Impact of proteostasis and the ubiquitin proteasome system on myeloid cell function in the CNS

Cellular protein homeostasis (proteostasis) maintains a functional proteome and thus proper cell function. Proteostasis is facilitated by the ubiquitin-proteasome system (UPS), an intracellular protein turnover machinery ensuring clearance of damaged, misfolded, old and/or unneeded regulatory proteins. This is particularly important in the central nervous system (CNS), where it is linked to neurodegeneration. Disruptions of the proteostasis systems cause the accumulation of misfolded proteins which are commonly seen in progressive neurodegenerative diseases also linked to neuroinflammation. Proper UPS function can protect cells from the accumulation of defective proteins, neurodegeneration and neuroinflammation. Furthermore, it has been found that loss of function mutations in the genes encoding UPS components are linked to systemic inflammation including neuroinflammation and/or neurodevelopmental disorders. Proteasome defects in patients suffering from these disorders cause decreased proteasome activity, accumulation of proteins, activation of proteotoxic stress responses and systemic inflammation. However, the molecular link between proteotoxic stress and the initiation of inflammatory signalling remained unclear. In Article 2, we summarized the importance of the UPS in immune cell proteostasis and function including activation of innate and adaptive immune responses. Although UPS function is notably important in innate immune signalling, the current understanding of the role of UPS in myeloid cell function in the CNS is limited. We also indicated the involvement of impaired UPS function in sterile systemic inflammation including neuroinflammation as well as tumour diseases and pathogen manipulation of immune cells.
To investigate the molecular link behind proteasome impairment and systemic inflammation in the brain, we focused on microglia cells as the only immune residents of the CNS. In Article 1, we used a pharmacological inhibitor called bortezomib which targets β5 and β5i/LMP7 subunit activities in standard proteasome (SP) and immunoproteasome (IP), respectively. We showed for the first time on the molecular level that inhibition of proteasome activity by bortezomib triggers the accumulation of ubiquitylated proteins, proteotoxic stress responses and innate immune signalling activation depending on the induced proteotoxic stress response called unfolded protein response (UPR) in murine microglia. In particular, activation of the inositol-requiring protein 1α arm of UPR upon bortezomib treatment leads to systemic inflammation as indicated by type I interferon (IFN) response.
IP enhance the proteolytic capacity of UPS by rapid clearance of proteins upon immune signalling activation. Microglia, like other immune cells, exhibit constitutive expression of IP as well as SP to maintain their cellular proteostasis. In Manuscript 3, we studied the particular impact of IP impairment on microglial cellular function. We showed accumulation of ubiquitin-modified proteins and activation of proteotoxic stress responses in IP-impaired mouse and human microglia models. Moreover, we identified possible IP substrates in microglia using β5i/LMP7 knockout mice as an IP deficiency model and, examined how IP deficiency affects microglia function. IP deficient microglia affected the ubiquitylation levels of proteins involved in multiple pathways such as immune responses, energy metabolism, cytoskeleton organisation, cell cycle and ribosome function. Based on the molecular analysis, we confirmed sterile activation of innate immune signalling mechanisms in IP impaired microglia. This is driven by the proteotoxic stress sensor protein kinase R (PKR). In addition, we were able to show that IP impairment altered levels of the microglial activation markers, which are also involved in motility, adhesion and phagocytosis of microglia.
In this thesis, we highlight that UPS function is necessary to maintain microglial proteostasis and, that impairment of proteasome activities triggers sterile inflammation in microglia via activation of proteotoxic stress responses. The described activation of innate immune signalling mechanisms in microglia upon proteasome impairment may be considered as new therapeutic targets for patients suffering from rare protesomapathies or other disorders linked to dysregulated immune signalling.
Oral mucositis is the most common and severe non-hematological complication associated with cancer radiotherapy, chemotherapy, or their combination. Treatment of oral mucositis focuses on pain management and the use of natural anti-inflammatory, sometimes weakly antiseptic mouth rinses in combination with optimal oral cavity hygiene. To prevent negative effects of rinsing, accurate testing of oral care products is necessary. Due to their ability to mimic realistic in-vivo conditions, 3D models may be an appropriate option in compatibility testing of anti-inflammatory and antiseptically effective mouth rinses. We present a 3D model of oral mucosa based on the cell line TR-146 with a physical barrier, characterized by high transepithelial electrical resistance (TEER) and confirmed cell integrity. Histological characterization of the 3D mucosa model showed a stratified, non-keratinized multilayer of epithelial cells similar to that of human oral mucosa. By means of immuno-staining, tissue-specific expression of cytokeratin 13 and 14 was shown. Incubation of the 3D mucosa model with the rinses had no effects on cell viability, but TEER decreased 24h after incubation in all solutions except ProntOral®. Analogous to skin models, the established 3D model meets the quality control criteria of OECD guidelines and may therefore be suitable for comparing the cytocompatibility of oral rinses.
Objectives
Oral mucositis caused by intensive cancer chemotherapy or radiotherapy frequently results in pronounced damage of the oral mucosa leading to painful oral hygiene. To support oral care, antimicrobial effective mouth rinses may be used. Thus, the efficacy of a hypochlorite-based mouth rinse (Granudacyn®), assumed to be highly biocompatible because of the compounds being part of the natural pathogen defense, as possible antiseptic agent in case of oral mucositis was compared to that of an octenidine based antiseptic mouth rinse (Octenidol® md).
Materials and methods
The study was conducted as monocentric, controlled, randomized, blind cross over comparative study on 20 volunteers. As a proof of principle, we performed the study on orally healthy subjects and not cancer patients. The efficacy was determined as reduction of colony forming units (cfu) on buccal mucosa as well as in saliva. After mouth rinsing for 30 s, samples were taken after 1 min, 15 min, 30 and 60 min. The lg-reduction was calculated as difference between lg-values of cfu pre- and post-treatment.
Results
Both antiseptic mouth rinses induced a significant reduction of cfu on buccal mucosa and in saliva 1 min after mouth rinsing. The effect persisted up to 60 min. The octenidine based rinse was significantly superior to the hypochlorite-based rinse up to the last sample 60 min after rinsing. However, the known cytotoxicity of octenidine argues against its application.
Conclusion
Within the limits of this study, due to its antiseptic efficacy, the hypochlorite-based rinse Granudacyn® can be regarded appropriate to support the oral hygiene in patients with a sensitive oral mucosa during an aggressive cancer chemotherapy and radiation treatment in case of oral mucositis.
Plus‐strand RNA [(+)RNA] viruses are the largest group of viruses, medically highly relevant human pathogens, and are a socio‐economic burden. The current global pandemic of the severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) shows how a virus has been rapidly spreading around the globe and that– without an antiviral treatment– virus trans mission is solely dependent on human behavior. However, other (+)RNA viruses such as rhino‐, noro‐, dengue‐ (DENV), Zika, and hepatitis C virus (HCV) are constantly spreading and expanding geographically. As in the case of hepatitis C, since its first identification in the 1970s, it took more than 30 years to understand the HCV structure, genome organiza t ion, life cycle, and virus‐host interplay leading to the cure of a chronic and life‐threatening disease. However, no vaccination or antiviral treatment exists for most (+)RNA viruses. Con sequently, a precise and comprehensive analysis of the viruses, their life cycles, and parasitic interactions with their hosts remains an important field of research. In the presented thesis, we use mathematical modeling to study the life cycles of (+)RNA viruses. We analyze replication strategies of closely related (+)RNA viruses, namely HCV, DENV, and coxsackievirus B3 (CVB3), to compare their life cycles in the presence and ab sence of the host’s immune response and antiviral drug treatment and consider different viral spreading mechanisms. Host dependency factors shape the viral life cycle, contribut ing to permissiveness and replication efficiency. Our mathematical models predicted that host dependency factors, such as ribosomes, and thus the virus’ ability to hijack the host cell’s translation machinery play an essential role in the viral genome replication efficiency. Furthermore, our mathematical model suggested that the availability of ribosomes in the vi ral life cycle is a crucial factor in disease outcome: the development of an acute or chronic disease. Even though the host developed strategies to attack the virus, e.g., by degrading the viral genome, blocking the viral protein production, and preventing viral spread, viruses found strategies to countermeasure those so‐called host restriction factors derived from the immune system. Our mathematical models predicted that DENV might be highly effective in blocking the cell’s attempts to recognize the invader. Moreover, we found ongoing HCV RNAreplication even with highly effective antiviral drugs that block processes in the viral life cycle. Furthermore, we found alternative pathways of infection spread, e.g., by HCV RNA carrying exosomes, which may be a possible explanation for reported plasma HCV RNA at the end of treatment, found in a subset of patients. Hence, the mathematical models presented in this thesis provide valuable tools to study the viral replication mechanism in detail. Even though being a simplification of reality, our model predictions confirm and explain known and suggest novel biological mechanisms. In the pre sented thesis, I will summarize and discuss key findings and contextualize model predictions in the broader scientific literature to improve our understanding of the viral dynamics and the virus‐host interplay.
The vast majority of RNA splicing in today‘s organisms is achieved by the highly regulated and precise removal of introns from pre-mRNAs via the spliceosome. Here we present a model of how RNA splicing may have occurred in earlier life forms. We have designed a hairpin ribozyme derived spliceozyme that mediates two RNA cleavages and one ligation event at specific positions and thus cuts a segment (intron) out of a parent RNA and ligates the remaining fragments (exons). The cut-out intron then performs a downstream function, acting as a positive regulator of the activity of a bipartite DNAzyme. This simple scenario shows how small RNAs can perform complex RNA processing dynamics, involving the generation of new phenotypes by restructuring segments of given RNA species, as well as delivering small RNAs that may play a functional role in downstream processes.
Mass spectrometry-based Proteome analysis of porcine cells infected with African swine fever virus

ASFV, a highly contagious, pathogenic and lethal pathogen of swine, poses a major threat to domestic and wild suids worldwide as neither vaccines nor treatments are available. Compared to other well-characterized similarly complex viruses like herpesviruses or adenoviruses, the understanding of ASFV biology is poor.
To improve the understanding of ASFV biology, following the establishment of a robust protocol for the isolation of primary monocyte-derived porcine macrophages (moMΦ) and their infection with ASFV for mass spectrometry (MS)-based proteome analysis was performed.
Under both conditions, naïve and infected, the isolated cells showed cell type-specific characteristics like phagocytosis and antigen presentation and protein expression patterns, including the expression of swine leucocyte antigens and CD markers. Furthermore, moMΦ could be reproducibly infected with ASFV isolates of different genotypes and pathogenicity.
The ASFV protein expression patterns in moMΦ correlate well with those observed in established cell lines at transcript and protein level. The expression of 27 ASFV proteins was confirmed at the protein level. Among them, 9 members of multi-gene families (MGF) and 12 novel open reading frames (nORFs) were recently predicted based on transcription start site mapping.
The direct comparison of closely related ASFV genotype II isolates revealed no virulence-associated protein expression patterns beyond those expected based on the genome sequences of the isolates.
Using different MS quantification strategies, it was shown that ASFV affects both static protein expression levels and protein synthesis. These changes in protein expression impact proteins and pathways known to be targeted by ASFV, including CD-markers, ER-stress and cell death pathways, and cellular antiviral responses. Beyond these observations that further validated the moMΦ infection model, novel effects of the ASFV infection on the cellular proteome were noticed.
These effects include the decreased expression levels of cathepsins, especially cathepsins D (CTSD), H (CTSH) and L (CTSL) as well as the transient activation of MAPK14/p38 prior to its strong downregulation. In addition to MAPK14/p38 further members of the MAPK14/p38 signaling pathway, like MAPKAPK2, were affected by ASFV infection.
As these modulations of the cellular proteome would in general result in decreased pro-inflammatory responses, it did stand out that the synthesis of interferon-response related genes including MX1 and ISG15 evaded the ASFV-induced global reduction of protein synthesis. In contrast, the synthesis of genes involved in RNA processing and splicing was significantly impaired. In total, the regulations of individual host proteins assessed in the context of the whole cellular proteome integrate well with each other and other cellular responses to ASFV infection and may help to improve the understanding of host-virus interactions.
Overall, this thesis provides novel insights into the expression of ASFV-encoded ORFs of different isolates and the host response to ASFV infection. It points out that the current knowledge of the ASFV coding capacity, temporal protein expression patterns, protein functionality, post-translational modifications and host interactions is still sketchy as many aspects of ASFV replication have yet to be understood. The established moMΦ-model to study ASFV infections in vitro provides a powerful tool for future applications to increase the understanding of ASFV biology.
Die Kv7-Kaliumkanalöffner Flupirtin und Retigabin waren wertvolle Alternativen bei der Pharmakotherapie von Schmerzen und Epilepsie. Beide Wirkstoffe werden aufgrund von unerwünschten Arzneimittelwirkungen derzeit jedoch nicht mehr eingesetzt. Die Flupirtin-induzierte Hepatotoxizität und die durch Retigabin hervorgerufenen Gewebeverfärbungen scheinen dabei auf den ersten Blick nicht zusammenzuhängen. Gleichwohl lassen sich wahrscheinlich beide Nebenwirkungen auf das gemeinsame oxidationsempfindliche Triaminoaryl-Grundgerüst zurückführen, welches zur Bildung von reaktiven Chinondiimin-Metaboliten neigt. Da hingegen der Wirkungsmechanismus, d. h. die Öffnung der Kv7-Kanäle, nicht an der Toxizität beteiligt zu sein scheint, hatte diese Arbeit zum Ziel, sicherere Alternativen für Flupirtin und Retigabin zu entwickeln. In einem Liganden-basierten Ansatz wurde eine Umgestaltung des Triaminoaryl-Kerns, den beide Wirkstoffe gemeinsam haben, vorgenommen, was zu Carba-Analoga führte, die durch eine erhöhte Oxidationsbeständigkeit sowie ein vernachlässigbares Risiko für die Bildung von chinoiden Metaboliten charakterisiert sind. Zusätzlich zu diesen verbesserten Sicherheitsmerkmalen offenbarten einige der neuartigen Derivate eine überlegene Kv7.2/3-Kanalöffnungsaktivität. Im Vergleich zu Flupirtin konnte die Potenz der Verbindungen um den Faktor 150 gesteigert werden, während die intrinsische Aktivität auf bis zu 176 % verbessert werden konnte, was die betreffenden Carba-Analoga zu vielversprechenden Kandidaten für eine weitergehende Entwicklung macht. Andererseits ermöglichten einige inaktive Verbindungen sowie die insgesamt deutlich abgestuften Kv7.2/3-Aktivitätsdaten die Etablierung von validen Struktur-Wirkungs-Beziehungen und Hypothesen zum Bindungsmodus, die mit Dockingergebnissen und Molekulardynamik-Simulationen korrelierten.
Die zweite Leichenschau vor der Feuerbestattung dient im Wesentlichen der Erkennung von nichtnatürlichen Todesfällen, die im Rahmen der ersten Leichenschau nicht als solche erkannt wurden. Immer wieder fallen dabei punktförmige Einblutungen im Gesichtsbereich, insbesondere in die Lid- und Lidbindehäute auf. Neben krankhaften und lagebedingten Ursachen können diese Einblutungen der einzige äußerlich erkennbare Hinweis auf eine stattgehabte Halskompression sein. Bei Feststellung derartiger Einblutungen im Rahmen der zweiten Leichenschau wird der Sterbefall „angehalten“ und bei der Kriminalpolizei angezeigt. Für einen 12-Jahres-Zeitraum (2010–2021) wurden retrospektiv alle angehaltenen Sterbefälle mit festgestellten Petechien analysiert. In diesem Zeitraum wurden in den Krematorien, für die das Institut für Rechtsmedizin Greifswald verantwortlich ist, insgesamt 2822 Sterbefälle angehalten. In 282 Fällen erfolgte diese Anhaltung aufgrund petechialer Einblutungen im Gesichtsbereich. Durch die zuständige Staatsanwaltschaft wurde in 47 % dieser Fälle eine gerichtliche Obduktion angeordnet. Im Beobachtungszeitraum fanden sich 2 Tötungsdelikte, die erst durch die zweite Leichenschau vor der Feuerbestattung im Krematorium festgestellt wurden. Die Untersuchung zeigt, dass petechiale Einblutungen als Anhaltspunkt für einen nichtnatürlichen Tod nicht ausreichend bei Leichenschauärzten bekannt sind.
The stratospheric aerosol layer plays an important role in the radiative balance of Earth primarily through scattering of solar radiation. The magnitude of this effect depends critically on the size distribution of the aerosol. The aerosol layer is in large part fed by volcanic eruptions strong enough to inject gaseous sulfur species into the stratosphere. The evolution of the stratospheric aerosol size after volcanic eruptions is currently one of the biggest uncertainties in stratospheric aerosol science. We retrieved aerosol particle size information from satellite solar occultation measurements from the Stratospheric Aerosol and Gas Experiment III mounted on the International Space Station (SAGE III/ISS) using a robust spectral method. We show that, surprisingly, some volcanic eruptions can lead to a decrease in average aerosol size, like the 2018 Ambae and the 2021 La Soufrière eruptions. In 2019 an intriguing contrast is observed, where the Raikoke eruption (48∘ N, 153∘ E) in 2019 led to the more expected stratospheric aerosol size increase, while the Ulawun eruptions (5∘ S, 151∘ E), which followed shortly after, again resulted in a reduction in the values of the median radius and absolute distribution width in the lowermost stratosphere. In addition, the Raikoke and Ulawun eruptions were simulated with the aerosol climate model MAECHAM5-HAM. In these model runs, the evolution of the extinction coefficient as well as of the effective radius could be reproduced well for the first 3 months of volcanic activity. However, the long lifetime of the very small aerosol sizes of many months observed in the satellite retrieval data could not be reproduced.
Deteriorations in slow wave sleep (SWS) have been linked to brain aging and Alzheimer’s disease (AD), possibly due to its key role in clearance of amyloid-beta and tau (Aß/tau), two pathogenic hallmarks of AD. Spermidine administration has been shown to improve sleep quality in animal models. So far, the association between spermidine levels in humans and parameters of SWS physiology are unknown but may be valuable for therapeutic strategies. Data from 216 participants (age range 50–81 years) of the population-based Study of Health in Pomerania TREND were included in our analysis. We investigated associations between spermidine plasma levels, key parameters of sleep macroarchitecture and microarchitecture that were previously associated with AD pathology, and brain health measured via a marker of structural brain atrophy (AD score). Higher spermidine levels were significantly associated with lower coupling between slow oscillations and spindle activity. No association was evident for SWS, slow oscillatory, and spindle activity throughout non-rapid eye movement sleep. Furthermore, elevated spermidine blood levels were significantly associated with a higher AD score, while sleep markers revealed no association with AD score. The association between higher spermidine levels and brain health was not mediated by coupling between slow oscillations and spindle activity. We report that higher spermidine blood levels are associated not only with deteriorated brain health but also with less advantageous markers of sleep quality in older adults. Future studies need to evaluate whether sleep, spermidine, and Aß/tau deposition are interrelated and whether sleep may play a mediating role.
Biorelevante In-vitro-Freisetzungsmodelle werden u. a. für das Screening neuartiger Formulierungen, zur Etablierung von In-vitro-/In-vivo-Korrelationen und zur Vorhersage des In-vivo-Verhaltens einer applizierten Darreichungsform angewendet. Die Entwicklung von In-vitro-Freisetzungsmodellen für peroral verabreichte Arzneiformen fokussierte bisher vorwiegend auf die Abbildung der gastrointestinalen Physiologie eines gesunden, „durchschnittlichen“ Erwachsenen. Patientenspezifische Faktoren, wie z. B. das Alter, Erkrankungen oder Geschlecht sowie individuelle Unterschiede, die die gastrointestinalen Verhältnisse und folglich auch das Freisetzungsverhalten einer peroral applizierten Arzneiform beeinflussen können, wurden bisher kaum berücksichtigt. Der Fokus dieser Arbeit lag auf der Entwicklung und Etablierung von patientenspezifischen, bioprädiktiven In-vitro-Freisetzungsmodellen für perorale Darreichungs-formen unter Berücksichtigung der gastrointestinalen Gegebenheiten zweier unterschiedlicher Patientenpopulationen: pädiatrische Patienten und Parkinson-Patienten.
Eine wichtige Voraussetzung für eine sichere und wirksame perorale Arzneimitteltherapie bei pädiatrischen Patienten sind altersgerechte Darreichungsformen sowie eine geeignete Einnahmepraxis. Peroral applizierte Arzneimittel werden pädiatrischen Patienten häufig zusammen mit Applikationsvehikeln verabreicht, um die Einnahme der Arzneimittel zu erleichtern. Es muss jedoch bei einer solchen Anwendungspraxis sichergestellt werden, dass die eingenommene Arzneiform mit dem jeweiligen Applikationsvehikel kompatibel ist. Die Beurteilung der Kompatibilität ist anhand klinischer In-vivo-Studien an gesunden Kindern jedoch aufgrund ethischer Bedenken kaum möglich. Zur Evaluierung der Kompatibilität könnten In-vitro-Freisetzungsmethoden als eine mögliche Alternative eingesetzt werden. Im ersten Teil der vorliegenden Arbeit wurden pädiatrische In-vitro-Freisetzungsmodelle entwickelt, um zu evaluieren, ob die Stabilität und das In-vivo-Freisetzungsverhalten der neuartigen Alkindi®-Formulierung durch Co-Verabreichung mit alterstypischen Applikationsvehikeln beeinträchtigt werden. Zur Beantwortung dieser Fragestellung wurden im Anschluss an eine intensive Literaturrecherche Physiologie-basierte In-vitro-Modelle auf Basis der Mini-Paddle-Apparatur entwickelt. In der ersten Studie wurde die In-vitro-Wirkstofffreisetzung nach simulierter Applikation der Alkindi®-Formulierung mit typischen Applikationsvehikeln für Kinder unter 6 Jahren, d. h. Muttermilch, Formulamilch und Vollmilch, untersucht. In der zweiten In-vitro-Studie wurde der Altersbereich der adressierten Patientenpopulation auf 2 - 16 Jahre verändert und eine Reihe weiterer flüssiger sowie halbfester Applikationsvehikel, wie z. B. Orangensaft und Joghurt, verwendet. In beiden Studien konnte deutlich gezeigt werden, dass die Alkindi®-Formulierung ein robustes Freisetzungsverhalten aufwies und kompatibel mit den untersuchten Matrices war. Auf Grundlage der Ergebnisse der In-vitro-Untersuchungen wurde geschlussfolgert, dass die In-vivo-Freisetzung und die Bioverfügbarkeit der untersuchten Arzneiform nicht durch die untersuchten Applikationsvehikel beeinflusst werden und folglich diese Vehikel zur gemeinsamen Einnahme mit der Alkindi®-Formulierung geeignet sind. Diese Beobachtungen wurden darüber hinaus durch publizierte Ergebnisse einer korrespondierenden In-vivo-Studie in Erwachsenen bestätigt.
Der zweite Teil der Arbeit befasste sich mit der Entwicklung eines neuartigen, Parkinson-spezifischen und Physiologie-basierten In-vitro-Freisetzungsmodells. Für die Entwicklung von biorelevanten In-vitro-Modellen zur Simulation der luminalen Bedingungen im Gastrointestinal-trakt einer spezifischen Patientenpopulation sind umfangreiche Kenntnisse über die jeweiligen gastrointestinalen In-vivo-Bedingungen und deren Variabilität unerlässlich. Im Rahmen einer Literaturrecherche wurde der aktuelle Wissensstand zu den gastrointestinalen Gegebenheiten in Parkinson-Patienten recherchiert, ausgewertet und zusammengefasst. Die Ergebnisse der Literaturstudie machen deutlich, dass sich die gastrointestinalen Bedingungen von Parkinson-Patienten teilweise erheblich von gesunden Erwachsenen unterscheiden. Das bedeutendste gastrointestinale Merkmal von Parkinson-Patienten ist die beeinträchtigte Motilität des Gastrointestinaltrakts, was sich u. a. in einer Verlangsamung der Magenentleerung sowie der intestinalen Passage äußert. Demgegenüber steht jedoch ein großer Mangel an Daten für eine Reihe von gastrointestinalen Parametern. Dies betrifft z. B. die Zusammensetzung und physiko-chemischen Eigenschaften der luminalen Flüssigkeiten des Gastrointestinaltrakts.
Als geeignete In-vitro-Testplattform wurde die USP-3-Apparatur – auch als Eintauchender Zylinder (Europäisches Arzneibuch, Ph. Eur.) und Reciprocating cylinder (Ph. Eur. und US-amerikanisches Arzneibuch, USP) bezeichnet – ausgewählt, da sich diese Testplattform insbesondere zur Untersuchung von Darreichungsformen mit modifizierter Wirkstofffreisetzung eignet und bereits in einer Vielzahl von analytischen Laboren etabliert ist. Die Nutzung der kompendialen USP-3-Apparatur ließ aufgrund der geringen Variationsmöglichkeiten keine Simulation typischer Motilitätsmuster im humanen Gastrointestinaltrakt zu und eignete sich noch weniger für die Entwicklung und Etablierung von individuellen, patientenspezifischen Motilitätsprofilen. Um diese technischen Limitationen zu überwinden, wurde für die Weiterentwicklung des arzneibuchkonformen Modells ein Lastenheft erstellt, welches detaillierte Anforderungen für die Entwicklung der neuen Testapparatur enthielt. Auf Grundlage des beschriebenen Übersichtsartikels und unter Anwendung einer auf Basis des Lastenheftes modifizierten USP-3-Apparatur wurden unter besonderer Berücksichtigung von Motilität, Passagezeiten und Flüssigkeitsvolumina Parkinson-spezifische In-vitro-Freisetzungsmodelle entwickelt. Für ausgewählte modifiziert freisetzende Levodopa-Fertigarzneimittel wurde anschließend eine vergleichende Serie von In-vitro-Freisetzungsuntersuchungen unter Anwendung von Parkinson-spezifischen- oder „standardmäßigen“ Testmodellen durchgeführt, wobei letztere die gastrointestinalen Gegebenheiten eines „durchschnittlichen“, gesunden Erwachsenen simulierten. Für eine Beurteilung der Aussagekraft der entwickelten Parkinson-spezifischen Testmodelle wurden die generierten In-vitro-Freisetzungsdaten aus den Parkinson-spezifischen- und den „standardmäßigen“ Freisetzungsuntersuchungen in ein In-silico-PBPK-Modell implementiert und die jeweiligen simulierten Plasmakonzentrations-Zeit-Profile von Levodopa anschließend mit klinischen, durchschnittlichen In-vivo-Daten korreliert. Für PBPK-Modelle mit integrierten Parkinson-spezifischen In-vitro-Freisetzungsdaten wurde eine höhere Prädiktivität des In-vivo-Verhaltens der untersuchten Levodopa-Darreichungsformen beobachtet. Es konnte gezeigt werden, dass die entwickelten Parkinson-spezifischen In-vitro-Modelle ein vielversprechendes und prädiktives Instrument zur Vorhersage der In-vivo-Wirkstofffreisetzung von modifiziert freisetzenden Levodopa-Darreichungsformen darstellen. Der diskutierte methodische Ansatz der vorliegenden Studie könnte zukünftig das Screening neuartiger Formulierungen deutlich optimieren und somit zu einer verbesserten Arzneimitteltherapie für Parkinson-Patienten, aber auch für andere spezifische Patientengruppen beitragen
Simple Summary
This German multicenter study investigated the importance of different supportive measures offered to patients with prostate cancer who undergo surgery (radical prostatectomy). A number of these supportive measures are required during the certification of a urologic hospital as prostate cancer center. However, a broad scientific basis evaluating these measures from the patient’s perspective is still lacking. In this study, patients were asked to rate the relevance of several supportive measures and to estimate the effective availability of these different supportive measures at their urologic clinic about 15 months after surgery. Our study highlights that only six of fifteen different supportive measures were rated as very relevant by patients. None of these six supportive measures were offered more intensively at the certified clinics compared to the non-certified clinics according to the patients. Our study helps to identify those supportive measures with the highest subjective impact on patients in this setting.
Abstract
Certification as a prostate cancer center requires the offer of several supportive measures to patients undergoing radical prostatectomy (RP). However, it remains unclear how patients estimate the relevance of these measures and whether the availability of these measures differs between certified prostate cancer centers (CERTs) and non-certified centers (NCERTs). In 20 German urologic centers, a survey comprising questions on the relevance of 15 supportive measures was sent to 1000 patients at a median of 15 months after RP. Additionally, patients were asked to rate the availability of these measures using a four-item Likert scale. The aim of this study was to compare these ratings between CERTs and NCERTs. The response rate was 75.0%. In total, 480 patients underwent surgery in CERTs, and 270 in NCERTs. Patients rated 6/15 supportive measures as very relevant: preoperative medical counselling concerning treatment options, a preoperative briefing answering last questions, preoperative pelvic floor exercises (PFEs), postoperative PFEs, postoperative social support, and postoperative rehabilitation addressing physical fitness recovery. These ratings showed no significant difference between CERTs and NCERTs (p = 0.133–0.676). In addition, 4/9 of the remaining criteria were rated as more detailed by patients in CERTs. IMPROVE represents the first study worldwide to evaluate a patient-reported assessment of the supportive measures accompanying RP. Pertinent offers vary marginally between CERTs and NCERTs.
Inflammasome activation and formation of ASC specks in patients with juvenile idiopathic arthritis

