Fixateur interne bei Frakturen der unteren Brust- und Lendenwirbelsäule : Analyse und Bewertung
- Ziel dieser Arbeit war die Darstellung der Indikationen und Grenzen der dorsalen Stabilisierung. Dabei wurden 88 Patienten (52 Männer, 36 Frauen, Alter 9-66) mit einer Fraktur der unteren Brust- oder Lendenwirbelsäule untersucht, die mit dieser Methode operiert wurden. Die neurologischen Ausfalle, die radiologischen Befunde (Kyphosewinkel, Keilwinkel, Skoliosewinkel, achsiale Kompression, Spinalkanalweite) und die postoperativen Komplikationen sind als vergleichbare Parameter in einem Langzeitverlauf von 3 Jahren ausgewertet worden. Hierbei boten 50 Patienten keine präoperativen neurologischen Ausfalle, 6 Patienten eine radikuläre Symptomatik, 26 eine inkomplette und 6 Patienten eine komplette Querschnittsymptomatik. Abgesehen der Patienten mit der kompletten Querschnittlähmung zeigten alle Patienten mit dem inkompletten Transversalsyndrom eine Verbesserung der neurologischen Defizite. Als Komplikationen traten in 4,5% Pedikelschraubenbruch, 6,8% Trägerbruch, 6,8% Schraubenfehllage und 2,2% Sekundärinstabilität nach Fixateurentfernung auf. Die Durchführung einer zusätzlichen intrakorporalen Spongiosaplastik konnte keine signifikant besseren Ergebnisse erzielen. Aufgrund der Vorteile (Reposition, Dekompression und Stabilisierung als ein Eingriff; geringe Zugangsmorbidität; Winkelstabilität) und der in der Studie gewonnenen Ergebnisse kann die dorsale Spondylodese als ein suffizientes Verfahren und als bevorzugte Methode bei den genanten Verletzungen empfohlen werden.
- In this study, we investigated the indications and limitations of dorsal stabilization. We included 88 patients (52 males, 36 females, aged 9-66 years) with a fracture of the lower thoracic or lumbar spine treated with this technique. Neurological deficits, radiological results (including the angle of kyphosis, wedge angle, angle of scoliosis, axial compression, width of spinal canal) and postoperative complications were analysed over a period of 3 years. In 50 patients, there had not been any preoperative neurological deficits, 6 patients had shown radicular symptoms, 26 patients had suffered from an incomplete and 6 patients from a complete paraplegia. All patients with incomplete paraplegia showed a significant improvement of the neurological symptoms after treatment. Complications included broken pedicular screws in 4.5%, broken rods in 6.8%, dislocation of screws in 6.8% and secondary instability after removal of fixateur in 2.2%. Additional transpedicular bone grafting did not result in significant improvement of outcome. Summary: Posterior instrumentation has several advantages, including the combination of repositioning, decompression and stabilisation in one intervention, a low morbidity associated with the surgical approach and good angle stability. Therefore, considering the results of this study, it must be regarded as an efficient surgical technique and should be recommended as the method of choice in patients with fractures of lower thoracic or lumbar spine.
