und eine diffuse Verteilung im Zytoplasma infizierter Zellen zeigt. Beide Proteine sind für die Virusreplikation nicht essentiell, und ihre Deletion führte zu keiner (285L) oder einer nur mäßigen (K145R) Titerreduktion in infizierten WSL Zellen oder primären Blutzellkulturen (PBMC). Durch in vivo Analysen der Deletionsmutanten muss nun geklärt werden, ob p285L und pK145R für die Virulenz bzw. die Wechselwirkung von ASPV mit dem Wirtsimmunsystem wichtig sind und ob sich die Mutanten deshalb als lebend attenuierte Impfstoffe eignen könnten.
In der dritten Studie wurde untersucht, ob das CRISPR/Cas9 System zur Inhibition der ASPV Infektion in vitro geeignet ist. Dazu wurden WSL-Zelllinien generiert, die Cas9 und verschiedene sgRNAs konstitutiv exprimierten. Durch die Expression einer sgRNA gegen das Gen des ASPV Phosphoproteins p30 (CP204L) wurde die Virusreplikation nahezu komplett inhibiert. Die Spezifität des Effektes konnte durch parallele Versuche mit einem Virusisolat, dessen Zielsequenz nicht mit der genutzten sgRNA Sequenz übereinstimmte, gezeigt werden. Dieses ASPV Isolat konnte im Gegensatz zum Virus mit der passenden Zielsequenz in den rekombinanten Zellen genauso gut replizieren, wie in nicht modifizierten Zellen. Zudem wurde die Spezifität durch die Analyse von sporadisch auftretenden Escape-Mutanten bestätigt, die verschiedene Basenaustausche in der Zielsequenz aufwiesen. Somit konnte gezeigt werden, dass das CRISPR/Cas9 System eine effiziente Inhibition der ASPV Replikation in Zellkultur bewirken kann und deshalb möglicherweise auch in entsprechenden transgenen Schweinen eine Resistenz gegen letale ASPV-Infektionen vermitteln könnte.
Zusammenfassend lässt sich sagen, dass in allen drei Studien grundlegende Experimente erfolgreich durchgeführt wurden, die zur Prävention der Afrikanischen Schweinepest beitragen können.
Die Zusammenhänge zwischen der Aktivität Epigenetik-assoziierter Enzyme und Krankheitsgeschehen neoplastischer Art konnten nicht nur experimentell bestätigt, sondern auch spätestens mit der Zulassung von Inhibitoren der DNA-Methyltransferase und Histon-Desacetylasen kausal bewiesen werden. Die weniger gut untersuchten 2-Oxoglutarat- und Eisen(II)-abhängigen JumonjiC-Domänen enthaltenden Histon-Demethylasen (KDMs) werden aufgrund von Überexpression in einigen Tumorzellen ebenfalls als mögliches epigenetisches Target in der Tumortherapie angesehen.
Die in dieser Arbeit synthetisierten KDM4-Inhibitoren basierten zum einen auf Tetrazol- und Hydrazid-haltigen Motiven, für welche synthetische Routen in weitergehenden Untersuchungen entwickelt wurden, um das aliphatische Rückgrat der Moleküle mit aromatischen Resten zu modifizieren oder komplett durch eines auf Basis der Anthranilsäure zu ersetzen. Zum anderen sollten Synthesewege für verschiedenartige (1H-Tetrazol-5 yl)pyridine gefunden werden. Dafür wurde ein Tetrazol in Position vier des Pyridinrings eingefügt, um anschließend systematische Untersuchungen der Seitenkette in Position zwei des Pyridinrings vorzunehmen. Neben direkten Heteroatomverknüpfungen wurden auch Carbonylverbindungen wie Carbonsäureamide oder eine Hydroxamsäure dargestellt. Über zahlreiche Wege konnten ebenfalls Methylengruppen-haltige Verbindungen erfolgreich synthetisiert werden.
Die biologische Testung der Verbindungen erfolgte mit einem Antikörper-basierten LANCE-Assay gegen KDM4A. Zusätzlich konnten vier der Substanzen röntgenkristallographisch in der Isoform KDM4D vermessen werden. Eine in Position zwei des Pyridinrings eingefügte Hydroxamsäure zeigte relevante Interaktionen im Enzym und veranschaulichte zugleich, dass (1H-Tetrazol-5 yl)pyridine zu einer hohen Affinität bestimmter Isoformen der Histon-Demethylasen neigen können.
Zusätzlich zu den bereits ansatzweise verstandenen Zusammenhängen zwischen Epigenetik und neoplastischen Krankheiten, weisen Experimente im Nagermodell darauf hin, dass epigenetisch modifizierende Enzyme auch an der Entstehung von depressionsartigen Phänotypen beteiligt sind. Die grundsätzliche Erforschung neuer Therapieoptionen abseits der Monoaminhypothese für die Indikation Depression ist jedoch auch deshalb notwendig, weil zugelassene medikamentöse Behandlungsoptionen zum Teil zu starken Nebenwirkungen führen und bis zur Entfaltung der vollen Wirkung oftmals mehrere Wochen vergehen können. Mit dem NMDA-Rezeptorantagonisten Ketamin behandelte depressive Patienten zeigten häufig eine rasche Symptomlinderung bei vergleichsweise günstigem Nebenwirkungsprofil. Welche Moleküle für diese Effekte ursächlich sind, ist noch Gegenstand aktueller Forschung, da neben Ketamin selber auch Metaboliten wie 2R,6R-Hydroxynorketamin mit antidepressiven Wirkungen assoziiert werden. Für die qualitativen und quantitativen Analyse von R- und S-Ketamin sowie den Metaboliten R- und S-Norketamin, R- und S-Dehydronorketamin und 2R,6R- und 2S,6S Hydroxynorketamin in menschlichem Urin wurde in der vorliegenden Arbeit eine chromatographische Methode entwickelt und validiert, welche die Techniken der überkritischen Fluidchromatographie und Massenspektrometrie nutzt (SFC-MS). Die für die SFC charakteristischen Eigenschaften wie hohe Flussraten und Trenneffizienz sowie die Nutzung von Kohlenstoffdioxid als mobile Phase konnten ausgenutzt werden, um das Trennproblem in nur einer Methode mit kurzer Laufzeit zu lösen. Gleichzeitig wurde der Verbrauch von organischen Lösungsmitteln reduziert. Damit kann die erarbeitete Methode einen nachhaltigen Beitrag für zukünftige Ketaminforschung leisten.
