Zum Vergleich von Goldmann-Applanationstonometrie und Selbsttonometrie mit dem Ocuton S alter sowie neuer Konfiguration

  • Hintergrund: In dieser Studie wurde das Ocuton S*TT-MV neuer Spezifikation mit dem Ocuton S alter Spezifikation sowie der Goldmann Applanationstonometrie (GAT) verglichen. Ziel war es, zu evaluieren, ob die Neukonfigurierung des Ocuton S die Messgenauigkeit und die Handhabung des Selbsttonometers in der Glaukom-Diagnostik und –Überwachung verbessern konnte. Methoden: Insgesamt 101 Glaukompatienten führten Selbstmessungen des Augeninnendruckes mit dem Ocuton S alter Spezifikation (4 Messungen pro Auge) und mit dem Ocuton S*TT-MV (4 Messungen pro Auge) durch. Die neue Gerätespezifikation Ocuton S*TT-MV überprüft automatisch die messtechnische Verwertbarkeit der Applanationsfläche. Als Referenzmessungen wurden pro Proband jeweils zwei Messungen pro Auge mittels GAT durchgeführt. Die Reihenfolge der drei verschiedenen Messreihen erfolgte randomisiert. Ergebnisse: Das arithmetische Mittel des Augeninnendruckes betrug beim alten Ocuton S 18,3 ± 4,2 mmHg, beim neuen Ocuton S*TT-MV 17,5±3,6 mmHg und bei der Goldmann-Applanationstonometrie 15,1±3.4 mmHg. Die Minimal- bzw. Maximalwerte betrugen bei GAT 9 bzw. 30,5 mmHg, beim neuen Ocuton S 7 bzw. 42 mmHg und beim alten Ocuton S 5 bzw. 38 mmHg. Die mittlere Differenz der Messungen aller Patienten mit dem alten Ocuton S zur GAT betrug 3,2±3,5 mmHg, mit dem neuen Ocuton S*TT-MV zu Goldmann 2,4±3,3 mmHg und mit dem alten zum neuen Ocuton S 0,9±2,9 mmHg (p<0,01). Die Probanden maßen mit dem Ocuton S alter Spezifikation mit einer durchschnittlichen Streuung von 1,9 mmHg innerhalb der Messreihe mit vier Messungen pro Auge. Mit dem neuen Ocuton S*TT-MV ergab sich eine durchschnittliche Streuung von 1,7 mmHg. Diese Messgenauigkeit hing weder beim alten Ocuton S noch beim Ocuton S*TT-MV vom Alter des Patienten ab (Ocuton S: Korrelationskoeffizient R=0,2; Ocuton S*TT-MV: Korrelationskoeffizient R=0,053). Schlussfolgerung: Die Neukonfigurierung des Ocuton S führt zu einer statistisch signifikanten Verbesserung der Messgenauigkeit und Annäherung an den Goldstandard der IOD-Messung GAT. Von besonderer klinischer Relevanz ist jedoch, dass gleichzeitig die technische Handhabung des Selbsttonometers Ocuton S*TT-MV neuer Spezifikation verbessert worden ist. Die Überprüfung des Messabdruckes am Gerät durch den Glaukompatienten während IOD-Messung zu Hause entfällt vollständig. Dies wird zu einem vermehrten Einsatz der Selbsttonometrie in der Glaukom-Diagnostik und –Überwachung beitragen.
  • Background: This study compared the new Ocuton S*TT-MV to the Ocuton S (former configuration) and GAT. The aim was to evaluate whether the reconfiguration of the Ocuton S can improve measuring accuracy and handling in terms of glaucoma diagnostics and monitoring. Methods: 101 patients suffering from glaucoma performed self-measurements using the old Ocuton S (four measurements per eye) and the Ocuton S*TT-MV (four measurements per eye). The new Ocuton S*TT-MV automatically scrutinises the metrological usability of the applanated area. Two measurements per eye of each patient were taken with the GAT as a reference. The three series of measurements were performed in random order. Results: The mean IOP measured with the old Ocuton S was 18.3 ± 4.2mmHg and 17.5±3.6mmHg with the Ocuton S*TT-MV. The mean IOP measured with GAT was 15.1±3.4mmHg. The minimum and maximum IOP were 9 and 30.5mmHg, respectively, with GAT, 7 and 42mmHg, respectively, with Ocuton S*TT-MV. The minimum IOP measured with the old Ocuton S was 5 mmHg and the maximum IOP 38mmHg. The mean difference between the old Ocuton S and GAT was 3.2±3.5mmHg, between Ocuton S*TT-MV and GAT 2.4±3.3mmHg and between old and new Ocuton S 0.9±2.9mmHg (p<0.01). The standard deviation within a row of four readings per eye each patient performed with the old Ocuton S averaged 1.9mmHg. With the new Ocuton S*TT-MV, the standard deviation averaged 1.7mmHg. This measuring accuracy did not depend on the age of the patients, neither when using the old nor the new Ocuton S (old Ocuton S: coefficient of correlation R=0.2; Ocuton S*TT-MV: coefficient of correlation R=0.053). Conclusion: The newly configured Ocuton S*TT-MV leads to a significant improvement in the measuring accuracy and approach to GAT. The simplification of handling the new Ocuton S*TT-MV though is of particular clinical relevance. Formerly, every patient was compelled to check the corneal cast on the measuring prism. Now, this is completely unnecessary. This will lead to an increased usage of self-tonometry in diagnosing and monitoring the glaucoma disease.
