Over the last decades, thyroid hormone metabolites (THMs) received marked attention as it has been demonstrated that they are bioactive compounds. Their concentrations were determined by immunoassay or mass-spectrometry methods. Among those metabolites, 3,5-diiodothyronine (3,5-T2), occurs at low nanomolar concentrations in human serum, but might reach tissue concentrations similar to those of T4 and T3, at least based on data from rodent models. However, the immunoassay-based measurements in human sera revealed remarkable variations depending on antibodies used in the assays and thus need to be interpreted with caution. In clinical experimental approaches in euthyroid volunteers and hypothyroid patients using the immunoassay as the analytical tool no evidence of formation of 3,5-T2 from its putative precursors T4 or T3 was found, nor was any support found for the assumption that 3,5-T2 might represent a direct precursor for serum 3-T1-AM generated by combined deiodination and decarboxylation from 3,5-T2, as previously documented for mouse intestinal mucosa. We hypothesized that lowered endogenous production of 3,5-T2 in patients requiring T4 replacement therapy after thyroidectomy or for treatment of autoimmune thyroid disease, compared to production of 3,5-T2 in individuals with intact thyroid glands might contribute to the discontent seen in a subset of patients with this therapeutic regimen. So far, our observations do not support this assumption. However, the unexpected association between high serum 3,5-T2 and elevated urinary concentrations of metabolites related to coffee consumption requires further studies for an explanation. Elevated 3,5-T2 serum concentrations were found in several situations including impaired renal function, chronic dialysis, sepsis, non-survival in the ICU as well as post-operative atrial fibrillation (POAF) in studies using a monoclonal antibody-based chemoluminescence immunoassay. Pilot analysis of human sera using LC-linear-ion-trap-mass-spectrometry yielded 3,5-T2 concentrations below the limit of quantification in the majority of cases, thus the divergent results of both methods need to be reconciliated by further studies. Although positive anti-steatotic effects have been observed in rodent models, use of 3,5-T2 as a muscle anabolic, slimming or fitness drug, easily obtained without medical prescription, must be advised against, considering its potency in suppressing the HPT axis and causing adverse cardiac side effects. 3,5-T2 escapes regular detection by commercially available clinical routine assays used for thyroid function tests, which may be seriously disrupted in individuals self-administering 3,5-T2 obtained over-the counter or from other sources.
Abstract
Background
Twenty five‐hydroxy vitamin D (25OHD) levels have been proposed to protect against periodontitis based on in vitro and observational studies but evidence from long‐term randomized controlled trials (RCTs) is lacking. This study tested whether genetically proxied 25OHD is associated with periodontitis using Mendelian randomization (MR).
Methods
Genetic variants strongly associated with 25OHD in a genome‐wide association study (GWAS) of 417,580 participants of European ancestry were used as instrumental variables, and linked to GWAS summary data of 17,353 periodontitis cases and 28,210 controls. In addition to the main analysis using an inverse variance weighted (IVW) model, we applied additional robust methods to control for pleiotropy. We also undertook sensitivity analyses excluding single nucleotide polymorphisms (SNPs) used as instruments with potential pleiotropic effects and used a second 25OHD GWAS for replication. We identified 288 SNPs to be genome‐wide significant for 25OHD, explaining 7.0% of the variance of 25OHD levels and providing ≥90% power to detect an odds ratio (OR) of ≤ 0.97.
Results
MR analysis suggested that a 1 standard deviation increase in natural log‐transformed 25OHD was not associated with periodontitis risk (IVW OR = 1.04; 95% confidence interval (CI): 0.97–1.12; P‐value = 0.297). The robust models, replication, and sensitivity analyses were coherent with the primary analysis.
Conclusions
Collectively, our findings suggest that 25OHD levels are unlikely to have a substantial effect on the risk of periodontitis, but large long‐term RCTs are needed to derive definitive evidence on the causal role of 25OHD in periodontitis.