Im Forschungsfeld Klimaschutz und Nachhaltigkeitszertifizierung werden in dieser Arbeit die Treibhausgasemissionen durch Verwendung von Rapsbiodiesel gemäß EU Nachhaltigkeitsvorgaben der EU RED thematisiert. Regionales Bezugssystem sind gemäß EU-Vorgabe die NUTS2-Gebiete. Für 35 NUTS2-Regionen Deutschlands wurden Treibhausgasemissionen, die dem praxisnahen Rapsanbau zugerechnet werden können, berechnet.
Ein entwickeltes sogenanntes Rechenmodul mit dem Rapsanbauverfahren von der Bodenbearbeitung über die Düngung erfasst, beinhaltet einen vom Ifeu-Institut erschaffenen THG-Rechner, der die THG-Emissionen gemäß IPCC-und RED-Vorgabe berechnet. Um zwei Forschungslücken zu schließen, wurden mithilfe des Rechenmoduls insbesondere die Differenzierung der Stickstoffdüngung und die Regionalspezifik von Rapsanbauverfahren beschrieben. Zentraler Parameter ist die Düngung, mit Unterscheidung üblicher Stickstoffmineraldünger, und zusätzlich im Rechenmodul implizierte Wirtschaftsdünger. Die eigentliche Modellierung typischer regionaler Rapsanbauverfahren für 35 NUTS2-Gebiete mit 10 erforderlichen Anbauparametern erfolgte mit der Unterstützung und dem Fachwissen regionaler Experten der Landeseinrichtungen für Landwirtschaft. Folgende Ergebnisse werden dargestellt:
o Quantifizierung der THG-Anbauemissionen definierter Rapsanbauverfahren.
o Quantifizierung der entsprechenden Stickstoffsalden unter Berücksichtigung der Anrechenbarkeit des Stickstoffs aus Wirtschaftsdünger nach Vorgaben der Dün-geverordnung.
o Wirtschaftlichkeit definierter regionaler Rapsanbauverfahren.
o Modellierung von Minderungsverfahren nach Expertenrat.
o Rechenbeispiel Mecklenburg-Vorpommern – Optimierungsvariante nach Dün-geverordnung
o Einschätzung der Erreichbarkeit des Untersuchungszielwerts im Modell.
o Die von der EU gewünschte räumliche Bezugsgröße NUTS2-Region erfordert Durchschnittswerte für unterschiedlich große Gebiete.
o Mit dem entwickelten Rechenmodul sind Anbauverfahren und die erforderlichen Rechengrößen präziser bestimmbar und einzelbetrieblich planbar.
Die THG-Verminderung ist mit folgenden drei Stellschrauben möglich:
(1) N-Düngung (N2O-Feldemissionen) - Mineraldünger (die Vorkettenemissionen implizieren) sind möglichst zu vermindern und am Bedarf gemäß Düngeverordnung zu orientieren (auch um hohe N-Salden zu vermeiden),
(2) Wirtschaftsdünger (generell ohne Vorkettenemissionen) sind effizient einzusetzen, d.h. der N-Gehalt muss berücksichtigt werden,
(3) Dieselkraftstoffeinsatz im Anbauverfahren sollte möglichst verringert werden.
Ziel der Studie war die Beantwortung der Fragestellung, ob es im Klinikum Pasewalk aus krankenhaushygienischer Sicht Einwände gegen einen Wechsel der orthopädischen Operationen von einer mit TAV ausgestatteten OP-Einheit in eine mit TVS belüftete OP-Einheit gibt. Zusätzlich sollten Einflussfaktoren auf die partikuläre und mikrobielle Belastung der Raumluft einschließlich der Rolle der RLT-A analysiert werden. Besonderes Interesse galt hierbei dem Messpunkt direkt am OP-Gebiet.
Als Qualitätsparameter der Raumluft wurden bei insgesamt 48 Operationen die Partikelkonzentration an drei Messpunkten und die mikrobielle Belastung der Raumluft an vier Messpunkten während des regulären OP-Betriebs in zwei OP-Einheiten gemessen. Zur Beurteilung der Partikelkonzentration der Raumluft an den 3 Messpunkten wurden Partikel der Größen 0,5 – 4,9µm und >5 µm kumulativ gemessen. Für die Erfassung der mikrobiellen Belastung der Raumluft wurden Sedimentationsplatten an 4 Messpunkten eingesetzt.
Die mikrobielle Belastung war an den Messpunkten am OP-Gebiet, am Instrumententisch und in der Nähe der aseptischen Arbeitsflächen im OP 1 mit TAV signifikant geringer als im OP 2 mit TVS. Zusätzlich wiesen alle Messpunkte eine signifikant geringere partikuläre Belastung (sowohl unterhalb als auch außerhalb der TAV) im OP 1 mit TAV auf.