Objective
The formation of large intracellular protein aggregates of the inflammasome adaptor ASC is a hallmark of inflammasome activation and characteristic of autoinflammation. Inflammasome activated cells release the highly proinflammatory cytokine IL-1β in addition to ASC specks into the extracellular space. Autoinflammatory activity has been demonstrated in systemic JIA, however minimal data exist on the role of inflammasomes in other JIA subtypes. We therefore investigated, if pyroptotic cells are present in the circulation of oligo- and poly-articular JIA.
Methods
Peripheral blood of JIA patients (n = 46) was investigated for ASC speck formation, a key step in inflammasome activation, by flow cytometry and immunofluorescence. Free ASC and proinflammatory cytokine levels were determined by ELISA and multiplex assay.
Results
Oligo-articular JIA patients showed a significantly increased proportion of ASC speck+ monocytes compared to poly-articular JIA patients. In serum free ASC alone is not sufficient to assess inflammasome activity and does not correlate with ASC speck+ monocytes. Compared to control several cytokines were significantly elevated in samples of JIA patients. JIA serum containing antinuclear antibodies, incubated with ASC specks boosts a secondary inflammation by IL-1β production in macrophages.
Conclusion
For the first time, we detect ex vivo inflammasome activation by ASC speck formation in oligo- and poly-articular JIA patients. Most notably, inflammasome activation was significantly higher in oligo- compared to poly-articular JIA patients. This data suggests that inflammasome derived autoinflammation may have a greater influence in the previously thought autoimmune oligo-articular JIA patients.
Ziel der vorliegenden Arbeit war es funktionsanalytische Untersuchungsergebnisse aus der Perimetrie mit den Daten der HRT II Papillenanalyse, also den rein morphologischen Untersuchungsergebnissen, bei Glaukompatienten vergleichend zu analysieren.
Patienten und Methodik: Dieser Untersuchung liegen Laser-Scanning-Tomografien des Sehnervenkopfes mittels HRT II und Gesichtsfeldanalysen von 184 Glaukom-Patientinnen und Patienten der Klinik und Poliklinik für Augenheilkunde der Universitätsmedizin Greifswald aus dem Zeitraum von 2004 bis 2011 zugrunde. Die Laser-Scanning-Tomografien wurden mit dem HRT II erstellt. Die Gesichtsfeldanalysen wurden mit dem Octopus 101 der Fa. Haag-Streit Interzeag durchgeführt. Die Daten von 110 Frauen und 74 Männern fanden in der statistischen Auswertung Berücksichtigung. Das durchschnittliche Patientenalter betrug 55 Lebensjahre. Es standen 1.384 Datensätze zur Verfügung, die sich auf drei statistische Analysestrategien verteilten. Die Analysestrategie 1 umfasste ausschließlich die Laser-Scanning-Tomografien mit dem HRTII, die Analysestrategie 2 ausschließlich perimetrische Untersuchungsereignisse und die Analysestrategie 3 papillenmorphologische und perimet-rische Untersuchungen.
Ergebnisse: Die statistische Betrachtung der Analysestrategie 1 erbrachte keine ausreichende Signifikanz hinsichtlich der Primärparameter Patientenalter, Augenseite und Geschlecht (allein der HRT-Parameter mittlere retinale Nervenfaserschichtdicke zeigte eine Unterscheid-barkeit zwischen den Geschlechtern) ebenso wie die isolierte statistische Betrachtung der Gesichtsfeldbefunde in der Analysestrategie 2 bezüglich der Perimetriehauptparameter MD und MS im Hinblick auf die Untersuchungsstrategien (TOP und Dynamisch). Für den Vergleich beider Untersuchungsverfahren in der Analysestrategie 3 konnte der Gesichtsfeldbefund in Bezug auf die Parameter MD und MS betrachtet werden. Diese wurden hinsichtlich der fünf Hauptparameter aus den HRT Untersuchungen, der Randsaumfläche, dem Randsaumvolumen, der Exkavationsform, der Höhenprofilkontur und der mittleren retinalen Nervenfaserschichtstärke, analysiert. Pathologische Werte der mittleren Defekttiefe (MD GF-Hauptparameter) konnten mit einer signifikanten Verringerung der Randsaumfläche und einer signifikanten Abnahme des Randsaumvolumens assoziiert werden. Ebenfalls korrelierten pathologische MD-Werte mit einer Größenzunahme der Exkavationsform.
Der HRT-Hauptparameter Höhenvariation der Kontur wies keine signifikante Verminderung in Bezug auf die im Gesichtsfeld pathologisch identifizierten MD-Werte auf. Eine signifikante Reduktion hingegen zeigte die mittlere retinale Nervenfaserschichtdicke.
Die Differenzierung des Gesichtsfeldhauptparameters MD in Bezug auf das HRT-Qualitätskriterium SD zeigte, dass die Untersuchungsergebnisse mit einer SD > 40 µm im Vergleich der pathologischen zu glaukomunauffälligen Werten eine größere Varianz aufwiesen als jene, die dem Qualitätskriterium (SD < 40 µm) entsprachen.
Die Betrachtung des Qualitätsmaßes der Gesichtsfeldprüfung mit Beantwortung „falsch-negativer“ und „falsch-positiver“ Fangfragen ergab hinsichtlich der Differenzierung des HRT-Hauptparameters mittlere Nervenfaserschichtdicke, dass die pathologischen Werte einer größeren Schwankungsbreite unterlagen als die glaukomunauffälligen und somit als unzuverlässigere Untersuchungen einzuschätzen sind, wenn krankhafte morphologische Veränderungen vorliegen. Die Untersuchung der Arbeitshypothese zum Zusammenhang zwischen Gesichtsfeldverlust (MD) und den HRT-Hauptparametern auf eine vermutete Altersabhängigkeit, konnte keine deutliche Korrelation objektivieren.
Schlußfolgerung: Nur unter Berücksichtigung der äußerst vielfältigen anatomischen Gegebenheiten, z. B. Bulbuslänge, Makropapille, Pathologien, zunehmende Katarakt und Einhaltung der Qualitätsstandards, kann die Laser-Scanning-Tomografie ein sehr wichtiges diagnostisches Mittel zur Glaukomdetektion bzw. zum Glaukommonitoring in Kombination mit funktionsanalytischen perimetrischen Untersuchungen sein.
Im Ergebnis war festzustellen, dass sich die Sensitivität der HRT Parameter als nicht besonders effektiv bewerten ließen. Die statistische Auswertung der Parameter zeigte keine signifikanten Änderungen hinsichtlich der Morphologie der Papilla nervi optici über den im Zeitraum 2004 bis 2011 an der Universitätsmedizin Greifswald untersuchten Glaukompatienten im Längsschnittverlauf.
Background and purpose
The insula has important functions in monitoring and integrating physiological responses to a personal experience of multimodal input. The experience of chills in response to auditory stimuli is an important example for a relevant arousing experience coupled with bodily response. A group study about altered chill experiences in patients with insula lesions is lacking.
Methods
Twenty-eight stroke patients with predominantly insula lesions in the chronic stage and 14 age-matched controls were investigated using chill stimuli of both valences (music, harsh sounds). Group differences were analyzed in subjective chill reports, associated bodily responses (skin conductance response), lesion mapping, diffusion-weighted imaging and functional magnetic resonance imaging. Other neuropsychological deficits were excluded by comprehensive testing. Diffusion-weighted imaging was quantified for four insula tracts using fractional anisotropy.
Results
The frequency of chill experiences was comparable between participant groups. However, bodily responses were decreased for the stroke group. Whereas there was no association of lesion location, a positive association was found for the skin conductance response during aversive sounds and the tract connecting anterior inferior insula and left temporal pole in the stroke group. Similarly, functional magnetic resonance imaging activation in areas hypothesized to compensate for damage was increased with bodily response.
Conclusions
A decoupling of felt arousal and bodily response after insula lesion was observed. Impaired bodily response was related to an impaired interaction of the left anterior insula and the temporal pole.
Objectives: Several clinical disease activity indices (DAIs) have been developed to noninvasively assess mucosal healing in pediatric Crohn’s disease (CD). However, their clinical application can be complex. Therefore, we present a new way to identify the most informative biomarkers for mucosal inflammation from current markers in use and, based on this, how to obtain an easy-to-use DAI for clinical practice. A further aim of our proof-of-concept study is to demonstrate how the performance of such a new DAI can be compared to that of existing DAIs.
Methods: The data of two independent study cohorts, with 167 visits from 109 children and adolescents with CD, were evaluated retrospectively. A variable selection based on a Bayesian ordinal regression model was applied to select clinical or standard laboratory parameters as predictors, using an endoscopic outcome. The predictive performance of the resulting model was compared to that of existing pediatric DAIs.
Results: With our proof-of-concept dataset, the resulting model included C-reactive protein (CRP) and fecal calprotectin (FC) as predictors. In general, our model performed better than the existing DAIs. To show how our Bayesian approach can be applied in practice, we developed a web application for predicting disease activity for a new CD patient or visit.
Conclusions: Our work serves as a proof-of-concept, showing that the statistical methods used here can identify biomarkers relevant for the prediction of a clinical outcome. In our case, a small number of biomarkers is sufficient, which, together with the web interface, facilitates the clinical application. However, the retrospective nature of our study, the rather small amount of data, and the lack of an external validation cohort do not allow us to consider our results as the establishment of a novel DAI for pediatric CD. This needs to be done with the help of a prospective study with more data and an external validation cohort in the future.
Purpose
The German Retina.net ROP registry and its Europe-wide successor, the EU-ROP registry, collect data from patients treated for ROP. This analysis compares input parameters of these two registries to establish a procedure for joint analyses of different registry data using exemplary datasets from the two registries.
Methods
Exemplary datasets from the two databases over a 1-year period each (German Retina.net ROP Registry, 2011, 22 infants; EU-ROP Registry, 2021, 44 infants) were compared. The parameters documented in the two databases were aligned and analysed regarding demographic parameters, treatment modalities, complications within first 24 h and retreatments.
Results
The current analysis showed that data can be aligned for joint analyses with some adjustments within the data structure. The registry with more detailed data collection (EU-ROP) needs to be reduced regarding granularity in order to align the different registries, as the registry with lower granularity determines the level of analyses that can be performed in a comparative approach. In the exemplary datasets, we observed that the overall most common ROP severity in both registries was zone II, 3+ (2011: 70.5%; 2021: 65%), with decreasing numbers of clock hours showing preretinal neovascularisations (2011: 10–12 clock hours in 29% of cases, 2021: 4–6 clock hours in 38%). The most prevalent treatment method was laser coagulation in 2011 (75%) and anti-VEGF therapy in 2021 (86.1%). Within the anti-VEGF group, all patients were treated with bevacizumab in 2011 and with ranibizumab in 2021. Retreatment rates were comparable in 2011 and 2021.
Conclusion
Data from two different ROP registries can be aligned and jointly analysed. The analysis reveals a paradigm shift in treatment modalities, from predominantly laser to anti-VEGF, and within the anti-VEGF group from bevacizumab to ranibizumab in Germany. In addition, there was a trend towards earlier treatment in 2021.
Stigmatisierung tritt bei psychischen, körperlichen sowie chronisch neurologischen Erkrankungen auf. Stigma kann vielfältige Auswirkungen auf Betroffene haben: Es vergrößert Gesundheitsunterschiede, verringert Lebensqualität und schafft Hürden, Gesundheitsleistungen zu nutzen. Internationale Studien zu diesem Thema zeig-ten, dass Stigma bei MS-Patient*innen u.a. die Lebensqualität, das psychische Wohlbefinden, das Offenlegen der Erkrankung und die Einhaltung von Therapien beeinflusst. Hinsichtlich der Stigmatisierung bei chronisch neurologischen Erkran-kungen, wie Multipler Sklerose (MS), gibt es in Deutschland bisher keine Studien. Ziel dieser Arbeit war eine erstmalige Datenerhebung zu Stigmatisierung bei MS. Endpunkte der Erhebung sind, welche Formen von Stigma in dieser Kohorte vorlie-gen und ob es psychische Komponenten, krankheitsspezifische Eigenschaften o-der soziodemographische Daten gibt, die im Zusammenhang mit Stigma stehen. Diese Daten wurden daraufhin in Vergleich zu internationalen Daten gestellt. Auch bisher noch kaum erforschte Assoziationen zu Stigma und Fatigue wurden näher betrachtet.
Die Studie wurde als prospektive Kohortenstudie in Form validierter Fragebögen an der Universität Greifswald (Klinik für Psychiatrie und Klinik für Neurologie) durchge-führt. Zur Auswertung unserer Daten wurden zunächst Basistabellen mit Angaben aus Mittelwert, Standardabweichung, Median und Interquartilenabstand verwendet. Um einen monotonen Zusammenhang zwischen den Variablen zu untersuchen, wurde der Rangkorrelationskoeffizient nach Spearman angewandt; um Assoziatio-nen aufzuzeigen die negative binomiale Regression.
Die Zusammensetzung der Kohorte mit dem Anteil an Männern (26%) und Frauen (74%) ist repräsentativ für MS. Alter und Erkrankungsdauer sind heterogen verteilt. 88 Personen hatten den schubförmig remittierenden MS-Typ, 11 den sekundär pro-gredienten und ein Patient den primär progredienten Verlaufstyp. Der Stigmatisie-rungsgrad in dieser MS-Kohorte ist gering. Der Modalwert für beide Stigma-Skalen liegt jeweils beim Minimum. Stigmatisierung korreliert signifikant auf hohem zweisei-tigen Signifikanzniveau (p>0,05) mit Depression (Korrelationskoeffizient 0,55), Fati-gue (0,51) und Behinderung (0,34). Für Lebensqualität liegt eine negative Korrelati-on vor (-0,54). Bei hohem Signifikanzniveau (p=0,001) erhöhen Behinderung und Depression das Risiko für MS-bezogene Stigmatisierung im Vergleich zu einer ge-sunden Referenzgruppe: Behinderung erhöht es jährlich um 38% und Depression um 5%. Mit jedem weiteren Lebensjahr der Patient*innen sinkt das Stigma-Risiko um 2,7 %. Bei Menschen mit Fatigue steigt das Risiko stigmatisiert zu werden jähr-lich um 2%.
Durch die vorliegende Arbeit konnten Ergebnisse internationaler Studien hinsicht-lich der Zusammenhänge zwischen Depression und Behinderung zu Stigma bestä-tigt werden. Ebenfalls konnte bestätigt werden, dass der Stigmatisierungsgrad bei MS eher gering ist. Der Grad der Behinderung beeinflusst das Stigmatisierungser-leben am stärksten, was sich häufiger in Form von internalisiertem statt öffentlichem Stigma äußert. Dass Jüngere eher betroffen sind, kann mit dem Vorkommen von erwartetem Stigma bei Unvorhersagbarkeit der Diagnose erklärt werden. Das erwar-tete Stigma kann schließlich besonders bei jüngeren Patient*innen zur Verheimli-chung der Erkrankung führen. Dies wurde im Prozess dieser Arbeit herausgearbei-tet und sollte in weiteren Studien noch eingehender untersucht werden. Da im Alter Stigmatisierung vorliegt und Behinderung ebenso wie behinderungsbezogenes Stigma mit dem Alter zunehmen, liegt die Vermutung nahe, dass im Alter andere Formen von Stigma eine Rolle spielen.
Introduction
Patients with bariatric surgery often show poor long-term compliance to recommendations for prevention of nutrient deficiency but it is unclear which factors contribute. We investigated the associations of age, sex, and socioeconomic status (SES) with adherence to guideline recommendations on protein intake and micronutrient supplementation.
Methods
In a monocentric cross-sectional study we prospectively recruited patients with sleeve gastrectomy (SG) or Roux-en-Y gastric bypass (RYGB) and a minimum postoperative period of 6 months. Clinical and demographic data were obtained from the patients’ medical files and by questionnaire. Patients reported on supplement usage, recorded their dietary intake for seven days and underwent physical examinations including blood testing.
Results
We included 35 patients (SG: n = 25, RYGB: n = 10) with a mean (+SD) postoperative period of 20.2 (±10.4) months. Distributions of age, sex and SES were comparable between the SG and RYGB groups. Non-adherence to recommended protein intake was associated with age ≥ 50 years (p = 0.041) but not sex or SES. Protein intake inversely correlated with markers of obesity. There were no significant associations of age or sex with micronutrient supplementation. Only for vitamins A (p = 0.049) and B1 (p = 0.047) higher SES was associated with greater compliance. The only manifest deficiency associated with non-adherence to micronutrient supplementation was that for folic acid (p = 0.044).
Conclusion
In patients after bariatric surgery, those of older age and of lower SES might have a greater risk of unfavorable outcome and may require greater attention to micronutrient and protein supplementation.
Seit Einführung der multimodalen Chemotherapie vor über 30 Jahren liegt die 5- Jahres-Überlebensrate des Osteosarkoms (OS) unverändert bei ca. 70 %. Als potentielle neue Therapieoption ruft kaltes atmosphärisches Plasma (cold atmospheric plasma, CAP) in vitro antiproliferative Effekte in OS-Zellen hervor. Die zugrundeliegenden zellulären und molekularen Mechanismen wurden unter der Hypothese einer Induktion von Apoptose infolge CAP Behandlung untersucht. Effekte von CAP wurden anhand von zwei CAP-Quellen (kINPen MED und MiniJet-R) an zwei OS-Zelllinien (U2OS und MNNG/HOS) überprüft. Hinsichtlich früher apoptotischer Prozesse auf zellulärer Ebene erfolgte die Aktivitätsbestimmung der Effektorcaspasen 3 und 7 im Caspase-Assay. Spät in der apoptotischen Kaskade auftretende molekulare Prozesse wurden durch zwei unabhängige Nachweisverfahren von DNA-Strangbrüchen untersucht – Komet-Assay und TUNEL-Assay.
CAP Behandlungen mit dem kINPen MED führten zu signifikanter Hemmung der Zellproliferation zwischen 24 h und 120 h. Die Effektorcaspasen 3 und 7 zeigten infolge CAP Behandlung nach 24 h und 48 h erhöhte Aktivitätsniveaus. Im Komet- Assay wurden 24 h nach CAP Behandlung in U2OS-Zellen signifikant mehr DNA- Strangbrüche detektiert als in Kontrollansätzen. Der TUNEL-Assay ergab in beiden OS-Zelllinien nach 24 h und 48 h signifikant mehr DNA-Strangbrüche infolge CAP Behandlung. Die Effekte von CAP des kINPen MED konnten durch den MiniJet-R, der erstmals hinsichtlich biologischer Effekte auf maligne Zellen charakterisiert wurde, bestätigt werden. Sowohl antiproliferative Effekte als auch die Prozesse der frühen und späten apoptotischen Kaskade traten signifikant häufiger infolge CAP Behandlung mit dem MiniJet-R auf. Schlussfolgernd gehen antiproliferative Effekte von CAP mit Induktion von Apoptose in OS-Zellen einher, unabhängig von der verwendeten CAP-Quelle.
Die in vitro gezeigte CAP Effekte sollten hinsichtlich der klinischen Anwendung in vivo bestätigt werden. Obgleich die OS-Therapie weiterhin Domäne der Chirurgie und Chemotherapie bleiben wird, bilden die dargestellten zellulären und molekularen Effekte eine aussichtsreiche Grundlage für einen erfolgreichen adjuvanten Einsatz von CAP am OS.
Background
Neuroinflammation and maladaptive neuroplasticity play pivotal roles in migraine (MIG), trigeminal autonomic cephalalgias (TAC), and complex regional pain syndrome (CRPS). Notably, CRPS shares connections with calcitonin gene-related peptide (CGRP) in its pathophysiology. This study aims to assess if the documented links between CRPS and MIG/TAC in literature align with clinical phenotypes and disease progressions. This assessment may bolster the hypothesis of shared pathophysiological mechanisms.
Methods
Patients with CRPS (n = 184) and an age-/gender-matched control group with trauma but without CRPS (n = 148) participated in this case–control study. Participant answered well-established questionnaires for the definition of CRPS symptoms, any headache complaints, headache entity, and clinical management.
Results
Patients with CRPS were significantly more likely to suffer from migraine (OR: 3.23, 95% CI 1.82–5.85), TAC (OR: 8.07, 95% CI 1.33–154.79), or non-classified headaches (OR: 3.68, 95% CI 1.88–7.49) compared to the control group. Patients with MIG/TAC developed CRPS earlier in life (37.2 ± 11.1 vs 46.8 ± 13.5 years), had more often a central CRPS phenotype (60.6% vs. 37.0% overall) and were three times more likely to report allodynia compared to CRPS patients with other types of headaches. Additionally, these patients experienced higher pain levels and more severe CRPS, which intensified with an increasing number of headache days. Patients receiving monoclonal antibody treatment targeting the CGRP pathway for headaches reported positive effects on CRPS symptoms.
Conclusion
This study identified clinically relevant associations of MIG/TAC and CRPS not explained by chance. Further longitudinal investigations exploring potentially mutual pathomechanisms may improve the clinical management of both CRPS and primary headache disorders.

