Background/Aims: To develop a clinically relevant immunocompetent murine model to study pancreatic cancer using two different syngeneic pancreatic cancer cell lines and to assess MRI for its applicability in this model. Methods: Two cell lines, 6606PDA and Panc02, were employed for the experiments. Cell proliferation and migration were monitored in vitro. Matrigel™ was tested for its role in tumor induction. Tumor cell growth was assessed after orthotopic injection of tumor cells into the pancreatic head of C57/BL6 mice by MRI and histology. Results: Proliferation and migration of Panc02 were significantly faster than those of 6606PDA. Matrigel did not affect tumor growth/migration but prevented tumor cell spread after injection thus avoiding undesired peritoneal tumor growth. MRI could reliably monitor longitudinal tumor growth in both cell lines: Panc02 had a more irregular finger-like growth, and 6606PDA grew more spherically. Both tumors showed local invasiveness. Histologically, Panc02 showed a sarcoma-like undifferentiated growth pattern, whereas 6606PDA displayed a moderately differentiated glandular tumor growth. Panc02 mice had a significantly shorter (28 days) survival than 6606PDA mice (50 days). Conclusion: This model closely mimics human pancreatic cancer. MRI was invaluable for longitudinal monitoring of tumor growth thus reducing the number of mice required. Employing two different cell lines, this model can be used for various treatment and imaging studies.
Chronic pancreatitis has long been thought to be mainly associated with immoderate alcohol consumption. The observation that only ∼10% of heavy drinkers develop chronic pancreatitis not only suggests that other environmental factors, such as tobacco smoke, are potent additional risk factors, but also that the genetic component of pancreatitis is more common than previously presumed. Either disease-causing or protective traits have been indentified for mutations in different trypsinogen genes, the gene for the trypsin inhibitor SPINK1, chymotrypsinogen C, and the cystic fibrosis transmembane conductance regulator (CFTR). Other factors that have been proposed to contribute to pancreatitis are obesity, diets high in animal protein and fat, as well as antioxidant deficiencies. For the development of pancreatic cancer, preexisting chronic pancreatitis, more prominently hereditary pancreatitis, is a risk factor. The data on environmental risk factors for pancreatic cancer are, with the notable exception of tobacco smoke, either sparse, unconfirmed or controversial. Obesity appears to increase the risk of pancreatic cancer in the West but not in Japan. Diets high in processed or red meat, diets low in fruits and vegetables, phytochemicals such as lycopene and flavonols, have been proposed and refuted as risk or protective factors in different trials. The best established and single most important risk factor for cancer as well as pancreatitis and the one to clearly avoid is tobacco smoke.
Das Pankreaskarzinom ist eine Erkrankung, welche durch ein aggressives Wachstum und Metastasierungsverhalten, eine späte klinische Manifestation und eine hohe Mortalität der Patienten gekennzeichnet ist. In den letzten Jahren rückte der Einfluss des Tumormikromilieus auf die Wachstumseigenschaften des Pankreaskarzinoms zunehmend in den Fokus der Forschung. Hierbei scheinen nicht nur die zellulären Bestandteile eine Rolle zu spielen, sondern auch Wachstumsfaktoren, die zu einer Modulation von Wachstums- und Metastasierungsbedingungen beitragen. Die lokale und systemische Bedeutung von TGF-beta 1 als Wachstumsfaktor für das Pankreaskarzinom scheint dabei ambivalent zu sein. In Abhängigkeit von der Tumorentität, seiner Konzentration und der Verteilung über einen bestimmten Zeitraum scheint er teilweise gegensätzliche Funktionen zu übernehmen. Zu evaluieren galt es, ob Stresshormone einen Einfluss auf die TGF-beta 1 Sekretion im Pankreaskarzinom ausüben. In vitro konnten wir in Versuchen mit Stresshormonen zeigen, dass die murine Pankreaszelllinie 6606-PDA sowohl unter dem α-Agonisten Norepinephrin, als auch unter dem beta-Agonisten Isoproterenol mit einer Steigerung der TGF-beta 1 Sekretion in einem zeit- und konzentrationsabhängigen Kotext reagiert. Dieser Effekt wurde nicht durch eine signifikant gesteigerte Proliferation verursacht. In vivo konnte dieser Effekt ebenfalls nachgewiesen werden, dahingehend, dass TGF-beta auch in den Tumoren der chronisch gestressten Versuchstiere, sowohl intrazellulär, als auch zytosolisch, erhöht war. Im Gegensatz hierzu waren die TGF-beta 1 Serumkonzentrationen der Versuchstiere unter Stressbedingungen tendenziell erniedrigt. In der Vergangenheit konnte bereits gezeigt werden, dass Mäuse unter Stressbedingungen größere Pankreastumore ausbilden. Ob die Größe der Tumoren unter Stressbedingungen mit der lokal gesteigerten TGF-β Sekretion direkt korreliert, sollte in zukünftigen Arbeiten untersucht werden. Desweiteren sollte in weiteren Arbeiten eine genaue Unterscheidung bezüglich der Steigerung der einzelnen Isoformen bzw. deren inaktiven Vorstufen unter Stressbedingungen getroffen werden. Insgesamt sind weitere Untersuchungen notwendig um das Interaktionspotential von Stresshormonen mit anderen Wachstumsfaktoren, Signalkaskaden und zelluläre Komponenten in Bezug auf die TGF-beta bzw- TGF-beta 1 Sekretion Pankreaskarzinoms näher zu verstehen. Bei Kenntnis der zugrundeliegenden molekularbiologioschen Vorgänge und Interaktionen innerhalb des Tumomikromilieus und deren Auswirkungen auf die Tumorbiologie des Pankreaskarzinoms ließe sich dessen Wachstumsverhalten gezielt beeinflussen. Somit wäre eine gezielte Therapie des Pankreaskarzinoms möglich, welche das Patientenüberleben verbessern könnte.