Es konnte gezeigt werden, dass die Anzahl der im Hintergrund tätigen Personen die Partikelkonzentration (>5,0 µm) und die mikrobielle Belastung am OP-Gebiet erhöht. Diese Effekte sind nur in OP 2 mit TVS nachweisbar. Zudem konnte ein positiver Zusammenhang zwischen Partikelkonzentration (>5,0 µm) und mikrobieller Belastung festgestellt werden.
Als Fazit kann eine Empfehlung zum Wechsel der OP-Einheiten nicht ausgesprochen werden. Zudem konnte gezeigt werden, dass eine RLT-A mit TAV einen verringernden Einfluss auf die Erregerbelastung und Partikelbelastung am OP-Gebiet haben kann. Sie ist zudem über ihren Schutzbereich in der Lage Auswirkungen von Umgebungseinflüssen auf mikrobielle und partikuläre Belastung am OP-Gebiet signifikant zu verringern.
Currently, plastic materials are an integral part of our lives, but their production mostly bases on fossil fuels or derivatives, which resources are decreasing. Extraction and processing of non-renewable resources have also negative impact on environment. One of the most promising and environmentally friendly approaches is use of microorganism. This PhD dissertation presents the non-conventional yeast Arxula adeninivorans as a host for production of bio-based and biodegradable poly(hydroxyalkanoates) plastics poly(hydroxybutyrate) and co-polymer poly(hydroxybutyrate-co-hydroxyvalerate). Additionally, the constructed yeast strain was able to secrete enantiomerically pure (R)-3-hydroxybutyric acid.
The production of PHAs requires three enzymes: β-ketothiolase, acetoacetyl-CoA reductase and PHA synthase. The strategy followed in this project was divided into two parts. While all three enzymes are responsible for intracellular production of PHA polymer, first two only lead to secretion of (R)-3-HB into culture media, which was used in a first stage of work to establish and optimize polymer production. Both, different bacterial strains and yeast A. adeninivorans were taken into account in screening of the genes encoding aforementioned enzymes. Bacterial genes were chemically synthesized using codon optimization pattern and endogenous genes were obtained using PCR and genomic DNA template from A. adeninivorans LS3 wild-type strain. Each gene was cloned into Xplor2 vector between TEF1 constitutive promoter and PHO5 terminator. Vector containing both thiolase and reductase genes was used for A. adeninivorans transformation.
The best combination of heterologous genes was overexpression of β-ketothiolase gene from Clostridium acetobutylicum and acetoacetyl-CoA reductase gene from Cupriavidus necator which led to secretion of 4.84 g L−1 (R)-3-HB, at a rate of 0.023 g L−1 h−1 over 214 h in shaking flask cultivation. Further optimization by fed-batch culturing with glucose as a carbon source did not improve (R)-3-HB secretion, but the rate of production was doubled to 0.043 g L−1 h−1 [3.78 g L−1 of (R)-3-HB at 89 h].
The product of acetoacetyl-CoA reductase is (R)-3-HB-CoA and further removing of CoA moiety is needed for acid secretion into culture media. A. adeninivorans is able to conduct this process without any additional modification but the conversion rate is unknown. Two thioesterases, cytosolic TesBp encoded by TesB gene from E. coli and mitochondrial ATes1p encoded by ATES1 gene from A. adeninivorans, were analysed to enhance secretion process. Additionally, a cytosolic version of ATES1 gene (ATES1cyt) was tested. All three genes were expressed in A. adeninivorans cells under TEF1 constitutive promoter together with thiolase and reductase genes. Despite detected enzymatic activity the yield of (R)-3-HB synthesis and secretion was not increased. Moreover, overexpressed thioesterases negatively influenced cell growth, indicating that they act on other metabolic components. The results provided two sets of information, first, the endogenous secretion system is sufficient for (R)-3-HB production; second, further screening of suitable genes needs to be performed.
Based on optimization of (R)-3-HB synthesis, thiolase gene (thl) from C. acetobutylicum and reductase gene (phaB) from C. necator were chosen to combine with PHA synthase gene (phaC) for creating the PHB-V producing strain. The PHA synthase expression module, containing TEF1 promoter and PHO5 terminator, was cloned into Xplor2 vector together with thiolase and reductase expression modules and used for A. adeninivorans transformation. The engineered strain accumulated up to 7.47% PHB of dcw. During the set of cells passaging A. adeninivorans lost the ability to accumulate polymer with maximal 23.1 % of primary accumulation level. Additionally, use of a vector including hygromycin B antibiotic resistance marker (instead of auxotrophic marker in Xplor2) did not improve polymer accumulation and stability.
To counteract the effect of loss of accumulation stability, phasin gene (phaP1), originated from C. necator, was introduce together with PHA pathway genes. First screening cultivations resulted in stabilizing of polymer production reaching 9.58 % PHB of dcw and only 12.0 % loss of production ability. Further experiments increased PHB content with 19.9% PHB of dcw (3.85 g L-1) after 180 h of cultivation using rich medium. Use of another thiolase gene, the second thiolase from C. necator (bktB), which theoretically should induce production of PHBV copolymer, led to accumulation only 11.4% PHB of dcw after 139 h and no PHV fraction was detected.
Variation of the ratio between flask volume and amount of media influences the level of aeration. Importantly, decrease of aeration level significantly increased polymer synthesis. Additionally, PHB-V copolymer accumulation has been induced by use of different carbon source co-substrates. Use of rich media supplemented with ethanol allow the strain with thl thiolase to accumulate up to 42.9 % PHB of dcw without PHV fraction and with bktB thiolase to 30.5 % PHB of dcw. Nevertheless, despite of lower total amount of polymer, supplementation with 1-propanol allow both strains to accumulate PHB-V copolymer with 7.30 %mol and 22.5 %mol of PHV for thl and bktB strains, respectively.