Prüfungsangst ist eine besondere Form der Situationsangst, welche unter einer Vielzahl von Studierenden weit verbreitet ist und durchaus zur negativen Beeinträchtigung der akademischen Leistungen von diesen führt. Es sind bereits mehrere Instrumente erprobt, welche die Angst vor Prüfungen verringern können. Eine mitunter am häufigsten beschriebene Methode ist das Expressive Schreiben, die ausdrucksstarke Offenlegung der Gedanken und Emotionen über eine bevorstehende Prüfung. Die Ohrakupunktur, eine komplementärmedizinische Methode, ist ebenfalls eine vielversprechende Verfahrensweise zur Behandlung von situationsbedingter Angst.
Die vorliegende Studie basiert auf der Grundlage der Ergebnisse einer 2012 verfassten randomisierten Cross-over-Studie durch Klausenitz et al. , wobei gezeigt wurde, dass die Ohrakupunktur bei der Behandlung von Angstzuständen dem Placebo-Verfahren überlegen ist. Um die tatsächliche klinische Wirksamkeit der Ohrakupunktur bei der Behandlung von Prüfungsangst bei Medizinstudierenden vor einer Prüfung weiter zu bewerten, wurde als Vergleich das Expressive Schreiben herangezogen. Die Hypothese, dass sowohl Ohrakupunktur als auch Expressives Schreiben die Angst reduzieren, den Schlaf verbessern und somit die akademische Leistung der Medizinstudenten bei Anatomieprüfungen steigern, hinterfragt den Forschungsbestand dieser Arbeit. In die Studie wurden 37 Zahn- und Humanmedizinstudierende eingeschlossen. Diese legten im Zeitraum von April bis Juli 2014 drei vom Umfang vergleichbare mündliche makroskopische Anatomietestate ab. Vor der jeweiligen Prüfung durchlief jeder Proband randomisiert eine der drei Studienbedingungen: „Ohrakupunktur“, „Expressives Schreiben“ und „keine Intervention“. Die Prüfungsangst wurde mit dem Spielberger State-Trait-Angst-Inventar (STAI) und einer visuellen Analogskala (VAS 100) erfasst. Des Weiteren wurden neben der Analyse der Speichel-Alpha-Amylase die Schlafqualität, die Kreislaufparameter sowie die Prüfungsleistung dokumentiert.
Gegenüber den Ausgangswerten und dem Expressiven Schreiben konnte für die Ohrakupunktur eine signifikante Reduktion der Prüfungsangst, gemessen mit STAI, gezeigt werden. Dabei war der größte Effekt der Ohrakupunktur nach der Intervention am Abend vor der Prüfung zu verzeichnen (STAI State Werte 49 ± 8 vs. 56 ± 11, P = 0.015; Mittelwerte ± Standardabweichung). Des Weiteren konnte eine verbesserte Schlafqualität nach Ohrakupunktur registriert werden.
Das Ziel der vorliegenden Studie bestand darin, die Morphologie der Nasenhöhle und insbesondere der Conchae nasales zweier Primatenspezies, die unterschiedlichen Unterfamilien der geschwänzten Altweltaffen (Cercopithecoidea) angehören, morphometrisch zu untersuchen und zu vergleichen. Die Untersuchungen erfolgten an mazerierten adulten Schädeln beiderlei Geschlechts von Japanischen Makaken (10 Präparate) und Mantelaffen (13 Präparate). Während die Japanischen Makaken (Macaca fuscata) typischerweise einen Sinus maxillaris aufweisen, sind bei den Mantelaffen (Colobus guereza) keine Nasennebenhöhlen nachweisbar. Vor diesem Hintergrund sollte weiterhin geprüft werden, inwiefern das Vorhandensein eines Sinus maxillaris die Morphologie der Nasenhöhle und der Conchae nasales beeinflusst.
Die Messungen erfolgten an definierten koronaren DVT- bzw. CT-Schichtaufnahmen der Schädelpräparate. Dazu wurden die Datensätze mit der 3D MPR-Funktion des Programms RadiAnt® Dicom Viewer (Vers. 5.5.0, Medixant) so ausgerichtet, dass die Horizontalebene durch die Messpunkte Prosthion und Spina nasalis posterior verlief. Diese Ebene wurde nachfolgend als Nasenbodenebene bezeichnet. Senkrecht zur Nasenbodenebene wurden koronare Messebenen definiert, die entsprechend der palatinalen Wurzeln der Oberkieferzähne des zweiten Prämolaren bis zum dritten Molaren positioniert wurden. Die so definierten koronaren Schichtaufnahmen bildeten die Grundlage für Erhebung der Breiten- und Höhenmaße der Nasenhöhle sowie Flächenmaße des Meatus nasi inferior. Für die Erfassung der Conchae nasales wurden die vertikalen Abstände der jeweiligen Grundlamellen in Bezug zur Nasenbodenebene vermessen. Zudem wurden unter der Verwendung der Programme WinSurf (Version 4.0, Lozanoff and Moody) und ImageJ (Version 1.52n, Rasband, National Institutes of Health) 3D-Rekonstruktionen der Nasenmuscheln erstellt und deren Volumina berechnet. Darüber hinaus wurden ausgewählte externe Schädelmaße in den Vergleich mit einbezogen.
Mit Ausnahme der Schädelbasislänge sind die externen Schädelmaße sowie das berechnete Gesichtsschädelvolumen bei Macaca fuscata signifikant größer als bei Colobus guereza. Zugleich wies Macaca fuscata signifikant größere Volumina der Concha nasalis inferior und media auf. Auch die Größenverhältnisse der Conchae nasales innerhalb der jeweiligen Stichproben unterschieden sich zwischen beiden Spezies. Anders als bei Colobus guereza wurde bei Macaca fuscata eine größere Concha nasalis media im Vergleich zur Concha nasalis inferior beobachtet. Die Symmetrieverhältnisse der Nasenmuscheln und ihrer Grundlamellen konnten durch die Berechnung eines Asymmetrie-Index dargestellt werden. Hierbei konnte eine zufällige Verteilung der Asymmetrien bei beiden Spezies festgestellt werden, die als fluktuierende Asymmetrie eingeordnet wurde.
Die Grundlamelle der Concha nasalis inferior wurde bei beiden Primatenspezies in allen vier koronaren Messebenen, vom zweiten Prämolaren bis zum dritten Molaren des Oberkiefers, lokalisiert. Die Grundlamelle der Concha nasalis media lag bei Macaca fuscata auf Höhe der Messebene des dritten Oberkiefermolaren, während sie bei Colobus guereza auf Höhe des zweiten und dritten Oberkiefermolaren beobachtet wurde. Die Grundlamelle der Concha nasalis superior lag bei beiden Primatenspezies vorwiegend im Bereich des zweiten Oberkiefermolaren.
Die Ergebnisse der Breiten- und Höhenmaße der Nasenhöhle zeigten, dass Macaca fuscata in drei von vier koronaren Messebenen eine signifikant höhere Nasenhöhle aufweist. Die Nasenhöhlenbreite ließ hingegen keine Tendenzen in Bezug auf die Größenverhältnisse zugunsten einer Spezies erkennen. Mithilfe eines Breiten-Höhen-Index konnte außerdem das Verhältnis der Breite zur Höhe der Nasenhöhle beschrieben werden. Während dieses Verhältnis im mittleren Bereich der Nasenhöhle bei beiden Spezies annährend gleich ist, besitzt Colobus guereza im anterioren und posterioren Bereich eine breitere Nasenhöhle in Relation zur Gesamthöhe der Cavitas nasi. Es wurden weiterhin signifikante Unterschiede in der Größe des Meatus nasi inferior zwischen beiden Primatenspezies beobachtet.
Mögliche Merkmalszusammenhänge zwischen ausgewählten Maßen der Nasenhöhle sowie externen Schädelmaßen wurden mit der Korrelationsanalyse nach Spearman geprüft. Hierbei wurden bei Colobus guereza keine signifikanten Zusammenhänge zwischen den Volumina der Nasenmuscheln und den externen Schädelmaßen festgestellt. Für Macaca fuscata konnte nur zwischen dem Volumen der rechten Concha nasalis inferior und der Gesichtsschädellänge sowie dem Gesichtsschädelvolumen eine signifikante Korrelation beobachtet werden.
Signifikante Zusammenhänge zwischen den Höhenmaßen der Grundlamellen und den Höhenmaßen der Nasenhöhle wurden bei Macaca fuscata nur für die Concha nasalis inferior im Bereich des zweiten Oberkieferprämolaren sowie bei Colobus guereza für die Concha nasalis media im Bereich des dritten Oberkiefermolaren beobachtet. Die Ergebnisse der Korrelationsanalyse lassen ferner vermuten, dass die Grundlamelle der Concha nasalis inferior eher mit der Nasenhöhlenbreite als mit der Höhe der Nasenhöhle zusammenhängt. Die Ergebnisse der Regressionsanalyse zeigten, dass diejenigen Korrelationen, die bei beiden Primatenspezies gleichermaßen auftraten, keine signifikanten Unterschiede aufweisen.
Die vorliegende Arbeit stellt eine Methodik vor, die es erlaubt, ausgewählte Maße der Nasenhöhle mithilfe von DVT- und CT-Aufnahmen zu erfassen und zwischen verschiedenen menschlichen oder nichtmenschlichen Populationen bzw. unterschiedlichen Primatenspezies zu vergleichen. Die Primatenspezies Macaca fuscata und Colobus guereza wiesen signifikante Unterschiede in verschiedenen Aspekten der Nasenhöhle auf, welche möglicherweise mit den geografischen Lebensräumen sowie der unterschiedlichen Schädelpneumatisation zusammenhängen. Ein Einfluss des Sinus maxillaris auf die Morphologie der Conchae nasales und ihrer Grundlamellen kann nicht ausgeschlossen werden. Weitere Studien sind nötig, um zu prüfen, inwiefern die Anordnung und Größe der Grundlamellen der Conchae nasales artspezifisch sind und in welchem Zusammenhang sie mit anatomischen und physiologischen Charakteristika der Nasenhöhle sowie des Gesichtsschädels stehen. Zudem können die Ergebnisse dieser Studie als Grundlage für weiterführende biometrische Untersuchungen der Nasenhöhle sowie der Conchae nasales dienen.
Life-threatening toxic shock syndrome is often caused by the superantigen toxic shock syndrome toxin-1 (TSST-1) produced by Staphylococcus aureus. A well-known risk factor is the lack of neutralizing antibodies. To identify determinants of the anti-TSST-1 antibody response, we examined 976 participants of the German population-based epidemiological Study of Health in Pomerania (SHIP-TREND-0). We measured anti-TSST-1 antibody levels, analyzed the colonization with TSST-1-encoding S. aureus strains, and performed a genome-wide association analysis of genetic risk factors. TSST-1-specific serum IgG levels varied over a range of 4.2 logs and were elevated by a factor of 12.3 upon nasal colonization with TSST-1-encoding S. aureus. Moreover, the anti-TSST-1 antibody levels were strongly associated with HLA class II gene loci. HLA-DRB1*03:01 and HLA-DQB1*02:01 were positively, and HLA-DRB1*01:01 as well as HLA-DQB1*05:01 negatively associated with the anti-TSST-1 antibody levels. Thus, both toxin exposure and HLA alleles affect the human antibody response to TSST-1.
BACKGROUND
Eosinophilic granuloma (EG) belongs to the family of Langerhans cell histiocytosis (LCH) and is considered to be a benign disease typically found in children younger than 15 years of age. Here, the authors describe an EG of unusual localization and clinical presentation.
OBSERVATIONS
The authors report a 9-year-old girl with an EG presenting as an osteolytic lesion of the clivus. After transsphenoidal resection and histological confirmation, adjuvant chemotherapy was initiated. Presenting signs and symptoms were weight loss, episodic grimacing, and moderate ballism-like movements. After a follow-up-period of 32 months, the patient presented with a total resolution of initial symptoms and no further tumor growth.
LESSONS
Although these lesions are rare, one should consider EG as a differential diagnosis when confronted with osteolytic lesions of the clivus.
In den letzten Jahren gewannen ω-Transaminasen zunehmend an Bedeutung. Ihr breites Substratspektrum, das sowohl Aminosäuren als auch Amine umfasst, macht sie interessant für biotechnologische Anwendungen. Im Gegensatz zu α-Aminotransferasen sind ω-Aminotransferasen nicht auf α-Aminosäuren als Aminodonor bzw. α-Ketosäuren als Aminoakzeptoren beschränkt. Auch sind einige ω-Transaminasen in der Lage, Aldehyde oder Ketone zu aminieren. Dadurch sind sie vielseitig einsetzbar. Seit ihrer Entdeckung wurden ω-Transaminasen in einer Vielzahl von Organismen nachgewiesen. Viele dieser Enzyme stammen aus Pilzen und Bakterien. Da es ständig Bedarf an neuen Transaminasen gibt, wurden verschiedene Organismen auf das Vorhandensein solcher Enzyme untersucht. Die Hefe Blastobotrys raffinosifermentans LS3 ist einer dieser Organismen. Für diese Hefe existiert bereits eine Vielzahl biotechnologischer Anwendungen, was unter anderem an ihren vielseitigen physiologischen Möglichkeiten liegt. Um das Spektrum dieses Stammes noch zu erweitern, wurde sein Genom auf ORFs gescannt. Die ermittelten ORFs wurden translatiert und die so erhaltenen, theoretischen Proteine in einer Proteindatenbank gespeichert. Die Einträge dieser Datenbank wurden einem „hmmerscan“ (hmm ist kurz für „hidden Markov model“) unterzogen. Dabei werden die Proteine in sogenannte Pfams, kurz für Proteinfamilien, eingeteilt. Drei Proteine wurden der Familie PF00202.21 zugeordnet. Das ist die sogenannte Aminotran_3 Familie. In dieser Familie befinden sich eukaryotische ω-Transaminasen. Die Gene brota1, brota2 und brota3 codieren für diese Enzyme. Jeweils eins der Gene wurde in den Vektor XPLOR®3 kloniert, damit die potentiellen ω-Transaminasen in B. raffinosifermentans G1212 [aleu2 atrp1:ALEU2] [1] überexprimiert werden können. Eine Besonderheit von BroTA1 ist, dass es neben der Aminotran_3 Domäne noch eine AAA Domäne aufweist. Deshalb ist es mit etwa 85 kDa auch deutlich größer als die meisten ω-Transaminasen, die meist zwischen 45 und 50 kDa liegen. BroTA2 und BroTA3 beinhalten nur die Aminotran_3 Domäne. Alle drei Enzyme zeigen niedrige Aktivität bei der kinetischen Auflösung racemischer β-Aminosäuren.
Neben den eukaryotischen ω-Transaminasen wurden auch einige bakterielle Enzyme untersucht. Literatursuche und das Screenen der Stammsammlung der Arbeitsgruppe Hefegenetik des Leibniz-Instituts für Pflanzengenetik und Kulturpflanzenforschung führten zu mehreren potentiellen bakteriellen ω-Transaminasen. Das zu Beginn dieser Arbeit noch als hypothetisches Protein bezeichnete Enzym von Variovorax boronicumulans hat sich als ω-Transaminase herausgestellt. Das Enzym wurde detailliert hinsichtlich des Substratspektrums und seiner biochemischen Eigenschaften charakterisiert. Es handelt sich hierbei um eine ω-Transaminase mit β-Aktivität. Diese Transaminase akzeptiert sowohl aromatische als auch aliphatische β-Aminosäuren als Substrat. Sequenzvergleiche dieses Enzyms mit anderen ω-Transaminasen, die nur aliphatische Aminosäuren akzeptieren, führten zu tieferen Einblicken in konservierte Bereiche dieser beiden Gruppen von ω-Transaminasen.
Der dritte Ansatz war das Anpassen einer bekannten ω-Transaminase des thermophilen Bakteriums Sphaerobacter thermophilus an ein potentielles Motiv für aromatische ω-Transaminasen. Dadurch sollte die Aktivität des Enzyms erhöht werden. Dieser Ansatz führte zu 7 Varianten des Enzyms mit höherer Aktivität als der Wildtyp. Durch diese Versuche wurden einige für die Transferaseaktivität wichtige Aminosäurereste offenbart. So hat sich zum Beispiel herausgestellt, dass N70 offenbar wichtig für den Umsatz von γ-Aminosäuren ist, da ein Austausch gegen Glutamat zu einer verminderten Aktivität mit γ-Aminopentansäure führte.
Background: Fatigue, dyspnea, and lack of energy and concentration are commonly interpreted as indicative of symptomatic anemia and may thus play a role in diagnostic and therapeutic decisions. Objective: To investigate the association between symptoms commonly attributed to anemia and the actual presence of anemia. Methods: Data from two independent cohorts of the Study of Health in Pomerania (SHIP) were analyzed. Interview data, laboratory data, and physical examination were individually linked with claims data from the Association of Statutory Health Insurance Physicians. A complete case analysis using logistic regression models was performed to evaluate the association of anemia with symptoms commonly attributed to anemia. The models were adjusted for confounders such as depression, medication, insomnia, and other medical conditions. Results: A total of 5979 participants (53% female, median age 55) were included in the analysis. Of those, 30% reported fatigue, 16% reported lack of energy, 16% reported lack of concentration, and 29% reported dyspnea and/or weakness. Anemia was prevalent in about 6% (379). The symptoms were more prevalent in participants with anemia. However, participants with anemia were older and had a poorer health status. There was no association in multivariate logistic regression models between the symptoms fatigue, lack of concentration, dyspnea, and/or weakness and anemia. Anemia was associated (OR: 1.45; 95% CI: 1.13–1.86) with lack of energy in the multivariate analysis. Other factors such as depression, insomnia, and medication were more strongly associated with the symptoms. Conclusion: The clinical symptoms commonly attributed to anemia are unspecific and highly prevalent both in non-anemic and anemic persons. Even in the presence of anemia, other diagnoses should be considered as causes such as depression, heart failure, asthma, and COPD, which are more closely associated with the symptoms. Further diagnostic research is warranted to explore the association of symptoms in different subgroups and settings in order to help clinical decision making.
The interdisciplinary study investigates the relationship between Norse and Saami peoples in the medieval period and focuses on the multifaceted portrayal of Saami peoples in medieval texts. The investigative analysis is anchored in postcolonial methodologies and argues for the inherent need to decolonise the medieval source-material as well as recent historiography. This is achieved by presenting the historiographic and political background of research into Norse-Saami relations, before introducing an overview of textual sources discussing Saami peoples from the classical period to the late 1400s, an analysis of the textual motifs associated with the Saami in medieval literature (their relevance and prevalence), geo-political affairs, trading relations, personal relations and Saami presence in the south. By using decolonising tools to read Norse-Saami relations in medieval texts, influenced by archaeological material and postcolonial frameworks, the study challenges lingering colonial assumptions about the role of the Saami in Norse society. The current research episteme is re-adjusted to offer alternative readings of Saami characters and emphasis is put on agency, fluidity and the dynamic realities of the Saami medieval pasts.
Explosive volcanic eruptions emitting large amounts of sulfur can alter the temperature of the lower stratosphere and change the circulation of the middle atmosphere. The dynamical response of the stratosphere to strong volcanic eruptions has been the subject of numerous studies. The impact of volcanic eruptions on the mesosphere is less well understood because of a lack of large eruptions in the satellite era and only sparse observations before that period. Nevertheless, some measurements indicated an increase in mesospheric mid-latitude temperatures after the 1991 Pinatubo eruption. The aim of this study is to uncover potential dynamical mechanisms that may lead to such a mesospheric temperature response. We use the Upper-Atmospheric ICOsahedral Non-hydrostatic (UA-ICON) model to simulate the atmospheric response to an idealized strong volcanic injection of 20 Tg S into the stratosphere (about twice as much as the eminent 1991 Pinatubo eruption). Two experiments with differently parameterized effects of sub-grid-scale orography are compared to test the impact of different atmospheric background states. The simulations show a significant warming of the polar summer mesopause of up to 15–21 K in the first November after the eruption. We argue that this is mainly due to intrahemispheric dynamical coupling in the summer hemisphere and is potentially enhanced by interhemispheric coupling with the winter stratosphere. This study focuses on the first austral summer after the eruption because mesospheric temperature anomalies are especially relevant for the properties of noctilucent clouds, whose season peaks around January in the Southern Hemisphere.
The classical Buscher rules d escribe T-duality for metrics and B-fields in a topologically trivial setting. On the other hand, topological T-duality addresses aspects of non-trivial topology while neglecting metrics and B-fields. In this article, we develop a new unifying framework for both aspects.
Introduction: The environmental bacterium Burkholderia pseudomallei causes the often fatal and massively underreported infectious disease melioidosis. Antigens inducing protective immunity in experimental models have recently been identified and serodiagnostic tools have been improved. However, further elucidation of the antigenic repertoire of B. pseudomallei during human infection for diagnostic and vaccine purposes is required. The adaptation of B. pseudomallei to very different habitats is reflected by a huge genome and a selective transcriptional response to a variety of conditions. We, therefore, hypothesized that exposure of B. pseudomallei to culture conditions mimicking habitats encountered in the human host might unravel novel antigens that are recognized by melioidosis patients.
Methods and results: In this study, B. pseudomallei was exposed to various stress and growth conditions, including anaerobiosis, acid stress, oxidative stress, iron starvation and osmotic stress. Immunogenic proteins were identified by probing two-dimensional Western blots of B. pseudomallei intracellular and extracellular protein extracts with sera from melioidosis patients and controls and subsequent MALDI-TOF MS. Among B. pseudomallei specific immunogenic signals, 90 % (55/61) of extracellular immunogenic proteins were identified by acid, osmotic or oxidative stress. A total of 84 % (44/52) of intracellular antigens originated from the stationary growth phase, acidic, oxidative and anaerobic conditions. The majority of the extracellular and intracellular protein antigens were identified in only one of the various stress conditions. Sixty-three immunoreactive proteins and an additional 38 candidates from a literature screening were heterologously expressed and subjected to dot blot analysis using melioidosis sera and controls. Our experiments confirmed melioidosis-specific signals in 58 of our immunoproteome candidates. These include 15 antigens with average signal ratios (melioidosis:controls) greater than 10 and another 26 with average ratios greater than 5, including new promising serodiagnostic candidates with a very high signal-to-noise ratio.
Conclusion: Our study shows that a comprehensive B. pseudomallei immunoproteomics approach, using conditions which are likely to be encountered during infection, can identify novel antibody targets previously unrecognized in human melioidosis.
Introduction
Proteasome inhibition is first line therapy in multiple myeloma (MM). The immunological potential of cell death triggered by defects of the ubiquitin-proteasome system (UPS) and subsequent perturbations of protein homeostasis is, however, less well defined.
Methods
In this paper, we applied the protein homeostasis disruptors bortezomib (BTZ), ONX0914, RA190 and PR619 to various MM cell lines and primary patient samples to investigate their ability to induce immunogenic cell death (ICD).
Results
Our data show that while BTZ treatment triggers sterile type I interferon (IFN) responses, exposure of the cells to ONX0914 or RA190 was mostly immunologically silent. Interestingly, inhibition of protein de-ubiquitination by PR619 was associated with the acquisition of a strong type I IFN gene signature which relied on key components of the unfolded protein and integrated stress responses including inositol-requiring enzyme 1 (IRE1), protein kinase R (PKR) and general control nonderepressible 2 (GCN2). The immunological relevance of blocking de-ubiquitination in MM was further reflected by the ability of PR619-induced apoptotic cells to facilitate dendritic cell (DC) maturation via type I IFN-dependent mechanisms.
Conclusion
Altogether, our findings identify de-ubiquitination inhibition as a promising strategy for inducing ICD of MM to expand current available treatments.
Das Neuroblastom (NB) stellt als Tumor des sympathischen Nervenstranges den häufigsten extrakraniellen soliden Tumor des Kindesalters dar. Ein Bedarf innovativer Therapien, welche eine effektive Anti-Tumorantwort induzieren, ergibt sich aus dessen heterogener Charakteristik, höchst aggressivem Wachstum und trotz Therapien hohen Rezidivraten der high-risk-Patient*innen. In den letzten Jahren konnten durch zunehmende Erkenntnisse im Bereich der Tumor- immunologie verschiedene immuntherapeutische Verfahren etabliert werden. So stellt die Anti-GD2-Therapie (ch14.18/CHO) einen zentralen klinischen Pfeiler der multimodalen Therapie des high-risk-NB dar. Hierbei werden Anti-GD2-AK verab- reicht, die das NB-spezifische Antigen GD2 erkennen, was zur gezielten Aktivierung des körpereigenen Immunsystems hauptsächlich durch NK-Zellen mit anschlie- ßender Tumorzelllyse führt. Herausforderungen ergeben sich aus der Überwindung der immuninhibitorischen Tumorumgebung sowie der Reduzierung natürlicher Regulationsmechanismen.
Vorherige Arbeiten der Arbeitsgruppe haben eine Anti-GD2-Immuntherapie abhängige Induktion des Immuncheckpoints PD-1/PD-L1 sowie der regulatorischen myeloiden CD11b+-Zellen (MRC) gezeigt. Für die vorliegende Arbeit war nun die Rolle der CD11b+ MRC von besonderem Interesse. Hierfür wurden CD11b+ Zellen in einem syngenen NB-Mausmodell mittels Anti-CD11b-AK blockiert oder durch 5-FU selektiv depletiert, was zu einem deutlich reduzierten Tumorwachstum und verbessertem Überleben führte. Zusätzlich wurde mittels RT-PCR die relative Gen- expression MRC-modulierender Gene (Arg1, CCL2, GM-CSF, IDO, IFNγ, IL-1β, IL-4, IL-6, IL-6R, IL-8, IL-10, M-CSF, M-CSFR, iNOS, TGF-β1 und VEGF-A) in der Tumormikroumgebung sowie der Milz evaluiert. Hier wurde eine Reduktion der modulierenden Gene nicht nur in der Tumormikroumgebung, sondern sogar peri- pher durch die Depletion von MRC nachgewiesen. Somit konnte eine negative Rolle der MRC beim NB bestätigt werden.
Diese Ergebnisse stellten eine Grundlage für die kombinierte Immuntherapie aus Anti-GD2-AK und MRC-Depletion dar. Eine Steigerung der antitumoralen Wirksam- keit im Vergleich zu den entsprechenden Monotherapien wurde gezeigt. Dabei wurden eine verbesserte Überlebenswahrscheinlichkeit und ein reduziertes Tumor- wachstum festgestellt. Die vorliegenden Ergebnisse liefern daher die Grundlage für einen neuartigen kombinierten Therapieansatz, um die immer noch schlechten Überlebenschancen der high-risk-Patient*innen zu verbessern.
Poly(vinyl alcohol) (PVA) is a water‐soluble synthetic vinyl polymer with remarkable physical properties including thermostability and viscosity. Its biodegradability, however, is low even though a large amount of PVA is released into the environment. Established physical‐chemical degradation methods for PVA have several disadvantages such as high price, low efficiency, and secondary pollution. Biodegradation of PVA by microorganisms is slow and frequently involves pyrroloquinoline quinone (PQQ)‐dependent enzymes, making it expensive due to the costly cofactor and hence unattractive for industrial applications. In this study, we present a modified PVA film with improved properties as well as a PQQ‐independent novel enzymatic cascade for the degradation of modified and unmodified PVA. The cascade consists of four steps catalyzed by three enzymes with in situ cofactor recycling technology making this cascade suitable for industrial applications.
Background
Long periods of uninterrupted sitting, i.e., sedentary bouts, and their relationship with adverse health outcomes have moved into focus of public health recommendations. However, evidence on associations between sedentary bouts and adiposity markers is limited. Our aim was to investigate associations of the daily number of sedentary bouts with waist circumference (WC) and body mass index (BMI) in a sample of middle-aged to older adults.
Methods
In this cross-sectional study, data were collected from three different studies that took place in the area of Greifswald, Northern Germany, between 2012 and 2018. In total, 460 adults from the general population aged 40 to 75 years and without known cardiovascular disease wore tri-axial accelerometers (ActiGraph Model GT3X+, Pensacola, FL) on the hip for seven consecutive days. A wear time of ≥ 10 h on ≥ 4 days was required for analyses. WC (cm) and BMI (kg m− 2) were measured in a standardized way. Separate multilevel mixed-effects linear regression analyses were used to investigate associations of sedentary bouts (1 to 10 min, >10 to 30 min, and >30 min) with WC and BMI. Models were adjusted for potential confounders including sex, age, school education, employment, current smoking, season of data collection, and composition of accelerometer-based time use.
Results
Participants (66% females) were on average 57.1 (standard deviation, SD 8.5) years old and 36% had a school education >10 years. The mean number of sedentary bouts per day was 95.1 (SD 25.0) for 1-to-10-minute bouts, 13.3 (SD 3.4) for >10-to-30-minute bouts and 3.5 (SD 1.9) for >30-minute bouts. Mean WC was 91.1 cm (SD 12.3) and mean BMI was 26.9 kg m− 2 (SD 3.8). The daily number of 1-to-10-minute bouts was inversely associated with BMI (b = -0.027; p = 0.047) and the daily number of >30-minute bouts was positively associated with WC (b = 0.330; p = 0.001). All other associations were not statistically significant.
Conclusion
The findings provide some evidence on favourable associations of short sedentary bouts as well as unfavourable associations of long sedentary bouts with adiposity markers. Our results may contribute to a growing body of literature that can help to define public health recommendations for interrupting prolonged sedentary periods.

