Colonic atresia (CA) is a rare disease with an incidence range between one of 20 000 and one of 66 000 live births. Most CA are located within the proximal colon; distal CA are even rarer. Because of its rarity, another case shall be described herewith. A 37th week of pregnancy born child was noticed occurring multiple vomiting, a distended abdomen and additional whitish-bloody stool shortly thereafter. In the first operation, a double-barrel stoma was created. After sufficient weight gain and alignment of the stoma ends, a secondary anastomosis was created in the child after 2 months. The diagnosis can be made reliably on the basis of an X-ray and leads to a good outcome with prompt surgical intervention. However, accompanying malformations should always be considered.
Abstract
Background and purpose:Diagnosing a patient with headache as a migraineur is critical for state-of-the-art migrainemanagement. Screening tools are imperative means to improve the diagnostic yield in the primary care settings andspecialized clinics. This study aims to translate and assess the diagnostic accuracy of a German version of theID Migraine™as a widely used and efficient screening instrument.
Methods:
The Functional Assessment of Chronic Illness Therapy translation methodology was used to translate theoriginal three-itemID Migraine™, including a fourth question for aura, from the English language into the German language.Diagnostic accuracy of the GermanID Migraine™and predictors of false screening results were assessed among patientspresenting to a headache outpatient clinic of a tertiary care center in Germany over a 6-month period.
Results:
The translation procedure yielded a harmonized GermanID Migraine™and its diagnostic accuracy was assessedin 105 patients (80 female, 46.5+17.2 years of age), including 79 patients (75.2%) with migraine. The three-item GermanID Migraine™provides a sensitivity of 99%, specificity of 68%, and positive and negative predictive values of 90% and 95%,respectively, using a cutoff of2. Positive and negative predictive values in a general headache population are estimated tobe 74% and 98%, respectively. The aura question identified 18 out of 20 migraineurs with aura.
Conclusions:
The GermanID Migraine™is an accurate screening tool for migraine even in a challenging population of aspecialized outpatient clinic. Its diagnostic accuracy indicates a potential utility for screening in primary health care.
Über die Erfolge der modernen diagnostischen Verfahren (CT, MRT) und Therapien (Chirurgie, Chemotherapie, Radiotherapie) bei der Behandlung von Gliomen wird weiter intensiv diskutiert. Von Interesse ist deshalb der Vergleich von Patientenpopulationen, die vor und nach der Einführung der MRT und der Mikrochirurgie behandelt wurden. Deswegen wurde eine retrospektive statistische Analyse für alle Patienten mit einem Gliom durchgeführt, die zwischen den Jahren 1965 und 1974 (88 Patienten, Gruppe I) oder zwischen den Jahren 1986 und 1995 (249 Patienten, Gruppe II) an der Universitätsklinik Greifswald operiert wurden. Es erfolgte zwischen den beiden Gruppen eine deutliche Reduktion sowohl der perioperativen Morbidität und Mortalität als auch des Zeitintervalls vom Symptombeginn bis zur Behandlung. Es gab eine Verbesserung der postoperativen Überlebenszeit in der Gruppe II für Patienten mit low-grade Gliom. Jedoch hat sich die Prognose der high-grade Gliome von den 70er-Jahren bis zu den 90-er Jahren insgesamt kaum verändert.