A New Laboratory Workflow Integrating the Free Light Chains Kappa Quotient into Routine CSF Analysis

We performed this cohort study to test whether further analysis of intrathecal inflammation can be omitted if the free light chain kappa (FLCκ) quotient is within the reference range in the corresponding quotient diagram. FLCκ concentrations were measured in serum and cerebrospinal fluid (CSF) samples. The intrathecal fraction (IF) of FLCκ was calculated in relation to the hyperbolic reference range. 679 patient samples were used as a discovery cohort (DC). The sensitivity and negative predictive value (NPV) of the FLCκ-IF for the detection of an intrathecal humoral immune response (CSF-specific OCB and/or IF IgG/A/M > 0%) was determined. Based on these data, a diagnostic algorithm was developed and prospectively validated in an independent validation cohort (VC, n = 278). The sensitivity of the FLCκ-IF was 98% in the DC and 97% in the VC with a corresponding NPV of 99%. The use of the FLCκ-IF as a first line analysis would have reduced the Ig and OCB analysis by 62% in the DC and 74% in the VC. The absence of a FLCκ-IF predicts the absence of a humoral intrathecal immune response with a very high NPV of 99%. Thus, integration of our proposed algorithm into routine CSF laboratory analysis could help to reduce analytical efforts.
Hohe Aldosteron-Konzentrationen haben einen nicht zu unterschätzenden Einfluss auf kardiovaskuläre Risikofaktoren wie arterielle Hypertonie, Störungen des Glukosemetabolismus, eingeschränkte Nierenfunktion und Fettstoffwechselstörung. In vorausgehenden Studien an speziellen Patientenkollektiven aber auch in klinischepidemiologischen Studien wurde eine Assoziation zwischen der Plasma AldosteronKonzentration (PAC) und bestimmten Komponenten des Lipidmetabolismus beschrieben. Ob dieser Zusammenhang auch in der allgemeinen Bevölkerung, und innerhalb des Referenzbereiches für die PAC besteht ist unklar. Um dies zu beantworten, beschäftigte sich die vorliegende Arbeit mit der Assoziation zwischen der PAC und ausgewählten Lipoproteinen (High density lipoprotein cholesterol (HDL-C), Low density lipoprotein cholesterol (LDL-C), Triacylglyceride, gesamt Cholesterol, non-HDL-C) in der allgemeinen Bevölkerung. Hierfür wurden Daten von 793 Männern und 938 Frauen zwischen 25-86 Jahren genutzt die am ersten Follow-up der Study of Health in Pomerania (SHIP-1) teilnahmen. Die Assoziation zwischen der PAC und den oben genannten Lipoproteinen wurde anhand von multivariablen, linearen Regressionsmodellen erfasst. Diese wurden adjustiert für Geschlecht, Alter, BMI, geschätzter glomerulärer Filtrationsrate (eGFR) und HbA1c. Es zeigte sich, eine statistisch signifikante positive Assoziation von PAC mit LDL-C und non-HDL-C sowie eine inverse Assoziation mit HDL-C.
Erstmalig konnte in dieser Arbeit somit gezeigt werden, dass die PAC im physiologischen Bereich im engen Zusammenhang mit dem Lipidmetabolismus steht. Die beobachteten und bereits aus vorherigen Studien bekannten Assoziationen sind daher nicht auf ein bestimmtes Patientenkollektiv beschränkt, sondern auch in der allgemeinen Bevölkerung vorhanden. Weiterhin bestätigen die Ergebnisse aus SHIP-1 die bereits bekannte inverse Assoziation zwischen der PAC und HDL-C. Die hier beschriebene Assoziation zwischen der PAC und LDL-C sowie dem non-HDL-C wurde bis Dato noch nicht beschrieben.
Zusammenfassung
Zielsetzung: Ziel dieser Studie war es einerseits zu ermitteln, ob Frauen mit hohen Testosteron-Konzentrationen oder niedrigen Sexualhormon-bindendes GlobulinKonzentrationen vermehrt kardiovaskuläre Krankheiten (KVK) entwickeln und andererseits mögliche Assoziationen von Androgenen und Gesamt- und KVKspezifischer Mortalität in der weiblichen Allgemeinbevölkerung zu untersuchen.