Optimization based on genetic engineering further enhanced polymer production yield led to exceeding of 50 % PHB-V of dcw. For doubling the gene dosage, PHA synthesizing strains of A. adeninivorans were again transformed with Xplor2 vector containing PHA pathway genes. Resulting strains exhibited twice the level of enzymatic activities of thiolase and reductase compared with strains transformed once with expression vector. In a shaking flask experiment the strain transformed twice with vector containing bktB thiolase reached after 240 h 52.1% PHB-V of dcw (10.8 g L-1) with 12.3 %mol of PHV fraction which is the highest level found in yeast. As another genetic approach, a fusion strain has been created. Two different strains have been established and merged using protoplast fusion technique. Doubling of genetic material resulted in similar level of copolymer produced by Arxula as in former experiments (50.2% of dcw, 10.7 g L-1).
Culture conditions were optimized in controllable cultivation using fed-batch mode. Although optimal oxygen and pH level and continuous carbon source and nitrogen feeding were maintained, final polymer level in % of dry mass was around three times lower than for shaking flask experiment. Nevertheless, efficient growth of Arxula in fed-batch mode led to increase of total copolymer level in g L-1 (16.5 g L-1 compare to 10.8 g L-1 for shaking flasks) showing the feasibility of using Arxula strain for up-scaling production of copolymer.
Acetyl-CoA is a main precursor in synthesis of PHB-V copolymer and change of its pool was investigated. ATP citrate lyase is a cytosolic enzyme converting citrate into oxaloacetate and acetyl-CoA, supporting the biosynthesis of fatty acids. Two genes encoding Acl subunits from Aspergillus nidulans (AnAcl1 and AnAcl2) were again cloned into Xplor2 vector and transformed into A. adeninivorans PHA producing strain. Despite of higher enzymatic activity of AnAclp, accumulation of polymer was around three times higher for control without expression of lyase genes. Expectedly, the strain expressing AnAcl1/2 genes accumulated larger amount of each stearic, palmitic and oleic acid in both standard and fatty acid inducing conditions (lower nitrogen level). Thus, overexpression of AnAcl1/2 genes in A. adeninevorans cells may improve biosynthesis of fatty acids but is ineffective for PHB polymer accumulation.
The aim of the project was use of starch-based media, manufactured as by-products, for polymer production. Genetically engineered Arxula strains were cultivated using these media instead of glucose-based media. Although yeast cells were both able to secrete (R)-3-HB and to accumulate PHB, the yield was lower than for previous media. Additionally, only trace of PHV was found at the end of cultivation time when 1-propanol was supplemented. Obtained results showed that use of cheaper media is a promising approach to decrease production costs but further optimization needs to be performed especially for extended scale of production.
Determination of produced copolymer has been done based on microscopic analysis and studies of physical and chemical properties. Results revealed that Arxula accumulated PHA polymer in cytosolic granules with a similar size range compared to the ones produced by bacteria. The physicochemical study showed that produced polymer exhibited slightly different properties in comparison to bacterial polymer with similar content of PHV, i.e. very-low molecular mass, higher melting and glass transition temperature.
All above results showed that A. adeninivorans is a promising host for PHB-V production. Expression of phasin greatly increased production and stability of polymer, which led to an accumulation level never found before in yeast. Further optimization in higher production scale using cheap starch-based media may establish Arxula strain as a valuable tool for industrial production of PHB-V copolymer.
Hintergrund: Patientennahe und klinische Erhebungsverfahren zur Dokumentation des Therapieverlaufs und des Behandlungserfolgs in der Ambulanten Muskuloskeletalen Rehabilitation (AMR) wurden bisher nur selten im direkten Vergleich hinsichtlich ihrer methodischen Eigenschaften, speziell der Änderungssensitivität untersucht. Für die angemessene Beurteilung medizinischer Behandlungsergebnisse sind begründete Empfehlungen für die Auswahl und Beurteilung geeigneter Outcomeparameter erforderlich.
Ziel: Die methodischen Eigenschaften verschiedener klinischer Messverfahren (Bestimmung der Gelenkbeweglichkeit und des Fingerbodenabstands mit der Neutral-Null-Methode, manuelle Muskelkraftmessung, Gehstreckenbestimmung mit dem Laufband, Staffelsteinscore Version Hüfte und Knie, Harris Hip Score und Knee Society Score) werden ermittelt und verglichen. Es wird untersucht, ob und wie gut diese Erhebungsmethoden mit generischen (SF-36) und spezifischen (WOMAC® bzw. Funktionsfragebogen Hannover-Rückenschmerz) patientennahen Erhebungsinstrumenten korrespondieren. Neben den klassischen Haupt- und Nebentestgütekriterien und den Verteilungseigenschaften wird die Änderungssensitivität bestimmt, um zu zeigen, wie gut die Instrumente in der Lage sind, Veränderungen über die Zeit abzubilden.
