Predicting complications in pediatric Crohn's disease patients followed in CEDATA-GPGE registry

Background
Complications of Crohn's disease (CD) often impair patients' quality of life. It is necessary to predict and prevent these complications (surgery, stricturing [B2]/penetrating [B3] disease behavior, perianal disease, growth retardation and hospitalization). Our study investigated previously suggested and additional predictors by analyzing data of the CEDATA-GPGE registry.
Methods
Pediatric patients (< 18 years) diagnosed with CD with follow up data in the registry were included in the study. Potential risk factors for the selected complications were evaluated by performing Kaplan-Meier survival curves and cox regression models.
Results
For the complication surgery, the potential risk factors older age, B3 disease, severe perianal disease and initial therapy with corticosteroids at the time of diagnosis were identified. Older age, initial therapy with corticosteroids, low weight-for-age, anemia and emesis predict B2 disease. Low weight-for-age and severe perianal disease were risk factors for B3 disease. Low weight-for-age, growth retardation, older age, nutritional therapy, and extraintestinal manifestations (EIM) of the skin were identified as risk factors for growth retardation during the disease course. High disease activity and treatment with biologicals were predictors for hospitalization. As risk factors for perianal disease, the factors male sex, corticosteroids, B3 disease, a positive family history and EIM of liver and skin were identified.
Conclusion
We confirmed previously suggested predictors of CD course and identified new ones in one of the largest registries of pediatric CD patients. This may help to better stratify patients’ according to their individual risk profile and choose appropriate treatment strategies.
Background
The COVID-19 pandemic and the imposed lockdowns severely affected routine care in general and specialized physician practices.
Objective
To describe the long-term impact of the COVID-19 pandemic on the physician services provision and disease recognition in German physician practices and perceived causes for the observed changes.
Design
Observational study based on medical record data and survey data of general practitioners and specialists' practices.
Participants
996 general practitioners (GPs) and 798 specialist practices, who documented 6.1 million treatment cases for medical record data analyses and 645 physicians for survey data analyses.
Main measures
Within the medical record data, consultations, specialist referrals, hospital admissions, and documented diagnoses were extracted for the pandemic (March 2020–September 2021) and compared to corresponding pre-pandemic months in 2019. The additional online survey was used to assess changes in practice management during the COVID-19 pandemic and physicians' perceived main causes of affected primary and specialized care provision.
Main results
Hospital admissions (GPs: −22% vs. specialists: −16%), specialist referrals (−6 vs. −3%) and recognized diseases (−9 vs. −8%) significantly decreased over the pandemic. GPs consultations initially decreased (2020: −7%) but compensated at the end of 2021 (+3%), while specialists' consultation did not (−2%). Physicians saw changes in patient behavior, like appointment cancellation, as the main cause of the decrease. Contrary to this, they also mentioned substantial modifications of practice management, like reduced (nursing) home visits (41%) and opening hours (40%), suspended checkups (43%), and delayed consultations for high-risk patients (71%).
Conclusion
The pandemic left its mark on primary and specialized healthcare provision and its utilization. Both patient behavior and organizational changes in practice management may have caused decreased and non-compensation of services. Evaluating the long-term effect on patient outcomes and identifying potential improvements are vital to better prepare for future pandemic waves.
Somatoforme Störungen, Depression, Angststörungen und Alexithymie stellen eine hohe Belastung bei herzkranken Patienten dar. Aktuelle Studien weisen auf die Relevanz des frühzeitigen Erkennens dieser Komorbiditäten hin. In der vorliegenden Studie wurden 105 Patienten der Klinik für Kardiologie der Universitätsmedizin Greifswald im Zeitraum von April bis Oktober 2010 unter Verwendung des „Goldstandards“ DIA-X, welches aus einem „Zwei-Phasen-Design“ besteht, untersucht. Bei positivem Screening (SSQ/ASQ) erfolgte ein computerassistiertes standardisiertes diagnostisches Interview auf somatoforme Störungen bzw. Depression. Die Angststörungen wurden aufgrund der Zumutbarkeit lediglich als Screening erfasst. Mittels TAS-20 wurde auf das Vorliegen einer alexithymen Persönlichkeit untersucht. Laut aktueller Forschung begünstigen die einzelnen Störungen bzw. Merkmale und die kardiale Erkrankung ihr gegenseitiges Auftreten als Vulnerabilitätsfaktoren. Ein kausaler Zusammenhang ist nicht bekannt. Als Teil eines größeren Projektes fand die Untersuchung zeitgleich in der dermatologischen und neurologischen Klinik der Universitätsmedizin Greifswald statt. Bei spärlicher Literatur bezüglich des subjektiven Unterstützungswunsches wurde dieser mittels Fragebogen erhoben und indirekt zur Ermittlung eines Zusammenhangs zwischen dem subjektiven und objektiven Gesundheitszustande genutzt, welcher laut Literaturangaben nicht besteht. Eine komorbiden psychische Störung verlängere Studien zufolge die stationäre Aufenthaltsdauer. Unter diesen Annahmen lag eine hohe Prävalenz psychischer Störungen in der Kardiologie vor (somatoformen Störungen 44%; Depression 33%; Angststörungen 44%). Eine Alexithymie wurde bei 7% der Patienten erfasst. Im Vergleich der drei Kliniken konnte ein signifikanter Unterschied bezüglich des Vorliegens einer somatoformen Störung (p<.0001**) und einer affektiven Störung (p=.002*) ermittelt werden. Ein signifikanter Zusammenhang mit dem Grad der psychischen Störung und dem Vorliegen (p=.013*) einer Alexithymie konnte gezeigt werden. Der subjektive Unterstützungsbedarf wurde von 38% der Patienten angegeben. Es lag kein höherer Unterstützungswunsch bei Patienten mit komorbider psychischer Erkrankung bzw. Multimorbidität vor. Es besteht keine Assoziation zwischen dem objektiven und subjektiven Gesundheitszustand. Bezüglich der Dauer des stationären Aufenthaltes, wiesen Patienten mit komorbider psychischer Störung einen um zwei Tage verlängerten Krankenhausaufenthalt auf (p=.031*).