Studiendesign und Methodik: Aus der Study of Health in Pomerania (SHIP) wurden Daten von 2129 Frauen (mittleres Alter 49,0 Jahre) mit einem mittleren Follow-up von 10,9 Jahren verwendet. Die Assoziationen von Sexualhormon-Konzentrationen (totales Testosteron (TT), Sexualhormon-bindendes Globulin (SHBG), Androstendion (ASD), freiem Testosteron (fT), freiem Androgene Index (FAI)) und kardiovaskulären Krankheiten (KVK), sowie KVK-spezifischer und Gesamtmortalität wurden im Quer- und Längsschnittdesign mittels multivariabel-adjustierten Regressionsmodellen analysiert.
Ergebnisse: Die Ausgangsmessung zeigte eine KVK-Prävalenz von 17,8% (378 Frauen). Im Follow-up war die KVK-Inzidenz 50,9 pro 1000 Personenjahre. Im altersadjustierten Model zeigte sich eine inverse Assoziation von SHBG und prävalenten KVK (Relatives Risiko pro Standardabweichungsanstieg: 0,83; 95% Konfidenzintervall: 0,74-0,93), diese Assoziation verschwand jedoch im multivariabeladjustierten Modell. KVK-Inzidenz und den Sexualhormon-Konzentrationen waren sowohl im alters-, als auch im multivariabel-adjustierten Regressionsmodell nicht signifikant assoziierte. Für keines der untersuchten Sexualhormone konnte eine Assoziation mit der Gesamtmortalität aufgezeigt werden.
Fazit: Zusammenfassend erbrachte diese populationsbasierte Kohortenstudie keine konsistenten Assoziationen zwischen Androgenen und KVK-Inzidenz oder Mortalität.
Background: Fetal growth failure has been associated with an increased risk of hypertension, cardiovascular disease and diabetes in adulthood. Exploring the mechanisms underlying this association should improve our understanding of these common adult diseases. Patients and Methods: We investigated 225 SNPs in 10 genes involved in growth and glucose metabolism (GH1, GHR, IGF1, IGF1R, STAT5A, STAT5B, MAPK1, MAPK3, PPARγ and INS) in 1,437 children from the multinational NESTEGG consortium: 345 patients born small for gestational age who remained short (SGA-S), 288 who showed catch-up growth (SGA-Cu), 410 idiopathic short stature (ISS) and 394 controls. We related genotype to pre- and/or postnatal growth parameters, response to growth hormone (if applicable) and blood pressure. Results: We found several clinical associations for GH1, GHR, IGF1, IGF1R, PPARγ and MAPK1. One SNP remained significant after Bonferroni's correction: IGF1R SNP rs4966035's minor allele A was significantly more prevalent among SGA and associated with smaller birth length (p = 0.000378) and birth weight (weaker association), independent of gestational age. Conclusion:IGF1R SNP rs4966035 is significantly associated with birth length, independent of gestational age. This and other associations suggest that polymorphisms in these genes might partly explain the phenotype of short children born SGA and children with ISS.
Background and objectives
Various cross-sectional studies have observed an association between high circulating concentrations of the adipokine chemerin and an unfavorable metabolic profile. However, the prognostic value of chemerin for the risk of associated diseases and mortality was examined only in a few studies mostly using small and highly selected patient populations. We aimed to analyze the association between plasma chemerin concentrations and all-cause as well as cause-specific mortality in the general population.
Study design and methods
From the Study of Health in Pomerania (SHIP), participants of two independent cohorts (SHIP-START-1 [n = 3037], SHIP-TREND-0 [n = 4193]) were followed up for 15 and 9 years (median), respectively. The association between plasma chemerin and all-cause mortality was analyzed using multivariable Cox proportional hazard regression models. Additionally, cause-specific hazards for cardiovascular disease (CVD) and cancer mortality were modeled considering competing events.