Methoden: In die prospektive unkontrollierte Längsschnittstudie mit indirekter Ergebnismessung im Vorher-Nachher-Design werden alle Patienten konsekutiv eingeschlossen, welche vom 1.1.2013 bis zum 1.5.2014 eine durchschnittlich dreiwöchige Ambulante Muskuloskeletale Rehabilitation nach Hüft- oder Knie-TEP-Implantation sowie nach operativer oder konservativer Behandlung der Claudicatio spinalis im Zentrum für Rehabilitation und Physikalische Medizin am SRH Zentralklinikum Suhl absolviert haben. Bei Aufnahme (T1) und bei Entlassung (T2) werden die o.g. klinischen Erhebungsverfahren, anamnestische Daten und bei den Patienten mit Claudicatio spinalis der Funktionsfragebogen Hannover-Rückenschmerz, bei den Endoprothese-Patienten der WOMAC® und bei allen Patienten der SF-36 bestimmt. Die Datenerhebung für WOMAC® und SF-36 erfolgt per Selbstauskunft, mit Hilfe oder als Interview. Die Reliabilität der patientennahen Instrumente wird für T1 mit Cronbach's alpha ermittelt. Für die Änderungssensitivität werden Effektstärken und Standardisierte Mittelwertdifferenzen berechnet und miteinander verglichen. Mit der Pearson-Korrelation wird gemessen, ob und wie gut die Ergebnisse der verschiedenen klinischen Befunde untereinander und mit den patientennahen Erhebungsverfahren korrelieren.
Ergebnisse: Die Untersuchung umfasst 175 Patienten (40 nach Hüft- und 60 nach Knie-TEP-Implantation sowie 75 mit Claudicatio spinalis, davon 24 nach einer Operation). Im Verlauf der Ambulanten Muskuloskeletalen Rehabilitation zeigen sich unabhängig von Alter, Geschlecht und Bildungsstand positive Veränderungen. Die heterogene Datenerhebung mittels schriftlicher Befragung bzw. durch ein persönliches Interview für SF-36 und WOMAC® hat keinen Einfluss auf das Untersuchungsergebnis. Wegen der unterschiedlichen Ergebnisse für die beiden Indikatoren zur Bestimmung der Änderungssensitivität (Effektstärke und Standardisierte Mittelwertdifferenz) ist es empfehlenswert, in Studien grundsätzlich beide Kennziffern zu berechnen und zu vergleichen. Die höchste Änderungssensitivität aller Instrumente erzielt der Staffelsteinscore in beiden Versionen (Knie stärker als Hüfte). Die Änderungssensitivität der anderen klinischen Erhebungsverfahren und des Funktionsfragebogen Hannover-Rückenschmerz fällt schwächer aus, ist aber der Änderungssensitivität der körperlichen Domänen des SF-36 überlegen, die wiederum eine höhere Änderungssensitivität aufweisen als die WOMAC® Subscores. Am niedrigsten ist die Änderungssensitivität für die Psychischen Domänen des SF-36. Es gibt keine relevanten Antwortausfälle. Nennenswerte Boden- und Deckeneffekte zeigen testimmanent nur der WOMAC® und für die Items "Körperliche Rollenfunktion", "Emotionale Rollenfunktion" und "Soziale Funktionsfähigkeit" der SF-36. Der Funktionsfragebogen Hannover-Rückenschmerz, der WOMAC® und mit Einbußen der SF-36 für die Items "Schmerz" und "Allgemeine Gesundheitswahrnehmung" sind zum Zeitpunkt T1 hoch reliabel (Cronbach's alpha). Mit der Pearson-Korrelation wird außer für die Hüft-TEP-Patienten in T1 die Konstruktvalidität für den SF-36 und den WOMAC® bestätigt. Als mögliche Fehlerursache kann nur die geringe Anzahl von 40 Hüft-TEP-Patienten identifiziert werden. Staffelsteinscore, Harris Hip Score und Knee Society Score korrelieren miteinander hochsignifikant. Der Staffelsteinscore ist in beiden Versionen bezüglich der Nebentestgütekriterien und der Korrelation mit dem SF-36 bzw. dem WOMAC® dem Harris Hip Score und dem Knee Society Score insgesamt leicht überlegen. Bei Patienten mit Claudicatio spinalis korrelieren Gehstrecke, Funktionsfragebogen Hannover-Rückenschmerz und mit Abstrichen für T1 der Fingerbodenabstand hochsignifikant miteinander und auch akzeptabel mit dem SF-36. Die Gehstrecke der Totalendoprothese-Patienten korreliert eher weniger gut mit allen Kenngrößen.
Diskussion: Im Ergebnis der Studie kann zur Evaluation und Dokumentation der Prozess- und Ergebnisqualität in der Ambulanten Muskuloskeletalen Rehabilitation für Endoprothese-Patienten die Verwendung des Staffelsteinscores in beiden Versionen und für Patienten mit Claudicatio spinalis die Bestimmung des Funktionsfragebogen Hannover-Rückenschmerz, der Gehstrecke und des Fingerbodenabstandes empfohlen werden. Dabei sind die differierenden Ergebnisse der Hüft-TEP-Patienten zum Zeitpunkt T1 und die heterogene Form der Datenerhebung für SF-36 und WOMAC® zu berücksichtigen. Die Kombination des Staffelsteinscores mit dem Harris Hip Score für die Hüft-TEP- und mit dem Knee Society Score für die Knie-TEP-Patienten bringt keinen Vorteil. Der Einsatz des SF-36 und des WOMAC® in der Routine ist nicht sinnvoll, genau wie das Messen der Gehstrecke mit dem Laufband bei den Endoprothese-Patienten. Weitere Studien an größeren Patientengruppen, im Einrichtungsvergleich, mit praktikableren patientenberichteten Endpunkten für die Endoprothese-Patienten und mit spezifischeren patientennahen Erhebungsverfahren für die Patienten mit Claudicatio spinalis sind genauso notwendig wie Untersuchungen mit anderen Assessmentinstrumenten der Mobilität sowie zur Bestimmung der Gehstrecke in anderer Art und Weise.