This dissertation explores and tries to unravel the fundamental basis of G-quadruplex end-folding as well as G-quadruplex interactions with small molecules by thermodynamic and structural approaches. Selective targeting of G-quadruplexes with ligands remains elusive, either because the ligand has
considerable binding affinity for other DNA structures or because it fails to discriminate between different G-quadruplex topologies. Unique structural motifs on the G-quadruplex may enhance or inhibit ligand binding to the G-quadruplex. For such aspects, it is necessary to understand the effect of G-quadruplex motifs or elements on the end-folding in order to better tune certain G-quadruplex topologies as model systems. Importantly for targeting G-quadruplex with ligands, motifs called Quadruplex-duplex (QD) junctions and interfaces are shown to be a binding hotspot
for various G-quadruplex ligands containing an intercalator motif. Binding affinity and selectivity of the ligands are discussed with the support of the NMR structures.
Tissue sections, which are widely used in research and diagnostic laboratories and have already been examined by immunohistochemistry (IHC), may subsequently provide a resource for proteomic studies, even though only small amount of protein is available. Therefore, we established a workflow for tandem mass spectrometry-based protein profiling of IHC specimens and characterized defined brain area sections. We investigated the CA1 region of the hippocampus dissected from brain slices of adult C57BL/6J mice. The workflow contains detailed information on sample preparation from brain slices, including removal of antibodies and cover matrices, dissection of region(s) of interest, protein extraction and digestion, mass spectrometry measurement, and data analysis. The Gene Ontology (GO) knowledge base was used for further annotation. Literature searches and Gene Ontology annotation of the detected proteins verify the applicability of this method for global protein profiling using formalin-fixed and embedded material and previously used IHC slides.
Seventeen bacterial strains able to suppress plant pathogens have been isolated from healthy Vietnamese crop plants and taxonomically assigned as members of the Bacillus cereus group. In order to prove their potential as biocontrol agents, we perform a comprehensive analysis that included the whole-genome sequencing of selected strains and the mining for genes and gene clusters involved in the synthesis of endo- and exotoxins and secondary metabolites, such as antimicrobial peptides (AMPs). Kurstakin, thumolycin, and other AMPs were detected and characterized by different mass spectrometric methods, such as MALDI-TOF-MS and LIFT-MALDI-TOF/TOF fragment analysis. Based on their whole-genome sequences, the plant-associated isolates were assigned to the following species and subspecies: B. cereus subsp. cereus (6), B. cereus subsp. bombysepticus (5), Bacillus tropicus (2), and Bacillus pacificus. These three isolates represent novel genomospecies. Genes encoding entomopathogenic crystal and vegetative proteins were detected in B. cereus subsp. bombysepticus TK1. The in vitro assays revealed that many plant-associated isolates enhanced plant growth and suppressed plant pathogens. Our findings indicate that the plant-associated representatives of the B. cereus group are a rich source of putative antimicrobial compounds with potential in sustainable agriculture. However, the presence of virulence genes might restrict their application as biologicals in agriculture.
Methane (CH4) is a potent greenhouse gas with rising atmospheric concentrations.
Microorganisms are essential players in the global methane cycle. In fact, the largest part of methane emissions derives from microbial production by methanogenic Archaea (methanogens). Microorganisms do not only produce methane: methanotrophs can also oxidize the methane produced by methanogens. In addition, soil methanotrophs are the only biological methane sink, oxidizing up to 30-40 Tg of this potent greenhouse gas per year worldwide.
However, intensified management of grasslands and forests may reduce the methane sink capacity of soils.
In general, the interaction of methanogens and methanotrophs determines whether a soil is a source or a sink for methane. It is, therefore, crucial to understand the microbial part of the methane cycle and which factors influence the abundance and activity of methane-cycling microbes. However, capturing the soil microbiome's abundances, activity, and identity is
challenging. There are numerous target molecules and myriad methods, each with certain
limitations. Linking microbial markers to methane fluxes is therefore challenging. This thesis aimed to understand how methane-cycling microbes in the soil are related to soil methane fluxes and how soil characteristics and human activity influence them.
The first publication investigated the biotic and abiotic drivers of the atmospheric methane sink of soils. It assessed the influence of grassland land-use intensity (150 sites) and forest management type (149 sites) on potential atmospheric methane oxidation rates (PMORs) and the abundance and diversity of CH4-oxidizing bacteria (MOB) with qPCR in topsoils of three temperate regions in Germany. PMORs measured in microcosms under defined conditions were approximately twice as high in forest than in grassland soils. High land-use intensity of grasslands negatively affected PMORs (−40%) in almost all regions. Among the different aspects of land-use intensity, fertilization had the most adverse effect reducing PMORs by 20%.
In contrast, forest management did not affect PMORs in forest soils. Upland soil cluster (USC)α was the dominant group of MOBs in the forests. In contrast, USCγ was absent in more than half of the forest soils but present in almost all grassland soils. USCα abundance had a direct positive effect on PMOR in forests, while in grasslands, USCα and USCγ abundance affected PMOR positively with a more pronounced contribution of USCγ than USCα.
In the second publication, we used quantitative metatranscriptomics to link methane-cycling microbiomes to net surface methane fluxes throughout a year in two grassland soils. Methane fluxes were highly dynamic: both soils were net methane sources in autumn and winter and net methane sinks in spring and summer. Correspondingly, methanogen mRNA abundances per
gram soil correlated well with methane fluxes. Methanotroph to methanogen mRNA ratios were higher in spring and summer when the soils acted as net methane sinks. Furthermore, methane uptake was associated with an increased proportion of USCα and γ pmoA and pmoA2 transcripts. High methanotroph to methanogen ratios would indicate methane sink properties.
Our study links the seasonal transcriptional dynamics of methane-cycling soil microbiomes for the first time to gas fluxes in situ. It suggests mRNA transcript abundances as promising indicators of dynamic ecosystem-level processes.
We conclude that reduction in grassland land-use intensity and afforestation can potentially increase the methane sink function of soils and that different parameters determine the microbial methane sink in forest and grassland soils. Furthermore, this thesis suggests mRNA transcript abundances as promising indicators of dynamic ecosystem-level processes. Methanogen transcript abundance may be used as a proxy for changes in net surface methane emissions from grassland soils.
Because of the importance of gastric emptying for pharmacokinetics, numerous methods have been developed for its determination. One of the methods is the salivary tracer technique, which utilizes an ice capsule containing caffeine as a salivary tracer. Despite the ice capsule’s advantage in labeling ingested fluids with caffeine for subsequent salivary detection, its risk of premature melting before swallowing, and its complicated storage and preparation, limit its application, particularly in special populations (e.g., older people). For this reason, here, a compression-coated tablet was developed and validated against the ice capsule in a cross-over clinical trial. The two dosage forms were administered simultaneously to 12 volunteers in an upright position under fasted and fed state conditions. To distinguish the caffeine concentrations in saliva from each dosage form, regular type of caffeine (12C) was added to the tablet, while for the ice capsule 13C3 labelled caffeine was used. The salivary caffeine concentrations showed no statistically significant differences for the pharmacokinetic parameters tmax and AUC0→60 (p > 0.05). Thus, the new formulation is a useful tool for determining gastric emptying that can also be used in special populations.
Although the outcome of patients with acute myeloid leukemia (AML) has
improved in the past decades, the overall survival is below 50% [1, 2] and there
is still an unmet need for the development of new therapeutic strategies. Here,
we aimed to identify functional vulnerabilities in AML and investigated the
therapeutic potential of target structures involved in proteostasis, cell polarity and
RNA-binding molecular pathways.
We determined that genetic deletion of the cell fate determinant and polarity
regulator Scribble delays AML development, however, its deletion also seems to
affect the proliferative capacity of normal hematopoietic cells, lowering its value
as a therapeutic target. In contrast, inactivation of YBX1 (a pleiotropic protein with
DNA/RNA binding capacity that excerpts post-transcriptional control on its
targets) and PSMB8/LMP7 (a catalytic subunit of the immunoproteasome multiprotein
complex that belongs to the ubiquitin-proteasome system (UPS)) inhibit
leukemic cells without influencing normal hematopoietic stem and progenitor cell
function, establishing these targets as potential novel therapeutic strategies
against AML.
Genetic deletion of YBX1 caused reduced proliferation and colony forming
capacity in leukemic cells independent of the oncogenic driver mutation and
delayed AML development in vivo. The role of Ybx1 in leukemia maintenance
was investigated using a conditional knockout model, confirming the functional
requirement of Ybx1 in AML maintenance. Mechanistically, YBX1 recruited
oncogenic transcripts to polysomes, increasing their translation. Displacement of
these transcripts from polysomes after YBX1 deletion decreased their protein
expression.
Genetic and pharmacologic inhibition of PSMB8/LMP7 decreased proliferation
and colony forming capacity selectively in KMT2A (MLL)-rearranged leukemic
cells. In vivo treatment with a PSMB8/LMP7 inhibitor delayed disease
development in KMT2A-rearranged leukemic mice or patient derived xenografts
(PDX). We identified the transcriptional corepressor BASP1 as a functional
effector of the immunoproteasome. BASP1 was enriched after PSMB8/LMP7
inhibition and it was found binding to KMT2A-target genes. Moreover,
pharmacologic inhibition of PSMB8/LMP7 led to decreased expression of bonafide
KMT2A-fusion target genes and enrichment for genes deregulated by
inhibitors of the KMT2A complex partners DOT1L and MEN1. This prompted us
to investigate a potential synergism between MEN1 inhibition and
immunoproteasome inhibition. Combination treatment in AML cells revealed
decreased proliferation in vitro and increased survival in vivo as compared to the
single treatments, demonstrating the therapeutic potential of combining
immunoproteasome and MEN1 inhibitors.
This article reports on the development, implementation and management of a German–Polish telemedicine network in the field of pediatric oncology and hematology in the Euroregion Pomerania. The achievements and challenges of joint medical case reviews involving patients and their care givers, as well as cross-border education activities for physicians, students and nursing staff, are presented. In addition to a progress report, the results of an evaluation of the participants and teachers, likewise the measurement of knowledge growth, are given.
Endoscopic and Histopathological Characteristics of Gastrointestinal Lymphoma: A Multicentric Study

Background: Extranodal non-Hodgkin lymphoma (NHL) is more prevalent in the gastrointestinal (GI) tract than in other sites. This study aimed to determine the endoscopic characteristics of primary gastrointestinal non-Hodgkin lymphomas. Methods: We investigated 140 patients from three tertiary referral hospitals with primary malignant lymphoma of the gastrointestinal tract. Characteristics of the lesions were evaluated and analyzed using image-enhanced endoscopy, endoscopic ultrasound, and histopathology. Results: The median age was 60.5 (range: 11–99), and 59 (42.1%) were female. The most frequent complaint was abdominal pain (74.3%), followed by bloody feces (10%) and diarrhea (2.9%). B symptoms were observed in 15 (10.7%) patients. GI obstruction was the most common complication (10.0%), followed by hemorrhage (7.9%) and perforation (1.5%). Regarding endoscopic findings, the identified sites were the following: the stomach (61.4%), colon (10%), small intestine (10%), ileocecum (8.6%), rectum (6.4%), and duodenum (3.6%). Diffuse large B-cell lymphoma (DLBCL) and mucosa-associated lymphoid tissue (MALT) lymphoma are most prevalent in the stomach. Helicobacter pylori was identified in 46 cases (39.0%), with MALT lymphoma being the most infected subtype. Nearly all gastrointestinal non-Hodgkin lymphomas manifested as superficial type (25–59.6%) and ulcer type (15.6–50%) under endoscopy. We found that fungating type and protruding with ulcer type were more frequent types of aggressive lymphomas (diffuse large B-cell lymphoma, mantle cell lymphoma, and T-cell lymphoma) compared to the indolent types (MALT lymphoma, follicular lymphoma, duodenal-type follicular lymphoma, and small lymphocytic lymphoma) (p < 0.05). Conclusions: This study showed that most subtypes of gastrointestinal non-Hodgkin lymphomas exhibited same endoscopic features (superficial type and ulcer type). Aggressive gastrointestinal non-Hodgkin lymphomas (diffuse large B-cell lymphoma, mantle cell lymphoma, and T-cell lymphoma) were highly suspected when fungating lesions and protruding with ulcer lesions were encountered under endoscopy. Endoscopists should be aware of the connection between enhanced endoscopic characteristics and histological varieties of gastrointestinal lymphoma to improve diagnosis.
Acute pancreatitis (AP) is one of the most common and widely increasing gastrointestinal
diseases leading to hospitalization without specifically available therapy. Among various
etiologies, biliary origin is the most common cause. However, the effects of BAs, given
systemically, on AP remains elusive. A detailed characterization of the mechanisms through
which BAs contribute to the pathogenesis and severity of AP will greatly improve our
understanding of the underlying pathophysiology and may facilitate the development of
treatment, early identification of complications, and prevention for AP. In this view, the roles of
different circulating BAs using in vitro-to-in vivo models were investigated and the underlying
mechanisms through which BAs modulate the severity of AP were addressed. The impact of
hydrophobic and hydrophilic BAs on both, isolated acinar cells and different animal models
induced by repetitive injections of caerulein or L-arginine, ligation of the pancreatic duct (PDL)
or combined bile and pancreatic duct ligation (BPDL), were tested. Disease severity was
assessed by biochemical and histological parameters. Serum CCK concentrations were
determined by enzyme immunoassay. The binding of CCK1 receptor was measured using
fluorescent-labeled CCK. Human BA profiles in AP patients were quantified and that were
correlated with etiology as well as clinical course. In acinar cells, hydrophobic BAs mitigated
the damaging effects of CCK. The same BAs further enhanced pancreatitis in L-arginine and
PDL-based pancreatitis whereas they ameliorated pancreatic damage in the caerulein and
BPDL models, in which CCK was involved. The chemical effect of BAs on protease trypsin
was also observed, however, it was similar between hydrophobic and hydrophilic compounds.
Mechanistically, the binding affinity of the CCK1 receptor was significantly reduced by
hydrophobic BAs. In patients, the sum of hydrophobic but not hydrophilic BAs correlated with
the etiology and severity of AP.
Post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) was
reported to be related with CCK and several pharmaceutical agents have been used to prevent
this most common and potentially severe complication, but those are of limited benefit. In this
regard, our multicenter multinational randomized control trial was designed to compare the
efficacy of indomethacin and N-acetylcysteine (NAC) for the prevention of PEP. A total of 432
ERCP patients from 6 countries were recruited and randomly assigned to receive either NAC
(group A, 84 cases), indomethacin (group B, 138 cases), NAC + indomethacin (group C, 115
cases) or placebo (group D, 95 cases) two hours before procedure. The rate of PEP in groups
A, B and C in comparison with placebo were 10.7%, 17.4%, 7.8% vs 20% (p = 0.08, 0.614 &
0.01, respectively).
Among complications of AP, infection of pancreatic necrosis is one of the most severe
consequence that mostly necessitates interventional therapy. A model to identify parameters
that are useful for the prediction of infected necrosis at an early stage was developed. A
retrospective analysis was conducted in 705 AP patients, who underwent contrast-enhanced computed tomography (CT scan). Both laboratory and clinical parameters were analyzed for
an association with infected pancreatic necrosis, which was microbiologically confirmed. A
logistic regression analysis with stepwise inclusion of significant variables was used to develop
a prediction model. We tested the model quality by receiver operating characteristics analysis.
We found a significant association between 11 parameters with an infection including albumin,
creatinine, C-reactive protein (CRP), and alcoholic etiology, which were independent variables
in the final predictive model with an area under the curve of 0.819.
In the same cohort in which we developed the prediction model above, 89 AP cases with
necrotic complications diagnosed by CT scan were identified. These complications with high
morbidity and mortality required endoscopic drainage, which possibly accompanies severe
adverse events. All complications which occurred in patients who underwent those procedures
and their associated features were retrospectively analyzed. Positive necrosis cultures and a
larger diameter of the intervened necroses were significant factors associated with the
occurrence of adverse events, in which the former was the most significant predictor with Odds
Ratio of 6.1.
The entire work demonstrated that hydrophobicity of BAs and the involvement of CCK are
relevant for the clinical course of AP. Systemic BAs may affect the severity of AP by interfering
with the binding of the CCK1 receptor. Oral NAC is effective for prevention of PEP and the
combination of NAC plus indomethacin resulted in the lowest PEP rate. A model using albumin,
creatinine, CRP, and alcoholic etiology can differentiate infected and sterile pancreatic
necrosis and control of infection is crucial for successful endoscopic drainage therapy in
complicated AP. The exact pathophysiologic mechanisms, especially in CCK-related pathways,
and the potential impact of BAs in human AP, in particular in preventing PEP, need to be investigated in further studies.
Das Pankreaskarzinom zählt zu den fünf häufigsten Krebserkrankungen in Deutschland, die durch eine spät einsetzende Symptomatik und ein schnelles Wachstum zum Zeitpunkt der Diagnose eine minimale Aussicht auf Heilungserfolg hat. Ziel dieser Arbeit war es, die onkolytische Therapie des Pankreaskarzinoms sicherer zu machen und eine bessere Implementierung in den klinischen Alltag zu erreichen.
In vorherigen Tierversuchen dieser Arbeitsgruppe wurde das Virus im aktiven oder inaktiven Zustand intravenös appliziert und so eine in vivo Stimulation von Spendermäusen vorgenommen. Nach adoptivem Zelltransfer dieser stimulierten Splenozyten in tumortragenden Mäusen konnte eine Onkolyse beobachtet werden, jedoch war die Versuchsdurchführung hinsichtlich der klinischen Anwendung noch zu fern. Aus diesen vorangegangenen Versuchen entwickelte sich das in dieser Arbeit besprochene Modell in dem die Stimulation von entnommenen Splenozyten oder Blutleukozyten in vitro erfolgt. Zusätzlich wurde überprüft, ob nach der Behandlung des Organismus eine weitere Virusübertragung stattfinden kann.
Der Transfer in tumortragende Mäuse wurde zu zwei Zeitpunkten (24h und 72h nach in vitro Stimulation) vorgenommen. Die Onkolyse war zu beiden Zeitpunkten und mit beiden Leukozytengruppen erfolgreich, sodass die Tumore makroskopisch nicht mehr zu erkennen waren oder 10% des Volumens der Kontrollgruppen maßen. Darüberhinaus wurden die reisolierten Tumorzellen der Versuchstiere hinsichtlich der Veränderung ihrer Oberflächenproteine sowie die mesenteriale Lymphknoten auf die Veränderung der Leukozytenpopulationen analysiert. Die Ergebnisse zeigten, dass die Anzahl regulatorischer T-Lymphozyten, deren Rolle die Immunsuppression sowohl im physiologischen als auch im pathologischen Milieu ist, nach dem Transfer von stimulierten Leukozyten deutlich abnimmt. Die Analyse der reisolierten Tumorzellen hat gezeigt, dass die Menge von MHC I ansteigt und die des PD-L1 sinkt, wodurch die Tumorzellen anfälliger für den zytotoxischen Angriff durch T-Lymphozyten sind. Diese Ergebnisse liefern den deutlichen Hinweis, dass der Rückgang der Tumormasse nicht auf das aktive Virus im Organismus, sondern auf die Aufhebung der Immunsuppression und die Erhöhung der Immunogenität des Tumors zurückzuführen ist.
Acute pancreatitis (AP), which is characterized by self-digestion of the pancreas by its own prematurely activated digestive proteases, is a major reason for hospitalization. The autodigestive process causes necrotic cell death of pancreatic acinar cells and the release of damage associated molecular pattern which activate macrophages and drive the secretion of pro-inflammatory cytokines. The MYD88/IRAK signaling pathway plays an important role for the induction of inflammatory responses. Interleukin-1 receptor associated kinase-3 (IRAK3) is a counter-regulator of this pathway. In this study, we investigated the role of MYD88/IRAK using Irak3−/− mice in two experimental animal models of mild and severe AP. IRAK3 is expressed in macrophages as well as pancreatic acinar cells where it restrains NFκB activation. Deletion of IRAK3 enhanced the migration of CCR2+ monocytes into the pancreas and triggered a pro-inflammatory type 1 immune response characterized by significantly increased serum levels of TNFα, IL-6, and IL-12p70. Unexpectedly, in a mild AP model this enhanced pro-inflammatory response resulted in decreased pancreatic damage, whereas in a severe AP model, induced by partial pancreatic duct ligation, the increased pro-inflammatory response drives a severe systemic inflammatory response syndrome (SIRS) and is associated with an increased local and systemic damage. Our results indicate that complex immune regulation mechanism control the course of AP, where moderate pro-inflammation not necessarily associates with increased disease severity but also drives tissue regenerative processes through a more effective clearance of necrotic acinar cells. Only when the pro-inflammation exceeds a certain systemic level, it fuels SIRS and increases disease severity.
Biological invasions pose global threats to biodiversity and ecosystem functions. Invasive species often display a high degree of phenotypic plasticity, enabling them to adapt to new environments. This study examines plasticity to water stress in native and invasive Opuntia ficus-indica populations, a prevalent invader in arid and semi-arid ecosystems. Through controlled greenhouse experiments, we evaluated three native and nine invasive populations. While all plants survived the dry treatment, natives exhibited lower plasticity to high water availability with only a 36% aboveground biomass increase compared to the invasives with a greater increase of 94%. In terms of belowground biomass, there was no significant response to increased water availability for native populations, but plants from the invasive populations showed a 75% increase from the dry to the wet treatment. Enhanced phenotypic plasticity observed in invasive populations of O. ficus-indica is likely a significant driver of their success and invasiveness across different regions, particularly with a clear environmental preference towards less arid conditions. Climate change is expected to amplify the invasion success due to the expansion of arid areas and desertification. Opuntia ficus-indica adapts to diverse environments, survives dry spells, and grows rapidly in times of high-water supply, making it a candidate for increased invasion potential with climate change.
Ecological Impacts and Phenotypic Plasticity of a Global Invasive Cactus, Opuntia ficus-indica

