Malokklusion als Risikofaktor für Parodontalerkrankungen in der Bevölkerungsstudie „Study of Health in Pomerania“ (SHIP 0)

  • In der Querschnittsanalyse der Daten der bevölkerungsrepräsentativen Studie „Study of Health in Pomerania“ (SHIP 0) konnten allgemein anerkannte und gut erforschte Zusammenhänge zwischen Parodontalerkrankungen und deren Risikofaktoren belegt werden. Die hohe Probandenanzahl von 2208 Teilnehmern ermöglichte es, die Prävalenz von Zahnfehlstellungen als Risikofaktor bezüglich parodontaler Erkrankungen in Mecklenburg-Vorpommern zu berechnen. Ziel dieser Arbeit war es, einen Überblick über die soziodemographischen Aspekte des definierten Probandenguts zu geben. Neben den allgemeinen Faktoren sollten prävalenzbezügliche Aussagen über parodontale und kieferorthopädische Parameter getroffen werden. Dabei wurden Dysgnathiesymptome des Frontzahnbereiches und des Gesamtgebisses separat untersucht und eugnathen Gebissverhältnissen gegenüber gestellt. Darauf folgte das Auffinden möglicher signifikanter Zusammenhänge parodontaler Faktoren mit zunächst allgemeinen Faktoren und im Weiteren mit Zahn- und Kieferfehlstellungen. Es wurden Regressionsmodelle erstellt, mit deren Hilfe mögliche Risikofaktoren für parodontale Erkrankungen bestimmt werden konnten. Letztendlich wurden durch rückwärts bedingte Regression mögliche Assoziationen zwischen Parodontalerkrankungen und Malokklusionen untersucht. Durch die Analyse der Daten konnte belegt werden, dass neben gut untersuchten statistischen Zusammenhängen mit allgemeinen Risikofaktoren wie Geschlecht, Schulbildung, Alter, Rauchverhalten, Familienstand sowie Plaque- und Blutungsindizes, auch ziemlich sichere Zusammenhänge zwischen parodontalen Erkrankungen und bestimmten Zahn- und Kieferfehlstellungen bestehen. Es fanden sich deutliche Hinweise, dass insbesondere eine vergrößerte sagittale Schneidekantendistanz (SKD) als Risikofaktor für Parodontalerkrankungen gesehen werden muss (p < 0,001). Hier fiel eine Dosis-Wirkungsbeziehung auf, da mit der Größe des Abstands der Schneidekanten die statistische Signifikanz in jeder der vier Betrachtungen zunahm. Des Weiteren schienen Lücken im Frontzahnbereich sowie extreme Engstände in Verbindung mit parodontalen Erkrankungen zu stehen. Es stellte sich die Frage nach der Kausalität, also ob diese Fehlstellungen, insbesondere eine lückige Zahnstellung im Frontzahnbereich, aber auch eine vergrößerte sagittale Schneidekantendistanz sowie extremer Engstand, Auslöser oder Folge einer Parodontitis marginalis chronica sind. Klarheit über die Frage nach Ursache und Wirkung kann hier nur eine Longitudinalstudie schaffen. Zusätzlich zeigte sich, dass neben allgemeinen Faktoren wie beispielsweise guter Schulbildung, Nichtrauchen und dem weiblichen Geschlecht anzugehören, auch spezielle Zahn- und Kieferfehlstellungen einer Parodontalerkrankung entgegenwirken können. Ein tiefer Biss ohne Gingivakontakt war hoch signifikant (p < 0,001) mit deutlich weniger Attachmentverlust assoziiert. Auch ein tiefer Biss mit Gingivakontakt und ein Kopfbiss schienen signifikant protektiv auf Attachmentverluste zu wirken. Zusammenfassend lässt sich sagen, dass insgesamt wenige Malokklusionen mit höheren Attachmentverlusten und Sondierungstiefen assoziiert waren. Somit ist eine kieferorthopädische Behandlung mit dem Ziel der Verbesserung der parodontalen Situation nur bei den Malokklusionen mit erhöhtem Risiko für Attachmentverluste und höheren Sondierungstiefen indiziert. Eine gute Mundhygiene und die Vermeidung von Nikotin waren und sind für die Parodontitisprophylaxe wesentlichere Faktoren.
  • In the cross-sectional analysis of data of the population-based study "Study of Health in Pomerania" (SHIP 0) common and well researched links between periodontal diseases and their risk factors could be proved. The high number of 2,208 participants allowed us to calculate the prevalence of malocclusion as a risk factor for periodontal diseases in the German region of Pomerania. The objective of this research was to provide an overview of the socio-demographic aspects of this defined patient population. In addition to the general factors, prevalence related statements about periodontal and orthodontic parameters should be taken. Therein symptoms of dysgnathia in the anterior segment and in the complete jaws were examined separately and were compared with eugnathic conditions. This was followed by the examination of potential significant correlations between periodontal factors and both, general risk factors and malocclusions. With the aid of regression models, possible risk factors for periodontal disease could be ascertained. Ultimately, possible associations between periodontal disease and malocclusion were analyzed stepwise-backward. The analysis of the data clearly shows that, in addition to the already well studied statistical correlations with periodontal diseases and general risk factors such as gender, education, age, smoking habits, marital status and plaque and bleeding indices, there is also a fairly safe connection with certain malocclusions. There were clear indications that particularly the extreme overjet has to be seen as a risk factor for periodontal diseases (p <0.001). One conspicuous thing is the dose-effect relationship. It was noticeable that the larger the space between the anterior incisors the higher the statistical significance in each of the four considerations. Furthermore it was evident that gaps in the anterior segment and extreme crowding are in conjunction with periodontal disease. It led to the question of causality. In particular whether these malocclusions like a patchy tooth position in the anterior segment and an enlarged overjet and extreme crowding are trigger or result of chronical periodontitis. The question of cause and effect must be viewed in a longitudinal study. It further emerged that besides general factors such as good education, non-smoking and the female sex, even special malocclusions of periodontal disease can counteract. A deep bite without gingival contact was highly significant (p < 0.001) associated with significantly less attachment loss. Also a deep bite without gingival contact and an edge-to-edge bite had produced results. In summary it can be said that in total only a few malocclusions were associated with periodontal deseases. Thus orthodontic treatment with the aim of improving the periodontal situation is only indicated in the malocclusion with increased risk of attachment loss and probing depths. More essential factors of periodontal prophylaxis were and still are a good oral hygiene and the prevention of nicotine.