Results
A total number of 507 and 208 deaths occurred during follow-up in SHIP-START-1 and SHIP-TREND-0, respectively. Multivariable regression analyses revealed a significant association between high plasma chemerin concentrations and greater overall mortality that was independent of major confounders. Each 30 ng/mL increase in chemerin was associated with a 17% higher risk of all-cause mortality (95%-confidence interval: 1.10–1.26). Cause-specific analyses further showed that the chemerin concentration was significantly associated with cancer mortality but not with CVD mortality.
Conclusion
The present study detected a positive association between plasma chemerin concentrations and all-cause mortality in a large population-based study sample. Cause-specific analyses have shown that chemerin is likely to play a decisive role in cancer-related deaths. However, a direct association with cardiovascular mortality could not be established.
Objective
This study provides a comprehensive overview of the associations of five adipokines (adiponectin, chemerin, galectin‐3, leptin, and resistin) with fat deposits, behavioral risk factors, and metabolic phenotypes.
Methods
Using multivariable linear and logistic regression models, cross‐sectional data from 4,116 participants of the population‐based Study of Health in Pomerania were analyzed.
Results
Participants with obesity showed higher chemerin, galectin‐3, and leptin but showed lower adiponectin concentrations. Independently of other fat compounds, liver fat content, visceral adipose tissue, and subcutaneous adipose tissue (SAT) were inversely associated with adiponectin. Independent positive associations of liver fat content and SAT with chemerin as well as of SAT with galectin‐3 and leptin were observed. Physically inactive participants had higher chemerin and leptin concentrations. Smokers had higher chemerin and galectin‐3 as well as lower leptin. Alcohol consumption was associated with adiponectin (positive) and resistin (inverse). All adipokines were associated with at least one lipid marker. Associations with glucose metabolism were seen for adiponectin, chemerin, galectin‐3, and leptin.
Conclusions
High adiponectin concentrations were related to favorable metabolic conditions, whereas high chemerin, galectin‐3, and leptin were associated with an unfavorable metabolic profile. High leptin seems to be primarily indicative of obesity, whereas high adiponectin and chemerin are associated with a broader range of metabolic phenotypes.
Aus früheren populationsbasierten Studien ist bekannt, dass IGF-I womöglich einen schützenden Einfluss auf die kardiovaskuläre Gesundheit ausübt, wobei die genauen Wirkmechanismen auf zellulärer Ebene noch nicht identifiziert wurden. Die Endpunkte dieser umfangreichen Studien entsprachen in der Regel definierten kardiovaskulären Erkrankungen. Vor dem Hintergrund der nach wie vor nicht eindeutigen Wissenslage war es das Ziel der hier präsentierten Arbeit, Serumkonzentrationen von IGF-I oder IGFBP-3 in Zusammenhang mit Veränderungen subklinischer kardiovaskulärer Parameter über die Zeit zu untersuchen. Analysiert wurden dazu Daten der populationsbasierten Study of Health in Pomerania (SHIP). In SHIP wurden initial die Serumkonzentrationen von IGF-I und IGFBP-3 ermittelt. Weiterhin wurden bis zu drei Mal, mit mehrjährigen Abständen, die Intima-Media-Dicke der A. carotis communis (IMD) mittels Sonografie untersucht, der linksventrikuläre Massenindex (LVMI) durch Sonografie sowie Kalkulation bestimmt und NT-proBNP als bekannter Herzinsuffizienzmarker quantifiziert. Durch die mehrmaligen Untersuchungen konnten Rückschlüsse auf Veränderungen dieser drei kardiovaskulären Parameter über die Jahre festgestellt und statistisch in Bezug zu den ursprünglichen IGF I- und IGFBP-3-Spiegeln gesetzt werden. Während keine Relationen zwischen IGF-I und dem LVMI entdeckt wurden, waren niedrigere Ausgangskonzentrationen von IGF-I insbesondere bei Frauen mit einer vermehrten Zunahme der IMD assoziiert. Außerdem konnten eine positive Assoziation zwischen IGF I und NT-proBNP bei Frauen sowie eine U-förmige Assoziation zwischen IGF-I und NT proBNP bei Männern detektiert werden. Damit stützt und erweitert die vorliegende Studie zum Teil die überwiegende Annahme ähnlich konzipierter Studien, wonach niedrige IGF-I-Serumkonzentrationen einen eher ungünstigen Einfluss auf das kardiovaskuläre System nehmen. Gleichzeitig legen die hier präsentierten Ergebnisse geschlechtsspezifische sowie endpunktspezifische Unterschiede in Bezug auf Assoziationen zwischen dem IGF-I-System und dem kardiovaskulären System nahe. Da die präsentierte Arbeit nach unserer Kenntnis die erste große populationsbasierte Studie ist, die sich längsschnittlich dem Thema Geschlechterunterschiede widmete, sind diese bisher nicht ausreichend erforscht und erklärt. Insgesamt deuten die Ergebnisse an, dass IGF-I als laborchemischer Biomarker für subklinische kardiovaskuläre Veränderungen interessant sein könnte und dass die Interpretation dieses Laborwerts im Hinblick auf jene Veränderungen in Abhängigkeit vom Geschlecht des Patienten erfolgen sollte.