Background: Pancreatic ductal adenocarcinoma (PDAC) is the 4th leading cause of cancer death worldwide and compared to other malignancies its share in cancer mortality is expected to rise further. This is due to a lack of sensitive diagnostic tools that would permit earlier detection in a potentially curable stage and the very slow progress in finding effective drug treatments for pancreatic cancer. Key Messages: Aside from genetic predispositions and environmental agents, chronic pancreatitis is by far the greatest risk factor for PDAC. It also shares several etiological factors with pancreatic cancer and represents its most challenging differential diagnosis. Biomarkers that can distinguish between chronic pancreatitis and PDAC may therefore be suitable for the latter's early detection. Moreover, targeting the natural history of chronic pancreatitis would be one approach to prevent PDAC. Targeting tumor-cell signaling directly by interfering with receptor tyrosine kinases has shown some efficacy, although the results in clinical trials were less encouraging than for other cancers. Other compounds developed have targeted the formation of extracellular matrix around the tumor, the proteolytic activity in the tumor environment, histone deacetylases, hedgehog signaling and heat shock proteins, but none has yet found its way into routine patient care. Attempts to individualize treatment according to the tumor's somatic mutation profile are novel but so far impractical. Conclusions: Progress in the treatment of pancreatic cancer has been exceedingly slow and mostly dependent on improved pharmaceutical preparations or combinations of established chemotherapeutic agents. The promise of major breakthroughs implied in targeting tumor signal transduction events has so far not materialized.
Introduction: Recently, FOLFIRINOX and gemcitabine + nab-paclitaxel have been introduced as a novel intensified chemotherapy regimen for patients with metastasized pancreatic cancer. This study aims to analyze the real-world clinical practice with FOLFIRINOX and gemcitabine + nab-paclitaxel across Europe. Methods: Invitations to participate in an anonymous web-based questionnaire were sent via e-mail to 5,420 doctors in 19 European countries through the network of national gastroenterological, oncological, surgical and pancreatic societies as well as the European Pancreatic Club. The questionnaire consisted of 20 questions, 14 regarding the use of intensified chemotherapy, 4 regarding demographics of the participants, and 1 to verify the active involvement in the management of metastatic pancreatic cancer. Results: Two hundred and thirteen responses were received and 153 entries were valid for analysis. Of those, 63.4% came from an academic institution, 51% were oncologists, and 52% treated more than 25 cases per year. A majority of responses (71%) were from Italy (40%), Germany (23%), and Spain (8%). As first-line therapy, 11% used gemcitabine +/- erlotinib, 42% used FOLFIRINOX, and 47% used gemcitabine + nab-paclitaxel. Of the intensified regimens, both were applied to equal parts, but the likelihood of protocol deviation was higher when using FOLFIRINOX (p < 0.01). FOLFIRINOX was considered more toxic than gemcitabine + nab-paclitaxel (neutropenia 88 vs. 68%; polyneuropathy 42 vs. 41%; rapid deterioration 42 vs. 31%). FOLFIRINOX was rated to achieve longer survival with an acceptable quality of life (52 vs. 44%). Moreover, 57% of participants thought that gemcitabine + nab-paclitaxel should be the backbone for further clinical trials in pancreatic cancer. Conclusion: Intensified chemotherapy is widely used in pancreatic cancer patients in Europe following its recent clinical approval. Interestingly, nab-paclitaxel and FOLFIRINOX were used at comparable frequency although the latter had to be de-escalated more often.