Biological invasions by non-native species pose one of the major threats to biodiversity, the way ecosystems function, and the well-being of humans. These invasions can occur through various means, including accidental or intentional introductions by humans, natural dispersal, and climate change. Non-native species can harm the native species and ecosystems, by homogenizing plant communities, competing for resources, changing how the ecosystem operates, and eventually causing native species to go extinct. Even though not all non-native species become invasive, changes in climate and ecosystems can cause the successful establishment and spread of non-native species. Despite the advancements in our understanding of biological invasions in recent years, research has been biased towards temperate regions, whereas arid and semi-arid regions that are highly impacted by climate change are underrepresented. Thus, particularly focusing on the impacts of biological invasions in subtropical arid and semi-arid regions, the goal of this PhD project was to explore the effects of an invasive cactus on the local native communities and investigate the mechanisms of its successful invasion. Certain species are found to take advantage of the ever-drying climates in the arid/ semi-arid regions of the world. Opuntia ficus-indica, native to Mexico, is an exceptionally successful drought-tolerant invasive cactus that successfully grows in these regions. O. ficus-indica, a most widespread invasive cactus, is considered an ecosystem engineer as it modifies the habitats of indigenous plant species and dependent animals. This project aimed to identify the ecological impacts of O. ficus-indica in the highlands of Eritrea, the competitive potential of O. ficus-indica and the plastic changes that enabled its spread and invasion (Chapters I-III). For this purpose, field observations and common garden experiments were carried out throughout the project.
We investigated the effects of Opuntia ficus-indica on the spatial diversity of native plant communities (Chapter I), its competitive ability against native species (Chapter II) and the phenotypic plasticity of O. ficus-indica (Chapter III). To investigate the main ecological effects of O. ficus-indica on the native community, field data was collected from the highlands of Eritrea and comparisons were made between O. ficus-indica invaded and noninvaded areas (Chapter I). The study aimed to understand the effects of O. ficus-indica by examining species composition, richness, and diversity across vegetation layers and revealed that O. ficus-indica homogenises the species composition of the native ecosystem. This provides evidence that the presence of O. ficus-indica reduces landscape-level heterogeneity or spatial diversity. However, O. ficus-indica did not influence the species richness and diversity of the local communities. The mechanisms of the successful homogenisation of the local communities by O. ficus-indica were attributed to the potential competitive abilities of O. ficus-indica against the native species, and the plastic and adaptive traits it developed in the non-native ranges. The first assumption was tested by setting up a common garden competition experiment between two native Eritrean species, Ricinus communis and Solanum marginatum (Chapter II). The experiment used two water availability treatments, wet and dry, and categorized plants into intraspecific (native or invasive only) and interspecific (native and invasive) competition. The study evaluated the impacts by comparing the growth of O. ficus-indica alone to the growth alongside native species which revealed the weak competitive potential of O. ficus-indica. However, O. ficus-indica was observed to outgrow the native species in several folds which can be attributed to its successful invasion. The second assumption of the successful spread of O. ficus-indica was attributed to the phenotypic plastic traits adapted by O. ficus-indica in the non-native ranges (Chapter III). The phenotypic plasticity of O. ficus-indica was assessed by exposing it to water stress across dry and wet environments. The species were cultivated from a diverse set of 12 populations, encompassing its native range in Mexico with three cultivars and nonnative ranges in Africa (Algeria, Eritrea, Ethiopia), the island of Madeira off the coast of Africa, and in Europe, Italy with two cultivars and in Portugal from three sites. In Mexico and Italy, we collected various cultivars to ensure a wide representation of genotypes. We found that the species originating from the native range of O. ficus-indica exhibited lower plasticity to conditions of elevated water availability. Furthermore, a trial gradient experiment on O. ficus-indica was conducted to determine the appropriate watering levels for the species and the experiment revealed not only the species' capacity to endure a lack of water for nine months but also its ability to withstand prolonged waterlogged conditions.
This thesis illustrates the fact that invasive species are a major threat to biodiversity and ecosystem functioning worldwide, especially in rarely studied regions with dry climates and limited resources. How can invasive plants spread and cause negative impacts on native ecosystems (Chapter I), despite their weak competitive abilities (Chapter II)? This thesis explored these questions by examining the case of O. ficus-indica, an invasive species in arid/ semi-arid climates (Chapter I). It showed that O. ficus-indica has a high growth potential that allows it to overcome resource limitations, that its growth is not affected by competition from native species (Chapter II), and that it exhibits adaptive plasticity that enhances its invasion success in different environments (Chapter III). This thesis revealed the complex mechanisms and consequences of biological invasions in dry climates and contributes to the understanding of invasive species. It also suggests that more research is needed in understudied regions to assess the impacts of O. ficus-indica or invasive species in general on native biodiversity and ecosystem services and to identify the factors that influence the competitive and adaptive potentials.
Until today, more than 17% of the population in Mecklenburg Western-Pomerania suffer from chronic kidney disease (CKD) which was revealed by the SHIP study (Study of Health in Pomerania). 20% of CKD cases can be traced back to glomerulopathies. One common characteristic of glomerulopathies is the morphologic change of the glomerular filtration barrier which consists of endothelial cells, the glomerular basement membrane and podocytes. Under healthy conditions, the foot processes of the podocytes interdigitate with the foot processes of the neighboring podocytes with a filtration slit in between. Apart from the slit membrane protein nephrin, typical adherens junction proteins like occludin or JAM-A are also expressed at this cell-cell junction. This junction is therefore considered to be a specialized type of adherens junction, necessary to maintain the size-selectivity of the filtration barrier. During podocyte injury, podocyte foot processes lose their characteristic morphology and the typical meandering filtration slit becomes linearized, a process which is described as foot process effacement.
Since morphological change is directly linked to change or loss of function, ultrastructural analysis of the foot processes is necessary for diagnostics and research. By using 3D-structured illumination microscopy (3D-SIM), we quantified these morphological changes as well as studied a possible biomarker, the tight junction protein claudin 5 (CLDN5). Our study showed a spatially restricted up-regulation of CLDN5 in effaced filtration slit areas in biopsies of patients suffering from minimal change disease (MCD), focal and segmental glomerulosclerosis (FSGS) as well as in mice after NTS injection and in the uninephrectomy DOCA-salt mouse model. CLDN5/nephrin ratios of biopsies from patients with glomerulopathies and of tissue received from NTS-treated mice were significantly higher compared to controls. We found that in patients the CLDN5/nephrin ratios were negatively correlated with the filtration slit density. Since CLDN5 up-regulation was observed in several areas of high filtration slit density, we hypothesized that CDLN5 upregulation preceded visible foot process effacement. Taken together, we suggest that CLDN5 could be a helpful biomarker to identify an early change of the foot process morphology in addition to filtration slit density measurement. Additionally, correlation analysis of foot process effacement with patient data showed a significant negative correlation of the filtration slit density with proteinuria in MCD patients.
Combining solid acid catalysts with enzyme reactions in aqueous environments is challenging because either very acidic conditions inactivate the enzymes, or the solid acid catalyst is neutralized. In this study, Amberlyst-15 encapsulated in polydimethylsiloxane (Amb-15@PDMS) is used to deprotect the lignin depolymerization product G−C2 dioxolane phenol in a buffered system at pH 6.0. This reaction is directly coupled with the biocatalytic reduction of the released homovanillin to homovanillyl alcohol by recombinant horse liver alcohol dehydrogenase, which is subsequently acylated by the promiscuous acyltransferase/hydrolase PestE_I208A_L209F_N288A in a one-pot system. The deprotection catalyzed with Amb-15@PDMS attains up to 97 % conversion. Overall, this cascade enables conversions of up to 57 %.
In this thesis, new catalysts as well as unprecedented approaches for the
valorization of sustainable carbon sources were investigated. The first part deals with the design of catalysts for photocatalytic CO2 reduction (Articles I&II). The promiscuous activity of phenolic acid decarboxylase from Bacillus subtilis (BsPAD) was found to catalyze CO2 reduction (Article I). This cofactor-free enzyme could facilitate the replacement of (noble) metal catalysts regularly employed in CO2 reduction. Based on these findings, additional enzyme catalysts were identified for photocatalytic CO2 reduction. The second part (Articles III-VII) focuses on the valorization of resources obtained from biomass, such as olive mill waste water or lignin, by the promising acyltransferases/hydrolase PestE from Pyrobaculum calidifontis VA1 (Articles IV-VII). The potential of PestE for the valorization of sustainable sources has been demonstrated by enzyme engineering and use in (chemo)enzymatic cascade reactions leading to value-added products.
Der Nationale Kompetenzbasierte Lernzielkatalog Zahnmedizin (NKLZ) wurde 2015 verabschiedet und definiert die Lernziele für die zahnmedizinische Ausbildung in Deutschland. Er legt fest, welche Kompetenzen Studierende erwerben sollen, und dient als Grundlage für die curriculare Gestaltung des Studiums, der Prüfungen und der Unterrichtsmaterialien. Der NKLZ fördert eine umfassende Ausbildung, die neben fachlichem Wissen auch klinische Fähigkeiten und Fertigkeiten, professionelles Verhalten und kommunikative Kompetenzen einschließt. Er trägt zur Vorbereitung angehender Zahnmediziner:innen auf ihren Beruf bei und standardisiert die Ausbildung, um Qualität und Vergleichbarkeit sicherzustellen.
Der vorliegende Artikel beschreibt Hintergründe, Geschichte, Aufbau und die Weiterentwicklung des NKLZ. Derzeit wird der NKLZ zur Version 2.0 in einem mehrstufigen Prozess weiterentwickelt. Seine Struktur orientiert sich am NKLM 2.0, dem Nationalen Kompetenzbasierten Lernzielkatalog Medizin. Ziel ist es, die Anforderungen für die Zahnärztliche Approbationsordnung umfassend abzubilden.
Eine wichtige Voraussetzung für die offizielle Anerkennung des NKLZ als grundlegender Leitfaden für die Ausbildung von Zahnmediziner:innen ist die Verankerung in einer zeitnah zu reformierenden Approbationsordnung Zahnmedizin. Dadurch werden Klarheit und Verbindlichkeit für Lehrende und Studierende geschaffen. Eine solche Verankerung ermöglicht zudem eine bessere Abstimmung zwischen Ausbildungszielen und den Anforderungen der Berufspraxis. Da die Approbationsordnung weniger häufig angepasst wird, bietet die Einbindung des NKLZ die Möglichkeit, Aktualisierungen und Anpassungen des Lernzielkatalogs strukturiert und reguliert vorzunehmen. Dies gewährleistet, dass die Ausbildung den aktuellen Standards und Entwicklungen entspricht.
Posttranslational modifications are involved in the regulation of virtually all cellular processes, including immune response, nevertheless, they are also targets manipulated by invading pathogens. The first investigated example is protein citrullination which is an important posttranslational modification that acts on a multitude of processes like supervision of cell pluripotency and rheumatoid arthritis. Citrullination of targeted arginine residues is performed by the Peptidylarginine deiminase. Within the first published manuscript, being part of this thesis, it was possible to show the use of this posttranslational modification by the human pathogen Porphyromonas gingivalis to facilitate innate immune evasion at three distinct level. P. gingivalis was demonstrated to citrullinate proteins by Porphyromonas peptidylarginine deiminase resulting in diminished phagocytosis and subsequent killing by neutrophils. Furthermore, it was shown that citrullination of histone H3 enables P. gingivalis to survive in neutrophil extracellular traps and incapacitate the lysozyme-derived peptide LP9.
The second investigated posttranslational modification is ubiquitination and its role in respiratory tract infections. Ubiquitination is the covalent attachment of a small protein that consisting of only 76 amino acids to the ε-amino group of lysine residues to posttranslational modify proteins. Acute infections of the lower respiratory tract such as viral and bacterial co-infections are among the most prevalent reasons of fatal casualties worldwide. Therefore, the interactions between host and pathogens resulting in the impairment of the hosts immune response and immune evasion of the pathogens, need to be elucidated. To get new insights in the infection driven changes in protein polyubiquitination and alterations in the abundance of ubiquitin E3 ligases involved in ubiquitination, cellular proteomes were monitored in detail by high resolution mass spectrometry. Therefore, the epithelial cell lines 16HBE14o- (Manuscript II) and A549 (Manuscript III) were co-infected with influenza A virus H1N1 and Streptococcus pyogenes or Staphylococcus aureus or with influenza A virus H1N1 and Streptococcus pneumoniae, respectively. Here, it could be shown in 16HBE14o- cells that co-infection of epithelial cells is not characterized by decreased cell survival and that observable effects on the proteome and ubiquitinome are mostly additive rather than synergistic. S. pyogenes infection affected the mitochondrial function, cell-cell adhesion, endocytosis and actin organization. Viral infection affected mRNA processing and Rho signaling. Viral and bacterial co-infection was detected to affect processes that were already affected by both of the corresponding single infections. No further pathways were strongly affected by the co-infection. A similar result has been observed in A549 cells co-infected IAV and S. pneumoniae. Overrepresented gene ontology terms depict the sum of those observed in the viral and bacterial single infection. Moreover, no significant change in cell survival upon co-infection compared to single bacterial infection was noticed for A549 cells either. This led to the suggestion that co-infection of investigated epithelial cells under examined conditions possesses additive rather than synergistic effect and thus, may not worsen the outcome of the infection within the studied conditions. Infections in other systems, may provide varying results and thus should be examined in future studies.
Das Krankheitsbild des Hydrozephalus stellt mit seiner komplexen Pathophysiologie eine Herausforderung in der Neurochirurgie dar. Die Folgen dieser Erkrankung können von leichten Beschwerden, über schwere demenzielle Symptome bis hin zum vorzeitigen Tod reichen. [7] Durch eine frühzeitige Behandlung können dessen Symptome und das Risiko für krankheits-bedingte Komplikationen reduziert werden. Als Therapieformen stehen verschiedene Shuntsysteme und die endoskopische Drittventrikulostomie (ETV) zur Verfügung, wobei sich letztere insbesondere bei obstruktivem Hydrozephalus als bevorzugte Therapieoption etablierte [14, 68, 92, 107]. In der bisherigen Literatur wurden die Erfolgsraten bei einem kommunizierenden Hydrozephalus zwischen 21 % und 75 % angegeben [14, 43, 54, 58-61, 64]. Zudem können bei bestimmter Indikation mehrere ETV durchgeführt werden. Bislang wurden diese sogenannten Rezidiv-ETV in 15 Studien analysiert, wobei die Erfolgsrate zwischen 37,1 % und 100% lag [25, 33, 34, 53, 55, 73-82].
Ziel dieser Arbeit war es die Effektivität und Sinnhaftigkeit einer ETV bei kommunizierendem Hydrozephalus und einer Rezidiv-ETV zu überprüfen und im Hinblick auf Alternativtherapien kritisch zu analysieren sowie mögliche Prädiktoren für einen Erfolg statistisch auszuwerten.
Für die vorliegende Dissertation wurden Daten von 378 Patienten mit insgesamt 408 ETV über einen Langzeitverlauf von 294 Monaten ausgewertet, von denen jeweils 29 Patienten eine ETV bei kommunizierendem Hydrozephalus oder mindestens eine Rezidiv-ETV erhielten. Dabei konnten keine Erfolge bei kommunizierendem Hydrocephalus verzeichnet werden, weshalb dieser keine geeignete ETV-Indikation darstellt und somit zukünftig eine Shunt-Implantation sinnhafter ist. Die Gesamterfolgsrate bei Patienten mit Rezidiv-ETV betrug 31%. In der statistischen Analyse fiel auf, dass tendenziell mehr pädiatrische Patienten in der Misserfolgsgruppe der Rezidiv-ETV vorkamen und die Erfolgsgruppe ausschließlich aus männlichen Patienten bestand. Trotzdessen zeigte sich kein statistisch signifikanter Unterschied zwischen den Patienten mit Erfolg und Misserfolg der Rezidiv-ETV in Bezug auf Ätiologie, den Misserfolgsgrund der initialen ETV und das Lebensalter. Die statistische Signifikanz hinsichtlich des Geschlechtes sollte kritisch hinterfragt werden. Eine VP-Shunt-Implantation vor initialer ETV zeigte eine leichte, jedoch nicht signifikante, Reduktion der Erfolgsrate einer Rezidiv-ETV. Der umfangreiche Langzeitverlaufs dieser Arbeit erlaubte einen Vergleich zwischen frühem und spätem Versagen einer Rezidiv-ETV, wobei die Erfolgsrate bei frühen Rezidiv-ETV tendenziell geringer, jedoch ohne statistische Signifikanz, ausfiel.
The cortical silent period (CSP), assessed with transcranial magnetic stimulation (TMS), provides insights into motor cortex excitability. Alterations in the CSP have been observed in multiple sclerosis (MS), although a comparison of the sometimes contradictory results is difficult due to methodological differences. The aim of this study is to provide a more profound neurophysiological understanding of fatigue’s pathophysiology and its relationship to the CSP. Twenty-three patients with MS, along with a matched control group, underwent comprehensive CSP measurements at four intensities (125, 150, 175, and 200% resting motor threshold), while their fatigue levels were assessed using the Fatigue Scale for Motor and Cognitive Functions (FSMC) and its motor and cognitive subscore. MS patients exhibited a significantly increased CSP duration compared to controls (p = 0.02), but CSP duration was not associated with the total FSMC, or the motor or cognitive subscore. Our data suggest a systematic difference in MS patients compared to healthy controls in the CSP but no association with fatigue when measured with the FSMC. Based on these results, and considering the heterogeneous literature in the field, our study highlights the need for a more standardized approach to neurophysiological data collection and validation. This standardization is crucial for exploring the link between TMS and clinical impairments in diseases like MS.
The reduction of individual carbon consumption could make an important contribution to the worldwide effort to limit global warming. Based on Bandura’s theory of moral disengagement, we hypothesized that the propensity to morally disengage concerning high-carbon behaviors (e.g., eating meat or traveling by plane) is one important factor that prevents individuals from reducing their carbon footprint. To measure the propensity to morally disengage in high-carbon-related behavior contexts, a questionnaire (MD-HCB) was developed and psychometrically validated in an online study with a German sample (N = 220). Confirmatory factor analyses revealed that the final nine-item scale had a one-dimensional structure, as intended. The internal consistency of the scale was excellent (Cronbach’s α = 0.94) and the scale interpretation had predictive validity for both past low-carbon consumption behavior and the intention to engage in such behavior in the future. Correlational analyses with relevant existing instruments confirmed the construct validity of the interpretations that can be drawn from the MD-HCB, as its resulting score is related to, yet separable from, the general tendency to morally disengage and is meaningfully connected to related constructs. A pre-study with a student sample (N = 89) not only helped to identify limitations in the study design but also showed a weak predictive ability of moral competence concerning high-carbon consumption behavior and intention to change. Based on our findings, future media campaigns designed to increase people’s intention to reduce high-carbon behavior could focus on the modification of common cognitive disengagement strategies.
Ziel von Statistikkursen an Universitäten ist es, den Studierenden statistische Kompetenz als Grundlage des
wissenschaftlichen Arbeitens zu vermitteln. Trotzdem verlassen Studierende diese Kurse teilweise mit statistischen Fehlkonzepten und können das Gelernte in Studium und Praxis nicht anwenden. Zudem bereiten den Studierenden hohe Statistikangst und ein geringes Interesse an Statistik als Teil ihres Studienfachs Probleme.
In der vorliegenden Arbeit geht es um die Frage, wie Statistikkurse zu Studienbeginn gestaltet sein sollten, um die statistische Kompetenz der Studierenden besser zu fördern. Dabei wird davon ausgegangen, dass für statistische Kompetenz neben kognitiven Voraussetzungen in Form von gut vernetztem und in Schemata gespeichertem Wissen in Statistik und anderen Bereichen, auch motivationale Voraussetzungen notwendig sind. Als zentrale motivationale Voraussetzung werden dabei Kompetenzüberzeugungen in Statistik aufgefasst. Solche Kompetenzüberzeugungen zeigen nicht nur kurz- und langfristige Zusammenhänge zur Leistung, sondern stehen auch mit anderen motivationalen Konstrukten wie Angst oder Interesse in Verbindung. In der folgenden Arbeit wurden zwei Untersuchungen durchgeführt, welche auf die Bedeutung dieser Kompetenzüberzeugungen bei der Entwicklung und Förderung der statistischen Kompetenz in Statistikkursen fokussierten.
Die erste Untersuchung befasste sich damit, ob Kompetenzüberzeugungen zu Beginn eines Statistikkurses mit Lernverhalten und Leistung zum Ende des Kurses zusammenhängen. Dabei interessierte vor allem, ob a) eine besonders hohe oder eine besonders realistische Überzeugung von der eigenen statistischen Kompetenz zu Kursbeginn von Vorteil ist und b) ob eine Überzeugung von der Veränderbarkeit der statistischen Kompetenz die Einflüsse der anfänglichen Überzeugungen moderieren kann. In der Untersuchung wurden n = 88 Psychologiestudierende in mehreren Statistikkursen zu Beginn und zum Ende eines Semesters befragt. Es zeigte sich, dass eine hohe Kompetenzüberzeugung mit besseren Leistungen einhergeht – während die Kompetenzüberzeugung für bessere Leistungen nicht unbedingt auch realistisch sein muss. Zudem ist eine Überzeugung von der Veränderbarkeit von Kompetenz von Vorteil: sie kann negative Effekte einer geringen Kompetenzüberzeugung zu Beginn des Semesters auf die Leistung kompensieren, führt aber auch bei einer unrealistisch hohen Kompetenzüberzeugung zu besseren Leistungen. In der zweiten Untersuchung wurde analysiert, ob ein nach dem Lehrformat des „Inverted Classroom“ (IC) unterrichteter Statistikeinführungskurs die Kompetenzüberzeugungen und damit verbundene Konstrukte von Statistikangst und Interesse sowie die Leistung von Studierenden fördern kann. Letzteres sollte darauf zurückzuführen sein, dass im Gegensatz zu „traditionellen Vorlesungen“ (TL) im IC Möglichkeiten zur individuellen Anpassung des Kurses an die Voraussetzungen von Studierenden zur Verfügung stehen und die Studierenden so beim Lernen weniger überfordert und motivierter sind. In der Untersuchung wurden n = 27 Studierende ein Semester lang in einem Statistikkurs im IC und n = 43 Studierende in einem Kurs als TL unterrichtet. Eine zusätzliche Kontrollgruppe (KG) von n = 24 Studierenden erhielt keinen Kurs. Die Ergebnisse zeigten, dass Studierende im IC zwar bessere und auch homogener Leistungen erreichen als im TL, dies kann jedoch nicht auf die Überforderung oder Motivation beim Lernen zurückgeführt werden. Auch die Kompetenzüberzeugungen und das Interesse waren im IC deutlich höher. Sowohl im IC als auch in der TL wurde die Statistikangst im Semesterverlauf geringer, während sich in der KG keine Veränderung zeigt.
Zu Beginn der Statistikausbildung sollten infolgedessen Kompetenzüberzeugungen – insbesondere auch die Überzeugung von der Veränderbarkeit statistische Kompetenz – gefördert werden. Dazu sind individualisierte Lehrformate wie der IC geeignet, welche die Entwicklung statistischer Kompetenz und statistischer Kompetenzüberzeugungen ermöglichen. Solche individualisierten Lehrformate können nicht nur in Statistikkursen zu Beginn, sondern auch in Statistikkursen im weiteren Studienverlauf eingesetzt werden.
In dieser Arbeit sollte untersucht werden welche Probleme aus der ersten ärztlichen Leichenschau bei der zweiten Leichenschau vor Feuerbestattung im Zeitraum vom 01.01.2018-31.12.2021 im Versorgungsgebiet des Institutes für Rechtsmedizin der Universitätsmedizin Greifswald auffielen und zum Anhalten des Leichnams führten. Für den gleichen Zeitraum wurde zusätzlich im Rahmen der amtsärztlichen Kontrolle die Fehler der Todesbescheinigungen des Landkreises Vorpommern-Greifswald ausgewertet. Außerdem wurde die Einflussmöglichkeit durch Fortbildungen auf die Qualität der ärztlichen Leichenschau betrachtet.
Einleitend wurde die obligatorische ärztliche Leichenschau und die Vielfältigkeit der Maßnahmen zur Qualitätsverbesserung und -sicherung in den unterschiedlichen Bundesländern betrachtet. Des Weiteren wurde das zeitaufwendige Verfahren des Institutes für Rechtsmedizin der Universitätsmedizin Greifswald geschildert mit dem versucht wurde durch kleine, individuelle Schulungen und Fortbildungen die leichenschauenden Ärzte auf Fehler hinzuweisen und so die Qualität der ärztlichen Leichenschau zu verbessern.
Es wurden daher die Angaben des Blatt 3 des Vertraulichen Teiles der Todesbescheinigung in M-V der 36.419 Sterbefälle, die in den Krematorien Greifswald und Neubrandenburg kremiert wurden und auch die Ergebnisse der zweiten Leichenschauen vor Feuerbestattung, retrospektiv untersucht. Die Fehler und Auffälligkeiten wurden in vordefinierte Kategorien erfasst und den verschiedenen Arztkategorien zugeordnet. Bei der amtsärztlichen Kontrolle der Todesbescheinigungen wurden die dokumentierten Fehler der 15.449 Sterbefälle des Landkreises Vorpommern-Greifswald retrospektiv untersucht. Hierbei wurden die Fehler in vordefinierte Kategorien erfasst und den verschiedenen Arztkategorien zugeordnet. Es wurden die Fortbildungsmaßnahmen erfasst die durch die Ärzte des Institutes für Rechtsmedizin der Universitätsmedizin Greifswald in den Jahren 2018-2021 zum Thema „Ärztliche Leichenschau“ durchgeführt wurden. Außerdem wurden im Zeitraum von 2019-2021 die dokumentierten Anrufe von leichen-schauenden Ärzten auf dem Diensttelefon der Rechtsmedizin mit Nachfragen zur ärztlichen Leichen-schau dokumentiert und retrospektiv ausgewertet.
Festzustellen war, dass im untersuchten Zeitraum zunächst ein Rückgang der angehaltenen Fälle bei der zweiten Leichenschau vor Feuerbestattung zu verzeichnen war mit einem erneuten Anstieg 2021 sogar über das Ausgangsniveau hinaus. Nummerisch wurden die meisten Fehler durch im Krankenhaus tätige Ärzte gemacht. In den einzelnen Fehlerkategorien konnte kein eindeutiger rückläufiger Trend fest-gestellt werden, bzw. zumeist zeigte sich ein erneuter Anstieg der Zahlen im Jahr 2021. Bei den Versuchen die angehaltenen Fälle telefonisch zu klären, konnten 40 % der Fälle nach einem Telefonanruf direkt wieder durch die Rechtsmedizin freigegeben werden. Bei den gerichtlich angeordneten Sektionen zeigte sich prozentual eine deutliche Zunahme im Vergleich zu den bei der Polizei angezeigten Fällen. Die meisten Fälle (55 %), die einer gerichtlichen Obduktion zugeführt wurden, waren bei der zweiten Leichenschau vor Feuerbestattung wegen Petechien angehalten worden. Bei den amtsärztlich kontrollierten Todesbescheinigungen des Landkreises Vorpommern-Greifswald lagen die Fehlerquoten zwischen 8,31 % und 12,72 %. Dabei entfielen auf die Kategorien der formalen Fehler bis zu 60 % der zu beanstandenden Todesbescheinigungen, wobei hier zumeist „Kodierung fehlend“ (44 %) und „Sterbezeitpunkt fehlend“
(33 %) auffielen. In der Kategorie der Diagnosefehler entfielen 33 % der fehlerhaften Todesbescheinigungen auf „keine kausale Verknüpfung“. In der Kategorie „Sterbezeitpunkt fehlend“ war über den unter-suchten Zeitraum ein statistisch signifikanter Rückgang zu verzeichnen. Bei den Diagnosefehlern waren statistisch signifikante Rückgänge in den Kategorien „keine Diagnose angegeben“ und „nur Endzustand angegeben“ feststellbar. Die Anzahl der telefonischen Nachfragen von Ärzten zur Leichenschau auf dem Diensttelefon stieg im untersuchten Zeitraum stetig an. Ein deutlicher Rückgang war im Bereich der Fortbildungstätigkeiten zum Thema „Ärztliche Leichenschau“ zu verzeichnen, da durch die SARS-CoV-2-Pandemie die Fortbildungen abgesagt werden mussten.
Die Qualität der ärztlichen Leichenschau war vergleichbar mit anderen Studien. Ganz entscheidend für die Qualität scheint neben der studentischen Lehre, wiederkehrende Fortbildungsmaßnahmen zu sein. Dies deckt sich mit dem deutlichen Rückgang der Schulungsmaßnahmen im Jahr 2020 und 2021 und dem deutlichen Anstieg an angehaltenen Fälle in den Krematorien im Jahr 2021 und der Zunahme der fehlerhaften Todesbescheinigungen. Es sollte über verpflichtende Schulungen zum Thema „Ärztlichen Leichenschau“ und die Vorstellung des bundeslandspezifischen BestattGes und der Todesbescheinigung bei Aufnahme der ärztlichen Tätigkeit in einem anderen Bundesland nachgedacht werden.
In M-V wäre zudem eine Anpassung der Todesbescheinigungen, bezüglich der Möglichkeit eine natürliche Todesart zu deklarieren hilfreich, die Möglichkeit eine Todesbescheinigung als „vorläufig“ zu markieren sollte geschaffen werden.
Gerade auch bei den Todesbescheinigungen scheint es trotz vorgegebener Kriterien, gerade bei den Diagnosefehlern, einen größeren Ermessensspielraum zu geben, so dass einzelnen Studienergebnisse nicht unmittelbar miteinander vergleichbar scheinen. Trotz des hohen zeitlichen Aufwandes der telefonischen Kontaktaufnahmen bei Anhaltungen im Krematorium und den schriftlichen Korrekturvorschlägen bei fehlerhaft ausgestellten Todesbescheinigungen ist unsererseits die enge Zusammenarbeit mit den klinischen Fachrichtungen wichtig und erfolgsversprechend.
In this work, 2-dimensional measurements in the THz frequency range with self-made spintronic THz emitters were presented. The STE were used to optimize the spatial resolution and determine the magnetization in geometric shapes. At the beginning, various combinations of FM and NM layers were produced and measured to achieve an optimal composition of the STE. The layer thickness of the ferromagnetic CoFeB layer and the nonmagnetic PT layer was also varied. The investigations have shown that a layer combination of 2 nm thick CoFeB and 2 nm thick Pt, applied to a fused silica glass substrate and covered with a 300 nm thick SiO2 layer, emits the highest THz amplitude. Based on these, a structured sample, consisting of an STE and an additional layer system of 5 nm Cr and 100 nm Au, was produced. Further, three wedge-shaped structures were removed from the gold layer by an etching process so that the THz radiation generated by the STE can pass through these areas. This enables the optimization of the resolution of the system. For this purpose, the sample was moved perpendicular to the laser beam by two stepping motors with a step size of 5 μm and imaged 2-dimensionally. By reducing the step size to 0.2 μm, the beam diameter could be measured at the edge of the structure using the knife-edge method. Based on this measurement, the resolution of the system could be determined as 5.1 ± 0.5 μm at 0.5 THz, 4.9 ± 0.4 μm at 1 THz, and 5.0 ± 0.5 μm at 1.5 THz. These results are confirmed by simulations considering the propagation of THz wave packets through the SiO2. The expansion of the FWHM of the waves, passing through the 300 nm thick layer, is about 1%. Only a SiO2 layer with a thickness in the μm range occurs an expansion of around 10%. This shows that it is possible to perform 2-dimensional THz spectroscopy with a resolution in the dimension of the exciting laser beam by using near-field optics. Afterward, the achieved spatial resolution was used to investigate the influence of external magnetic fields on the STE and the emitted THz radiation. By implementing a pair of coils above the sample, an external magnetic field could be applied parallel to the pattern. The used sample was designed in such a way that only certain geometric areas on the fused silica glass substrate were coated with an STE so that THz radiation is emitted only in those areas. The 2-dimensional images show the geometric structures for f = 1.0 THz and f = 1.5 THz clearly. By applying a permanent, positive magnetic field (+M), a positive course of the THz amplitude can be seen. A rotation of the magnetic field by 180° (-M) leads to a reversal of the orientation of the emitted THz radiation, whereby the magnetic field does not influence the corresponding frequency spectrum. By using minor loops, the sample was demagnetized by the constant reduction of the magnetic field strength with alternating magnetic field direction. The 2-dimensional representation of the pattern with a step size of 10 μm shows that the sample was demagnetized since both, positively and negatively magnetized structures, could be imaged. In addition, in the 2nd row from the top, a completely demagnetized circle and a rectangle with a division into two domains can be seen. These structures have both positive and negative magnetized areas, which are separated by a domain wall. To investigate this in more detail a 2-dimensional measurement of the divided regions was made with a step size of 2.5 μm. These images confirm the division of the structures into positive and negative domains, separated by a domain wall, which was verified by Kerr-microscope measurements. Both data show a similar course of the domains and the domain wall. However, to be able to examine the domain wall more precisely using 2-dimensional THz spectroscopy, the resolution of the system must be improved to a range of a few nm, because the expected domain wall width is between 𝑙𝑊 = 12.56 nm and 𝑙𝑊 = 125.6 nm. The improved resolution would make it possible to image foreign objects, such as microplastics in biological cells or tissue. For this purpose, different plastics, such as polypropylene, polyethylene, and polystyrene, were investigated in the THz frequency range up to 4 THz. While no specific absorption could be determined for PP, characteristic absorption peaks were found for PE and PS. The energy of the photons with a frequency of about 2.2 THz excites lattice vibrations in the PE. Therefore, this frequency is specifically absorbed, and the intensity in the transmission spectrum is lower than for other frequencies. PS absorbs especially THz radiation with a frequency of 3.2 THz. In addition, all of the investigated plastics are mostly transparent for THz radiation, which makes imaging of these materials feasible. Based on these basic properties, it will be possible to image and identify these types of plastic.
Background
Pregnancy and the postpartum period are times when women are at increased risk for depression and mental problems. This may also negatively affect the foetus. Thus, there is a need for interventions with low-threshold access and care. Telemedicine interventions are a promising approach to address these issues. This systematic literature review examined the efficacy of telemedicine interventions for pregnant women and/or new mothers to address mental health-related outcomes. The primary objective was to analyse whether telemedicine interventions can reduce mental health problems in pregnant women and new mothers. The secondary aim was to clarify the impact of type of interventions, their frequency and their targets.
Methods
Inclusion criteria: randomized controlled trials, with participants being pregnant women and/or new mothers (with infants up to twelve months), involving telemedicine interventions of any kind (e.g. websites, apps, chats, telephone), and addressing any mental health-related outcomes like depression, postnatal depression, anxiety, stress and others. Search terms were pregnant women, new mothers, telemedicine, RCT (randomised controlled trials), mental stress as well as numerous synonyms including medical subject headings. The literature search was conducted within the databases PubMed, Cochrane Library, Web of Science and PsycINFO. Screening, inclusion of records and data extraction were performed by two researchers according to the PRISMA guidelines, using the online tool CADIMA.
Results
Forty four articles were included. A majority (62%) reported significantly improved mental health-related outcomes for participants receiving telemedicine interventions compared to control. In particular (internet-delivered) Cognitive Behavioural Therapy was successful for depression and stress, and peer support improved outcomes for postnatal depression and anxiety. Interventions with preventive approaches and interventions aimed at symptom reduction were largely successful. For the most part there was no significant improvement in the symptoms of anxiety.
Conclusion
Telemedicine interventions evaluated within RCTs were mostly successful. However, they need to be designed to specifically target a certain mental health issue because there is no one-size-fits-all approach. Further research should focus on which specific interventions are appropriate for which mental health outcomes in terms of intervention delivery modes, content, target approaches, etc. Further investigation is needed, in particular with regard to anxiety.
Die weltweit zunehmende gesellschaftliche Mobilität, Globalisierungsbewegungen, Flüchtlingsströme und der Fernreisetourismus führen verstärkt zur Einreise von Migrant*innen mit genitalen, dentalen und weiteren Mutilationen ihres Körpers nach Europa. Dieses hat neben der gesellschaftlichen eine außerordentliche medizinische Relevanz. Eine patientenorientierte, erfolgreiche medizinische Behandlung ohne negative psychische und physische Folgen ist nur auf der Grundlage profunder Kenntnisse der kulturellen und ethnienspezifischen Besonderheiten möglich. Die Doktorarbeit wendet sich daher zunächst der Analyse von Zahndeformierungen mit ihren Ursachen, Folgen und den assoziierten Mutilationen zu und beschreibt bekannte Formen. Diese sind innerhalb des subsaharischen Raums Afrikas die einfache Zuspitzung der Zähne, die Zacken- und Lückenfeilung der Zähne, das Ausbrechen von Zähnen und das Verdrängen der Zähne aus ihrer ursprünglich anatomisch korrekten Stellung. In einem folgenden Schritt werden die genitalen Mutilationen wie u.a. die Klitorektomie, Labienexzision und Infibulation dargestellt und dazu die Ursachen, Folgen, juristische Dimension sowie Handlungsmöglichkeiten erläutert. Weiterhin werden Mutilationsformen aufgeführt, die im europäischen Raum kaum populär sind. Zu diesen zählen das sog. „Brustbügeln“, die Brustinzisionen, die Verlängerung der Genitalien sowie die Mutilationen der Skopzen. Die Prozedur des Brustbügelns ist in der Arbeit hinsichtlich ihres protektiven Motivs und ihrer mutilationsspezifischen Eigenheiten eingehend beschrieben. Die Brustinzisionen erfolgen bei der gezielten Anlegung von Narben (Skarifikationen) innerhalb eines Abhärtungsprozesses. Die Elongation der Labien richtet sich durch exzessives Dehnen der Genitalien auf eine rein ästhetische Komponente. In der Sekte der Skopzen wird die weibliche Brust mit dem Ziel eines Askesezustandes entfernt.
Weiter zählen zu den Mutilationspraktiken die im Kindesalter angebrachten „Infant Mutilation“. Mit der Enukleation, dem Ausreißen von Zahnkeimen und dem „Tea-Tea, der Zufügung von Schnitten auf der Brust, soll im Fall einer kindlichen Krankheit eine vermeintliche Kuration erreicht werden.
Mutilationspraktiken des subsaharischen Raums sind nach Literatur, allgemeinen Berichten und Berichten von Augenzeugen und Augenzeuginnen nicht auf einen Körperteil begrenzt und werden nicht auf lediglich eine Art ausgeführt. Es gibt einzelne Hinweise darauf, dass sie in Kombination auftreten. Dieses mögliche Auftreten von Kombinationen begründet in der vorliegenden Arbeit die Entwicklung der elementaren These, dass bei dem Vorhandensein offensichtlich erkennbarer Mutilationen im Sinne einer Korrelation auf weitere vorhandene Mutilationen zu schließen ist.
Für den Beleg dieser These wird auf der Grundlage einer systematischen Literaturrecherche aus einer tabellarischen Erfassung, einer kartographischen und einer inhaltlichen Betrachtung das zentrale Fazit dieser Arbeit gewonnen.
Die genannte tabellarische Auflistung erfasst in einer Dreigliederung einerseits Ethnien mit Zahndeformierungen, kategorisiert gemäß der Einteilung von Schröder, andererseits gleichzeitig auftretende Genitalmutilationen, kategorisiert gemäß der Einteilung der WHO und letztlich weitere Mutilationen. Die quantitativ am stärksten mit Korrelationen vertretene Zahnmutilationsart ist das Ausbrechen von Zähnen (Typ III), gefolgt von der einfachen Zuspitzung der Zähne (Typ I). Hinsichtlich der genitalen Verstümmelungen zeigt die Kliterektomie (WHO-Typ I), gefolgt von der Labienexzision (WHO-Typ II) die meisten Korrelationen.
Die genannte Auswertung kartographischen Materials lässt den Schluss zu, dass sich ein räumlich geographisches Vorkommen dentaler und genitaler Mutilationen wie ein Gürtel über den afrikanischen Kontinent zieht und mit geringen Ausbreitungs-unterschieden im subsaharischen Raum beträchtliche Korrespondenzen aufweist. Der genannte Überschneidungsbereich verläuft gleichermaßen gürtelartig. Dieser ist in einer eigenhändig erstellten Karte dargestellt und ermöglicht Ärzt*innen schnell eine Risikoeinschätzung ihrer Patientinnen bei Wissen um deren Herkunft.
Die letzte und inhaltliche Korrelationsebene ergibt sich aus ethnienspezifischen Grundlagen. Bei vielen afrikanischen Völkern ist das Leben in Abschnitte mit unterschiedlichen gesellschaftsinternen Rechten und Verpflichtungen gegliedert. Die jeweilige Stufe findet ihren Ausdruck in entsprechend angebrachten Mutilationen. Die sog. „Initiationsstufen“ werden im Lebenszyklus durchlaufen. Es werden sukzessive Mutilationen an unterschiedlichen Körperstellen vorgenommen. Sofern eine Ethnie dem Initiationsritus folgt, ist das Vorliegen mehrerer Mutilationsarten wahrscheinlich und künftige Deformierungen sind nicht auszuschließen. Entsprechend ist dieses exemplarisch für den Lebenszyklus der Massai dargelegt.
Die vorliegende Arbeit soll einen wichtigen Beitrag zur medizinischen Aufklärung über subsaharische Mutilationspraktiken leisten. Sie gibt Hilfestellung für ein interdisziplinäres Vorgehen in Diagnostik und Therapie. Die Dissertation soll als Ausgangspunkt für weitere interdisziplinäre Forschung verstanden werden.
Post-COVID-19 syndrome (PCS) has been described as ‘the pandemic after the pandemic’ with more than 65 million people worldwide being affected. The enormous range of symptoms makes both diagnosis complex and treatment difficult. In a post-COVID rehabilitation outpatient clinic, 184 patients, mostly non-hospitalized, received a comprehensive, interdisciplinary diagnostic assessment with fixed follow-up appointments. At baseline, three in four patients reported more than 10 symptoms, the most frequent symptoms were fatigue (84.9%), decreased physical capacity (83.0%), tiredness (81.1%), poor concentration (73.6%), sleeping problems (66.7%) and shortness of breath (67.3%). Abnormalities were found in the mean values of scores for fatigue (FAS = 34.3), cognition (MoCA = 25.5), psychological alterations (anxiety, depression, post-traumatic stress disorder), limitation of lung function (CAT) and severity scores for PCS (PCFS, MCRS). Clinical abnormalities were found in elevated values of heart rate, breathing rate at rest, blood pressure and NT-proBNP levels. As the frequency of the described symptoms decreases only slowly but most often significantly over the course, it is important to monitor the patients over a longer period of time. Many of them suffer from an immense symptom burden, often without pre-existing clinical correlates. Our results show a clear association with objectifiable assessments and tests as well as pronounced symptoms.
Comparative neuroanatomy of the central nervous system in web-building and cursorial hunting spiders