Das Angiopoetin-Tie Ligand-Rezeptor System spielt eine wichtige Rolle für das Blutgefäßsystem,
wobei es an Gefäßaufbau und -rückbildung, aber auch der Vermittlung dynamischer
Anpassungsprozesse, wie der Permeabilitätsänderung, in Reaktion auf Entzündungsvorgänge,
beteiligt ist. Dem Ang2-Tie-System wird generell ein Einfluss auf Inflammationsprozesse
zugeschrieben und verschiedene Auswirkungen, wie beispielsweise die Steigerung der Bereitschaft
von Monozyten und Granulozyten zur Gefäßwandadhäsion, sind bereits nachgewiesen. Die
Erwägung von Ang2 als Biomarker in Bezug auf inflammatorische Erkrankungen ist somit
naheliegend. Bisherige, meist kleinere Studien basierend auf Patientenkollektiven ergaben
widersprüchliche Resultate hinsichtlich der Assoziationen zwischen Ang2 bzw. Tie2 und
Inflammationsmarkern. Das Ziel der vorliegenden Arbeit war daher, die Assoziationen zwischen
Ang2 sowie Tie2 und verschiedenen generellen Entzündungsmarkern (hsCRP, WBC, Fibrinogen)
in der Allgemeinbevölkerung zu untersuchen.
Als Datengrundlage dienten die Study of Health in Pomerania-1 (SHIP-1) und die unabhängige
SHIP-Trend Kohorte. Insgesamt wurden Daten von 6624 Teilnehmern herangezogen und mittels
Varianzanalysen und linearen sowie logistischen Regressionsmodellen statistisch ausgewertet. Da
sowohl Hypertonie als auch Diabetes mellitus Einflussfaktoren für Inflammationsmarker und
Ang2-Werte darstellen, wurde neben der Gesamtpopulation auch eine gesunden Subpopulation
ohne Individuen mit Hypertonie und Diabetes mellitus gesondert betrachtet (n = 3195).
Alle drei untersuchten Entzündungsmarker zeigten in der Gesamtpopulation Assoziationen zu
Ang2. Diese Beziehungen blieben mit Ausnahme der Assoziation zu hsCRP auch in der gesunden
Subpopulation bestehen. In Hinblick auf Tie2 konnte lediglich eine konsistente Beziehung zu
Fibrinogen nachgewiesen werden.
Obwohl die Wirkmechanismen hinter den jeweiligen Beziehungen nicht bekannt sind, weist die
vorliegen Studie eine deutliche Verbindung zwischen verschiedenen Markern der Inflammation
und dem Ang2-Tie-System nach. Ang2 ist besonders in der Rolle eines möglichen Biomarkers zur
Diagnose und Prognose schwerwiegender inflammatorischer Erkrankungen im Gespräch. Die
Ergebnisse unserer Studie stützen diese Überlegung. Für Rückschlüsse hinsichtlich eines
Behandlungsansatzes für inflammatorische Erkrankungen basierend auf den Parametern des Ang2-
Tie-Systems sind aber weiterführende Studien, die die ursächlichen Mechanismen für die
gefundenen Zusammenhänge untersuchen, dringend erforderlich.
