Ziel dieser Arbeit war es, die Rolle des sympathischen Nervensystems in der Entstehung der Schlaganfall-induzierten Immunveränderungen zu beleuchten. Dabei charakterisierten wir den Immunphänotyp der Lymphozyten mit Hilfe der DBH-transgenen Mauslinie. In einem Vorversuch wurde die Kontrollgruppe zu den DBH-Knock-Out-Mäusen aus Wildtypen und DBH-heterozygoten Tieren definiert. Der experimentelle Schlaganfall induzierte eine vielseitige Immunsuppression auch in Abwesenheit des sympathischen Nervensystems. Dabei waren periphere Immunveränderungen durch eine Milzatrophie, reduzierte Keimzentrumsreaktion, Lymphozytopenie und T-Zell-Aktivierung gekennzeichnet. Die Keimzentrumsreaktion war hoch signifikant in Fläche und Anzahl zu frühen Zeitpunkten über Tage hinweg erniedrigt, was für eine reduzierte humorale Immunreaktion verantwortlich sein könnte. Das könnte wiederum eine erhöhte Infektanfälligkeit, welche bei Schlaganfallpatienten und auch im Mausmodell beschrieben wurde, begründen. Zusätzlich zur gestörten humoralen Infektabwehr kam es zur frühen Aktivierung von T-Lymphozyten, was durch eine vermehrte Expression von CTLA4 auf T-Effektor-Zellen sichtbar wurde. Die CTLA4-Expression nahm auch auf regulatorischen FoxP3+ T-Zellen zu. Das könnte ein Äquivalent zu der erhöhten suppressorischen Funktion von Tregs sein, welche somit einen Beitrag zur Herausbildung der peripheren Immunsuppression leisten könnten. In der post-akuten Phase des experimentellen Schlaganfalls war eine tendenziell schnellere Erholung der Keimzentrumsreaktion in Katecholamin-depletierten Mäusen erkennbar. Des Weiteren zeigten regulatorische T-Zellen ein relatives Überleben nach experimentellem Schlaganfall in Katecholamin-Abwesenheit. Die Lymphozytopenie und Milzatrophie war ebenfalls in Abwesenheit der Katecholamine weniger stark ausgeprägt. Diese Ergebnisse lassen einen geringen Stellenwert der Katecholamine in der Entstehung peripherer Immunveränderungen erkennen. Die zuvor postulierte Schlüsselfunktion des sympathischen Nervensystems in der Herausbildung der Immunsuppression sollte kritisch betrachtet werden. Vielmehr sollte ein größeres Augenmerk auf die vielseitigen in Wechselwirkung stehenden neuroendokrinen Regelkreise wie HHN-Achse und PNA und immunodulatorische Transmitter wie Substanz P, vasoaktives intestinales Peptid und Neuropeptid Y als mögliche Ursache der Immunsuppression gelegt werden. Weitere Forschung ist nötig, um die detaillierten Vorgänge der Immunsuppression und vor allem ihre Ursachen zu analysieren.
Non-invasive physical plasma (NIPP) achieves biomedical effects primarily through the formation of reactive oxygen and nitrogen species. In clinical use, these species interact with cells of the treated tissue, affecting the cytoplasmic membrane first. The present study investigated the permeability of the cytoplasmic membrane of breast cancer cells with different fluorescent dyes after NIPP treatment and determined the subsequent effects on cell viability. After NIPP treatment and the associated formation of reactive oxygen species, low molecular weight compounds were able to pass through the cytoplasmic membrane in both directions to a higher extent. Consequently, a loss of cellular ATP into the extracellular space was induced. Due to these limitations in cell physiology, apoptosis was induced in the cancer cells and the entire cell population exhibited decreased cell growth. It can be concluded that NIPP treatment disturbs the biochemical functionality of the cytoplasmic membrane of cancer cells, which massively impairs their viability. This observation opens a vast application horizon of NIPP therapy to treat precancerous and malignant diseases beyond breast cancer therapy.
Aims
Averaged measurements, but not the progression based on multiple assessments of carotid intima-media thickness, (cIMT) are predictive of cardiovascular disease (CVD) events in individuals. Whether this is true for conventional risk factors is unclear.
Methods and results
An individual participant meta-analysis was used to associate the annualised progression of systolic blood pressure, total cholesterol, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol with future cardiovascular disease risk in 13 prospective cohort studies of the PROG-IMT collaboration (n = 34,072). Follow-up data included information on a combined cardiovascular disease endpoint of myocardial infarction, stroke, or vascular death. In secondary analyses, annualised progression was replaced with average. Log hazard ratios per standard deviation difference were pooled across studies by a random effects meta-analysis. In primary analysis, the annualised progression of total cholesterol was marginally related to a higher cardiovascular disease risk (hazard ratio (HR) 1.04, 95% confidence interval (CI) 1.00 to 1.07). The annualised progression of systolic blood pressure, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol was not associated with future cardiovascular disease risk. In secondary analysis, average systolic blood pressure (HR 1.20 95% CI 1.11 to 1.29) and low-density lipoprotein cholesterol (HR 1.09, 95% CI 1.02 to 1.16) were related to a greater, while high-density lipoprotein cholesterol (HR 0.92, 95% CI 0.88 to 0.97) was related to a lower risk of future cardiovascular disease events.
Conclusion
Averaged measurements of systolic blood pressure, low-density lipoprotein cholesterol and high-density lipoprotein cholesterol displayed significant linear relationships with the risk of future cardiovascular disease events. However, there was no clear association between the annualised progression of these conventional risk factors in individuals with the risk of future clinical endpoints.