Spiders (Araneae) include cursorial species that stalk their prey and more stationary species that use webs for prey capture. While many cursorial hunting spiders rely on visual cues, web-building spiders use vibratory cues (mechanosensation) for prey capture. We predicted that the differences in primary sensory input between the species are mirrored by differences in the morphology/architecture of the central nervous system (CNS). Here, we investigated the CNS anatomy of four spider species, two cursorial hunters Pardosa amentata (Lycosidae) and Marpissa muscosa (Salticidae), and two web-building hunters Argiope bruennichi (Araneidae) and Parasteatoda tepidariorum (Theridiidae). Their CNS was analyzed using Bodian silver impregnations, immunohistochemistry, and microCT analysis. We found that there are major differences between species in the secondary eye pathway of the brain that pertain to first-order, second-order, and higher order brain centers (mushroom bodies [MB]). While P. amentata and M. muscosa have prominent visual neuropils and MB, these are much reduced in the two web-building species. Argiope bruennichi lacks second-order visual neuropils but has specialized photoreceptors that project into two distinct visual neuropils, and P. tepidariorum lacks MB, suggesting that motion vision might be absent in this species. Interestingly, the differences in the ventral nerve cord are much less pronounced, but the web-building spiders have proportionally larger leg neuropils than the cursorial spiders. Our findings suggest that the importance of visual information is much reduced in web-building spiders, compared to cursorial spiders, while processing of mechanosensory information requires the same major circuits in both web-building and cursorial hunting spiders.
Foraging behavior, neuroanatomy and neuroplasticity in cursorial and stationary hunting spiders

The central nervous system (CNS) is the integration center for the coordination and regulation of
all body activities of animals and the source of behavioral patterns, behavioral plasticity and
personality. Understanding the anatomy and the potential for plastic changes of the CNS not only
widens the knowledge on the biology of the respective species, but also enables a more
fundamental understanding of behavioral and ecological patterns. The CNS of species with
different sensory ecologies for example, will show specific differences in the wiring of their CNS,
related to their lifestyle. Spiders are a group of mesopredators that include stationary hunting
species that build webs for prey capture, and cursorial hunting species that do not build capture
webs. These distinct lifestyles are associated with major differences in their sensory equipment,
such as size of the different eyes.
In this thesis, I aimed to answer if a cursorial mesopredator would change its behavior due to
different levels of perceived predation risk, and if this behavior would be influenced by individual
differences (chapter 1); how the visual pathways in the brain of the cursorial hunting jumping
spider Marpissa muscosa differs from that of the nocturnal cursorial hunting wandering spider
Cupiennius salei (chapter 2); to what degree the visual systems of stationary and cursorial hunting
spiders differ and whether CNS areas that process vibratory information show similar differences
(chapter 3); and finally if the CNS in stationary and cursorial hunting spiders shows different
patterns of neuroplasticity in response to sensory input and deprivation during development
(chapter 4).
In chapter 1, I found that jumping spiders adjust their foraging behavior to the perceived level of
risk. By favoring a dark over a light substrate, they displayed a background-matching strategy.
Short pulses of acute risk, produced by simulated bird overflights, had only small effects on the
behavior. Instead, a large degree of variation in behavior was due to among-individual differences
in foraging intensity. These covaried with consistent among-individual differences in activity,
forming a behavioral syndrome. Our findings highlight the importance of consistent amongindividual
differences in the behavior of animals that forage under risk. Future studies should
address the mechanisms underlying these stable differences, as well as potential fitness
consequences that may influence food-web dynamics.
In chapter 2, I found that the visual pathways in the brain of the jumping spider M. muscosa differ
from that in the wandering spider C. salei. While the pathway of the principal eyes, which are
responsible for object discrimination, is the same in both species, considerable differences occur
in the pathway of the secondary eyes, which detect movement. Notably, M. muscosa possesses
an additional second-order visual neuropil, which is integrating information from two different
secondary eyes, and may enable faster movement decisions. I also showed that the tiny posterior
median eye is connected to a first-order visual neuropil which in turn connects to the arcuate body
(a higher-order neuropil), and is thus not vestigial as suggested before. Subsequent studies should
focus on exploring the function of the posterior median eyes in different jumping spider species,

as they show considerable inter-specific size differences that may be correlated with a differing
connectivity in the brain.
In chapter 3, I described all neuropils and major tracts in the CNS of two stationary (Argiope
bruennichi and Parasteatoda tepidariorum) and two cursorial hunting spiders (Pardosa amentata
and M. muscosa). I found major differences in the visual systems of the secondary eyes between
cursorial and stationary hunting spiders, but also within the groups. A. bruennichi has specialized
retinula cells in two of the secondary eyes, which connect to different higher-order neuropils. P.
tepidariorum has only a single visual neuropil connected to all secondary eyes, and lacks
recognizable mushroom bodies. The neuroanatomy of CNS areas that process mechanosensory
information on the other hand, is remarkably similar between cursorial and stationary hunting
species. This suggests that the same major circuits are used for the processing of mechanosensory
information in both cursorial and stationary hunting spiders. Future studies on functional aspects
of sensory processing in spiders can build on the findings of our study.
In chapter 4, I found that developmental neuroplasticity in response to sensory input differs
between a cursorial (M. muscosa) and a stationary hunting spider (P. tepidariorum). While
deprivation of sensory input leads to a volume increase in several visual and mechanosensory
neuropils M. muscosa, neither sensory deprivation nor sensory enrichment had an effect on the
volume of neuropils in P. tepidariorum. However, exposure to mechanical cues during
development had an effect on the allometric scaling slope of the leg neuropils in both M. muscosa
and P. tepidariorum. Future studies should focus on the genetic and cellular basis of
developmental neuroplasticity in response to sensory input in order to explain the observed
patterns.
Introduction: At the cellular level, acute temperature changes alter ionic conductances, ion channel kinetics, and the activity of entire neuronal circuits. This can result in severe consequences for neural function, animal behavior and survival. In poikilothermic animals, and particularly in aquatic species whose core temperature equals the surrounding water temperature, neurons experience rather rapid and wide-ranging temperature fluctuations. Recent work on pattern generating neural circuits in the crustacean stomatogastric nervous system have demonstrated that neuronal circuits can exhibit an intrinsic robustness to temperature fluctuations. However, considering the increased warming of the oceans and recurring heatwaves due to climate change, the question arises whether this intrinsic robustness can acclimate to changing environmental conditions, and whether it differs between species and ocean habitats.
Methods: We address these questions using the pyloric pattern generating circuits in the stomatogastric nervous system of two crab species, Hemigrapsus sanguineus and Carcinus maenas that have seen a worldwide expansion in recent decades.
Results and discussion: Consistent with their history as invasive species, we find that pyloric activity showed a broad temperature robustness (>30°C). Moreover, the temperature-robust range was dependent on habitat temperature in both species. Warm-acclimating animals shifted the critical temperature at which circuit activity breaks down to higher temperatures. This came at the cost of robustness against cold stimuli in H. sanguineus, but not in C. maenas. Comparing the temperature responses of C. maenas from a cold latitude (the North Sea) to those from a warm latitude (Spain) demonstrated that similar shifts in robustness occurred in natural environments. Our results thus demonstrate that neuronal temperature robustness correlates with, and responds to, environmental temperature conditions, potentially preparing animals for changing ecological conditions and shifting habitats.
Animals often respond to climate change with changes in morphology, e.g., shrinking body size with increasing temperatures, as expected by Bergmann’s rule. Because small body size can have fitness costs for individuals, this trend could threaten populations. Recent studies, however, show that morphological responses to climate change and the resulting fitness consequences cannot be generalized even among related species. In this long-term study, we investigate the interaction between ambient temperature, body size and survival probability in a large number of individually marked wild adult female Natterer’s bats (Myotis nattereri). We compare populations from two geographical regions in Germany with a different climate. In a sliding window analysis, we found larger body sizes in adult females that were raised in warmer summers only in the northern population, but not in the southern population that experienced an overall warmer climate. With a capture-mark-recapture approach, we showed that larger individuals had higher survival rates, demonstrating that weather conditions in early life could have long-lasting fitness effects. The different responses in body size to warmer temperatures in the two regions highlight that fitness-relevant morphological responses to climate change have to be viewed on a regional scale and may affect local populations differently.
Objectives
To investigate the effect of layer height of FFF-printed models on aligner force transmission to a second maxillary premolar during buccal torquing, distalization, extrusion, and rotation using differing foil thicknesses.
Materials and methods
Utilizing OnyxCeph3™ Lab (Image Instruments GmbH, Chemnitz, Germany, Release Version 3.2.185), the following movements were programmed for the second premolar: buccal torque (0.1–0.5 mm), distalization (0.1–0.4 mm), extrusion (0.1–0.4 mm), rotation (0.1–0.5 mm), and staging 0.1 mm. Via FFF, 91 maxillary models were printed for each staging at different layer heights (100 µm, 150 µm, 200 µm, 250 µm, 300 µm). Hence, 182 aligners, made of polyethylene terephthalate glycol (PET-G) with two thicknesses (0.5 mm and 0.75 mm), were prepared. The test setup comprised an acrylic maxillary model with the second premolar separated and mounted on a sensor, measuring initial forces and moments exerted by the aligners. A generalized linear model for the gamma distribution was applied, evaluating the significance of the factors layer height, type of movement, aligner thickness, and staging on aligner force transmission.
Results
Foil thickness and staging were found to have a significant influence on forces delivered by aligners, whereas no significance was determined for layer height and type of movement. Nevertheless, at a layer height of 150 µm, the most appropriate force transmission was observed.
Conclusions
Printing aligner models at particularly low layer heights leads to uneconomically high print time without perceptible better force delivery properties, whereas higher layer heights provoke higher unpredictability of forces due to scattering. A z-resolution of 150 µm appears ideal for in-office aligner production combining advantages of economic print time and optimal force transmission.

Zusammenfassung der Dissertation
Vor dem Hintergrund der In-Office-Alignerherstellung beschäftigt sich die vorliegende Dissertation mit der Fragestellung, inwiefern die Layerhöhe im FFF-Druck Einfluss auf die Oberflächengüte der Modelle sowie auf die Kraftabgabe der darauf tiefgezogenen Aligner hat.
Intention des ersten Teils war es, herauszufinden, ob es im Wechselspiel zwischen Druckgeschwindigkeit und Oberflächenqualität eine optimale Layerhöhe im FFF-Druck gibt. Des Weiteren wurde der Einfluss des Druckmaterials (Lignin basiertes Biopolymer und Polylactid basiertes Biopolymer) auf die Oberflächengüte untersucht. Im zweiten Teil sollte eruiert werden, welchen Effekt die Layerhöhe auf die Kraftabgabe des Aligners hat. Neben dem Parameter Layerhöhe wurden die Faktoren geplanter Setupschritt, Alignerdicke sowie Zahnbewegungsart, und ihre Auswirkungen auf die resultierenden Kräfte der Aligner untersucht.
Für den ersten Teil wurden, basierend auf einem randomisiert ausgewählten Oberkieferteilmodell, 18 Modelle mittels FFF jeweils in neun verschiedenen Layerhöhen (50,0 µm, 80,9 µm, 100,0 µm, 150,0 µm, 160,8 µm, 200 µm, 250 µm, 300 µm, 332,6 µm) aus jeweils zwei verschiedenen Materialien (das Polylactid PLA NX2 und das Lignin basierte Biopolymer Green-TEC PRO) gedruckt. Als Kontrollgruppe fungierten 2 DLP hergestellte Oberkieferteilmodelle mit einer Layerhöhe von 20 µm. All diese physischen Modelle wurden nachfolgend digital gescannt und mit 3D-Überlagerungsmesstechnik via GOM Inspect Software bezüglich ihrer Oberflächengüte verglichen. Um den Einfluss der Layerhöhe FFF gedruckter Modelle auf die Kraftübertragung von Alignern zu untersuchen, wurden im zweiten Part an einem randomisiert gewählten, digitalen Oberkiefermodell mittels OnyxCeph3™Lab Software folgende Bewegungen für den zweiten linken Oberkieferprämolaren virtuell in je 0,1mm-Setup-Schritten geplant: Bukkaltorque (0,1 mm - 0,5 mm), Distalisation (0,1 mm - 0,4 mm), Extrusion (0,1 mm - 0,4 mm) und Rotation (0,1 mm - 0,5 mm). Nachfolgend wurden mittels FFF 91 physische Oberkiefermodelle gedruckt, eines für jeden Setup-Schritt und jede Layerhöhe (100 µm; 150 µm, 200 µm, 250 µm, 300 µm). Mithilfe der 3D-gedruckten Modelle wurden im Tiefziehverfahren 192 Aligner gefertigt in jeweils zwei Schichtstärken (0,5 mm, 0,75mm). Die Messapparatur bestand im Wesentlichen aus einem Oberkieferacrylmodell mit separiertem zweiten Oberkieferprämolaren, welcher mit einem Sensor verbunden war. Dieser erfasste die vom aktiven Aligner übertragenen initialen Kräfte und Drehmomente. Zur Evaluation des Einflusses der untersuchten Parameter Layerhöhe, Alignerdicke, Setupschritt und Bewegungsart, diente ein verallgemeinertes lineares Modell mit der Gammafunktion.
Das im ersten Teil angewandte Messverfahren, die 3D-Überlagerungtechnik, wies laut Intraklassen -Korrelationskoeffizient (ICC) und Methodenfehler nach Dahlberg eine exzellente Reliabilität auf. Bei Lignin basierten FFF Modellen nahm die Oberflächengüte im Bereich von 100 µm bis 332,6 µm mit steigender Layerhöhe ab. Unterhalb von 100 µm zeigte sich jedoch kein weiterer Gewinn an Genauigkeit, sondern im Gegenteil, ein weiterer Verlust an Modellgenauigkeit mit abnehmender Layerhöhe. Im Vergleich wiesen die Lignin-basierten Biopolymer-Modelle durchweg eine bessere Oberflächengüte als die Polylactid basierten Modelle auf, und konnten bei einer optimalen Layerhöhe von 100 µm sogar mit der Präzision der DLP geduckten Modelle der Kontrollgruppe mithalten. In dem zweiten Teil der Untersuchung zeigten die Parameter Alignerdicke und Setupschritt einen signifikanten Einfluss auf durch Aligner abgegebene initiale Kräfte bzw. Drehmomente. Die Parameter Zahnbewegungsart und Layerhöhe waren nicht signifikant. Allerdings wurde bei einer Layerhöhe von 150 µm ein optimales Kraftniveau mit geringster Streuung bei den untersuchten Alignern verzeichnet.
Die Ergebnisse des ersten Teils der Untersuchung zeigten, dass die Oberflächengüte im FFF-Druck regulierbar durch die Layerhöhe ist, darüber hinaus jedoch weitere Faktoren, wie beispielweise das Druckmaterial, eine Rolle spielen. Weiterhin stellt die FFF-Technologie eine genaue, kosteneffektive und nachhaltige Technologie zur Herstellung von Aligner-Modellen in der kieferorthopädischen Praxis dar. Beide Untersuchungen zeigen, dass es keinen erkennbaren Vorteil gibt, FFF-Dentalmodelle in besonders niedrigen Layerhöhen zu drucken, da daraus exponentiell höhere Druckzeiten entstehen, ohne vergleichbar bessere Kraftabgabe der kieferorthopädischen Aligner. Bei zu hohen Layerhöhen, jedoch, wird die Streuung der von den Alignern emittierten Kräften größer, demzufolge die Kraftabgabe unvorhersehbarer. Ein Kompromiss, welcher klinische Vorteile der adäquaten Kraftübertragung durch Aligner und wirtschaftliche Vorteile durch effiziente Druckzeiten vereint, stellt eine Optimum-Layerhöhe von 150 µm für FFF-Modelle für die In-Office-Alignerherstellung dar.
Obwohl das Erkrankungsbild Sepsis auch in Deutschland immer stärker in den Fokus der Forschung gerückt ist, liegt die Mortalität noch immer zwischen 30-50%. Besonders die abdominelle Sepsis ist mit einer höheren Letalität assoziiert, da dieses Krankheitsbild häufiger mit schweren Verläufen und Organversagen einhergeht. Unter einer abdominellen Sepsis versteht man eine intraabdominelle Infektion mit einer extraperitonealen Begleitreaktion. Eine Peritonitis ist das Resultat einer intraabdominellen Infektion. Grampositive Enterococcus species sind mit 32-36% nach den gramnegativen Enterobacteriaceae die zweithäufigsten Erreger, die bei sekundären Peritonitiden isoliert werden können. Zu den am häufigsten isolierten Enterococcus species zählen Enterococcus faecalis und faecium. Ihre Rolle und Therapiebedürftigkeit bei einer abdominellen Sepsis ohne einen Nachweis in der Blutkultur ist nach wie vor sehr umstritten. Es existieren bereits zahlreiche Studien, die sich mit der Therapiebedürftigkeit von Enterokokken bei abdomineller Sepsis beschäftigen. Die Ergebnisse und die daraus resultierenden Empfehlungen bleiben aber kontrovers diskutiert. In dieser Arbeit soll die Auswirkung eines ab- dominellen bzw. intraabdominellen positiven Enterokokkenabstrichs auf die Behandlung von Patienten mit abdomineller schwerer Sepsis und septischem Schock untersucht werden. Weiterhin soll der Effekt einer Behandlung mit enterokokken- spezifischen Antibiotika auf die 28- und 90-Tage Letalität geprüft werden. Dafür wurden in einem Zeitraum von drei Jahren 179 Patienten mit abdominellem Sepsisfokus einbezogen, bei denen ein abdomineller/intraabdomineller Abstrich oder ein Abstrich aus einer abdominellen/intraabdominellen Drainage durchgeführt worden ist. Patienten mit einem Nachweis von Enterokokken in einer Blutkultur wurden ausgeschlossen. Durch die Berechnung von logistischen Regressionen zeigte sich, dass ein positiver abdomineller bzw. intraabdomineller Enterokokkenabstrich signifikant mit der Verabreichung von enterokokkenspezifischen Antibiotika assoziiert war. Die Chance, ein enterokokkenspezifisches Antibiotikum zu erhalten, stieg dabei um das Sechsfache. Beim Vergleich zweier Gruppen war die Behandlung mit
enterokokkenspezifischen Antibiotika mit einer statistisch signifikanten Reduktion der 28-Tage Letalität assoziiert. Dabei hatte die eine Gruppe enterokokkenspezifische Antibiotika erhalten, die andere nicht. Dieses Ergebnis konnte mittels logistischer Regressionen für die 28-Tage Letalität bestätigt werden. Es zeigte sich aber kein statistisch signifikanter Effekt auf die 90-Tage Letalität. Es sollten deswegen weitere, größer angelegte Studien angeregt werden, die den Effekt eines positiven Enterokokkenabstrichs und den Effekt einer Gabe von enterokokkenspezifischen Antibiotika auf die Letalität bei Patienten mit abdominellem Sepsisfokus untersuchen.
Boronate esters, formed by the reaction of an oligonucleotide bearing a 5′-boronic acid moiety with the 3′-terminal cis-diol of another oligonucleotide, support the assembly of functional nucleic acid architectures. Reversible formation of boronate esters occurs in templated fashion and has been shown to restore the activity of split DNA and RNA enzymes as well as a split fluorescent light-up aptamer. Apart from their suitability for the design and application of split nucleic acid enzymes and aptamers in the field of biosensing, boronate esters may have played an important role in early life as surrogates of the natural phosphodiester bond. Their formation is reversible and thus fulfills an important requirement for biological self-assembly. Here we discuss the general concept of stimuli-dependent boronate formation and its application in biomolecules with implications for future research.
Bei der familiären Form zerebraler kavernöser Malformationen (CCMs) handelt es sich um eine autosomal-dominant erbliche Erkrankung, die durch pathogene Keimbahnvarianten in den Genen CCM1/KRIT1, CCM2 bzw. CCM3/PDCD10 ausgelöst wird und mit einer Prävalenz von 1:3300 bis 1:3800 in der Allgemeinbevölkerung vorkommt. Neben Punktmutationen und kleinen Indels werden auch Strukturvarianten (SVs) in den Krankheitsgenen beobachtet. Diese sind jedoch mithilfe von Short-Read-basierten Genpanel- oder Exomsequenzierungen in der Routinediagnostik zum Teil nur schwer zu identifizieren.
Im Rahmen der vorliegenden Arbeit wurde daher eine neue CRISPR/Cas9-basierte Nanopore-Sequenzierung der Gene CCM1, CCM2 und CCM3 sowie der flankierenden genomischen Regionen etabliert. Die Abdeckung der Zielregionen mit Reads länger als 20 kb erlaubte die zuverlässige Detektion von Kopienzahlveränderungen. Darüber hinaus konnte anhand einer interchromosomalen Insertion und einer 24 kb großen Inversion gezeigt werden, dass die Methode auch komplexere SVs nachweisen kann. Das Protokoll einer Cas9-basierten Sequenzierung ausgewählter genomischer Zielregionen auf der MinION-Plattform kann gut auf andere Fragestellungen übertragen werden und lässt sich mit vergleichsweise geringem Aufwand in vielen Laboren etablieren.
Ein zweiter Schwerpunkt der Promotionsarbeit lag auf der Entwicklung neuer, vielseitig einsetzbarer Zellkulturmodelle der CCM-Erkrankung. Mithilfe CRISPR/Cas9-vermittelter Genomeditierungen in fluoreszenzmarkierten humanen induzierten pluripotenten Stammzellen (hiPSCs) konnten komplexe Kokulturmodelle sowie dreidimensionale vaskuläre Organoidkulturen etabliert werden. Anhand von aus CCM3-defizienten hiPSCs differenzierten humanen Endothelzellen (ECs) und vaskulären Mosaikorganoiden ließ sich ein klonaler Überlebensvorteil von CCM3-/--Zellen in Kokultur mit Wildtyp-Zellen nachweisen, welcher kürzlich auch in CCM-Mausmodellen als Schlüsselelement der CCM-Pathogenese beschrieben wurde. In den neuen Zellkulturmodellen ließ sich auch der positive Effekt des in der Greifswalder Arbeitsgruppe identifizierten Kandidatenwirkstoffs NSC59984 auf die tumorähnliche Proliferation von CCM3-/--Zellen in Kokultur validieren. Durch systematische RNA-Sequenzierungen konnte zudem nachgewiesen werden, dass CCM1 für die Differenzierung von ECs aus hiPSCs nicht erforderlich ist, jedoch die physiologische Funktion von ECs aufrechterhält.
Die Ergebnisse der vorliegenden Arbeiten belegen nicht nur die Praktikabilität und Sicherheit der Identifizierung komplexer genomischer Strukturvarianten mittels CRISPR/Cas9-vermittelter Nanopore-Sequenzierungen, sondern sie ebnen zudem den Weg zur effektiven Testung neuer CCM-Therapieansätze im Hochdurchsatzverfahren und erlauben einen neuen Einblick in die noch immer unvollständig verstandene CCM-Pathogenese.
The multi-cell Penning–Malmberg trap concept has been proposed as a way to accumulate and confine unprecedented numbers of antiparticles, an attractive but challenging goal. We report on the commissioning and first results (using electron plasmas) of the World's second prototype of such a trap, which builds and improves on the findings of its predecessor. Reliable alignment of both ‘master’ and ‘storage’ cells with the axial magnetic field has enabled confinement of plasmas, without use of the ‘rotating wall’ (RW) compression technique, for over an hour in the master cell and tens of seconds in the storage cells. In the master cell, attachment to background neutrals is found to be the main source of charge loss, with an overall charge-confinement time of 8.6 h. Transfer to on-axis and off-axis storage cells has been demonstrated, with an off-axis transfer rate of 50% of the initial particles, and confinement times in the storage cells in the tens of seconds (again, without RW compression). This, in turn, has enabled the first simultaneous plasma confinement in two off-axis cells, a milestone for the multi-cell trap concept.
The combination of a linear quadrupole ion-filter and linear Paul trap operated with a rectangular guiding field for the filtering and accumulation of ions within the Mass Spectrometry for Single Particle Imaging of Dipole Oriented protein Complexes (MS SPIDOC) prototype [T. Kierspel et al., Anal. Bioanal. Chem., published online] is characterized. Using cationic caesium-iodide clusters, the ion-separation performance, ion accumulation, cooling, and ejection via in-trap pin electrodes is evaluated. Furthermore, proof-of-principle measurements are performed with 64 kDa multiply-charged non-covalent protein complexes of human hemoglobin and 804 kDa non-covalent complex of GroEL, to demonstrate that the module meets the criteria to handle high-mass ions which are the main objective of the MS SPIDOC project. The setup's performance is found to be in line with previous results from ion-trajectory simulations [F. Simke et al., Int. J. Mass Spectrom.473 (2022) 116779].
Hintergrund
Die chronische Nierenkrankheit (CKD) ist eine häufige Erkrankung, insbesondere im höheren Alter. Um der Progression der Erkrankung und deren Komplikationen vorzubeugen, ist eine leitliniengerechte ambulante Versorgung von Patient:innen mit CKD anzustreben. Zur Messung und Bewertung der Versorgungsqualität können Qualitätsindikatoren (QI) genutzt werden. In Deutschland existieren bisher keine QI für CKD. Ziel der Arbeit war die Entwicklung von QI für die Qualitätsüberprüfung der ambulanten Versorgung von Patient:innen über 70 Jahren mit nichtdialysepflichtiger CKD.
Material und Methoden
Auf Grundlage der nationalen S3-Leitlinie CKD und eines Reviews internationaler QI wurde eine Liste von QI erstellt. Die ausgewählten QI wurden in 2 Sets eingeteilt: basierend auf Routinedaten (z. B. Abrechnungsdaten der Krankenkassen) und auf Datenerhebung in der Praxis (Chart-Review). Expert:innen verschiedener Fachrichtungen sowie ein Patient:innenvertreter bewerteten diese in einem Delphi-Verfahren mit 2‑stufiger Onlinebefragung im Oktober 2021 und Januar 2022 und abschließender Konsensuskonferenz im März 2022. Zusätzlich wurden Ranglisten der wichtigsten QI von jedem Set erstellt.
Ergebnisse
Ein Inzidenz- und ein Prävalenzindikator wurden a priori festgelegt und standen nicht zur Abstimmung. Weitere 21 QI standen zur Abstimmung durch die Expert:innen. Für jedes QI-Set wurden die 7 wichtigsten Indikatoren ausgewählt. Nur 1 QI wurde von dem Expert:innenpanel für den zusätzlichen Einsatz bei Erwachsenen unter 70 Jahren als nicht geeignet eingestuft.
Diskussion
Die QI sollen es ermöglichen, die Qualität der ambulanten Versorgung von Patient:innen mit CKD zu untersuchen, mit dem Ziel, die leitlinienkonforme ambulante Versorgung zu optimieren.
Immunotherapies against high-risk neuroblastoma (NB), using the anti-GD2 antibody (Ab) dinutuximab beta (DB), significantly improved patient survival. Ab-dependent cellular cytotoxicity (ADCC) is one of the main mechanisms of action and it is primarily mediated by NK cells. To further improve antitumor efficacy, we investigated here a combinatorial immunotherapy with DB and the double immune checkpoint blockade of T-cell immunoreceptor with immunoglobulin and ITIM domain (TIGIT) and programmed cell death ligand-1 (PD-L1). The effects of ADCC, mediated by DB against NB cells on NK-cell activity, and the expression of TIGIT and CD226 and their ligands CD112 and CD155, as well as of PD-1 and PD-L1 on NB and effector cells, were investigated using flow cytometry. ADCC was assessed with a calcein-AM-based cytotoxicity assay. The efficacy of a combinatorial immunotherapy with DB, given as a long-term treatment, and the double immune checkpoint blockade of TIGIT and PD-L1 was shown using a resistant murine model of NB, followed by an analysis of the tumor tissue. We detected both TIGIT ligands, CD112 and CD155, on all NB cell lines analyzed. Although ADCC by DB resulted in a strong activation of NK cells leading to an effective tumor cell lysis, a remarkable induction of PD-L1 expression on NB cells, and of TIGIT and PD-1 on effector cells, especially on NK cells, was observed. Additional anti-TIGIT or anti-PD-L1 treatments effectively inhibited tumor growth and improved survival of the mice treated with DB. The superior antitumor effects were observed in the “DB + double immune checkpoint blockade” group, showing an almost complete eradication of the tumors and the highest OS, even under resistant conditions. An analysis of tumor tissue revealed both TIGIT and TIGIT ligand expression on myeloid-derived suppressor cells (MDSCs), suggesting additional mechanisms of protumoral effects in NB. Our data show that the targeting of TIGIT and PD-L1 significantly improves the antitumor efficacy of anti-GD2 immunotherapy, with DB presenting a new effective combinatorial treatment strategy against high-risk tumors.
Guidelines and Standard Frameworks for AI in Medicine: Protocol for a Systematic Literature Review