In aktuellen Studien aus den letzten Jahren mehren sich die Hinweise auf metabolische Einflüsse
von Osteocalcin. Neben dem Skelettsystem scheint besonders der Energiestoffwechsel,
speziell auf der Ebene der Distribution und des Verbrauchs von Glucose mit Osteocalcin
zu interagieren. In der vorliegenden Studie wurden Blutplasma- und Urinproben von 931
gesunden Probanden mittels Massenspektrometrie (Tandem-Massenspektroskopie mit vorgeschalteter
Flüssigkeitschromatographie) auf niedermolekulare Substanzen untersucht, um
dann auf systemische Effekte von Osteocalcin zu schließen. Diese Methodik erlaubt eine
breite Untersuchung von Wirkungen von Osteocalcin in allen Organsystemen, auch in jenen,
für die bisher noch keine Interaktionen mit Osteocalcin bekannt sind. Die Berechnung eines
ersten Modells zeigte viele Zusammenhänge. Diese waren jedoch stark durch die Nierenfunktion
beeinflusst. Nach Adjustierung für die Nierenfunktion blieben insgesamt 29 signifikante
Ergebnisse erhalten. Zu diesen Ergebnissen zählten zuvorderst Zwischenprodukte des
Kollagenstoffwechsels, besonders Prolinderivate, was die Bedeutung von Osteocalcin im
Knochenstoffwechsel unterstreicht. Die weiteren Ergebnisse umfassten eine Assoziation mit
Kynurenin, ein Hinweis auf die Möglichkeit, dass Entzündungen Einflüsse auf zirkulierendes
Osteocalcin haben könnten. Weitere Hinweise auf die bereits bekannte Verknüpfung zwischen
dem Energiestoffwechsel und Osteocalcin bietet die vorliegende Studie durch die Detektion
einer Assoziation zwischen Osteocalcin und Abbauprodukten von verzweigtkettigen
Aminosäuren. Auch scheint Osteocalcin vom Lebensstil, wie beispielsweise dem Tabakrauchen,
beeinflusst zu werden. Zusammenfassend bietet die vorliegende Studie einen umfassenden
Überblick über die metabolischen Einflüsse von Osteocalcin. Darin war eine Vielzahl
von Assoziationen nachweisbar, die jedoch insgesamt für eine eher geringe Rolle von Osteocalcin
im menschlichen Stoffwechsel sprechen.
The obligate anaerobe, spore forming bacterium Clostridioides difficile (formerly Clostridium difficile) causes nosocomial and community acquired diarrhea often associated with antibiotic therapy. Major virulence factors of the bacterium are the two large clostridial toxins TcdA and TcdB. The production of both toxins was found strongly connected to the metabolism and the nutritional status of the growth environment. Here, we systematically investigated the changes of the gene regulatory, proteomic and metabolic networks of C. difficile 630Δerm underlying the adaptation to the non-growing state in the stationary phase. Integrated data from time-resolved transcriptome, proteome and metabolome investigations performed under defined growth conditions uncovered multiple adaptation strategies. Overall changes in the cellular processes included the downregulation of ribosome production, lipid metabolism, cold shock proteins, spermine biosynthesis, and glycolysis and in the later stages of riboflavin and coenzyme A (CoA) biosynthesis. In contrast, different chaperones, several fermentation pathways, and cysteine, serine, and pantothenate biosynthesis were found upregulated. Focusing on the Stickland amino acid fermentation and the central carbon metabolism, we discovered the ability of C. difficile to replenish its favored amino acid cysteine by a pathway starting from the glycolytic 3-phosphoglycerate via L-serine as intermediate. Following the growth course, the reductive equivalent pathways used were sequentially shifted from proline via leucine/phenylalanine to the central carbon metabolism first to butanoate fermentation and then further to lactate fermentation. The toxin production was found correlated mainly to fluxes of the central carbon metabolism. Toxin formation in the supernatant was detected when the flux changed from butanoate to lactate synthesis in the late stationary phase. The holistic view derived from the combination of transcriptome, proteome and metabolome data allowed us to uncover the major metabolic strategies that are used by the clostridial cells to maintain its cellular homeostasis and ensure survival under starvation conditions.
Mast cells reside on and near the cerebral vasculature, the predominant site of pneumococcal entry into the central nervous system (CNS). Although mast cells have been reported to be crucial in protecting from systemic bacterial infections, their role in bacterial infections of the CNS remained elusive. Here, we assessed the role of mast cells in pneumococcal infection in vitro and in vivo. In introductory experiments using mouse bone marrow-derived mast cells (BMMC), we found that (i) BMMC degranulate and release selected cytokines upon exposure to Streptococcus pneumoniae, (ii) the response of BMMC varies between different pneumococcal serotypes and (iii) is dependent on pneumolysin. Intriguingly though, apart from a slight enhancement of cerebrospinal fluid (CSF) pleocytosis, neither two different mast cell-deficient Kit mutant mouse strains (WBB6F1-KitW/Wv and C57BL/6 KitW-sh/W-sh mice) nor pharmacologic mast cell stabilization with cromoglycate had any significant impact on the disease phenotype of experimental pneumococcal meningitis. The incomplete reversal of the enhanced CSF pleocytosis by local mast cell engraftment suggests that this phenomenon is caused by other c-Kit mutation-related mechanisms than mast cell deficiency. In conclusion, our study suggests that mast cells can be activated by S. pneumoniae in vitro. However, mast cells do not play a significant role as sentinels of pneumococcal CSF invasion and initiators of innate immunity in vivo.