Diese Arbeit befasste sich mit dem Einfluss der tumorassoziierten Makrophagen (TAM) auf die 6606-PDA-Pankreaskarzinomzellen in vivo und in vitro. Bei der Betrachtung der Interaktion zwischen Tumorzellen und Makrophagen in vitro zeigte sich, dass die Pankreaskarzinomzellen ein Milieu erzeugten, in dem sich M1-Makrophagen zu M2-Makrophagen differenzierten. Des Weiteren wurde anhand des Vitalitätsassays nachgewiesen, dass M2-Makrophagen einen stimulierenden Effekt auf die Tumorzellproliferation hatten, während M1-Makrophagen die Tumorzellproliferation hemmten. Im Mausmodell wurde der Einfluss der Makrophagen untersucht. Hierbei wurde die Infiltration von Makrophagen in das Pankreaskarzinom nachgewiesen. Durch medikamentöse Makrophagen-Reduktion wurde signifikant die Rekrutierung von Makrophagen in die Pankreaskarzinome der Versuchstiere verringert. Unter Reduktion der TAM kam es zu einem signifikant verminderten Tumorwachstum und zu einer deutlich verringerten Tumorangiogenese. Für das Tumorwachstum sind MMP (Matrix Metallo Proteinasen) wichtige Voraussetzung. Der Gehalt von MMP-9 und MMP-12, welche von Makrophagen produziert werden können, ergaben keinen signifikanten Unterschied in beiden Versuchsgruppen. Hier muss die Fähigkeit der Tumorzellen, diese Enzyme selbst zu produzieren, als mögliche Ursache für die ähnlich hohe Expression trotz Reduktion der TAM-Infiltration diskutiert werden. Die Bestimmung von VEGF in den Tumoren und in den Seren der Versuchstiere ergab einen Anstieg dieses Wachstumsfaktors in der Gruppe mit Makrophagen-Reduktion. Der höhere VEGF-Gehalt in den Tumoren mit verminderter Makrophagen-Infiltration kann als Ausdruck der Tumorhypoxie und der fehlenden Mikrovaskularisation sowie als Versuch der Makrophagen-Rekrutierung interpretiert werden. Die Betrachtung der Interleukin-Expression unter Makrophagen-Reduktion zeigte, dass MCP-1 im Serum der Versuchsgruppe mit TAM-Reduktion signifikant erhöht war. Ebenfalls wurde in den Tumoren selbst ein hoher Gehalt an MCP-1 nachgewiesen. MCP-1 ist ein sehr potenter Mediator der Makrophagen-Migration. Seine Expression könnte als Folge der verminderten Makrophagen-Population ebenfalls kompensato-risch erhöht gewesen sein. Mit dem Nachweis von IL-4 und IL-10 im Tumorgewebe, auch unabhängig von der Infiltration durch Makrophagen, konnte die Präsenz zweier wichtiger Induktoren der alternativen Makrophagen-Aktivierung bewiesen werden. Dies stützt die Hypothese, dass die Differenzierung der M2-Makrophagen im Tumorgewebe selbst stattfindet. Das Pankreaskarzinom ist eine maligne Erkrankung, für deren Therapie nach wie vor nur begrenzte Maßnahmen zur Verfügung stehen. Ein präziseres Verständnis der Pa-thogenese ist daher von großer Wichtigkeit. Durch nähere Betrachtungen des Tumor-mikromilieus und der Wechselwirkungen zwischen Immunsystem und Tumorzellen könnte es möglich sein, neue therapeutische Ansätze für die Behandlung von Patien-ten mit einem Pankreaskarzinom zu generieren. Insbesondere könnte die Hemmung der M2-Makrophagendifferenzierung sowie die Induktion von M1-Makrophagen im Tumor für die Therapie des Pankreaskarzinoms ein vielversprechender Ansatz sein.
Background: Pancreatic cancer is the fourth leading cause of cancer-related mortality in both genders. More than 80% of patients suffer from significant weight loss at diagnosis and over time develop severe cachexia. Early nutritional support is therefore essential. Summary: This review evaluates the different nutritional therapies, such as enteral nutrition, parenteral nutrition and special nutritional supplements, on nutritional status, quality of life and survival. Key Message: Due to the high prevalence of malnutrition and the rapid development of anorexia-cachexia-syndrome, early nutritional intervention is crucial and supported by clinical data. Practical Implications: Enteral nutrition should be preferred over parenteral nutrition. Omega-3 fatty acids and <smlcap>L</smlcap>-carnitine are promising substances for the prevention of severe cachexia, but further randomized controlled trials are needed to establish generally accepted guidelines on nutrition in pancreatic cancer.