Background: Applications of artificial intelligence (AI) are pervasive in modern biomedical science. In fact, research results suggesting algorithms and AI models for different target diseases and conditions are continuously increasing. While this situation undoubtedly improves the outcome of AI models, health care providers are increasingly unsure which AI model to use due to multiple alternatives for a specific target and the “black box” nature of AI. Moreover, the fact that studies rarely use guidelines in developing and reporting AI models poses additional challenges in trusting and adapting models for practical implementation.
Objective: This review protocol describes the planned steps and methods for a review of the synthesized evidence regarding the quality of available guidelines and frameworks to facilitate AI applications in medicine.
Methods: We will commence a systematic literature search using medical subject headings terms for medicine, guidelines, and machine learning (ML). All available guidelines, standard frameworks, best practices, checklists, and recommendations will be included, irrespective of the study design. The search will be conducted on web-based repositories such as PubMed, Web of Science, and the EQUATOR (Enhancing the Quality and Transparency of Health Research) network. After removing duplicate results, a preliminary scan for titles will be done by 2 reviewers. After the first scan, the reviewers will rescan the selected literature for abstract review, and any incongruities about whether to include the article for full-text review or not will be resolved by the third and fourth reviewer based on the predefined criteria. A Google Scholar (Google LLC) search will also be performed to identify gray literature. The quality of identified guidelines will be evaluated using the Appraisal of Guidelines, Research, and Evaluation (AGREE II) tool. A descriptive summary and narrative synthesis will be carried out, and the details of critical appraisal and subgroup synthesis findings will be presented.
Results: The results will be reported using the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses) reporting guidelines. Data analysis is currently underway, and we anticipate finalizing the review by November 2023.
Conclusions: Guidelines and recommended frameworks for developing, reporting, and implementing AI studies have been developed by different experts to facilitate the reliable assessment of validity and consistent interpretation of ML models for medical applications. We postulate that a guideline supports the assessment of an ML model only if the quality and reliability of the guideline are high. Assessing the quality and aspects of available guidelines, recommendations, checklists, and frameworks—as will be done in the proposed review—will provide comprehensive insights into current gaps and help to formulate future research directions.

Synthesis of Quercetin-Loaded Silver Nanoparticles and Assessing Their Anti-Bacterial Potential

The study delves into the multifaceted potential of quercetin (Qu), a phytoconstituent found in various fruits, vegetables, and medicinal plants, in combination with silver nanoparticles (AgNPs). The research explores the synthesis and characterization of AgNPs loaded with Qu and investigates their pharmaceutical applications, particularly focusing on antibacterial properties. The study meticulously evaluates Qu’s identity, and physicochemical properties, reaffirming its suitability for pharmaceutical use. The development of Qu-loaded AgNPs demonstrates their high drug entrapment efficiency, ideal particle characteristics, and controlled drug release kinetics, suggesting enhanced therapeutic efficacy and reduced side effects. Furthermore, the research examines the antibacterial activity of Qu in different solvents, revealing distinct outcomes. Qu, both in methanol and water formulations, exhibits antibacterial activity against Escherichia coli, with the methanol formulation displaying a slightly stronger efficacy. In conclusion, this study successfully synthesizes AgNPs loaded with Qu and highlights their potential as a potent antibacterial formulation. The findings underscore the influence of solvent choice on Qu’s antibacterial properties and pave the way for further research and development in drug delivery systems and antimicrobial agents. This innovative approach holds promise for addressing microbial resistance and advancing pharmaceutical formulations for improved therapeutic outcomes.
The microbiome of the colon is characterized by its great diversity. This varies not only intra- but also interindividually and is influenced by endogenous and exogenous factors, such as dietary and lifestyle factors. The aim of this work was to investigate the extent to which the degradation of the drug sulfasalazine is influenced by different microbiota. Therefore, the in vitro model MimiCol3 was used, which represents the physiological conditions of the ascending colon. In addition to a representative physiological volume, the pH value, redox potential and an anaerobic atmosphere are important to provide the bacteria with the best possible growth conditions. Stool samples were taken from three healthy subjects, comparing omnivorous, vegetarian and meat-rich diets, and cultured for 24 h. However, the nutrient medium used for cultivation led to the alignment of the bacterial composition of the microbiota. The previously observed differences between the diets could not be maintained. Nevertheless, the similar degradation of sulfasalazine was observed in all microbiota studied in MimiCol3. This makes MimiCol3 a suitable in vitro model for metabolism studies in the gut microbiome.
Background
The ‘wallflower’ hypothesis proposes females mate indiscriminately to avoid reproductive delays. Post-copulatory mechanisms may then allow ‘trading up’, favouring paternity of future mates. We tested links between pre- and post-copulatory choice in Latrodectus geometricus female spiders paired sequentially with two males. These females copulate as adults or as subadults and store sperm in paired spermathecae. Choosy adults have a higher risk of delays to reproduction than subadults.
Results
We predicted low pre-copulatory, but high post-copulatory choice at first matings for adults and the opposite for subadults. At second matings, we expected all females would prefer males superior to their first. We found all females mated indiscriminately at their first pairing, but in contrast to subadults, adults usually allowed only a single insertion (leaving one of their paired spermatheca empty); a mechanism of post-copulatory choosiness. Adult-mated females were more likely to remate than subadult-mated females when they became adults, showing a preference for larger males, while subadult-mated females tended to prefer males of greater size-corrected mass.
Conclusions
Our results show that the ‘wallflower’ effect and ‘trading up’ tactics can be utilized at different life stages, allowing females to employ choice even if rejecting males is costly.
Ausgehend von den Erfahrungswerten sechs Klinischer Ethiker*innen an Universitätsklinika in Deutschland wurde eine strukturierte Auseinandersetzung mit dem Arbeitsfeld der Fortbildungen im Rahmen der Klinischen Ethikarbeit vorgenommen. Die Zusammenarbeit wurde von der Fragestellung geleitet, was bei der Konzeption einer Ethik-Fortbildung innerhalb einer Einrichtung zu berücksichtigen ist. Der methodische Zugang zur Beantwortung der explorativen Fragestellung bestand in einem mehrstufigen, deskriptiven Verfahren, das alternierend in gemeinsamen Arbeitssitzungen und individueller Arbeit zwischen den Sitzungen umgesetzt wurde.
Als Resultat zeigt der Aufsatz eine Übersicht über die Bandbreite möglicher Fortbildungsformate auf, die denjenigen Hilfestellung bieten kann, die in ihrer Einrichtung Fortbildungen zu ethischen Themen konzipieren und durchführen wollen. Darüber hinaus bietet er einen Überblick über die bei der Umsetzung jeweils zu berücksichtigenden direkt strukturierenden sowie indirekt strukturierenden Merkmale. Es wird an Beispielen aufgezeigt, welche Formatvarianten sich in Kombination mit welchen direkt strukturierenden Merkmalen in den ausgewählten Universitätsklinika bewährt haben.
Der Beitrag liefert eine Diskussionsgrundlage, um die strukturierte Auseinandersetzung mit konzeptionellen Fragen ethischer Fortbildung in der Klinik zu befördern.
Background
Both vulnerability and integrity represent action-guiding concepts in nursing practice. However, they are primarily discussed regarding patients—not nurses—and considered independently from rather than in relation to each other.
Aim
The aim of this paper is to characterize the moral dimension of nurses' vulnerability and integrity, specify the concepts' relationship in nurses' clinical practice and, ultimately, allow a more fine-grained understanding.
Design
This discursive paper demonstrates how vulnerability and integrity relate to each other in nursing practice and carves out which types of vulnerability pose a threat to nurses' moral integrity. The concept of vulnerability developed by Mackenzie et al. (2014) is applied to the situation of nurses and expanded to include the concept of moral integrity according to Hardingham (2004). Four scenarios are used to demonstrate where and how nurses' vulnerabilities become particularly apparent in clinical practice. This leads to a cross-case discussion, in which the vulnerabilities identified are examined against the background of moral integrity and the relationship between the two concepts is determined in more detail.
Results and Conclusion
Vulnerability and integrity do not only form a conceptual pair but also represent complementary moral concepts. Their joint consideration has both a theoretical and practical added value. It is shown that only specific forms of vulnerability pose a threat to moral integrity and the vulnerability–integrity relationship is mediated via moral distress.
Implications for the Profession and/or Patient Care
The manuscript provides guidance on how the concrete threat(s) to integrity can be buffered and moral resilience can be promoted. Different types of threats also weigh differently and require specific approaches to assess and handle them at the micro-, meso- and macro-level of the healthcare system.
Decreased inflammatory profile in oral leukoplakia tissue exposed to cold physical plasma ex vivo

Background
Oral leukoplakia (OL) is an unfavorable oral disease often resistant to therapy. To this end, cold physical plasma technology was explored as a novel therapeutic agent in an experimental setup.
Methods
Biopsies with a diameter of 3 mm were obtained from non-diseased and OL tissues. Subsequently, cold atmospheric pressure plasma (CAP) exposure was performed ex vivo in the laboratory. After 20 h of incubation, biopsies were cryo-conserved, and tissue sections were quantified for lymphocyte infiltrates, discriminating between naïve and memory cytotoxic and T-helper cells. In addition, the secretion pattern related to inflammation was investigated in the tissue culture supernatants by quantifying 10 chemokines and cytokines.
Results
In CAP-treated OL tissue, significantly decreased overall lymphocyte numbers were observed. In addition, reduced levels were observed when discriminating for the T-cell subpopulations but did not reach statistical significance. Moreover, CAP treatment significantly reduced levels of C-X-C motif chemokine 10 (CXCL10) and granulocyte-macrophage colony-stimulating factor in the OL biopsies' supernatants. In idiopathically inflamed tissues, ex vivo CAP exposure reduced T-cells and CXCL10 as well but also led to markedly increased interleukin-1β secretion.
Conclusion
Our findings suggest CAP to have immuno-modulatory properties, which could be of therapeutic significance in the therapy of OL. Future studies should investigate the efficacy of CAP therapy in vivo in a larger cohort.
An Serin139 phosphoryliertes gamma Histon H2A.X (γH2A.X) hat sich über die Jahrzehnte als sensitiver Surrogat-Endpunkt Strahlen-induzierter DNA-Schäden, insbesondere von DNA-Doppelstrangbrüchen (DSBs) etabliert. Daher wurde γH2A.X in seiner biomedizinischen Anwendung als geeigneter Marker und genereller Indikator direkter DNA-Schäden anderer Agenzien, beispielsweise niedrig konzentrierter primär exogener reaktiver Sauerstoffspezies (ROS), deren intrazelluläre Effekte weniger direkt auf eine DNA-Oxidation oder das Einfügen von Strangbrüchen abzielen, angesehen. Nicht-thermisches physikalisches Plasma (NTPP), ein teilweise ionisiertes Gas, dessen biochemische Wirkung in vitro hauptsächlich über Freisetzung reaktiver Sauerstoff- und Stickstoffspezies (RONS) in der flüssigen Phase der Zellumgebung vermittelt wird, setzt lebende Systeme dosisabhängig oxidativem Stress aus und wird therapeutisch in der Wundbehandlung sowie Krebsbehandlung angewendet. Die γH2A.X Quantifizierung wurde häufig zur Beurteilung der Genotoxizität in Plasma-behandelten Zellen verwendet, wobei die NTPP-Exposition in vitro bereits häufig einen γH2A.X Anstieg zeigte, was zu der Hypothese führte, dass ein (oxidativer) DNA-Schaden die direkte Konsequenz Plasma-generierter ROS sei. Darüber hinaus tritt γH2A.X auch während der Apoptose auf, die wiederum selbst mit der NTPP- und ROS-Exposition assoziiert ist. Die vorliegende Arbeit untersuchte die γH2A.X-Induktion in einer Lymphoblasten-Zelllinie bei ROS-Exposition (NTPP, H2O2 oder HOCl) bzw. Behandlung mit UVB-Licht. Durch Verwendung von Antioxidantien wurden sowohl die Zytotoxizität als auch die γH2A.X-Induktion bei allen Arten der ROS-Quellen außer bei UVB-Strahlung aufgehoben. Inhibition der Signalwege für oxidativen Stress (p38-MAPK) und Apoptose (Pan-Caspase) hemmten signifikant die γH2A.X-Induktion bei ROS, jedoch nicht bei UVB-Licht. Letztlich und trotz H2A.X Phosphorylierung führte die UVB-Licht- und eben nicht die NTPP-Exposition zu einer signifikanten Bildung von Mikronuklei (MN), die einen funktionellen Endpunkt genotoxischer DSBs darstellen, sodass trotz der γH2A.X Präsenz kein nachhaltiges mutagenes Potential der medizinischen Plasmabehandlung identifiziert werden konnte. Im Gegensatz zu Strahlen- oder Chemotherapeutika- induziertem und somit DNA-Schaden-assoziiertem γH2A.X zeigen die Daten dieser Arbeit die H2A.X Phosphorylierung weniger als Folge eines ROS-vermittelten primären DNA-Schadens, sondern vielmehr im Licht redox-sensitiver Signalwege der Apoptose, sodass künftige γH2A.X Messungen im medizinischen Bereich, insbesondere in der Plasmamedizin und Redoxbiologie einer sorgfältigen Interpretation bedürfen.
Ocean literacies: the promise of regional approaches integrating ocean histories and psychologies

The current concept of ocean literacy reflects a prerequisite for achieving ocean sustainability. Existing ocean literacy reflects a fundamentally western view of oceans that works in tension with ocean literacy goals. Although ocean literacy practitioners and researchers are, laudably, starting to incorporate Indigenous knowledges and perspectives from BIPOC communities, attention to historical change continues to be left out of ocean literacy, to the detriment of ocean literacy goals. This article points out that, given the reality that human-ocean relationships have changed over time, and differed among cultural groups in the past as well as in the present, ocean literacy needs to incorporate ocean history at a foundational level. Because there are historical differences in human relationships with oceans, it stands to reason that regional ocean literacies must be more effective than a universal and timeless ocean literacy framework. Following the logical efficacy of a regional approach to ocean literacy, this article further argues that regional ocean literacies should involve the systematic inclusion of emotional elements. Regional ocean literacies should be constructed through knowledge co-production, involving diverse types of expertise, knowledge and actors to produce context-specific knowledge and pathways towards a sustainable future. To fully exploit the potential of ocean literacy, there is a need for the UN Ocean Decade to work towards regional and place-based approaches that incorporate history as well as culture in an iterative and collaborative process involving diverse types of expertise, knowledge and actors.

Sichtbar geworden ist dies zuletzt in Auseinandersetzungen mit Sozialen Medien, bei denen in Experimenten mit digitalen Tools wie im Rahmen konventionellerer Formen von Literatur und korrespondierender Theoriediskurse literarische und zeitdiagnostische Schreibweisen aneinanderrücken und auf je verschiedene Weise das hervorbringen, was als »Gegenwartsvergegenwärtigung« bezeichnet werden kann.
Seas and oceans are essential for the global ecosystem. Entire societies, economies and countless livelihoods rely on their good environmental status. Yet, pressures on marine environments are increasing. An extensive assessment and monitoring of marine habitats is a vital precondition for understanding these systems and their sustainable conservation. Remote sensing methods can temporally accelerate the mapping, improve the spatial resolution and support the interpretation of large areas. Hydroacoustic becomes the method of choice for areas deeper than the coastal zone as optical signals are limited by strong attenuation in the water column. Apart from depth measurements for the creation of bathymetric charts, the recording of backscatter strength is useful for the characterization of the seafloor surface. The direct influence of the inhabiting benthic community on the backscattered signal is rarely considered, although it can be utilized for the detection of benthic life. Information about habitat-specific backscatter responses or a hydroacoustic remote sensing catalog for benthic habitats is missing so far.
The multibeam echosounder (MBES) has the advantage of recording both, bathymetry and backscatter strength simultaneously with related incidence angle. Further, recent technological developments allow to change between frequencies. Angular range curves supported the quantification of backscatter strength of different frequencies. Acoustic data sets were complemented by ground truthing in form of sedimentological and biological samples as well as video profiles. Study areas were located offshore the island of Sylt in the North Sea as well as in vicinity to Oder Bank and close to the coast offshore Hohe Düne/Rostock, both in the Baltic Sea. Investigated habitats included sand areas inhabited by tubeworms, loose mussel clusters on top of sand areas, seagrass meadows, coarse sand and gravel areas, and a reef covered by mussels.
Multifrequency backscatter maps, combining frequencies between 200 kHz and 700 kHz, illustrate small-scale features at the seafloor not visible in monofrequent maps. Key habitats showed a specific backscatter response, which can partly be related to macrobenthic flora and fauna. Data sets recorded with a (partly calibrated) MBES in three different month (May, August, October) revealed that backscatter strength can further detect spatial as well as temporal habitat dynamics. Alterations in the sediment composition at the seafloor surface of the ecologically valuable coarse sand and gravel areas were caused by seasonal changes in local hydrodynamics.
A newly developed 3D seismic lander has the ability to support hydroacoustic remote sensing as an additional, non-destructive ground truthing method utilizing a high frequency of 130 kHz to image the shallow subsurface. Buried objects, e.g., stones, shells, fruit gummy worms, as well as sediment disturbances could be detected and visualized in a laboratory experiment. The 3D seismic lander is likely to improve the investigation of volume scatter contribution to backscatter strength and is potentially applicable for the imaging of bioturbation.
Bone generally displays a high intrinsic capacity to regenerate. Nonetheless, large osseous defects sometimes fail to heal. The treatment of such large segmental defects still represents a considerable clinical challenge. The regeneration of large bone defects often proves difficult, since it relies on the formation of large amounts of bone within an environment impedimental to osteogenesis, characterized by soft tissue damage and hampered vascularization. Consequently, research efforts have concentrated on tissue engineering and regenerative medical strategies to resolve this multifaceted challenge. In this review, we summarize, critically evaluate, and discuss present approaches in light of their clinical relevance; we also present future advanced techniques for bone tissue engineering, outlining the steps to realize for their translation from bench to bedside. The discussion includes the physiology of bone healing, requirements and properties of natural and synthetic biomaterials for bone reconstruction, their use in conjunction with cellular components and suitable growth factors, and strategies to improve vascularization and the translation of these regenerative concepts to in vivo applications. We conclude that the ideal all-purpose material for scaffold-guided bone regeneration is currently not available. It seems that a variety of different solutions will be employed, according to the clinical treatment necessary.
Für die kieferorthopädische Therapie ist die Kenntnis über die Veränderungen der Gebissmorphologie, welche sich natürlicherweise ereignen, von grundlegender Bedeutung. Bislang sind die Veränderungen der ersten beiden Jahrzehnte des Lebens vom Milchgebiss bis hin zum jugendlichen permanenten Gebiss gut dokumentiert worden, es sind jedoch nur wenige Studien über die physiologischen Veränderungen des Gebisses im Erwachsenenalter vorhanden. Gegenstand der vorliegenden longitudinalen Studie waren Veränderungen der Zahnbogendimensionen hinsichtlich Breite, Länge, Platzverhältnisse im Frontzahnbereich, Overbite und Overjet bei Erwachsenen.
Die dreidimensionalen Datensätze von Modellen von 103 Probanden wurden untersucht, von welchen im Rahmen der Study of Health in Pomerania (SHIP) Abformungen des Ober- und des Unterkiefers in einem Abstand von 16 ± 1 Jahren gemacht worden sind. Die Probanden waren zum Zeitpunkt T0 zwischen 20 und 55 Jahren alt, und zum Zeitpunkt T1 zwischen 37 und 71 Jahren. Mit Hilfe der Software Landmark und Meshmixer wurden Messpunkte platziert, aus welchen Strecken berechnet wurden. Zur Überprüfung der Messgenauigkeit wurden an 12 Modellpaaren die Messpunkte für den Overbite und den Overjet und an 20 Modellpaaren alle weiteren Messpunkte ein zweites Mal platziert. Die Ergebnisse sind in Form von Histogrammen, Box-Whisker-Plots und Mittelwerten dargestellt. Sie wurden mit Hilfe des t-Tests für unverbundene Stichproben auf Unterschiede in Abhängigkeit vom Ge-schlecht sowie mit Hilfe des t-Tests für verbundene Stichproben auf Unterschiede mit dem Alter untersucht.
Für die meisten der untersuchten Variablen zeigte sich ein Geschlechtsdimorphismus, welcher sich vor allem durch größere Messwerte bei den männlichen im Vergleich zu den weiblichen Probanden auszeichnete.
Die intercanine Distanz verringerte sich im Unterkiefer in der Gesamtgruppe und bei den weiblichen Probanden signifikant und blieb im Oberkiefer mit einer Tendenz zur Ver-kleinerung stabil. Die anteriore Breite verringerte sich in beiden Kiefern signifikant in der Gesamtgruppe und bei den weiblichen Probanden. Die posteriore Breite vergrößerte sich in beiden Kiefern, war jedoch nur im Unterkiefer der weiblichen Probanden signifikant unterschiedlich. Insgesamt zeigten die Veränderungen der Zahnbogenbreite einen mesiodistalen Gradienten, mit nach anterior hin schmaler werdenden Zahnbögen.
Die Zahnbogenlänge verringerte sich in beiden Kiefern beiderlei Geschlechts, war jedoch nur im Oberkiefer der Gesamtgruppe signifikant unterschiedlich.
Der Engstand im Frontzahnbereich des Unterkiefers verstärkte sich im Untersuchungszeitraum bei den männlichen und weiblichen Probanden signifikant. Im Oberkiefer wurde eine Tendenz zur Vergrößerung des Platzangebotes im Frontzahnbereich festgestellt, die Veränderung war allerdings nicht signifikant unterschiedlich.
Der Overbite reduzierte sich in der Gesamtgruppe signifikant. Der Overjet blieb im Untersuchungszeitraum nahezu unverändert.
Die vorliegenden Ergebnisse deuten darauf hin, dass altersassoziierte Veränderungen im kraniofazialen Bereich auch beim Erwachsenen zu beobachten sind. Möglicherweise ist eine komplexe Interaktion von zahlreichen kraniofazialen und dentalen Matrizen bei den beobachteten Veränderungen involviert. Das Wissen um die Altersveränderungen des Gebisses, welche sich natürlicherweise ereignen, hilft dabei die Veränderungen bei Patienten nach abgeschlossener kieferorthopädischer Behandlung korrekt zu bewerten und von einem Rezidiv zu unterscheiden. Aufgrund unserer Ergebnisse empfehlen wir lebenslange Retentionsmethoden, wenn eine optimale Zahnstellung lebenslang angestrebt wird, da ein mit der Zeit verstärkter Engstand im Unterkiefer in unterschiedlichem Ausmaß als Teil des normalen Reifungsprozesses erwartet werden kann.
The potential of several ion-sensitive electrodes responds to the incorporated cations and anions. This has led some authors to misinterpret the potential of metal salt membrane electrodes and of electrodes of the second kind. Neglecting the kinetics of potential establishment and interpreting the potentials solely based on thermodynamics produce completely irrelevant data and suggest that ion concentrations down to 10−45 mol L−1 are accessible by simple potentiometric measurements. The switching from cation to anion response mechanism cannot be derived from thermodynamic equations. It bears complete similarity to the switching of response in the case of foreign interfering ions.
In teaching electrochemistry, it is of primary importance to make students always aware of the relations between electrochemistry and all the non-electrochemical topics, which are taught. The vast majority of students will not specialise in electrochemistry, but they all can very much benefit from the basics and concepts of electrochemistry. This paper is aimed to give suggestions how the teaching of electrochemistry can easily be interrelated to topics of inorganic, organic, analytical, environmental chemistry, biochemistry and biotechnology.
Beeinflussung der Wundheilung von respiratorischen Epithelzellen in vitro durch Roxithromycin