We aimed to evaluate the outcome of different treatment modalities for extremity venous thrombosis (VT) in neonates and infants, highlighting the current debate on their best tool of management. This retrospective study took place over a 9-year period from January 2009 to December 2017. All treated patients were referred to the vascular and pediatric surgery departments from the neonatal intensive care unit. All patients underwent a thorough history-taking as well as general clinical and local examination of the affected limb. Patients were divided into 2 groups: group I included those who underwent a conservative treated with the sole administration of unfractionated heparin (UFH), whereas group II included those who were treated with UFH plus warfarin. Sixty-three patients were included in this study. They were 36 males and 27 females. Their age ranged from 3 to 302 days. Forty-one (65%) patients had VT in the upper limb, whereas the remaining 22 (35%) had lower extremity VT. The success rate of the nonsurgical treatment was accomplished in 81% of patients. The remaining 19% underwent limb severing, due to established gangrene. The Kaplan-Meier survival method revealed a highly significant increase in both mean and median survival times in those groups treated with heparin and warfarin compared to heparin-only group (P < .001). Nonoperative treatment with anticoagulation or observation (ie, wait-and-see policy) alone may be an easily applicable, effective, and a safe modality for management of VT in neonates and infants, especially in developing countries with poor or highly challenged resource settings.
One of the major problems in the study of the dynamics of proteins is the visualization of changing conformations that are important for processes ranging from enzyme catalysis to signaling. A protein exhibiting conformational dynamics is the soluble blood protein beta 2-glycoprotein I (beta2GPI), which exists in two conformations: the closed (circular) form and the open (linear) form. It is hypothesized that an increased proportion of the open conformation leads to the autoimmune disease antiphospholipid syndrome (APS). A characteristic feature of beta2GPI is the high content of lysine residues. However, the potential role of lysine in the conformational dynamics of beta2GPI has been poorly investigated. Here, we report on a strategy to permanently open up the closed protein conformation by chemical acetylation of lysine residues using acetic acid N-hydroxysuccinimide ester (NHS-Ac). Specific and complete acetylation was demonstrated by the quantification of primary amino groups with fluoraldehyde o-phthalaldehyde (OPA) reagent, as well as western blot analysis with an anti-acetylated lysine antibody. Our results demonstrate that acetylated beta2GPI preserves its secondary and tertiary structures, as shown by circular dichroism spectroscopy. We found that after lysine acetylation, the majority of proteins are in the open conformation as revealed by atomic force microscopy high-resolution images. Using this strategy, we proved that the electrostatic interaction of lysine residues plays a major role in stabilizing the beta2GPI closed conformation, as confirmed by lysine charge distribution calculations. We foresee that our approach will be applied to other lysine-rich proteins (e.g. histones) undergoing conformational transitions. For instance, conformational dynamics can be triggered by environmental conditions (e.g. pH, ion concentration, post-translational modifications, and binding of ligands). Therefore, our study may be relevant for investigating the equilibrium of protein conformations causing diseases.
Medical education research has focused almost entirely on the education of future physicians. In comparison, findings on other health-related occupations, such as medical assistants, are scarce. With the current study, we wanted to examine the knowledge-is-power hypothesis in a real life educational setting and add to the sparse literature on medical assistants. Acquisition of vocational knowledge in vocational education and training (VET) was examined for medical assistant students (n = 448). Differences in domain-specific vocational knowledge were predicted by crystallized and fluid intelligence in the course of VET. A multiple matrix design with 3 year-specific booklets was used for the vocational knowledge tests of the medical assistants. The unique and joint contributions of the predictors were investigated with structural equation modeling. Crystallized intelligence emerged as the strongest predictor of vocational knowledge at every stage of VET, while fluid intelligence only showed weak effects. The present results support the knowledge-is-power hypothesis, even in a broad and more naturalistic setting. This emphasizes the relevance of general knowledge for occupations, such as medical assistants, which are more focused on learning hands-on skills than the acquisition of academic knowledge.
Die therapeutische Anwendung kalter Atmosphärendruckplasmen (CAP) ist der zentrale Schwerpunkt des in einer hoch interdisziplinären Schnittstelle zwischen Natur- und Lebenswissenschaften angesiedelten Forschungsgebiets der Plasmamedizin. Trotz eines auf Basis grundlagentechnologischer und anwendungsorientierter Forschung zunehmenden Erkenntnisgewinns, wirft die Plasmamedizin sowohl bezüglich spezifischer Wirkeffekte als auch Risikopotentiale nach wie vor verschiedene Fragestellungen auf und zeigt, vor dem Hintergrund einer nicht ausschließlich auf Grundlage von in vitro Daten zu prognostizierenden unbedenklichen in vivo Anwendbarkeit, vor allem ein Defizit an klinischen Daten.
Im Rahmen der vorliegenden Arbeit erfolgte erstmals eine systematische Langzeituntersuchung eines unter standardisierten Bedingungen mittels kaltem atmosphärischen Plasma behandelten Probandenkollektivs. Die 5 Probanden waren 5 Jahre zuvor Teil einer Untersuchung zum CAP-Einfluss auf die Wundheilung CO2-Laser-induzierter Hautläsionen, im Zuge derer jeweils 3 der 4 Läsionen mit unterschiedlichen Applikationsschemata (1 x 10 s; 1 x 30 s; 3 x 10 s) mittels des kINPen® MED behandelt wurden.