In der vorliegenden Arbeit sollte der Einfluss des Makrolid-Antibiotikums Roxithromycin auf die frühe Phase der postoperativen Wundheilung am Zellkulturmodell untersucht werden. Gründe für den Einsatz des Medikamentes sind abseits der bekannten bakterios-tatischen Wirkung insbesondere die in der Langzeitanwendung beschriebenen immunmodulatorischen Effekte. Es sollte untersucht werden, ob diese bereits in der Akutsituation zu einem Benefit in der Wundheilung im HNO-Bereich führen, da Wundhei-lung hier sekundär verläuft und Wundheilungsstörungen durch Fibrosierungen und Re- Stenosen nicht selten zu einer operativen Korrektur zwingen.
Gewählt wurden S9-Zellen als Modell für respiratorisches Epithel, welche einer standardi-sierten Wundsetzung durch Stanzung unterzogen wurden. Mittels AlamarBlue-Assay wurden zytotoxische Medikamentenkonzentrationen herausgearbeitet und im Folgenden durch Konzentrationsfindungsversuche über 120 Stunden mit kombinierter Wundsetzung bestätigt. Im Vergleich zur Kontrollgruppe beschleunigt Roxithromycin in Konzentrationen von 10 µg/ml, 20 µg/ml und 40 µg/ml eher die Wundheilung, während Konzentrationen von 80 µg/ml und 160 µg/ml zu einer signifikanten (p ≤ 0.01 bzw. p ≤ 0.05) Hemmung der Wundheilung führen. Die Effekte waren jeweils über die ganze Beobachtungsdauer und im Ausmaß zunehmend nachweisbar. Roxithromycin in einer Konzentration von 160 μg/ml konnte die Zellproliferation 96 und 120 h nach Wundsetzung so stark hemmen, dass die residuale Wundfläche sogar anstieg.
In einem Validierungsansatz konnte zudem herausgearbeitet werden, dass die Wundsetzung konsequenterweise von nur einer/m Experimentierenden erfolgen sollte, um ein re-produzierbares Ergebnis zu erhalten, während sich kein Unterschied in der Auswertung der Wundflächen durch verschiedene Personen zeigte.
Zum Verständnis der molekularbiologischen Effekte des Medikamentes auf Epithelzellen wurden unter Berücksichtigung der in der Literatur beschriebenen maximalen Serumkonzentration bei systemischer Anwendung von Roxithromycin (7,28 ± 1,85 mg/l), fehlenden Hinweisen auf einen zytotoxischen Effekt sowie tendenziell beschleunigter Wundheilung nachfolgend 2D–DIGE-Experimente gekoppelt mit massenspektrometrischen Proteinidentifikationen für die Roxithromycinkonzentration von 10 µg/ml durchgeführt. Es ergaben sich Hinweise auf eine Förderung des Zellwachstums durch Beeinflussung des Polyaminmetabolismus über Induktion der Spermidinsynthase und darüber hinaus Spermidin-abhängige Modifikation des Translationsfaktors EIF5A. Des Weiteren wurde eine Nrf2-assoziierte Antwort auf oxidativen Streß bereits bei niedriger Roxithromycin- Konzentration von 10 µg/ml als protektiver Faktor ermittelt. Durch quantitative LC-MS/MS-Analysen wurde dieser Effekt ebenso für Konzentrationen von 20 und 40 µg/ml Roxithromycin bestätigt. Der weitläufig dokumentierte konzentrationsabhängige zytotoxische Effekt des Medikamentes bei höheren Konzentrationen konnte auch in dieser Arbeit gezeigt werden. Während apoptoseauslösende Proteine in der Konzentration von 10 µg/ml nicht nachgewiesen werden konnten, gelang in höheren Roxithromycin-Konzentrationen eine Darstellung von regulierten Proteinen des intrinsischen und extrinsischen Apoptoseweges, bis hin zur bereits mikroskopisch erfassbaren Zelldepletion bei Konzentrationen von 80 und 160 µg/ml. Insbesondere bei der Konzentration von 20 µg/ml fanden sich Hinweie, dass eine Regulation über den ERK1/2-Pathway stattfindet, welcher umfangreiche Funktionen wie Zytokinfreisetzung, Zellmigration, Zellproliferation und auch Zelltod moderiert.
Die Behandlung mit Roxithromycin führt in niedriger Konzentration (10 µg/ml) tendenziell zu einer beschleunigten Wundheilung ohne Auftreten zytotoxischer Effekte. Grundlage könnte die Aktivierung von Translationsfaktoren (EIF5A) und Protektion vor den Schäden durch oxidativen Streß sein. Damit stellt diese Therapie eine Option in der frühen postoperativen Situation im HNO-Bereich dar, welche allerdings weiterer Untersuchung z. B. durch Testung an Fibroblasten bedarf. Attraktiver erscheint das Outcome bei einer Medikamentenkonzentration von 20 µg/ml, da die epitheliale Wundheilung signifikant schneller abläuft und somit zu einem schnelleren Wundverschluß führt. Die molekularbiologischen Hinweise auf Induktion der Apoptose werden bei dieser Konzentration offenbar noch durch protektive Mechanismen über Beeinflussung der Zellmigration, Neutralisierung von Schäden durch oxidativen Streß und Eingriff in den ERK-Pathway aufgefangen, was bei höheren Roxithromycinkonzentrationen nicht mehr gelingt und in einer Zelldepletion mündet. Daher bietet sich für die Konzentration von 20 µg/ml insbesondere die topische Applikation als Salbenstrang oder Nasenspray an, um die positiven Effekte zu nutzen und die Limitation durch vorheriges Erreichen der maximalen Serumkonzentration zu umgehen. Empfehlenswert ist die Untersuchung des Medikamentes in weiteren Zell-reihen und in vivo da Wundheilung ein multimodaler Prozeß ist. Eine topische Applikation von ggf. höheren lokalen Konzentrationen scheint hier keinen zusätzlichen Nutzen darzustellen.
Die Volkskrankheiten Diabetes mellitus und Karies setzten wir unter Verwendung der Daten der großen populationsbasierten SHIP-Studie in Beziehung, um die Hypothese zu bekräftigen, dass Diabetes mellitus Auswirkungen auf Koronalkaries hat. Durch SHIP-0, -1 und -2 war es uns möglich, Daten über einen Zeitraum von insgesamt 15 Jahren (1997-2012) zu verwenden, bei der in der Baseline-Studie 3731 Personen teilnahmen. Dies ist die erste Longitudinalstudie mit einem großen Stichprobenumfang, die eine mögliche Assoziation zwischen Diabetes mellitus sowie der metabolischen Kontrolle und der Koronalkaries untersucht hat.
Die Koronalkarieserfahrung wurde mittels DMFS-Index und deren Komponenten bei den Teilnehmern im Kiefer halbseitig ermittelt. Ein Typ-2-Diabetiker wurde definiert, wenn er eine ärztliche Diagnose angegeben hatte oder blutzuckersenkende Medikamente einnahm oder einen HbA1c-Wert ≥ 6,5% hatte oder einen Gelegenheits-Blutglukosespiegel von ≥ 11,1 hatte. Die Teilnehmer wurden in drei Gruppen eingeteilt: kein Diabetes mellitus, gut eingestellter Typ‑2-Diabetes (HbA1c 6,5%-7%) und schlecht eingestellter Typ-2-Diabetes (HbA1c > 7%). Die Daten wurden mit voll adjustierten linearen gemischten Modellen ausgewertet.
Die Ergebnisse unserer populationsbasierten 11-jährigen Longitudinalstudie wiesen auf eine HbA1c-abhängige Assoziation zwischen Diabetes mellitus und der Koronalkarieserfahrung hin. Ein schlecht eingestellter Diabetiker wies eine höhere DMFS-Langzeitprogression (0,761 Flächen pro Jahr) auf als ein gut eingestellter Diabetiker (0,473 Flächen pro Jahr) oder ein Teilnehmer ohne Diabetes mellitus (0,480 Flächen pro Jahr). Ein gut eingestellter Diabetiker hatte zudem die geringste DFS-Langzeitprogression (0,259 Flächen pro Jahr) gegenüber eines schlecht eingestellten Diabetikers (0,426 Flächen pro Jahr) und auch gegenüber eines Teilnehmers ohne Diabetes mellitus (0,337 Flächen pro Jahr).
Die Ergebnisse unserer Studie zeigten, dass die Koronalkarieserfahrung bei schlecht eingestellten Diabetikern größer war als bei gut eingestellten oder bei Teilnehmern ohne Diabetes mellitus. Dieses Kernergebnis unserer Studie ist möglicherweise durch die Veränderungen des Speichels, den höheren Glukosegehalt in der Zahnfleischtasche und eine anzunehmende mangelnde Mundhygiene eines schlecht eingestellten Diabetikers, beziehungsweise durch eine gute Compliance, anzunehmende gesündere Lebensweise und damit bessere Mundhygiene eines gut eingestellten Diabetikers zu erklären.
Langfristig sind große epidemiologische Langzeitstudien anzustreben, die die Beeinflussung zwischen Diabetes mellitus und Karies abhängig von HbA1c analysieren und somit Patienten mit einem gut oder mit einem schlecht eingestellten Diabetes mellitus differenziert betrachten. Um den möglichen kausalen Zusammenhang von der Diabetes geförderten Koronalkariesentstehung zu beleuchten, ist eine Analyse der Parameter Glukosegehalt, pH-Wert und Mikroorganismen im Speichel bei der Untersuchung von Teilnehmern in zukünftigen Studien zusätzlich zielführend.
Unsere Studie stellt den Anfang für weitere umfassende epidemiologische Studien dar, um zukünftig Patienten über die beiden Volkskrankheiten Diabetes mellitus und Karies aufklären und auf eine mögliche Assoziation von Diabetes mellitus auf Koronalkaries zum Schutz vor Folgeerkrankungen hinweisen zu können.
Die Dissertation befasst sich mit dem Spannungsfeld von subjektivem Persönlichkeitsschutz und Pressefreiheit im deutschen und italienischen Recht. Neben einer umfassenden Bestandsaufnahme beider Rechtsordnungen, hinterfragt die Arbeit die jeweiligen nationalen Lösungen im Vergleich und überprüft diese auf deren Aktualität und Praktikabilität in der gegenwärtigen Medienwirklichkeit. Dafür untersucht die Autorin vor allem die einzelfallbezogene Abwägungspraxis, analysiert die jeweilige richterrechtliche Bildung von Abwägungskriterien auch in Hinblick auf den Einfluss und die jeweilige Berücksichtigung der Rechtsprechungen von EuGH und EGMR und evaluiert Notwendigkeit und Grad der Harmonisierung des Spannungsfelds.
Purpose
The WHO 2016 re-classification of myeloproliferative neoplasms resulted in a separation of essential thrombocythemia (ET) from the pre-fibrotic and fibrotic (overt) phases of primary myelofibrosis (MF). This study reports on a chart review conducted to evaluate the real life approach regarding clinical characteristics, diagnostic assessment, risk stratification and treatment decisions for MPN patients classified as ET or MF after implementation of the WHO 2016 classification.
Methods
In this retrospective chart review, 31 office-based hematologists/oncologists and primary care centers in Germany participated between April 2021 and May 2022. Physicians reported available data obtained from patient charts via paper–pencil based survey (secondary use of data). Patient features were evaluated using descriptive analysis, also including diagnostic assessment, therapeutic strategies and risk stratification.
Results
Data of 960 MPN patients diagnosed with essential thrombocythemia (ET) (n = 495) or myelofibrosis (MF) (n = 465) after implementation of the revised 2016 WHO classification of myeloid neoplasms was collected from the patient charts. While they met at least one minor WHO-criteria for primary myelofibrosis, 39.8% of those diagnosed with ET did not have histological BM testing at diagnosis. 63.4% of patients who were classified as having MF, however, did not obtain an early prognostic risk assessment. More than 50% of MF patients showed characteristics consistent with the pre-fibrotic phase, which was emphasized by the frequent use of cytoreductive therapy. Hydroxyurea was the most frequently used cytoreductive medication in 84.7% of ET and 53.1% of MF patients. While both ET and MF cohorts showed cardiovascular risk factors in more than 2/3 of the cases, the use of platelet inhibitors or anticoagulants varied between 56.8% in ET and 38.1% in MF patients.
Conclusions
Improved histopathologic diagnostics, dynamic risk stratification including genetic risk factors for cases of suspected ET and MF are recommended for precise risk assessment and therapeutic stratification according to WHO criteria.
Aim
Climate change challenges temperate forest trees by increasingly irregular precipitation and rising temperatures. Due to long generation cycles, trees cannot quickly adapt genetically. Hence, the persistence of tree populations in the face of ongoing climate change depends largely on phenotypic variation, that is the capability of a genotype to express variable phenotypes under different environmental conditions, known as plasticity. We aimed to quantify phenotypic variation of central Europe's naturally dominant forest tree across various intraspecific scales (individuals, mother trees (families), populations) to evaluate its potential to respond to changing climatic conditions.

Methods
We conducted a fully reciprocal transplantation experiment with more than 9000 beech seeds from seven populations across a Europe-wide gradient. We compared morphological (Specific Leaf Area), phenological (leaf unfolding) and fitness-related (growth, survival) traits across various biological scales: within single mother trees, within populations and across different populations under the contrasting climates of the translocation sites.
Results
The experiment revealed significant phenotypic variation within the offspring of each mother tree, regardless of geographic origin. Initially, seedling height growth varied among mother trees and populations, likely due to maternal effects. However, the growth performance successively aligned after the first year. In summary, we observed a consistent growth response in different beech populations to diverse environments after initial maternal effects.
Main Conclusions
The study strikingly demonstrates the importance of considering intraspecific variation. Given the surprisingly broad spectrum of phenotypes each mother tree holds within its juvenile offspring, we conclude that Fagus sylvatica might have the potential for medium-term population persistence in face of climate change, provided that this pattern persists into later life stages. Hence, we also suggest further investigating the inclusion of passive adaptation and natural dynamics in the adaptive management of forests.
Introduction: The topics of bullying, school anxiety and school absenteeism are of steady interest for the scientific community in recent decades. However, it seems surprising that investigations into the combination of these constructs are rare, especially considering their interconnectedness. Due to the lack of joint investigation of these factors, it is hardly possible to compare results of these related, yet distinct factors across other studies, let alone the predictive power of specific factors. The goal of the current study is to investigate how bullying, school anxiety and school absenteeism are related, considering the variables gender and grade level.
Methods: For this purpose, N = 195 secondary school children in the 7th–9th grades in northern Germany were surveyed via self-report questionnaires and additionally collecting their school records. We present complex descriptive analyses with scales and subscales of bullying, anxiety and absenteeism. Further, a structural equation modelling (SEM) approach is utilized to discover the interconnectedness of the constructs.
Results: On the one hand, the descriptive statistics show significant gender and grade level differences regarding bullying and anxiety. On the other hand, the SEM reveals that high values on the bullying victim scale are accompanied by significantly higher school displeasure (anxiety). School displeasure—as well as high bullying offender values—are associated with significantly more days of absence from school.
Discussion: We discuss how school environment improvement through specific interventions such as the cognitive-behavioral approach, could aid to ameliorate this issue.
Podocytes are highly specialized kidney cells that are attached to the outer aspect of the glomerular capillaries and are damaged in more than 75% of patients with an impaired renal function. This specific cell type is characterized by a complex 3D morphology which is essential for proper filtration of the blood. Any changes of this unique morphology are directly associated with a deterioration of the size-selectivity of the filtration barrier. Since podocytes are postmitotic, there is no regenerative potential and the loss of these cells is permanent. Therefore, identification of small molecules that are able to protect podocytes is highly important. The aim of this work was to establish an in vivo high-content drug screening in zebrafish larvae. At first, we looked for a reliable podocyte injury model which is fast, reproducible and easy to induce. Since adriamycin is commonly used in rodents to damage podocytes, we administered it to the larvae and analyzed the phenotype by in vivo microscopy, (immuno-) histology and RT-(q)PCR. However, adriamycin did not result in a podocyte-specific injury in zebrafish larvae. Subsequently, we decided to use a genetic ablation model which specifically damages podocytes in zebrafish larvae. Treatment of transgenic zebrafish larvae with 80 µM metronidazole for 48 hours generated an injury resembling focal and segmental glomerulosclerosis which is characterized by podocyte foot process effacement, cell depletion and proteinuria. Following this, we established an in vivo high-content screening system by the use of a specific screening zebrafish strain. This screening strain expresses a circulating 78 kDa eGFP-labeled Vitamin D-binding fusion protein, which passes the filtration barrier only after glomerular injury. Therefore, we had an excellent readout to follow podocyte injury in vivo. We generated a custom image analysis software that measures the fluorescence intensity of podocytes and the vasculature automatically on a large scale. Furthermore, we screened a specific drug library consisting of 138 compounds for protective effects on larval podocytes using this in vivo high-content system. The analysis identified several initial hits and the subsequent validation experiments identified belinostat as a reliable and significant protective agent for podocytes. These results led to a patent request and belinostat is a promising candidate for a clinical use and will be tested in mammalian podocyte injury models.
Durch den demografischen Wandel mit einer älter werdenden Bevölkerung ist zukünftig mit einem Inzidenzanstieg von progredienten, lebenslimitierenden Erkrankungen zu rechnen, der zu einem wachsenden Bedarf an palliativmedizinischer Versorgung in Deutschland führt. Damit verbundene Kos-tenanstiege erhöhen den wirtschaftlichen Druck auf politische Entscheidungsträger bei der Ressourcenallokation. Eine besonders große Herausforderung stellt dabei die flächendeckende Versor¬gung in ländlichen Regionen dar.
Was kostet spezialisierte Palliativversorgung in einer ländlichen Region wie Vorpom¬mern-Greifswald (VG)? Wie werden sich die Kosten entwickeln, wenn sich der Bedarf und die Versorgungsstruktur verändern? Aktuell liegen nur rudimentäre Daten über die Ausgaben für palliativmedizinische Versorgung in Deutschland vor. Fundierte Kenntnisse sind für den Ausbau eines kosteneffizienten Netzwerks allerdings unerlässlich. Das Ziel dieser Arbeit ist daher die Darstellung der Kosten für spezialisierte Palliativversorgung im Raum Vorpommern-Greifswald (VG) sowie die Prognose über die Kostenentwicklung bei Variation der beteiligten Einrich¬tungen (Normalstation, Palliativstation, Hospiz sowie SAPV zu Hause bzw. im (Kurzzeit-) Pflegeheim).
Durch eine retrospektive Studie vom 01.04.2013 bis 31.03.2014 wurden die Behandlungskosten von 464 Patienten mittels Sekun¬därdatenanalyse ausgewertet. Ferner wurden die Verweildauern in den Institutionen sowie deren Übergänge bestimmt. Mithilfe dieser Parameter erfolgten Simulationen mit dem Markov-Modell, die Prognosen über durchschnittliche Jahres-, Behandlungs- und Tageskosten für verschiedene Szenarien lieferten.
In den Simulationen mit variabler Mortalität übte die Restlebenszeit (RZ) neben den Tagessätzen zusätzlich Einfluss auf die Ergebnisse aus: Die Versorgung im Hospiz und auf der Palliativstation gingen mit einer kurzen RZ und folg¬lich geringeren Kosten einher. SAPV zu Hause und Therapien auf der Normalstation ließen die simulierten Jahreskosten durch eine lange RZ oder hohe Tageskosten ansteigen, im Pflegeheim war der Trend nicht eindeutig. Änderungen der Kapazitäten im KZP hatten nur einen mar¬ginalen Einfluss auf die Ausgaben.
Die durchschnittlichen Jahreskosten für spezialisierte palliativmedizinische Versor-gung lagen im Raum VG in der Basis-Simulation (BS) bei 7.814.430,47€. Bei alleiniger Versorgung auf der Normalstation stiegen sie bei variabler Mortalität im Vergleich dazu um das 18-Fache an. Existierte daneben zusätzlich eine Palliativstation, wurde der Kostenanstieg auf 24% gedrosselt und betrug 9.688.127,25€. Eine Reduktion der Plätze in der Akut-Klinik führte zu Einsparungen zwischen 52% und 57%. Die kombinierte Versorgung im Hospiz und auf der Palliativstation ging mit minimalen simulierten Jahreskosten von 2.590.943,67€ einher. Komplett ohne SAPV ließen sich die Ausgaben um durchschnittlich 22% senken und weiter um ca. 29%, wenn SAPV nur in der Häuslichkeit entfiel.
Die Ergebnisse änderten sich bei konstanter Mortalität. In allen Simulationen mit einheitlicher RZ zeigte sich SAPV – unabhängig davon, wo sie erfolgte – den anderen Institutionen ökonomisch überlegen. Eine ausschließliche SAPV ging mit den geringsten mortalitätsadjustierten Jahreskosten von 3.1711.387,99€ einher. Umgekehrt stiegen diese ohne SAPV bis zu 116% auf 16.863.368,85€ an. Die übrigen Szenarien zeigten ähnliche Trends im Vergleich zur variablen Mortalität. Bei alleiniger Versorgung auf der Normalstation stiegen die mortalitätsadjustierten Jahreskosten ebenfalls maximal an. Sie erhöhten sich im Vergleich zur BS allerdings we¬niger stark um 315% auf 32.427.040,75€. Dieser Zuwachs wurde durch eine zusätzliche Palliativstation ebenfalls auf 198% bzw. 23.314.102,25€ gedrosselt. Unter Reduktion der statio¬nären Kapazitäten waren Einsparungen zwischen 46% und 54% zu beobachten. Die Ausgaben lagen dann zwischen 3.573.443,09€ und 4.250.803,62€. Komplett ohne Akut-Krankenhaus sanken die mortalitätsadjustierten Jahreskosten weiter um 55% auf durchschnittlich 3.506.578,56€.
Hospitalisierung, vor allem ohne Palliativstation, führte somit auch bei konstanter RZ zu Kostenanstiegen. Die Betreuung im Hospiz ging mit Einsparungen einher. SAPV senkte die Aus¬gaben in der Simulation am meisten.
Zusammenfassend war die ambulante palliativmedizinische Versorgung in der Simu¬lation mit konstanter RZ im Hinblick auf einen möglichst kosteneffizienten Ressourceneinsatz den anderen Institutionen überlegen. Die Studie deckte allerdings auch auf, dass Verlegungen in stationäre Institutionen trotz SAPV notwendig waren. Die Gründe dafür sind zukünftig zu ermitteln – insbesondere in Hin¬blick auf die Frage, ob sie durch bedarfsgerechte Optimierung der Versorgung vermeidbar sind.
Kann SAPV zu Hause nicht gewährleistet werden, sind aus ökonomi¬scher und medizinischer Sicht alternative Strukturen wie SAPV im Pflegeheim oder eine Betreuung im Hospiz einer Akut-Klinik gegenüber zu bevorzugen. Stationäre Aufenthalte sollten, sofern sie unvermeidbar sind, nach Möglichkeit auf der Palliativstation erfolgen, mit möglichst zügiger Entlassung nach Hause, um dem Wunsch der meisten Patienten zu entsprechen, in der gewohnten häuslichen Umgebung in Würde und Selbstbestimmung zu sterben.