Neben einer klinischen Evaluation wurde die Nachuntersuchung durch aufwendige, bildgebende Diagnostika wie Dermatoskopie, konfokale Laserscanmikroskopie (KLSM) und Hyperspectral Imaging (HSI) assistiert sowie fragebogengestützt hinsichtlich plasmavermittelter Hautveränderungen (POSAS) und einer möglichen therapieassoziierten Beeinträchtigung der Lebensqualität (DLQI) ergänzt.
Im Ergebnis konnten weder klinisch-makroskopisch, dermatoskopisch, noch mittels KLSM oder HSI malignitätssuspekte Veränderungen, Anzeichen chronischer Entzündungsreaktionen, pathologische Modifikationen der Zellarchitektur und Zellmorphologie oder mikrozirkulatorische Pathologien in den plasmabehandelten Arealen, gegenüber dem umliegenden, nicht mittels Kaltplasma therapierten Gewebe, belegt werden.
Die in dieser Komplexität im Rahmen einer Langzeituntersuchung am Menschen bisher einzigarten Daten tragen, insbesondere auf dem hochaktuellen Diskussionsfeld plasmaassoziierter Risikopotentiale, zu einem weiteren Erkenntnisgewinn bei. Zur Sicherung indikationsspezifischer Dosisempfehlungen und Behandlungsfenster bedarf es, aufgrund verschiedener allgemeiner und spezieller Limitationen, jedoch weiterer klinischer Studien.
Ion thrusters are Electric Propulsion systems used for satellites and space missions. Within
this work, the High Efficient Multistage Plasma Thruster (HEMP-T), patented by the
THALES group, is investigated. It relies on plasma production by magnetised electrons.
Since the confined plasma in the thruster channel is non-Maxwellian, the near-field plume
plasma is as well. Therefore, the Particle-In-Cell method combined with a Monte-Carlo
Collision model (PIC-MCC) is used to model both regions. In order to increase the sim-
ulated near-field plume region, a non-equidistant grid is utilised, motivated by the lower
plasma density in the plume. To minimise artificial self-forces at grid points bordered by
cells of different size a modified method for the electric field calculation was developed in
this thesis. In order to investigate the outer plume region, where electric field and collisions
are negligible, a ray-tracing Monte-Carlo model is used. With these simulation methods,
two main questions are addressed in this work.
What are the basic mechanisms for plasma confinement, plasma-wall-interaction
and thrust generation?
For the HEMP-T the plasma is confined by magnetic fields in the thruster channel, generated
by cylindrical permanent magnets with opposite polarity. Due to different Hall parameters,
electrons are magnetised, while ions are not. Therefore, the dominating electron transport
is parallel to the magnetic field lines. In the narrow cusp regions, the magnetic mirror effect
reduces the electron flux towards the wall and confines the electrons like in a magnetic
bottle. At the anode, propellant gas streams into the thruster channel, which gets ionised
by the electrons creating the plasma. As a result of the electron oscillation between the two
cusp regions, ionisation of the propellant gas is efficient.
The magnetic field configuration of the HEMP-T also influences the plasma potential inside
the thruster channel. Close to the symmetry axis, the mainly axial magnetic field results in
a flat potential. At the inner wall, the field configuration reduces the plasma wall interaction
to only the narrow cusp regions. Here, the floating potential of the dielectric channel wall
and its plasma sheath result in a rather low radial potential drop compared to the applied
anode potential. As a result, the electric potential is rather flat and impinging ions at the
thruster channel wall have energies below the sputter threshold energy of the wall material.
Therefore, no sputtering appears at the dielectric wall. At the thruster exit the confinement
by the magnetic field is weakened and the potential drops with nearly the full anode voltage.
The resulting electric field accelerates the generated ions into the plume and generate the
thrust, but they are also able to sputter surfaces. During terrestrial testing, sputteringat vacuum vessel walls leads to the production of impurities. The amount of back-flux
towards the channel exit is determined by the sputter yield of the vacuum chamber wall. A
large distance between thruster exit and vessel wall reduces the back-flux and smooths the
pattern of deposition inside the thruster channel. Dependent on their material, the evolving
deposited layers can get conductive, modify by this the potential distribution and reduce
the thrust.
For the HEMP-T, ions are mainly generated at high potential close to the applied anode
potential. Therefore, the accelerated ions producing the thrust gain the maximum energy
as observed in experiment. Ions emitted from the thruster into different angles in the
plume contribute mainly to the ion current at angles between 30 ◦ and 90 ◦ . They mainly
originate from ionisation at the thruster exit. The resulting angular distribution of the
ejected ion current is close to the one of the experiment, slightly shifted by about ten
degrees to higher emission angles. In front of the thruster exit, electrons are trapped by
electrostatics forces. This enhanced density allows ionisation and an additional electron
density structure establishes.
What are possible physics based ideas for optimisation of an ion thruster?
An optimised thruster should have a high ionisation rate inside the thruster channel, low
erosion and an ion angular distribution with small contributions at high angles for min-
imised thruster satellite interactions. In experiments, the HEMP-T satisfies already quite
nicely these requests. In the simulations, low erosion inside the thruster channel and angular
ion distributions close to the experimental data are demonstrated. However, the ionisation
efficiency is lower and radial ion losses are larger than in experiment. A possible explanation
of these differences is an underestimated transport perpendicular to the magnetic field lines,
well known for magnetised plasmas.
A successful example for an optimisation using numerical simulations is the reduction of
back-flux of sputtered impurities during terrestrial experiments by an improved set-up of
the vacuum vessel. The implementation of baffles reduces the back-flux towards the thruster
exit and therefore deposition inside the channel. These improvements were successfully im-
plemented in the experiment and showed a reduction of artefacts during long time measure-
ments. This leads to a stable performance, as it is expected in space.
